Objectives Shift work that involves circadian disruption has been associated with a higher cancer risk. Most epidemiological studies to date have focused on breast cancer risk and evidence for other common tumors is limited. We evaluated the risk for colorectal cancer (CRC) in relation to shift work history in a population-based case-control study in Spain. Methods This analysis included 1626 incident CRC cases and 3378 randomly selected population controls of both sexes, enrolled in 11 regions of Spain. Sociodemographic and lifestyle information was assessed in face-to-face interviews. Shift work was assessed in detail throughout lifetime occupational history. We estimated the risk of colon and rectal cancer associated with rotating and permanent shift work (ever, cumulative duration, age of first exposure) using unconditional logistic regression analysis adjusting for potential confounders. Results Having ever performed rotating shift work (morning, evening and/or night) was associated with an increased risk for CRC [odds ratio (OR) 1.22, 95% confidence interval (95% CI) 1.04-1.43], as compared to day workers. Having ever worked permanent night shifts (≥3 nights/month) was not associated with CRC risk (OR 0.79, 95% CI 0.62-1.00). OR increased with increasing lifetime cumulative duration of rotating shift work (P-value for trend 0.005) and were highest among subjects in the top quartiles of exposure (3 rdquartile, 20-34 years, OR 1.38, 95%CI 1.06-1.81; 4 thquartile, ≥35 years, OR 1.36, 95% CI 1.02-1.79). Background: Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used despite their risk of gastrointestinal bleeding or cardiovascular events. We report the profile of people taking NSAIDs in Spain, and we include demographic factors, health-related behaviours and cardiovascular disease history. Methods: Four thousand sixtyparticipants were selected using a pseudorandom number list from Family Practice lists in 12 Spanish provinces. They completed a face-to-face computerized interview on their NSAID consumption, demographic characteristics, body mass index, alcohol and tobacco consumption and medical history. In addition, participants completed a self-administered food-frequency and alcohol consumption questionnaire. Factors associated with ever and current NSAID consumption were identified by logistic regression. Results: Women consumed more non-aspirin NSAIDs (38.8% [36.7-41.0]) than men (22.3 [20.5-24.2]), but men consumed more aspirin (11.7% [10.3-13.2]) than women (5.2% [4.3-6.3]). Consumption of non-aspirin NSAIDs decrease with age from 44.2% (39.4-49.1) in younger than 45 to 21.1% (18.3-24.2) in older than 75, but the age-pattern for aspirin usage was the opposite. Aspirin was reported by about 11% patients, as being twice as used in men (11.7%) than in women (5.2%); its consumption increased with age from 1.7% (< 45 years old) to 12.4% (≥75 years old). Aspirin was strongly associated with the presence of cardiovascular risk factors or established cardiovascular disease, reaching odds ratios of 15.2 (7.4-31.2) in women with acute coronary syndrome, 13.3 (6.2-28.3) in women with strokes and 11.1 (7.8-15.9) in men with acute coronary syndrome. Participants with cardiovascular risk factors or diseases consumed as much non-aspirin NSAID as participants without such conditions. Conclusions: Non-aspirin NSAIDs were more consumed by women and aspirin by men. The age patterns of aspirin and non-aspirin NSAIDs were opposite: the higher the age, the lower the non-aspirin NSAIDs usage and the higher the aspirin consumption. Occupational exposure to heat can affect the absorption of carcinogenic chemicals into the body, and the metabolism of sexual hormones. We explored the association between occupational exposure to heat and breast, chronic lymphocytic leukaemia, colorectal, prostate, and stomach cancer in the MCC-Spain population based case-control study. Methods The analyses were based on 3047 controls, and 1499 breast, 1539 colorectal, 332 LLC, 1070 prostate, and 382 stomach cancer cases. Heat working environment from either natural or artificial sources was assessed with the MatEmEsp job-exposure matrix. Logistic regression models accounting for education, sex, geographic area, BMI (colorectal, prostate), smoking (stomach, colorectal), menopause (breast) and number of alive children (breast) were fit to estimate Odds Ratios (OR) and 95% Confidence Limits (95 CL). Results Prostate cancer risk was over two and a half fold higher among men working for more than 10 years on a heat working environment than among who did not. Women working more than 10 years in occupations entailing heat exposure showed a higher risk of having breast cancer (OR: 1.3, 95% CL 1.04–1.7). The same figures where obtained for those working in occupations with higher probability*intensity of the exposure. Significant higher risk associations were also observed for colorectal cancer, among both men and women separately. While for stomach cancer, statistical significance was obtained only among men. LLC showed a suggestion of an association for both men and women. Our results provide further support to the hypothesis that working in a heat environment might be a risk factor for chemically and hormone related cancers.

Background and study aim: Several factors have been shown to be related to colonoscopy quality; however, little is known about the effects of endoscopist factors. This study analyzed the influence of endoscopist-related characteristics on quality indicators for colonoscopy. Patients and methods: The study included 48 endoscopists who each performed at least 20 colonoscopies in the colonoscopy arm of a randomized controlled trial comparing fecal immuno-chemical test vs. colonoscopy in colorectal cancer screening. These endoscopists performed a total of 3838 procedures in the trial. The following were calculated for each endoscopist: adenoma detection rate (ADR), advanced ADR, proximal ADR, distal ADR, and adenoma per colonoscopy rate (APCR). The characteristics of endoscopists were assessed with regard to colonoscopy quality using multivariate regression analysis. Endoscopist characteristics included age, sex, exclusive endoscopy practice, years as a physician, years as a specialist, specialty, total (life-long) number of colonoscopies performed, annual colonoscopy volume, number of hours/week dedicated to endoscopy and number of educational activities in the previous year. Results: Factors associated with ADR were age of the endoscopist (odds ratio [OR] 1.11, 95% confidence interval [CI] 1.01-1.21; P = 0.01) and lifelong number of colonoscopies (OR 1.06, 95% CI 1.01-1.11; P = 0.01). Only exclusive dedication to endoscopy practice was found to be independently related to proximal ADR (OR 1.71, 95% CI 1.15-2.74; P = 0.001). Life-long number of colonoscopies was independently related to detection of distal adenomas (OR 1.07, 95% CI 1.01-1.13; P = 0.01). None of the analyzed endoscopist characteristics was associated with advanced ADR or APCR. Conclusions: This study found that the experience of the endoscopist and exclusive dedication to endoscopy practice, but not annual colonoscopy volume, were associated with better colonoscopy quality.

We conducted a case–control and screening method studies to estimate influenza vaccine effectiveness (IVE) in the age group ≥65 years, based on the Spanish Influenza Sentinel Surveillance System (SISSS). Cases (influenza laboratory-confirmed) were compared to influenza-negative ILI patients (test-negative) and patients without ILI since the beginning of the season (non-ILI). For the screening method, cases’ vaccination coverage was compared to the vaccination coverage of the GPs’ catchment population. The results suggested a protective effect of the vaccine against laboratory-confirmed influenza in elderly in 2008–2009. The screening method and the test-negative control designs enable estimating IVE using exclusively SISSS data.

Geographical differences in type 1 diabetes (T1D) prevalence in Europe have been well documented, but little is known about the geographical distribution of autoantibodies specific to GAD65 (GAD65Ab) in the general population without diabetes, which is reported to range between 0.4 and 3%. However, these studies used different methods to define GAD65Ab positivity with cutoff values based on the 97–99th centile or at +3 SD above the mean among healthy individuals without T1D or type 2 diabetes (T2D). In doing so, the prevalence of GAD65Ab among the study cohorts was, by definition, 1–3%. The application of different cutoff levels greatly impairs the direct comparison of prevalence data between studies. Our aims were to 1) explore the prevalence of GAD65Ab positivity using a cutoff defined by specific competition of antibody binding to radiolabeled GAD65 with added autoantigen across eight European countries and 2) compare characteristics of age, sex, and BMI in relation to GAD65Ab positivity. A center-stratified random subcohort of 16,835 (4.9%) individuals was selected from the original European Prospective Investigation into Cancer and Nutrition (EPIC)-InterAct study (1). After exclusion of individuals with known diabetes, GAD65Ab were analyzed in 15,802 (men/women 5,927/9,875, mean age 52.4 ± 9.2 years) samples. The cutoff for GAD65Ab positivity was determined through a competition assay at ≥65 WHO units/mL, and all samples were analyzed at a single laboratory using a radioligand binding assay (2).In all, 316/15,802 (2.0%) samples were GAD65Ab positive. Sweden, Denmark, U.K., and Spain had the highest and France had the lowest prevalence of GAD65Ab positivity (Table 1); however, these differences were not statistically significant (P = 0.80). We did not detect any association between GAD65Ab positivity and age, sex, or BMI.

Introduction The agreement between interim and final influenza vaccine effectiveness (VE) estimates would support the use of interim assessments as a proxy for final VE results to guide health authorities in influenza prevention. We aimed to compare interim/final VE estimates in Spain. Methods We used a test-negative case-control study (cycEVA) for 2010/11–2013/14 seasons. Sensitivity analyses were carried out by type/subtype of influenza virus and by target groups for vaccination. Results In general, interim estimates were higher compared to end-season estimates. Interim and final VE differences were higher for the target groups compared to all population. Subtype-specific interim/final VE estimates showed greater concordance (3–13%) than for any virus (7–24%). Conclusion In Spain, interim influenza VE estimates over 2010–2014 were a good proxy of the final protection of the vaccine. Interim and final estimates showed greater concordance for all population and if performed subtype-specific.

Background: We aimed to evaluate whether oral anticoagulants (OACs) alter faecal immunochemical test (FIT) performance in average-risk colorectal cancer (CRC) screening. Methods: Individuals aged 50–69 years were invited to receive one FIT sample (cutoff 75¿ng¿ml–1) between November 2008 and June 2011. Results: Faecal immunochemical test was positive in 9.3% (21 out of 224) of users of OAC and 6.2% (365 out of 5821) of non-users (P-trend=0.07). The positive predictive value (PPV) for advanced neoplasia (AN) in non-users was 50.4% vs 47.6% in users (odds ratio, 0.70; 95% CI, 0.3–1.8; P=0.5). The PPV for AN in OAC more antiplatelets (aspirin or clopidogrel) was 75% (odds ratio, 2; 95% CI, 0.4–10.8; P=0.4). Conclusions: Oral anticoagulant did not significantly modify the PPV for AN in this population-based colorectal screening program. The detection rate of advanced adenoma was higher in the combination OAC more antiplatelets.

ResumenObjetivosEstimar la prevalencia de prácticas de cribado de cáncer en los controles del estudio multi-caso control (MCC-Spain) y evaluar su asociación con los estilos de vida.MétodosAnálisis transversal de 1505 controles poblacionales del estudio MCC-Spain, de 20-85 años, recogidos entre 2008-2010 en cinco áreas de España. Se ha calculado la prevalencia de pruebas de cribado en los últimos 5 años, evaluando la relación con los estilos de vida y analizando su asociación mediante modelos de regresión logística.ResultadosLas pruebas más prevalentes en las mujeres fueron mamografía (98,2%, en mujeres de 50-69 años) y citología (94,5%, en las de 25-65 años), y antígeno específico de próstata en los hombres (71,5%, 50 años o más). La realización de la prueba de sangre oculta en heces fue muy inferior (11,2% y 16,2%, en mujeres y hombres de 50-74 años, respectivamente). Consumir al menos tres raciones de fruta y verdura al día se asocia con una mayor participación en mamografías (odds ratio ajustadas [ORa]: 5,24; intervalo de confianza del 95% [IC95%]: 1,96-14,07). En los hombres, el sobrepeso-obesidad se relaciona con la mayor realización de la prueba del antígeno específico de próstata (ORa: 1,88; IC95%: 1,15-3,08). En general, destaca la presencia de comorbilidad asociada con la realización de pruebas de cribado.ConclusionesEste estudio muestra una elevada prevalencia de prácticas de cribado de cáncer de mama, cuello uterino y próstata, y baja del cáncer colorrectal. No se observa un patrón común de estilos de vida asociado al cribado, sino que hay diferencias por sexo y tipo de prueba. Estos resultados pueden ser útiles para orientar nuevos estudios sobre las prácticas de cribado y revisar posibles ineficiencias.AbstractObjectivesTo estimate the prevalence of screening practices in the population controls of the multi-case control study (MCC-Spain) study and to evaluate its association with lifestyles.MethodsWe carried out a cross-sectional analysis of 1505 population controls of the MCC-Spain study, aged 20-85, enrolled from 2008-2010 in five areas of Spain. The prevalence of screening tests in the last 5 years was estimated and its association with lifestyles was assessed using logistic regression models.ResultsThe most prevalent tests were mammography (98.2%, age 50-69 years) and cytology (94.5%, age 25-65 years) in women and prostate-specific antigen in men (71.5%, age 50 or over). Participation in fecal occult blood testing was lower (11.2% and 16.2% in women and men aged 50-74, respectively). Eating at least three pieces of fruit and vegetables per day was associated with higher participation in mammography (ORa: 5.24, 95% CI: 1.96-14.07). In men, overweight-obesity was linked to higher participation in prostate-specific antigen testing (ORa: 1.88; 95% CI: 1.15-3.08). In general, the presence of comorbidities was associated with utilization of cancer screening tests.ConclusionsThe prevalence of screening practices was high for breast, cervical and prostate cancer and was low for colorectal cancer. No common lifestyle patterns were associated with participation, although differences were found by sex and type of screening test. These results may be useful to guide further studies on screening practices and review possible inefficiencies.

Background In Spain, the influenza vaccine effectiveness (VE) was estimated in the last three seasons using an observational study (cycEVA) conducted in the frame of the Spanish Influenza Sentinel Surveillance System. We aimed to measure the effectiveness of the seasonal trivalent vaccine in preventing influenza like illness (ILI) laboratory-confirmed influenza infection at the end of the season 2010–11. Methods We conducted a test-negative case–control study between weeks 50/2010 and 12/2011. Cases were ILI laboratory-confirmed influenza infection and controls were those testing negative. Sentinel physicians collected data on demographic and clinical characteristics, vaccination status, and on covariates related to confounding factors associating with influenza VE. We calculated adjusted odds ratios (OR), using logistic regression and computed influenza VE as (1 − OR) × 100. Results The adjusted influenza VE against A(H1N1)pdm09 infection was 46% (95% confidence interval (95%CI): 0; 72). In A(H1N1)pdm09 infected patients who had received both 2010–11 trivalent influenza seasonal and 2009 monovalent pandemic vaccines, influenza VE was 74% (95%CI: 13; 93). The adjusted influenza VE against B infection was 23% (95%CI: −180; 79). Conclusion The trivalent influenza vaccine 2010–11 showed a moderate VE for preventing ILI laboratory confirmed influenza infections. Influenza VE estimates were higher in patients who had received both 2010–11 seasonal trivalent and 2009 monovalent pandemic vaccines.

Analyses were based on a subsample of the European Prospective Investigation into Cancer and Nutrition (EPIC study), a prospective cohort. The analyses included 703 incident lung cancer cases among men in Denmark, the United Kingdom, Germany, Italy, Spain and Greece. The socioeconomic position was measured using the highest level of education. The estimates of relative indices of inequality (RII) were computed with Cox regression models. We first adjusted for smoking (with detailed information on duration and quantity) and dietary factors (fruits and vegetables consumption) and then for occupational exposures. The exposure to three carcinogens [asbestos, heavy metals and polycyclic aromatic hydrocarbons (PAH)] was analyzed. The occupational exposures explained 14% of the socioeconomic inequalities remaining after adjustment for smoking and fruits and vegetables consumption. The inequalities remained nevertheless statistically significant. The RII decreased from 1.87 (95% CI: 1.36-2.56) to 1.75 (1.27-2.41). The decrease was more pronounced when adjusting for asbestos than for heavy metals or PAH. Analyses by birth cohort suggested an effect of occupational exposures among older men, while due to small number of endpoints, no conclusion could be drawn about the role of occupational exposures in educational inequalities among younger men. Our study revealed that the impact of occupational exposures on socioeconomic inequalities in cancer incidence, rarely studied until now, exists while of modest magnitude.
