1. Modal Decomposition in Numerical Computation of Eddy Current Transients

Abstract: A methodology to reduce the computational cost of time domain computations of eddy currents problems is proposed and implemented in a parallel computing environment. It is based on the modal decomposition of the current density and it is applicable even in presence of injected currents into the electrodes of a conducting domain. Using a theta-method integration algorithm, the performances of the the proposed approach are compared against those of a classical method based on the Cholesky factorization, for a case of interest from eddy current nondestructive testing. For this large eddy current problem (number of unknowns greater than 100k, number of time steps of interest equal to 100k) the proposed solution method is shown to be much faster than those based on standard time integration schemes., Comment: 18 pages, 14 Figures in 6 captions (1a, 1b, 2a, 2b, 2c, 2d, 3, 4a, 4b, 5a, 5b, 5c, 5d, 6)

2. Implementation of matrix compression in the coupling of JOREK to realistic 3D conducting wall structures

Abstract: JOREK is an advanced non-linear simulation code for studying MHD instabilities in magnetically confined fusion plasmas and their control and/or mitigation. A free-boundary and resistive wall extension was introduced via coupling to the STARWALL and CARIDDI codes, both able to provide dense response matrices describing the electromagnetic interactions between plasma and conducting structures. For detailed CAD representations of the conducting structures and high resolutions for the plasma region, memory and computing time limitations restrict the possibility of simulating the ITER tokamak. In the present work, the Singular Value Decomposition provided by routines from the ScaLAPACK library has been successfully applied to compress some of the dense response matrices and thus optimize memory usage. This is demonstrated for simulations of Tearing Mode and Vertical Displacement Event instabilities. Abstract: In a recent study, we have identified BPH03 as a promising scaffold for the development of compounds aimed at modulating the interaction between PED/PEA15 (Phosphoprotein Enriched in Diabetes/Phosphoprotein Enriched in Astrocytes 15) and PLD1 (phospholipase D1), with potential applications in type II diabetes therapy. PED/PEA15 is known to be overexpressed in certain forms of diabetes, where it binds to PLD1, thereby reducing insulin-stimulated glucose transport. The inhibition of this interaction reestablishes basal glucose transport, indicating PED as a potential target of ligands capable to recover glucose tolerance and insulin sensitivity. In this study, we employ computational methods to provide a detailed description of BPH03 interaction with PED, evidencing the presence of a hidden druggable pocket within its PLD1 binding surface. We also elucidate the conformational changes that occur during PED interaction with BPH03. Moreover, we report new NMR data supporting the in-silico findings and indicating that BPH03 disrupts the PED/PLD1 interface displacing PLD1 from its interaction with PED. Our study represents a significant advancement toward the development of potential therapeutics for the treatment of type II diabetes.

6. Healthcare professionals’ beliefs, attitudes, and thoughts toward cardiopulmonary telerehabilitation: A mixed-methods study

Abstract: Healthcare professionals' opinions, attitudes, and thoughts toward new digital healthcare are often overlooked. However, they play a crucial role in accepting new digital care strategies. This research aimed to understand the perceptions of cardiopulmonary rehabilitation professionals towards telerehabilitation by conducting a mixed-method study at various Fondazione Don Carlo Gnocchi centers in Italy. A total of 14 healthcare workers, including 7 experts in telerehabilitation, participated through surveys and semi-structured focus groups, covering 12 thematic areas derived from the Theoretical Domains Framework (knowledge; professional role; beliefs about capabilities; beliefs about consequences; optimism; reinforcement, goals; memory, attention, and decision process; environmental context and resources; social influences; emotions; ideal patient's profile). Participants (mean age = 45.00 ± 9.06; M:F = 3:11) shared diverse experiences and views on telerehabilitation. All participants had a good knowledge of telerehabilitation. While non-experts indicated technological expertise and preserved cognitive level as a prerequisite for telerehabilitation use, experts believed that no ideal patient exists and that all people can benefit from it. They converged in defining it as a new delivery path of rehabilitation with the same objective of face-to-face rehabilitation services, enhancing the patient's quality of life. Environmental, infrastructural, and institutional resources are needed to enhance accessibility. Positive attitude, optimism, and expectation were reported, but uncertainties about how to manage safety issues and increased workload were mentioned. The study showed a complex picture of staff rehabilitation beliefs about telerehabilitation. Overall, telerehabilitation was considered a great opportunity for patients who face barriers to in-person clinical interventions.

Abstract: Abstract Telerehabilitation is emerging as a promising digital method for delivering rehabilitation to Parkinson’s Disease (PD) patients, especially in the early stages to promote brain resilience. This study explores how cognitive reserve (CR), the brain’s ability to withstand aging and disease, impacts the effectiveness of telerehabilitation. It specifically examines the influence of lifelong cognitive activities on the relationship between neural reserve and improved functional abilities following rehabilitation. In the study, 42 PD patients underwent a 4-month neuromotor telerehabilitation program. CR proxies were assessed using the Cognitive Reserve Index questionnaire (CRIq), brain changes via 3T-MRI, and functional response through changes in the 6-Minute Walk Distance (6MWD). Participants were divided into responders (n = 23) and non-responders (n = 19) based on their 6MWD improvement. A multiple regression model was run to test significant predictors of 6MWD after treatment in each group. The results revealed a significant correlation between 6MWD and CRIq scores, but only among responders. Notably, the CRIq Leisure-Time sub-index, along with baseline 6MWD, were predictors of post-treatment 6MWD. These findings highlight CR’s role in enhancing the benefits of telerehabilitation on PD patients’ neuromotor functions. Clinically, these results suggest that neurologists and clinicians should consider patients’ lifestyles and cognitive engagement as important factors in predicting and enhancing the outcomes of telerehabilitation. The study underscores the potential of CR as both a predictor and booster of telerehabilitation’s effects, advocating for a personalized approach to PD treatment that takes into account individual CR levels.

Abstract: Abstract Background Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Abstract: The Phase Retrieval problem is dealt with for the challenging case where just a single set of (phaseless) radiated field data is available. In particular, even still emulating the solution of crosswords puzzles, we provide decisive improvements over our recent approaches. In fact, by exploiting bandlimitedness and a suitable set of intersecting curves, we definitively lower the computational complexity (thus eliminating drawbacks) of our previous techniques. Numerical examples, concerning applications of actual interest, support the given theory and confirm the effectiveness of the developed procedure.

Abstract: IntroductionThe relationship between neural social cognition patterns and performance on social cognition tasks in daily life is a topic of debate, with key consideration given to the extent to which theory of mind (ToM) brain circuits share properties reflecting everyday social functioning. To test the efficacy of ecological stimuli in eliciting brain activation within the ToM brain circuits, we adapted the Edinburgh Social Cognition test social scenarios, consisting of dynamic ecological contextually embedded social stimuli, to a fMRI paradigm.MethodsForty-two adults (21 men, mean age ± SD = 34.19 years ±12.57) were enrolled and underwent an fMRI assessment which consisted of a ToM task using the Edinburgh Social Cognition test scenarios. We used the same stimuli to prompt implicit (movie viewing) and explicit (silent and two-choice answers) reasoning on cognitive and affective mental states. The fMRI analysis was based on the classical random effect analysis. Group inferences were complemented with supplemental analyses using overlap maps to assess inter-subject variability.ResultsWe found that explicit mentalizing reasoning yielded wide neural activations when two-choice answers were used. We also observed that the nature of ToM reasoning, that is, affective or cognitive, played a significant role in activating different neural circuits.DiscussionThe ESCoT stimuli were particularly effective in evoking ToM core neural underpinnings and elicited executive frontal loops. Future work may employ the task in a clinical setting to investigate ToM network reorganization and plasticity.

13. Reliability and minimal detectable change of the Yoni task for the theory of mind assessment

Abstract: IntroductionThe Theory of Mind (ToM) assessment is becoming essential to evaluate the response to a social cognition intervention and to monitor the progression of social abilities impairment in atypical conditions. In the Italian setting, the Yoni task has been recently validated in its short version (the Yoni-48 task) to evaluate ToM in the clinical setting. The present study aimed to verify the test-retest reliability and the Minimal Detectable Change (MDC) of the Yoni-48 task.MethodsThe Yoni-48 task was administered to 229 healthy adults at two evaluation sessions 3 weeks apart (mean days between sessions = 20.35 ± 1.75) by a psychologist. The test-retest reliability of the Yoni-48 task accuracy and response time was tested by the Intraclass Correlation Coefficient (ICC2,1, two-way random model, absolute agreement type). Then, the MDC95 and MDC90 were computed based on the standard error of measurement. Finally, the 95% limits of agreement were plotted (LOA plot) to visualize the difference and mean score of each pair of measurements.ResultsThe total Yoni-48 task accuracy, but not the response time score, showed a high ICC (>0.80), with an MDC of 0.10. By plotting the LOA plot for the accuracy score no systematic trends were observed.DiscussionThis evidence will support the adoption of the Yoni task in longitudinal designs.

Abstract: Magnetic Resonance imaging based Electrical Properties Tomography (MR-EPT) is a non-invasive technique that measures the electrical properties (EPs) of biological tissues. In this work, we present and numerically investigate the performance of an unrolled, physics-assisted method for 2D MR-EPT reconstructions, where a cascade of Convolutional Neural Networks is used to compute the contrast update. Each network takes in input the EPs and the gradient descent direction (encoding the physics underlying the adopted scattering model) and returns as output the updated contrast function. The network is trained and tested in silico using 2D slices of realistic brain models at 128 MHz. Results show the capability of the proposed procedure to reconstruct EPs maps with quality comparable to that of the popular Contrast Source Inversion-EPT, while significantly reducing the computational time.

17. A digital health home intervention for people within the Alzheimer’s disease continuum: results from the Ability-TelerehABILITation pilot randomized controlled trial

Abstract: AbstractPurpose This study tested the efficacy of digital-health home intervention for people within the Alzheimer’s disease (AD)-continuum.Methods Thirty people within the AD continuum were randomly assigned to a telerehabilitation (ABILITY; 6 males, Mage=78.2 ± 3.95) or treatment as usual (TAU; 8 males, Mage=77.13 ± 6.38), performing cognitive and physical activities at home for six weeks. The ABILITY intervention additionally included a digital platform enabling communication between the hospital and the patient’s home. Efficiency, such as adherence, perceived fit of demands and skills, usability, and effectiveness measures, including neuropsychological level, neuropsychiatric symptoms, and autonomy in daily living, were collected before (T0), after the treatment (T1), and at the 1-year-follow-up (T2).Results The ABILITY program was efficient, with a higher adherence (81% vs. 62%), a higher perceived fit of demands and skills than TAU (p TAU) emerged on the global cognitive level, especially in language, executive functions, and memory domains. Moreover, a treatment carry-over effect (1-year follow-up) was observed in global cognitive functions (especially language) (ABILITY > TAU), behavioral symptoms, and caregiver distress (TAU > ABILITY).Conclusions Our preliminary findings suggest that ABILITY is a promising eHealth intervention to improve at-home treatment adherence and to preserve cognitive and behavioral abilities.

Abstract: Summary: Background: Although immunomodulators have established benefit against the new coronavirus disease (COVID-19) in general, it is uncertain whether such agents improve outcomes without increasing the risk of secondary infections in the specific subgroup of previously immunocompromised patients. We assessed the effect of immunomodulators on outcomes of immunocompromised patients hospitalized for COVID-19. Methods: The protocol was prospectively registered with PROSPERO (CRD42022335397). MEDLINE, Cochrane Central Register of Controlled Trials and references of relevant articles were searched up to 01-06-2022. Authors of potentially eligible randomized controlled trials were contacted to provide data on immunocompromised patients randomized to immunomodulators vs control (i.e., placebo or standard-of-care). Findings: Eleven randomized controlled trials involving 397 immunocompromised patients hospitalized for COVID-19 were included. Ten trials had low risk of bias. There was no difference between immunocompromised patients randomized to immunomodulators vs control regarding mortality [30/182 (16.5%) vs 41/215 (19.1%); RR 0.93, 95% CI 0.61–1.41; p = 0.74], secondary infections (RR 1.00, 95% CI 0.64–1.58; p = 0.99) and change in World Health Organization ordinal scale from baseline to day 15 (weighed mean difference 0.27, 95% CI -0.09–0.63; p = 0.15). In subgroup analyses including only patients with hematologic malignancy, only trials with low risk of bias, only trials administering IL-6 inhibitors, or only trials administering immunosuppressants, there was no difference between comparators regarding mortality. Interpretation: Immunomodulators, compared to control, were not associated with harmful or beneficial outcomes, including mortality, secondary infections, and change in ordinal scale, when administered to immunocompromised patients hospitalized for COVID-19. Abstract: We introduce and discuss a new approach to the phase retrieval of fields radiated by continuous aperture sources having a circular support, which is of interest in many applications including the detection of shape deformations on reflector antennas. The approach is based on a decomposition of the actual 2-D problem into a number of 1-D phase retrieval problems along diameters and concentric rings of the visible part of the spectrum. In particular, the 1-D problems are effectively solved by using the spectral factorization method, while discrimination arguments at the crossing points allows to complete the retrieval of the 2-D complex field. The proposed procedure, which just requires a single set of far field amplitudes, takes advantage from up to now unexplored field properties and it is assessed in terms of reflector aperture fields.

Abstract: In this paper, a new inversion model for 2D microwave imaging is introduced by means of a convenient rewriting of the usual Lippmann Schwinger integral scattering equation. Such model is derived by decomposing the Greens function and the corresponding internal radiation operator in two different contributions. In fact, one of them can be easily computed from the collected scattered data. In case of lossless backgrounds, the resulting model turns out to be more convenient than the traditional one, as it exhibits a lower degree of nonlinearity with respect to parameters embedding the unknown dielectric characteristics. This interesting property suggests its exploitation in the solution of the inverse scattering problem. The achievable performances are tested by comparing the proposed model with the usual one based on the Lippman-Schwinger equation in both cases of linearly approximated and full non-linear frameworks. Abstract: Objective Alterations in voice intensity and quality may constitute a social life limitation in people with multiple sclerosis (MS), but only 2% of cases receive speech therapy. Especially the Lee Silverman Voice Treatment (LSVT)-Loud is a highly effective intensive method for voice intensity, requiring subjects’ repeated attendance at the clinic. Telerehabilitation may represent a feasible solution to bypass potential barriers related to speech therapy attendance, scaling up the beneficial effects of the treatment to a broader population. The proposed protocol aims to test the feasibility and the pilot efficacy of the LSVT-Loud delivered in telerehabilitation (Tele-LSVT-Loud), compared to the same treatment delivered in the clinic (LSVT-Loud). Methods A single-blinded, parallel, two-arm, pilot randomized (1:1 ratio) controlled trial will be performed involving 20 people with MS. Patients will be allocated to 4 weeks of Tele-LSVT-Loud by accessing a telerehabilitation platform at home or LSVT-Loud conventionally delivered in the clinic. Feasibility and pilot effectiveness will be evaluated three times: before (T0), after the treatment (T1), and 3-month follow-up (T2). Feasibility measures will include adherence, adverse events, user experience, motivation, engagement, and acceptability. Vocal intensity during a 1-minute monologue will be the primary outcome measure. Secondary outcome measures will be the vocal quality during a 1-minute monologue, sustained /a/ voice intensity, quality and stability, voice use in daily life, voice subjective perception in daily life, and quality of life. Results Expected results will be (1) high feasibility of Tele-LSVT-Loud and (2) a non-inferiority effect of Tele-LSVT-Loud compared with face-to-face treatment delivery on voice intensity and quality outcomes. Conclusions Tele-LSVT-Loud may be a feasible intervention for MS alteration in voice intensity and quality with a non-inferior effect compared to LSVT-Loud.

Despite a well-acknowledged association of frailty with cognitive impairment, depression, and gray matter morphology, no clear data are available regarding the nature of this relationship. This cross-sectional study aims to disentangle the role of the behavioral, neuropsychological, and neural components as predictors or moderators of frailty.MethodsNinety-six older adults (mean age = 75.49 ± 6.62) were consecutively enrolled and underwent a clinical and MRI (3 T) evaluation to assess frailty, physical activity, global cognitive level, depression, wellbeing, autonomy in daily living, cortical thickness, and subcortical volumes.ResultsResults showed a full mediation of depression on the link between cortical thickness and frailty, while the cognitive level showed no significant mediating role. In particular, left supramarginal thickness had a predicting role on depression, that in turn impacted frailty occurrence. Finally, handgrip weakness was an early key indicator of frailty in this study’s cohort.ConclusionThese data substantiate the role of depression in mediating the link between neural integrity of the supramarginal gyrus and frailty. In the complexity of frailty, handgrip weakness seems to be an early key indicator. These results are relevant for the design of rehabilitation interventions aimed at reversing the frail condition.

Abstract: We describe a successful case of hybrid revascularization of a totally occluded aortobifemoral bypass with retrograde use of the Rotarex S catheter (BD) and complete relining with a Gore Excluder iliac branch endoprosthesis (W.L. Gore & Associates). The repair procedure was performed with femoral surgical access and percutaneous brachial access. Despite left renal artery endoclamping, after the final angiography, deployment of a covered stent in the left renal artery was needed because of residual thrombotic material at the vessel ostium. The procedure was completed with reconstruction using a common femoral artery Dacron graft and bilateral complete iliac surgical branch relining using self-expanding covered stents, with recovery of distal pulses.

Abstract: Abstract Background The Najuta stent-graft (Kawasumi Laboratories Inc., Tokyo, Japan) is usually easily advanced to the correct deployment position in the ascending aorta thanks to the pre-curved delivery J-sheath with all fenestrations automatically oriented towards the supra-aortic vessels. Aortic arch anatomy and delivery system stiffness could however represent limitations for proper endograft advancement, especially when the aortic arch bends sharply. The aim of this technical note is to report a series of bail-out procedures that could be useful to overcome the difficulties encountered during the Najuta stent-graft advancement up to the ascending aorta. Main body The insertion, positioning and deployment of a Najuta stent-graft requires a through-and-through guidewire technique using a .035″ 400 cm hydrophilic nitinol guidewire (Radifocus™ Guidewire M Non-Vascular, Terumo Corporation, Tokyo, Japan) with right brachial and both femoral accesses. When standard maneuver to put the endograft tip into the aortic arch, some bail-out procedures can be applied to obtain proper positioning. Five techniques are described into the text: positioning of a coaxial extra-stiff guidewire; positioning of a long introducer sheath down to the aortic root from the right brachial access; inflation of a balloon inside the ostia of the supra-aortic vessels; inflation of a balloon inside the aortic arch (coaxial to the device); and transapical access technique. This is a troubleshooting guide for allowing physicians to overcome various difficulties with the Najuta endograft as well as for other similar devices. Short conclusion Technical issues in advancing the delivery system of Najuta stent-graft could occur. Therefore, the rescue procedures described in this technical note could be useful to guarantee the correct positioning and deployment of the stent-graft.

Abstract: This paper presents an efficient numerical tool for solving the problem of transient nonlinear evolution of plasma equilibrium in presence of eddy currents. The code, named FRIDA-TD (FRIDA- Time Domain), and based on the existing FRIDA (FRee-boundary Integro-Differential Axisymmetric) code, exploits two different approaches for solving the elated nonlinear system of Partial Differential Equations (PDE): (i) Finite Element Method - Boundary Element Method (FEM-BEM) for plasma equilibrium, and (ii) Axisymmetric Volume Integral (AVI) formulation for eddy currents. The resulting FRIDA-TD is a fast and flexible tool suitable for engineering-oriented purposes, such as the design of Tokamak devices, set up of plasma operations, prediction of performance scenarios, and design of feedback control systems. FRIDA-TD is successfully validated at first with the well-established CarMa0NL code, and then against experimental data of the RFX-mod device.

Abstract: Objective: To report methodology and first-year results of a new educational project called Televascular Games,” which took place during coronavirus disease 2019 pandemic. Methods: Complex aneurysmal aortic disease was discussed during a 2-hour competition webinar, according to three modalities. (1) Planning case competition (PCC): Two to four preoperative computed tomography angiography (CTA) scans of an already performed selected case were submitted for the competition. CTA scans were uploaded anonymously, without any reference to the center or the surgeon who performed the case. The competitor had to prepare a presentation of how he or she would have diagnosed, sized, planned, treated, and followed up the case, of the medical therapy and of the bail-out maneuvers. (2) Challenging case competition (CCC): The competitor elaborates a presentation of an already treated case concerning an aortic topic and discusses sizing, planning, treatment, possible bail-out maneuvers and obtained results. For the CCC and PCC, the competitors with the best score were preselected to present and discuss their plan during the webinars. (3) Quiz competition: Two to six CTA scans of already performed selected aortic cases were submitted for the competition. A quiz with multiple choice questions was answered by the competitors. The top four competitors were selected for the webinars and then they discussed the cases during the webinar. Finally, at the end of the case discussion, the effective case resolution and follow-up were shown. A final winner was voted via televoting, based on six preestablished criteria. The project was endorsed by different national and international societies. Results: Between October 2020 and December 2021, there were 12 Italian and 1 international webinars with 1695 participants overall (mean, 130; range, 86-177). Competitors were 54 years of age (mean, 27 years; range, 22-38 years). Two editions were CCCs, two quiz competitions, and nine PCCs. The reliability of the interobserver sizing of competitors was κ = 0.43 and κ = 0.62 for the proximal and distal sealing measurements respectively and very good (κ = 0.88-0.95) in the evaluation of orientation of the vessels, presence of angulations, calcifications, and thrombus. The sizing discrepancy resulted in a significant variability of the planning (κ = 0.45). The project ranked 9.6 on a 10-point rating scale by all the participants and competitors. Conclusions: The formula of gaming and collegial discussion of aortic cases herein reported has proved valid and attractive during coronavirus disease 2019 pandemic period. The variability of the results on sizing and planning suggested to confer with a second opinion, especially for less experienced surgeons.

Abstract: Theory of Mind (ToM), the ability to understand and attribute mental states to ourselves and others, could be impaired in Multiple Sclerosis (MS), a neurodegenerative disease affecting young adults. Considering that ToM is strictly connected to Quality of Life (QoL) in MS and that could enhance the social support network -which is particularly important for this population-, we aimed to design and implement a novel ToM rehabilitation training. To make the training as much ecological as possible, we have devised a protocol enhancing ToM through stimuli depicting real-world conditions (video-clips taken from cinema movies, literary fictions, and audio voices). We test training’s effect on both cognitive and affective components of ToM in a sample of 13 subjects, randomly assigned to the ToM training Group and to the Control Group. The following ToM tasks were administered: the Reading the Mind in the Eyes (RMET), the Strange Stories task, the Faux Pas Task and the False Belief First- and Second - Order Task (FB II and III order). We also administered a psycho-behavioral assessment through the Toronto Alexithymia Scale (TAS-20). Results show that our novel ToM training is useful in enhancing ToM abilities measured by the following tasks: the RMET (affective task, p = 0.015) and the FB II-order task (FB, cognitive task, p = 0.032). Our ToM training had also a significant effect on the total score of the TAS-20 Scale (p = 0.018) and on its “Difficulty Describing Feelings subscale” (p = 0.018), indicating a reduction of the alexithymia traits. Future works with larger samples could investigate the ToM training effectiveness in a more representative MS populations.

Abstract: In 2021 JET exploited its unique capabilities to operate with T and D–T fuel with an ITER-like Be/W wall (JET-ILW). This second major JET D–T campaign (DTE2), after DTE1 in 1997, represented the culmination of a series of JET enhancements—new fusion diagnostics, new T injection capabilities, refurbishment of the T plant, increased auxiliary heating, in-vessel calibration of 14 MeV neutron yield monitors—as well as significant advances in plasma theory and modelling in the fusion community. DTE2 was complemented by a sequence of isotope physics campaigns encompassing operation in pure tritium at high T-NBI power. Carefully conducted for safe operation with tritium, the new T and D–T experiments used 1 kg of T (vs 100 g in DTE1), yielding the most fusion reactor relevant D–T plasmas to date and expanding our understanding of isotopes and D–T mixture physics. Furthermore, since the JET T and DTE2 campaigns occurred almost 25 years after the last major D–T tokamak experiment, it was also a strategic goal of the European fusion programme to refresh operational experience of a nuclear tokamak to prepare staff for ITER operation. The key physics results of the JET T and DTE2 experiments, carried out within the EUROfusion JET1 work package, are reported in this paper. Progress in the technological exploitation of JET D–T operations, development and validation of nuclear codes, neutronic tools and techniques for ITER operations carried out by EUROfusion (started within the Horizon 2020 Framework Programme and continuing under the Horizon Europe FP) are reported in (Litaudon et al Nucl. Fusion accepted), while JET experience on T and D–T operations is presented in (King et al Nucl. Fusion submitted).

Abstract: Transient magneto-hydrodynamic (MHD) events like edge localized modes (ELMs) or disruptions are a concern for magnetic confinement fusion power plants. Research with the MHD code JOREK towards understanding control of such instabilities is reviewed here in a concise way to provide a complete overview, while we refer to the original publications for details. Experimental validation for unmitigated vertical displacement events progressed. The mechanism of vertical force mitigation by impurity injection was identified. Two-way eddy current coupling to CARIDDI was completed. Shattered pellet injection was simulated in JET, KSTAR, ASDEX Upgrade (AUG) and ITER. Benign runaway electron beam termination in JET and ITER was studied. Coupling of kinetic REs to the MHD is ongoing and a virtual RE synchrotron radiation diagnostic was developed. Regarding pedestal physics, regimes devoid of large ELMs in AUG were simulated and predictive JT60-SA simulations are ongoing. For ELM suppression by resonant magnetic perturbations (RMPs), AUG, ITER and EAST simulations were performed. A free boundary RMP model was validated against experiments. Evidence for penetrated magnetic islands at the pedestal top based on AUG experiments and simulations was found. Simulations of the naturally ELM-free quiescent H-mode in AUG and HL-3 show external kink mode formation prevents pedestal build-up towards an ELM within windows of the edge safety factor. With kinetic neutral particles, high field side high density formation in ITER was simulated and with kinetic impurities, tungsten transport in AUG RMP plasmas was studied. To capture turbulent transport, electro-static full-f particle in cell models for ion temperature gradient and trapped electron modes were established and benchmarked. Application to RMP plasmas shows enhanced turbulence in comparison to unperturbed states. Energetic particle interactions with MHD were studied. Flux pumping that prevents the safety factor on axis from dropping below unity was simulated. First non-linear stellarator applications include current relaxation in l = 2 stellarators, while verification for advanced stellarators progresses.

Abstract: A new approach to the design of graded Photonic Crystals (GPCs) devices is proposed by exploiting the inverse scattering framework as a synthesis tool. The introduced general methodology can be applied to arbitrary far-field specifications, thus allowing to design non-canonical devices. In particular, two different strategies are developed which allow to deal with GPCs with both graded refractive index (GPC_R) and graded filling factor (GPC_F). In both strategies, the inverse scattering problem is solved by a proper reformulation of the Contrast Source Inversion method wherein a proper rescaling of the amplitude of the primary sources is pursued. In particular, in the first one, the GPCs are obtained by exploiting homogenization theories. In the second one, the GPC_R profile is synthesized by exploiting a suitable representation basis for the unknown contrast function and, then, simple analytical formulas are used to determine a GPC_F. The proposed approach is assessed through the synthesis of an antenna generating ${\Sigma}/{\Delta}$ reconfigurable patterns.

Abstract: IntroductionTelerehabilitation systems represent a promising way for the management of chronic disability, delivering technology-enabled rehabilitation outside the hospital setting. However, usability and acceptability assessment with users represents a critical starting point when using digital healthcare solutions. This study aims at evaluating the user experience with a Telerehabilitation system (SIDERA∧B) from the end-user side.MethodsSIDERA∧B consists of an asynchronous delivery of rehabilitation activities through multimedia digital contents and tele-monitoring of vital parameters with technological devices for individualized, home-based management of chronic conditions. Usability (with the System Usability Scale, SUS) and acceptability (using the Technology Acceptance Model, TAM - and The Service User Technology Acceptance Questionnaire, SUTAQ) data were analyzed from the dataset of the SIDERA∧B project (N = 112 patients with Chronic Heart Failure, Parkinson's Disease and Chronic Obstructive Pulmonary Disease). The possible influence of five external factors (i.e., technological expertise, education, sex, age, and level of disability) on TAM domains was tested using Spearman's Correlation analysis.ResultsResults showed a satisfactory level of technological usability (SUS Median = 77.5) and good scores in usability and learnability SUS subdomains (mean scores > 2.5). Regarding technological acceptability, participants showed high scores (Median > 4) in “Behavioral Intention”, “Perceived Usefulness”, and “Perceived Ease of Use” TAM domains. Finally, results from the SUTAQ scale highlighted that the SIDERA∧B system obtained optimal scores in all domains, especially in “Increased accessibility,” “Care personnel concerns,” and “Satisfaction.” Age (rho = −0.291, p = 0.002) and disability level (WHODAS Total score: rho = −0.218, p = 0.021) were the two external factors inversely associated with the Perceived Ease of Use.DiscussionThe age of digital transformation requires everyone to understand, accept and master the changes affecting modern-day healthcare. The usability and acceptability of the SIDERA∧B system were high across all end-users, despite the medium-low level of the technological expertise of the sample. These findings support the efficiency and the suitability of these digital solutions in the modern digital age transition of rehabilitation from inside to outside the clinic.

41. Risk Factors Associated with Severe/Critical COVID-19 in People Living with HIV-1

Abstract: Introduction: Our objective was to determine the risk factors of a “severe/critical” form of COVID-19 in a cohort of people living with HIV-1 (PLWH1) followed in the Bichat University Hospital center in PARIS, FRANCE. Methods: This study was an observational retrospective monocentric cohort of PLWH1 diagnosed with COVID-19 between February 1 st and November 31 st, 2020. Risk factors associated with “severe/critical” forms were determined using stepwise forward selection. Results: One-hundred-and-twenty-nine PLWH1 with COVID-19 were included. COVID-19 diagnosis was confirmed in 98 cases (75.9%) and deemed probable according to the association of clinical criteria and contact case in 31 cases (24.1%). Clinical presentation of COVID-19 was “asymptomatic/mild/moderate” in 95 (73.6%), “severe” in 26 (21.7%) and “critical” in eight (6%). Patients with “severe/critical” COVID-19 tended to be older (median 54 year old), have a higher BMI (median 28.8 kg/m²) and were likely to have diabetes (9 versus 5) or chronic kidney disease (5 versus 2). Transgender women had higher risk too (OR: 4.9 (IC95: 1.35-24.0)). No association was observed between severity of COVID-19 and viral suppression or CD4 rates. Conclusion: Risk factors for severe COVID-19 were similar in PLWH1 than in the general population and PLWH1 transgender women were at higher risk.

45. Towards Printable Natural Dielectric Cloaks via Inverse Scattering Techniques

Abstract: The synthesis of non-magnetic 2D dielectric cloaks as proper solutions of an inverse scattering problem is addressed in this paper. Adopting the relevant integral formulation governing the scattering phenomena, analytic and numerical approaches are exploited to provide new insights on how frequency and direction of arrival of the incoming wave may influence the cloaking mechanism in terms of permittivity distribution within the cover region. In quasi-static (subwavelength) regime a solution is analytically derived in terms of homogeneous artificial dielectric cover with $\varepsilon<\varepsilon_0$ which is found to be a necessary and sufficient condition for achieving omnidirectional cloaking. On the other hand, beyond quasi-static regime, the cloaking problem is addressed as an optimization task looking for only natural dielectric coatings with $\varepsilon>\varepsilon_0$ able to hide the object for a given number of directions of the incident field. Simulated results confirm the validity of both analytic and numerical methodologies and allow to estimate effective bandwidths both in terms of frequency range and direction of arrival of the impinging field.

Abstract: Quantitative microwave imaging employing an ideal Veselago lens is demonstrated using synthetic 2D experiments. Reconstructions of the complex-valued dielectric constant of an arbitrary object-of-interest are obtained from noiseless data using a non-iterative technique. A closed-form Veselago lens Green’s function is utilized in the formulation of the problem. The contrast sources corresponding to a single illumination of the object-of-interest is recovered using total-field measurements at several receiver points located in the lens’ focusing region. Collecting the field in the presence of the ideal Veselago lens produces well-conditioned matrices for the discretized inverse source problem. The contrast is then directly obtained by first calculating the total-field within the object. Synthetic examples having features as small as $\lambda /25$ show that the resolution is only limited by the signal-to-noise ratio of data. Experiments with noisy data reveals that the inversion procedure directly mirrors the noise in the data. Two new filtering processes are introduced based on the truncated Singular Value Decomposition of the data operator and the domain operator involved in the inversion process. These successfully deal with noisy reconstructions. Results show that using the new filtering methods, even with a single illuminating source, outperforms the least-squares method with several illuminating sources.

Abstract: The canonical problem of designing the complex excitations feeding an array such to ensure a desired field intensity distribution inside a given region of interest, while keeping it under control in some other regions, is addressed. To this end, an auxiliary physics inspired model for the induced total field is proposed, whose off-line analysis allows a simplified approach to understand the convenient and non-convenient field interferences between canonical solutions for the induced total field, that are zero order Bessel functions. Moreover, its analysis drastically reduces the computational burden associated to the multi-control points based approaches. The problem at hand plays a key role in many different applications, including radio communications, wireless power transfer as well as hyperthermia treatment planning, and in this paper attention is paid to radiofrequency shimming in magnetic resonance imaging. In fact, the proposed model and tools are tailored to the challenging case of leveling of the magnetic field intensity within an MRI scanner and in case of a bidimensional realistic head phantom.
