18. Atypical presentations of ectopic pregnancies in a rural tertiary care hospital—A case series.

Abstract

In spite of advances in medical sciences, diagnosis of ectopic pregnancy eludes the clinician. In this case series, we describe three variants of life-threatening pregnancies. The successful management of these cases hinged on a strong index of suspicion and early detection. Heterotopic pregnancy is the occurrence of two pregnancies in different implantation sites at the same time, which is uncommon. Heterotopic pregnancy is uncommon in natural conception, despite its prevalence in assisted reproductive technologies. A strong index of suspicion and timely laparotomy was life-saving for the woman. Caesarean scar pregnancy is a form of ectopic pregnancy in which an aberrant pregnancy is implanted at the hysterotomy site of a prior caesarean procedure. Unexplained excessive bleeding at the time of medical termination of pregnancy prompted us to explore and make a timely diagnosis of caesarean scar pregnancy. Finally, tubal choriocarcinoma is a very aggressive, extremely rare trophoblastic tumour that can be either gestational or non-gestational. High serum beta HCG raised the suspicion and timely laparotomy confirmed the diagnosis. An alert clinician who takes timely decisions can avert life-threatening complications in these pregnancies. 35. Prediction of solubility of etodolac in binary solvents using extended hildebrand solubility approach

Abstract

The solubility of etodolac in binary solvent system i.e. methanol water system was analyzed in term of solute solvent interaction using extended Hildebrand solubility approach (EHSA).The solubility equation employs the term interaction energy (W) to replace the geometric mean (δ1δ2), where δ1 and δ2 are the cohesive energy densities for the solvent and solute, respectively. The new equation provides an accurate prediction of solubility once the interaction energy ‘W’ is obtained. In this case, the energy term is regressed against a polynomial in δ1 of the binary mixture. TisconSonicator A-72 & UV-Visibile spectrophotometer shimadzu UV-1700 were used in prediction & evaluation of solubility of Etodolac. Ideal Solubility of Etodolac was calculated by heat of fusion and absolute melting point obtained from DSC data reported in literature. Molar volume and solubility parameter of Etodolac were calculated from Fedor fragmental constant using floatation & experimentally solubility method. Results showed the solubility of Etodolac in the binary solvent, methanol-water shows a bell-shaped profile with a solubility maximum well below the ideal solubility of the drug. This is an attribute to solvation of the drug with the methanol-water mixture, and indicates that the solute-solvent interaction energy is larger than the geometric mean (δ1δ2) of regular solution. It was observed that in binary Methanol water system maximum solubility occurs at 17.17δ i.e.(30:70) & 21.6 δ (80:20) system.

Abstract

Titration is a method of quantitative chemical analysis which can be used to determine the concentration of a known reactant. Mixed solvency has been widely used to enhance the aqueous solubility of a large number of poorly water soluble drugs. Various organic solvents like methanol, chloroform, dimethyl formamide and ethanol have been employed for solubilization of poorly water soluble drugs. Organic solvents because of their higher cost, toxicities and pollution are not used as solvent. In the present investigation a sodium benzoate, sodium citrate, sodium salicylate solution an economic agent was employed as a hydrotropic solubilizing agent to solubilize the poorly water soluble drug aceclofenac for its titrimetric analysis in bulk sample and tablets precluding the use of organic solvent. The proposed method is new, simple, precise and inexpensive. The results of the analysis have been validated statistically. The mean recoveries were found to be close to 100, indicating the accuracy of the proposed method. Abstract

Background: Ectopic pregnancy remains a leading cause of maternal mortality and morbidity in the first trimester of pregnancy worldwide. Ampullae of the fallopian tubes are the most common site of ectopic pregnancy. They are usually reported to grow upto 8 - 10 week's of gestation before rupture or abortion. Isthmic segment of the tube is the second commonest site of tubal ectopic and they usually get ruptured by 6-8 week's of gestation. Materials & methods: We came across four cases where pregnancy continued upto 18 and 16 weeks in the ampulla and upto 14 weeks in the isthmus of the fallopian tube before rupture. These are very rare events, hence presented here. All the four patients presented with vague pain in lower abdomen and amenorrhoea and features of compensated hypovolemic shock and were managed by urgent laparotomy followed by salpingectomy and were discharged on the seventh post- operative day. Conclusion: We conclude that ectopic pregnancy should be considered even in second trimester as a rare possibility whenever the patient presents with acute abdominal pain and shock.
