Scoliosis is a curvature of the spine, which can involve lateral (sideways) or rotational movement of the vertebral axis. Lateral bends of the spine have usually been thought to be caused by an uneven pelvis, but few studies have analyzed this relationship, and results have been conflicting. Part of the difficulty has been in how the pelvic level has been measured. In addition, one authority suggested that differences in leg lengths (which result in an uneven pelvis) not exceeding 3/4 inch have no effect on scoliosis and therefore do not need correction. The effect of correcting unlevel pelvises in 42 patients with a pelvic unlevelness up to 17 mm and who had lateral scoliosis with spinal bends from 2 to 19 degrees was evaluated. Pelvic unevenness was determined from X-rays of the sacral base, the uppermost portion of the pelvis. Unevenness was gradually corrected by addition of 1.6 mm thick heel lifts to shoes every two weeks. Some patients had musculoskeletal discomfort with additional lift height, but this generally lessened within 10 days. Patients who had a greater initial scoliosis tended to experience muscle pain, occasional nausea, and malaise during correction of the final 20 percent of unlevelness. Pelvic unlevelness decreased significantly from an average of 6.7 mm to 2.6 mm with heel lift treatments. Lateral bending of the spine significantly decreased, from 7.5 degrees to 5.3 degrees on average. The results indicate that even mild pelvic unevenness can cause lateral scoliosis, and correction of the pelvic distortion can improve the condition. • Sixty-six patients admitted to the coronary care unit within 12 hours of the onset of symptoms of acute myocardial infarction (AMI) were subjected to blood sampling at one- to two-hour intervals for analysis of creatine phosphokinase (CPK) and lactic dehydrogenase (LDH) enzymes and isoenzymes. Complete MB CPK curves were obtained in 27 patients, and these were analyzed for optimum times for sampling and minimum number of analyses for detection of AMI. The present study indicates that (1) optimum detection of AMI can be achieved by analysis for MB CPK in a minimum of two samples obtained at 12 and 24 hours after onset of symptoms of AMI; (2) negative results of analyses for MB CPK in samples obtained before 12 hours or after 24 hours should not be used to exclude the diagnosis of AMI; and (3) a total CPK value within the normal range is not a reliable screening test to exclude analysis of MB CPK.(Arch Intern Med 140:329-334, 1980)
