Although the impacts of coccidian infection are significant among HIV/AIDS infected cases, proper diagnosis and management of coccidian infection is limited in sub-Saharan Africa including Ethiopia.Objective: The aim of this study was to determine the prevalence of coccidian parasitic infections among HIV/AIDS cases before and after commencement of antiretroviral treatment.Methods: An institution-based longitudinal study was conducted among 304 randomly selected HIV/AIDS cases from February to July 2018 before and after commencement of antiretroviral therapy. A structured questionnaire was used to collect sociodemographic and associated factors data. Stool and blood samples were collected before and three months after treatment. Coccidian detection and CD4+ count were conducted via modified acid fast stain technique and fluorescence-activated cell scanning, respectively. Data were entered and analyzed using SPSS version 20. Descriptive statistics were used to compute coccidian prevalence. Logistic regression was used to compute possible association between associated factors and coccidian parasitic infection. Variables with P< 0.05 were considered to be statistically significant.Results: Among 304 HIV/AIDS cases, prevalence of coccidian parasitic infection before and after antiretroviral treatment was 23.4% and 8.9%, respectively. Prevalence of Cryptosporidium spp. (19.7%) and Isospora belli (4.3%) before antiretroviral treatment were higher than Cryptosporidium spp. (7.9%) and Isospora belli (1.0%) after treatment. Drinking unprotected water (AOR: 7.41; 95%CI: 1.64– 33.45), poor knowledge of HIV/AIDS and coccidian parasite (AOR: 4.19; 95%CI: 1.69– 10.40), and CD4+ count below 200 cells/mm3 (AOR: 62.49; 95%CI: 25.32– 154.21) were significantly associated with coccidian infection.Conclusion: Prevalence of coccidian parasites among HIV/AIDS cases decreases after antiretroviral treatment. Drinking unsafe water, limited knowledge of HIV/AIDS and coccidian parasite and low CD4+ cell count are factors associated with coccidian infection. Therefore, proper detection and treatment of coccidian parasites among HIV/AIDS cases should be prioritized.Keywords: Cryptosporidium spp., Isospora belli, antiretroviral treatment, HIV/AIDS

Introduction: Cystoisospora belli (C. belli) is the only pathogenic species of the Cystoisospora genus responsible for severe diarrhea in immunocompromised patients. Most common microscopic method of diagnosis is less sensitive due to intermittent shedding of oocysts. We developed a new single-run polymerase chain reaction (PCR)–based diagnostic assay for C. belli. Methods: A new single-run PCR-based diagnostic assay was standardized for the detection of C. belli. Diagnostic reproducibility and repeatability of the PCR assay were evaluated. A cross-sectional analytical study was done on a total of 354 stool samples collected from 331 immunocompromised patients with diarrhea. All the stool samples were tested for the presence of oocysts of C. belli and were also tested by our new PCR assay for C. belli. Three of the representative PCR products were confirmed by sequencing. Fisher's exact test was used to compare the two proportions. Results: Microscopy detected C. belli in 11/354 (3.1%) of stool samples, and the new PCR-based assay detected C. belli in 16/354 (4.5%). The new single-run PCR-based assay detected C. belli in all the stool samples which were tested positive by microscopy and additionally detected C. belli in five stool samples. The developed PCR assay detected statistically significant proportion of C. belli (p < 0.001) as compared to microscopy. The 795 base pair PCR product from one microscopy positive stool sample and two microscopy negative stool samples were confirmed by sequencing. Conclusion: Our newly developed single-run PCR-based detection assay for C. belli is robust and reproducible. It may be used for molecular diagnosis of cystoisosporiasis especially in transplant, pediatrics, and human immunodeficiency virus (HIV) positive patients. [ABSTRACT FROM AUTHOR]

The current study aimed to revealing of isosporiasis in different species of birds and mammals which caused by Isospora belli, to achieve this target a 287 stool samples were collected from two groups which are : ten species belong to birds and five species belong to mammals distributed as follows: the birds included 30 samples of passer domesticus, 15 samples of streptopeliadecaocto, 25 samples of Columba Livia, 30 samples of Columba domesticus, 15 samples of Anasplatyrhynchos, 10 samples of Anseranserdomesticus, 10 samples of Pycnonotusleucotis, 30 samples of Gallus gallusdomesticus, 20 samples of Coturnixcoturnix, 10 samples of Meleagrisgallopava; the mammals included 20 samples for these species: Canis lupus, Equusferuscaballus, Oryctolaguscuniculus, 24 samples of Feliscatus and 8 samples of Caviaporcellus. All types of specieshave been collected from different provinces in middle of Iraq for the period from January 2017 until September 2018. The samples were kept in the refrigerator at 4°C until it tested in vitro by the preparation of the direct smear and then staining with Ziel- Neelsen stain (acid fast stain) to observe the oocysts of the parasite. DNA of positive fecal samples was extraction and amplification with 18S rRNA by using PCR technique. The results of stool test revealed that 127(44.25%) birds and mammals were infected with isosporiasisand there is no infection in fecal samples which isolated from guinea pig in this study. Also results showed a disparity in the infection rates among birds and mammals under study with a significant difference in the rates of infection between Gallus gallusdomesticus and Pycnonotusleucotis (73.3% and 10.0% respectively) at P < 0.05, the results of DNA electrophoresis confirmed examination results by staining with Ziel – Neelsen, the amplicons of DNA which belong to infected birds and mammals showed size at 550-630 bp. and the tested samples which belong to guinea pig didn’t appear any band in agarose gel media. Objectives: Indications for cholecystectomy have changed dramatically over the past three decades. Cystoisospora belli has been reported in cholecystectomy specimens of immunocompetent patients. The present study was designed to determine the prevalence and clinical association of C belli in the gallbladder.Methods: The study included retrospective review of cholecystectomy specimens (n = 401) removed for various indications, and a prospective cohort of cholecystectomy specimens (n = 22) entirely submitted for histologic evaluation. Correlations of presence of C belli with age, sex, clinical indication, and abnormalities of preoperative laboratory values were assessed by Fisher exact test.Results: C belli was identified in 39/401 (9.7%) of the retrospective cohort, and 6/22 (27.3%) of the entirely submitted specimens. The presence of C belli showed no correlation with age, sex, clinical indication, or laboratory abnormalities.Conclusions: C belli resides in a latent state in the gallbladder and may be best considered a commensal organism. Patients with AIDS are particularly susceptible to infection with intestinal coccidia. In this study the prevalence of infections with Cryptosporidium sp and Cystoisospora belli were evaluated among HIV/AIDS patients in the Triângulo Mineiro region, Brazil. Between July 1993 and June 2003 faecal samples from 359 patients were collected and stained by a modified Ziehl-Neelsen method, resulting in 19.7% of positivity for coccidian (8.6% with Cryptosporidium sp, 10.3% with Cystoisospora belli and 0.8% with both coccidian). Patients with diarrhoea and T CD4+ lymphocyte levels < 200 cells/mm3 presented higher frequency of these protozoans, demonstrating the opportunistic profile of these infections and its relationship with the immunological status of the individual. It was not possible to determine the influence of HAART, since only 8.5% of the patients positive for coccidian received this therapy regularly. Parasitism by Cryptosporidium sp was more frequent between December and February and thus was characterised by a seasonal pattern of infection, which was not observed with Cystoisospora belli.Pacientes com AIDS são particularmente susceptíveis a infecção por coccídios intestinais e nesse estudo foi avaliada a freqüência de Cryptosporidium sp. e Cystoisospora belli entre pacientes HIV/AIDS na região do Triângulo Mineiro, Brasil. No período de julho de 1993 a junho de 2003, amostras de fezes de 359 pacientes foram submetidas à coloração pelo método de Ziehl-Neelsen modificado, sendo detectada a presença de coccídios em 19,7% destas (8,6% de Cryptosporidium sp, 10,3% de Cystoisospora belli e 0,8% de ambos coccídios). Pacientes com diarréia e níveis de linfócitos T CD4+ < 200 células/mm3 apresentaram maior frequência destes protozoários, demonstrando o perfil oportunista destas infecções e a relação com o status imunológico do indivíduo. Não foi possível determinar a influência da HARRT, pois apenas 8,5% dos pacientes positivos para coccídios fazriam uso regular desta terapia. Parasitismo por Cryptosporidium sp foi mais freqüente no período compreendido de dezembro a fevereiro caracterizando padrão sazonal desta infecção, fato não observado com Cystoisospora belli.

Aim: This study was conducted to estimate the prevalence of enteric parasites in HIV patients in Chennai and to correlate with CD4 counts and diarrhoeal status. Material and Methods: Faecal specimens from 100 HIV infected individuals with CD4 < 1000/µl were screened for enteric parasites with wet mounts, modified acid-fast stain for coccidian parasites, modified trichrome stain for Microsporidia, before and after the stool concentration. Agar plate culture for Strongyloides was put up. Chi-square and ANOVA tests were used for statistical analysis. Results: Study group comprised of 38 subjects with acute diarrhoea, 30 with chronic diarrhoea (> 2 weeks) and remaining 32 without diarrhoea. Enteric parasites were detected in 33% of subjects; Isoapora belli (21) being the commonest followed by E.histolyt/Entamoeba dispar (5), Entamoeba coli (2), Cryptosporidium spp (2), Hookworms (2), Strongyloides stercoralis (2), Giardia lamblia (1) and Microsporidium spp (1). There was a significant inverse relation between CD4 counts and duration of diarrhoea. Opportunistic parasites were isolated from the subjects with wide range of CD4 counts and different diarrhoeal status but most commonly from chronic diarrhoea patients. Descreve-se a presença de cistos unizóicos de Isospora belli em linfonodos mesentéricos e de gametócitos no epitélio da vesícula biliar de um paciente brasileiro de 26 anos de idade, com a Síndrome da Imunodeficiência Adquirida que recebeu tratamento, por diversas vezes, com sulfametoxazol-trimetoprim. Discute-se a importância dos cistos teciduais de I. belli como possíveis focos de resistência do parasita e a associação destes a episódios de recidivas da infecção mesmo após tratamento com medicação anticoccídios.

Para investigar la prevalencia de Cryptosporidium sp e Isospora beIIi entre pacientes con infección por el Virus de Inmunodeficiencia Humana (VIH) y diarrea, se estudiaron 20 pacientes VIH seropositivos con evacuaciones líquidas o semilíquidas, en número de 3 ó más por día, con 7 ó más días de evolución. A cada uno se le practicó examen de heces al fresco y con la coloración de Zielh Neelsen (ZN) modificada, realizándose estos exámenes en forma seriada según los resultados, e individualizando cada caso para la solicitud de otros estudios. Delos 20 pacientes, la mayoría fueron del sexo masculino y de 25 a 44 años de edad, de ellos 11 (55.5%) presentaron coccidios asociados a la diarrea. Entre los pacientes con coccidios, 8 presentaron Cryptosporidium sp y a 3 se les encontró Isospora belli correspondiéndole el 40% y 15% respectivamente del total. La observación del primer examen de heces al fresco y/o con la tinción de ZN modificada, permitió el diagnóstico en 7 de los 8 pacientes con Cryptosporidium sp y en 2 de los 3 con Isospora beIli. En algunos de los casos, Criptosporidiosis (75%) e Isosporiasis (100%) fueron la primera infección oportunista. Condiciones sanitarias inadecuadas parecen ser un factor de riesgo importante.

ABSTRACT Cystoisospora belli is an opportunistic protozoan that causes human cystoisosporiasis, an infection characterized by diarrhea, steatorrhea, abdominal pain, fever, and weight loss. The lack of animal models susceptible to C. belli, and the difficulty in obtaining clinical samples with fair amounts of oocysts have limited the research pertaining to the basic biology of this parasite. This study aimed to describe the ultrastructure of endogenous stages of C. belli in Monkey Rhesus Kidney Cells (MK2) and Human Ileocecal Adenocarcinoma cells (HCT-8). Zoites of C. belli exhibited typical morphological features of coccidia, which included a trilaminar pellicle, an apical complex formed by a conoid, polar rings, rhoptries, and micronemes, in addition to dense granules and the endoplasmic reticulum. No crystalloid body was observed but various lipid and amylopectin granules were usually present in the cytoplasm of zoites. We observed a tendency of the endoplasmic reticulum of the host cell to be located near the parasitophorous vacuole membrane. Merozoites were formed by endodyogeny and during replication, the apical complex of the mother cell remained intact. The formation of gametes or oocysts was not observed. The ultrastructural findings of C. belli are further evidence of its proximity to Sarcocystidae family members and corroborate their reclassification as Cystoisospora spp. Microsc. Res. Background: Diarrhea is one of the most common presenting complaints in human immunodeficiency virus-infected individuals. Aims: The study was designed to determine the magnitude of opportunistic and nonopportunistic intestinal parasitic infections among diarrheal patients and association between CD4+ T-cell counts and human immunodeficiency virus (HIV)-infected intestinal parasites. Materials and Methods: A cross-sectional study was conducted among 207 enrolled diarrheal patients attending HIV integrated counseling and testing center from January 2012 to December 2012. Stool samples were subjected to special modified Ziehl-Neelsen and chromotrope staining method for detection of opportunistic protozoans. Blood samples were also collected from all study subjects for HIV testing and CD4+ T-cell counts were estimated by only in HIV-infected patients. Results: Intestinal parasitic pathogens were detected in 46.1% HIV-infected patients and the major pathogens were opportunistic protozoans 32.2% (37/115), most common being Isospora belli 16.5% (19/115) followed by Cryptosporidium parvum 12.2% (14/115). In HIV noninfected diarrheal patients, major pathogens detected were Entamoeba histolytica/Entamoeba dispar 8.7% (8/92) and Ascaris lumbricoides 3.3% (3/92). Conclusions: The opportunistic intestinal protozoans especially I. belli and C. parvum were most commonly isolated in HIV-infected patients with diarrhea. Se presenta un caso de un paciente masculino, bisexual, con diarrea crónica intermitente, a quien por esta sintomatología, se investiga para infección por el Virus de Inmunodeficiencia Humana, resultando positivo por las técnicas de ELISA y WESTERN BLOTT; y diagnosticándosele Isosporiasis, después de varios exámenes copro-parasitológicos. El paciente fue tratado con trimetoprim-sulfametoxazole, observándose recurrencia de su sintomatología. Â

Chronic diarrhea with a 35 kg weight loss (75 kg to 40 kg) occurred during 2 years in an alcoholic patient was diagnosed with Isospora belli infection in the Republic of Korea. The patient, a 70-year old Korean male, had been a heavy drinker for more than 30 years. He was admitted to the Seoul National University Hospital because of long-standing diarrhea and severe weight loss. He had an increased white blood cell (WBC) count with high peripheral blood eosinophilia (36.8-39.9%) and lowered protein and albumin levels but without any evidence of immunosuppression. A parasitic infection was suspected and fecal examination was repeated 3 times with negative results. Peroral endoscopy with mural biopsy was performed in the upper jejunum. The biopsy specimens revealed villous atrophy with loss of villi together with various life cycle stages of L belli, including trophozoites, schizonts, merozoites, macrogamonts, and microgamonts. The patient was treated successfully with oral doses of trimethoprim 160-320 mg and sulfamethoxazole 800-1,600 mg daily for 4 weeks. A follow-up evaluation at 2.5 years later revealed marked improvement of body weight (68 kg), increased protein and albumin levels, and normal WBC count with low eosinophils (3.1%). Abstract: Isospora belli infection, characterized by peripheral blood eosinophilia, is often seen as an opportunistic infection in patients with acquired immunodeficiency syndrome (AIDS). It is also reported in patients with underlying lymphoproliferative disorders including lymphoma and leukemia. Eosinophil-associated gastrointestinal disorders (EGID), including eosinophilic gastroenteritis (EGE), is characterized by eosinophilic infiltration of the gastrointestinal (GI) tract with various GI symptoms. We report a case of a 50-year-old male who developed Isospora superinfection of the small bowel while receiving systemic corticosteroids for EGE. He presented with worsening diarrhea, abdominal pain, nausea and vomiting with worsening peripheral eosinophilia. I. belli infection was diagnosed by the detection of oocysts in stool samples and by the presence of the parasite on duodenal biopsy in the background of tissue eosinophilia. I. belli can cause severe chronic diarrhea in immunocompromised patients on corticosteroids. Trimethoprim–sulfamethoxazole often provided rapid cure. Even though peripheral blood eosinophilia was seen in both EGE and Isospora infection, the identification of subnuclear protozoal inclusions as a new histologic finding, as well as the absence of this finding in previous duodenal biopsies coupled with the continued presence of tissue eosinophilia, favored a parasitic superinfection in the setting of underlying EGE. A total of 450 stool samples were collected from inpatient and outpatient clinics of Pediatric Department, Minia University Hospital, Minia District, Egypt. Two groups of patients were studied, including 200 immunosuppressed and 250 immunocompetent children. Stool samples were subjected to wet saline and iodine mounts. A concentration technique (formol-ether sedimentation method) was carried out for stool samples diagnosed negative by wet saline and iodine mounts. Samples were stained by 2 different methods; acid fast stain (modified Ziehl-Neelsen stain) and Giemsa stain. Total 188 cases (94%) were diagnosed positive for parasitic infections among immunosuppressed children, whereas 150 cases (60%) were positive in immunocompetent children (P<0.0001). The most common protozoan infection in immunosuppressed group was Cryptosporidium parvum (60.2%), followed by Blastocystis hominis (12.1%), Isospora belli (9.7%), and Cyclospora caytenensis (7.8%). On the other hand, Entamoeba histolytica (24.6%) and Giardia lamblia (17.6%) were more common than other protozoans in immunocompetent children. [ABSTRACT FROM AUTHOR]

Abstract: Cyclospora, Cystoisospora, and Microsporidia are eukaryotic enteropathogens that are difficult to detect in stool samples because they require special stains and microscopy. We developed a multiplex polymerase chain reaction (PCR) reaction with 4 primer sets to amplify Cyclospora cayetanensis, Cystoisospora belli, Enterocytozoon bieneusi, and Encephalitozoon intestinalis. Detection of the amplicon is through specific probes coupled to Luminex beads. Sensitivity of the assay was evaluated using Encephalitozoon intestinalis spores and revealed detection of 101 spores spiked into stool. No cross-reactivity was observed. We evaluated the assay on diarrheal specimens from Thailand, Tanzania, Indonesia, and the Netherlands that had been previously tested by microscopy, and the assay yielded 87–100% sensitivity and 88–100% specificity. Microscopy-negative/PCR-positive samples had lower Luminex values, suggesting they were true but with lower burden infections. In summary, this is a convenient single PCR reaction that can detect Cyclospora, Cystoisospora, and Microsporidia without the need for cumbersome microscopic analysis. [Copyright &y& Elsevier]

Human Immunodeficiency Virus (HIV) infection causes progressive damage to both limbs of the immune system, which results in a plethora of opportunistic infections. Among the various opportunistic infections, gastrointestinal infections are very common in HIV / Acquired Immunodeficiency Syndrome (AIDS). Opportunistic spore-forming protozoal parasites, namely, Cryptosporidium parvum, Isospora belli, Cyclospora cayetanensis, and Microsporidia, play a major role in causing chronic diarrhea, accompanied with weight loss, in patients with HIV / AIDS. The purpose of this review is to discuss the salient microbiological, clinical, and diagnostic aspects of important enteric spore-forming opportunistic parasites in HIV / AIDS. Immunocompetent patients usually do not develop the infection. Immunocompromised hosts may have profuse diarrhea with other gastrointestinal symptoms. Treatment is based on trimethoprim-sulfamethoxazole. In 2006 we performed an isolated intestinal transplantation in a patient with ultra-short bowel syndrome. Neither rejection nor clinical problems occurred after transplant, but signs of intestinal inflammation were seen in every protocol biopsy starting at the first month post transplant. Almost 3 months after the procedure, the patient was re-admitted with diarrhea. I. belli infection was diagnosed by detection of the oocysts in stool samples. Antibiotic treatment with trimethoprim–sulfamethoxazole was initiated with excellent outcome and without relapses. Background: The patients with the infection by the human of immunodeficiency virus (HIV) commonly display diarrhea. Several opportunistic infections are responsible for this picture, among them the coccidia in particular Isospora belli, are responsible for a considerable number of cases. The objective of this study was to determine the prevalence of I. belli in a group of patients with HIV who displayed diarrhea. Methods: A cross-sectional study was made. To each patient a questionnaire created for this study was applied to him and three samples were taken in consecutive days for the search of I. belli through the technique of modified Kinyoun. Results: We studied 50 patients, 48 (96%) men and 2 (4%) women. The average of age was of 35.3 (19-73) years. The I. belli was founded in 6 patients for a prevalence of 12%. The unique variable associated to the presence of I. belli was the loss of greater weight between positive patients (7 versus 2.7 kg, p = 0.03). Conclusions: The prevalence of I. belli in this group of patients was similar to the reported one in other Latin American countries. In this study the variable epidemiological who have been related to the presence of I. belli were not present and the antiretroviral therapy did not have major influences. Perhaps the presence of I. belli in this group of patients contributed of significant way to a greater loss of weight and to a greater wasting syndrome. [ABSTRACT FROM AUTHOR]

Abstract: Extraintestinal cystoisosporosis by Cystoisospora belli has already been reported in HIV/AIDS patients, generally involving preferential invasion of mesenteric and trachaeobronchial lymph nodes, liver and spleen by unizoic cysts of this parasite, which may infect macrophages. To test this hypothesis, murine and human macrophages were exposed to sporozoites of C. belli and cultures were observed daily after contact with these cells. The parasites penetrated and multiplied by endodyogeny in both cell types and inserted themselves inside perinuclear vacuoles. After 48h, extracellular parasites were removed from macrophage cultures and incubated in Monkey Kidney Rhesus cells (MK2) where there was intense multiplication. This is the first report of infection of macrophages by this parasite, which supports the hypothesis that these could act as C. belli host cells in extraintestinal sites. [Copyright &y& Elsevier]

Background: The role of opportunistic infections in accelerating disease progression in HIV-positive individuals, leading to quick death, is still receiving serious attention. The objective of this study was to determine the prevalence of Isospora belli infections in HIV-positive patients in Edo State, Nigeria between August 2007 and March 2008. Methods: A total of 268 samples from HIV-positive patients and 20 samples from HIV-negative patients were processed using the modified Ziehl-Neelsen staining technique to microscopically identify the presence of I. belli oocysts. Results: The overall prevalence of the coccidian was 3.1%. Gender and age had no correlation with the prevalence of the parasite (P > 0.05). There was a significant relationship between isosporiasis and CD4+ T cell counts in HIV-positive patients (OR=11.388, 95% CI= 2.797-46.371, P=0.0004). Abstract: A real-time polymerase chain reaction (PCR) assay targeting the internal transcribed spacer 2 region of the ribosomal RNA gene was developed for the detection of Isospora belli DNA in fecal samples, including an internal control to detect inhibition during the amplification process. The assay was performed on species-specific DNA controls (n = 27) and a range of positive (n = 21) and negative (n = 120) stool samples, and achieved 100% specificity and sensitivity. The simple fecal sample collection procedure, the high-throughput potential, and the possibility of quantification makes the I. belli real-time PCR assay a powerful diagnostic tool for epidemiologic studies with possibilities for extension to other helminthes and protozoa using additional molecular targets. In addition, this Isospora PCR could augment the clinical laboratory diagnosis of isosporiasis, in particular, in patients with a travel history to developing countries. [Copyright &y& Elsevier]

Abstract: Travellers'' diarrhoea represents the most frequent health problem when travelling in developing countries. In most cases it is a self-limited disease. Nonetheless it can cause incapacitation and significant morbidity. It can become persistent in up to 3% of travellers who have suffered from acute diarrhoea in regions at high risk. Objective To determine the incidence of Isospora belli infection in HIV-infected patients in France, and to study risk factors. Methods The French Hospital Database on HIV (FHDH) is a prospective cohort study that collects demographic, clinical and therapeutic data on patients managed in 62 hospitals. We reviewed all cases of I. belli infection recorded between 1992 and 2003. We compared the incidence in 1992–1994 [before the use of dual therapy and combination antiretroviral therapy (cART)] and in 1997–2003 (when use of cART was widespread), after stratification for CD4 cell count (< 50, 50–99, 100–199 and > 200 cells/μL). Results A total of 164 patients had I. belli infection either at enrolment ( n=71) or during follow up ( n=93). During the study period, I. belli infection tended to occur less frequently during follow up, and to be diagnosed mainly at database enrolment. The incidence of I. belli infection during follow up fell by 79% [relative hazard (RH) 0.21; 95% confidence interval (CI) 0.13–0.33] in the cART period compared with the pre-cART period; no such change was noted among patients with CD4 cell counts below 50 cells/μL. In multivariable analysis, the risk of I. belli infection was significantly higher among patients from sub-Saharan Africa (RH 4.3; 95% CI 2.6–7.3). After adjustment for CD4 cell count, patients receiving cotrimoxazole prophylaxis were found to be at a lower risk of I. belli infection (RH 0.3; 95% CI 0.2–0.6). Abstract: Intracellular development of Cystoisospora belli was demonstrated in 4 different mammalian cell lines. Human ileocecal adenocarcinoma (HCT-8), epithelial carcinoma of lung (A549), Madin-Darby bovine kidney (MDBK), and African green monkey kidney (VERO) were exposed in vitro to C. belli sporozoites, which had been isolated from the feces of HIV-AIDS patients. Parasites invaded all the cellular types between 4 and 12h after exposure and multiplication was demonstrated after 24h. Grater number of merozoites formed in VERO cells, followed by HCT-8. In the MDBK and HCT-8 cells, the parasitophorous vacuole was less evident and immobile merozoites were observed in the cytoplasm. In VERO cells, one or several parasitophorous vacuoles contained up to 16 mobile sporozoites. No oocysts were found in any of the cell types used. Human infection by Cryptosporidium spp and other coccidia are due to opportunist non-host specific microorganisms. In HIV seropositive patients, the gastrointestinal symptoms accompanying such infections may be serious and prolonged and may include nausea, low-grade fever, abdominal cramps, anorexia and watery diarrhoea. We studied 188 stool samples from 111 patients (84 men and 27 women) with diarrhoea. A modified Ziehl-Nielsen technique for the detection of Cryptosporidium spp and Isospora belli was employed. The mean age of the patients was 31 years. Cryptosporidium spp was seen in 18% (n=20) of the patients, 90% (n=18) of whom were HIV seropositive. Isospora belli was recorded only from HIV seropositive patients (5.4% of all the patients studied and 6.5% of those who were HIV seropositive). These data confirm the good results obtained with this technique for the identification of Cryptosporidium spp and other coccidia and also reaffirm the clinical importance of correctly diagnosing the cause of diarrhoea, particularly in HIV seropositive patients.Infecção humana por Cryptosporidium spp e outros coccideos deve-se a microrganismos oportunistas hospedeiros não-específicos e, em pacientes HIV positivos, os sintomas gastro-intestinais podem ser sérios e prolongados incluindo náuseas, febre baixa, dores abdominais, anorexia e diarréia aquosa. Estudamos 188 amostras de fezes provenientes de 111 pacientes (84 homens e 27 mulheres, com idade média de 31 anos) com diarréia, atendidos no Hospital das Clínicas da Universidade Estadual de Campinas. Para detecção de Cryptosporidium spp e lsospora belli empregou-se a técnica de Ziehl-Nielsen modificada. Oitenta e três porcento dos pacientes (n=92) eram HIV positivos. Cryptosporidium spp foi observado em 18% de todos os pacientes, sendo que destes, noventa porcento eram HIV positivos, lsospora belli foi evidenciada somente em pacientes HIV positivos, com uma freqüência de 5.4% de todos os pacientes e de 6.5% considerando-se apenas os pacientes HIV positivos. Este estudo confirma os bons resultados obtidos com a técnica empregada para identificação de Cryptosporidium spp e outros coccideos, além de ressaltar a importância da investigação etiológica apropriada em casos de diarréia prolongada, especialmente em pacientes imunossuprimidos.

Persistent diarrhea is a major manifestation of Acquired Immunodeficiency Syndrome (AIDS) which might be more complicated in Human Immunodeficiency Virus (HIV) infected children especially those from developing countries. There are numerous reports showing the emergence of intestinal opportunistic coccidian parasites, mostly Cryptosporidium parvum and Isospora belli in HIV-infected individuals. The prevalence of isosporiasis is probably underestimated in developing countries because routinely not all HIV-infected patients are examined for the presence of this protozoan infection. Here we report a case of HIV-infected isosporiasis presenting with failure to thrive and persistent diarrhea. Since I. belli infection in children responds well to therapy with trimethoprim-sulfamethoxazole, isosporiasis should be considered as a treatable infection in AIDS, if it is detected at proper time. Background: To determine the prevalence of Isospora belli and its correlation with CD4+ cell counts in HIV-positive patients with diarrhea in this region. Materials and Methods: Stool samples from 250 HIV-positive patients, including 200 with diarrhea and 50 without diarrhea included in the study were examined for the presence of enteric parasites under microscopy. Prevalence of the enteric parasites with special reference to I. belli in HIV-positive patients with and without diarrhea were calculated and correlated with their CD4+ cell counts. Results: Enteric parasites were detected in 39% of the HIV patients with diarrhea compared to 30% without diarrhea. I. belli was detected in 22% of the patients with diarrhea and in 4% without diarrhea (P = 0.0019). I. belli was the most common parasite, followed by Entamoeba histolytica/dispar (8%) and Cryptosporidium parvum (5%) in HIV-positive patients with diarrhea. In HIV-positive patients without diarrhea, the most common parasite detected was E. histolytica/dispar (12%) followed by C. parvum (6%) and I. belli (4%). Aim: This study was conducted to estimate the prevalence of enteric parasites in HIV patients in Chennai and to correlate with CD4 counts and diarrhoeal status. Material and Methods: Faecal specimens from 100 HIV infected individuals with CD4 < 1000/μl were screened for enteric parasites with wet mounts, modified acid-fast stain for coccidian parasites, modified trichrome stain for Microsporidia, before and after the stool concentration. Agar plate culture for Strongyloides was put up. Chi-square and ANOVA tests were used for statistical analysis. Results: Study group comprised of 38 subjects with acute diarrhoea, 30 with chronic diarrhoea (> 2 weeks) and remaining 32 without diarrhoea. Enteric parasites were detected in 33% of subjects; Isoapora belli (21) being the commonest followed by E.histolyt/Entamoeba dispar (5), Entamoeba coli (2), Cryptosporidium spp (2), Hookworms (2), Strongyloides stercoralis (2), Giardia lamblia (1) and Microsporidium spp (1). There was a significant inverse relation between CD4 counts and duration of diarrhoea. Opportunistic parasites were isolated from the subjects with wide range of CD4 counts and different diarrhoeal status but most commonly from chronic diarrhoea patients. Conclusion: The prevalence of intestinal parasitic infections in HIV patients is high in Chennai, India, especially at CD4 <1000/ μl, I.belli infection being the commonest. Routine screening of all HIV patients with low CD4 counts for coccidian parasitic infections by using simple stool microscopic techniques can help in early diagnosis and treatment. Isospora belli infection is frequent in patients with acquired immunodeficiency syndrome in tropical areas. It has also been reported in other immunodepressive diseases, such as lymphoblastic leukemia, adult T-cell leukemia, and Hodgkin's disease. To date, no case of non-Hodgkin's lymphoma-related isosporiasis has been reported in a non-HIV-infected patient. We describe a case of non-Hodgkin's lymphoma with chronic diarrhea due to I. belli. In Europe, I. belli can cause severe chronic diarrhea in patients with malignancies whose country of origin is in an endemic area. To determine the dynamics of dissemination of the different forms of enteric sporozoa in the groundwater of Mbankomo, a study was carried out from August 2018 to July 2019. The physico-chemical analyzes were carried out both in the field and in the hydrobiology and environment laboratory of the University of Yaoundé I. The observation of protozoan oocysts was carried out after application of the Ziehl-Neelsen staining. Physico-chemical analysis showed slightly acidic water (6.32 ± 0.43 UC), with high turbidity (31.30 ± 23.56 mg/l). Biological analysis indicated the presence of protozoan oocysts with an average density of 42 ± 69 oocysts/l of Cryptosporidium spp., 17 ± 34 oocysts/l of Cyclospora cayetanensis, 9 ± 19 oocysts/l of Isospora belli and 6 ± 11 oocysts/l of Sarcocystis spp. The highest concentrations of oocysts were recorded during the rainy season. The occurrence of intestinal parasites, its relation with the transmission mechanism of HIV, and the clinical state of the AIDS patients, were analyzed in 99 Group IV patients (CDC, 1986), treated at "Hospital Universitário Pedro Ernesto" (HUPE), between 1986 and 1988. The group consisted of 79 (79.8%) patients whose HIV transmission mechanism took place through sexual contact and of 16 (20.2%) who were infected through blood. Feces samples from each patient were examined by four distincts methods (Faust et al, Kato-Katz, Baermann-Moraes and Baxby et al.). The moste occuring parasites were: Cryptosporidium sp., Entamoeba coli and Endolimax nana (18.2%), Strongyloides stercoralis and Giardia lambia (15.2%). E. histolytica and/or E. hartmanni (13.1%), Ascaris lumbricoides (11.1%) and Isospora belli (10.1%). Furthermore, 74.7% of the patients carried at least one species. Intestinal parasites were found in 78.5% of the patients who acquired the HIV through sexual intercourse and in 56,3% of those infected by blood contamination. The difference, was not statistically significant (p > 0.05). In the group under study, the increase of the occurrence of parasitc infections does not seem to depend on the acquisiton of HIV through sexual contact. It appears that in developing countries, the dependancy is more related to the classic mechanisms of parasites transmission and its endemicity.A ocorrência de parasitas intestinais, sua relação com o mecanismo de transmissão do HIV e a apresentação clínica da AIDS foram analisadas em 99 pacientes do grupo IV (CDC, 1986), atendidos no Hospital Universitário Pedro Ernesto (HUPE) entre 1986 e 1988. O grupo era constituído de 79 (79,9%) pacientes cujo mecanismos de transmssão do HIV se deu por via sexual e de 16 (20,2%) que se infectaram por via sanguínea. Amostras de fezes de cada paciente foram examinadas por quatro métodos distintos (Faust et al., Kato-Katz, Baermann-Moraes e Baxby et al.). Os parasitos mais frqüentemente encontrados foram: Cryptosporidium sp., Entamoeba coli e Endolimax nana (18,2%) seguidos de Strongyloides stercoralis e Giardia lamblia (15,2%), E. histolytica e/ou E. hartmanni (13,1%), Ascaris lumbricoides (11,1%) e Isospora belii (10,1%), sendo que 74,4% dos pacientes com AIDS albergavam pelo menos uma espécie. Observamos que 78,5% dos pacientes que adquiriram o HIV por via sexual e 56,3% dos que foram infectados por via sanguínea albergavam parasitos intestinais, mas a diferença não foi estatisticamente significativa (p>0,05). O mecanisto de infecção por via sexual não parece contribuir de forma importante para o aumento da ocorrência de infecções parasitárias no grupo estudado e sim os mecanismos clássicos de transmissão de parasitos em nosso meio.

In order to evaluate the abundance of oocysts in the Mezam watershed in Bamenda, Northwest Region of Cameroon, a study was carried out from January to June 2017. Samples were collected monthly from 13 stations within the watershed. The direct concentration method and the Ziehl-Neelsen technique were employed in the identification of these parasites. The physicochemical analysis showed that the water samples had a neutral pH (7.46±0.46), lowly mineralized (165.61±110.02μS/cm), moderately oxygenated (60.64 ± 17, 25%), present moderate organic pollution (2.85±2.49mg/l KMnO4). KMnO 4 ) and low levels of orthophosphate (1.8±1.88 mg/l PO 4 3- ) and nitrates (2.47±5.06 mg/l NO 3 - ). Biological analysis revealed the presence of Cryptosporidium spp. (143.98±203.35 oocysts/l), Isospora belli (88.47 ± 123.19 oocysts/l), Cyclospora cayetanensis (141.31±143.19 oocysts/l) and Sarcocystis hominis (76 ± 111.04 oocysts/l). The highest densities of these parasites were recorded at the Mufueh stream, situated in the periurban area. Meanwhile, the lowest densities were found in the urban area (Formuki, Mankon, Ayaba and Mezam streams). The dry season showed higher densities of oocysts (471.42±216.32 oocysts /l). Statistical analysis revealed a significant correlation (P ≤ 0.05) between the density of the organisms and the physico-chemical parameters such as pH, oxidability, dissolved oxygen and nitrates.
