Resumen: Introducción: El objetivo del proyecto EPICON es desarrollar una serie de recomendaciones sobre la forma adecuada de realizar el cambio de carbamazepina (CBZ) y oxcarbazepina (OXC) a acetato de eslicarbazepina (ESL) en determinados pacientes con epilepsia. Métodos: Un comité coordinador preparó un cuestionario con 56 preguntas en relación con el cambio de CBZ u OXC a ESL en la práctica clínica (metodología y situaciones del cambio). Posteriormente, se consultó a 54 expertos en epilepsia con el empleo de metodología Delphi (2 rondas de consulta). Se definió un consenso en acuerdo o desacuerdo si las respuestas para el ítem estudiado alcanzaban una mediana ≥ 7 o ≤ 3, respectivamente, y un rango intercuartílico relativo ≤ 0,40. Se analizaron los resultados y se formularon las conclusiones. Resultados: Las recomendaciones fundamentales fueron: el cambio de CBZ a ESL debe ser realizado en 1-3 semanas, con una equivalencia de dosis CBZ:ESL de 1:1.3, siendo recomendado en pacientes con olvidos de medicación, trabajos por turnos, polimedicados, problemas cognitivos, osteoporosis-osteopenia severa, dislipidemia o enfermedad hepática (ausencia de fallo hepático grave), así como en varones con disfunción eréctil causada por CBZ. El cambio de OXC a ESL puede realizarse de un día para otro con una equivalencia de dosis 1:1 y es recomendado en pacientes con olvidos de medicación, trabajos por turnos, polimedicados o problemas cognitivos. Se desaconsejó el cambio en caso de rash con CBZ u OXC. Conclusión: El proyecto EPICON proporciona algunas recomendaciones sobre el manejo clínico del cambio de CBZ u OXC a ESL, mediante el empleo de la metodología Delphi. Introduction: Prognosis in epilepsy refers to the probability of either achieving seizure remission (SR), whether spontaneously or using antiepileptic drugs (AED), or failing to achieve control of epileptic seizures (ES) despite appropriate treatment.Use of AED is recommended after a second unprovoked ES. For a first episode, the decision of whether or not to start drug treatment depends on the risk of recurrence and the advantages or disadvantages of the antiepileptic drug. The main goal of treatment is achieving absence of ES without adverse effects (AE). AED is selected according to epilepsy type and the demographic and clinical characteristics of the patient. Development: A PubMed search located articles and recommendations by the most relevant scientific societies and clinical practice guidelines concerning epilepsy prognosis and treatment. Evidence and recommendations are classified according to the prognostic criteria of the Oxford Centre for Evidence-Based Medicine (2001) and the European Federation of Neurological Societies (2004) for therapeutic actions. Conclusions: Most newly diagnosed epileptic patients achieve good control over their ES. The majority of the AEDs available at present provide effective control over all types of ES, and choice therefore depends on the patient's individual characteristics. Treatment should be initiated as monotherapy at the lowest effective dose, which in half of all patients provides ES control and is well tolerated. In cases in which the first AED is not effective, alternative therapy should be started, and monotherapy should be employed before combination therapy where possible. The probability of achieving good control over ES decreases with each successive treatment failure. Conclusiones: La mayoría de pacientes que inician una epilepsia consigue el control de sus CE. La mayoría de los FAE disponibles son útiles para el control de cualquier tipo de CE, su elección depende de las características del paciente. Se debe iniciar el tratamiento en monoterapia y a la menor dosis eficaz del FAE elegido, que suele controlar las CE en la mitad de los pacientes y con buena tolerancia. Ante la falta de eficacia del primer FAE, debe intentarse otra terapia alternativa, a ser posible en monoterapia, antes de instaurar una politerapia. Las posibilidades de control de las CE disminuyen con sucesivos fracasos terapéuticos. Objetivo: Evaluar el efecto de la inflamación en yeyuno inducida por LPS de E. coli sobre la expresión molecular de P β D-2 en lechones posdestete. Materiales y métodos: el trabajo de campo se realizó en el Centro de Producción San Pablo de la Universidad Nacional de Colombia, sede Medellín. El estudio se realizó con 32 cerdos destetados a los 21 días de edad, los cuales fueron alimentados durante 10 días con una dieta basal que tuvo como componentes leche y algunos de sus derivados, y que además cumplía con todos los mínimos nutricionales. Los cerdos fueron sacrificados escalonadamente los días 1, 5, 7 y 10 posdestete y se les extrajo completamente el yeyuno. Para inducir la inflamación intestinal los animales fueron alimentados con la dieta basal adicionada con 0.3 μg de LPS/mg de alimento. Objetivos: evaluar los cambios en la actividad enzimática intestinal de algunos cerdosdurante varios periodos posdestete. Materiales y métodos: el trabajo de campo serealizó en el Centro San Pablo, perteneciente a la Universidad Nacional de Colombia,sede Medellín. El estudio se realizó con 16 cerdos destetados a los 21 días de edad. Losanimales experimentales fueron alimentados durante 10 días con una dieta basal (DB)que tuvo como componentes leche y algunos de sus derivados y que además cumplíacon los requerimientos nutricionales. Luego, los cerdos fueron sacrificados escalonadamentelos días uno, cinco, siete y diez posdestete, y se realizó extracción completa delintestino delgado, el cual fue dividido en dos secciones (duodeno y yeyuno) de igualtamaño. El análisis de la actividad enzimática (APN, DPP-IV, LPH, MGA y SI) se realizómediante el método de velocidades iniciales y el diseño estadístico que se empleófue completamente al azar, no obstante, los datos se analizaron mediante la aplicaciónde los modelos lineales generales (GLM) y de la prueba de duncan. Resultados:se evidencia que las enzimas presentaron diferencias (P < 0,01) durante los periodosposdestete. En el día cinco se observaron cambios negativos en todas las enzimas encomparación con su estado durante el primer día posdestete; sin embargo, entre el díauno y diez posdestete se presentaron diferencias entre APN, DPP-IV y LPH (P < 0,01).Además, se encontró que la sección proximal (duodeno) presentó las mayores actividadesenzimáticas (P < 0,01). Conclusiones: el destete temprano disminuye la actividadenzimática a nivel intestinal, provocando la subutilización de los nutrientes de la dietay, por ende, alta incidencia de diarreas durante esta etapa.

Resumen: Introducción: En este artículo comunicamos resultados de adherencia a los 2 años en la subpoblación de pacientes del estudio Global de Adherencia GAP (n = 2.648) en España. Métodos: Pacientes y neurólogos completaron cuestionarios anuales para evaluar la adherencia a los tratamientos inmunomoduladores (IMA). Se definió la falta de adherencia como la pérdida de una inyección o el cambio de dosis en las últimas 4 semanas previas a completar el cuestionario. Los pacientes firmaron el consentimiento informado. Estudio aprobado en 15 de 18 centros en los seguimientos anuales de la visita 1 (V1) y la visita 2 (V2) por los comités éticos. Resultados: Se incluyó a 254 pacientes en España; la media de edad fue 37,9 años y el 70,4% eran mujeres; la mediana de tiempo en tratamiento fue 28 meses y la tasa de adherencia conjunta, del 85,4%. Los pacientes en tratamiento con interferón beta (IFNB)-1a intramuscular (Avonex®) fueron significativamente más cumplidores (96,4%) que los pacientes tratados con IFNB-1a 22 μg subcutáneo (s.c.) (Rebif®22) (79,1%; p = 0,0064), IFNB-1a 44 μg s.c. (Rebif®44) (79,6%; p = 0,0064) y acetato de glatiramero (Copaxone®) (82,7%; p = 0,0184). En V1 (n = 142), la tasa de adherencia fue del 86,6%; los pacientes con Avonex® fueron significativamente más cumplidores que aquellos con Rebif®22 (el 93,9 frente al 66,7%; p = 0,0251). En V2 (n = 131), la tasa de adherencia fue del 82,4% (Avonex®, 87,5%; Rebif®22, 80%; Rebif®44, 77,8%; Betaferon®, 85,2%, y Copaxone®, 80%) sin diferencias significativas. Conclusiones: La adherencia permaneció alta los 2 años observados. Avonex® mostró mayor adherencia frente al resto; esta diferencia fue significativa en la visita basal, tras 40,5 meses de media en tratamiento y en V1 frente a Rebif®22. Se presenta el caso de un osteoblastoma recidivante del hueso ganchoso y se revisa el tratamiento de esta lesión. Paciente varón joven con dolor y tumefacción en el dorso de la mano de seis meses de evolución sin respuesta al tratamiento sintomático. A la exploración radiológica se apreció una tumoración lítica del hueso ganchoso, y la RM mostró una tumoración con edema. Por biopsia incisional se diagnóstico que se trataba de un osteoblastoma, por lo que fue intervenido para practicar curetaje intralesional con aporte de esponjosa de cresta ilíaca autóloga. A los seis meses se observó una recidiva local, por lo que fue reintervenido mediante resección en bloque, con resultado final satisfactorio.

Summary: Biochar obtained via microwave-assisted pyrolysis (MAP) at 720 W and 15 min from cocoa pod husk (CPH) is an efficient adsorbent of Cd2+(aq). Biochar of residual biomass of CPH (BCCPH) possesses favorable physicochemical and morphological properties, featuring a modest surface area yet a suitable porous structure. Adsorption, predominantly governed by physisorption, is influenced by the oxygen-containing active sites (-COOR, -C(R)O, and -CH2OR; R = H, alkyl). CdCO3 formation occurs during adsorption. Experimental data were well-fitted into various kinetic models for a broad understanding of the sorption process. Langmuir model indicates a maximum adsorption capacity of 14.694 mg/g. The thermodynamic study confirms the spontaneous and endothermic sorption. Studies at the molecular level have revealed that the Cd2+ ion tends to bind to surface aromatic carbon atoms. This sustainable approach produces BCCPH via MAP as a solution for waste transformation into water-cleaning materials.

Vacuolar ATP-dependent proton pumps (V-ATPases) belong to a super-family of rotary ATPases and ATP synthases. The V1 complex consumes ATP to drive rotation of a central rotor that pumps protons across membranes via the Vo complex. Eukaryotic V-ATPases are regulated by reversible disassembly of subunit C, V1 without C, and VO. ATP hydrolysis is thought to generate an unknown rotary state that initiates regulated disassembly. Dissociated V1 is inhibited by subunit H that traps it in a specific rotational position. Here, we report the first single-molecule studies with high resolution of time and rotational position of Saccharomyces cerevisiae V1-ATPase lacking subunits H and C (V1ΔHC), which resolves previously elusive dwells and angular velocity changes. Rotation occurred in 120° power strokes separated by dwells comparable to catalytic dwells observed in other rotary ATPases. However, unique V1ΔHC rotational features included: 1) faltering power stroke rotation during the first 60°; 2) a dwell often occurring ∼45° after the catalytic dwell, which did not increase in duration at limiting MgATP; 3) a second dwell, ∼2-fold longer occurring 112° that increased in duration and occurrence at limiting MgATP; 4) limiting MgATP-dependent decreases in power stroke angular velocity where dwells were not observed. The results presented here are consistent with MgATP binding to the empty catalytic site at 112° and MgADP released at ∼45°, and provide important new insight concerning the molecular basis for the differences in rotary positions of substrate binding and product release between V-type and F-type ATPases.

Introduction: Teratomas are most frequently located in the thoracic anterior mediastinum. Case presentation: A 26-year-old female patient who was admitted after respiratory distress was protocolized with a tomography that reported data of cyst tumor involving the upper left pulmonary lobe, a thoracotomy and tumor resection were performed, the pathology report mentioning data from mature cystic teratoma. The presence of pulmonary teratoma has a few reports in the world literature, the thomography provides suggestive findings of cystic tumor and allows surgical planning for its resection. Conclusion: Pulmonary teratoma is a rare and infrequent entity, its finding requires surgical resection due to the risk of rupture and malignancy.
