2. Effectiveness and safety of the XEN45 gel stent compared to trabeculectomy in primary open-angle glaucoma: the Gold-Standard Pathway Study

Abstract

Aim To compare the gel stent to trabeculectomy in the Gold-Standard Pathway Study (GPS) patients with primary open-angle glaucoma (POAG).Methods In the GPS, patients with elevated intraocular pressure (IOP) poorly controlled with IOP-lowering medication were randomised (2:1, gel stent:trabeculectomy). Aggregate primary endpoint: patients (%) at month 12 achieving ≥20% IOP reduction from baseline without medication increase, clinical hypotony, vision loss to counting fingers or secondary surgical intervention (SSI). Secondary endpoints included changes in mean IOP and medication count from baseline; postoperative interventions; visual recovery; postoperative complications; and the Symptom and Health Problem Checklist (SHPC-18) questionnaire.Results Of 130 eyes with POAG treated/analysed (gel stent, n=88; trabeculectomy, n=42); 61.4% and 69.0% met the primary endpoint, respectively (p=0.394). At month 12, the change from baseline in mean IOP was statistically greater post-trabeculectomy (by 2.8 mm Hg; p=0.028) than post-gel stent. Postoperative intervention and postoperative complication rates were 39.8% and 75.0% (gel stent) versus 76.2% and 92.9% (trabeculectomy), respectively. Hypotony (IOP ≤6 mm Hg at two consecutive visits) rates were 14.8% (gel stent) and 28.6% (trabeculectomy). Visual recovery was faster/better (per the mean and time to first return to baseline best corrected visual acuity) and SHPC-18-related improvements were greater post-gel stent than post-trabeculectomy.Conclusion Similar proportions of eyes with POAG achieved the primary endpoint post-gel stent and post-trabeculectomy. The mean IOP reduction was statistically greater post-trabeculectomy. The gel stent resulted in fewer postoperative interventions, faster/better visual recovery, favourable patient-reported outcomes and fewer specific adverse events, although more SSIs and IOP elevations were observed.

3. Elastoviscoplastic fluid flow past a circular cylinder

Abstract

The combined effect of fluid elasticity and yield-stress on the flow past a circular cylinder is studied by two-dimensional direct numerical simulation. We analyze the effects of yield-stress, elasticity, shear-thinning, and shear-thickening on the wake characteristics using the Saramito constitutive model. The elastoviscoplastic (EVP) wake flow is studied at a moderate Reynolds number (Re = 100) where two-dimensional vortex shedding occurs in the Newtonian case. We find that in the shear-thinning elastoviscoplastic flow, when yield stress increases, the drag coefficient and root mean square of the lift coefficient both decrease, while the length of the recirculation bubble $L_{RB}$ increases. These changes indicate that the wake oscillation amplitude decreases with an increasing yield stress. For shear-thickening however, the drag coefficient $C_D$ increases at a large Bingham number, and the wake becomes chaotic. The comparison of viscoelastic fluid and EVP fluid reveals that the polymer stresses, tr($\tau^p$), decay considerably less downstream of the cylinder in the EVP case, indicating that significant stresses persist at large distances. We observe that shear-thinning competes with elastic and yield stresses and counteracts their effect, while shear-thickening enhances elastic and yield stress effects, so that the flow pattern can change from periodic to a chaotic flow.

Abstract

Capsules consist of a drug in a soluble hard or soft Shell. Capsule shells are generally made of gelatin which is made from pork skin and bones. This study aims to produce plant-based capsule shells using pectin and glucomannan. The composition used in the study was pectin to glucomannan ratio of 1:1, 1:2, 1:3, 1:4, 1:5. The results showed that the fastest disintegration time in the water was obtained at a composition ratio of 1:1 with a disintegration time of 24 min 18 s. The fastest dissolution time in acid was obtained at a composition ratio of 1:5 with a dissolution time of 2 min 52 s. The increase in glucomannan content affects the tensile strength of the material. Increasing glucomannan content increased the tensile strength so that the capsule shell becomes stronger. The increase in pectin content affects the modulus of elasticity. Increasing the pectin content would reduce the modulus elasticity and therefore the capsule shell becomes more elastic.

10. Bio-Interventional Cyclodialysis and Allograft Scleral Reinforcement for Uveoscleral Outflow Enhancement in Open-Angle Glaucoma Patients: One-Year Clinical Outcomes.

Abstract

Background: To evaluate the one-year safety and effectiveness of bio-interventional cyclodialysis and scleral reinforcement in open-angle glaucoma (OAG) patients undergoing cataract surgery. Methods: An ab-interno approach was used to create a sectoral cyclodialysis in OAG patients who were prospectively followed in a consecutive case series. Subsequent visco-cycloplasty with scleral reinforcement using homologous minimally modified allograft scaffold was completed to maintain patency of the cyclodialysis reservoir and increase uveoscleral outflow. Outcomes were mean medicated IOP and mean number of IOP-lowering medications. Safety outcomes were adverse events (AEs) and best-corrected visual acuity (BCVA) changes. Results: Successful cyclodialysis and allograft bio-scaffold reinforcement was achieved in 117 eyes. There was minimal intraoperatie bleeding and few post-operative adverse events. At baseline, mean BCVA was 0.48 (95% CI: 0.42‒0.54; 20/40 Snellen) and mean ± SD medicated IOP was 20.2 ± 6.0 mmHg on 1.4 ± 1.3 IOP-lowering medications. At 12 months, there was a 27.1% reduction from baseline mean medicated IOP. In eyes with medicated baseline IOP > 21 mmHg (n = 45), there was a 39.7% paired IOP reduction at 12 months with a concurrent reduction in the mean number of IOP lowering medications to 0.8 ± 0.9 which were statistically significant (p < 0.01). For all eyes, 81.9% achieved a medicated IOP ≤ 18 mmHg with no increase in medications at 12 months. To achieve target IOP control, secondary glaucoma surgery was performed in 3.2% of the cases. Conclusion: IOP lowering through uveoscleral outflow enhancement can be achieved by means of a bio-interventional cyclodialysis procedure with allograft scleral reinforcement. [ABSTRACT FROM AUTHOR]

Abstract

Pollution is one reasons for increase temperature which leads to increase the heat waves which have large socioeconomic and healthy impacts on Middle East. By using monthly daily mean of maximum temperature (C°) at height (2m) covered middle east as a grid of (1581) points for selected months (March, April, May) represent spring and (Jun, July, August) represent Summer for the period 1979 to2018, from the ECMWF, model ERA-interim. Many PDFs have been proposed in recent past, but in present study Logistic, Rayleigh and Gamma distribution are used to describe the characteristics of maximum temperature. This paper attempts to determine the best fitted probability distribution of maximum temperature. To check the accuracy of the predicted data using theoretical probability distributions the goodness of fit criteria Z-test used in this paper. According to the goodness-of-fit criteria and from the graphical comparisons it can be said that Logistic distribution provides the best fit for the observed monthly daily mean of maximum temperature data.

Abstract

Communication systems are susceptible to degradation in performance because of interference received through their side lobes. The interference may be deliberate electronic counter measure (ECM), Accidental RF Interference (RFI) or natural noise. The growth of interference communication systems have given rise to different algorithms, Adaptive array techniques offer a possible solution to this problem of interference received through side lobes because of their automatic null steering in both spatial and frequency domains. Key requirement for space-time architecture is to use robust adaptive algorithms to ensure reliable operation of the smart antenna. Space division multiple access (SDMA) involves the use of adaptive nulling to allow two or more users (mobiles) in the same cell to share same frequency and time slot. One beam is formed for each user with nulls in the direction of other users. Different approaches have been used to identify the interferer from desired user. Thus a basic model for determining the angle of arrival of incoming signals, an appropriate antenna beam forming and adaptive algorithms are used for array processing. There is an insatiable demand for capacity in wireless data networks and cellular radio communication systems. However the RF environment that these systems operate in is harsh and severely limits the capacity of traditional digital wireless networks. With normal wireless systems this limits the data rate in cellular radio environments to approximately 200 kbps whereas much higher data rates in excess of 25Mbps are required. A common wireless channel problem is that of frequency selective multi-path fading. To combat this problem, new types of wireless interface are being developed which utilise space, time and frequency diversity to provide increasing resilience to the channel imperfections. At any instant in time, the channel conditions may be such that one or more of these diversity methods may offer a superior performance to the other diversity methods. The overall aim of the research is to develop new systems that use a novel combination of smart antenna MIMO techniques and an advanced communication system based on advanced system configuration that could be exploited by IEEE 802.20 user specification approach for broadband wireless networking. The new system combines the Multi-input Multi-output communication system with frequency diversity in the form of an OFDM modulator. The benefits of each approach are examined under similar channel conditions and results presented.

18. Predicting changes in cataract surgery health outcomes using a cataract surgery appropriateness and prioritization instrument.

Abstract

ObjectiveDetermine whether items in a cataract surgery appropriateness and prioritization questionnaire can predict change in best corrected visual acuity (BCVA) and health related quality of life (HRQOL) following cataract surgery.Methods313 patients with a cataract in Ontario, Canada were recruited to participate. BCVA was measured using the Snellen chart. HRQOL was measured using a generic instrument (EQ5D), a visual functioning instrument (Catquest-9SF), and an appropriateness and prioritization instrument (17 items). Outcomes were measured preoperatively and 3-6 months postoperatively. Descriptive statistics were used to describe demographics and outcomes. For each appropriateness and prioritization questionnaire item, a one-way ANOVA was used to compare group means of the change in BCVA, EQ5D, and Catquest-9SF.ResultsParticipants had a mean age of 69 years and were 56% female. BCVA improved in 81%, EQ5D in 49.6%, and Catquest-9SF score in 84% of patients. Improvement in both BCVA and Catquest-9SF scores were found in 68.5% of patients. The ANOVA showed a statistically significant association between a change in BCVA and the ability to participate in social life, and a statistically significant association between a change in Catquest-9SF and glare, extent of impairment in visual function, safety and injury concerns, ability to work and care for dependents, ability to take care of local errands, ability to assist others and ability to participate in social life.ConclusionsAlmost all patients had improved BCVA and/or visual functioning after surgery. Seven variables from the cataract appropriateness and prioritization instrument were found to be predictors of improvement in Catquest-9SF measuring visual functioning.

20. Ecology, economy and society in the eastern Bengal Delta, c.1840-1943

Abstract

This thesis argues that remarkable fluctuations in the economic and societal performance in colonial eastern Bengal were intimately related to changing ecological relations. During the nineteenth century, new alluvial lands were created by the fluvial actions of the rivers and huge tracts of land were reclaimed from the Sundarbans forest system. The internal dynamics of the new lands were matched by the external factor of the demand for commodity produce in the world market. The result was a successful commercialisation of agriculture which eventually played an important role in the upward social and economic mobility of ordinary cultivators. By the beginning of the twentieth century, however, this formative role of nature, particularly the water regime, ceased to be operative for a number of natural and manmade factors. Particularly, the haphazard extension of railway embankments in a fluid deltaic environment and the importation and growth of the water hyacinth caused enormous obstacles to the free flow of numerous rivers and canals, which adversely affected traditional agriculture and navigation facilities. While the deteriorating ecological regime contributed to the shift from a relatively buoyant economy and society to a declining one, far-reaching changes took place in the social organisation of agrarian productions. In the nineteenth century proprietary (occupancy) rights rested with the peasant labourer who reclaimed land from alluvial formations and forests where he produced his cash and subsistence crops. In the course of early twentieth century, there emerged a new category of people who did not cultivate land themselves, but controlled the proceeds from it. Both rich peasants and the bhadralok, emerging from a Western-educated middle class, successfully attempted to shift the ordinary cultivators’ occupancy rights in their favour in order to secure a rent-receiving position. This process led to the growth of a large number of rent-receivers at the expense of the actual tillers of the land. Thus, while deterioration in the ecological regime of the Delta brought a decline in agricultural production and public health, a strained agrarian relation produced a landless and land-poor rural population which proved to be the easiest prey to the great Bengal famine of 1943.

21. Mechanics of laminated bi-stable tubular structures

Abstract

Bi-stable silt tubes made of laminated composites are novel deployable structures with two different stable configurations. In the first configuration the tubes are straight and strain-free. In the second configuration they are tightly rolled-up and subject to high levels of strain. To achieve this configuration, they have to climb up and across an energy barrier until a potential energy well is reached from where they can only jump out with some external help. These tubes can be used for large deployable structures as deployment actuators in a manner similar to a STEM. But, unlike the STEM, they do not need any drum to roll upon. The key to this bi-stable behaviour is a particular type of composite construction, invented by Daton-Lovett (1996), in which stiff fibres are arranged to ±α to the longitudinal axis, where α is about 45o. The fibre layout is anti-symmetric with respect to the mid-surface of the tube, to avoid twisting the tube when it is rolled up. Analytical models are presented to predict the radius of curvature in second stable configuration. These models assume linear-elastic behaviour of the material and derive the total strain energy expression, which includes the bending strain energy, membrane strain energy and coupled strain energy, within the tube. These models clearly show that the strain energy increases as we move away from the zero energy point, which corresponds to the extended, strain free configuration. This increase is particularly sensitive to the variations in the longitudinal curvature of the tube and less so to the variations in the transverse curvature. Another analytical model is presented to predict the edge deformations in coiled configuration using an anisotropic closed cylinder. It is shown that linear shell theory with appropriate adjustments works very well for laminated slit tubes.

26. Biotissue stent for supraciliary outflow in open-angle glaucoma patients: surgical procedure and first clinical results of an aqueous drainage biostent.

Abstract

Background/aims To report a first-in-human trial in open-angle glaucoma (OAG) subjects treated with a new microinterventional biostent-reinforced cyclodialysis technique to enhance supraciliary aqueous drainage. Methods Subjects (N=10; 74.1±7.9 years old) with OAG and cataracts underwent combined phacoemulsification cataract surgery with implantation of a permanent endoscleral supraciliary biostent to reinforce a controlled cyclodialysis cleft. The biostent comprised decellularised scleral allograft tissue microtrephined into a polymer tubular implant intraoperative/postoperative safety, intraocular pressure (IOP) and glaucoma medications were tracked through 12 months postimplantation. Results Baseline medicated IOP averaged 24.2±6.9 mm Hg with subjects using 1.3±0.8 IOP-lowering medications. Successful biostent implantation was achieved in all individuals without significant complications. Immediate IOP lowering was sustained through 1 year. Twelve-month mean IOP was reduced 40% from baseline to 14.6±3.2 mm Hg (p=0.004; paired two-tailed t-test), and 80% of patients achieved >20% IOP reduction. Biostenting reduced glaucoma medication use 62%, from a baseline mean of 1.3 required medications to 0.5 medications (p=0.037) at postoperative 12 months. The biotissue implant was well tolerated and demonstrated good endothelial safety with only 11% endothelial cell loss at 12 months after combined phaco-biostenting surgery, similar to that expected after phacoemulsification alone. Mean BCVA increased from baseline 20/130 Snellen to 20/36 at postoperative 12 months (p=0.001). Conclusion Supraciliary biostenting in OAG patients is well tolerated, has a good safety profile and produces long-term IOP-lowering while reducing glaucoma medication requirements. Thus, we investigated whether a seasonal rhythm in food-energy density exists, and how this relates to quality of sleep. Subjects/Methods: Two hundred and thirty adult volunteers were investigated across the four seasons. Anthropometrical measurements were obtained and The Pittsburgh Sleep Quality Index was used for an assessment of sleep quality and disturbances. The dietary intake was evaluated using a 24 h dietary recall. Generalized estimating equations were used to estimate seasonal changes in energy density and sleep quality, as well as the association of energy density with sleep quality. All analyses were adjusted for age, sex, education, occupation and shift-work. Results: Mean food energy density was significantly higher in winter as compared with other seasons (P5) in all seasons, with the lowest quality occurring in winter and the highest in spring (P Conclusions: An inverse association between seasonal fluctuation of food energy density and sleep quality was found with winter time, associated with the intake of higher energy dense food products and the lowest sleep quality.

Abstract

The mortality of coronavirus disease 2019 (COVID-19) disease is very high among the elderly or individuals having comorbidities such as obesity, cardiovascular diseases, lung infections, hypertension, and/or diabetes. Our study characterizes the metagenomic features in severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)-infected patients with or without type 2 diabetes, to identify the microbial interactions associated with its fatal consequences.This study compared the baseline nasopharyngeal microbiome of SARS-CoV-2-infected diabetic and nondiabetic patients with controls adjusted for age and gender. The metagenomics based on next-generation sequencing was performed using Ion GeneStudio S5 Series and the data were analyzed by the Vegan-package in R. All three groups possessed significant bacterial diversity and dissimilarity indexes (p 0.05). Spearman's correlation coefficient network analysis illustrated 183 significant positive correlations and 13 negative correlations of pathogenic bacteria (r = 0.6-1.0, p 0.05), and 109 positive correlations between normal flora and probiotic bacteria (r 0.6, p 0.05). The SARS-CoV-2 diabetic group exhibited a significant increase in pathogens and secondary infection-causing bacteria (p 0.05) with a simultaneous decrease of normal flora (p 0.05). The dysbiosis of the bacterial community might be linked with severe consequences of COVID-19-infected diabetic patients, although a few probiotic strains inhibited numerous pathogens in the same pathological niches. This study suggested that the promotion of normal flora and probiotics through dietary supplementation and excessive inflammation reduction by preventing secondary infections might lead to a better outcome for those comorbid patients.

Abstract

Background: Sustainable Development Goal 3.2 has targeted elimination of preventable child mortality, reduction of neonatal death to less than 12 per 1000 livebirths, and reduction of death of children younger than 5 years to less than 25 per 1000 livebirths, for each country by 2030. To understand current rates, recent trends, and potential trajectories of child mortality for the next decade, we present the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 findings for all-cause mortality and cause-specific mortality in children younger than 5 years of age, with multiple scenarios for child mortality in 2030 that include the consideration of potential effects of COVID-19, and a novel framework for quantifying optimal child survival. Methods: We completed all-cause mortality and cause-specific mortality analyses from 204 countries and territories for detailed age groups separately, with aggregated mortality probabilities per 1000 livebirths computed for neonatal mortality rate (NMR) and under-5 mortality rate (U5MR). Scenarios for 2030 represent different potential trajectories, notably including potential effects of the COVID-19 pandemic and the potential impact of improvements preferentially targeting neonatal survival. Optimal child survival metrics were developed by age, sex, and cause of death across all GBD location-years. The first metric is a global optimum and is based on the lowest observed mortality, and the second is a survival potential frontier that is based on stochastic frontier analysis of observed mortality and Healthcare Access and Quality Index. Findings: Global U5MR decreased from 71·2 deaths per 1000 livebirths (95% uncertainty interval [UI] 68·3–74·0) in 2000 to 37·1 (33·2–41·7) in 2019 while global NMR correspondingly declined more slowly from 28·0 deaths per 1000 live births (26·8–29·5) in 2000 to 17·9 (16·3–19·8) in 2019. Abstract

Background: Sustainable Development Goal 3.2 has targeted elimination of preventable child mortality, reduction of neonatal death to less than 12 per 1000 livebirths, and reduction of death of children younger than 5 years to less than 25 per 1000 livebirths, for each country by 2030. To understand current rates, recent trends, and potential trajectories of child mortality for the next decade, we present the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 findings for all-cause mortality and cause-specific mortality in children younger than 5 years of age, with multiple scenarios for child mortality in 2030 that include the consideration of potential effects of COVID-19, and a novel framework for quantifying optimal child survival. Methods: We completed all-cause mortality and cause-specific mortality analyses from 204 countries and territories for detailed age groups separately, with aggregated mortality probabilities per 1000 livebirths computed for neonatal mortality rate (NMR) and under-5 mortality rate (U5MR). Scenarios for 2030 represent different potential trajectories, notably including potential effects of the COVID-19 pandemic and the potential impact of improvements preferentially targeting neonatal survival. Optimal child survival metrics were developed by age, sex, and cause of death across all GBD location-years. The first metric is a global optimum and is based on the lowest observed mortality, and the second is a survival potential frontier that is based on stochastic frontier analysis of observed mortality and Healthcare Access and Quality Index. Findings: Global U5MR decreased from 71·2 deaths per 1000 livebirths (95% uncertainty interval [UI] 68·3-74·0) in 2000 to 37·1 (33·2-41·7) in 2019 while global NMR correspondingly declined more slowly from 28·0 deaths per 1000 live births (26·8-29·5) in 2000 to 17·9 (16·3-19·8) in 2019. In 2019, 136 (67%) of 204 countries had a U5MR at or below the SDG 3.2 threshold and 133 (65%) had an NMR at or below the SDG 3.2 threshold, and the reference scenario suggests that by 2030, 154 (75%) of all countries could meet the U5MR targets, and 139 (68%) could meet the NMR targets. Deaths of children younger than 5 years totalled 9·65 million (95% UI 9·05-10·30) in 2000 and 5·05 million (4·27-6·02) in 2019, with the neonatal fraction of these deaths increasing from 39% (3·76 million [95% UI 3·53-4·02]) in 2000 to 48% (2·42 million; 2·06-2·86) in 2019. NMR and U5MR were generally higher in males than in females, although there was no statistically significant difference at the global level. Neonatal disorders remained the leading cause of death in children younger than 5 years in 2019, followed by lower respiratory infections, diarrhoeal diseases, congenital birth defects, and malaria. The global optimum analysis suggests NMR could be reduced to as low as 0·80 (95% UI 0·71-0·86) deaths per 1000 livebirths and U5MR to 1·44 (95% UI 1·27-1·58) deaths per 1000 livebirths, and in 2019, there were as many as 1·87 million (95% UI 1·35-2·58; 37% [95% UI 32-43]) of 5·05 million more deaths of children younger than 5 years than the survival potential frontier. Interpretation: Global child mortality declined by almost half between 2000 and 2019, but progress remains slower in neonates and 65 (32%) of 204 countries, mostly in sub-Saharan Africa and south Asia, are not on track to meet either SDG 3.2 target by 2030. Focused improvements in perinatal and newborn care, continued and expanded delivery of essential interventions such as vaccination and infection prevention, an enhanced focus on equity, continued focus on poverty reduction and education, and investment in strengthening health systems across the development spectrum have the potential to substantially improve U5MR. 42. Biotissue stent for supraciliary outflow in open-angle glaucoma patients: surgical procedure and first clinical results of an aqueous drainage biostent

Abstract

Background/aimsTo report a first-in-human trial in open-angle glaucoma (OAG) subjects treated with a new microinterventional biostent-reinforced cyclodialysis technique to enhance supraciliary aqueous drainage.MethodsSubjects (N=10; 74.1±7.9 years old) with OAG and cataracts underwent combined phacoemulsification cataract surgery with implantation of a permanent endoscleral supraciliary biostent to reinforce a controlled cyclodialysis cleft. The biostent comprised decellularised scleral allograft tissue microtrephined into a polymer tubular implant intraoperative/postoperative safety, intraocular pressure (IOP) and glaucoma medications were tracked through 12 months postimplantation.ResultsBaseline medicated IOP averaged 24.2±6.9 mm Hg with subjects using 1.3±0.8 IOP-lowering medications. Successful biostent implantation was achieved in all individuals without significant complications. Immediate IOP lowering was sustained through 1 year. Twelve-month mean IOP was reduced 40% from baseline to 14.6±3.2 mm Hg (p=0.004; paired two-tailed t-test), and 80% of patients achieved >20% IOP reduction. Biostenting reduced glaucoma medication use 62%, from a baseline mean of 1.3 required medications to 0.5 medications (p=0.037) at postoperative 12 months. The biotissue implant was well tolerated and demonstrated good endothelial safety with only 11% endothelial cell loss at 12 months after combined phaco-biostenting surgery, similar to that expected after phacoemulsification alone. Mean BCVA increased from baseline 20/130 Snellen to 20/36 at postoperative 12 months (p=0.001).ConclusionSupraciliary biostenting in OAG patients is well tolerated, has a good safety profile and produces long-term IOP-lowering while reducing glaucoma medication requirements.

Abstract

Background/AimsThe current study evaluates the efficacy and safety of the stand-alone implantation of the MINIject (iSTAR Medical, Wavre, Belgium) supraciliary, microinvasive glaucoma drainage device in patients with medically uncontrolled open-angle glaucoma.MethodsThis prospective, multicentre, first-in-human, single-arm interventional study evaluated stand-alone, ab interno implantation in 25 patients of a 5 mm long uveoscleral device made of STAR biocompatible material, which is a soft, microporous, flexible silicone. The primary outcome was the reduction of intraocular pressure (IOP) at 6 months compared with baseline, and follow-up continued until 2 years for 21 patients. Secondary outcomes included success defined as diurnal IOP of ≤21 mmHg and >5 mmHg with an IOP reduction of 20% without (complete) or with/without (qualified) glaucoma medication.ResultsMean baseline IOP was 23.2±2.9 mmHg on 2.0±1.1 glaucoma medication ingredients and decreased to 13.8±3.5 mmHg (−40.7% reduction) on 1.0±1.3 medications 2 years after implantation. Complete success was achieved in 47.6% of patients (10/21) and qualified success in 100% of patients (21/21) at the 2-year follow-up. All patients achieved a 20% IOP reduction with 48% of patients medication-free. No serious ocular adverse events or additional glaucoma surgery were reported. Mean central endothelial cell density (ECD) mildly decreased from 2411 cells/mm2 (n=26) to 2341 cells/mm2 (n=21) at 24 months, which represents a 5% decrease for matched eyes. No patient had a ≥30% decrease in central ECD.ConclusionThis first-in-human study on the stand-alone implantation of the MINIject supraciliary drainage system shows promising IOP-lowering results and medication reduction over 24 months with few adverse events.Trial registration numberNCT03193736.
