1. Analyzing big tobacco’s global youth marketing strategies and factors influencing smoking initiation by Nigeria youths using the theory of triadic influence

Abstract

Abstract Background Major transnational tobacco companies (TTCs) have identified Nigeria, the seventh most populous country in the world, as a market with a significant revenue potential, given its high youth population and growing gross domestic product (GDP). This research analyses tobacco industry-related strategies and activities targeting youth (aged 15 to 24 years) in Nigeria involving existing, but most importantly, future tobacco users. Nigeria is the focus of this study because the tobacco industry has viewed it as a major emerging market since the 1990s. Successful marketing in Nigeria could provide the industry with a template for similar initiatives in other emerging markets in low- and middle-income countries. Methods The research began with a systematic review of secondary literature to determine how the tobacco industry has targeted youth globally and factors contributing to youth smoking initiation. It then used the theory of triadic influence as a heuristic framework to categorize the various factors influencing youth smoking initiation and industry strategies related to increasing tobacco use among youths. Quotations from internal tobacco industry documents were organized into the three streams of the theory of triadic influence: biology/personality, social and cultural/environmental streams. A total of 12 interviews were conducted with 6 policymakers and governmental officials, 2 civil society organization representatives, a high school principal, a journalist and 2 researchers to investigate how the tobacco industry had targeted youth in Nigeria. Results The findings indicate that TTCs have actively targeted youth in Nigeria since the 1990s, focusing on changing behaviour through the biology/personality, social and environmental/ cultural streams. Conclusion The study suggests that Nigeria implement and vigorously enforce its 2015 National Tobacco Control Bill as well as a package of other measures to prevent tobacco companies targeting youth.

2. Intravenous injection of extracellular vesicles to treat chronic myocardial ischemia

Abstract

Background Mesenchymal stem cell-derived extracellular vesicles (EVs) appear to be a very exciting treatment option for heart disease. Here, we used a swine model of chronic myocardial ischemia to evaluate the efficacy of a less-invasive method of injection of EVs via a peripheral intravenous route. Methods Sixteen Yorkshire swine underwent placement of an ameroid constrictor on the left circumflex (LCx) artery at age 11 weeks to induce chronic myocardial ischemia. Two weeks later, they were divided into two groups: control (CON; n = 8), and intravenous injection of EVs (EVIV; n = 8). At 18 weeks of age, animals underwent final analysis and euthanasia. The chronically ischemic myocardium (LCx territory) was harvested for analysis. Results Intravenous injection (IV) of EVs induced several pro-angiogenic markers such as MAPK, JNK but not Akt. Whereas IV injections of EVs decreased VEGFR2 expression and inhibited apoptotic signaling (caspase 3), they increased expression of VEGFR1 that is believed to be anti-angiogenic. Injection of EVs did not result in an increase in vessel density and blood flow when compared to the control group. Conclusions Although IV injection of EVs upregulated several pro-angiogenic signaling pathways, it failed to induce changes in vascular density in the chronically ischemic myocardium. Thus, a lack of increase in vascular density at the doses tested failed to elicit a functional response in ischemic myocardium.

Abstract

BACKGROUND: Patients with peripheral artery disease have an increased risk of cardiovascular morbidity and mortality. Antiplatelet agents are widely used to reduce these complications.METHODS: This was a multicentre, double-blind, randomised placebo-controlled trial for which patients were recruited at 602 hospitals, clinics, or community practices from 33 countries across six continents. Eligible patients had a history of peripheral artery disease of the lower extremities (previous peripheral bypass surgery or angioplasty, limb or foot amputation, intermittent claudication with objective evidence of peripheral artery disease), of the carotid arteries (previous carotid artery revascularisation or asymptomatic carotid artery stenosis of at least 50%), or coronary artery disease with an ankle-brachial index of less than 0·90. After a 30-day run-in period, patients were randomly assigned (1:1:1) to receive oral rivaroxaban (2·5 mg twice a day) plus aspirin (100 mg once a day), rivaroxaban twice a day (5 mg with aspirin placebo once a day), or to aspirin once a day (100 mg and rivaroxaban placebo twice a day). Randomisation was computer generated. Each treatment group was double dummy, and the patient, investigators, and central study staff were masked to treatment allocation. The primary outcome was cardiovascular death, myocardial infarction or stroke; the primary peripheral artery disease outcome was major adverse limb events including major amputation. This trial is registered with ClinicalTrials.gov, number NCT01776424, and is closed to new participants.FINDINGS: Between March 12, 2013, and May 10, 2016, we enrolled 7470 patients with peripheral artery disease from 558 centres. The combination of rivaroxaban plus aspirin compared with aspirin alone reduced the composite endpoint of cardiovascular death, myocardial infarction, or stroke (126 [5%] of 2492 vs 174 [7%] of 2504; hazard ratio [HR] 0·72, 95% CI 0·57-0·90, p=0·0047), and major adverse limb events including major amputation (32 [1%] vs 60 [2%]; HR 0·54 95% CI 0·35-0·82, p=0·0037). Rivaroxaban 5 mg twice a day compared with aspirin alone did not significantly reduce the composite endpoint (149 [6%] of 2474 vs 174 [7%] of 2504; HR 0·86, 95% CI 0·69-1·08, p=0·19), but reduced major adverse limb events including major amputation (40 [2%] vs 60 [2%]; HR 0·67, 95% CI 0·45-1·00, p=0·05). The median duration of treatment was 21 months. The use of the rivaroxaban plus aspirin combination increased major bleeding compared with the aspirin alone group (77 [3%] of 2492 vs 48 [2%] of 2504; HR 1·61, 95% CI 1·12-2·31, p=0·0089), which was mainly gastrointestinal. Similarly, major bleeding occurred in 79 (3%) of 2474 patients with rivaroxaban 5 mg, and in 48 (2%) of 2504 in the aspirin alone group (HR 1·68, 95% CI 1·17-2·40; p=0·0043).INTERPRETATION: Low-dose rivaroxaban taken twice a day plus aspirin once a day reduced major adverse cardiovascular and limb events when compared with aspirin alone. Although major bleeding was increased, fatal or critical organ bleeding was not. This combination therapy represents an important advance in the management of patients with peripheral artery disease. Rivaroxaban alone did not significantly reduce major adverse cardiovascular events compared with asprin alone, but reduced major adverse limb events and increased major bleeding.FUNDING: Bayer AG.

Abstract

Background Major transnational tobacco companies (TTCs) have identified Nigeria, the seventh most populous country in the world, as a market with a significant revenue potential, given its high youth population and growing gross domestic product (GDP). This research analyses tobacco industry-related strategies and activities targeting youth (aged 15 to 24 years) in Nigeria involving existing, but most importantly, future tobacco users. Nigeria is the focus of this study because the tobacco industry has viewed it as a major emerging market since the 1990s. Successful marketing in Nigeria could provide the industry with a template for similar initiatives in other emerging markets in low- and middle-income countries. Methods The research began with a systematic review of secondary literature to determine how the tobacco industry has targeted youth globally and factors contributing to youth smoking initiation. It then used the theory of triadic influence as a heuristic framework to categorize the various factors influencing youth smoking initiation and industry strategies related to increasing tobacco use among youths. Quotations from internal tobacco industry documents were organized into the three streams of the theory of triadic influence: biology/personality, social and cultural/environmental streams. A total of 12 interviews were conducted with 6 policymakers and governmental officials, 2 civil society organization representatives, a high school principal, a journalist and 2 researchers to investigate how the tobacco industry had targeted youth in Nigeria. Results The findings indicate that TTCs have actively targeted youth in Nigeria since the 1990s, focusing on changing behaviour through the biology/personality, social and environmental/ cultural streams. Conclusion The study suggests that Nigeria implement and vigorously enforce its 2015 National Tobacco Control Bill as well as a package of other measures to prevent tobacco companies targeting youth.

Abstract

BACKGROUND: Coronary artery disease is a major cause of morbidity and mortality worldwide, and is a consequence of acute thrombotic events involving activation of platelets and coagulation proteins. Factor Xa inhibitors and aspirin each reduce thrombotic events but have not yet been tested in combination or against each other in patients with stable coronary artery disease.METHODS: In this multicentre, double-blind, randomised, placebo-controlled, outpatient trial, patients with stable coronary artery disease or peripheral artery disease were recruited at 602 hospitals, clinics, or community centres in 33 countries. This paper reports on patients with coronary artery disease. Eligible patients with coronary artery disease had to have had a myocardial infarction in the past 20 years, multi-vessel coronary artery disease, history of stable or unstable angina, previous multi-vessel percutaneous coronary intervention, or previous multi-vessel coronary artery bypass graft surgery. After a 30-day run in period, patients were randomly assigned (1:1:1) to receive rivaroxaban (2·5 mg orally twice a day) plus aspirin (100 mg once a day), rivaroxaban alone (5 mg orally twice a day), or aspirin alone (100 mg orally once a day). Randomisation was computer generated. Each treatment group was double dummy, and the patients, investigators, and central study staff were masked to treatment allocation. The primary outcome of the COMPASS trial was the occurrence of myocardial infarction, stroke, or cardiovascular death. This trial is registered with ClinicalTrials.gov, number NCT01776424, and is closed to new participants.FINDINGS: Between March 12, 2013, and May 10, 2016, 27 395 patients were enrolled to the COMPASS trial, of whom 24 824 patients had stable coronary artery disease from 558 centres. The combination of rivaroxaban plus aspirin reduced the primary outcome more than aspirin alone (347 [4%] of 8313 vs 460 [6%] of 8261; hazard ratio [HR] 0·74, 95% CI 0·65-0·86, pINTERPRETATION: In patients with stable coronary artery disease, addition of rivaroxaban to aspirin lowered major vascular events, but increased major bleeding. There was no significant increase in intracranial bleeding or other critical organ bleeding. There was also a significant net benefit in favour of rivaroxaban plus aspirin and deaths were reduced by 23%. Thus, addition of rivaroxaban to aspirin has the potential to substantially reduce morbidity and mortality from coronary artery disease worldwide.FUNDING: Bayer AG.

Harry Brighouse discusses two conceptions of educational equality. The first is a type of equality of opportunity, heavily influenced by the work of John Rawls, which he calls the meritocratic conception. According to this conception, an individual’s educational prospects should not be influenced by factors such as their social class background. The other, radical conception, suggests a person’s natural talents should not influence their educational prospects either. Brighouse favors the meritocratic conception, but this article argues that it is flawed and that the radical conception ought to be preferred. Although a superior conception of educational equality, the radical conception is still not quite right, so this article develops a luck egalitarian conception of educational equality. Similarly, there are few regulations or legislation pertaining to post-trial access. If regulatory changes are necessary, we need to understand the current arguments, legislation, and attitudes towards post-trial access and participants and community members. Given that clinical trials conducted in low-income countries will likely continue, there is an urgent need for consideration of post-trial benefits for participants, communities, and citizens of host countries. While this issue may not be as pressing in countries where participants have access to healthcare and medicines through public schemes, it is particularly important in regions where this may not be available.

Well known throughout the Islamic world as the foundational thinker for a significant portion of the contemporary Muslim intelligentsia, Sayyid Qutb (1906–1966) was a member of the Muslim Brotherhood and was jailed by Gamal Abdul Nasser's government in 1954. He became one of the most uncompromising voices of the movement we now call Islamism and is perhaps best known for his book, Ma`lam fi al-tariq. A Child from the Village was written just prior to Qutb's conversion to the Islamist cause and reflects his concerns for social justice. Interst in Qutb's writing has increased in the West since Islamism has emerged as a power on the world scene.In this memoir, Qutb recalls his childhood in the village of Musha in Upper Egypt. He chronicles the period between 1912 and 1918, a time immensely influential in the creation of modern Egypt. Written with much tenderness toward childhood memories, it has become a classic in modern Arabic autobiography. Qutb offers a clear picture of Egyptian village life in the early twentieth century, its customs and lore, educational system, religious festivals, relations with the central government, and the struggle to modernize and retain its identity. The basic principle of educational equality is that each child should receive an equally good education. This sounds appealing, but is rather vague and needs substantial working out. Also, educational equality faces all the objections to equality per se, plus others specific to its subject matter. Together these have eroded confidence in the viability of equality as an educational ideal. This article argues that equality of educational opportunity is not the best way of understanding educational equality. It focuses on Brighouse and Swift's well worked out meritocratic conception and finds it irretrievably flawed; they should, instead, have pursued a radical conception they only mention. This conception is used as a starting point for developing a luck egalitarian conception, pluralistic and complex in nature. It is argued that such a conception accounts for the appeal of equality of opportunity, fits with other values in education and meets many of the objections. Thus, equality is reasserted as what morally matters most in education.

It is essential that people with a learning disability and mental disorder receive the appropriate medicines in order to promote both their physical and mental health. However, medication administration errors (MAEs) are relatively frequent occurrences, and can have serious consequences. There is mixed evidence as to whether they can be reduced through a two-nurse administration system. Two-nurse administration is relatively expensive, but could be more cost-effective if the second administrator were a specially trained health care assistant (HCA) rather than a registered nurse. Such a system may also be more practicable in areas where only one registered nurse is available. This paper describes the evaluation of the introduction of an HCA observer into the medicines administration process utilising a before and after audit design. There was a significant fall in the MAE rate 9 months after the introduction of an observer, from 2.92% to 0.85% of all prescribed doses.

Sayyid Qutb (1906-1966) was an influential Egyptian ideologue credited with establishing the theoretical basis for radical Islamism in the post colonial Sunni Muslim world. Lacking a pure understanding of the leader's life and work, the popular media has conflated Qutb's moral purpose with the aims of bin Laden and al-Qaeda. He is often portrayed as a terrorist, Islamo-Fascist, and advocate of murder. This book rescues Qutb from misrepresentation, tracing the evolution of his thought within the context of his time. An expert on social protest and political resistance in the modern Middle East, as well as Egyptian nationalism, John Calvert recounts Qutb's life from the small village in which he was raised to his execution at the behest of Abd al-Nasser's regime. His study remains sensitive to the cultural, political, social, and economic circumstances that shaped Qutb's thought-major developments that composed one of the most eventful periods in Egyptian history. These years witnessed the full flush of Britain's tutelary regime, the advent of Egyptian nationalism, and the political hegemony of the Free Officers. Qutb rubbed shoulders with Taha Husayn, Naguib Mahfouz, and Abd al-Nasser himself, though his Islamism originally had little to do with religion. Only in response to his harrowing experience in prison did Qutb come to regard Islam and kufr (infidelity) as oppositional, antithetical, and therefore mutually exclusive. Calvert shows how Qutb repackaged and reformulated the Islamic heritage to pose a challenge to authority, including those who claimed (falsely, he believed) to be Muslim.

The article analyzes the images of America put forward by Sayyid Qutb (1906-1966), the prominent Egyptian ideologue of Islamism, during and immediately following his study mission to the US between 1948 and 1950. It argues that Qutb's understanding of America was conditioned by his tendency, already explicit in his 1938 critique of Taha Husayn's Mustaqbal al-Thaqafa fi Misr, to posit a cultural divide between a 'spiritual East' and a 'materialist West'. In addition to Qutb's articles and letters on the subject, many of which were published in al-Risala, the article draws upon hitherto untapped materials uncovered by the author at the University of Northern Colorado, where Qutb studied in 1949.

Abstract

One of the core configurations being considered under the EU NEWAC (NEW A ero-engine C ore concepts) programme is for an intercooled recuperative aero-engine (IRA). The IRA concept exploits the heat of the engine exhaust gas and maximises the heat pick up capacity of the combustor inlet air by intercooling in front of the high pressure compressor (HPC). The datum configuration adopted for the HPC is a single stage radial compressor, but comparative design studies into alternative configurations have also been carried out. The ducting arrangements for the HPC in the IRA engine are more complicated than in a conventional turbofan engine, with the inlet flow coming from the intercooler via a C-shaped duct and the outlet flow going to the heat exchangers in the exhaust stream via 8 pipes on the outside of the engine core. A radial compressor fits quite neatly into this configuration, but the size required for a 70,000 lbf (310kN) thrust engine is significantly larger than for existing designs. In addition, the use of intercooling reduces the flow coefficient below the optimum value. By contrast, an axial compressor is likely to achieve higher efficiency, but it will be longer and have considerably more aerofoils. Therefore, preliminary designs for axial, radial and axial-CF configurations have been set up and analysed using CFD and finite element methods to assess their aerodynamic performance and to investigate their mechanical requirements. Abstract

To compare transvaginal ultrasound measurements of endometrial thickness with direct anatomical measurements and consider the implications of these findings on clinical practice.Prospective observational study using two modalities blinded to each other's findings.Singleton Hospital, Swansea, a medium-sized District General Hospital.Forty-seven women admitted for hysterectomy.All women underwent transvaginal ultrasound scan to measure the endometrial thickness within 16 hours of surgery. Anatomical measurement of the fresh specimen was carried out immediately after surgery.Agreement between ultrasound and anatomical measurements of the endometrial thickness.No ultrasound measurement was possible in 15% of patients. When both values were obtained, transvaginal ultrasound measurements were2 mm different from the ruler measurement in 13/40 (33%) with an obvious tendency of the ultrasound scan to over-estimate the endometrial thickness. The mean difference between the two measurements was -0.8 mm (limits of agreement -7.1 to +5.5 mm). The discrepancy was greater in women with endometrial thicknessor = 5 mm (-1.6 mm, limits of agreement -5.7 to +2.6 mm) compared with that in women with endometrial thickness5 mm (-0.2 mm, limits of agreement -7.6 to +7.2 mm). Kappa statistics showed good agreement between the two measurements in discriminating between thin and thick endometrium in 77% (kappa = 0.55). Transvaginal ultrasound misdiagnosed a thick endometrium as thin in 3/40 (8%) and misdiagnosed a thin endometrium as thick in 6/40 (15%).Transvaginal ultrasonography is of limited value as a screening test for abnormal endometrium in patients with postmenopausal bleeding if the only parameter of normality is an endometrial thickness of 5 mm or less.

Abstract

A major demand of public sector unions in recent years has been for greater control over their members’ pension plans. Recently, several provincial governments, most notably British Columbia, have agreed to joint trusteeship, a development which gives union trustees a voice in investment policy.This article focuses on the implications for union trustees of investments in Public Private Partnerships (P-3s) and related privatization initiatives. Examples of such investments include: transportation infrastructure projects, hospitals and health services, schools, municipal water and sewer systems, electrical utilities, and other projects that, historically, have been within the public sector.It argues that trustees should be wary of such investments. Public sector unions have criticized privatization initiatives as a threat to public sector jobs and services. P-3 investments are problematic because they may threaten the jobs of their union’s members, undermine the credibility of their union’s public policy objections to privatization and, in the end, may prove far more risky than P-3 promoters contend.

The Construction of the $1.2 billion Vancouver Island Highway Project provided an opportunity for the building trades unions and the Government of BC to negotiate an innovative collective agreement that included union membership, training for local residents and members of equity groups, new employment opportunities for members of designated equity groups and a comprehensive health and safety program.The Project implemented the most comprehensive system of tracking progress in employment equity in BC’s history. By its completion, women, First Nations, persons with disabilities and visible minorities accounted for just under 20% of total hours worked in an industry where 2% representation is the norm. Over 94% of payroll went to local residents, ensuring their communities the benefits of this major capital project. Finally, the health and safety record was significantly better than on any comparable construction project. Far from being an impediment to the efficient and timely completion of this major construction project, the collective agreement made it possible to deliver training, employment opportunities and regional development

Abstract

Aircraft gas turbine engines require compression systems with high performance and low weight and cost. There is therefore a continuing drive to increase compressor stage pressure ratios, particularly for military fans. To meet this need, a technology acquisition programme has been carried out by QinetiQ and Rolls-Royce. Firstly, the stage matching issues for an advanced two-stage military fan were investigated, including the effects of employing variable inlet guide vanes. From this, the requirements for the first stage together with key operating conditions for the blading were defined. The blade profiles were then designed to satisfy the range of aerodynamic conditions using a quasi-3D calculation system. A satisfactory compromise between the aerodynamic and mechanical design requirements was reached in which a blisk construction was employed for the rotor, machined from a single piece of titanium. The new stage was manufactured and tested successfully, and it achieved its target flow, pressure ratio and efficiency on the first build. Detailed measurements of the internal flows using laser anemometry and high response pressure transducers were taken. Finally, these data have been analysed and used to calibrate current 3D multi-row CFD methods.Copyright © 2003 by QinetiQ Ltd.

Abstract

Computational Fluid Dynamics (CFD) codes play an increasingly important role in the design and development of turbomachinery for modern gas turbine engines. As a result additional emphasis is being placed on the evaluation of the codes to ensure that they are working correctly and to indicate the accuracy which is likely to be achieved in practice. At DERA a programme of work has been carried out to evaluate the TRANSCode 3D viscous flow code, which was developed from the BTOB3D solver written by Dawes in 1986. A three part strategy for the validation and calibration of the code was adopted, covering comparisons with boundary layer test cases, Q3D compressor cascades and full 3D cases. The results indicated that the grids currently employed for turbomachinery flows limit the accuracy achieved for cases where there is significant laminar flow. For turbulent flows the Baldwin-Lomax turbulence model gives reasonably accurate results for 2D flows in near equilibrium, but it is less satisfactory for more complex flows, when the concept of a simple 2D boundary layer does not apply, and for strong shock wave/boundary layer interactions. Overall it is considered that the code is a valuable tool for turbomachinery analysis and design, but solutions must be assessed with care. Alternative turbulence models and other developments are being pursued for future versions of the code.
