3. Supervivencia de implantes dentales de zirconio monoblock a largo plazo (5 años): Una revisión sistemática

Los implantes dentales de circonio se han propuesto como una alternativa a los tradicionales implantes de titanio buscando mejorar propiedades biológicas, mecánicas y estéticas. Objetivos: Analizar y sintetizar la evidencia científica disponible sobre la supervivencia de los implantes de circonio de cuerpo único con más de 5 años de seguimiento. Métodos: Se realizó una revisión sistemática a través de las bases de datos Pub Med, Embase, BVSalud combinando las palabras implante dental, cuerpo único, circonio, óxido de circonio, circonio estabilizado con itria. Se incluyeron ensayos clínicos aleatorizados, estudios de cohorte, estudios de caso-control y estudios retrospectivos en humanos. Se llevó adelante un metaanálisis empleando la unidad de estudio tasa de supervivencia absoluta de los implantes dentales después de 5 años en la cavidad bucal. Resultados: La búsqueda inicial arrojó 1434 artículos, luego de eliminar duplicados, lectura crítica de títulos y resúmenes y acceso a texto completo se incluyeron 9 artículos. Los estudios se publicaron entre 2015 y 2023, con un total 2387 implantes en 1075 pacientes, reportando seguimientos a 5, 8, 10 y 15 años. Los estudios tuvieron tamaños muestrales dispares. Al considerar la heterogeneidad entre estudios se obtuvo un valor de I2 del 88%. La proporción de éxito combinada que se obtuvo del metaanálisis fue del 95% (88%; 98%). Conclusiones: Si bien los estudios son heterogéneos, después de 5 años, los implantes de circonio mostraron altas tasas de supervivencia, siendo una alternativa posible a los implantes de titanio. Sin embargo, se requieren más estudios clínicos para confirmar esta tendencia.

13. Performance and Maximum Functional Masticatory Force in Patients with Dentomucosal-supported and Implant-supported Removable Partial Dentures

Introduction: Due to edentulousness, masticatory performance and masticatory force decrease, which implies a disorganization and decompensation of the stomatognathic system with forces provoked by mastication, which are distributed in the bone through teeth and mucous membranes and cause bone resorption in edentulous areas. Objective: To compare masticatory performance and maximum functional masticatory force in partially edentulous patients with dentomucosally supported removable partial dentures before and after surgical installation of osseointegrated implants as auxiliary prosthetic abutments. Methods: Ten patients rehabilitated with removable partial dentures were selected. The biomechanical test of masticatory force measurement and the Manly test were performed to evaluate masticatory performance before implant installation and at three- and six-months post-surgery. Results: An average masticatory performance value of 24.2 ± 9.73 % was obtained before implant surgery, 44.1 ± 13.28 % at three months and 45.6 ± 14.33 % at six months after implant surgery (p = 0.001). Before implant surgery, an average masticatory force of 10.6 ± 4.34 kg, 12.5 4.28 kg at three months and 15.1 ± 5.12 kg at six months after implant surgery was obtained (p = 0.008). Conclusions: The use of osseointegrated implants as auxiliary abutments in dentomucosally supported removable prostheses are associated with increased masticatory performance and increased maximum functional masticatory force, associated with relief of direct load on edentulous maxillary ridges.

Objetivo: Comparar dos procedimientos de soldadura convencionales empleando una aleación de Cr-Co, para conectar barras coladas seccionadas a ser fijadas sobre implantes. Materiales y métodos: A partir de un modelo maestro que representa un maxilar desdentado con cuatro implantes, se confeccionaron veinte (n=20) probetas seccionadas en tres partes. Se conformaron dos grupos, cada uno con diez (n=10) ejemplares. Una vez acondicionadas, fueron atornilladas al modelo maestro. Su desajuste inicial se analizó utilizando una lupa estereoscópica, con una cámara incorporada y un software. Las partes fueron soldadas empleando un procedimiento diferente para cada grupo. Las correspondientes al Grupo I se invistieron en un block refractario a base de sílico-fosfato. Las del Grupo II se montaron en una estructura metálica Clever Spider. El desajuste fue mensurado y los resultados procesados estadísticamente. El nivel de significación fue establecido en p0,05. Al comparar entre sí los grupos I y II, por medio de la prueba t y de comprobación no paramétrica de Mann-Whitney, se observaron diferencias no significativas, p=0,41 y p=0,38 respectivamente. Conclusiones: Bajo las condiciones de este estudio, se observó que los dos métodos de soldadura analizados fueron confiables para unir supraestructurasos metálicas sin que se afecte su ajuste final. //Aim: Compare two conventional welding procedures using a Cr-Co alloy, to connect sectioned cast bars to be fixed on implants. Materials and methods: From a master model representing a toothless jaw with four implants, twenty (n=20) specimens sectioned into three parts were made. Two groups were formed, each with ten (n=10) specimens. Once conditioned, they were screwed to the master model. Its initial mismatch was analyzed using a stereoscopic magnifier, with a built-in camera and a software. The parts were welded using a different procedure for each group. Those corresponding to Group I were invested in a refractory block based on silyl-phosphate. Those of Group II were mounted on a Clever Spider metal structure. The mismatch was measured, and the results processed statistically. The level of significance was established at p0.05. When comparing groups I and II, using the t-test and the Mann-Whitney nonparametric verification, non-significant differences were observed, p=0.41 and p=0.38 respectively. Conclusions: Under the conditions of this study, it was observed that the two welding methods analyzed were reliable for joining metallic superstructures without affecting their final fit.

La pérdida de órganos dentales a causa de enfermedad periodontal en pacientes adulto es muy frecuente afectando su calidad de vida. Objetivo: Restituir la función, estética por medio de prótesis implanto soportada. Caso Clínico: paciente de 53 años de edad de sexo masculino se le diagnóstico periodontitis estadio IV, generalizada grado C, el tratamiento propuesto en este caso fue realizar tres fases, dos quirúrgicas y una protésica, fase 1 exodoncias y colocación de cinco implantes e instalación de dos prótesis acrílicas removibles inmediatas con acondicionador de tejido, fase 2 apertura y colocación de cicatrizadores y fase 3 consta de rehabilitación protésica con una prótesis acrílica removible total superior y prótesis implanto soportada inferior, obtenidos los modelos funcionales estos son montados en un articulador semi ajustable por medio de rodetes de altura, determinará la dimensión vertical, relación céntrica se realiza un escaneo de modelos y usando el sistema CAD/CAM se diseñar la barra en L y posteriormente se fabricara en aleación de cromo cobalto luego por método convencional o análogo se acrilán las dos prótesis. Conclusión: se rehabilitó al paciente con prótesis implanto soportada o híbrida inferior fija siendo una opción viable al ser segura para el paciente en el contexto de estabilidad funcional y estéticamente, la prótesis superior convencional es una opción aceptable, ya que se tiene mayor estabilidad en comparación a una convencional inferior y protege a la prótesis inferior híbrida de cargas excesivas que puedan originar fracturas o pérdida ósea a largo plazo.

Dental implant treatment is the most predictable therapeutic option in the partial or total replacement of missing teeth; currently it has a considerable success rate, however, failures do not escape them. Being a complex procedure, it is essential to have a good working protocol, i.e. a correct diagnosis in order to identify risk factors that may lead to implant failure and to restore the health, esthetics and function of our patients for many years, implementing preventive measures. The purpose of this article is to make known by means of an updated literature analysis the risk factors that could affect the osseointegration process, as well as the different lesions that can occur. Among the variables analyzed were: osteoporosis, smoking, consumption of bisphosphonates, receiving radiotherapy treatment, as well as the age and sex of the patient. The subject of the risk factors that condition the vitality of dental implants can help more professionals to contribute with alternatives and development so that dental implants continue to evolve. A bibliographic, documentary, exploratory and non-experimental, qualitative research was carried out by means of a search of articles in databases such as: Pubmed, Scielo, Google Scholar.

[Objetivo] Determinar las diferencias en el nivel óseo de implantes adyacentes a dientes e implantes. [Metodología] en este estudio transversal se incluyeron 17 pacientes con un total de 51 implantes dentales estables con carga funcional de, al menos, 6 meses. Los implantes estaban localizados adyacentes a dientes naturales/implantes o ambos. Se realizó análisis clínico completo de los implantes y dientes adyacentes, así como medición radiográfica del nivel óseo adyacente a diente o implante. Los datos fueron analizados para determinar diferencias en el nivel óseo del lado adyacente a diente en comparación con el lado adyacente a implante (p ≤ 0.05). [Resultados] La distancia de la cresta ósea en sentido coronal a la plataforma del implante, en relación con la presencia de diente o implante fue alrededor de 1 mm. El nivel óseo en zonas adyacentes a implantes se localizó más apical (2.9 ± 1.4 mm) en comparación con zonas adyacentes a dientes naturales (1.3 ± 0.07 mm; p < 0.05). El análisis bivariado indicó que es más probable encontrar el nivel óseo más coronal a la plataforma de un implante adyacente a diente, en comparación con la presencia de otro implante adyacente (p = 0.04). [Conclusión] los resultados de este estudio indican que el nivel óseo de implantes adyacentes a dientes naturales se mantiene más coronal a la plataforma del implante en comparación con las zonas adyacentes a otros implantes. Esto sugiere mayor estabilidad de implantes cuando están adyacentes a dientes naturales.

Resumen Objetivo: Describir el manejo de un caso de quiste odontogénico glandular, con tratamiento conservador, terapias adyuvantes y rehabilitación implantoasistida a los 7 años de seguimiento. Caso clínico: Se presenta un caso clínico de un paciente masculino con una lesión mandibular con diagnóstico de quiste odontogénico glandular. El tratamiento que se realizó fue una enucleación con tratamiento de superficie con solución de Carnoy y ostectomía periférica. Se realizó seguimiento clínico y radiográfico durante 7 años verificando ausencia de recidiva, momento en cual se realizó mediante planificación virtual la colocación de implantes con guías quirúrgicas de precisión para rehabilitación implanto asistida. Palabras claves: CBCT, cirugía bucal, implantes dentales, quiste odontogénico// Abstract Aim: To describe the management of a case with a glandular odontogenic cyst, treated with a conservative treatment, adyuvant therapy and guided implantologic rehabilitation at 7 years of follow up. Clinical case: A case report of a male patient with a mandibular lesion with a diagnose of glandular odontogenic cyst is presented. The treatment that was performed was an enucleation with surface treatment with Carnoy’s solution and peripheral ostectomy. Clinical and radiographic follow up was carried out for 7 years, verifying the absence of recurrence. At that time, it was done placement of implants with virtual planning and precision surgical guides for implant-assisted rehabilitation. Introduction: Microorganism infiltration through the im-plant-abutment interface causes oral health problems such as periimplantitis, leading to implant loss. Materials and Methods: A feasible new method to quantify the Streptococcus mutans (S. mutans) infiltration through the implant-abutment interface gap is introduced in the present work. Internal hexagon (IH; n = 10), external hexagon (EH; n = 10), Morse taper (MT; n = 10), and a control for each group (n = 1) were tested. Bacteria suspension was prepared at 1.5x108 CFU/mL (CFU: colony forming units), and the implants were individually submerged up to the connection level, allowing the bacteria to contact it. The abutment was removed, and bacteria count was performed. Results: The implant sets were tested under normal bacterial growth and early and late biofilm growth conditions. Colony-forming units per mL were obtained, and the results were compared among groups. Differences in bacterial count between the MT and EH (p<0.001) and the MT and IH (p<0.001) groups were significantly higher in the MT-type implant. There was a significant increment of bacterial infiltration in the MTs submitted to late biofilm growth conditions. EH and IH connections are more effective in preventing bacterial infiltration independent of the growth condition. Conclusions: The proposed methodology is feasible to evaluate the infiltration of microorganisms through the implant-abutment interface, Introducción: La infiltración de microorganismos a través de la interfaz implante-pilar provoca problemas de salud bucal como la periimplantitis, que conduce a la pérdida del implante. Materiales y Métodos: En el presente trabajo se presenta un nuevo método factible para cuantificar la infiltración de Streptococcus mutans (S. mutans) a través de la brecha de la interfaz implante-pilar. Se probaron el hexágono interno (IH; n = 10), el hexágono externo (EH; n = 10), el cono Morse (MT; n = 10) y un control para cada grupo (n = 1). Se preparó una suspensión de bacterias a 1,5x108 UFC/mL y los implantes se sumergieron individualmente hasta el nivel de conexión, permitiendo que las bacterias entraran en contacto con él. Resultados: Se retiró el pilar y se realizó recuento de bacterias. Los conjuntos de implantes se probaron en condiciones de crecimiento bacteriano normal y de crecimiento temprano y tardío de biopelículas. Se obtuvieron unidades formadoras de colonias por ml y los resultados se compararon entre grupos. Las diferencias en el recuento bacteriano entre los grupos MT y EH (p<0,001) y MT e IH (p<0,001) fueron significativamente mayores en el implante tipo MT. Hubo un incremento significativo de la infiltración bacteriana en los MT sometidos a condiciones tardías de crecimiento de biopelículas. Las conexiones EH e IH son más efectivas para prevenir la infiltración bacteriana independientemente de las condiciones de crecimiento. Conclusión: La metodología propuesta es factible para evaluar la infiltración de microorganismos a través de la interfaz implante-pilar.

Background: The dimensions of the infrazygomatic ridge must be previously analyzed for the installation of mini-dental implants, in this way to avoid loss of them in the application of different biomechanics. Objective: The present study aimed to analyze the dimensions of the infrazygomatic ridge "thickness and height" of patients treated at the "Scanner Cuenca" radiological center, in the period November 2021-June 2023, Cuenca- Ecuador Methodology: Involves the use of CBCTs to classify patients according to their skeletal pattern, sex and age. Subsequently, measurements were carried out on the CBCT images, specifically in the mesiobuccal and distobuccal root of the first permanent upper molar, as well as in the mesiobuccal root of the second permanent upper molar. Results: It was determined if there are significant differences in the dimensions of the infrazygomatic ridge between the different groups.

- Abstract
At the beginning of the COVID-19 pandemic, strict measures of confinement and social distancing were taken. Dentists were considered essential personnel and their activity was restricted to emergency treatment. The present observational study aims to determine the situation of oral implantology practice in Spain during the initial period of the COVID-19 pandemic. This is a cross-sectional observational study based on the STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) guidelines. An electronic survey consisting of three blocks of questions was sent to all members of the Spanish Society of Implants. The data were analyzed using descriptive analysis. A total of 237 participants (14.3%) responded to the questionnaire. The majority of participants (60.8%) only attended emergencies during the first 9 months of the pandemic. Despite this, 77.2% reported having performed dental implant surgeries and 75.5% indicated that they performed non-essential treatments. The activity was fully recovered by 64.1% of the surveyed dentists. The majority of dentists (90.7%) considered that sufficient/adequate preventive measures were carried out at their workplace, which possibly contributed to the fact that 49.3% were not afraid of becoming infected. This concern was significantly and directly proportionally associated with the age of the surveyed dentists. Aim: To evaluate the efficacy of a 0.03% chlorhexidine and 0.05% cetylpyridinium chloride mouth rinse, as an adjunct to professionally and patient-administered mechanical plaque removal, in the treatment of peri-implant mucositis (PiM). Material and methods: Patients displaying PiM in, at least, one implant were included in this randomized, double-blinded, clinical trial. Subjects received professional prophylaxis (baseline and 6 months) and were instructed to regular oral hygiene practices and to rinse, twice daily, with the test or placebo mouth rinses, during one year. Clinical, radiographic and microbiological outcomes were evaluated at baseline, 6 and 12 months. Disease resolution was defined as absence of bleeding on probing (BOP). Data were analysed by repeated measures ANOVA, Student's t and chi-square tests. Results: Fifty-four patients were included and 46 attended the final visit (22 in control and 24 in test group). In the test group, there was a 24.49% greater reduction in BOP at the buccal sites (95% confidence interval [3.65-45.34%]; p = 0.002) than in controls. About 58.3% of test implants and 50% controls showed healthy peri-implant tissues at final visit (p > 0.05). Conclusions: The use of the test mouth rinse demonstrated some adjunctive benefits in the treatment of PiM. Complete disease resolution could not be achieved in every case. - Abstract
Objectives: The impact of implant surfaces in dental biofilm development is presently unknown. The aim of this investigation was to assess in vitro the development of a complex biofilm model on titanium and zirconium implant surfaces, and to compare it with the same biofilm formed on hydroxyapatite surface. Methods: Six standard reference strains were used to develop an in vitro biofilm over sterile titanium, zirconium and hydroxyapatite discs, coated with saliva within the wells of pre-sterilized polystyrene tissue culture plates. The selected species used represent initial (Streptococcus oralis and Actinomyces naeslundii), early (Veillonella parvula), secondary (Fusobacterium nucleatum) and late colonizers (Porphyromonas gingivalis and Aggregatibacter actinomycetemcomitans). The developed biofilms (growth time 1 to 120h) were studied with confocal laser scanning microscopy using a vital fluorescence technique and with low-temperature scanning electron microscopy. The number (colony forming units/biofilm) and kinetics of the bacteria within the biofilm were studied with quantitative PCR (qPCR). As outcome variables, the biofilm thickness, the percentage of cell vitality and the number of bacteria were compared using the analysis of variance. Results: The bacteria adhered and matured within the biofilm over the three surfaces with similar dynamics. Different surfaces, however, demonstrated differences both in the thickness, deposition of the extracellular polysaccharide matrix as well as in the organization of the bacterial cells. Background: An accurate fit at the implant-abutment interface is an important factor to avoid biological and mechanical complications. The aim of this study was to evaluate the marginal misfit at the implant-abutment interface on external and Morse taper connection, with straight and angulated abutments under different insertion torque loads. Materials and Methods: A total of 120 implants were used, 60 with external connection (EC) and 60 with Morse taper connection (IC). Straight (SA) (n = 60) and angulated abutments (AA) (n = 60) were randomly screwed to each connection at different torque levels (n = 10 each): 10, 20 and 30 Ncm. All specimens were subjected to thermal and cyclic loading and the misfit was measured by scanning electron microscopy. Data were analyzed with one-way ANOVA, t-test and Kruskal-Wallis test. Results: Significant differences (p < 0.001) were found between connections and abutments regardless of the torque applied. Morse taper connections with straight and angulated abutments showed the lowest misfit values (0.6 µm). Misfit values decreased as torque increased. Conclusions: The misfit was affected by the type of connection. The type of abutment did not influence the fit in the Morse taper connection. 38. Structural and histological differences between connective tissue grafts harvested from the lateral palatal mucosa or from the tuberosity area

Summary: Tuberosity grafts had a greater percentage of lamina propria and lower percentage of submucosa when compared to lateral palate grafts. Objective: The study aims to understand the differences in the structural composition of soft tissue autografts harvested from the lateral palate or the tuberosity. Material and methods: Patients were randomly allocated to receive autografts harvested either from palatal or tuberosity sites to augment horizontal volume deficiencies around single-tooth implants. Tissue biopsies were analyzed for histological and histo-morphometric analysis. Picro-sirius red stain was used to evaluate collagen 1 and 3. Also, immuno-histochemical analysis was performed against MMP1, MMP2, cytokeratin-10, cytokeratin-13, and lysine hydroxylase-2. Results: Twenty specimens were harvested from 9 subjects in the lateral palate group (PG) and 11 subjects in the tuberosity group (TG). The percentage of lamina propria represented 51.08% in the PG group and 72.79% in the TG group, while the area of submucosa was minimal in the TG group representing 4.89% of the total sample vs 25.75% in the PG. The total area of COL-1 and 3 in the TG was 1.19 ± 0.57 and 0.72 ± 0.44 mm2, respectively, while in the PG, the corresponding values were 1.4 ± 0.7 and 1.04 ± 0.5 mm2. The immuno-histochemical analysis generally showed a higher expression of LLH-2, MMP2, CYT-10, and CYT-13 in the TG when compared with the PG. Conclusion: Tuberosity grafts had a greater percentage of lamina propria and lower percentage of submucosa. The collagen content in the lamina propria was similar for both groups while the immuno-histochemical profile showed differences in the antibody expression of the epithelial cells. - Abstract
This systematic literature review set out to investigate the clinical outcomes of autogenous tooth root blocks used for ridge augmentation: survival rates, block resorption, implant survival, post-surgical complications, and histology findings. This review followed PRISMA guidelines. An automated search was made in four databases, supplemented by a manual search for relevant articles published before December 2020. The quality of evidence provided was assessed with the Newcastle-Ottawa Quality Assessment Scale and the Joanna Briggs Institute Critical Appraisal tool. Seven articles fulfilled the inclusion criteria and underwent analysis. The articles included a total of 136 patients, who received 118 autogenous tooth root blocks and 26 autogenous bone blocks showing block survival rates of 99.15% and 100%, respectively. Tooth root blocks presented a mean bone gain that was similar to autologous bone blocks but showed less resorption. The implant survival rate was 98.32% for autogenous tooth root blocks. Reconstruction of alveolar crests by means of autogenous tooth root blocks appears to be a satisfactory option for single-tooth gaps and low grades of bone atrophy in terms of the survival of the bone block and the implants placed subsequently. 40. Analysis of Resorption and Need for Overcorrection in Alveolar Distraction

Purpose: The aim of this study was to evaluate the degree of resorption that occurs following a period of consolidation in alveolar distraction osteogenesis and assess the amount of overcorrection necessary to achieve the planned height. Materials and methods: A retrospective observational study was conducted comprising 19 surgically treated patients, where a total of 19 extraosseous distractors were placed. All patients undergoing surgery had a panoramic radiograph and a computed tomography scan, both at the end of distraction and at the end of the consolidation period. To assess the degree of resorption, the distance from the mandibular border, maxillary sinus floor, or nasal floor to the limit of the crestal bone following consolidation was compared with the height recorded at the end of the distraction period. Results: Resorption occurred in all cases ranging from 7% to 25%. - Abstract
Purpose: To evaluate the influence of implant scan body (ISB) design (height, diameter, geometry, material, and retention system) on the accuracy of digital implant scans. Material and methods: A literature search was completed in five databases: PubMed/Medline, Scopus, Embase, World of Science, and Cochrane. A manual search was also conducted. Studies reporting the evaluation of ISB design on the accuracy of digital scans obtained by using IOSs were included. Two investigators evaluated the studies independently by applying the Joanna Briggs Institute critical appraisal. A third examiner was consulted to resolve any lack of consensus. Articles were classified based on the ISB features of height, geometry, material, and retention system. Results: Twenty articles were included. Among the reviewed studies, 11 investigations analyzed the influence of different ISB geometries, 1 study assessed the impact of ISB diameter, 4 studies investigated the effect of ISB splinting, 2 articles evaluated ISB height, and 2 studies focused on the effect of ISB material on scan accuracy. In addition, 8 studies involved ISBs fabricated with different materials (1- and 2-piece polyetheretherketone and 1-piece titanium ISBs), and all of the reviewed articles tested screw-retained ISBs, except for 3 in vitro studies. Conclusions: The findings did not enable concrete conclusions regarding the optimal ISB design, whether there is a relationship between IOS technology and a specific ISB design, or the clinical condition that maximizes intraoral scanning accuracy. - Abstract
Purpose: To evaluate short-term clinical and radiographic outcomes of bone regeneration procedures using thin cortical porcine xenogeneic bone plates in combination with autogenous bone chips compared with thin autogenous cortical plates and autogenous bone chips. Materials and methods: A total of 19 patients (12 women and 7 men, mean age 58.24 ± 3.09 years) were randomly allocated to two different groups regarding surgical procedure: autogenous cortical plates (ACP group) and xenogeneic cortical plates (XCP group). Preoperative CBCT scans were performed for each patient. Surgical time and postoperative pain were recorded, as well as tissue healing and graft resorption after 4 months, then another surgical procedure was performed to place dental implants. Data were analysed using an analysis of covariance. Results: Twenty-one surgical procedures were performed on 19 patients (10 from the XCP group and 9 from the ACP group). The operative time was significantly lower in the XCP group (25.45 ± 3.88 minutes) than in the ACP group (44.10 ± 3.60 minutes). The XCP group also showed less pain, but not significantly less, than the ACP group. The graft resorption rate in the ACP and XCP groups was 2.03 ± 1.58% and 3.49 ± 2.38% respectively, showing no statistically significant difference. Conclusions: Despite the limited sample size and non-uniform distribution between the maxilla and mandible as surgical sites, the results suggest that XCP and ACP grafts are similar in terms of bone volume gain and graft resorption rate, with no significant differences in wound healing or complication rate. Objectives: To compare biofilm formation on whole dental titanium implants with different surface micro‐topography. Methods: A multispecies in vitro biofilm model consisting of initial (Streptococcus ora‐lis and Actinomyces naeslundii), early (Veillonella parvula), secondary (Fusobacterium nucleatum) and late colonizers (Porphyromonas gingivalis and Aggregatibacter actino‐mycetemcomitans) was grown for 96 hr on sterile titanium dental implants with either minimal (Sa: 0.5–1.0 mm) or moderate‐roughness titanium surfaces (Sa: 1.1–2.0 mm). The resulting biofilms were studied with Confocal Laser Scanning Microscopy (CLSM) and Scanning Electron Microscope. Concentrations (colony‐forming units per mL [CFU/ml]) of each bacterium were measured by quantitative Polymerase Chain Reaction (qPCR) and compared by Student t tests. Results: A biofilm, located mainly at the peak and lateral areas of the implant threads, was observed on both implant surfaces, with a greater biomass and a greater live/dead ratio in moderate‐ compared to minimal‐roughness surface implants. Statistically sig ‐nificant higher values of total bacteria (mean difference = 2.61 × 107 CFU/ml; 95% confidence interval — CI [1.91 × 106; 5.02 × 107]; p = 0.036), F. Nucleatum (mean difference = 4.43 × 106 CFU/ml; 95% CI [1.06 × 106; 7.80 × 106]; p = 0.013) and A. actinomycetemcomitans (mean difference = 2.55 × 107 CFU/ml; 95% CI [1.07 × 107; 4.04 × 107]; p = 0.002), were found in the moderate‐ compared to minimal‐roughness surface dental implants. Background: A main drawback of bone block graft surgery is the resorption occurring in early stages of healing. To our knowledge, there are no studies comparing out- comes of freeze-dried bone allograft (FDBA) blocks with different architecture. Purpose: The aim of this work was to investigate different factors that can affect graft resorption and to compare the resorption rates of two different types of alloge- neic blocks, corticocancellous and cancellous. Materials and Methods: A randomized clinical trial was designed. Twenty-eight patients referred for onlay bone augmentation prior to implant placement were included in the study. Preoperative computerized tomography (CT) was taken for all patients. Patients received FDBA blocks of either cancellous or corticocancellous bone obtained from the iliac crest. After a 4-month follow-up, postoperative CT was taken. Then, another surgery was performed, with the purpose to place dental implants. The aforementioned groups were compared for bone resorption and implant outcome using analysis of covariance (ANCOVA) and repeated ANOVA mea- sures, respectively. Demographic data, trabecular bone density, and graft sites were also analyzed. Results: A total of 93 implants were placed in the augmented bone sites over 28 patients. A 100% survival rate was achieved during a mean follow-up period of 24 months in both groups. Higher bone resorption rate was found with cancellous bone grafts (29.2%±2.6) compared with corticocancellous grafts (19.3%±2.3). Moreover, higher resorption rates in patients with lower bone density (<185 Houns- field Units) (31.7% ± 3.1) and smokers (26.39% ± 2.3) were observed when compared with patients with higher bone density (>185 Hounsfield Units) (16.8% ± 2.1) and nonsmokers (22.1% ± 2.3), respectively. - Abstract
Background: Implant rehabilitation of posterior mandibular defects is frequently associated to a horizontal bone loss. There exist several regenerative techniques to supply this bone deficiency, one of which is the Periosteal Pocket Flap Technique (PPF) proposed by Steigmann et al. to treat small horizontal bone defects. The present study proposes a modification of this technique based on the concurrent use of PPF with the use of xenogeneic and autologous bone and Plasma Rich in Growth Factors (PRGF). The aim of this study is to evaluate clinical and radiographic outcomes of the PPF with the use of xenogeneic and autologous bone and PRGF in comparison with conventional Guided Bone Regeneration (GBR) procedures. Methods: Nine patients were enroled in the study (7 women and 2 men, mean age: 53 ± 2.74 years) and allocated to PPF or GBR. In both groups implant placement was performed simultaneously to bone regeneration. Preoperative CBCT scans were performed for each patient. Surgical time and postoperative pain were recorded, as well as tissue healing. Moreover, horizontal bone gain (mm), graft surface area (mm2) and graft volume (mm3) were evaluated. Results: Nine surgeries were performed: 6 PPF and 3 GBR. Regarding clinical outcomes, operative time was significative greater in GBR group than in PPF group (51.67 ± 3.51 min vs. 37 ± 5.69 min; p = 0.008). Postoperative pain was higher in GBR compared to PPF (p = 0.011). Regarding radiographical results, there were not significant differences in horizontal bone gain (PPF: 9.43 ± 1.8 mm; GBR: 9.28 ± 0.42 mm), surface area (PPF: 693.33 ± 118.73 mm2; GBR: 655.61 ± 102.43 mm2), and volume (PPF: 394.97 ± 178.72 mm3; GBR: 261.66 ± 118 mm3) between groups. Conclusions: This prospective study demonstrates that the combination of autograft/xenograft and PRGF in PPF technique is a simpler, cheaper, and faster technique than GBR technique for achieving moderate lateral bone augmentation in implant treatment. Objectives: The main objective of this prospective study was to evaluate the long-term outcome of implant therapy in liver transplant patients (LTP). The secondary goal was to assess several implant- and patient-dependent variables, such as peri-implantitis (PI), peri- implant mucositis (PIM), bone loss (BL), and immediate postoperative complications. Material and methods: Two groups, including 16 pharmacologically immunosup- pressed LTP and 16 matched controls, received 52 and 54 implants, respectively, be- tween 1999 and 2008. After evaluating the postoperative healing, a mean follow-up of more than 8 years was carried out, and radiographic, clinical, and periodontal pa- rameters were recorded to evaluate implant survival and implant- and patient- dependent outcomes. Results: The early postsurgical complications were similar in both groups. Implant sur- vival rate was 100% in the LTP group and 98.15% in the CG. PIM was diagnosed in 35.42% of the implants and 64.29% of the patients of LTP group (LTPG) and in 43.40% of the implants and 56.25% of the patients in the CG. PI was detected in 4.17% of the implants and 7.10% of the patients in the LTPG and in 9.43% of the implants and 18.80% of the patients in the CG. Conclusion: Pharmacologically immunosuppression in liver transplant patients was not a risk factor for implant failure, nor for the incidence of peri-implant diseases. Objective: To determine the efficacy of a gel containing propolis extract, nanovitamin C and nanovitamin E as adjuvant to mechanical debridement in the treatment of peri- implant mucositis (PM). Background: Propolis has anti-inflammatory and antibacterial effect that may im- prove peri-implant health. Methods: A randomized, double-blind study was performed on patients with at least one implant with PM. Participants received a professional prophylaxis and were in- structed to use either test or a control gel as toothpaste three times/day for 1 month. Clinical and microbiological parameters were evaluated. PM resolution was considered in absence of bleeding on probing (BOP). Data were analysed with Mann-Whitney U, Wilcoxon signed-rank and chi-square tests. Results: Forty-six patients participated (23 in each group). After treatment, 26.1% of test patients showed complete PM resolution versus 0% in control group (p = .02). Significant reductions were observed in plaque index (p = .03), BOP (p = .04) and prob- ing depths (p = .027) in test compared with control group. The reduction in Tannerella forsythia was statistically greater in test than in control group at 1-month follow-up (p = .02). Porphyromonas gingivalis was statistically reduced in test group from baseline to 1-month follow-up (p = .05). Conclusion: Test gel clinically improved PM and showed certain antimicrobial effect after 1 month in comparison with control group. The aim of this study was to assess the effectiveness of minimally invasive antral membrane balloon elevation (MIAMBE). Twenty-seven patients with severe resorption of the posterior maxilla were treated by balloon catheter-assisted sinus lift procedure with deferred implant placement. Panoramic radiographs and computed tomography scans were obtained prior to surgery and at 6 months after surgery, before implantation. Data collected following surgery included inflammation, pain, bleeding, infection, and haematoma. Pain and inflammation were recorded using a six-point verbal rating scale (VRS). The patients were followed up for an average of 15 months. The initial sinus floor height was measured for each planned implant and compared with the height at 6 months post-surgery. The average bone height gain was 8.10 ± 3.45 mm (range 0.5–13.95 mm). Inflammation on the VRS ranged from 0 to 3 (mean 0.97 ± 0.85), while pain ranged from 0 to 4 (mean 0.87 ± 1.19). There was a perforation of the Schneiderian membrane smaller than 2 mm in one case. In another case, the balloon lift procedure had to be aborted and changed to the conventional Tatum technique due to breakage of the balloon inside the sinus. The results of this study show the balloon sinus lift technique to be an easy procedure to perform, with apparently low rates of inflammation and pain, and to provide sufficient quantity and quality of bone for the placement of osseointegrated implants., Depto. de Especialidades Clínicas Odontológicas, Fac. de Odontología, TRUE, pub

This study evaluated bone density, morphological changes, and histomorphometric results undergone by alveolar bone after applying a new biomaterial composed of calcium phosphate modified with silicon (CAPO-Si) compared with hydroxyapatite of bovine origin (BHA). Alveolar ridge preservation (ARP) was performed in 24 alveoli, divided into a test group filled with CAPO-Si and a control group filled with BHA. Three months later, the mineral bone density obtained by the biomaterials, horizontal and vertical bone loss, the degree of alveolar corticalization, and histomorphometric results were evaluated. Both biomaterials presented similar behavior in terms of densitometric results, vertical bone loss, and degree of alveolar corticalization. Alveoli treated with CAPO-Si showed less horizontal bone loss in comparison with alveoli treated with BHA (0.99 ± 0.2 mm vs. 1.3 ± 0.3 mm), with statistically significant difference (p = 0.017). Histomorphometric results showed greater bone neoformation in the test group than the control group (23 ± 15% vs. 11 ± 7%) (p = 0.039) and less residual biomaterial (5 ± 10% vs. 17 ± 13%) (p = 0.043) with statistically significant differences. Purpose: To assess the accuracy of totally guided implant placement with static surgical splints in relation to the different types of supporting tissues (tooth, mucosa, or bone). Materials and Methods: This review was carried out following the PRISMA guidelines. An electronic search was done of the MEDLINE (PubMed), Embase, and Cochrane Library databases, without publication year or language restrictions. Results: The literature search yielded a total of 877 articles; 18 were included in the qualitative synthesis, and 16 of these articles were included in the quantitative analysis. The included studies presented a high risk of bias, except for one randomized clinical trial. The strength of the recommendations is therefore weak. In the angular deviation treatment, statistically significant differences were observed in the accuracy of the implants with tooth vs bone support: Bone support yielded 1.31 degrees greater deviation vs tooth support (SD = 0.43; 95% CI: 0.47, 2.15, P = .002). No significant differences were observed in the linear deviations. Conclusion: Tooth support proved to be significantly more precise than bone support splints.
