7. Promoting Population Stabilization: Incentives for Small Families. Worldwatch Paper 54.

Abstract

A wide variety of incentive and disincentive programs are presented in an effort to stabilize the population and prevent bankruptcy of physical, economic, and social resources, particularly in countries like India and China. Following an introduction, the document discusses several programs, including (1) the use of small one-time payments for individuals who become sterilized and to family planning workers or doctors for each acceptor recruited; (2) incentives that improve welfare such as deferred incentive schemes like the "No-Birth Bonus Scheme" by private industry or government pension programs, in which accounts are credited with later collection contingent upon success in having a small family; (3) community development incentives in which whole villages are rewarded with development programs if the birth rate falls; (4) penalization of large families by imposing costs or withholding benefits; (5) emergency measures such as India's compulsory sterilization and China's one-child policy; and (6) expansion of the role of other incentive and disincentive schemes such as paying women to remain childless longer and developing programs to improve the lot of women. Also provided are four data tables illustrating countries offering small, one-time payments; average number of living children of women undergoing sterilization; fertility declines of all of India and the Tea Estates with and without "No-Birth Bonus" schemes; and Singapore's birthrate from 1960-1982. Abstract

The Global Change Instruction Program was designed by college professors to fill a need for interdisciplinary materials on the emerging science of global change. This instructional module concentrates on interactions between population growth and human activities that produce global change. The materials are designed for undergraduate students lacking in advanced training in mathematics and the technical aspects of science. Each module contains content material, exercise questions, and an answer key and discussion of the exercise questions. An introduction examines interactions between global change and population growth using carbon dioxide emissions as an example. Succeeding chapters cover: (1) demographic basics; (2) patterns of population growth and fertility; (3) determinants of fertility; (4) policies for encouraging smaller families; (5) the future of world population; and (6) population in context. A supplementary world population data sheet lists statistics on population trends and demographics by region and country, and for more and less developed countries. Abstract

Intended for policymakers, this document focuses on environmental factors limiting the expansion of the world's food supply, the dilemma of fostering third world economic development, and prospects for energy development. The first of five parts gives an overview of a 1983 Population Reference Bureau conference which focused on current thinking about population, resources, and environmental issues. The second part examines current population growth and efforts to curb world population increase. Countries at zero population growth are listed and tables show absolute increases in world population, slowly growing countries, and regions and countries where families average five or more children. The next part discusses short term (technical, political, and social) and long-term obstacles (soil erosion, misuse of irrigation, and altering natural ecosystems) to feeding future populations. The fourth part addresses ecological, economic, and social implications of using fossil fuels in third world countries and explains ways in which the use of fossil fuels can lead to a higher gross national product (GNP) and a corresponding increase in Physical Quality of Life Index (PQLI). Figures showing relationships between GNP, birthrates, and PQLI are given. The final section, policy choices, concludes that curbing population growth is the most important element of bringing about a sustainable world. (LH)

23. The impact of concurrent HIV-infection on women with vulvar cancer: Comparison of clinical characteristics and outcome

Abstract

Background HPV-related vulvar cancer is increasing in prevalence, especially in women living with HIV. Treatment of vulva cancer is based on evidence from HPV-independent cancers, which affect older women. The impact of HIV on vulvar cancer characteristics and treatment outcomes needs to be elucidated.Patients and Methods A retrospective observational study compared the clinical characteristics, treatment, and outcomes of 92 HIV-positive and 131 HIV-negative women with vulvar cancer at our institution. Using descriptive statistics, HIV-positive and negative patients were compared and Cox regression models were tested for differences in mortality and recurrence.Results HIV-positive patients were 20 years younger than HIV-negative patients (p< 0.001). More than 50% of patients presented with advanced stage cancer, however this was independent of HIV-status. Although HIV infection was associated with poorer survival (p= 0.022); rates of cure (p= 0.933) and recurrence rates (p= 0.8) were similar in HIV-positive and negative women.Conclusions Vulvar cancer occurs at a much younger age in women living with HIV. Awareness among HIV-positive women and health care providers would lead to diagnosis of vulvar cancer at an earlier stage. Treatment protocols for HPV-related vulvar cancer should not be altered due to HIV status and should take into consideration the young age of the patients.

33. Dynamic modelling of processes in rivers affected by precipitation runoff

Abstract

In this thesis, models for the dynamics of oxygen and organic matter in receiving waters (such as rivers and creeks), which are affected by rain, are developed. A time series analysis framework is used, but presented with special emphasis on continuous time state space models. Also, the concept of model identifiability is attended. For estimation of the parameters in the models the maximum likelihood method is used and the Kalman filter employed to evaluate the likelihood function. In the case of non-linear models, the extended Kalman filter is used. To evaluate the models, various residual analysis methods and model validation tools are employed. To develop the water quality model, including hydraulic relations and the states of oxygen and organic matter, the qualitative concepts of the physical, biological and chemical models are introduced. The model types used in this thesis are one-dimensional stochastic models. Most of the models are based on measurements from one measuring station, though models based on the linear reservoir description applied to measurements from two measuring stations are also considered. Both time varying and time invariant models are employed. In the models, the oxygen dynamic complex includes reaeration, photosynthesis, respiration and degradation of organic matter. The effect of pre-filtering data is investigated, as is various functions for photosynthesis as a function of solar radiation. For the degradation of organic matter delayed reactions have been studied. In most models, precipitation in the form of rain have been included to study the impact from this. Finally, the future and industrial perspectives are presented, along with a list of suggestions for future research related to the subjects considered in this thesis.

Abstract

Grey-box pharmacokinetic/pharmacodynamic (PK/PD) modelling is presented as a promising way of modelling PK/PD systems. The concept behind grey-box modelling is based on combining physiological knowledge along with information from data in the estimation of model parameters. Grey-box modelling consists of using stochastic differential equations (SDEs) where the stochastic term in the differential equations represents unknown or incorrectly modelled dynamics of the system. The methodology behind the grey-box PK/PD modelling framework for systematic model improvement is illustrated using simulated data and furthermore applied to Bergman's minimal model of glucose kinetics using clinical data from an intravenous glucose tolerance test (IVGTT). The grey-box estimates of the stochastic system noise parameters indicate that the glucose minimal model is too simple and should preferably be revised in order to describe the complicated in vivo system of insulin and glucose following an IVGTT.
