Purpose To investigate the prevalence and causes of near vision impairment (NVI) in a population of older adults from the Brazilian Amazon Region. Design Population-based cross-sectional study. Methods Cluster sampling was used in randomly selecting subjects 45 years of age and older from urban and rural areas of Parintins city, Brazil. Participants underwent ophthalmic examination, including uncorrected (UCNVA), presenting (PNVA), and best-corrected near visual acuity (BCNVA) from each eye; biomicroscopy; funduscopy; and subjective refraction, including testing with additional lenses for near vision optical correction. A principal cause for NVI was assigned by the ophthalmologist and presbyopia was defined as UCNVA ≤ 20/40 changing to > 20/40 with BCNVA. Free-of-charge glasses were provided for those in need. Results A total of 2384 subjects were enumerated and 2025 had reliable NVA measurements from both eyes. The prevalence of NVI in the better-seeing eye was 96.5% with UCNVA, decreasing to 81.1% with PNVA and to 20.5% with BCNVA. Presbyopia was the principal cause of NVI in 71.8%, followed by cataract (16.5%) and pterygium (2.5%), and was associated with younger age and high schooling. Glasses for near vision were prescribed and provided to 1414 (69.8%) participants. Conclusions A high prevalence of NVI was detected even in those wearing glasses for near. Prescription and provision of low-cost reading glasses should be considered by Brazilian health authorities to address this easily and promptly correctable form of vision impairment.

Purpose To determine host and pathogen factors predictive of outcomes in a large clinical cohort with keratoconjunctivitis. Design Retrospective analyses of the clinical and molecular data from a randomized, controlled, masked trial for auricloscene for keratoconjunctivitis (NVC-422 phase IIB, NovaBay; clinicaltrials.gov identifier, NCT01877694). Participants Five hundred participants from United States, India, Brazil, and Sri Lanka with clinical diagnosis of keratoconjunctivitis and positive rapid test results for adenovirus. Methods Clinical signs and symptoms and bilateral conjunctival swabs were obtained on days 1, 3, 6, 11, and 18. Polymerase chain reaction (PCR) analysis was performed to detect and quantify adenovirus in all samples. Regression models were used to evaluate the association of various variables with keratoconjunctivitis outcomes. Time to resolution of each symptom or sign was assessed by adenoviral species with Cox regression. Main Outcome Measures The difference in composite scores of clinical signs between days 1 and 18, mean visual acuity change between days 1 and 18, and time to resolution of each symptom or sign. Results Of 500 participants, 390 (78%) showed evidence of adenovirus by PCR. Among adenovirus-positive participants, adenovirus D species was most common (63% of total cases), but a total of 4 species and 21 different types of adenovirus were detected. Adenovirus D was associated with more severe signs and symptoms, a higher rate of subepithelial infiltrate development, and a slower decline in viral load compared with all other adenovirus species. The clinical courses of all patients with non–adenovirus D species infection and adenovirus-negative keratoconjunctivitis were similar. Mean change in visual acuity between days 1 and 18 was a gain of 1.9 letters; worse visual outcome was associated with older age. Conclusions A substantial proportion of keratoconjunctivitis is not associated with a detectable adenovirus. The clinical course of those with adenovirus D keratoconjunctivitis is significantly more severe than those with non–adenovirus D species infections or adenovirus-negative keratoconjunctivitis; high viral load at presentation and non-United States origin of participants is associated with poorer clinical outcome.

Morning Glory Syndrome is a very rare condition. It is characterized by an enlarged and funnel-shaped optic disc excavated and is associated with other craniofacial anomalies such as hypertelorism, cleft lip, cleft palate, and basal encephalocele. CaseReport. A case of a 30 year-old man who came for neurosurgical evaluation after a magnetic resonance imaging (MRI) scan that showed spheno-ethmoidal encephalocele is reported. Conclusions. There is a high association between basal encephalocele,midface abnormalities and Morning Glory Syndrome. A neurosurgical, ophtalmological and endocrinological evaluation must always be done. The patients present with ophtalmic signs and/or strabismus or poor vision. An image study should always be performed to delineate the extent of the lesion and a complete hormone screening should be carried out to exclude pituitary deficiency.

To describe the study design, operational strategies, procedures, and baseline characteristics of the Brazilian Amazon Region Eye Survey (BARES), a population-based survey of the prevalence and causes of distance and near visual impairment and blindness in older adults residing in the city of Parintins.Cluster sampling, based on geographically defined census sectors, was used for cross-sectional random sampling of persons 45 years and older from urban and rural areas. Subjects were enumerated through a door-to-door survey and invited for measurement of uncorrected, presenting and best-corrected visual acuity and an ocular examination.Of 9931 residents (5878 urban and 4053 rural), 2384 individuals (1410 urban and 974 rural) were eligible and 2041 (1180 urban and 861 rural) had a clinical examination (response rate 85.6%). The majority of participants were female (1041, 51.0%); the average age was 59.9 ± 11.1 years (60.2 ± 11.2 years for urban and 59.4 ± 11.1 years for rural); 1360 (66.6%) had primary schooling or less (58.1% in urban and 78.4% in rural) and 57.8% were resident in urban areas. The age distribution between sexes was similar (p = 0.178). Both sex and age distributions of the sample were comparable to that of the Brazilian Amazon Region population.The BARES cohort will provide information about the prevalence and causes of near and distance vision in this underprivileged and remote population in Brazil.

PURPOSE To investigate cataract surgery prevalence, complications, visual outcomes, and coverage in a population of older adults from the Brazilian Amazon region. DESIGN Population-based cross-sectional study. METHODS Individuals ≥45 years of age from urban and rural areas of Parintins City, Brazil, were identified by cluster random sampling. Participants underwent ophthalmic examination, including uncorrected visual acuity, acuity with presenting correction (APC), and best-corrected visual acuity (BCVA) from each eye. In those with previous cataract surgery, surgical technique and complications including its contribution to vision impairment were noted. The association of surgical complications and surgical coverage with sociodemographic variables was assessed by multiple logistic regression. RESULTS A total of 2,384 persons were determined to be eligible, and 2,041 (85.6%) were examined. Overall, prevalence of cataract surgery was 8.6%, with 11.3% urban and 5.0% rural. Surgical complications were associated with males, older age, and urban residency and were found in 60.4%, with posterior capsule opacification in 29.6% and posterior capsule rupture in 16.3%. Among the 270 eyes having cataract surgery, APC ≥20/32 was found in 44.4%, APC 20/40 to 20/63 in 20.8%, APC

AimsTo determine prevalence of pterygium, its role as main cause of unilateral and bilateral visual impairment and blindness and its impact on refractive errors from adults living in a high ultraviolet exposure area in the Brazilian Amazon Region.MethodsCluster sampling was used in randomly selecting subjects ≥45 years of age from urban and rural areas of Parintins city. Eligible subjects were enumerated through a door-to-door household survey and invited for an eye exam including refraction. Pterygium was assessed considering location (nasal, temporal or both) and size (ResultsA total of 2384 persons were enumerated and 2041 (85.6%) were examined. Prevalence of pterygium was 58.8% (95% CI 53.8% to 63.7%) and associated with male gender (OR=1.63; 95% CI 1.37 to 1.94; p=0.001), while higher education was a protective factor (OR=0.63; 95% CI 0.44 to 0.92; p=0.018). Older age and rural residence were associated with pterygium ≥3 mm reaching or not pupillary margin, while higher education was a protective factor for pterygium ≥3 mm reaching pupillary margin. Prevalence of pterygium as cause of visual impairment and blindness was 14.3% and 3.9%, respectively. Significantly higher hyperopic refractive errors were found in eyes with pterygium ≥3 mm reaching or not pupillary margin.ConclusionsPterygium was highly prevalent and the second cause of visual impairment and blindness after provision of refractive correction. Risk factors for pterygium were male gender, advanced age, lower education and rural residency. Strategies to provide pterygium early detection and proper management should be considered by healthcare authorities in this population.

We conducted a population-based survey to determine the vision status in subjects 45 years of age and older from urban and rural areas of Parintins city, Brazilian Amazon Region. Participants underwent ophthalmic examination, including uncorrected (UCVA), presenting (PVA) and best-corrected visual acuity (BCVA). Vision status was described as lines of visual acuity (VA) impairment and lines of VA improvement from UCVA to BCVA and from PVA to BCVA in the better-seeing eye. A total of 2384 subjects were enumerated, 2041 (85.6%) were examined, with reliable VA measurements obtained from 2025 participants. Vision status in lines of VA impairment was (mean +/- standard deviation): 3.44 +/- 3.53 for UCVA, 2.85 +/- 3.52 for PVA and 1.50 +/- 3.51 for BCVA. Female gender, older age and lower education were associated with >= 6 lines of UCVA impairment. Lines of improvement >= 3 was found in 626 (30.9%) participants and associated with female gender and rural residency. In conclusion, a third of participants could have at least three lines of VA improvement with proper refraction. Strategies to improve access to eye care and affordable glasses are needed. A sharp drop in the prevalence of leprosy occurred in the last three decades. However, the incidence has not decreased at the same rate. Three years after the World Health Organization last deadline for leprosy control, patients considered healed still need special care for their incapacities and immunopathological reactions. Medical literature reffers blindness in 4% to 11% of studied patients and more than 20% with severe visual problems due to corneal exposure, bacillary invasion and hipersensibility. These mechanisms result in a population of nearly one million blind leprosy patients even though official prevalence accounts no more than 250,000 patients worldwide. The author calls for better patients management and follow-up and urges ophthalmologists to become more aware and interested in the treatment of the ocular complications of leprosy.

OBJETIVOS: Avaliar as alterações oculares observadas em indivíduos com mansonelose residentes na área rural do município de Coari, AM, na região do médio Amazonas. MÉTODOS: Foi conduzido um estudo de corte transversal envolvendo 543 moradores residentes na área rural do município de Coari, na margem direita do rio Solimões. Todos os indivíduos foram submetidos a exame de gota espessa para pesquisa de microfilária e os indivíduos positivos foram submetidos a exame oftalmológico. Lesões oculares suspeitas foram biopsiadas e enviadas para exame histopatológico. RESULTADOS: Mansonella ozzardi foi encontrada no sangue periférico em 103 (18,9%) indivíduos. Noventa e cinco pacientes com mansonelose foram examinados, observando ceratite puntata em 12 (11,7%) destes, ceratite numular em um indivíduo (1%) e esclerosante em outro (1%). Biópsia de pele foi negativa em todos os indivíduos. Biópsia de conjuntiva e limbo foi realizada em cinco pacientes com mansonelose e biópsia de córnea em outros três pacientes. Duas biópsias de conjuntiva e limbo e uma biópsia de córnea foram enviadas para exame histopatológico, não sendo observadas microfilárias ou alterações sugestivas da presença destas nas peças estudadas. O restante das biópsias foi enviado para pesquisa de DNA do parasita por PCR, não se encontrando microfilária. CONCLUSÕES: Associação entre infecção por Mansonella ozzardi e lesões corneanas sem etiologia aparente foi verificada neste estudo, mas a ausência de achados específicos no exame histopatológico e a não identificação de microfilárias por PCR em lesões suspeitas põem em dúvida a o acometimento corneano na mansonelose. Estudos mais amplos de populações com alta prevalência de M. ozzardi e a realização de outros testes para identificação de microfilárias em tecido ocular são necessários para investigar a patogenicidade da microfilária ao tecido ocular. PURPOSE: To evaluate eye manifestations in mansonelliasis patients from Coari, AM, rural area on the middle Amazonas River. METHODS: Cross-sectional study consisted of evaluation of 543 subjects from Coari rural area on Solimões River right margin. All subjects were submitted to blood test for microfilaria and the subjects who had a positive test also were submitted to skin biopsies and ophthalmologic examination. Histopathology and PCR research for microfilaria were done on eye biopsies of suspect lesions. RESULTS: Mansonella ozzardi was found in peripheral blood of 103 (18.9%) subjects. Ninety-five mansonelliasis patients were examined. Punctate keratitis was observed in 12 of them, nummular keratitis in one subject and sclerosing keratitis in another one. Skin biopsies were negative in all subjects. Conjunctival and limbal biopsies were done in five mansonelliasis patients and corneal biopsy in another three patients. Conjunctival and limbal biopsies of two patients and corneal biopsy of one patient showed no microfilaria or alterations due to its presence on histopathology. PCR showed no evidence of microfilaria in the other biopsies. CONCLUSION: Association between Mansonella ozzardi infection and corneal lesions with no evident etiology was observed in this study, but the lack of conclusive findings on histopathology and PCR make us doubt the corneal involvement in mansonelliasis. Large studies of high mansonelliasis prevalence populations and extensive employment of microfilaria identification tests in ocular tissue are needed to evaluate the microfilaria corneal pathogenicity.

Objectives To characterise and confirm the presence of Mansonella ozzardi microfilariae in the cornea by biomicroscopy and corneal confocal microscopy. Design Cross-sectional study. Settings Clinical practice study in patients from rural communities in Coari city on the Solimoes river, Amazonas state, Brazil. Participants The eyes of 212 consecutive volunteer patients were examined using a flash light and their blood checked for the presence of microfilariae by an expert microscopist. Patients with suspicious corneal lesions (characterised as nummular keratitis) were submitted to biomicroscopy, fundoscopy and corneal confocal microscopy evaluation (CCME). In two patients, a biopsy of the limbal conjunctiva adjacent to the nummular keratitis was carried out and blood collected from the surgical wound for microfilariae investigation by thick blood film examination. Primary and secondary outcome measures Positive correlation between corneal biomicroscopic and confocal lesions and M ozzardi microfilaremia. Results Of the 212 patients, 56 (26.4%) were positive for microfilaremia. 22 patients with nummular keratitis identified under flash light examination underwent biomicroscopy and CCME. Corneal lesions were positively correlated to microfilaremia (p=0.0001). At biomicroscopy, lesions were classified as quiescent or active. At CCME, lesions were categorised as circular or filiform. The associations between corneal lesions, CCME findings and microfilaremia are shown. Conclusions We describe M ozzardi microfilariae in the cornea and the associated eye pathology. Further studies using ocular tissue PCR and other imaging techniques would be helpful.

A prevalência mundial da hanseníase vem demonstrando redução no número de casos, porém, no Brasil e em alguns países, ela ainda representa um grave problema de saúde pública, podendo levar a incapacidades funcionais graves como a cegueira. O objetivo do estudo foi avaliar a relação entre as alterações do piscar e a superfície ocular em hansenianos. Todos os pacientes estudados foram submetidos à mesma anamnese e avaliação oftalmológica: acuidade visual, ectoscopia, biomicroscopia, avaliação da superfície ocular, teste da graduação da força muscular do orbicular, sensibilidade corneana, distância da margem reflexa, medida da pressão intraocular e mensuração do piscar espontâneo palpebral por meio de um método de imagem por vídeo. Dos 56 pacientes examinados, 69,6% eram do sexo masculino, com média de idade de 55,96 ± 16,63 dp, 46,4% se declararam negros e 28,55% pardos, 71,4% apresentavam a forma multibacilar e 73,2% estavam fora do registro ativo da doença. Desses 56 pacientes, 43 apresentaram significativa simetria interocular no acometimento do nervo facial e do trigêmeo (p=0,11), o que foi corroborado pela alta correlação entre as medidas da amplitude do piscar entre os olhos (r=0,90). Apenas 12,5% apresentaram tempo de ruptura do filme lacrimal menor que 10 segundos e em um paciente este foi menor que 5. Evidenciou-se sofrimento da superfície ocular em cerca de 14% dos olhos. As alterações de sensibilidade foram mais prevalentes, pois 51,8% apresentaram algum grau de diminuição. A média geral da taxa do piscar foi de 17,0 ± 2,6 blink/min. De acordo com o exame de Lissamina, observou-se taxa média de 16,0 ± 2,8 (média±dp) para os pacientes com resultado negativo e 23,2 ± 6,8 para os com resultado positivo (t=0,961; p=0,3407); e em relação à sensibilidade corneana, as taxas médias foram 14,6 ± 3,8 e 19,2 ± 3,6 para os pacientes com resposta imediata e alterada, respectivamente (t=0,875; p=0,3857). De acordo com o tônus muscular, a média das taxas do piscar para os pacientes normais e alterados não foi significativa (t=0,539; p=0,592). Apesar do número e da amplitude dos movimentos serem diferentes, a main sequence demonstrou comportamento linear em todos os casos, sendo a média geral 20,25 ± 6,9 (0,94 ep). As médias da taxa, amplitude e efetividade do piscar em pacientes com função do músculo orbicular do olho normal e naqueles com função alterada não demonstraram diferença estatística, já a média da velocidade máxima do piscar com função normal foi de 115,5 ± 47,2 mm/s, enquanto que naqueles com lagoftalmo foi 67,7 ± 27 (t=2,08; p=0,04) e a média do deslocamento horizontal de 2,1 ± 0,7 mm e 0,9 ± 0,8 mm, respectivamente (t=1,99; p=0,05). Embora os pacientes hansenianos não apresentem taxa de piscar diferente do normal, demonstram tendência à diminuição da velocidade e do deslocamento horizontal quando apresentam alterações da função do músculo orbicular. A maioria dos pacientes apresentou alteração de sensibilidade corneana, porém, sem sinais de sofrimento da superfície ocular, principalmente com a cinemática palpebral preservada. Global prevalence of leprosy has demonstrated a reduction in the number of cases, however, in Brazil and some countries, it still represents a serious public health problem, often leading to severe functional disabilities such as blindness for example. This study aimed to evaluate the relationship between blink and ocular surface in patients with leprosy. Leprosy patients underwent the same history and ophthalmologic evaluation: visual acuity, ectoscopy, slit lamp examination, evaluation of ocular surface, test the eyelid function and the degree of orbicularis function, corneal sensitivity, margin reflex distance, measured intraocular pressure and measuring eyelid spontaneous blinking through an image video system. 69.6% were male, with a mean age of 55.96 ± 16.63 SD. 46.4% declared themselves as black and brown were 28.55%, 71.4% were multibacillary and 73.2% were out of active disease registry. Between the 56 patients examined, 43 of those had significant interocular symmetry of the involvement of the facial and the trigeminal nerves (p=0.11), which was confirmed by the high correlation between the measurements of amplitude flashing of the eyes (r=0,90). Only 12.5% had TFRL was under 10 seconds and in one patient it was under 5 seconds. It was evidenced suffering from ocular surface in about 14% of the eyes. Sensitivity changes were more prevalent, 51.8% had some degree of impairment. Overall average blink rate was 17.0 ± 2.6 blinks/min. In the lissamine test, we observed an average rate of 16.0 ± 2.8 for patients with negative and 23.2 ± 6.8 for a positive result (t=0.961, p=0.3407). The mean values were 14.6 ± 3.8 (mean ± SD) and 19.2 ± 3.6 for for patients with immediate corneal sensitivity response and altered response, respectively (t=0.875, p=0.3857). According muscle tone, mean blink rates for normals and for injured patients, were not different (t=0.539, p=0.592). Although the number and range of motion are different, the main sequence had a linear behavior in all cases, with an overall mean 20.25 ± 6.9 (p 0.94). The mean blink rate, amplitude and effectiveness in patients with normal orbicularis function and those with altered function, showed no statistical diference, while the mean of maximum velocity blink was, with normal function, 115.5 ± 47.2 mm/s, while those with lagophthalmos was 27 ± 67.7 (t=2.08, p=0.04) and the mean horizontal displacement was 2.1 ± 0.7 mm and 0.9 ±0.8 mm respectively (t=1.99, p=0.05). Although leprosy patients do not have a different flash rate of the normal population, blinking characteristics show a tendency to decrease in speed and horizontal scrolling when they show important changes in orbicularis function. The vast majority of patients had corneal sensitivity changes, but without signs of ocular surface suffering, especially those with eyelid kinematics preserved.

A hanseníase representa ainda um grave problema de saúde pública no Brasil. O presente estudo teve como objetivo investigar alterações oculares em pacientes portadores de hanseníase após alta do registro ativo da Secretaria de Saúde do Estado do Maranhão. Foram avaliados 370 pacientes, sendo 249 após alta do registro ativo (GI) e 121 do grupo ativo (GA). Os dois grupos foram submetidos à mesma avaliação, considerando-se: variáveis classificatórias (sexo, idade, raça, idade do início da doença, duração, fase e tipo de tratamento, recidiva e forma clínica da hanseníase); interrogatório dirigido aos principais sinais e sintomas relacionados à doença ocular (olho vermelho, ardor, lacrimejamento, prurido, sensação de corpo estranho, secreção, dor, fotofobia, sensação de baixa da acuidade visual, sensação de olho seco); uso de corticosteroides sistêmicos e exames oftalmológicos específicos (medida da acuidade visual com e sem correção óptica, ectoscopia, biomicroscopia da margem palpebral e segmento anterior, tempo de rotura do filme lacrimal, teste de Schirmer, teste da função palpebral do músculo orbicular, estesiometria corneana, medida do posicionamento da pálpebra superior (distância margem-reflexo), medida da pressão intraocular e fundoscopia). Diferenças significativas entre os grupos (GA/GI) foram encontradas somente em relação a: queixas de baixa acuidade visual (75,2% / 64,3%), secreção ocular (2,5% / 9,2%), uso de corticosteroides (28,1%/41,0%) e médias da distância margem palpebral-reflexo (4,5±0,9 / 4,8±1,0). As alterações oftalmológicas foram frequentes após alta do registro ativo, assim como no grupo com doença em atividade, demonstrando a necessidade da atenção à saúde ocular ao portador de hanseníase, tanto durante o processo ativo da doença como após ter completado seu tratamento, quando o seu registro se encontra inativo e, portanto, já considerado curado pelos serviços de saúde. Leprosy is still a major public health problem in Brazil. This study aimed to investigate ocular changes in leprosy patients after discharge from active record of the Secretariat of the Maranhão State Health. We evaluated 370 patients, 249 after discharge from active record (GI) and 121 in the active group (GA). Both groups underwent the same evaluation, considering categorical variables (gender, age, race, age of onset, duration, phase and type of treatment, recurrence and clinical form of leprosy) and Interrogation directed to the main signs and symptoms related to eye disease (red eye, burning, tearing, itching, foreign body sensation, discharge, pain, photophobia, feeling of blurred vision, feeling of dry eye). Use of systemic corticosteroids and specific eye examinations (visual acuity with and without optical correction, ectoscopy, slit lamp examination eyelid margin and anterior segment of the tear film breakup time, Schirmer test, test of eyelid function of the orbicularis muscle, esthesiometry corneal measured from the upper eyelid position (distance margin-reflex), measurement of intraocular pressure and fundus were also analised. Significant differences between groups (GA / GI) were found only in relation to complaints of visual impairment (75.2% / 64.3%), eye discharge (2.5% / 9.2%), the use of corticosteroids (28.1% / 41.0%) and eyelid margin-reflex distance (4,5±0,9 / 4,8±1,0).
