Locally advanced pancreatic cancer (LAPC) has several definitions but essentially is a nonmetastasized pancreatic cancer, in which upfront resection is considered not beneficial due to extensive vascular involvement and consequent high chance of a nonradical resection. The introduction of FOLFIRINOX chemotherapy and gemcitabine-nab-paclitaxel (gem-nab) has had major implications for the management and outcome of patients with LAPC. After 4–6 months induction chemotherapy, the majority of patients have stable disease or even tumor-regression. Of these, 12 to 35% are successfully downstaged to resectable disease. Several studies have reported a 30–35 months overall survival after resection; although it currently remains unclear if this is a result of the resection or the good response to chemotherapy. Following chemotherapy, selection of patients for resection is difficult, as contrast-enhanced computed-tomography (CT) scan is unreliable in differentiating between viable tumor and fibrosis. In case a resection is not considered possible but stable disease is observed, local ablative techniques are being studied, such as irreversible electroporation, radiofrequency ablation, and stereotactic body radiation therapy. Pragmatic, multicenter, randomized studies will ultimately have to confirm the exact role of both surgical exploration and ablation in these patients. Since evidence-based guidelines for the management of LAPC are lacking, this review proposes a standardized approach for the treatment of LAPC based on the best available evidence.

The aim of the study was to compare two methods of long-term ECG monitoring after atrial fibrillation (AF) ablation.The study included 279 patients with paroxysmal (n = 163) or persistent (n = 116) AF, who were followed up for 1 year after the first catheter ablation for AF. All patients were provided with episodic ECG recorder for 1 year and instructed to send at least 2 random ECG recordings per day and whenever they sensed symptoms. They were also provided with external loop recorder for 2-3 weeks at 6 and 12 months to enable more continuous ECG monitoring.At the end of 39 ± 12 (12-60) month follow-up, 143 (88%) patients with paroxysmal AF, and 105 (91%) patients with persistent AF, respectively, remained in stable sinus rhythm. The efficacy of episodic recorder expressed as a ratio of patients with identified AF/atrial tachycardia (AT) out of all patients with a documented episode of AF/AT was superior to the efficacy of loop recorder. No AF/AT episode was recorded with the loop recorder that was not also documented by the episodic recorder. Of the patients with the AF/AT recurrence detected by the episodic recorder, the arrhythmia was also revealed by the loop recorder in 70% of the patients after paroxysmal AF ablation (p0.001), and in 52% of the patients after persistent AF ablation (p = 0.006). Asymptomatic AF/AT was present in 42 (26%) of patients with paroxysmal AF, and in 28 (24%) patients with persistent AF (p = 0.810). From conventional follow-up unexpected and clinically significant episode of AF/AT that required change in therapy was detected in 9 (5.5%) patients after ablation for paroxysmal AF, and in 4 (3.4%) patients after ablation for persistent FS.More episodes of AF/AT after ablation were detected by 1-year daily ECG monitoring using episodic recorder as compared to periodic ECG monitoring with loop recorder. Identification of clinically significant episodes of AF/AT unrecognized from conventional follow-up was low.

The magnetic resonance imaging (MRI) of the rat brain after the epilepsy seizures has been performed. As a first step, the model of the kainic acid (KA) induced seizures has been conducted to examine the possibilities of magnetic resonance imaging system kept in disposition. Seven Wistar albino rats, weighing about 300 g, were used in this study. We administered six of them with intraperitoneal injection of 10 mg/kg of KA. The control animal received corresponding volume of the saline. Every animal was examined under systemic anaesthesia induced by an intraperitoneal injection of thiopental sodium approximately 15 minutes before scanning. Diffusion-weighted imaging (DWI) has been used to acquire the coronary scans of the rat brain. The progress of hyper intense signal at the cerebral cortex and amygdale has been observed. Marked asymmetry of the signal intensity between hemispheres has been discovered. Subsequently the experimental model of audiogenic epilepsy will be conducted.

Dentists may want to consider using an update of a unique complete denture technique that saves total chair time and, therefore, decreases cost. The technique combines appointments two and three, during which time the dentist both makes the master impressions and records the maxillomandibular relationships. In the past, protection of the maxillomandibular relationship records during the subsequent laboratory work was a problem. By applying new, tough, and thin denture base materials to the technique, the dentist will be less likely to distort the maxillomandibular records while boxing and pouring the impressions.

A case of nasopalatine duct cyst is described. Such cysts often remain asymptomatic for lengthy periods and may be discovered during routine radiography. The quiescent nature of this cyst underscores the importance of thorough clinical and radiographic examination before the fabrication of a prosthesis, as such cysts may give rise to acute infection, enlargement, and penetration through the overlying mucosa if not removed before denture insertion.

Application of a direct electric field (of the order of 104Vcm) causes a temporary increase of the luminescence of ZnS crystals (“Gudden-Pohl effect”). It is demonstrated that the light pulse is always accompanied by a current pulse. Studies of the effect showed that both pulses appear only if the crystal has been optically excited. The sizes and shapes of the pulses were systematically studied during the excitation, afterglow and thermal glow. Kinetic equations were set up, governing the free and trapped electron concentrations in the crystal. Most coefficients of these equations were experimentally determined by conventional luminescence and photoconductivity methods. Three different ways by which the field might cause release of electrons from traps, were considered: (a) tunneling from traps; (b) diminishing the effective trap depths by the field and thereby enhancing thermal release; (c) impact ionization of traps by accelerated conduction-band electrons. The kinetic equations showed that with either of these mechanisms current and light pulses appear, as observed in the experiments. However, tunneling (a) would yield completely negligible effects at even much higher fields than actually applied and is in disagreement with the experimental results on the dependence of the pulse heights on the state of excitation of the crystal. Mechanism (b) can account for the order of magnitude of the effects observed only if local fields, higher than the average one by a factor of about twenty exist in the crystal. It does not explain the observed behaviour of the pulses during the afterglow. Impact ionization (c) is in complete accord with the results on the dependence of the pulse heights on the state of excitation of the crystal but it also seems to need somewhat higher fields than the average fields applied.

- Abstract
Aims: Our aim was to evaluate the potential for safely imaging patients with a new type of implantable cardioverter-defibrillator called the subcutaneous implantable cardioverter-defibrillator (S-ICD) in a 1.5 T magnetic resonance imaging (MRI) scanner. With the increasing number of patients with cardiac implantable devices who are indicated for MRI, there is a growing need for establishing MRI compatibility of cardiac implantable devices., Methods and Results: Patients with implanted S-ICD systems underwent one or more types of anatomical MRI scans. The S-ICD was programmed off and patients were monitored throughout the imaging procedure. Device function was evaluated pre- and post-scan. Patients were asked to report immediately any pain, torqueing movement, or heating sensation in the area of the pocket or electrode. Fifteen patients underwent a total of 22 examinations at 1.5 T. Scans included brain, spine, knee, and heart. Two patients were re-scanned due to complaints of heating over the can during lumbar scans, which was caused by a thermistor probe placed on the skin to measure skin temperature. All the remaining scans occurred without incident. No evidence of device malfunction was observed., Conclusion: This study is the first to domonstrate the feasibility of exposing S-ICD patients to MRI using the scanning and monitoring protocol described. - Abstract
Aim: The aim of the study was to compare two methods of long-term ECG monitoring after atrial fibrillation (AF) ablation., Methods: The study included 279 patients with paroxysmal (n = 163) or persistent (n = 116) AF, who were followed up for 1 year after the first catheter ablation for AF. All patients were provided with episodic ECG recorder for 1 year and instructed to send at least 2 random ECG recordings per day and whenever they sensed symptoms. They were also provided with external loop recorder for 2-3 weeks at 6 and 12 months to enable more continuous ECG monitoring., Results: At the end of 39 ± 12 (12-60) month follow-up, 143 (88%) patients with paroxysmal AF, and 105 (91%) patients with persistent AF, respectively, remained in stable sinus rhythm. The efficacy of episodic recorder expressed as a ratio of patients with identified AF/atrial tachycardia (AT) out of all patients with a documented episode of AF/AT was superior to the efficacy of loop recorder. No AF/AT episode was recorded with the loop recorder that was not also documented by the episodic recorder. Of the patients with the AF/AT recurrence detected by the episodic recorder, the arrhythmia was also revealed by the loop recorder in 70% of the patients after paroxysmal AF ablation (p < 0.001), and in 52% of the patients after persistent AF ablation (p = 0.006). Asymptomatic AF/AT was present in 42 (26%) of patients with paroxysmal AF, and in 28 (24%) patients with persistent AF (p = 0.810). From conventional follow-up unexpected and clinically significant episode of AF/AT that required change in therapy was detected in 9 (5.5%) patients after ablation for paroxysmal AF, and in 4 (3.4%) patients after ablation for persistent FS., Conclusion: More episodes of AF/AT after ablation were detected by 1-year daily ECG monitoring using episodic recorder as compared to periodic ECG monitoring with loop recorder. Identification of clinically significant episodes of AF/AT unrecognized from conventional follow-up was low.

- Abstract
The magnetic resonance imaging (MRI) of the rat brain after the epilepsy seizures has been performed. As a first step, the model of the kainic acid (KA) induced seizures has been conducted to examine the possibilities of magnetic resonance imaging system kept in disposition. Seven Wistar albino rats, weighing about 300 g, were used in this study. We administered six of them with intraperitoneal injection of 10 mg/kg of KA. The control animal received corresponding volume of the saline. Every animal was examined under systemic anaesthesia induced by an intraperitoneal injection of thiopental sodium approximately 15 minutes before scanning. Diffusion-weighted imaging (DWI) has been used to acquire the coronary scans of the rat brain. The progress of hyper intense signal at the cerebral cortex and amygdale has been observed. Marked asymmetry of the signal intensity between hemispheres has been discovered. Subsequently the experimental model of audiogenic epilepsy will be conducted.
