29. Development of in vitro infection model methods for Gram-negative bacteria to assess potential broad-spectrum anti-virulence inhibitors

Abstract

Bacteria exhibit common virulence targets which can be inhibited for potential broad-spectrum activity across a range of bacterial infections. The macrophage infectivity potentiator (Mip) is a bacterial protein that has peptidylprolyl cis/trans isomerase activity and is part of the FK506-binding protein subgroup, in the superfamily of immunophilins. They have previously been identified to be involved in intracellular virulence of Legionella pneumophila and Burkholderia pseudomallei and inhibition of the protein with pipecolic acid derived small-molecule inhibitors resulted in a decrease in virulence. Due to the highly conserved nature of Mip proteins it is believed that these proteins exist in Klebsiella pneumoniae and Burkholderia cenocepacia; two pathogens contributing to a rise in multi-drug resistant infections. Therefore, this study aims to examine the potential broad-spectrum activity of the Mip inhibitors in K. pneumoniae and B. cenocepacia by developing in vitro model methods for inhibitor evaluation. The presence of putative Mip proteins in K. pneumoniae and B. cenocepacia were confirmed with the use of online bioinformatics tools. The Mip inhibitors were then examined by first developing in vitro cell based methods to identify the magnitude at which K. pneumoniae and B. cenocepacia adhered to macrophages, and internalised, survived and replicated within the macrophages. An optimised adherence assay, and internalisation, survival and replication assay were then used to test the efficacy of the Mip inhibitors in K. pneumoniae and B. cenocepacia. The results obtained demonstrated that K. pneumoniae and B. cenocepacia were able to adhere to the macrophages, however, the concentration at which they adhered varied between strains. Only two strains of K. pneumoniae were internalised into the macrophages and only one strain was able to replicate at low concentrations within the macrophages during the 24 hours post-infection. Abstract

Melioidosis, caused by the gram negative bacterium Burkholderia pseudomallei, is endemic in northern Australia. Using data collated from centres in Western Australia, the Northern Territory and Queensland, this report describes the epidemiology of this disease between 1 November, 2001 and 31 October, 2002. There were 47 cases seen during this period with an average annual incidence of 5.8 cases per 100,000 population. In Indigenous Australians, an incidence of 25.5 cases per 100,000 population was seen. The timing and location of cases was generally correlated with rainfall across northern Australia. A case-cluster in a Queensland community was associated with post-cyclonic flooding. Risk factors included diabetes, alcohol-related problems and renal disease. Pneumonia (51%) was the most common clinical diagnosis. The mortality rate attributable to melioidosis was 21 per cent, although a number of other patients died of underlying disease. Despite improvements in recognition and treatment, melioidosis is still associated with a high morbidity and mortality, particularly in Indigenous Australians.

Abstract

ABSTRACTRespiratory tract colonization with Scedosporium apiospermumin patients with chronic suppurative lung disease is a significant concern for lung transplantation candidates, sinceScedosporiuminfections occurring posttransplantation are usually untreatable. Up to 10% of patients with cystic fibrosis attending our respiratory medicine unit have hadScedosporiumorganisms isolated from sputum samples. We therefore developed a molecular typing method to examine these isolates. Typing by PCR amplification of ribosomal intergenic spacer sequences demonstrated 20 different types from 52 isolates collected from the respiratory medicine unit and elsewhere in Australia. A single common type was isolated from 11 respiratory medicine unit inpatients. Two other types were isolated from more than one source: one from two respiratory medicine unit inpatients and one from two epidemiologically linked nonhuman sources. Multiple isolates were obtained from nine patients. This method demonstrated persistent carriage of isolates of the same type in one patient for 7 months. Two patients showed carriage of isolates with multiple typing patterns within a 3-month period. The high rate of isolation and the predominance of isolates with a single typing pattern from respiratory medicine unit patients may suggest transmission to patients from a source in the unit. There was no epidemiological evidence of direct patient-to-patient spread, andScedosporiumorganisms were not isolated from dust, soil, or air samples from the unit. The source and route of transmission have yet to be determined.

Abstract

The article focuses on a soil-borne infection called melioidosis that was detected after the Indian Ocean tsunami in 2004. Experiments revealed that the Burkholderia pseudomallei, a bacteria that causes melioidosis, occurs in distilled water, which indicates a possibility that it lies in deep soil layers. The vulnerability of such bacteria to ultraviolet light is cited as a possible reason for the increased risk of melioidosis during wet season. Also mentioned is the absence of vaccine for such soil-borne infection.

Abstract

In some respects I still feel like a new arrival, since every now and then I discover something else that is done differently here in Australia. The third edition of Gardiner and Peel's book on sterilisation and disinfection has been one of those discoveries. The book is not as well-known in Europe, unjustifiably so. For those, like me, who are not familiar with Sterilisation, Disinfection and Infection Control, it provides a comprehensive introduction to current methods in sterilisation and disinfection practice in our hospitals.

Abstract

Background. The COVID-19 pandemic demonstrated a need for robust SARS-CoV-2 test evaluation infrastructure to underpin biosecurity and protect the population during a pandemic health emergency. Gap statement. The first generation of rapid antigen tests was less accurate than molecular methods due to their inherent sensitivity and specificity shortfalls, compounded by the consequences of self-testing. This created a need for more accurate point-of-care SARS-CoV-2 detection methods. Aim. Here we present the lessons-learned during the COVID-19 emergency response in Western Australia including the detailed set-up, evaluation and operation of rapid antigen test in a state-run drive-through sample collection service during the COVID-19 pandemic after the strict border shutdown ended. Methods. We report a conformity assessment of a novel, second-generation rapid antigen test (Virulizer) comprising a technician-operated rapid lateral flow immunoassay with fluorescence-based detection. Results. The Virulizer rapid antigen test demonstrated up to 100% sensitivity (95% CI: 61.0-100%), 91.94% specificity (95% CI: 82.5-96.5%) and 92.65% accuracy when compared to a commercial PCR assay method. Wide confidence intervals in our series reflect the limits of small sample size. Nevertheless, the Virulizer assay performance was well-suited to point-of-care screening for SARS-CoV-2 in a drive-through clinic setting. Conclusion. The adaptive evaluation process necessary under changing pandemic conditions enabled assessment of a simple sample collection and point-of-care testing process, and showed how this system could be rapidly deployed for SARS-CoV-2 testing, including to regional and remote settings.

Abstract

Peritoneal dialysis (PD) peritonitis cases require rapid clinical interventions to ensure the best possible patient outcomes. Culture-dependent microbiology tools are slow and cannot provide clinicians with evidence to guide antimicrobial prescription practices in an appropriate time frame. Genotypic methods have met with limited success for analyzing continuous ambulatory PD effluent, with most centers still relying on culture-dependent microbiology. We present a case study in which we apply flow cytometry techniques to antibiotic-compromised effluent. We demonstrate, with supporting evidence, direct enumeration of bacterial and human immune cells, with results reported within 2 hours of receiving the clinical specimen., (Copyright © 2018 International Society for Peritoneal Dialysis.)
