Introduction: The inclusion of suicide gestures in modern nomenclatures for self‐injurious thoughts and behaviors (SITB) is contentious due to their history of pejorative connotations and inconsistent operationalization and measurement. Here we sought to investigate the extent to which participants who endorse this behavior on a standardized SITB measure: (1) describe their behavior in a way that is consistent with contemporary definitions for suicide gestures; (2) accurately classify their behavior when presented with multiple SITB response options; and (3) consistently report their level of intent to die across survey items. Methods: Participants were 83 adults from a community‐based sample who endorsed lifetime suicide gesture(s) in an online survey containing self‐report measures assessing their prior SITB engagement, followed by open‐ended questions eliciting narrative descriptions of their behaviors. Results: Approximately 13% of participants who endorsed lifetime suicide gestures provided narrative descriptions that met criteria for the behavior, and around one‐third consistently reported zero intent to die in their explicit ratings. Additionally, some participants reported non‐zero intent to die from behaviors without direct potential for physical injury. Conclusions: Overall, this study highlights substantial issues with the validity of current approaches to measuring suicide gestures. This study aims to identify the characteristics and patterns among patients with SH and factors associated with the intent to die. Method This retrospective descriptive study where SH cases presented to private tertiary care teaching hospital in Karachi, Pakistan, from January 2013 to December 2017 were extracted from HIMS records. Notably, 81% of the cases do not have a history of SH. Drug overdose (61.6%) and insecticides (36.6%) were the two most common methods used. Depression was identified in nearly half of the cases. The most common reason for attempting SH was inter-personal relationship issues (54.3%). Conclusion: This paper provides recent data on the characteristics and patterns associated with the intent to die of individuals who have self-harmed. In most cases of SH, past psychiatric history was not evident. Current psychiatric diagnosis and young adults were favoured in this study. The data from this study has limited representation for all demographic representation of SH cases from Pakistan as being from a single private hospital. Definitions and classification schemes for suicide attempts vary widely among studies, introducing conceptual, methodological, and clinical problems. We tested the importance of the intent to die criterion by comparing self-injurers with intent to die, suicide attempters, and those who self-injured not to die but to communicate with others, suicide gesturers, using data from the National Comorbidity Survey (n = 5,877). Suicide attempters (prevalence = 2.7%) differed from suicide gesturers (prevalence = 1.9%) and were characterized by male gender, fewer years of education, residence in the southern and western United States; psychiatric diagnoses including depressive, impulsive, and aggressive symptoms; comorbidity; and history of multiple physical and sexual assaults. Background: Intent to die is an important component of suicide risk assessment. The authors compared the predictive effect of two forms of stress - military and perceived - in intent to die by suicide among young adult Israeli soldiers with a history of suicide attempts. Depression, suicide ideation, and habituation/acquired capacity for suicidality served as covariates., Methods: Participants were 60 young adult soldiers in the Israeli Defense Force (ISF; aged 18-21 years), who made a suicide attempt during their military service. Study variables were assessed using self-report measures., Results: Intent to die by suicide correlated with suicide ideation, habituation/acquired capacity, depressive symptoms, and perceived stress. In a multiple regression analysis, perceived stress predicted intent to die (b = .44, p = .002) over and above the prediction by suicide ideation (b = .42, p = .013) and acquired capacity/habituation (b = .28, p = .023)., Limitations: The cross-sectional design restricts causal inference. In addition, an exclusive reliance on self-report measures might have inflated shared method variance., Conclusions: Perceived stress captures a unique dimension of intent to die by suicide among young suicide attempters.
