Abstract

Predictive tools provide a unique opportunity to explain the observed differences in outcome between patients of the COVID-19 pandemic. The aim of this study was to associate individual demographic and clinical characteristics with disease severity in COVID-19 patients and to highlight the importance of machine learning (ML) in disease prognosis. The study enrolled 344 unvaccinated patients with confirmed SARS-CoV-2 infection. Data collected by integrating questionnaires and medical records were imported into various classification machine learning algorithms, and the algorithm and the hyperparameters with the greatest predictive ability were selected for use in a disease outcome prediction web tool. Of 111 independent features, age, sex, hypertension, obesity, and cancer comorbidity were found to be associated with severe COVID-19. Our prognostic tool can contribute to a successful therapeutic approach via personalized treatment. Although at the present time vaccination is not considered mandatory, this algorithm could encourage vulnerable groups to be vaccinated.

Abstract

Abstract Background The incidence of the highly morbid and potentially lethal gangrenous cholecystitis was reportedly increased during the COVID-19 pandemic. The aim of the ChoCO-W study was to compare the clinical findings and outcomes of acute cholecystitis in patients who had COVID-19 disease with those who did not. Methods Data were prospectively collected over 6 months (October 1, 2020, to April 30, 2021) with 1-month follow-up. In October 2020, Delta variant of SARS CoV-2 was isolated for the first time. Demographic and clinical data were analyzed and reported according to the STROBE guidelines. Baseline characteristics and clinical outcomes of patients who had COVID-19 were compared with those who did not. Results A total of 2893 patients, from 42 countries, 218 centers, involved, with a median age of 61.3 (SD: 17.39) years were prospectively enrolled in this study; 1481 (51%) patients were males. 3. Nickel—Alumina Catalysts for the Transformation of Vegetable Oils into Green Diesel: The Role of Preparation Method, Activation Temperature, and Reaction Conditions

Abstract

Two nickel alumina catalysts containing 60 wt. % Ni were synthesized by wet impregnation and co-precipitation in order to study the effect of preparation methods on the catalytic efficiency concerning the transformation of sunflower oil into green diesel. The effect of activation temperature on the catalytic efficiency of the most active catalyst was also studied. The catalysts were characterized using various techniques and which were evaluated in the aforementioned reaction using a semi-batch reactor. The catalyst prepared by co-precipitation exhibited a higher specific surface area and smaller mean crystal size of the nickel nanoparticle (higher nickel metallic surface). These justify its higher efficiency with respect to the corresponding catalyst synthesized by wet impregnation. The increase in the activation temperature from 400 to 600 °C increased the size of the nickel nanoparticles through sintering, thus destroying the small pores. These led to a decrease in the nickel surface and specific surface area and, thus, to a decrease in the catalytic efficiency. The optimization of the reaction conditions over the most active catalyst (prepared by co-precipitation and activated at 400 °C) leads to the complete transformation not only of the sunflower oil (edible oil) but also of waste cooking oil (non-edible oil) into green diesel. The liquid produced after the hydrotreatment for these two feedstocks for 7 h, at H2 pressure 40 bar and temperature 350 °C using 100 mL of oil and 1 g of catalyst was composed of 97 and 96 wt. % of green diesel, respectively.

Abstract

Abstract Background Timing and adequacy of peritoneal source control are the most important pillars in the management of patients with acute peritonitis. Therefore, early prognostic evaluation of acute peritonitis is paramount to assess the severity and establish a prompt and appropriate treatment. The objectives of this study were to identify clinical and laboratory predictors for in-hospital mortality in patients with acute peritonitis and to develop a warning score system, based on easily recognizable and assessable variables, globally accepted. Methods This worldwide multicentre observational study included 153 surgical departments across 56 countries over a 4-month study period between February 1, 2018, and May 31, 2018. Results A total of 3137 patients were included, with 1815 (57.9%) men and 1322 (42.1%) women, with a median age of 47 years (interquartile range [IQR] 28–66). The overall in-hospital mortality rate was 8.9%, with a median length of stay of 6 days (IQR 4–10). Using multivariable logistic regression, independent variables associated with in-hospital mortality were identified: age > 80 years, malignancy, severe cardiovascular disease, severe chronic kidney disease, respiratory rate ≥ 22 breaths/min, systolic blood pressure < 100 mmHg, AVPU responsiveness scale (voice and unresponsive), blood oxygen saturation level (SpO2) < 90% in air, platelet count < 50,000 cells/mm3, and lactate > 4 mmol/l. These variables were used to create the PIPAS Severity Score, a bedside early warning score for patients with acute peritonitis. The overall mortality was 2.9% for patients who had scores of 0–1, 22.7% for those who had scores of 2–3, 46.8% for those who had scores of 4–5, and 86.7% for those who have scores of 7–8. Conclusions The simple PIPAS Severity Score can be used on a global level and can help clinicians to identify patients at high risk for treatment failure and mortality.

Abstract

Abstract Background Acute appendicitis (AA) is the most common surgical disease, and appendectomy is the treatment of choice in the majority of cases. A correct diagnosis is key for decreasing the negative appendectomy rate. The management can become difficult in case of complicated appendicitis. The aim of this study is to describe the worldwide clinical and diagnostic work-up and management of AA in surgical departments. Methods This prospective multicenter observational study was performed in 116 worldwide surgical departments from 44 countries over a 6-month period (April 1, 2016–September 30, 2016). All consecutive patients admitted to surgical departments with a clinical diagnosis of AA were included in the study. Results A total of 4282 patients were enrolled in the POSAW study, 1928 (45%) women and 2354 (55%) men, with a median age of 29 years. Nine hundred and seven (21.2%) patients underwent an abdominal CT scan, 1856 (43.3%) patients an US, and 285 (6.7%) patients both CT scan and US. A total of 4097 (95.7%) patients underwent surgery; 1809 (42.2%) underwent open appendectomy and 2215 (51.7%) had laparoscopic appendectomy. One hundred eighty-five (4.3%) patients were managed conservatively. Major complications occurred in 199 patients (4.6%). The overall mortality rate was 0.28%. Conclusions The results of the present study confirm the clinical value of imaging techniques and prognostic scores. Appendectomy remains the most effective treatment of acute appendicitis. Mortality rate is low.

7. Pancreatic Pseudocyst Pleural Fistula in Gallstone Pancreatitis

Abstract

Extra-abdominal complications of pancreatitis such as pancreaticopleural fistulae are rare. A pancreaticopleural fistula occurs when inflammation of the pancreas and pancreatic ductal disruption lead to leakage of secretions through a fistulous tract into the thorax. The underlying aetiology in the majority of cases is alcohol-induced chronic pancreatitis. The diagnosis is often delayed given that the majority of patients present with pulmonary symptoms and frequently have large, persistent pleural effusions. The diagnosis is confirmed through imaging and the detection of significantly elevated amylase levels in the pleural exudate. Treatment options include somatostatin analogues, thoracocentesis, endoscopic retrograde cholangiopancreatography (ERCP) with pancreatic duct stenting, and surgery. The authors present a case of pancreatic pseudocyst pleural fistula in a woman with gallstone pancreatitis presenting with recurrent pneumonias and bilateral pleural effusions.

9. Intestinal Obstruction due to Bilateral Strangulated Femoral Hernias

Abstract

Introduction. Femoral hernias are at high risk of strangulation due to the narrow femoral canal and femoral ring. This can lead to symptoms of obstruction or strangulation requiring emergency surgery and possible bowel resection. To our knowledge, there is only one previous published report of bilateral strangulated femoral hernia. We present our case of this phenomenon. Case Report. An 86-year-old woman presented with symptoms of small bowel obstruction. Examination revealed two tender lumps in the area of the femoral triangle. CT scan revealed bilateral femoral hernias. Both hernias were repaired and a small bowel resection on the right side was performed with side to side anastomosis. She made an uneventful recovery. Conclusion. Bilateral femoral hernias are a rare occurrence with only one reported case of bilateral strangulation. Our case highlights the importance of meticulous history taking and clinical examination as any delay in diagnosis will increase the risk of mortality and morbidity for the patient. Hernias should always be considered as a cause if one presents with symptoms of abdominal pain or obstruction.

Abstract

This study aims to utilize biochars derived from residual biomass as supports for Ni-based catalysts. For the preparation of the biochars, byproducts of agro-industrial activities were used, such as espresso coffee residue (C) and rice husks (R). Sufficient quantities of the respective biochars (BioC and BioR) were prepared via pyrolysis at 850 °C of the aforementioned materials under limited oxygen conditions. The biochars were further treated with hot water (WBioC, WBioR), H3PO4 or H2SO4 solution (BioC-P, BioC-S, BioR-P, BioR-S), and NaOH solution (BioC-A and BioR-A), and the obtained solids were characterized using various physicochemical techniques. The biochars produced were microporous with high surface areas (367–938 m2g−1). The most promising biochars were selected as supports for the preparation of nickel catalysts (10 wt.% Ni) with high Ni dispersion (mean crystal size: 8.2–9.8 nm) and suitable acidity. The catalysts were evaluated in a high-pressure semi-batch reactor for the transformation of waste cooking oil (WCO) into green diesel. The 10Ni_BioC-P catalyst exhibited the best performance, resulting in a complete conversion of the WCO but a low hydrocarbon yield (7.5%). Yield improvement was achieved by promoting this catalyst with molybdenum. The addition of Mo increased the hydrocarbon yield by almost three times (19.5%).

Abstract

Background The SARS-CoV-2 pandemic is still ongoing and a major challenge for health care services worldwide. In the first WSES COVID-19 emergency surgery survey, a strong negative impact on emergency surgery (ES) had been described already early in the pandemic situation. However, the knowledge is limited about current effects of the pandemic on patient flow through emergency rooms, daily routine and decision making in ES as well as their changes over time during the last two pandemic years. This second WSES COVID-19 emergency surgery survey investigates the impact of the SARS-CoV-2 pandemic on ES during the course of the pandemic. Methods A web survey had been distributed to medical specialists in ES during a four-week period from January 2022, investigating the impact of the pandemic on patients and septic diseases both requiring ES, structural problems due to the pandemic and time-to-intervention in ES routine. Results 367 collaborators from 59 countries responded to the survey. The majority indicated that the pandemic still significantly impacts on treatment and outcome of surgical emergency patients (83.1% and 78.5%, respectively). As reasons, the collaborators reported decreased case load in ES (44.7%), but patients presenting with more prolonged and severe diseases, especially concerning perforated appendicitis (62.1%) and diverticulitis (57.5%). Otherwise, approximately 50% of the participants still observe a delay in time-to-intervention in ES compared with the situation before the pandemic. Relevant causes leading to enlarged time-to-intervention in ES during the pandemic are persistent problems with in-hospital logistics, lacks in medical staff as well as operating room and intensive care capacities during the pandemic. This leads not only to the need for triage or transferring of ES patients to other hospitals, reported by 64.0% and 48.8% of the collaborators, respectively, but also to paradigm shifts in treatment modalities to non-operative approaches reported by 67.3% of the participants, especially in uncomplicated appendicitis, cholecystitis and multiple-recurrent diverticulitis. Conclusions The SARS-CoV-2 pandemic still significantly impacts on care and outcome of patients in ES. Well-known problems with in-hospital logistics are not sufficiently resolved by now; however, medical staff shortages and reduced capacities have been dramatically aggravated over last two pandemic years.

Abstract

Background Artificial intelligence (AI) is gaining traction in medicine and surgery. AI-based applications can offer tools to examine high-volume data to inform predictive analytics that supports complex decision-making processes. Time-sensitive trauma and emergency contexts are often challenging. The study aims to investigate trauma and emergency surgeons’ knowledge and perception of using AI-based tools in clinical decision-making processes. Methods An online survey grounded on literature regarding AI-enabled surgical decision-making aids was created by a multidisciplinary committee and endorsed by the World Society of Emergency Surgery (WSES). The survey was advertised to 917 WSES members through the society’s website and Twitter profile. Results 650 surgeons from 71 countries in five continents participated in the survey. Results depict the presence of technology enthusiasts and skeptics and surgeons' preference toward more classical decision-making aids like clinical guidelines, traditional training, and the support of their multidisciplinary colleagues. A lack of knowledge about several AI-related aspects emerges and is associated with mistrust. Discussion The trauma and emergency surgical community is divided into those who firmly believe in the potential of AI and those who do not understand or trust AI-enabled surgical decision-making aids. Academic societies and surgical training programs should promote a foundational, working knowledge of clinical AI.

Abstract

Data regarding the association between preexisting cardiovascular risk factors (CVRFs) and cardiovascular diseases (CVDs) and the outcomes of patients requiring hospitalization for coronavirus disease 2019 (COVID-19) are limited. Therefore, the aim of this study was to investigate the impact of preexisting CVRFs or CVDs on the outcomes of patients with COVID-19 hospitalized in a Korean healthcare system.Patients with COVID-19 admitted to 10 hospitals in Daegu Metropolitan City, Korea, were examined. All sequentially hospitalized patients between February 15, 2020, and April 24, 2020, were enrolled in this study. All patients were confirmed to have COVID-19 based on the positive results on the polymerase chain reaction testing of nasopharyngeal samples. Clinical outcomes during hospitalization, such as requiring intensive care and invasive mechanical ventilation (MV) and death, were evaluated. Moreover, data on baseline comorbidities such as a history of diabetes, hypertension, dyslipidemia, current smoking, heart failure, coronary artery disease, cerebrovascular accidents, and other chronic cardiac diseases were obtained.Of all the patients enrolled, 954 (42.0%) had preexisting CVRFs or CVDs. Among the CVRFs, the most common were hypertension (28.8%) and diabetes mellitus (17.0%). The prevalence rates of preexisting CVRFs or CVDs increased with age (Based on the findings of this study, the patients with confirmed COVID-19 with preexisting CVRFs or CVDs had worse clinical outcomes. Caution is required in dealing with these patients at triage.

Abstract

Coffee is considered the most widely spread beverage and annually, approximately, 11 billion tons are produced all around the world. Raw spent coffee residue (Raw-SCG), biochar coffee residue (SCG), and the activated biochar with distilled H2O (W-SCG), H2SO4 (SCG-S), H3PO4 (SCG-P), and NaOH (SCG-ALK) were fully characterized for their surface area, density, ash, surface topography, surface functional groups and suspension pH. These are parameters that could affect sorption capacity and catalyst properties. In this study, Raw-SCG, obtained from a coffee shop in the campus of Patras University, was pyrolyzed at 850oC in a custom-made ceramic saggar box. BET surface area of biochar consists a basic characteristic that controls its nutrient and pollutant sorption capacity. SCGs produced at 850oC have high specific surface and micropore area and at the same time, low external surface area. The t-plot for the SCG disclosed that the activation with H3PO4 leads to high specific surface (921 m2/g) and micropore (626 m2/g) area compared to other SCG samples. Simultaneously, the activation of SCG increases the pore size of biochar and the highest pore size was observed for the SCG-ALK (37 Å) compared to activated SCG-S (34 Å) and SCG-P (34 Å). The Raw-SCG has slightly acidic nature (pH 5.5) than the biochar SCG (pH 10.6) which has the most alkaline nature. For the activated SCG biochars, SCG-S (pH 4.6) is more acidic than SCG-P (pH 5.2). Furthermore, W-SCG biochar (pH 9.1) is more alkaline than SCG-ALK (pH 8.8). A plethora of possible explanations can be given for the alkaline nature of biochar, such as the removal of acidic groups, but this is still under investigation. Moreover, the five samples present a peak at 1050 cm-1 that corresponds to (C-O) bond and a weak peak at 3450 cm-1 that reveals O-H groups. For the Raw-SCG, a carboxyl stretching mode (C=O) demonstrate a weak peak at 1740 cm-1; the sharp peaks at 2830 and 2950 cm-1 are related to aliphatic C-H2 bending. The presence of a shallow peak at 2450 cm-1 corresponds to CO2 bond for the SCG and W-SCG. It is very important to mention that the peaks of W-SCG and SCG-ALK are similar. The results indicates that biochars SCG produced at 850oC are acidic in nature, they are highly porous materials with micropores corresponding to the majority of pore volume and the activated SCG-P shows a rapid increase of micropore area compared to other activated and washed biochars (SCG-S, SCG-ALK, W-SCG). Abstract

Annually, large amounts of rice husk (>80 million tonnes) are produced around the world and high quantities of them are produced by developing countries. Determination of chemical and physical properties of new materials produced from the activation of rice husk biochars such as density, ash, surface area, surface topography, surface functional groups and suspension pH could affect sorption capacity and catalyst properties. In this study, biochars (RH), produced from raw rice husk (RAW-RH) at 850oC through pyrolysis process, were activated with H2O (W-RH), H2SO4 (RH-S), H3PO4 (RH-P), and NaOH (RH-ALK) and their physicochemical parameters were analysed. This study aims to characterize the surface area and the porosity, the suspension pH, and the functional groups observed on the surface of RAW-RH, RH, W-RH, RH-S, RH-P, and RH-ALK. The determination of the surface area, the pore volume and the average pore size was performed by using gas (N2) adsorption−desorption with the Micromeritics TriStar 3000 Analyzer system using the Brunauer, Emmett, and Teller (BET) equation. Before analysis, the RAW-RH was degassed at 60oC under mild nitrogen flow for 1 h and RHs were degassed at 300oC under mild nitrogen flow for 1 h. The results were recorded by Micromeritics TriStar 3000 software. The t-plot for the RHs disclosed that the activation with NaOH leads to high specific surface (938 m2/g) and micropore area (588 m2/g) compared to other RH samples (367-386 m2/g). Simultaneously, the activation of RH increases the pore size of biochar and the highest pore size was observed for the RH-ALK (51 Å) compared to activated RH-S and RH-P (48 Å). This can be attributed to the silica removal from RAW-RH by alkaline solution that creates a mesoporous material which is more suitable as a catalyst support for bulky reactants. RAW-RH presents various peaks on its surface corresponding to different functional groups. During the high pyrolysis temperature (850oC) most of these peaks either disappear or begin to diminish and only few new peaks appear. Furthermore, RAW-RH has more acidic behavior than the other samples (pH 6.0). For the activated RH, RH-S and RH-P are slightly acidic materials (pH~6.2) and RH-ALK and W-RH are both slightly alkaline (pH~8.5). To conclude, activation with alkali solution creates a more desirable sorbent material since it increases the surface area by 260% compared to activation with acidic solutions which increases the surface area by ~5%. Abstract

Background: The SARS-CoV-2 pandemic is a major challenge for health care services worldwide. It’s impact on oncologic therapies and elective surgery has been described recently, and the literature provides guidelines regarding appropriate elective patient treatment during the pandemic. However, the impact of SARS-CoV-2 pandemic on emergency surgery services has been poorly investigated up to now. Methods: A 17-item web survey had been distributed to emergency surgeons in June 2020 around the world, investigating the impact of SARS-CoV-2 pandemic on patients and septic diseases both requiring emergency surgery and the time-to-intervention in emergency surgery routine, as well as experiences with surgery in COVID-19 patients. Results: Ninety-eight collaborators from 31 countries responded to the survey. The majority (65.3%) estimated the impact of the SARS-CoV-2 pandemic on emergency surgical patient care as being strong or very strong. Due to the pandemic, 87.8% reported a decrease in the total number of patients undergoing emergency surgery and approximately 25% estimated a delay of more than 2 h in the time-to-diagnosis and another 2 h in the time-to-intervention. Fifty percent make structural problems with in-hospital logistics (e.g. transport of patients, closed normal wards etc.) mainly responsible for delayed emergency surgery and the frequent need (56.1%) for a triage of emergency surgical patients. 56.1% of the collaborators observed more severe septic abdominal diseases during the pandemic, especially for perforated appendicitis and severe septic cholecystitis (41.8% and 40.2%, respectively). 62.2% had experiences with surgery in COVID-19-infected patients. Conclusions: The results of The WSES COVID-19 emergency surgery survey are alarming. The combination of an estimated decrease in numbers of emergency surgical patients and an observed increase in more severe septic diseases may be a result of the fear of patients from infection with COVID-19 and a consecutive delayed hospital admission and diagnosis. A critical delay in time-to-diagnosis and time-to-intervention may be a result of changes in in-hospital logistics and operating room as well as intensive care capacities. Obstructive sleep apnea (OSA) is a highly prevalent condition related to several cardiovascular complications. We assessed the effects of continuous positive airway pressure (CPAP) treatment on myocardial performance and various somatometric parameters in patients with severe OSA receiving CPAP. This is a prospective longitudinal study of 44 patients aged 55.8 ± 10.6 years (79.5 % men), with newly diagnosed moderate and severe OSA (apnea–hypopnea index ≥15 with polysomnographic evaluation). Abstract

Echinococcosis (hydatid cyst disease) in humans is a rare parasitic disease that primarily involves the liver and the lungs. Skeletal manifestation of the disease is seldom and often presents as a clinical and radiological problem. The contamination of the primary hydatid cyst complicates the treatment which is also difficult because of the invasive nature of bone involvement and the spillage of fluid with subsequent contamination seeding. We present a case of pathological femoral fracture secondary to contaminated primary hydatid cyst that has been finally treated with static intramedullary nailing. We highlight the importance of including the hydatid bone disease in the differential diagnosis of destructive bone lesions, and we suggest the intramedullary nailing as the final treatment of specific type of pathological fractures.

Abstract

Extra-abdominal complications of pancreatitis such as pancreaticopleural fistulae are rare. A pancreaticopleural fistula occurs when inflammation of the pancreas and pancreatic ductal disruption lead to leakage of secretions through a fistulous tract into the thorax. The underlying aetiology in the majority of cases is alcohol-induced chronic pancreatitis. The diagnosis is often delayed given that the majority of patients present with pulmonary symptoms and frequently have large, persistent pleural effusions. The diagnosis is confirmed through imaging and the detection of significantly elevated amylase levels in the pleural exudate. Treatment options include somatostatin analogues, thoracocentesis, endoscopic retrograde cholangiopancreatography (ERCP) with pancreatic duct stenting, and surgery. The authors present a case of pancreatic pseudocyst pleural fistula in a woman with gallstone pancreatitis presenting with recurrent pneumonias and bilateral pleural effusions.

Abstract

Meckel's diverticulum is the commonest congenital anomaly of the small intestine. It can, however, present acutely relating to a number of complications that may require emergency surgery and possible bowel resection. Perforation is a rare complication and there are only seven cases reported of an inflamed Meckel's diverticulum due to a chicken bone. Laparoscopic repair of Meckel's diverticulum-associated complications has gained popularity over the recent years. We present a case of chicken bone perforation that was treated with laparoscopic-assisted extracorporeal resection of the affected small bowel segment.

BACKGROUND: To validate a new practical Sepsis Severity Score for patients with complicated intra-abdominal infections (cIAIs) including the clinical conditions at the admission (severe sepsis/septic shock), the origin of the cIAIs, the delay in source control, the setting of acquisition and any risk factors such as age and immunosuppression. ----- METHODS: The WISS study (WSES cIAIs Score Study) is a multicenter observational study underwent in 132 medical institutions worldwide during a four-month study period (October 2014-February 2015). Abstract

Introduction. Femoral hernias are at high risk of strangulation due to the narrow femoral canal and femoral ring. This can lead to symptoms of obstruction or strangulation requiring emergency surgery and possible bowel resection. To our knowledge, there is only one previous published report of bilateral strangulated femoral hernia. We present our case of this phenomenon.Case Report. An 86-year-old woman presented with symptoms of small bowel obstruction. Examination revealed two tender lumps in the area of the femoral triangle. CT scan revealed bilateral femoral hernias. Both hernias were repaired and a small bowel resection on the right side was performed with side to side anastomosis. She made an uneventful recovery.Conclusion. Bilateral femoral hernias are a rare occurrence with only one reported case of bilateral strangulation. Our case highlights the importance of meticulous history taking and clinical examination as any delay in diagnosis will increase the risk of mortality and morbidity for the patient. Hernias should always be considered as a cause if one presents with symptoms of abdominal pain or obstruction.

Abstract

Torsion of the gallbladder resulting in a volvulus is a rare clinical finding that poses a diagnostic challenge preoperatively to both surgeons and radiologists. It is thought to occur secondary to a redundant mesentery where rotation of the gallbladder occurs along the axis of the cystic duct and cystic artery. Gallbladder volvulus commonly presents as acute cholecystitis and is rarely diagnosed preoperatively. An early emergency laparoscopic cholecystectomy is the preferred treatment. Early consideration of this diagnosis enables prompt surgical intervention and quick postoperative recovery periods.

BACKGROUND Continuous positive airway pressure (CPAP) is the most effective method for treating obstructive sleep apnea syndrome (OSAS) and alleviating symptoms. Improved sleep quality with effective CPAP therapy might also contribute to attenuated systemic inflammation and improved endothelial function, with subsequent reduction of cardiovascular risk. The aim of this study was to assess the effect of 3-month CPAP therapy on brachial artery flow-mediated dilation (FMD) and plasma C-reactive protein (CRP) levels in patients with OSAS. Abstract

The objective of this study was to test the efficacy of an equine pericardial patch for repairing full-thickness defects of the stomach wall. Circular defects, 1.5 cm in diameter, were created on the anterior wall of the stomach of 12 female New Zealand rabbits. The defects were repaired by an equine pericardial patch. After euthanasia at different time intervals (3 days to 8 weeks) a macroscopic evaluation of the abdominal cavity (including adhesion scoring), mechanical testing and a histological examination of the stomach were performed. The animals survived the surgical procedure and underwent an uneventful recovery until euthanasia. None of the patches failed. Adhesions were observed in all animals and were significant in 3/12 animals. Bursting pressure testing indicated that the repair was durable and that adequate strength to prevent patch failure was achieved by the second week. A histological examination showed gradual narrowing of the perforation site by mucosal and limited muscular regeneration. The equine pericardial patch was successfully used to repair a gastric defect in our experimental model, and it seems that it could have potential as a material suitable for further research concerning the repair of upper gastrointestinal defects.

35. Cefditoren: Comparative efficacy with other antimicrobials and risk factors for resistance in clinical isolates causing UTIs in outpatients

Abstract

Background: To investigate a possible role of Cefditoren, a recently marketed in Greece third-generation oral cephalosporin in urinary infections of outpatients. Methods: During a multicenter survey of Enterobacteriaceae causing UTIs in outpatients during 2005-2007, Cefditoren MICs were determined by agar dilution method in a randomly selected sample of uropathogens. Susceptibility against 18 other oral/parenteral antimicrobials was determined according to Clinical and Laboratory Standards Institute methodology. Results: A total of 563 isolates (330 Escherichia coli, 142 Proteus mirabilis and 91 Klebsiella spp) was studied; MIC50/MIC90 of Cefditoren was 0.25/0.5 mg/L respectively, with 97.1% of the isolates being inhibited at 1 mg/L. All 12 strains producing ESBLs or AmpC enzymes were resistant to cefditoren. Susceptibility rates (%) for amoxicillin/clavulanic acid, cefuroxime axetil, cefotaxime, ciprofloxacin, trimethoprim/sulfamethoxazole and fosfomycin were 93.1-94.1-96.8-93.1-71.9 and 92.8% respectively. Cefditoren MIC was significantly higher in nalidixic/ciprofloxacin non-susceptible strains; resistance to cefditoren was not associated with resistance to mecillinam, fosfomycin nitrofurantoin and aminoglycosides. Multivariate analysis demonstrated history of urinary infection in the last two weeks or three months as risk factors for cefditoren resistance. Conclusions: Cefditoren exhibited enhanced in vitro activity against the most common uropathogens in the outpatient setting, representing an alternative oral treatment option in patients with risk factors for resistance to first-line antibiotics.

Abstract

Laparoscopic cholecystectomy has become the gold standard for treatment of symptomatic gall stone disease. However, its place remains controversial in the management of acute cholecystitis due to a high reported incidence of bile leaks and conversion rate. Tertiary referral centres have reported good results. We present a series of cases after the introduction of an urgent cholecystectomy pathway in a district general hospital.A practice of urgent cholecystectomy for acute cholecystitis was introduced by three consultant general surgeons. All prospective patients having an urgent laparoscopic cholecystectomy for acute cholecystitis, over an 8-month period were entered into a database. A dedicated ultrasound service was instituted to provide prompt diagnosis in these patients. Their demographic details, operative findings, laboratory results were recorded in a prospective database. Timing of ERCP, postoperative complications and conversion rate and hospital stay were also noted.There were 64 patients in the study with a median age of 51 years (range, 21-84 years). There were 21 males and 43 females. All patients underwent laparoscopic cholecystectomy during the index admission. Eleven patients had pre-operative ERCP and 12 patients had on-table cholangiogram. There were no conversions. Postoperative ERCP was required in six patients. The median time interval between admission and operation was 3 days (range, 2-7 days). There were two bile leaks but no common bile duct injury. There were two cases of superficial wound infection. One patient required re-operation for small bowel obstruction secondary to a port site hernia.Urgent laparoscopic cholecystectomy for acute cholecystitis is a feasible treatment option in a district general hospital. A safe practice can be ensured by adherence to a care pathway and a multidisciplinary, consultant-delivered service. Urgent cholecystectomy service can be provided safely in a district general hospital with outcomes comparable to previously published literature.

Given the fact that the hypothesis of respiratory microbe sterility is being degraded, researchers are currently studying healthy lung microbiota homeostasis as well as its disturbance in case of illness. Abstract

Vaginal evisceration is a rare condition most commonly associated with previous vaginal surgery. It usually presents with vaginal bleeding, lower abdominal pain and a protruding mass, and requires immediate assessment and surgical management to salvage the prolapsed bowel. Any delay in the treatment may result in bowel ischaemia and perforation which is associated with higher morbidity and mortality. We report a case of spontaneous vaginal evisceration during defaecation in a 56-year-old postmenopausal women 11 months post hysterectomy. This case highlights the benefits of a combined laparoscopic and transvaginal approach in the successful management of this surgical emergency.

Abstract

Computed tomography (CT) imaging is the most appropriate diagnostic tool to confirm suspected left colonic diverticulitis. However, the utility of CT imaging goes beyond accurate diagnosis of diverticulitis; the grade of severity on CT imaging may drive treatment planning of patients presenting with acute diverticulitis. The appropriate management of left colon acute diverticulitis remains still debated because of the vast spectrum of clinical presentations and different approaches to treatment proposed. The authors present a new simple classification system based on both CT scan results driving decisions making management of acute diverticulitis that may be universally accepted for day to day practice.
