Abstract

Abstract Objective To investigate the usefulness of 18F-FDG PET-CT for assessing large-vessel (LV) involvement in patients with suspected giant cell arteritis (GCA) and a negative temporal artery biopsy (TAB). Methods A retrospective review of our hospital databases was conducted to identify patients with suspected GCA and negative TAB who underwent an 18F-FDG PET-CT in an attempt to confirm the diagnosis. The gold standard for GCA diagnosis was clinical confirmation after a follow-up period of at least 12 months. Results Out of the 127 patients included in the study, 73 were diagnosed with GCA after a detailed review of their medical records. Of the 73 patients finally diagnosed with GCA, 18F-FDG PET-CT was considered positive in 61 cases (83.5%). Among the 54 patients without GCA, 18F-FDG PET-CT was considered positive in only eight cases (14.8%), which included 1 case of Erdheim-Chester disease, 3 cases of IgG4-related disease, 1 case of sarcoidosis, and 3 cases of isolated aortitis. Overall, the diagnostic performance of 18F-FDG PET-CT for assessing LV involvement in patients finally diagnosed with GCA and negative TAB yielded a sensitivity of 83.5%, specificity of 85.1%, and a diagnostic accuracy of 84% with an area under the ROC curve of 0.844 (95% CI: 0.752 to 0.936). The sensitivity was 89% in occult systemic GCA and 100% in extracranial LV-GCA. Conclusion Our study confirms the utility of 18F-FDG PET-CT in patients presenting with suspected GCA and a negative TAB by demonstrating the presence of LV involvement across different subsets of the disease.

Abstract

Objectives: This study aimed to assess the real use of cefazolin for methicillin-susceptible Staphylococcus aureus (MSSA) infective endocarditis (IE) in the Spanish National Endocarditis Database (GAMES) and to compare it with antistaphylococcal penicillin (ASP). Methods: Prospective cohort study with retrospective analysis of a cohort of MSSA IE treated with cloxacillin and/or cefazolin. Outcomes assessed were relapse; intra-hospital, overall, and endocarditis-related mortality; and adverse events. Risk of renal toxicity with each treatment was evaluated separately. Results: We included 631 IE episodes caused by MSSA treated with cloxacillin and/or cefazolin. Antibiotic treatment was cloxacillin, cefazolin, or both in 537 (85%), 57 (9%), and 37 (6%) episodes, respectively. 3. Validación de la escala de Comunidad de Indagación para la evaluación del Blended Learning

Abstract

El modelo de Comunidad de Indagación (Col), es una alternativa en el proceso de aprendizaje en línea con fundamentos teóricos constructivistas. Para que el aprendizaje en ambientes virtuales sea efectivo, se requiere la participación de una comunidad y considera tres presencias para el desarrollo de una experiencia de aprendizaje exitosa: la presencia cognitiva, la presencia social, y la presencia de enseñanza. El objetivo del estudio fue analizar la validez y confiabilidad de la encuesta Comunidad de Indagación (CoI) desarrollada por J. Ben Arbaugh para evaluar el modelo CoI. Para tal efecto, se aplicó el instrumento a 144 estudiantes de licenciatura en la Universidad Autónoma de Tamaulipas durante el periodo 2021-3. En la solución final se identificó que los ítems se agrupan en tres factores, explicado por el 68.66% de la varianza. En el análisis de consistencia interna mediante alfa de Cronbach, las tres presencias de la escala resultaron con alta confiabilidad (presencia de la enseñanza= 0.975, presencia social=0.929 y presencia cognitiva=0.958). La escala en su conjunto resultó en alfa de Cronbach= 0.975. Los resultados indican que el instrumento es válido para evaluar la implementación del Blended Learning bajo el modelo citado.

Abstract

The production of knowledge goes through a dominant moment that according to the typology of Gibbons, Limoges, Nowotny, Schwartzman, Scott and Trow (1997) and other authors is inserted in the production mode 2 called "Emergent mode" where a transdisciplinary, heterogeneous and heterarchical structure is predominant. The production of knowledge is carried out in context of application, configured by a diverse set of intellectual and social demands. The importance of knowledge and the impact that science-oriented policies have had on the forms of knowledge production by the scholars of the tertiary education is now recognized. These policies have somehow influenced the academy by moving from a personal activity to a community-based profession, fostering a trend towards collective work, with objectives linked to specific social and economic demands whose cost can only be absorbed by the State or those competitors of the industrial world; it moves between small science (made by individuals or small groups) to a great science characterized by groups of researchers participating in programs and wider networks, fully institutionalized, highly professionalized and clearly specialized. Thus, this communication aims to show the existing relationship between the policies that the Mexican State has been promoting to public universities about the production of knowledge that university scholars develop as part of their substantive functions. It is evident how these policies are being interpreted and operationalized through mechanisms that favor (or not) the production of knowledge and how the results of this production impact both inside the university study houses as well as the social and productive sectors. In particular, a comparative analysis is carried out among five public university institutions that make up the northeast of Mexico.

Abstract

Within the framework of experiences of teaching reflection on professional practice, the research arose from the need to know what is the professional practice of beginning teachers in primary education and what kind of factors and problems are incidental to its exercise. The general objective is to explore and describe the characteristics of the professional practice of the beginning teachers, identifying the allelic dimensional factors that intervene in them, as well as the problems they face in the classroom. The theoretical framework of the study places the responsibilities of the beginning teacher in the teaching task; the qualitative methodology of descriptive type allowed the analysis of information through interviews, observations and focus groups to a sample of 15 beginning primary school teachers from the city of Monterrey, Nuevo Leon, Mexico. The categories of Dorfsman (2012) were the foundation of the investigative construct. It is concluded and confirmed that the transition of the training process towards the real practice of the professional practice of the beginning teachers in primary education means that their acting in the classroom is in some cases limited and in other cases benefited by problems and factors of professional and personal type, as well as their initiative and creativity in their teaching work.

El presente trabajo reconoce que en el contexto de la globalización económica, los cambios que se dan al interior de las organizaciones laborales, demandan del trabajador nuevas capacidades que le permitan un mejor desempeño en sus funciones. El tipo de investigación es el estudio de casos múltiples. El instrumento utilizado fue la entrevista y documentos escritos y electrónicos.
