Introduction. Gastric volvulus is a condition characterized by abnormal twisting of the stomach by more than 180°; the organo-axial type associated with paraesophageal hernias is more common. This pathology primarily reaches its highest incidence in individuals in their fifth decade of life. Clinical symptoms vary depending on the magnitude of the rotation, presenting acutely with abdominal pain, nausea, and vomiting, or incidentally in chronic cases with subclinical manner. Clinical case. A 41-year-old woman presented with acute abdominal pain, located in the epigastrium, accompanied by nausea and vomiting. Abdominal CT scan revealed a hiatal hernia with abdominal organs protrusion into the thoracic cavity. During laparotomy, the diagnosis of organo-axial gastric volvulus and grade III hiatal hernia (mixed, sliding and paraesophageal) was confirmed. Damage control surgery was performed, reducing the stomach to the abdominal cavity and closing the hiatal defect. After two days of stabilization in the Intensive Care Unit, a sleeve gastrectomy was performed to address 60% necrosis at the greater curvature of the stomach, followed by closure of the abdominal cavity. Conclusion. Acute organo-axial gastric volvulus requires urgent surgical intervention, the nature of which will depend on the hemodynamic stability of the patient, so in severe cases damage control surgery can be considered.
