3. Irinotecan: promising activity in the treatment of malignant glioma

Abstract

Patients with malignant glioma continue to have a dismal outcome. Those with glioblastoma multiforme, the most common type of malignant glioma, have a median survival of 40 to 60 weeks following diagnosis and only 16 to 24 weeks after recurrence. While standard therapy is surgery and external-beam radiotherapy, data indicate that chemotherapy improves the clinical outcome of some patients. Irinotecan (CPT-11, Camptosar) possesses significant activity against malignant glioma, and potentiation of this activity with combination partners, including the alkylator 1,2-bis(2-chloroethyl)-1-nitrosourea (carmustine, BCNU), is being evaluated in an ongoing phase II study. Also, the combination of irinotecan plus temozolomide (Temodar) has produced synergism in preclinical studies as well as encouraging responses in ongoing clinical trials. Other investigative directions include studies of irinotecan plus temozolomide and O6-benzylguanine (O6-BG) to assess the ability of O6-BG to maximize the therapeutic benefits of irinotecan combined with temozolomide.

Abstract

We describe a 7-year-old girl with therapy-related acute myeloid leukemia (AML) associated with a single and novel karyotypic abnormality. The patient had been treated with alkylating agents and etoposide for hypothalamic pilocytic astrocytoma at age 17 months, and developed mixed lineage AML. Cytogenetic analysis of the leukemic blasts showed 46,XX,der(7)t(7;11)(q22;q14) in all cells examined. Southern blot analysis revealed three copies of an unrearranged MLL gene on chromosome 11q. This is the first report of a triplicated, unrearranged MLL gene in association with a deletion of 7q anomaly and an unbalanced translocation in therapy-related leukemia.

Abstract

An automated technique is reported for measuring erythrocyte 2,3-diphosphoglycerate (DPG). We have automated a specific method, in which the DPG phosphatase activity of phosphoglycerate mutase is used and potentiated by phosphoglycolate. Precision, accuracy, recovery, and comparability with a reference method are discussed.

Abstract

Three types of resin columns for determining serum thyroxine (T 4 ) were evaluated in this study. There were two types of commercial columns and the third type was a column prepared in our laboratory. The three types of columns produced significantly different results when compared after the initial use and when compared after regeneration and subsequent usages. In general, the laboratory columns were equal or superior to those obtained commercially. For large volume users, the preparation of one's own columns and their regeneration and reuse can represent a worthwhile economy.

Abstract

A turbidimetric test for hemoglobin S and other sickling hemoglobins has been adapted to the Auto-Analyzer. This procedure utilizes the lack of solubility of sickling hemoglobins in a phosphate buffer reagent. It is inexpensive and the results are comparable to those of other screening tests.

Abstract

Serum glucose measurements were performed on paired specimens from patients with uremia, before and after dialysis, to evaluate the automated glucose oxidase, ortho-toluidine, ferricyanide, and copper reduction methods. All methods were found to yield results statistically different from those of the glucose oxidase method whether glucose was measured before or after dialysis. Before dialysis, the mean difference between the ortho-toluidine and glucose oxidase method results was −9.0 mg. per dl., and after dialysis it was −2.8 mg. per dl.; other methods differed more from glucose oxidase. The glucose oxidase method yielded 2 mg. per dl. or less for uremic sera depleted of glucose by yeast; other methods were less specific for glucose. High uric acid concentrations depressed ortho-toluidine values. Fructose caused slight elevations in ortho-toluidine and large elevations in ferricyanide values. In view of the above findings, the glucose oxidase method is the method of choice for determining glucose values in patients with uremia. Results from 379 nonuremic sera determined simultaneously by the ortho-toluidine and ferricyanide methods were also compared. In nonuremic patients, serum fructose elevation after glucose ingestion partially accounts for the differences between the results of the ortho-toluidine and the ferricyanide methods observed.

Abstract

Benign adenoma of the middle ear emerged as a distinct entity following a review of 20 cases as reported from the Armed Forces Institute of Pathology in 1976. We report 4 additional cases and note that the pathologic diagnosis is not so clear cut. Benign primary adenoma may be confused with adenocarcinoma, ceruminoma, and glomus tumor. The otologic surgeon must take a hard look at the clinical signs and symptoms to determine if they are compatible with the diagnosis of primary middle ear adenoma. While middle ear adenoma has recently been implicated as a cause of facial paralysis (including a case herein), bone destruction should never be found in concert with this neoplasm.

Abstract

We have devised a rapid screening test for use in detecting sickling hemoglobins. This test depends on the insolubility of sickling hemoglobins in 2.24 molar phosphate buffer. Sickling hemoglobins form a turbid suspension in this test, in which positive and negative results, respectively, appear very much like those for the "Sickledex" test.
