40. Adjunctive Intermittent Theta-Burst Stimulation for Schizophrenia: A Systematic Review and Meta-analysis of Randomized Double-Blind Controlled Studies.

Abstract

Objective: In order to determine whether intermittent theta-burst stimulation (iTBS) is a viable adjunct treatment for schizophrenia, a meta-analysis of double-blind, randomized clinical trials (RCTs) was performed. Methods: Four independent researchers extracted and synthesized data from RCTs on adjunctive iTBS for patients suffering from schizophrenia. RevMan 5.3 software was used to calculate risk ratios (RRs) and standardized mean differences (SMDs) along with their 95% confidence intervals (CIs). Results: Fifteen RCTs involving 671 patients with schizophrenia were included. Adjunctive iTBS was significantly superior to sham interventions for improvement in overall psychopathology (SMD = -0.75, 95% CI: -1.10, -0.41, I² = 64%, P < .0001), negative symptoms (SMD = -0.76, 95% CI: -1.18, -0.35, I² = 78%, P = .0003), and general psychopathology (SMD = -0.51, 95% CI: -0.88, -0.14, I² = 71%, P = .007), though no significant group difference was found regarding positive symptoms. Adjunctive iTBS also demonstrated superiority over control treatments in improving cognitive functions as measured by the Spatial Span Test (SMD = 0.83, 95% CI: 0.16, 1.49, I² = 73%, P = .02) and Montreal Cognitive Assessment (SMD = 0.49, 95% CI: 0.11, 0.88, I² = 0%, P = .01). Discontinuation rates (RR = 0.92, 95% CI: 0.57, 1.50, I² = 0%, P = .75) and adverse events were comparable between groups. Conclusion: The use of iTBS in patients with schizophrenia appears to be effective in improving psychiatric symptoms and cognitive function. Abstract

Background: Depressive symptoms commonly co‐occur with cognitive decline in older adults. However, prospective interrelationships between different cognitive function domains and depressive symptoms are not well understood. This study evaluated prospective interrelationships between depressive symptoms and cognitive functioning components among individuals aged 50 years or older from a perspective of network analysis. Method: Longitudinal data from the English Longitudinal Study of Aging were analyzed. Depressive symptoms were measured with the eight‐item Center for Epidemiologic Studies Short–Depression Scale. Cognitive functions assessed included memory, orientation, and executive function. Contemporaneous network analyses were conducted using mixed graphical model, while a temporal network model was assessed using cross‐lagged panel network model. To identify important predictors and outcomes, centrality indices, including expected influence, out‐expected influence, and in‐expected influence, were calculated. Results: A total of 6,433 older adults were included in the network analysis. Baseline "Not enjoy life" (CESD‐6) was negatively associated with executive function at the follow‐up assessment. Moreover, improvements in "Everything was an effort" (CESD‐2) and "Loneliness" (CESD‐5) were related to less future decline of executive function and memory ability. Furthermore, analyses suggested targeting "Lack of happiness" (CESD‐4) could be useful in reducing the co‐occurrence of depression and cognitive decline among older adults. Conclusions: This network analysis study highlighted dynamic interrelationships between depressive symptoms and cognitive decline in older adults. Findings suggest that interventions targeting specific depressive symptoms may have the potential to alleviate declines in executive function and memory for this population. [ABSTRACT FROM AUTHOR]

Abstract

Body dissatisfaction is a global public health concern. Self-injurious thoughts and behaviours (SITB), including suicidal ideation, suicide attempts and non-suicidal self-injury (NSSI), have been documented as potentially significant correlates of body dissatisfaction. However, prior findings regarding associations between body dissatisfaction and SITB have been somewhat inconsistent. Therefore, this meta-analysis was conducted to determine the nature and strength of such associations in both clinical and non-clinical samples. A literature search identified 83 relevant articles and extracted 234 effect sizes. Using a three-level random-effects model, mean effect sizes (r values) for relationships between body dissatisfaction and suicidal ideation, suicide attempts, and NSSI in clinical samples were 0.29 (95% CI, 0.22–0.37), 0.16 (95% CI, 0.13–0.20) and 0.26 (95% CI, 0.19–0.34), respectively. In non-clinical samples, these values were 0.22 (95% CI, 0.16–0.28), 0.24 (95% CI, 0.17–0.30) and 0.22 (95% CI, 0.15–0.29), respectively. Several study features (e.g., participant age, geographic region and instrument validity) emerged as significant moderators. This meta-analysis provides robust support for body dissatisfaction as a significant correlate of SITB across clinical and non-clinical samples in addition to identifying study characteristics that contribute to effect size variability.
