1. Machiavellian Behavior and Social-Emotional Functioning in Middle Childhood and Early Adolescence

Abstract

Machiavellianism is an antisocial interpersonal style involving the use of manipulative, deceptive, and coercive behaviors in the pursuit of self-interest. Although widely studied as a "dark" personality trait in adults, relatively little is known about the developmental correlates of Machiavellian tendencies earlier in life. The present study addressed this knowledge gap by examining associations between Machiavellian behavior and three theoretically relevant social-emotional domains-prosocial emotions, emotion recognition skills, and self-control--in a community sample of 7- and 11-year-old Canadian children (N = 300, 50% female). Of particular interest was the extent to which individual differences in social-emotional capacities were uniquely associated with Machiavellian behavior after controlling for aggression, a relatively well-studied antisocial behavior in childhood. Parents reported on children's Machiavellian behavior and aggression. Social-emotional correlates were assessed via parent and child reports, behavioral tasks, and physiological assessments. Machiavellian behavior and aggression were similarly associated with lower parent-reported prosocial emotions and self-control. Machiavellian (but not aggressive) behavior was associated with lower child-reported guilt (a prosocial emotion), higher resting skin conductance levels (reflecting a general tendency for higher arousal), and better anger recognition. Semipartial correlational analyses indicated that these patterns of association with Machiavellian behaviors held independent of aggression. These findings indicate that, by middle childhood, Machiavellian behavior constitutes a distinct form of antisocial conduct that is associated with a unique social-emotional risk profile.

2. Gravito-capillary pinning of pendant droplets under wet uneven surfaces

Abstract

Pendant drops spontaneously appear on the underside of wet surfaces through the Rayleigh-Taylor instability. These droplets have no contact line, they are connected to a thin liquid film with which they exchange liquid and are thus mobile: any perturbation will set them in motion. Here, using experiments, numerical simulations, and theory I show that pendant drops sliding under a slightly tilted wet substrate can pin on topographic defects, despite their lack of contact line. Instead, this pinning force has a gravito-capillary origin: liquid has to moves up or down and the interface has to deforms for the drop the pass the defect. I propose a semi-analytical model for arbitrary substrate topographies that matches the pinning force observed experimentally and numerically, without any fitting parameter. I finally demonstrate how to harness this pinning force to guide pendant drops on complex paths.

Abstract

The interplay between thin film hydrodynamics and solidification produces formidably intricate geophysical structures, such as stalactites and icicles, whose shape is a testimony of their long growth. In simpler settings, liquid films can also produce regular patterns. When coated on the underside of a flat plate, these films are unstable and yield lattices of drops following the Rayleigh-Taylor instability. While this interfacial instability is well-studied in Newtonian fluids, much less is known about what happens when the thin film solidifies. Here, we coat the underside of a surface with liquid elastomer, allowing the film to destabilize and flow while it cures into an elastic solid. Once the first coating yields an array of solid droplets, this iterative coat-flow-cure process is repeated and gives rise to corrugated slender structures, which we name flexicles for their resemblance to icicles. We study the subtle combination of chaos and order that confers our flexicles, their structure, shape, arrangement, and, ultimately, deformability.

Abstract

Solutions of macromolecules can undergo liquid-liquid phase separation to form droplets with ultra-low surface tension. Droplets with such low surface tension wet and spread over common surfaces such as test tubes and microscope slides, complicating \textit{in vitro} experiments. Development of an universal super-repellent surface for macromolecular droplets has remained elusive because their ultra-low surface tension requires low surface energies. Furthermore, nonwetting of droplets containing proteins poses additional challenges because the surface must remain inert to a wide range of chemistries presented by the various amino-acid side-chains at the droplet surface. Here, we present a method to coat microscope slides with a thin transparent hydrogel that exhibits complete dewetting (contact angles $\theta\approx180^\circ)$ and minimal pinning of phase-separated droplets in aqueous solution. The hydrogel is based on a swollen matrix of chemically crosslinked polyethylene glycol diacrylate of molecular weight 12 kDa (PEGDA), and can be prepared with basic chemistry lab equipment. The PEGDA hydrogel is a powerful tool for \textit{in vitro} studies of weak interactions, dynamics, and internal organization of phase-separated droplets in aqueous solutions.

9. Neural Point Catacaustics for Novel-View Synthesis of Reflections

Abstract

View-dependent effects such as reflections pose a substantial challenge for image-based and neural rendering algorithms. Above all, curved reflectors are particularly hard, as they lead to highly non-linear reflection flows as the camera moves. We introduce a new point-based representation to compute Neural Point Catacaustics allowing novel-view synthesis of scenes with curved reflectors, from a set of casually-captured input photos. At the core of our method is a neural warp field that models catacaustic trajectories of reflections, so complex specular effects can be rendered using efficient point splatting in conjunction with a neural renderer. One of our key contributions is the explicit representation of reflections with a reflection point cloud which is displaced by the neural warp field, and a primary point cloud which is optimized to represent the rest of the scene. After a short manual annotation step, our approach allows interactive high-quality renderings of novel views with accurate reflection flow. Additionally, the explicit representation of reflection flow supports several forms of scene manipulation in captured scenes, such as reflection editing, cloning of specular objects, reflection tracking across views, and comfortable stereo viewing. 10. Phase-separated droplets swim to their dissolution

Abstract

Abstract Biological macromolecules can condense into liquid domains. In cells, these condensates form membraneless organelles that can organize chemical reactions. However, little is known about the physical consequences of chemical activity in and around condensates. Working with model bovine serum albumin (BSA) condensates, we show that droplets swim along chemical gradients. Active BSA droplets loaded with urease swim toward each other. Passive BSA droplets show diverse responses to externally applied gradients of the enzyme’s substrate and products. In all these cases, droplets swim toward solvent conditions that favor their dissolution. We call this behavior “dialytaxis”, and expect it to be generic, as conditions which favor dissolution typically reduce interfacial tension, whose gradients are well-known to drive droplet motion through the Marangoni effect. These results could potentially suggest alternative physical mechanisms for active transport in living cells, and may enable the design of fluid micro-robots.

Abstract

A child’s environment and early life experiences play an important role in shaping their development. The COVID-19 pandemic altered many aspects of everyday life for young children in Canada, and it has been argued that these changes did not impact all children equally. The current study explored the association between early child development and neighborhood sociodemographic indices (e.g., income, deprivation) before and after the onset of the COVID-19 pandemic. The Early Development Instrument (EDI) data, measuring child development and school readiness in kindergarten, were collected in 7 Canadian provinces and 1 territory from 2017-2020 (pre-COVID) and 2020-2023 (post-COVID) and were linked with neighborhood-level socioeconomic indices from 2016. We compared the gradients in development in a pre- (n=293,700) and a post-COVID (n=246,305) cohort of Canadian children. Rates of developmental vulnerability, as measured by the EDI, were examined for the sociodemographic variables in both the pre- and post-COVID cohorts. The overall vulnerability increased nationally from 27.3% in the pre- to 28.0% in the post-COVID cohort. A gradient in vulnerability rates for all the socioeconomic indices was found, with children in the highest quintiles, representing greater deprivation, having greater odds of being vulnerable than their peers in the lowest, least deprived quintile. Contrary to expectation, the magnitude of the gradient was very similar in both cohorts, with some jurisdictional differences. The largest gradient was found for average neighborhood income. These findings add the context of social determinants of health to the understanding of the impact of the COVID-19 pandemic on young children.

13. Parent-Child Conversations about Refugee Newcomers Are Associated with Children's Refugee-Specific Prosociality

Abstract

We examined Canadian host-society children's prosociality (i.e., emotions and behaviors that reflect care for the welfare of others) toward refugee newcomer peers and the role of parental socialization (i.e., frequency of parent-child conversations about refugee newcomers) in children's refugee-specific prosociality. The sample included 168 children (ages 6, 9, and 12 years; 51% girls; 58% European ethnicity) and their primary caregivers. We interviewed children to assess their ethical guilt-related emotions (based on emotions and reasoning) in response to a hypothetical vignette depicting prosocial omission involving a refugee newcomer peer or a non-refugee peer (between-subjects manipulation). A donation task was used to assess prosocial behavior wherein children were given the opportunity to donate chocolate coins to a refugee newcomer peer. Parents reported on how often they typically engage in conversations with their children about refugees and about inclusion. Children experienced similar intensities of ethical guilt-related emotions in the refugee compared with the nonrefugee condition, and donations to refugees increased across age groups. Furthermore, children whose parents engaged them in more frequent conversations about refugees expressed stronger ethical guilt-related emotions toward refugee peers (but not toward host-society peers), and donated more to a refugee peer. No significant associations between conversations about inclusion more broadly and refugee-specific prosociality were found. Encouraging parents to have conversations with their children that focus on the experiences of refugees may be important for fostering kindness between refugees and host-society children. Ultimately, these findings may contribute to initiatives that focus on promoting the inclusion of refugee newcomers in their postmigratory societies.

Abstract

The crystalline structure of minerals due to the highly ordered assembly of its constituent atoms, ions or molecules confers a considerable hardness and brittleness to the materials. As a result, they are generally subject to fracture. Here we report that microcrystals of natural inorganic salt hydrates such as sodium sulfate decahydrate (Na2SO4.10H2O) and magnesium sulfate hexahydrate (MgSO4.6H2O) can behave remarkably differently: instead of having a defined faceted geometrical shape and being hard or brittle, they lose their facets and become soft and deformable when in contact with their saturated salt solution at their deliquescence point. As a result, the hydrated crystals simultaneously behaves as crystalline and liquid-like. We show that the observed elasticity is a consequence of a trade-off between the excess capillary energy introduced by the deformation from the equilibrium shape and viscous flow over the surface of the microcrystals. This surprising, unusual mechanical properties reported here reveals the role of the water molecules present in the crystalline structure. Although many compounds can incorporate water molecules in their crystalline frameworks, the relationship between the different hydrates and anhydrate crystal forms are still poorly investigated. Abstract

Abstract The mainstay of acute myeloid leukemia (AML) treatment still relies on traditional chemotherapy, with a survival rate of approximately 30% for patients under 65 years of age and as low as 5% for those beyond. This unfavorable prognosis primarily stems from frequent relapses, resistance to chemotherapy, and limited approved targeted therapies for specific AML subtypes. Around 70% of all AML cases show overexpression of the transcription factor HOXA9, which is associated with a poor prognosis, increased chemoresistance, and higher relapse rates. However, direct targeting of HOXA9 in a clinical setting has not been achieved yet. The dysregulation caused by the leukemic HOXA9 transcription factor primarily results from its binding activity to DNA, leading to differentiation blockade. Our previous investigations have identified two HOXA9/DNA binding competitors, namely DB1055 and DB818. We assessed their antileukemic effects in comparison to HOXA9 knockdown or cytarabine treatment. Using human AML cell models, DB1055 and DB818 induced in vitro cell growth reduction, death, differentiation, and common transcriptomic deregulation but did not impact human CD34+ bone marrow cells. Furthermore, DB1055 and DB818 exhibited potent antileukemic activities in a human THP‐1 AML in vivo model, leading to the differentiation of monocytes into macrophages. In vitro assays also demonstrated the efficacy of DB1055 and DB818 against AML blasts from patients, with DB1055 successfully reducing leukemia burden in patient‐derived xenografts in NSG immunodeficient mice. Our findings indicate that inhibiting HOXA9/DNA interaction using DNA ligands may offer a novel differentiation therapy for the future treatment of AML patients dependent on HOXA9.

16. Gravity-driven coatings on curved substrates: a differential geometry approach

Abstract

Although the drainage and spreading processes of thin liquid films on substrates have received growing attention during the last decades, the study of three-dimensional cases is limited to a few studies on flat and axisymmetric substrates. In this work, we exploit differential geometry to study the drainage and spreading of thin films on generic curved substrates. We initially investigate the drainage and spreading processes on spheroidal and paraboloidal substrates by employing an asymptotic expansion in the vicinity of the pole and a self-similar approach, finding that the thickness distribution is set by the substrate metric and tangential gravity force components. Spheroids with a large ratio between height and equatorial radius are characterized by a growing thickness moving away from the pole, and vice versa. The non-symmetric coating on a toroidal substrate shows that larger thicknesses and a faster spreading are attained on the inner region than on the outer region of the torus. An ellipsoid with three different axes is chosen as a testing ground for three-dimensional drainage and spreading. Modulations in the drainage solution are observed, with a different variation of the thickness along the two axes. By imposing the conservation of mass, an analytical solution for the average spreading front is obtained. The analytical and numerical results are in good agreement. 18. Enhancing Immunotherapy Response Prediction in Metastatic Lung Adenocarcinoma: Leveraging Shallow and Deep Learning with CT-Based Radiomics across Single and Multiple Tumor Sites

Abstract

This study aimed to evaluate the potential of pre-treatment CT-based radiomics features (RFs) derived from single and multiple tumor sites, and state-of-the-art machine-learning survival algorithms, in predicting progression-free survival (PFS) for patients with metastatic lung adenocarcinoma (MLUAD) receiving first-line treatment including immune checkpoint inhibitors (CPIs). To do so, all adults with newly diagnosed MLUAD, pre-treatment contrast-enhanced CT scan, and performance status ≤ 2 who were treated at our cancer center with first-line CPI between November 2016 and November 2022 were included. RFs were extracted from all measurable lesions with a volume ≥ 1 cm3 on the CT scan. To capture intra- and inter-tumor heterogeneity, RFs from the largest tumor of each patient, as well as lowest, highest, and average RF values over all lesions per patient were collected. Intra-patient inter-tumor heterogeneity metrics were calculated to measure the similarity between each patient lesions. After filtering predictors with univariable Cox p < 0.100 and analyzing their correlations, five survival machine-learning algorithms (stepwise Cox regression [SCR], LASSO Cox regression, random survival forests, gradient boosted machine [GBM], and deep learning [Deepsurv]) were trained in 100-times repeated 5-fold cross-validation (rCV) to predict PFS on three inputs: (i) clinicopathological variables, (ii) all radiomics-based and clinicopathological (full input), and (iii) uncorrelated radiomics-based and clinicopathological variables (uncorrelated input). The Models’ performances were evaluated using the concordance index (c-index). Overall, 140 patients were included (median age: 62.5 years, 36.4% women). In rCV, the highest c-index was reached with Deepsurv (c-index = 0.631, 95%CI = 0.625–0.647), followed by GBM (c-index = 0.603, 95%CI = 0.557–0.646), significantly outperforming standard SCR whatever its input (c-index range: 0.560–0.570, all p < 0.0001). Thus, single- and multi-site pre-treatment radiomics data provide valuable prognostic information for predicting PFS in MLUAD patients undergoing first-line CPI treatment when analyzed with advanced machine-learning survival algorithms.

Abstract

Summary: Background: Although immunomodulators have established benefit against the new coronavirus disease (COVID-19) in general, it is uncertain whether such agents improve outcomes without increasing the risk of secondary infections in the specific subgroup of previously immunocompromised patients. We assessed the effect of immunomodulators on outcomes of immunocompromised patients hospitalized for COVID-19. Methods: The protocol was prospectively registered with PROSPERO (CRD42022335397). MEDLINE, Cochrane Central Register of Controlled Trials and references of relevant articles were searched up to 01-06-2022. Authors of potentially eligible randomized controlled trials were contacted to provide data on immunocompromised patients randomized to immunomodulators vs control (i.e., placebo or standard-of-care). Findings: Eleven randomized controlled trials involving 397 immunocompromised patients hospitalized for COVID-19 were included. Ten trials had low risk of bias. There was no difference between immunocompromised patients randomized to immunomodulators vs control regarding mortality [30/182 (16.5%) vs 41/215 (19.1%); RR 0.93, 95% CI 0.61–1.41; p = 0.74], secondary infections (RR 1.00, 95% CI 0.64–1.58; p = 0.99) and change in World Health Organization ordinal scale from baseline to day 15 (weighed mean difference 0.27, 95% CI -0.09–0.63; p = 0.15). In subgroup analyses including only patients with hematologic malignancy, only trials with low risk of bias, only trials administering IL-6 inhibitors, or only trials administering immunosuppressants, there was no difference between comparators regarding mortality. Interpretation: Immunomodulators, compared to control, were not associated with harmful or beneficial outcomes, including mortality, secondary infections, and change in ordinal scale, when administered to immunocompromised patients hospitalized for COVID-19. 23. Physical activity and unexpected weight change in Ontario children and youth during the COVID-19 pandemic: A cross-sectional analysis of the Ontario Parent Survey 2.

Abstract

The objective of this study was to investigate the association between children's parent-reported physical activity levels and weight changes during the COVID-19 pandemic among children and youth in Ontario Canada. A cross-sectional online survey was conducted in parents of children 5-17 years living in Ontario from May to July 2021. Parents recalled their child's physical activity and weight change during the year prior to their completion of the survey. Odds ratios (OR) and 95% confidence intervals (CI) were estimated using multinomial logistic regression for the association between physical activity and weight gain or loss, adjusted for child age and gender, parent ethnicity, current housing type, method of school delivery, and financial stability. Overall, 86.8% of children did not obtain 60 minutes of moderate-to-vigorous physical activity per day and 75.4% of parents were somewhat or very concerned about their child's physical activity levels. For all physical activity exposures (outdoor play, light physical activity, and moderate-to-vigorous physical activity), lower physical activity was consistently associated with increased odds of weight gain or loss. For example, the adjusted OR for the association between 0-1 days of moderate-to-vigorous physical activity versus 6-7 days and child weight gain was 5.81 (95% CI 4.47, 7.56). Parent concern about their child's physical activity was also strongly associated with child weight gain (OR 7.29; 95% CI 5.94, 8.94). No differences were observed between boys and girls. This study concludes that a high proportion of children in Ontario had low physical activity levels during the COVID-19 pandemic and that low physical activity was strongly associated with parent reports of both weight gain and loss among children.

Abstract

IntroductionPrevalence rates of emotional and behavior problems (EBP) in autistic children and youth are high (40–70%), and often cause severe and chronic impairment. Furthermore, autistic children are also more likely to experience family “social-ecological” adversity compared to neurotypically developing peers, including social isolation, child maltreatment, caregiver mental illness, and socioeconomic risk. These family stressors increase the risk of co-occurring EBP among autistic children and can often impede access to evidence-based care, thus amplifying long-term health inequities for autistic children and their caregivers. In the current autism services landscape, there are few scalable, evidence-based programs that adequately address these needs. The Family Check-Up (FCU®) is a brief, strength-based, and tailored family-centered intervention that supports positive parenting and explicitly assesses the social determinants of child and family mental health within an ecological framework. Studies have demonstrated long-term positive child and caregiver outcomes in other populations, but the FCU® has not been evaluated in families of autistic children and youth. Therefore, we aimed to evaluate FCU® implementation within an established, publicly funded Autism Program in Ontario, Canada, with delivery by autism therapists, to demonstrate sustainable effectiveness within real-world settings.MethodsIn this study, we outline the protocol for a hybrid implementation-effectiveness approach with two key components: (1) A parallel-arm randomized controlled trial of N = 80 autistic children/youth (ages 6–17 years) and high levels of EBP and their caregivers. Primary and secondary outcomes include child EBP, and caregiver well-being and parenting. (2) A mixed methods implementation study, to describe facilitators and barriers to implementation of the FCU® within an autism service setting.DiscussionScalable, ecologically focused family-centered interventions offer promise as key components of a public health framework aimed at reducing mental health inequities among autistic children, youth, and their caregivers. Results of this study will inform further program refinement and scale-up.

28. Too fast to grow: Dynamics of pendant drops sliding on a thin film

Abstract

Pendant drops suspended on the underside of a wet substrate are known to accumulate fluid from the surrounding thin liquid film, a process that often results in dripping. The growth of such drops is hastened by their ability to translate over an otherwise uniform horizontal film. Here we show that this scenario is surprisingly reversed when the substrate is slightly tilted ($\approx 2$ deg) ; drops become too fast to grow and shrink over the course of their motion. Combining experiments and numerical simulations, we rationalize the transition between the conventional growth regime and the previously unknown decay regime we report. Using an analytical treatment of the Landau-Levich meniscus that connects the drop to the film, we quantitatively predict the drop dynamics in the two flow regimes and the value of the critical inclination angle where the transition between them occurs.

Abstract

Our aim was to investigate which features were associated with clinical successes at short- and mid-terms following prostate artery embolization (PAE) for symptomatic benign prostate hypertrophy (BPH). All adults treated by PAE for BPH at our referral center between January 2017 and March 2021, with pre-treatment MRI, technical success, and follow-up at 3 months and 2 years were included in this single-center retrospective study. Radiologists reviewed the prostatic protrusion index (PPI), adenomatous dominant BPH (adBPH), and Wasserman classification on pre-treatment MRI. Radiomics analysis was achieved on the transitional zone on pre-treatment T2-weighted imaging (WI) and ADC, and comprised reproducibility assessment, unsupervised classifications, and supervised radiomics scores obtained with cross-validated Elasticnet regressions. Eighty-eight patients were included (median age: 65 years), with 81.8% clinical successes at 3 months and 60.2% at 2 years. No feature was associated with success at 3 months, except the radiomics score trained on T2-WI and ADC (AUROC = 0.694). Regarding success at 2 years, no radiomics approaches provided significant performances; however, Wasserman type-1 and change in international prostate symptom score (IPSS) at 3 months ≤ −35% were associated with success in multivariable analysis (OR = 5.82, p = 0.0296, and OR = 9.04, p = 0.0002). Thus, while radiomics provided limited interest, Wasserman classification and early IPSS changes appeared predictive of mid-term outcomes.

Abstract

From everyday experience, we all know that a solid edge can deflect a liquid flowing over it significantly, up to the point where the liquid completely sticks to the solid. Although important in pouring, printing and extrusion processes, there is no predictive model of this so-called "teapot effect". By grazing vertical cylinders with inclined capillary liquid jets, we here use the teapot effect to attach the jet to the solid and form a new structure: the liquid helix. 39. A Longitudinal Investigation of the Happy Victimizer Tendency in Childhood: A Matter of Control or Care?

Abstract

This study examined the course and correlates of the happy victimizer tendency--the expectation that harming others to achieve a goal will result in positive emotional outcomes for the transgressor--from 4 to 6 years of age in a community sample of Canadian children (N = 150; 50% female; Time 1 M[subscript age] = 4.53 years, SD = 0.30; 33% European background). At each time point, we conducted semistructured interviews with children to assess their emotion expectancies and reasoning in response to hypothetical acts of aggression. Caregivers (84% mothers; 91% college diploma or higher; 41% yearly household income [greater than or equal to]$125,000 CAD [approximately $90,500 USD]) provided ratings of children's inhibitory control and sympathy at study onset. Most 4-year-old children expected to feel good after victimizing others, but this tendency declined through age 6. Children higher in sympathy were less likely to happy victimize at age 4, whereas greater inhibitory control predicted faster decreases in happy victimizing over time.

43. Wrinkles, folds and plasticity in granular rafts

Abstract

We investigate the mechanical response of a compressed monolayer of large and dense particles at a liquid-fluid interface: a granular raft. Upon compression, rafts first wrinkle; then, as the confinement increases, the deformation localizes in a unique fold. This characteristic buckling pattern is usually associated to floating elastic sheets and as a result, particle laden interfaces are often modeled as such. Here, we push this analogy to its limits by comparing the first quantitative measurements of the raft morphology to a theoretical continuous elastic model of the interface. We show that although powerful to describe the wrinkle wavelength, the wrinkle-to-fold transition and the fold shape, this elastic description does not capture the finer details of the experiment. We describe an unpredicted secondary wavelength, a compression discrepancy with the model and a hysteretic behavior during compression cycles, all of which are a signature of the intrinsic discrete and frictional nature of granular rafts. Abstract

BackgroundThe COVID-19 pandemic has particularly burdened pregnant and postpartum women. It remains unclear how distress levels of pregnant and postpartum people have changed (or persisted) as the pandemic continues on and which factors may contribute to these trajectories of distress.MethodsThis longitudinal study included 304 pregnant people, who were followed during pregnancy, 6-weeks, 6-months and 15-months postpartum. At each time point, a latent “distress” factor was estimated using self-reported depressive symptoms, anxiety symptoms, and stress. Reported negative impact of COVID-19 and social support were assessed during pregnancy as risk and protective factors related to distress. Second-order latent growth curve modeling with a piecewise growth function was used to estimate initial levels and changes in distress over time.ResultsMean distress was relatively stable from the pregnancy to 6-weeks postpartum and then declined from 6-weeks to 15-months postpartum. Higher education, greater social support, and lower negative impact of COVID-19 were associated with a lower distress during pregnancy. Unexpectedly, negative impact of COVID-19 was associated with a faster decrease in distress and more social support was associated with a greater increase in distress from pregnancy to 6-weeks postpartum. However, these effects became non-significant after controlling for distress during pregnancy.ConclusionFindings indicate high but declining levels of distress from pregnancy to the postpartum period. Changes in distress are related to social support and the negative impact of the pandemic in pregnancy. Findings highlight the continued impact of COVID-19 on perinatal mental health and the need for support to limit the burden of this pandemic on pregnant people and families.

Abstract

Objectives Women of low socioeconomic status have been described as having suboptimal prenatal care, which in turn has been associated with poor pregnancy outcomes. Many types of conditional cash transfer (CCT) programmes have been developed, including programmes to improve prenatal care or smoking cessation during pregnancy, and their effects demonstrated. However, ethical critiques have included paternalism and lack of informed choice. Our objective was to determine if women and healthcare professionals (HPs) shared these concerns.Design Prospective qualitative research.Setting We included economically disadvantaged women, as defined by health insurance data, who participated in the French NAITRE randomised trial assessing a CCT programme during prenatal follow-up to improve pregnancy outcomes. The HP worked in some maternities participating in this trial.Participants 26 women, 14 who received CCT and 12 who did not, mostly unemployed (20/26), and - 7 HPs.Interventions We conducted a multicentre cross-sectional qualitative study among women and HPs who participated in the NAITRE Study to assess their views on CCT. The women were interviewed after childbirth.Results Women did not perceive CCT negatively. They did not mention feeling stigmatised. They described CCT as a significant source of aid for women with limited financial resources. HP described the CCT in less positive terms, for example, expressing concern about discussing cash transfer at their first medical consultation with women. Though they emphasised ethical concerns about the basis of the trial, they recognised the importance of evaluating CCT.Conclusions In France, a high-income country where prenatal follow-up is free, HPs were concerned that the CCT programme would change their relationship with patients and wondered if it was the best use of funding. However, women who received a cash incentive said they did not feel stigmatised and indicated that these payments helped them prepare for their baby’s birth.Trial registration number NCT02402855
