Search

L’accès aux revues médicales scientifiques est indispensable pour les médecins ou les paramédicaux qui souhaitent maintenir à jour leurs connaissances, se documenter pour la prise en charge d’un patient ou s’engager dans des travaux de recherche. Au-delà des outils anglo-saxons (Pub-Med, Google Scholar) qui impliquent de réaliser la recherche documentaire en anglais et qui listent principalement des ressources en anglais, il existe des outils qui permettent une recherche en français et couvrent des ressources en français de façon beaucoup plus complète que les outils anglosaxons. Nous présentons tout d’abord le portail en français CISMeF (Catalogue et Index des sites médicaux de langue française) ouvrant l’accès à une base de données centrée sur les ressources Internet médicales en français (doc’CISMEF) et sur la base LiSSa (Littérature scientifique médicale francophone en santé), tous deux utilisant un moteur de recherche en français : HeTOP (Health Terminology/Ontology Portal). D’autres bases de données et moteurs de recherche en français sont ensuite présentés, soit généralistes (Summon, Wikipédia Encyclopédie médicale), soit spécialisés (Recomedical, VIDAL, Orphanet, Agents tératogènes). Ils sont pour la plupart interfacés et reliés à PubMed. Leur utilité au quotidien nous paraît indiscutable que ce soit pour la recherche documentaire « à froid », mais aussi directement pour la prise en charge des patients au cabinet ou à l’hôpital.

Resume Introduction L’incendie de l’usine chimique Lubrizol a Rouen le 26 septembre 2019 a genere une immense colonne de fumee dirigee nord-est vers l’agglomeration. L’œil pouvant particulierement etre touche par les fumees et autres degagements toxiques de l’incendie, nous avons evalue l’impact de cette catastrophe industrielle et ecologique sur les pathologies irritatives de surface oculaire dans la semaine suivant l’accident. Materiel et methode Nous avons recueilli retrospectivement les donnees medicales des patients qui se sont presentes aux urgences ophtalmologiques (UOph) du CHU de Rouen (seul service d’UOph ouvert pendant les jours suivant l’accident) pendant la semaine suivant l’incendie (semaine S1). Nous avons compare ces donnees a celles des patients ayant consulte dans la semaine precedant l’incendie (semaine S-1). Nous avons egalement recueilli la frequentation aux urgences generales en semaine S-1 et S1, dont le nombre de consultations en lien direct avec l’incendie. Resultats Nous avons accueilli 361 patients aux UOph durant la semaine S1 suivant l’incendie, versus 384 en S-1. Parmi ces patients, 83 (23 %) presentaient une pathologie de la surface oculaire en S1, versus 76 (20 %) en S-1. Une conjonctivite etait retrouvee chez 54 patients en S1 (39 d’allure virale, 9 d’allure allergique, 6 indeterminee) versus 44 en S-1 (27 d’allure virale, 12 d’allure allergique, 5 indeterminee). Un syndrome sec irritatif existait chez 29 patients en S1 versus 32 en S-1. Seuls 4 patients ont mentionne que leurs symptomes etaient en lien direct avec l’incendie : 2 conjonctivites virales, une conjonctivite allergique et un patient inquiet (respectivement a J2, J5, J7 et J7 de l’incendie). Discussion Le nombre de consultants aux urgences ophtalmologiques n’a pas varie dans la semaine suivant l’incendie de l’usine Lubrizol (sauf une baisse le jour meme liee au confinement). On note un nombre plus eleve de consultants en S1 pour des conjonctivites, surtout d’allure virale et probablement sans lien direct avec l’incendie. Les consultations pour syndrome sec irritatif etaient comparables entre les 2 semaines. Malgre une mediatisation majeure de l’evenement au niveau national et un niveau d’inquietude tres important dans la population, l’incendie ne semble pas avoir eu de repercussions sur l’activite aux UOph du CHU de Rouen, pas plus que sur l’activite des urgences generales. Le confinement a domicile de la population le premier jour a eu possiblement un effet protecteur, evitant une exposition directe aux fumees. Les consequences a long terme des depots de suie sur le sol au passage du nuage de fumee restent indeterminees et sous surveillance. Une revue de la litterature sur les consequences oculaires des accidents industriels est presentee. Conclusion Les urgences ophtalmologiques n’ont pas connu de suractivite dans la semaine suivant l’incendie de l’Usine Lubrizol a Rouen et les pathologies de la surface oculaire n’ont pas donne lieu a plus de consultations que la semaine precedente. Ceci permet de conclure a une toxicite oculaire immediate absente ou minime des fumees de l’incendie.

A fire at the Lubrizol chemical factory in Rouen on September 26, 2019 generated a huge column of smoke directed northeast toward the city. As the eye might be particularly affected by the smoke and other toxic emissions from the fire, we assessed the impact of this industrial and ecological disaster on irritative eye surface disease in the week following the accident.We retrospectively collected the medical data of the patients who presented to the Ophthalmology Emergency Department (OED) of Rouen University Hospital (the only OED open during the days following the accident) during the week following the fire (W1). We compared these data with those of patients who presented during the week before the fire (W-1). We also collected data on patients presenting to the ED in general during W-1 and W1, including the number of visits directly related to the fire.361 patients presented to the OED during W1 following the fire, compared with 384 in W-1. Of these patients, 83 (23%) had ocular surface disease in W1, versus 76 (20%) in W-1. Conjunctivitis was found in 54 patients in W1 (39 viral, 9 allergic, 6 undetermined) versus 44 in W-1 (27 viral, 12 allergic, 5 undetermined). A dry irritative syndrome was present in 29 patients in W1 versus 32 in W-1. Only 4 patients directly attributed their symptoms to the fire: 2 viral conjunctivitis, 1 allergic conjunctivitis and 1 worried patient (at D2, D5, D7 and D7 following the fire respectively).The number of emergency eye consultations did not change in the week following the Lubrizol factory fire (except for a decrease the day of the accident, related to the lock-down). There was a higher number of consultations in W1 for conjunctivitis, mostly viral in appearance and probably not directly related to the fire. The number of consultations for dry irritative syndrome was comparable between the two periods. Despite major media coverage of the event at the national level and a very high level of concern among the population, the fire does not seem to have had an effect on OED activity at Rouen University Hospital, nor on general ED visits. The stay-at-home order on the first day may have had a protective effect, avoiding direct exposure to smoke. The long-term consequences of the soot deposits on the ground as the smoke cloud passed over remain undetermined and are under surveillance. A review of the literature on the ocular consequences of industrial accidents is presented.The Ophthalmology Emergency Department did not record increased activity in the week following the Lubrizol Rouen fire, and ocular surface disease did not give rise to more consultations than the week before the fire. This suggests that there was no or minimal immediate ocular toxicity of the smoke from the fire.

Resume Introduction Le syndrome d’hypersensibilite medicamenteuse, ou Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS syndrome), est une toxidermie grave, associant une toxidermie principalement cutanee, des anomalies biologiques dont une hypereosinophilie et/ou des lymphocytes atypiques, et une ou plusieurs atteintes viscerales. Nous rapportons un cas de DRESS syndrome induit par l’Hexaquine ® (contenant de la quinine et de la thiamine) avec une atteinte hepatique et renale et une reactivation virale de l’Human Herpes Virus 6 (HHV6) concomitante. Observation Une femme âgee de vingt-trois ans etait hospitalisee pour une erythrodermie febrile avec alteration de l’etat general, œdeme du visage et adenopathies cervicales douloureuses. Les examens biologiques trouvaient un syndrome inflammatoire, une hypogammaglobulinemie et la presence de lymphocytes actives hyperbasophiles. La recherche d’une etiologie infectieuse mettait en evidence une reactivation HHV6 avec une elevation franche du taux d’IgG en deux semaines et la presence d’ADN circulant en PCR. Le tableau se compliquait secondairement d’une hepatite cytolytique et d’une insuffisance renale severes. L’evolution etait favorable apres instauration d’une corticotherapie generale. L’interrogatoire repete trouvait seulement apres la sortie du service la prise prealable d’Hexaquine ® . Le medicament avait ete pris quotidiennement durant 27 jours avant le debut des symptomes pour des crampes musculaires essentielles. L’interrogatoire retrouvait la notion d’une prise quotidienne d’Hexaquine ® , durant 27 jours avant le debut des symptomes, pour des crampes musculaires essentielles. Aucun autre medicament n’avait ete consomme les semaines precedentes. Un an apres, au decours d’une infection a enterovirus, survenait une eruption cutanee similaire a la premiere mais cette fois isolee, faisant evoquer une reactivation mineure du DRESS syndrome. Discussion Nous rapportons, a notre connaissance, le premier cas de DRESS syndrome induit par l’Hexaquine ® . Notre observation, par la difficulte de trouver le medicament coupable, pourtant pris de maniere exclusive, illustre une nouvelle fois l’importance du temps consacre a l’interrogatoire. La physiopathologie du DRESS syndrome reste incompletement elucidee. Il est neanmoins probable qu’il s’agit d’un phenomene multifactoriel, medicamenteux et viral.

Myopericytoma is a benign tumour generally arising in the subcutaneous and superficial soft tissues of the extremities. Very few cases have been reported in other locations and intracranial examples are exceptional. We now report on three cases of primary intracranial myopericytoma and review the literature on that rare entity. The patients were women in their fifties who presented with decreased visual acuity in two cases and raised intracranial pressure in one case. The tumour involved, respectively, the anterior cranial fossa, the orbital apex and the pineal region. Gross total resection was achieved in all three cases. Histological analysis revealed oval-to-spindle shaped myoid-appearing cells with a striking tendency for concentric perivascular growth. The lesional cells showed apparent differentiation towards perivascular myoid cells as witnessed by smooth muscle actin expression. In one case, an epithelioid differentiation was also present. None of the patients received adjuvant therapy. One patient died of unrelated causes 6 months after surgery. The other two are alive and well at 9 and 12 month follow-up respectively. Myopericytoma is a recently described neoplasm, and it is likely that reappraisal of intracranial haemangiopericytoma with which it shares many histopathologic features will lead to more case reports of primary intracranial myopericytoma.

We propose a method to compute the Isgur–Wise form factors τ 1 / 2 ( 1 ) and τ 3 / 2 ( 1 ) for the decay of B mesons into orbitally excited (P wave) D * * charmed mesons on the lattice in the static limit. We also present the result of a quenched exploratory numerical simulation which shows that the signal/noise ratio allows for a more dedicated computation.

Resume Introduction Nous rapportons le cas d’une malade qui a developpe sous propylthiouracil une dermatose neutrophilique (pustulose sous cornee de Sneddon-Wilkinson et pyoderma gangrenosum) associee a l’apparition de p-ANCA (anticorps anticytoplasme des polynucleaires neutrophiles). Il s’agit a notre connaissance du premier cas de la litterature. Observation Une femme de 40 ans etait traitee par propylthiouracil pour une maladie de Basedow. Seize mois apres le debut du traitement, elle notait l’apparition de lesions pustuleuses entourees d’un halo erythemateux. Les lesions evoluaient de facon chronique, sans etre ameliorees par l’application de propionate de clobetasol (Dermoval ® ). En fevrier 2003, ces lesions persistaient, le diagnostic de pustulose sous-cornee de Sneddon-Wilkinson etait evoque cliniquement et confirme par l’examen histologique d’une pustule. Par la suite, apparaissait un placard inflammatoire de la cheville droite dans un contexte d’hyperthermie, qui evoluait rapidement vers une ulceration douloureuse a contour inflammatoire, bordee de clapiers purulents. Cette lesion etait evocatrice de pyoderma gangrenosum. La biopsie cutanee du bord de l’ulceration etait en faveur d’un pyoderma gangrenosum, montrant un infiltrat dermique de polynucleaires neutrophiles, sans vascularite associee. L’immunofluorescence directe etait negative. Depuis octobre 2002, il existait des p-ANCA de type antimyeloperoxydase. Il n’y avait pas de gammapathie monoclonale a IgA. Un traitement par corticotherapie par voie generale a raison d’1 mg/kg/j etait debute. On observait une amelioration clinique spectaculaire et la guerison en 8 semaines. Discussion Le propylthiouracil est connu pour etre responsable de l’apparition d’ANCA chez 20 a 64 p. 100 des sujets traites par propylthiouracil pour une maladie de Basedow. Le mecanisme physiopathologique est mal connu. Cliniquement, on peut observer des vascularites cutanees, des glomerulonephrites et des polychondrites. De rares cas de dermatoses neutrophiliques chez des sujets ayant des ANCA induits par propylthiouracil ont ete observes. Il s’agissait le plus souvent de syndrome de Sweet. Un cas de pyoderma gangrenosum a ete rapporte, associe a des ANCA induits par propylthiouracil, chez une femme de 44 ans. Ces manifestations dermatologiques surviennent comme chez notre malade dans des delais d’environ 2 ans. Les donnees de la litterature nous permettent de rapporter la presence de p-ANCA et d’une dermatose neutrophilique a la prise de propylthiouracil. L’association de pyoderma gangrenosum et de pustulose sous cornee de Sneddon-Wilkinson avec des p-ANCA n’a jamais ete rapportee dans ce contexte endocrinologique. Nous proposons d’inscrire les dermatoses neutrophiliques dans la liste des effets secondaires du propylthiouracil.

We have treated extensive lentigo maligna in two elderly patients with Imiquimod 5% (Aldara). It was applied 3 times weekly, for 3 to 5 months. Clinical and histological remission was observed over 13 months of follow-up care. Tolerance was good. We suggest that topical imiquimod could be an interesting therapeutic alternative.

Resume Introduction Le syndrome de Bardet-Biedl est une maladie genetique autosomique recessive rare, qui associe obesite, retinite pigmentaire, hexadactylie, hypogenitalisme, atteinte renale et retard mental. D’autres anomalies peuvent se rencontrer dans le syndrome de Bardet-Biedl, mais peu d’anomalies cutanees sont decrites. Observation Une femme de 41 ans, atteinte d’un syndrome de Bardet-Biedl diagnostique a l’âge de 7 ans, avait une lesion cutanee atypique pseudo-verruqueuse, rouge sombre, du pli interfessier, evoluant depuis une quinzaine d’annees, et devenue genante. L’examen histologique montrait une double composante : epitheliale, papillomateuse, acanthosique d’une part et vasculaire, lymphatique d’autre part, faisant conclure a un lymphangiome avec une hyperplasie epidermique. L’IRM de la region sacree visualisait une lesion mediane sous-cutanee, se prolongeant en profondeur jusqu’a la troisieme vertebre coccygienne. Discussion Cette lesion lymphangiomateuse est inhabituelle. Etant donnee sa survenue au cours d’un syndrome polymalformatif rare, nous proposons qu’il puisse s’agir d’un nouveau signe clinique pouvant se rencontrer dans le syndrome de Bardet-Bield.

Infections caused by Cunninghamella bertholletiae are rare but severe. Only 32 cases have been reported as yet, but in 26 of these this species was a contributing cause of the death of the patient. This opportunistic mould in the order Mucorales infects immunocompromized patients suffering from haematological malignancies or diabetes mellitus, as well as solid organ transplant patients. The lung is the organ most often involved. Two cases of primary cutaneous infection have been previously reported subsequent to soft-tissue injuries. We report a case of primary cutaneous C. bertholletiae zygomycosis in a 54-year-old, insulin-dependent diabetic man who was treated with tacrolimus and steroids after kidney transplantation. No extracutaneous involvement was found. In this patient, the infection may have been related to insulin injections. The patient recovered after an early surgical excision of the lesion and daily administration of itraconazole for 2 months. This case emphasizes the importance of an early diagnosis of cutaneous zygomycosis, which often presents as necrotic-looking lesions. Prompt institution of antifungal therapy and rapid surgical intervention are necessary to improve the prospects of patients who have contracted these potentially severe infections.

The running coupling constant can be estimated by computing gluon two- and three-point Green functions from the lattice. Computing in lattice implies working in a fixed gauge sector (Landau). A source of systematic uncertainty is then the contribution of non gauge-invariant condensates, as 〈A2〉, generating power corrections that might still be important at very high energies. We study the impact of this gluon condensate on the analysis of lattice data and the estimate of ΛMS. We finally try the relationship of this condensate with topological properties of QCD through the instanton picture.

Lattice data seems to show that power corrections should be convoked to describe appropriately the transition of the QCD coupling constant running from U.V. to I.R. domains. Those power corrections for the Landau-gauge MOM coupling constant in a pure Yang-Mills theory (N_f=0) are analysed in terms of Operator Product Expansion (O.P.E.) of two- and three-point Green functions, the gluon condensate emerging from this study. We perform an OPE analysis of the flavorless non-perturbative gluon propagator and the symmetric three-gluon vertex in the Landau gauge. The first subdominant operator is $A_\mu A^\mu$ which can condensate in the Landau gauge ``vacuum'' although being a non-gauge invariant operator. We neglect all higher dimension operators. Then the gluon propagator and the symmetric three gluon vertex only depend on one common unknown condensate. We propose a consistency check from lattice data. At two loops for the leading coefficient and with $1/p^2$ corrections at tree-level order the two fitted values for the condensate do not agree. At three loops we argue that the today unknown $\beta_2^{\rm MOM}$ should be equal to $1.5(3)\times \beta_2^{\widetilde{\rm MOM}}=7400(1500)$ to fulfill the OPE relation. Inclusion of the power corrections' anomalous dimensions should improve further the agreement. A patient with an acquired pigmentation related to lichen planus pigmentosus is described. Features of acrokeratosis of Bazex were associated, related to a head and neck carcinoma. Both cutaneous conditions disappeared after treatment of the neoplasia. Diagnostic criteria of lichen planus pigmentosus are reviewed. The paraneoplastic nature of this original observation of lichen planus pigmentosus is discussed.

Resume Introduction. – Les formes pseudotumorales des vascularites systemiques sont rares et concernent au premier chef l’appareil urogenital et le sein. Les formes pseudotumorales de la periarterite noueuse localisees au niveau musculaire sont tres exceptionnelles. Exegese. – Nous rapportons une pseudotumeur des muscles de l’avant-bras chez une femme âgee de 90 ans dont l’examen histologique a fait conclure a une periarterite noueuse localisee. Conclusion. – L’etude de la litterature consacree aux periarterites noueuse a forme pseudotumorale confirme le caractere exceptionnel des localisations musculaires qui, si l’on excepte une observation comparable a la notre, ont ete decrites au niveau des membres inferieurs. L’evolution est le plus souvent rapidement favorable sous traitement corticoide mais des rechutes peuvent survenir.

We review the infrared properties of the pure Yang-Mills correlators and discuss recent results concerning the two classes of low-momentum solutions for them reported in literature; i.e. decoupling and scaling solutions. We will mainly focuss on the Landau gauge and pay special attention to the results inferred from the analysis of the Dyson-Schwinger equations of the theory and from "{\it quenched}" lattice QCD. The results obtained from properly interplaying both approaches are strongly emphasized., Final version to be published in FBS (54 pgs., 11 figs., 4 tabs)

A new type of hereditary glomerulopathy was observed in ten children presenting with early and progressive glomerular symptoms, often associated with hypertension. Light microscopy showed a diffuse increase in the mesangial matrix and generalized widening of the capillary walls. Electron-microscopic examination of renal tissue, after phosphotungstic acid treatment, revealed the presence of fibrillar collagen within the mesangial matrix and the subendothelial aspect of the glomerular basement membrane, adjacent to normal lamina densa. Immunohistochemical studies identified the fibrillar collagen not usually present within the glomerular extracellular matrix as type III collagen. Clinical and family studies ruled out the diagnosis of nail-patella syndrome, an autosomal dominant disorder with typical extrarenal symptoms, which is also characterized by the presence of fibrillar collagen within the glomerular basement membranes. The poor renal outcome, the possible extrarenal haematological and pulmonary involvement and the transmission as an autosomal recessive trait strongly suggest that collagen type III glomerulopathy is a new type of hereditary disease. From the high incidence of superimposed haemolytic uraemic syndrome in patients or their siblings, it may be hypothesized that collagen type III glomerulopathy is the underlying defect in some of the familial cases of haemolytic uraemic syndromes.

The [ital B]-meson decay constant can be measured on the lattice using a 1/[ital m][sub [ital b]] expansion. To relate the physical quantity to Monte Carlo data one has to know the renormalizaton coefficient [ital Z] between the lattice operators and their continuum counterparts. We come back to this computation to resolve discrepancies found in previous calculations. We define and discuss in detail the renormalization procedure that allows the (perturbative) computation of [ital Z]. Comparing the one-loop calculations in the effective Lagrangian approach with the direct two-loop calculation of the two-point [ital B]-meson correlator in the limit of large [ital b]-quark mass, we prove that the two schemes give consistent results to order [alpha][sub [ital s]]. We show that there is, however, a renormalization prescription ambiguity that can have sizable numerical consequences. This ambiguity can be resolved in the framework of an [ital O]([ital a])-improved calculation, and we describe the correct prescription in that case. Finally we give the numerical values of [ital Z] that correspond to the different types of lattice approximations discussed in the paper.

We consider the quark mass function which, in spite of the very large Wilson term artefact, can be studied efficiently with clover fermions, by using the quark pseudoscalar vertex and Ward identities. We then study a series of questions about the chiral limit at N_F=0, through the standard chiral extrapolation method. We confirm that the corresponding OPE of the quark mass function, does not work by far at the available momenta ; this seems to be explainable by a recent high order perturbative calculation of the Wilson coefficient which implies very large high order corrections, much larger than in usual QCD perturbative expansions; the gap with the recognized estimate of the condensate remains large, around a factor 2 at the largest momenta available to us (p ~ 6 GeV), showing the need for very high momenta to test OPE in elementary Green functions. We also observe a remarkable property in function of the physical volume: there is a striking discontinuity in the properties of chiral extrapolation around some physical volume L_c ~ 6 GeV^{-1}=1.2 fm strongly reminiscent of the phenomenon of chiral symmetry restoration observed by Neuberger and Narayanan at $N_C=\infty$ around the same physical length; this discontinuity is observed for the quark mass function as well as for the pion mass and the quark condensate.

We study the low-momentum ghost propagator Dyson-Schwinger equation (DSE) in Landau gauge, assuming for the truncation a constant ghost-gluon vertex, as it is extensively done, and a simple model for a massive gluon propagator. Then, regular DSE solutions (the zero-momentum ghost dressing function not diverging) appear to emerge and we show the ghost propagator to be described by an asymptotic expression reliable up to the order ${\cal O}(q^2)$. That expression, depending on the gluon mass and the zero-momentum Taylor-scheme effective charge, is proven to fit pretty well the low-momentum ghost propagator obtained through big-volume lattice simulations., 11 pages, 1 fig.

We study a relation recently derived by K. Kondo at zero momentum between the Zwanziger's horizon function, the ghost dressing function and Kugo's functions $u$ and $w$. We agree with this result as far as bare quantities are considered. However, assuming the validity of the horizon gap equation, we argue that the solution $w(0)=0$ is not acceptable since it would lead to a vanishing renormalised ghost dressing function. On the contrary, when the cut-off goes to infinity, $u(0) \to \infty$, $w(0) \to -\infty$ such that $u(0)+w(0) \to -1$. Furthermore $w$ and $u$ are not multiplicatively renormalisable. Relaxing the gap equation allows $w(0)=0$ with $u(0) \to -1$. In both cases the bare ghost dressing function, $F(0,\Lambda)$, goes logarithmically to infinity at infinite cut-off. We show that, although the lattice results provide bare results not so different from the $F(0,\Lambda)=3$ solution, this is an accident due to the fact that the lattice cut-offs lie in the range 1-3 GeV$^{-1}$. We show that the renormalised ghost dressing function should be finite and non-zero at zero momentum and can be reliably estimated on the lattice up to powers of the lattice spacing ; from published data on a $80^4$ lattice at $\beta=5.7$ we obtain $F_R(0,\mu=1.5$ GeV)$\simeq 2.2$., Comment: 13 pages, 4 figues

A more compact and standard formulation for the quantum kinetic energy operator of a system of nuclei exercising arbitrary amplitude motion is presented, deriving from a direct transformation from Cartesian centre of mass source coordinates of a set of GJV describing the system to internal and rotational curvilinear coordinates.

In this paper we present a non-trivial check of the consistency of the quantization of a gauge theory with fermions (QCD) in the temporal gauge. We use the approach based on the finite time Feynman propagation kernel, in which the Gauss law is imposed as a constraint on the states by means of a functional integration over all the time independent gauge transformations acting on the boundary values of the fields. Despite the fact that the observed progression of torsional overtone transitions in ethylene appears “pure”, qualitatively correct theoretical potentials indicate substantial coupling of the torsional mode with CC stretching. An attempt is made to recover a single mode description by invoking an internal curvilinear coordinate which follows the minimum energy torsional pathway, together with a corresponding orthogonal coordinate. A theoretical spectrum is computed for the new “torsional” motion and this is shown to be in good agreement with experiment.

We extract the power corrections due to the A^2 condensate in the overlap quark propagator (vector part of the inverse propagator Z_psi). The results are consistent with the previous gluon analysis. The role of artefacts is extensively discussed.

Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS syndrome) is a severe drug eruption defined as an association of a drug reaction, mostly cutaneous, laboratory abnormalities with the presence of hypereosinophilia and/or atypical lymphocytes, and involvement of one or more organs. We report a case of DRESS syndrome associated with quinine or thiamine (which in France are combined in Hexaquine with serious hepatitis and renal failure and associated viral reactivation of Human Herpes Virus 6 (HHV6).A twenty-three year-old woman was hospitalized for asthenia, fever, erythroderma, facial edema and painful cervical lymph nodes. Laboratory results showed inflammatory syndrome, hypogammaglobulinemia and the presence of activated atypical lymphocytes. Broad screening for infectious serology showed HHV6 reactivation with marked increase in specific IgG levels within two weeks and detection of circulating DNA in blood by PCR (polymerase chain reaction). Two severe complications subsequently appeared: cytolytic hepatitis and interstitial nephritis. The outcome was favorable with oral corticosteroid therapy. Repeated history-taking did not initially identify any causal agent, but several weeks later, the patient remembered taking Hexaquine (quinine and thiamine) for essential muscular cramps daily for 27 days before the onset of symptoms. The patient had taken no other medication during the previous weeks. One year later, at the end of an enteroviral infection, a skin rash similar to the first episode was seen but in isolation on this occasion, suggesting minor reactivation of DRESS syndrome.To our knowledge, ours is the first reported case of DRESS syndrome associated with this drug. Because of the difficulty of identifying the causative drug, even though taken alone, our case once again illustrates the importance of spending adequate time on history taking. Although the physiopathology of DRESS syndrome has not yet been fully elucidated, it is nevertheless probable that it is a multi-factor phenomenon (involving medication and viruses among others).

We analysed the clinical, histochemical, ultrastructural and genetic data of patients affected by central core disease (CCD) studied during the last 20 years. From a total series of 86 CCD-families, we have identified 46 CCD families with RYR1 mutations (16 autosomal dominant, 8 autosomal recessive, 17 sporadic cases and 5 de novo mutations). Out of the other 40 CCD families, the RyR1 gene was entirely excluded in 7 families, by cDNA sequencing or linkage analysis, indicating a genetic heterogeneity of CCD.

We study the large momentum behavior of the ghost propagator in the quenched SU(3) lattice gauge theory with Wilson action. The study is performed on lattices with a physical volume fixed around 1.6 fm and different lattice spacings: 0.100, 0.070, and 0.055 fm. We implement an efficient algorithm for computing the Faddeev-Popov operator on the lattice. We are able to extrapolate the lattice data for the ghost propagator towards the continuum and to show that the extrapolated data on each lattice can be described up to four-loop perturbation theory from 2.0 to 6.0 GeV. The three-loop values are consistent with those extracted from previous perturbative studies of the gluon propagator. However the effective {lambda}{sub MS} scale which reproduces the data depends strongly upon the order of perturbation theory and on the renormalization scheme used in the parametrization. We show how the truncation of the perturbative series can account for the magnitude of the dependency in this energy range. The contribution of nonperturbative corrections will be discussed elsewhere.

Gluon two- and three-point Green Functions computed in Landau gauge on the lattice have shown the existence of 1 / q 2 power corrections to the purely perturbative expressions that can be explained through the Operator Product Expansion as the influence of a gauge dependent mass dimension two condensate. The analysis of the ghost propagator within this framework of gauge-dependent power corrections is now performed.

We report on a patient who progressively developed polymorphic expressions of neutrophilic dermatosis (Sneddon-Wilkinson subcorneal pustulosis and pyoderma gangrenosum) associated with p-antineutrophil cytoplasmic antibodies (p-ANCA), while receiving propylthiouracil for hyperthyroidism. To our knowledge, such associations have never been published so far.A 40 year-old woman was treated with propylthiouracil for Graves'disease. After 16 months of therapy, she noted flares of pustular lesions surrounded with erythematous halo mainly localized on the trunk. The lesions became chronic, and were not improved by potent topical corticosteroids. When first seen in our department in February 2003, the eruption was typical of Sneddon-Wilkinson subcorneal pustulosis. This diagnosis was confirmed by the histological examination of a skin biopsy of a pustule. One month later, she developed an inflammatory progressively ulcerative lesion on the right ankle, typical of pyoderma gangrenosum. The diagnosis was confirmed by the histological examination of a skin biopsy taken on the evolving border of the lesion and showed polynuclear neutrophilic infiltration without vasculitis. Direct immunofluorescence was negative. The presence of serum anti-myeloperoxydase p-ANCA was known for this patient since October 2002. No IgA monoclonal gammapathy was revealed on extensive biological check-up. Systemic oral corticosteroid therapy (1 mg/kg/day) dramatically improved skin lesions with complete healing within 8 weeks.Propylthiouracil is well known to induce the occurrence of ANCA in 20 to 64p. 100 of patients treated for Graves'disease. The mechanisms involved are badly recognized so far. Cutaneous vasculitis, glomerulonephritis and polychondritis may be clinically associated with those antibodies. Rare observations of neutrophilic dermatosis, mostly Sweet's syndrome, have been described in patients with propylthiouracil-induced ANCA. One case-report described a 44 year-old woman who developed pyoderma gangrenosum associated with propylthiouracil-induced p-ANCA. These manifestations usually appear within 2 years, as our patient. The data in the literature, allows us to report the polymorphic expressions of neutrophilic dermatosis in this patient with p-ANCA which could be related to propylthiouracil. Such association of Sneddon-Wilkinson subcorneal pustulosis and pyoderma gangrenosum with p-ANCA has never been described in this endocrinologic context so far. Furthermore we propose that neutrophilic dermatosis should be inscribed in the list of side effects induced by propylthiouracil therapy.

Congenital myasthenic syndromes (CMS) are genetic diseases characterized by dysfunctional neuromuscular transmission and usually start during the neonatal period. Most are due to postsynaptic abnormalities, specifically to mutations in the acetylcholine receptor (AChR) genes. In 2002, the group of A Engel reported the first cases of CMS with mutations in the gene coding rapsyn, a postsynaptic molecule which stabilizes AChR aggregates at the neuromuscular junction. Since this first publication, more than 30 other cases, including six in France, have been reported. Study of these published cases allows us to distinguish three classes of phenotypes: 1) severe neonatal cases; 2) more benign cases, starting during infancy; 3) cases with facial malformations, involving Jewish patients originating from the Near-East. Comparison of the observations of other groups with our own has led us to the following conclusions: the N88K mutation is frequent (homozygous in 50% of cases); besides the N88K mutation, the second mutation varies considerably; heterozygous allelic cases (N88K + another mutation) are severe; there is probably a founder effect in the European population. There is phenotypic variability in the homozygous N88K cases, with benign cases and severe cases of early expression. A Engel and colleagues report that the seven cases of benign CMS with facial malformation, previously described in the Jewish population of Iraq and Iran, were caused by mutation in the promoter region of the rapsyn gene.

The Bardet-Biedl syndrome is a rare autosomal recessive disorder, which associates obesity, pigmentary retinopathy, hexadactyly, hypogenitalism, renal dysfunction and mental retardation. Other abnormalities can be observed in the Bardet-Biedl syndrome, but few cutaneous abnormalities have been described.A 41 year-old woman, suffering from a Bardet-Biedl syndrome diagnosed when she was 7 Years old, presented with an atypical pseudo verruca-like, dark red lesion of the interbuttock area that had developed over fifteen Years and had become a handicap. The histological examination revealed a double component: epithelial, papillomatous and acanthosic on the one hand and vascular and lymphatic on the other, suggesting a lymphangioma with epidermal hyperplasia. Magnetic resonance imaging of the sacral area revealed a median subcutaneous lesion, extending deeply to the third coccygial vertebra.Such a lymphangioma is unusual. Because it occurred during a rare polymalformative syndrome, we suggest that it may represent a new clinical sign that can be observed during the Bardet-Biedl syndrome.

This is the first part of a study of the quark propagator and the vertex function of the vector current on the lattice in the Landau gauge and using both Wilson-clover and overlap actions. In order to be able to identify lattice artifacts and to reach large momenta we use a range of lattice spacings. The lattice artifacts turn out to be exceedingly large in this study. We present a new and very efficient method to eliminate the hypercubic (anisotropy) artifacts based on a systematic expansion on hypercubic invariants which are not SO(4) invariant. A simpler version of this method has been used in previous works. This method is shown to be significantly more efficient than the popular ``democratic'' methods. It can of course be applied to the lattice simulations of many other physical quantities. The analysis indicates a hierarchy in the size of hypercubic artifacts: overlap larger than clover and propagator larger than vertex function. This pleads for the combined study of propagators and vertex functions via Ward identities., 14 pags., 9 figs

Among the filamentous fungi of the Hyphomycete class, Annellospore group, the Scopulariopsis genera presently includes 16 species considered as opportunist pathogens. We report some unusual cutaneous manifestations due to Scopulariopsis brevicaulis in an immunodepressed patient.A 67 year-old man presented with a lymphomatous form of Waldenström's macroglobulinemia. During chemotherapy with medullar aplasia, inflammatory cutaneous nodules appeared on the forearm and leg. Microbiological and histological examinations identified Scopulariopsis brevicaulis. The patient improved with rapid remission from aplasia using hematopoietic growth factors and treatment with itraconazole.In a patient in medullar aplasia, the rapid onset of an extra-ungual cutaneous localization (forearm and leg) of a Scopulariopsis brevicaulis infection, without systemic involvement, suggests that the cutaneous inoculation occurred through scratching (from the nails) or from another cutaneous origin. Prolonged neutropenia enhanced the pathogenicity of this mycosis. The mycological and histological examinations are fundamental to confirm the diagnosis.

Neonatal lupus is rare and cutaneous lesions are usually suggestive of the diagnosis. We describe the case an infant with atypical clinical and histological aspects.A female newborn, 6 weeks of life, presented since 3 weeks a papulo-erythematous eruption involving the trunk and the 4 limbs. Cutaneous biopsy showed a dermal infiltrate of mononuclear cells, which corresponded morphologically to lymphocytes. But myelo-monocytic origin was proved by immunostaining. This result could be seen in hematodermia and macrophagic activation syndrome, but we had no clinical sign for these diagnosis. Later, she presented an erythema of the eyelids and erythematous papules of the face leading to suspicion of neonatal lupus. Questioning the mother revealed that she had Raynaud's syndrome since 1995. Antinuclear factors were positive in the patient and her mother, corresponding to antiSSA and antiSSB antibodies. She exhibited cytolytic hepatitis but no congenital heart block. Direct immunofluorescence was negative. The clinical evolution was good with complete clearing of the eruption at the age of 3 months. At 10 months, clinical and biological examinations were normal, with no arguments for haematological disease.This observation is original because of the initial papulo-erythematous eruption which is rare in neonatal lupus. Diagnosis was confirmed when specific secondary cutaneous lesions appeared. The biopsy of the first eruption showed a myelo-monocytic infiltrate which has never been described in neonatal lupus. However cutaneous biopsies are rarely performed in neonatal lupus and immunostaining is lacking in the literature.

Gluon two- and three-point Green Functions computed in Landau gauge from the lattice show the existence of power corrections to the purely perturbative expressions, that can be explained through an Operator Product Expansion as the influence of a non gauge invariant mass dimension two condensate. The relationship of this condensate with topological properties of QCD, namely instantons, will be studied, giving a first estimate of the contribution of instantons to this condensate based in the direct lattice measure, after a cooling process, of the instanton liquid properties. We give a more transparent understanding of the vacuum expectation value of the renormalized local operator ${A}^{2}$ by relating it to the gluon propagator integrated over the momentum. The quadratically divergent perturbative contribution is subtracted and the remainder, dominantly due to the ${O(1/p}^{2})$ correction to the perturbative propagator at large ${p}^{2}$ is logarithmically divergent. This provides a transparent derivation of the fact that this ${O(1/p}^{2})$ term is related to the vacuum expectation value of the local ${A}^{2}$ operator and confirms a previous claim based on the operator product expansion (OPE) of the gluon propagator. At leading logarithms the agreement is quantitative, with a standard running factor, between the local ${A}^{2}$ condensate renormalized as described above and the one renormalized in the OPE context. This result supports the claim that the BRST invariant Landau-gauge ${A}^{2}$ condensate might play an important role in describing the QCD vacuum.

We present the results of the first lattice QCD study of the strong coupling g_{D^* D pi}. From our simulations in the quenched approximation, we obtain g_{D^* D pi} = 18.8 +/- 2.3^{+1.1}_{-2.0} and hat(g)_c = 0.67 +/- 0.08^{+0.04}_{-0.06}. Whereas previous theoretical studies gave different predictions, our result favours a large value for hat(g)_c. It agrees very well with the recent experimental value by CLEO. hat(g) varies very little with the heavy mass and we find in the infinite mass limit hat(g)_infinity = 0.69(18).

Systemic vasculitis presenting as a tumorlike lesion are unusual and affect mainly the uro-genital system and breast. Muscle-localized tumorlike periarteritis nodosa are scarce.We report localized tumorlike polyarteritis nodosa affecting the forearm muscles in a 90-year-old woman.Other cases of polyarteritis nodosa localized to muscles are reviewed. This limited form has a benign clinical course and responds promptly to treatment with corticosteroids alone but relapses may occur.

We argue that the $〈{A}_{\mathrm{OPE}}^{2}〉$ condensate found in the Landau gauge on lattices, when an operator product expansion of Green's functions is performed, might be explained by instantons. We use cooling to estimate the instanton contribution and extrapolate back the result to the thermalized configuration. The resulting $〈{A}_{\mathrm{inst}}^{2}〉$ is similar to $〈{A}_{\mathrm{OPE}}^{2}〉.$

We report two patients with Dowling-Meara type epidermolysis bullosa simplex: who had trumpet nail deformity.Two 75 and 72 year-old sisters, had flare-ups of epidermolysis bullosa simplex since childhood. The aspect of the lesions and electron microscopy were in favour of the diagnosis of the Dowling-Meara variant of epidermolysis bullosa simplex. Both women had several fingernails with pincer and trumpet nail deformities. There were no such signs among the other siblings, who had never had epidermolysis.Nail involvement is not rare during the course of superficial epidermolysis bullosa. The trumpet lesions appear to be relatively specific to the present two cases of epidermolysis bullosa simplex. The electron microscopic findings of these two cases were also noteworthy, with whisk-like but not round clumping of the perinuclear tonofilaments.
