7. Surgical management and histopathological patterns of periampullary cancers in Sudanese patients: A single‐center prospective study

Abstract

Abstract Background Histopathological patterns of periampullary cancers (PAC) provide valuable information for clinicians in the selection of effective postoperative therapies and serve as a prognostic marker for patients. Therefore, this study was conducted to determine the surgical management and histopathological patterns of PAC necessary to achieve excellent surgical outcomes. Methods This prospective study was conducted from October 2021 to September 2023 at Ibn Sina Specialized Hospital, Khartoum, Sudan. Eighty patients diagnosed with resectable PAC were enrolled. Results PAC was more prevalent in male patients (52.5%), with a male: female ratio of 1.1:1.0. The peak incidence occurred between 46 and 60 years, representing 38.75% of patients. The most significant reported comorbidity was cigarette smoking (36.25% of patients). Scleral icterus was found in 47.5% of patients. Pancreatic head cancer was found in 55% of patients. Histopathological analysis revealed adenocarcinoma in 82.5% of patients. R0 resection (tumor‐free margins) was the most common resection category (87.5% of patients). Lymphovascular invasion was detected in 37.5% of patients. Malignant lymph node involvement (1–3 nodes) was found 47.5% of patients. Perineural invasion was found in 55%. Finally, the most common histopathological differentiation of PAC was well‐differentiated (Grade I) in 67.5% of patients. The mortality rate was 0%. Conclusions Updated knowledge regarding the management modalities of PAC is a prerequisite for accurate histopathological assessment and good surgical outcomes. Furthermore, histopathological analysis helps clinicians to select effective postoperative therapies for greater improvement in survival outcomes.

Abstract

Recommendation system or also known as a recommender system is a tool to help the user in providing a suggestion of a specific dilemma. Thus, recently, the interest in developing a recommendation system in many fields has increased. Fuzzy Logic system (FLSs) is one of the approaches that can be used to model the recommendation systems as it can deal with uncertainty and imprecise information. However, one of the fundamental issues in FLS is the problem of the curse of dimensionality. That is, the number of rules in FLSs is increasing exponentially with the number of input variables. One effective way to overcome this problem is by using Hierarchical Fuzzy System (HFSs). This paper aims to explore the use of HFSs for Recommendation system. Specifically, we are interested in exploring and comparing the HFS and FLS for the Career path recommendation system (CPRS) based on four key criteria, namely topology, the number of rules, the rules structures and interpretability. The findings suggested that the HFS has advantages over FLS towards improving the interpretability models, in the context of a recommendation system example. This study contributes to providing an insight into the development of interpretable HFSs in the Recommendation systems.

Abstract

The study aimed to identify the electronic management degree of application in public schools in Hail in light of Saudi Arabia's 2030 vision. The researcher developed a questionnaire consisting of three dimensions: (Management Operations, Human Resources, and Electronic Organization) which was distributed to a random sample of (65) female educational supervisors in public education schools in Hail city. The descriptive analysis method was used in the study. The study concluded that the three dimensions came with a medium mean. The results also showed no variations according to the educational qualification variable. Furthermore, there were significant differences according to the work experience variable, and the differences were for those with more experience.

Abstract

Honeypot is a decoy computer system that is used to attract and monitor hackers' activities in the network. The honeypot aims to collect information from the hackers in order to create a more secure system. However, the log file generated by honeypot can grow very large when heavy traffic occurred in the system, such as Distributed Denial of Services' (DDoS) attack. The DDoS possesses difficulty when it is being processed and analyzed by the network administrator as it required a lot of time and resources. Therefore, in this paper, we propose an approach to decrease the log size that is by using a Cron job that will run with a Perl-script. This approach parses the collected data into the database periodically to decrease the log size. Three DDoS attack cases were conducted in this study to show the increasing of the log size by sending a different amount of packet per second for 8 hours in each case. The results have shown that by utilizing the Cron job with Perl-script, the log size has been significantly reduced, the disk space used in the system has also decreased. Consequently, this approach capable of speeding up the process of parsing the log file into the database and thus, improving the overall system performance. This study contributes to providing a pathway in reducing honeypot log storage using the Cron job with Perl-Script.

Abstract

This paper discusses a simple recommender engine, which can match final year project student based on their interests with potential supervisors. The recommender engine is constructed based on Euclidean distance algorithm. The initial input data for the recommender system is obtained by distributing questionnaire to final year students and recording their response in CSV format. The recommender engine is implemented using Java class and application, and result of the initial tests has shown promises that the project is feasible to be pursued as it has the potential of solving the problem of final year students in finding their potential supervisors.

Abstract

This research work aims to develop low- and medium-temperature thermal energy storage (TES) systems using metallic alloys and solar salt as phase change materials (PCMs) for accumulating thermal energy in the temperature ranges between 120 and 140 °C and 215 and 250 °C, respectively. The low purity metallic alloy Bi 58%-Sn 42% was selected as the PCM for low-temperature applications because it is non-hazardous, relatively inexpensive, and it has a suitable melting temperature range. Commercially available high-purity metallic alloys (99.99%) are expensive, whereas lower purity alloys can be a cost-competitive alternative for PCM applications. The sample of a low purity metal alloy, namely Bi 58%-Sn 42% with a purity of 97% was sintered in the laboratory, and its thermal properties were characterized for application as a PCM. Experimental investigations demonstrated that deterioration of the thermal properties of the low purity metal alloy is not substantial in comparison to the pure metallic alloy and that it can be efficiently used as a PCM. Solar salt (NaNO3 60% - KNO3 40%), selected as the PCM for medium-temperature applications due to its high latent heat value, has a relatively low thermal conductivity. Therefore, two techniques were adopted to improve the heat transfer in TES: deploying metallic fins and using graphite as an additive. The experimental tests demonstrate that both methods considerably improve heat transfer and data obtained was used to quantify these effects. In addition, the computational fluid dynamics (CFD) simulations were carried out to evaluate the thermal performance of the metallic alloy and solar salt TES systems with different concentrations of additives and number of fins in terms of the evolution of the liquid fraction and amount of energy stored and released during charging and discharging processes as a function of time. The comparison of numerical and experimental results demonstrated the acceptable accuracy of the developed CFD models. Both experimental and numerical results were used to derive dimensionless correlations for estimation of the heat transfer intensity and time required for charging and discharging of the studied TES systems. These generated dimensionless correlations can be successfully used in engineering practice to design TES systems.
