Objective The advent of new technologies has made it possible to explore alternative ventilator manufacturing to meet the worldwide shortfall for mechanical ventilators especially in pandemics. We describe a method using rapid prototyping technologies to create an electro-mechanical ventilator in a cost effective, timely manner and provide results of testing using an in vitro–in vivo testing model. Results Rapid prototyping technologies (3D printing and 2D cutting) were used to create a modular ventilator. The artificial manual breathing unit (AMBU) bag connected to wall oxygen source using a flow meter was used as air reservoir. Controlled variables include respiratory rate, tidal volume and inspiratory: expiratory (I:E) ratio. In vitro testing and In vivo testing in the pig model demonstrated comparable mechanical efficiency of the test ventilator to that of standard ventilator but showed the material limits of 3D printed gears. RESUMO O objetivo deste estudo foi comparar a endurance muscular inspiratória e as respostas hemodinâmicas de indivíduos obesos e eutróficos. Trata-se de um estudo transversal com amostra composta por 20 indivíduos obesos (31±6 anos, 10 homens, 37,5±4,7 kg/m2) e 20 indivíduos eutróficos (29±8 anos, 10 homens, 23,2±1,5 kg/m2). A força muscular inspiratória e expiratória foi mensurada por manovacuometria, através da determinação da pressão inspiratória máxima e da pressão expiratória máxima. A endurance muscular inspiratória foi determinada por meio de exercício inspiratório com carga progressiva, iniciado com carga de 50% da pressão inspiratória máxima por 3 minutos, seguidos de incremento de 10% a cada 3 minutos até que o indivíduo fosse incapaz de continuar o teste. Verificou-se que os indivíduos obesos (470 ± 326 seg) apresentaram endurance muscular inspiratória reduzida em comparação com os eutróficos (651 ± 215 seg). A força muscular inspiratória e expiratória não diferiu entre os grupos. O teste de exercício progressivo induziu aumento da pressão arterial sistólica, diastólica e média e da frequência cardíaca, semelhante em indivíduos obesos e eutróficos. Foi possível concluir que embora a endurance muscular inspiratória de indivíduos obesos seja menor que a de eutróficos, as respostas hemodinâmicas induzidas pelo teste de endurance muscular inspiratória foram semelhantes nos dois grupos. RESUMEN Este estudio pretende comparar la resistencia muscular inspiratoria con las respuestas hemodinámicas de individuos obesos y eutróficos. Se trata de un estudio transversal con la muestra formada por 20 individuos obesos (31±6 años, 10 hombres, 37,5±4,7 kg/m2) y 20 individuos eutróficos (29±8 años, 10 hombres, 23,2±1,5 kg/m2). La fuerza muscular inspiratoria y espiratoria se midió por manovacuometría, por medio de la evaluación de la presión inspiratoria máxima y la presión espiratoria máxima. Se evaluó la resistencia muscular inspiratoria mediante el ejercicio inspiratorio con carga progresiva, que se inició con un 50 % de carga de la presión inspiratoria máxima durante 3 minutos, siendo aumentada en un 10 % cada 3 minutos hasta que el individuo no pudiese continuar la prueba. Se encontró que los individuos obesos (470±326 seg) tuvieron menor resistencia muscular inspiratoria comparados a los eutróficos (651±215 seg). La fuerza muscular inspiratoria y espiratoria no difirió entre los grupos. La prueba de esfuerzo progresiva provocó el aumento de la presión arterial sistólica, diastólica y media, así como de la frecuencia cardíaca, tanto en los individuos obesos como en los eutróficos. ABSTRACT The objective of this study was to compare inspiratory muscle endurance and hemodynamic responses between obese and eutrophic individuals. In this cross-sectional study participated 20 obese individuals (31±6 years old, 10 men, 37.5±4.7 kg/m2) and 20 healthy subjects (29±8 years old, 10 men, 23.2±1.5 kg/m2). Inspiratory and expiratory muscle strength were measured by a manovacuometry through the determination of the maximum inspiratory pressure (MIP) and the maximum expiratory pressure (MEP), respectively. Inspiratory muscle endurance was determined by na incremental test with an initial load of 50% MIP and increments of 10% MIP every 3 minutes until the individual was unable to continue the test. Obese (470±326 sec) showed a reduction in inspiratory muscle endurance compared to eutrophic individuals (651±215 sec). Inspiratory and expiratory muscle strength did not differ between groups. Inspiratory muscle endurance test induced a similar increase in systolic, diastolic and mean arterial pressure, as well as in heart rate in obese and eutrophic individuals. In conclusion, obese show reduction of inspiratory muscle resistance compared to eutrophic individuals, but the hemodynamic responses induced by inspiratory muscle resistance test did not differ between obese and eutrophic patients.

Background and Aims: Modern anesthetic practice utilizes low-flow anesthesia with evolving evidence on its pulmonary effects. Studies comparing measurement of vital capacity and inspiratory reserve volume using respirometer in both low-flow and high-flow anesthesia are sparse. We evaluated the effects of low-flow and high-flow anesthesia on postoperative pulmonary functions using respirometer. Material and Methods: This was a prospective randomized double blind study wherein One hundred and ten patients undergoing peripheral surgeries under general anesthesia were allocated into two groups Group I- Low-flow anesthesia with O2 + N2O + Sevoflurane (0.5L + 0.5L + 3.5%) and Group II- High-flow anesthesia with O2 + N2O + Sevoflurane (2L + 2L + 2%). The difference in vital capacity (VC), inspiratory reserve volume (IRV), and peak expiratory flow rates (PEFR) from the preoperative period were compared in both the groups postoperatively. Results: The difference in VC, IRV, and PEFR measured in both the groups between the preoperative and postoperative period were found to be similar and statistically insignificant (P - 0.173, 1.00 and 0.213 respectively). The difference in single breath count (SBC), breath holding time (BHT), and respiratory rates (RR) were also similar in both the groups (P – 0.101, 0.698, and 0.467) respectively. Conclusions: The pulmonary effects of low-flow anesthesia are comparable with the high-flow ones in patients undergoing elective surgeries under general anesthesia.

Intra-abdominal hypertension during laparoscopic operations increased the risk of complications from cardiovascular and respiratory systems. An application of laparolifting systems allowed doctors to avoid changes of pneumoperitoneum, although it was associated with technical difficulties in operation performance. The authors used a test in order to determine cardiorespiratory reserve in preoperative period. The reserve was characterized by decrease of stroke volume of the heart against the background of intra-abdominal hypertension. There was noted a reliable increase of complication rate in these patients in case of application of standard laparoscopic operation compared with operation using lifting systems.

Dyspnea has been reported to be a main contributor to exercise avoidance in asthma. While traditional markers of ventilation do not explain the heightened dyspnea during exercise in patients with asthma, this study proposed that exertional dyspnea in asthma was due to high-operating lung volumes, which may be improved with a short-acting β2-agonist. On two separate days, 16 patients with asthma and 16 controls completed a lung function test and incremental exercise tests to exhaustion. On one of the days (order randomized), 400 µg salbutamol was administered before exercise. Inspiratory capacity (IC), inspiratory reserve volume (IRV), and dyspnea (modified Borg scale) were evaluated throughout exercise. Compared with controls, patients with asthma reported greater dyspnea at the same absolute submaximal workloads. Furthermore, patients with asthma demonstrated altered breathing responses to exercise, characterized by reduced IC and IRV throughout exercise compared with controls. The reduced IRV was associated with increased dyspnea in patients with asthma. Salbutamol did not affect dyspnea or operating lung volumes in either group. The increased perception of dyspnea during incremental exercise in patients with asthma appears to be secondary to a reduction in IRV, which is unaffected by an inhaled β2-agonist. NEW & NOTEWORTHY Increased exertional dyspnea in asthma appears to be due to high operating lung volumes and is not affected by salbutamol.

Assessment of dynamic inspiratory function may provide valuable information about the degree and progression of pulmonary involvement in patients with Duchenne muscular dystrophy (DMD). The aims of this study were to characterize inspiratory function and to assess the efficacy of idebenone on this pulmonary function outcome in a large and well-characterized cohort of 10-18 year-old DMD patients not taking glucocorticoid steroids (GCs) enrolled in the phase 3 randomized controlled DELOS trial. We evaluated the effect of idebenone on the highest flow generated during an inspiratory FVC maneuver (maximum inspiratory flow; V'I,max(FVC)) and the ratio between the largest inspiratory flow during tidal breathing (tidal inspiratory flow; V'I,max(t)) and the V'I,max(FVC). The fraction of the maximum flow that is not used during tidal breathing has been termed inspiratory flow reserve (IFR). DMD patients in both treatment groups of DELOS (idebenone, n = 31; placebo: n = 33) had comparable and abnormally low V'I,max(FVC) at baseline. During the study period, V'I,max(FVC) further declined by -0.29 L/sec in patients on placebo (95%CI: -0.51, -0.08; P = 0.008 at week 52), whereas it remained stable in patients on idebenone (change from baseline to week 52: 0.01 L/sec; 95%CI: -0.22, 0.24; P = 0.950). The between-group difference favoring idebenone was 0.27 L/sec (P = 0.043) at week 26 and 0.30 L/sec (P = 0.061) at week 52. In addition, during the study period, IFR improved by 2.8% in patients receiving idebenone and worsened by -3.0% among patients on placebo (between-group difference 5.8% at week 52; P = 0.040). Although the clinical interpretation of these data is currently limited due to the scarcity of routine clinical practice experience with dynamic inspiratory function outcomes in DMD, these findings from a randomized controlled study nevertheless suggest that idebenone preserved inspiratory muscle function as assessed by V'I,max(FVC) and IFR in patients with DMD. Electrical impedance tomography (EIT) has been used to study regional ventilation distribution in neonatal and paediatric lung disease; however, little information has been obtained in healthy newborns and infants. Data on regional ventilation distribution and regional filling characteristics were obtained using EIT in the neonatal period, at 3 and 6 months of age, in spontaneously breathing infants during non-rapid eye movement sleep. Regional ventilation distribution was described using regional end-expiratory and end-inspiratory impedance amplitudes, and geometric centre of ventilation. Regional filling characteristics were described with the phase lag or lead of the regional impedance change in comparison to global impedance change. 32 infants were measured in the supine position. Regional impedance amplitudes increased with age but regional ventilation distribution remained unchanged in all infants at any age, with the dependent (posterior) lung always better ventilated. Regional filling characteristics showed that the dependent lung filled during inspiration before the nondependent lung during all follow-up measurements. Regional ventilation distribution and regional filling characteristics remained unchanged over the first 6 months of life, and the results obtained on regional ventilation distribution are very similar to those in adult subjects.

Background: It has yet to be determined whether the language of dyspnea responds to pulmonary rehabilitation programs (PRP). Objective: We tested the hypothesis that PRP affect both the intensity and quality of exercise-induced dyspnea in patients with chronic obstructive pulmonary disease (COPD). Methods: We studied 49 patients equipped with a portable telemetric spiroergometry device during the 6-min walking test before and 4 weeks after PRP. In a first screening visit, appropriate verbal descriptors of dyspnea were chosen that patients were familiar with during daily living activities. Tidal volume, respiratory frequency, inspiratory capacity, inspiratory reserve volume (IRV) and dyspnea intensity were evaluated by a modified Borg scale every minute during the test. Results: Qualitative descriptors of dyspnea were defined by three different sets of cluster descriptors (a–c) at the end of the exercise test, before and after PRP: a – work/effort (W/E); b – inspiratory difficulty (ID) and chest tightness (CT), and c – W/E, ID and/or CT. The three language subgroups exhibited similar lung function at baseline, and similar rating of dyspnea and ventilatory changes during exercise. The rehabilitation program shifted the Borg-IRV relationship (less Borg at any given IRV) towards the right without modifying the set of descriptors in most patients. Conclusions: Rehabilitation programs allowed patients to tolerate a greater amount of restrictive dynamic ventilatory defect by modifying the intensity, but not necessarily the quality of dyspnea.

Cardiac response to breath-hold diving in human beings is primarily characterized by the reduction of both heart rate and stroke volume. By underwater Doppler-echocardiography we observed a "restrictive/constrictive" left ventricular filling pattern compatible with the idea of chest squeeze and heart compression during diving. We hypothesized that underwater re-expansion of the chest would release heart constriction and normalize cardiac function. To this aim, 10 healthy male subjects (age 34.2 +/- 10.4) were evaluated by Doppler-echocardiography during breath-hold immersion at a depth of 10 m, before and after a single maximal inspiration from a SCUBA device. During the same session, all subjects were also studied at surface (full-body immersion) and at 5-m depth in order to better characterize the relationship of echo-Doppler pattern with depth. In comparison to surface immersion, 5-m deep diving was sufficient to reduce cardiac output (P = 0.042) and increase transmitral E-peak velocity (P < 0.001). These changes remained unaltered at a 10-m depth. Chest expansion at 10 m decreased left ventricular end-systolic volume (P = 0.024) and increased left ventricular stroke volume (P = 0.024). In addition, it decreased transmitral E-peak velocity (P = 0.012) and increased deceleration time of E-peak (P = 0.021). In conclusion the diving response, already evident during shallow diving (5 m) did not progress during deeper dives (10 m). The rapid improvement in systolic and diastolic function observed after lung volume expansion is congruous with the idea of a constrictive effect on the heart exerted by chest squeeze.

Objective It has proved that muscle paralysis was more protective for injured lung in severe acute respiratory distress syndrome (ARDS), but the precise mechanism is not clear. The purpose of this study was to test the hypothesis that abdominal muscle activity during mechanically ventilation increases lung injury in severe ARDS. Methods Eighteen male Beagles were studied under mechanical ventilation with anesthesia. Severe ARDS was induced by repetitive oleic acid infusion. After lung injury, Beagles were randomly assigned into spontaneous breathing group (BIPAPSB) and abdominal muscle paralysis group (BIPAPAP). All groups were ventilated with BIPAP model for 8h, and the high pressure titrated to reached a tidal volume of 6ml/kg, the low pressure was set at 10 cmH2O, with I:E ratio 1:1, and respiratory rate adjusted to a PaCO2 of 35–60 mmHg. Six Beagles without ventilator support comprised the control group. Respiratory variables, end-expiratory volume (EELV) and gas exchange were assessed during mechanical ventilation. The levels of Interleukin (IL)-6, IL-8 in lung tissue and plasma were measured by qRT-PCR and ELISA respectively. Lung injury scores were determined at end of the experiment. Results For the comparable ventilator setting, as compared with BIPAPSB group, the BIPAPAP group presented higher EELV (427±47 vs. 366±38 ml) and oxygenation index (293±36 vs. 226±31 mmHg), lower levels of IL-6(216.6±48.0 vs. 297.5±71.2 pg/ml) and IL-8(246.8±78.2 vs. 357.5±69.3 pg/ml) in plasma, and lower express levels of IL-6 mRNA (15.0±3.8 vs. 21.2±3.7) and IL-8 mRNA (18.9±6.8 vs. 29.5±7.9) in lung tissues. In addition, less lung histopathology injury were revealed in the BIPAPAP group (22.5±2.0 vs. 25.2±2.1). Conclusion Abdominal muscle activity during mechanically ventilation is one of the injurious factors in severe ARDS, so abdominal muscle paralysis might be an effective strategy to minimize ventilator-induce lung injury.

Purpose: To evaluate the relations between %HRmax, %HRR, %V?O2max, and %V?O2R in elite cyclists and to check whether the intensity scale recommended by ACSM in its 1998 position stand is also applicable to this specific population. Methods: Twenty-six male elite road cyclists (25.1 ± 0.7 yr, 71.0 ± 1.2 kg, 70.9 ± 1.2 mL·kg-1·min-1, 433.9 ± 9.8 W) performed an incremental maximal exercise test (50 W·3 min-1). Individual linear regressions based on HR and V?O2 values measured at rest, end of each stage, and maximum, were used to calculate slopes and intercepts, and to predict %HRmax, %HRR, %V?O2max, or %V?O2R for a given exercise intensity. Results: Below 85% V?O2max or V?O2R, predicted %HRmax values were significantly higher (P < 0.001) than the ACSM intensity scale (58, 65, 73, and 87% vs 55, 62, 70, and 85% HRmax at 40, 50, 60, and 80% V?O2max, and 48, 61, 74% vs 35, 55, and 70% HRmax at 20, 40, and 60% V?O2R). The %HRR versus %V?O2max regression mean slope (1.069 ± 0.01) and intercept (-5.747 ± 0.80) were significantly different (P < 0.0001) from 1 and 0, respectively. Conversely, the %HRR versus %V?O2R regression was indistinguishable from the line of identity (mean slope = 1.003 ± 0.01; mean intercept = 0.756 ± 0.7). Predicted %V?O2R values were equivalent to %HRR in the 35-95%HRR range. %V?O2max was equivalent to %HRR at and above 75%HRR, and it was significantly higher at (P < 0.05) and below 65%HRR (P < 0.001). Conclusion: The intensity scale recommended by ACSM underestimates exercise intensity in elite cyclists. Prediction of %HRR by %V?O2R is better than by %V?O2max. Thus, elite cyclists should use %HRR in relation to %V?O2R rather than in relation to %V?O2max.

Respiratory depression, the most serious side-effect of opioid treatment, is well documented for morphine, the most commonly used opioid in neonatal care. Less is known about methadone, a clinically relevant opioid analgesic, especially during neonatal development. This study was undertaken to determine the neonatal respiratory effects of methadone. We hypothesize that methadone is equipotent to morphine, compared to our previous morphine results in the same animal model, but has a much longer duration of action, due to its longer elimination half-life. Neonatal guinea pigs (3-14 days old) randomly received a single subcutaneous dose of methadone or saline. Using a non-invasive plethysmographic method, we measured ventilatory and metabolic parameters before injection and at intervals for 32 hr after injection while pups breathed "room air" or 5% CO(2) gas mixtures. Methadone-induced depression of ventilation was most evident during 5% CO(2) challenge. The onset of drug effects was within 15 min for all ages and doses, but the duration of action decreased with age. While the depth of methadone-induced respiratory depression did not depend on pup age, the control of breathing was different in 3-day-old pups, where inspiratory time increased fourfold; twice that of older pups. We conclude that methadone induces a naloxone reversible respiratory depression in guinea pig neonates and, in the very young, causes an abnormal breathing pattern due to changes in respiratory timing. Methadone is more potent than morphine with respect to neonatal respiratory depression, but surprisingly, the duration of methadone action was not longer than morphine.

The purpose of the present study was to examine the relationship between oxygen uptake (VO2) and heart rate (HR) responses during rest and exercise in Chinese children and youth and to evaluate the relationships between maximal heart rate (%HRmax), heart rate reserve (%HRR), peak oxygen uptake (% VO2peak), and oxygen uptake reserve (% VO2R) in Chinese children and youth. Forty-nine Chinese children and youth were studied at rest and during a graded maximal exercise test on treadmill. Resting, submaximal and peak HR and VO2 were collected. Regression analyses were conducted to investigate the associations between the various forms of HR and VO2 measures. The equivalency between %HRR and % VO2R for adults was examined for children using data obtained in this study. Results indicated that all regression lines between HR measures and VO2 measures were significantly different from the line of identity (p.05), except the regression line for %HRR versus %VO2 peak in boys. The equivalency between % VO2R and % HRR for adults was not demonstrated in children and adolescents in this study. In contrast, %HRR was more closely equivalent to % VO2 peak. Because a strong linear relationship was found between HR and VO2, HR measures, in terms of either %HRmax or %HRR, would still be a practical variable for prescribing appropriate exercise intensity for children and adolescents. Unlike results found for adults, a given % HRR in children and youth was not equivalent to its corresponding % VO2R.

Two groups of adults classified with depressed mood (10% highest scores on the Profile of Mood States-Short Form Depression scale) were contrasted on changes in POMS Depression scores over 10 weeks. The exercise group ( n = 26) was given a moderate cardiovascular exercise program of 3x/week for 20 to 30 min. per session. The control group ( n = 24) did not participate in exercise. Significant reductions in POMS Depression scores over 10 weeks were noted for the exercise group in between-group ( d = –.88) and within-group ( d = −1.32) analyses. Significantly more participants in the exercise group scored in the normal range at Week 10 (62%) than in the control group (29%). Neither changes in cardiorespiratory function (VO2 max) nor initial POMS Depression scores were significantly correlated with changes in POMS Depression scores after 10 weeks. Methodological improvements, limitations, and practical application of findings after replication were discussed.

Twenty-three subjects with stable moderate chronic obstructive pulmonary disease (age 73.8±5.8 years, all male) took part in this study. During the 6MWT, ventilatory and gas exchange parameters were measured using a portable respiratory gas analysis system. Dyspnea and oxygen saturation were recorded at the end of every 2 minute period during the test. There was a significant decrease in inspiratory capacity during the 6MWT. This suggested that dynamic hyperinflation had occurred. Dyspnea showed a significant linear increase, and there was a significant negative correlation with inspiratory capacity. It was suggested that one of the reasons that dyspnea developed during the 6MWT was the dynamic hyperinflation. Even though the tidal volume increased little after 2 minutes, dyspnea increased linearly to the end of the 6MWT. These results suggest that the mechanisms generating dyspnea during the 6MWT were the sense of respiratory effort at an early stage and then the mismatch between central motor command output and respiratory system movement. Presence of an air pocket and its size play an important role in survival of victims buried in the avalanche snow. Even small air pockets facilitate breathing. We hypothesize that the size of the air pocket significantly affects the airflow resistance and work of breathing. The aims of the study are (1) to investigate the effect of the presence of an air pocket on gas exchange and work of breathing in subjects breathing into the simulated avalanche snow and (2) to test whether it is possible to breathe with no air pocket. The prospective interventional double-blinded study involved 12 male volunteers, from which 10 completed the whole protocol. Each volunteer underwent two phases of the experiment in a random order: phase "AP"--breathing into the snow with a one-liter air pocket, and phase "NP"--breathing into the snow with no air pocket. Physiological parameters, fractions of oxygen and carbon dioxide in the airways and work of breathing expressed as pressure-time product were recorded continuously. The main finding of the study is that it is possible to breath in the avalanche snow even with no air pocket (0 L volume), but breathing under this condition is associated with significantly increased work of breathing. The significant differences were initially observed for end-tidal values of the respiratory gases (EtO2 and EtCO2) and peripheral oxygen saturation (SpO2) between AP and NP phases, whereas significant differences in inspiratory fractions occurred much later (for FIO2) or never (for FICO2). The limiting factor in no air pocket conditions is excessive increase in work of breathing that induces increase in metabolism accompanied by higher oxygen consumption and carbon dioxide production. The presence of even a small air pocket reduces significantly the work of breathing.

In order to characterize the variability and correlation properties of spontaneous breathing in humans, the breathing pattern of 16 seated healthy subjects was studied during 40 min of quiet breathing using opto-electronic plethysmography, a contactless technology that measures total and compartmental chest wall volumes without interfering with the subjects breathing. From these signals, tidal volume (VT), respiratory time (TTOT) and the other breathing pattern parameters were computed breath-by-breath together with the end-expiratory total and compartmental (pulmonary rib cage and abdomen) chest wall volume changes. The correlation properties of these variables were quantified by detrended fluctuation analysis, computing the scaling exponentα. VT, TTOT and the other breathing pattern variables showed α values between 0.60 (for minute ventilation) to 0.71 (for respiratory rate), all significantly lower than the ones obtained for end-expiratory volumes, that ranged between 1.05 (for rib cage) and 1.13 (for abdomen) with no significant differences between compartments. The much stronger long-range correlations of the end expiratory volumes were interpreted by a neuromechanical network model consisting of five neuron groups in the brain respiratory center coupled with the mechanical properties of the respiratory system modeled as a simple Kelvin body. The model-based α for VT is 0.57, similar to the experimental data. While the α for TTOT was slightly lower than the experimental values, the model correctly predicted α for end-expiratory lung volumes (1.045). In conclusion, we propose that the correlations in the timing and amplitude of the physiological variables originate from the brain with the exception of end-expiratory lung volume, which shows the strongest correlations largely due to the contribution of the viscoelastic properties of the tissues. This cycle-by-cycle variability may have a significant impact on the functioning of adherent cells in the respiratory system.

The peak-to-trough amplitude (sum of absolute values of maximum and minimum) of an isointegral QRST body surface map was shown, according to literary data, to be sensitive to changes in ventricular sympathetic tone. Statistics of this parameter were obtained, with an 80 electrodes array, in 135 healthy subjects of both sexes, aged from 10 to 67 years (37 +/- 16). The QRST amplitudes were smaller in females than in males in average by 28% (P < .0001). They decreased with increasing age (r = -0.237, P < .01). Larger QRST amplitudes went with longer RR intervals (r = .449, P < .0001). In held deep inspiration, the amplitude decreased in 79% and increased in 21% of subjects. The change of the amplitude was related to the inspiratory change of the RR interval (r = .343, P < .0001), but not to the RR interval preceding the deep inspiration. Body mass index and somatometric characteristics of chest configuration did not significantly affect the QRST amplitude. The results may serve as a basis for further studies on the usefulness of the QRST amplitude as another variable reflecting changes of ventricular recovery properties.

The purpose of this study was to determine the effect of imposed external resistance on the ventilatory response to He-O(2) breathing during peak exercise. To accomplish this purpose, separate inspiratory and expiratory external resistances were applied to offset for the decrease in intrapulmonary airway resistance with He-O(2) breathing. Seven men and three women (69+/-3 years, mean+/-S.D.) with normal pulmonary function performed graded cycle ergometry to exhaustion breathing room air, He-O(2) (79% He, 21% O(2)), He-O(2) with imposed expiratory resistance, and He-O(2) with imposed inspiratory resistance. Ventilation (VE), lung mechanics, and PET(CO(2)) were measured during each 1 min increment in work rate and were analyzed by one-way ANOVA for repeated measures at rest, ventilatory threshold (VTh), and peak exercise. Peripheral chemoreceptor function has been tested using either the hyperoxic test (HT), which decreases minute ventilation (V E) by causing physiologic chemodenervation, or the alternate breath test (ABT), which induces V E alternations by delivering rapid hypoxic stimuli through breath-by-breath alternations in fractional inspired O(2) between normoxia (0.21) and hypoxia (0.15). No previous studies have compared ventilatory responses to both tests in the same infants. We hypothesized that the V E decrease during HT would be significantly related to V E alternations during ABT. Eighteen infants (postnatal age 21 +/- 14 d) underwent two 30-s HTs and two ABTs (quiet sleep, face mask, and pneumotachograph; mass spectrometry measurement of inspired and expired O(2) and CO(2) fractions; and breath-by-breath analysis). The tests were done in random order. Decreases in V E and mean inspiratory flow (tidal volume over inspiratory time, VT/TI) during HTs were significantly correlated to their respective percentage coefficients of alternation during ABTs (r = 0.69 and 0.70, respectively, p0.01). Principal components analysis showed that the V E and VT/TI decreases during HTs were due chiefly to a fall in VT, whereas V E and VT/TI alternations were ascribable to alternations in both VT and TI. Intraindividual coefficients of variation of V E changes were significantly lower during HTs than during ABTs. We conclude that (1) ventilatory responses to HT and ABT are significantly correlated despite differences in the mechanisms of the V E changes; (2) the better reproducibility of the V E response to HT as compared with ABT may be an advantage in clinical practice.

Objective: We evaluated the usefulness of analyzing expired gas during exercise testing for the prediction of postoperative cardiopulmonary complications in patients with esophageal carcinoma. Background data: Radical esophagectomy with 3-field lymphadenectomy is performed in patients with thoracic esophageal carcinoma but has a high risk of postoperative complications. To reduce the surgical risk, we performed preoperative risk analysis using 8 factors. Although hospital mortality was decreased when this risk analysis was used, severe cardiopulmonary complications still occurred. Methods: The study group consisted of 91 patients who had undergone curative esophagectomy with 3-field lymphadenectomy. The maximum oxygen uptake, anaerobic threshold, vital capacity, percent vital capacity, forced expiratory volume in 1 second, percent forced expiratory volume, V 25 /HT, forced expired flow at 75% of forced vital capacity to height ratio (FEF 75% /HT), forced expired flow at 50% to 75% of forced vital capacity ratio (FEF 50% /FEF 75% ), percent diffusion capacity for carbon monoxide, and arterial oxygen tension were measured. Patients were divided into 2 groups on the basis of the presence or absence of postoperative cardiopulmonary complications. Results: Only the maximum oxygen uptake was significantly different between the 2 groups. All patients were grouped according to the value of the maximum oxygen uptake, and the occurrence of postoperative cardiopulmonary complications was calculated for each group. A cardiopulmonary complication rate of 86% was found for patients with a maximum oxygen uptake of less than 699 mL · min –1 · m –2 ; for those with a value of 700 to 799 mL · min –1 · m –2 , the complication rate was 44%. Conclusions: The maximum oxygen uptake obtained by expired gas analysis during exercise testing correlates with the postoperative cardiopulmonary complication rate. On the basis of these results, esophagectomy with 3-field lymphadenectomy can be safely performed in patients with a maximum oxygen uptake of at least 800 mL · min –1 · m –2 . Variability in inspired lung volume prior to speech is only partially accounted for by speech-related concerns such as the length and loudness of the planned utterance. Control mechanisms known to influence volume variability in non-speech breathing could potentially account for some of this variability, but only if they operate during speech as well. This investigation was designed to test for the presence of several such mechanisms during reading aloud. Lung volumes were recorded from 5 normal females as they read silently, then aloud. Inspired volumes were correlated with the volumes of the previous and following expirations and with inspiratory duration. Coefficients of variation were calculated for inspiratory volume, duration, and mean flow. Time-series analyses were used to compare periodicity in inspired volume for quiet and speech breathing. Control mechanisms operating during both quiet breathing and reading aloud included slow oscillations in inspired volume and minimized variability in mean flow. Inspired volume prior to speech was weakly but significantly correlated with preceding and following expired volume. It is concluded that some control strategies typical of quiet breathing contribute to volume variability in speech breathing.

The complete calibration of indirect calorimetry systems involves simultaneous checks of gas analyzers, volume device, and software, and this requires a machine that can mimic accurately and precisely the ventilation and expired gases of an athlete. While previous calibrators have been built successfully, none have matched the ventilatory flows produced by athletes during high intensity exercise. A calibrator able to simulate high aerobic power (VO2max calibrator) was fabricated and tested against conventional indirect calorimetry systems that use chain-compensated gasometers to measure expired volume (VE systems) and calibrated electronic gas analyzers. The calibrator was also checked against a system that measures inspired volume (VI system) with a turbine ventilometer. The pooled data from both VE and VI systems for predicted VO2 ranging from 2.9 to 7.9 L.min-1 and ventilation ranging from 89 to 246 L.min-1 how that the absolute accuracy (bias) of values measured by conventional indirect calorimetry systems compared with those predicted by the calibrator was excellent. The bias was < 35 mL.min-1 for VO2 and carbon dioxide production, < 0.50 L.min-1 for ventilator (VE BTPS), -0.02% absolute for the percentage of expired O2 and +0.02% absolute for the percentage of expired CO2. Overall, the precision of the measured VO2, VCO2, and VE BTPS was approximately 1%. This VO2max calibrator is a versatile device that can be used for routine calibration of most indirect calorimetry systems that assess the ventilation and aerobic power of athletes.

Objective: To determine the effect of frequency and percent inspiratory time on tidal volume and gas-trapping during high-frequency oscillatory ventilation (HFOV). Subjects: Nine preterm infants with respiratory distress syndrome tested in the first 48 h of life. Methods: Tidal volumes and the presence of gas-trapping were measured by respiratory jacket plethysmography at frequencies of 10, 14, and 17.8 Hz and at inspiratory times of 30%, 50% and 70%, using a commercially available high frequency oscillator. Results: Mean (SD) tidal volumes were 2.40 (1.06) ml/kg at 10 Hz, 2.52 (1.07) ml/kg at 14 Hz and fell significantly to 1.96 (0.92) at 17.8 Hz (p < 0.05). Tidal volumes at 50% inspiratory time were significantly greater than at 30% inspiratory time [2.81 (1.42) ml/kg and 2.32 (1.18) ml/kg, respectively] but fell to baseline levels at 70% inspiratory time. There was no significant gas-trapping with increases in either frequency or percent inspiratory time. Conclusions: Gas-trapping is not a significant problem during HFOV in premature infants. Changes in tidal volume with increases in frequency and percent inspiratory time are similar to that seen in animal models.

Coughing is an important protective mechanism for keeping the airway clear, and adequate voluntary coughing reduces the risk of aspiration in patients with deglutition disorders. The purpose of this study was to compare the peak cough flow (PCF) of stroke patients with and without dysphagia and to identify the physical and respiratory determinants of PCF.Using a spirometer, we measured and compared the PCFs of 10 stroke patients with dysphagia (SPD), 20 stroke patients without dysphagia (SP) and 10 gender and age matched healthy controls (HC) recruited by using a notice at a clinic and in newspapers. The PCF of the SPD (mean ± SD, 160.1 ± 68.7 l/min) was significantly lower than that of the SP and HC (297.2 ± 114.2 l/min and 462.0 ± 84.4 l/min, respectively; one-way ANOVA, Scheffe's test, P < 0.05). The vital capacity (VC) and inspiratory reserve volume (IRV) of the SPD were lower than those of the HC. Stepwise multivariate regression analysis revealed that IRV and ambulation function (Functional Ambulation Categories, FAC) contributed 50% and 17% to the variance of PCF (P < 0.05), respectively. It is suggested that respiratory function, especially IRV, is important for maintaining PCF in SPD.

There is considerable intersubject variability in the perceived intensity of breathlessness for a given level of activity among patients with chronic airflow limitation (CAL). To examine possible factors contributing to this variability we compared breathing pattern parameters, dynamic operational lung volumes, and Borg dyspnea ratings in 23 patients with severe CAL and in 10 healthy age-matched normal subjects during cycle ergometry to symptom-limitation. Patients with CAL had significantly (p < 0.01) higher levels of ventilation (% maximal voluntary ventilation) for a given work rate (slope of VE(%MVV)/WR(% pred max) = 1.51 +/- 0.18 versus 0.63 +/- 0.10; mean +/- SEM) and greater dynamic lung hyperinflation (DH) (change [delta] in end-expiratory lung volume [EELVdyn] = +0.31 +/- 0.11 L versus -0.16 +/- 0.22 L). Compared with normal subjects at a standardized VE (30 L/min), the CAL group was more breathless Borg = 4 +/- 1 versus 2 +/- 1, p < 0.01) and hyperinflated (EELVdyn = 75 +/- 3 versus 46 +/- 6% TLC, p < 0.001; end-inspiratory lung volume [EILVdyn] = 85 +/- 3 versus 67 +/- 5% TLC, p < 0.01). Within the CAL group, change in Borg ratings correlated with delta VE(%MVV) (r = 0.77, p < 0.001) and with slope of VE(%MVV)/WR(% pred max) (r = 0.48, p < 0.01). Regression analysis selected delta EILVdyn (or delta inspiratory reserve volume [delta IRVdyn]) from various dynamic ventilatory parameters as the strongest predictor of delta Borg (r = 0.63, p < 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

BACKGROUND AND STUDY AIMS: Physiological reactions during natural orifice transluminal endoscopic surgery (NOTES) mediastinoscopy may lead to cardiorespiratory depression. The aim of the current study was to assess cardiopulmonary changes during transesophageal mediastinoscopy in an acute porcine model. METHODS: Transesophageal mediastinoscopy was performed under general anesthesia in eight female pigs with a bodyweight of 39 ± 6 kg. Mediastinal access was achieved via a submucosal tunnel. The cardiac index and global end-diastolic volume index (reflecting preload) were measured every 3 minutes by transpulmonary thermodilution. The following parameters were also recorded: mediastinal pressure, heart rate, mean arterial pressure, systemic vascular resistance index (SVRI; reflecting afterload), peak inspiratory pressure, pH, pCO 2 , and pO 2 . RESULTS: In three animals, small tears in the parietal pleura resulted in tension pneumothoraces. The associated cardioplumonary deterioration was fatal in one pig. The other two pigs recovered after decompression with a chest tube. In the remaining five animals there were only mild hemodynamic and respiratory changes during mediastinoscopy. There was a significant ( P = 0.005) but minor transient fall in cardiac index, which correlated with a small rise in SVRI (r = - 0.857, P < 0.001). In the pigs with uncomplicated mediastinoscopy, on-demand insufflation via the endoscope resulted in median mediastinal pressures of 4.5 mm Hg (range 2.3 - 10.2 mm Hg). Overall, mediastinal and thoracic structures could be identified without difficulty via the transesophageal approach. CONCLUSIONS: NOTES mediastinoscopy carries a substantial risk of inadvertent development of a pneumothorax. Otherwise, it leads to negligible hemodynamic and pulmonary changes. In conclusion, close monitoring for the presence of a pneumothorax during NOTES mediastinoscopy appears to be mandatory.

Mechanical ventilation causes significant morbidity and mortality in patients with severe asthma. Hypoventilation may reduce this morbidity and mortality, but indicators to guide the degree of hypoventilation are unclear. We used a measure of pulmonary hyperinflation to assess the degree of airflow obstruction and to guide the extent and duration of hypoventilation. Ten patients who required mechanical ventilation for acute severe asthma were studied. All were sedated, paralyzed, and given an initial minute ventilation (VE) of 200 ml/kg/min. End-inspiratory lung volume (VEI) above FRC was measured from the total exhaled gas volume during 40 to 60 s of apnea. VEI was used to regulate VE to a safe level (VEsafe), irrespective of PaCO2, by reducing the rate when VEI was20 ml/kg and increasing it when VEI was20 ml/kg. Each patient was weaned when VEsafe resulted in PaCO2or = 40 mm Hg (the weaning point). FRC was measured computer analysis of anterior and lateral chest radiographs taken at the end of apnea. Using the weaning point criterion, 2 patients (PaCO240 mm Hg) were weaned shortly after arrival. The remaining eight (initial PaCO2, 63 +/- 17 mm Hg) continued hypoventilation until the weaning point was reached (30 +/- 29 h). The weaning point was reached by the VE required for PaCO2 40 mm Hg decreasing concurrent with the VEsafe increasing. All but 1 patient were successfully weaned within 24 h of the weaning point.(ABSTRACT TRUNCATED AT 250 WORDS)

One of the contributors to exercise limitation in COPD is dynamic hyperinflation. Although dynamic hyperinflation appears to occur during several exercise protocols in COPD and seems to increase with increasing disease severity, it is unknown whether dynamic hyperinflation occurs at different severity stages according to the Global initiative for chronic Obstructive Lung Disease (GOLD) in daily life. The present study, therefore, aimed to compare dynamic hyperinflation between COPD GOLD stages II-IV during daily activities.Thirty-two clinically stable patients with COPD GOLD II (n = 10), III (n = 12), and IV (n = 10) participated in this study. Respiratory physiology during a daily activity was measured at patients' homes with Oxycon Mobile. Inspiratory capacity maneuvers were performed at rest, at 2-min intervals during the activity, and at the end of the activity. Change in inspiratory capacity is commonly used to reflect change in end-expiratory lung volume (DeltaEELV) and, therefore, dynamic hyperinflation. The combination of static and dynamic hyperinflation was reflected by inspiratory reserve volume (IRV) during the activity.Overall, increase in EELV occurred in GOLD II-IV without significant difference between the groups. There was a tendency for a smaller DeltaEELV in GOLD IV. DeltaEELV was inversely related to static hyperinflation. IRV during the daily activity was related to the level of airflow obstruction.Dynamic hyperinflation occurs independent of GOLD stage during real-life daily activities. The combination of static and dynamic hyperinflation, however, increases with increasing airflow obstruction.

Smoking and obesity are leading global causes of death. The aim of the present study was to develop a test to detect early lung abnormalities caused by smoking and/or obesity and to elucidate the underlying mechanisms and major contributory factors.One hundred twenty-nine healthy adults, 20 to 40 years of age, with normal spirometry findings, were divided into the following five groups: 38 healthy subjects (15 female subjects); 46 smokers (5 female smokers); 18 overweight nonsmokers (2 female nonsmokers; normal body mass index [BMI],or= 25 kg/m(2); range, 25.0 to 37.2 kg/m(2)); 19 overweight smokers (1 female smoker; BMI range, 25.2 to 33.5 kg/m(2)); and 8 ex-smokers (BMI range, 20.8 to 24.1 kg/m(2)). A modified pulse oximeter was employed for measuring the fall in pulse oximetric saturation caused by 20-s breath-holding (dSpo(2)) at resting end expiration in the sitting posture.In healthy subjects, dSpo(2) had no significant correlation with age (r(2) = 0.009; p = 0.66). In smokers, dSpo(2) correlated with both the number of pack-years (r(2) = 0.590; p0.001) and closing volume (CV)/vital capacity (VC) ratio (r(2) = 0.573; p0.001). In overweight nonsmokers, dSpo(2) increased significantly with BMI alone (r(2) = 0.667; p0.001). In overweight smokers, the largest increase in the mean dSpo(2) was observed. Multiple linear regression analysis suggested that BMI and CV/VC are the two major contributing factors determining dSpo(2) during breath-holding. In young former smokers, no significant increase in the mean dSpo(2) was observed (p = 0.77) a mean (+/- SD) duration of 5.2 +/- 2.9 years after the cessation of smoking. No significant difference in dSpo(2) was observed between men and women.A new test that measures oxygen saturation during breath-holding reveals early lung abnormalities in subjects who either smoke or are overweight, especially if these factors are combined. Peripheral airway abnormalities and/or lung volume reduction may play roles in the greater desaturation.

Background Flexible bronchoscopy performed through endotracheal tubes (ETTs) in children receiving mechanical ventilation can significantly impact ventilation, but the magnitude of this impact has not been established. We used a lung model to simulate mechanical ventilation in a range of child sizes in order to determine how the insertion of pediatric flexible bronchoscopes into ETTs alters ventilatory parameters, especially tidal volume (V t ) and peak inspiratory pressure (PIP), in both healthy and diseased lungs. Methods We simulated five child sizes based on weight, and evaluated 22 bronchoscope/ETT combinations, first in pressure control (PC) ventilation mode and then in volume control (VC) ventilation mode. The combinations ranged from the 2.2-mm (bronchoscope outer diameter)/3.0-mm (ETT inner diameter) to 5.0-mm bronchoscope/8.0-mm ETT. The primary outcome measures were decrease in V t after bronchoscope insertion during PC ventilation and increase in PIP during VC ventilation. Results In the PC ventilator mode, V t decreased by > 50% with nine of the combinations, while during VC ventilation, PIP increased by ≥ 20 cm H 2 O with seven combinations. The 2.2-mm bronchoscope/3.0-mm ETT, 2.8-mm bronchoscope/5.0-mm ETT, and 3.6-mm bronchoscope/5.0-mm ETT combinations severely impaired ventilation, while the 3.6-mm bronchoscope/4.5-mm ETT, 5.0-mm bronchoscope/6.5-mm ETT, and 5.0-mm bronchoscope/7.0-mm ETT combinations were incompatible with adequate ventilation. Conclusions The insertion of bronchoscopes into ETTs can lead to clinically relevant decreases in V t when in the PC ventilator mode and large increases in PIP during VC ventilation. The minimum bronchoscope/ETT diameter difference required to maintain adequate ventilation increases with child size.

There is insufficient information about reference values for pulmonary volumes for Iranian populations. A study of lung function parameters was made on 302 non-smoking healthy Iranian students (152 male and 150 female). Lung function measures correlated strongly with height but not with body mass index. There were significant differences between some of the measured parameters and American Thoracic Society reference values for Caucasians (P0.05). Of note is the high functional residual capacity (110% higher) and low inspirational capacity (86% lower) in males compared with the reference values.

Pharyngeal size and the dynamic behavior of the pharynx may be important factors in the modulation of pharyngeal airflow. There are two measures of pharyngeal function: changes in pharyngeal area with lung volume and changes in pharyngeal area in response to externally applied positive pressure. Both measurements have been used for the assessment of pharyngeal function, and both reflect pharyngeal "floppiness." The aim of the present study was to examine the relationship between pharyngeal cross-sectional area, using acoustic reflection technique, and different lung volumes (tidal volume, inspiratory reserve volume, and expiratory reserve volume), to determine whether there are differences in mechanical properties of the pharynx of normal volunteers in response to changes in intrapharyngeal pressure. The acoustic technique was used to assess the pharyngeal cross-sectional area of 40 healthy volunteers (29 men and 11 women) at tidal volume, inspiratory reserve volume, and expiratory reserve volume. In men, the mean pharyngeal area at tidal volume was 3.191 cm2, the mean pharyngeal area at inspiratory reserve volume was 2.976 cm2, and the mean pharyngeal area at expiratory reserve volume was 2.975 cm2. In women, the corresponding pharyngeal area measurements were 2.832, 2.484, and 2.492 cm2. Statistical analysis of results showed that men have a larger pharyngeal cross-sectional area and the pharyngeal airways of men and women act in a similar manner in response to changes in intrapharyngeal pressure, with men having a greater change. Examination of the pharyngeal compliance by acoustic pharyngometry adds to the potential of this technique as a tool for the evaluation of the pharyngeal airway in terms of area and dynamic behavior assessment. This may be of relevance in promotion of the development of upper airway assessment in patients with obstructive sleep apnea.
