PURPOSE The aim of this pilot study was to prospectively evaluate the first integrated positron emission tomography (PET)/dual-energy computed tomography (DECT) system performance in patients with non-small cell lung cancer (NSCLC). MATERIALS AND METHODS In this single-center, prospective trial, consecutive patients with NSCLC referred for a PET study between May 2017 and June 2018 were enrolled. All patients received contrast-enhanced imaging on a clinical PET/DECT system. Data analysis included PET-based standard uptake values (SUVmax) and DECT-based iodine densities of tumor masses, lymph nodes, and distant metastases. Results were analyzed using correlation tests and receiver operating characteristics curves. RESULTS The study population was composed of 21 patients (median age 62 y, 14 male patients). Central airway obstruction (CAO) is a dangerous and increasingly common problem. CAO refers to lesions causing narrowing of the trachea or mainstem bronchi and is generally divided into malignant and nonmalignant categories. These 2 entities may be caused by a variety of thoracic and extrathoracic diseases. Imaging is critical during the initial assessment of CAO and may help thoracic physicians focus the differential diagnosis and plan the safest and most appropriate diagnostic and therapeutic interventions. However, direct visualization via flexible or rigid bronchoscopy is often necessary for diagnostic and treatment purposes. A large number of procedures can be performed through bronchoscopy, with the goal of relieving the obstruction and improving patency of the airway. Deciding which procedure to perform is based both upon the type of lesion and whether the lesion is due to a malignant or nonmalignant process. Possible interventions include mechanical debridement, laser therapy, argon plasma coagulation, electrocautery, brachytherapy, and stent placement. Immediate postoperative and follow-up imaging is crucial to monitor for immediate, subacute, and chronic complications as well as disease progression and recurrence.

Chronic dyspnea may result from a variety of disorders of cardiovascular, pulmonary, gastrointestinal, neuromuscular, systemic, and psychogenic etiology. This article discusses guidelines for the initial imaging of six variants for chronic dyspnea of noncardiovascular origin: (1) Chronic dyspnea of unclear etiology; (2) Chronic dyspnea with suspected chronic obstructive pulmonary disease; (3) Chronic dyspnea with suspected central airways disease; (4) Chronic dyspnea with suspected interstitial lung disease; (5) Chronic dyspnea with suspected disease of the pleura or chest wall; and (6) Chronic dyspnea with suspected diaphragm dysfunction. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. The guideline development and revision include an extensive analysis of current medical literature from peer reviewed journals and the application of well-established methodologies (RAND/UCLA Appropriateness Method and Grading of Recommendations Assessment, Development, and Evaluation or GRADE) to rate the appropriateness of imaging and treatment procedures for specific clinical scenarios. BACKGROUND: Improved imaging techniques have increased the incidence of subsegmental pulmonary embolism (ssPE). Indirect evidence is suggesting that ssPE may represent a more benign presentation of venous thromboembolism not necessarily requiring anticoagulant treatment. However, correctly diagnosing ssPE is challenging with reported low interobserver agreement, partly due to the lack of widely accepted diagnostic criteria. OBJECTIVES: We sought to derive uniform diagnostic criteria for ssPE, guided by expert consensus. METHODS: Based on an extensive literature review and expert opinion of a Delphi steering committee, two surveys including statements regarding diagnostic criteria and management options for ssPE were established. These surveys were conducted electronically among two panels, respectively: expert thoracic radiologists and clinical venous thromboembolism specialists. The Delphi method was used to achieve consensus after multiple survey rounds. Consensus was defined as a level of agreement >70%. RESULTS: Twenty-nine of 40 invited radiologists (73%) and 40 of 51 clinicians (78%) participated. Following two survey rounds by the expert radiologists, consensus was achieved on 15 of 16 statements, including on the established diagnostic criteria for ssPE (96% agreement): a contrast defect in a subsegmental artery, that is, the first arterial branch division of any segmental artery independent of artery diameter, visible in at least two subsequent axial slices, using a computed tomography scanner with a desired maximum collimator width of ≤1 mm. These criteria were approved by 83% of the clinical venous thromboembolism (VTE) specialists. The clinical expert panel favored anticoagulant treatment in case of prior VTE, antiphospholipid syndrome, pregnancy, cancer, and proximal deep vein thrombosis. Lung cancer screening has been shown in the National Lung Screening Trial (NLST) to result in a statistically significant decrease in lung cancer specific mortality. Also within that trial there was shown to be a 7% decrease in all-cause mortality. While the reasons for this benefit are not entirely clear, it may relate to the detection and treatment of other important findings. Smokers not only have a higher risk of lung cancer, but also increased risk of atherosclerosis. The latter can be detected by the discovery of aortic and/or coronary artery calcium on unenhanced CT. As coronary artery calcium scoring can be used as a screening tool to detect asymptomatic coronary artery atherosclerosis, its detection on lung cancer screening exams has the potential to provide both a teachable moment and treatment aimed at the reduction of major coronary artery events and mortality. In this review we will discuss the use of coronary artery calcium scoring for the detection of atherosclerotic disease and its potential application to lung cancer screening populations.

Summary Background Immunotherapy with PD-1 or PD-L1 blockade fails to induce a response in about 80% of patients with unselected non-small cell lung cancer (NSCLC), and many of those who do initially respond then develop resistance to treatment. Agonists that target the shared interleukin-2 (IL-2) and IL-15Rβγ pathway have induced complete and durable responses in some cancers, but no studies have been done to assess the safety or efficacy of these agonists in combination with anti-PD-1 immunotherapy. We aimed to define the safety, tolerability, and activity of this drug combination in patients with NSCLC. Methods In this non-randomised, open-label, phase 1b trial, we enrolled patients (aged ≥18 years) with previously treated histologically or cytologically confirmed stage IIIB or IV NSCLC from three academic hospitals in the USA. Key eligibility criteria included measurable disease, eligibility to receive anti-PD-1 immunotherapy, and an Eastern Cooperative Oncology Group performance status of 0 or 1. Patients received the anti-PD-1 monoclonal antibody nivolumab intravenously at 3 mg/kg (then 240 mg when US Food and Drug Administration [FDA]-approved dosing changed) every 14 days (either as new treatment or continued treatment at the time of disease progression) and the IL-15 superagonist ALT-803 subcutaneously once per week on weeks 1–5 of four 6-week cycles for 6 months. ALT-803 was administered at one of four escalating dose concentrations: 6, 10, 15, or 20 μg/kg. The primary endpoint was to define safety and tolerability and to establish a recommended phase 2 dose of ALT-803 in combination with nivolumab. Analyses were per-protocol and included any patients who received at least one dose of study treatment. This trial is registered with ClinicalTrials.gov, number NCT02523469; phase 2 enrolment of patients is ongoing. Findings Between Jan 18, 2016, and June 28, 2017, 23 patients were enrolled and 21 were treated at four dose levels of ALT-803 in combination with nivolumab. Two patients did not receive treatment because of the development of inter-current illness during enrolment, one patient due to leucopenia and one patient due to pulmonary dysfunction. No dose-limiting toxicities were recorded and the maximum tolerated dose was not reached. The most common adverse events were injection-site reactions (in 19 [90%] of 21 patients) and flu-like symptoms (15 [71%]). The most common grade 3 adverse events, occurring in two patients each, were lymphocytopenia and fatigue. A grade 3 myocardial infarction occurred in one patient. No grade 4 or 5 adverse events were recorded. The recommended phase 2 dose of ALT-803 is 20 μg/kg given once per week subcutaneously in combination with 240 mg intravenous nivolumab every 2 weeks. Interpretation ALT-803 in combination with nivolumab can be safely administered in an outpatient setting. The promising clinical activity observed with the addition of ALT-803 to the regimen of patients with PD-1 monoclonal antibody relapsed and refractory disease shows evidence of anti-tumour activity for a new class of agents in NSCLC. Objective The aim of this study was to determine prognostic value of tumor size and metabolic activity on survival for patients with early stage nonsmall cell lung cancer receiving stereotactic body radiation therapy. Methods We retrospectively evaluated the patients who underwent positron emission tomography-computed tomography scan before stereotactic body radiation therapy treatment. Tumor diameter, tumor volume, maximum standardized uptake value (SUVmax), standardized uptake value (SUV) average, and SUV volume were obtained. Cox regression analyses were performed to determine the associations between tumor characteristics and survival. Results The patients with large tumors and high SUVmax have worse survival than patients with small tumors and low SUVmax (hazard ratio [HR] = 3.47, P = 0.007). Patients with small tumors and high SUVmax (HR = 1.80; P = 0.24) and large tumors and low SUVmax (HR = 1.55; P = 0.43) had increased risk of death compared with patients with small tumors and low SUVmax. Conclusions Both increased tumor size and metabolic activity are associated with increased risk of death. Combining size and metabolic activity together is superior for predicting 2-year survival and identifying patients for whom survival is statistically worse.

Pulmonary embolism (PE) remains a common and important clinical condition that cannot be accurately diagnosed on the basis of signs, symptoms, and history alone. The diagnosis of PE has been facilitated by technical advancements and multidetector CT pulmonary angiography, which is the major diagnostic modality currently used. Ventilation and perfusion scans remain largely accurate and useful in certain settings. Lower-extremity ultrasound can substitute by demonstrating deep vein thrombosis; however, if negative, further studies to exclude PE are indicated. In all cases, correlation with the clinical status, particularly with risk factors, improves not only the accuracy of diagnostic imaging but also overall utilization. Other diagnostic tests have limited roles. Background Structured reports have been shown to improve communication between radiologists and providers. However, some radiologists are concerned about resultant decreased workflow efficiency. We tested a machine learning-based algorithm designed to convert unstructured computed tomography pulmonary angiography (CTPA) reports into structured reports. Methods A self-supervised convolutional neural network-based algorithm was trained on a dataset of 475 manually structured CTPA reports. Labels for individual statements included “pulmonary arteries,” “lungs and airways,” “pleura,” “mediastinum and lymph nodes,” “cardiovascular,” “soft tissues and bones,” “upper abdomen,” and “lines/tubes.” The algorithm was applied to a test set of 400 unstructured CTPA reports, generating a predicted label for each statement, which was evaluated by two independent observers. Per-statement accuracy was calculated based on strict criteria (algorithm label counted as correct if the statement unequivocally contained content only related to that particular label) and a modified criteria, accounting for problematic statements, including typographical errors, statements that did not fit well into the classification scheme, statements containing content for multiple labels, etc. Results Of the 4,157 statements, 3,806 (91.6%) and 3,986 (95.9%) were correctly labeled by the algorithm using strict and modified criteria, respectively, while 274 (6.6%) were problematic for the manual observers to label, the majority of which (n = 173) were due to more than one section being included in one statement. Conclusion This algorithm showed high accuracy in converting free-text findings into structured reports, which could improve communication between radiologists and clinicians without loss of productivity and provide more structured data for research/data mining applications.

The immunocompromised patient with an acute respiratory illness (ARI) may present with fever, chills, weight loss, cough, shortness of breath, or chest pain. The number of immunocompromised patients continues to rise with medical advances including solid organ and stem cell transplantation, chemotherapy, and immunomodulatory therapy, along with the continued presence of human immunodeficiency virus and acquired immunodeficiency syndrome. Given the myriad of pathogens that can infect immunocompromised individuals, identifying the specific organism or organisms causing the lung disease can be elusive. Moreover, immunocompromised patients often receive prophylactic or empiric antimicrobial therapy, further complicating diagnostic evaluation. Noninfectious causes for ARI should also be considered, including pulmonary edema, drug-induced lung disease, atelectasis, malignancy, radiation-induced lung disease, pulmonary hemorrhage, diffuse alveolar damage, organizing pneumonia, lung transplant rejection, and pulmonary thromboembolic disease. As many immunocompromised patients with ARI progress along a rapid and potentially fatal course, timely selection of appropriate imaging is of great importance in this setting. The aim of this prospective study is to investigate the diagnostic performance of integrated positron emission tomography (PET) /dual-energy computed tomography (DECT) imaging in determining the thoracic nodal status of patients with small-cell lung cancer (SCLC) or non-small-cell lung cancer (NSCLC) and the resulting impact on target volume delineation for radiation therapy planning. This is a single-center prospective study including approximately 50 patients with suspected or confirmed SCLC or NSCLC, referred for a PET study. All patients will be examined on a clinical PET/DECT system, where a dual-energy detector was recently installed. The patient will be placed in the system 70 min after the administration of 5 MBq/kg of 18F-fluorodeoxyglucose (18F-FDG). Then, DECT will be acquired after the injection of 100 mL of iodine contrast medium. A PET scan will be acquired from the top of the skull through the inguinal region. Data analysis will be performed on the PET, CT, and iodine map datasets. Information regarding tumor detection, adenopathies, and radiation therapy planning will be assessed based on all three sets of images by two experienced radiologists. Evidence-based imaging (EBI) requires the critical assessment and application of the best available evidence to patient imaging. Unfortunately, the published studies that comprise the available evidence are often limited by bias, small sample size, and methodological inadequacy. Further, the information provided in published reports may be insufficient to allow estimation of the quality of the research. Initiatives by journal editors to improve the reporting of research studies, including the CONSORT (1), STARD (2), SQUIRE (3), and others, provide useful guides but are incompletely implemented. The objective of this chapter is to summarize the common sources of error and bias in the imaging literature to guide the critical assessment required for EBI.

The respiratory system is often affected by complications of immunodeficiency, typically manifesting clinically as acute respiratory illness. Ongoing literature reviews regarding the appropriateness of imaging in these patients are critical, as advanced medical therapies including stem cell transplantation, chemotherapy, and immunosuppressive therapies for autoimmune disease continue to keep the population of immunosuppressed patients in our health care system high. This ACR Appropriateness Criteria topic describes clinical scenarios of acute respiratory illness in immunocompromised patients with cough, dyspnea, chest pain, and fever, in those with negative, equivocal, or nonspecific findings on chest radiography, in those with multiple, diffuse, or confluent opacities on chest radiography, and in those in whom noninfectious disease is suspected. Thoracic outlet syndrome is a clinical entity characterized by compression of the neurovascular bundle, and may be associated with additional findings such as venous thrombosis, arterial stenosis, or neurologic symptoms. The goal of imaging is to localize the site of compression, the compressing structure, and the compressed organ or vessel, while excluding common mimics. A literature review is provided of current indications for diagnostic imaging, with discussion of potential limitations and benefits of the respective modalities. Multidetector computed tomography (CT) plays an important role in the detection, risk stratification and prognosis evaluation of acute pulmonary embolism. This review will discuss the technical improvements for imaging peripheral pulmonary arteries, the methods of assessing pulmonary embolism severity based on CT findings, a multidetector CT technique for pulmonary embolism detection, and lastly, how to avoid overutilization of CT pulmonary angiography and overdiagnosis of pulmonary embolism. Introduction Cystic lung disease represents a rare set of conditions where the pulmonary parenchyma is replaced to some degree by cystic air spaces. Although scattered cysts may occasionally be found in the normal or near-normal lungs because of tobacco using or aging, multiple cysts are often indicative of underlying disease and should initiate a search for the underlying diagnosis. The purpose of this review is to categorize the imaging findings and clinical presentation associated with common cystic lung diseases.We also discuss the differentiation of these conditions with commonmimics of diffuse cystic lung disease. Finally, we discuss the application of quantitative imaging for use as an objective measure of the extent of disease.

Background Incorporating tobacco treatment within lung cancer screening programs has the potential to influence cessation in high-risk smokers. We aimed to better understand the characteristics of smokers within a screening cohort, correlate those variables with downstream outcomes, and identify predictors of continued smoking. Methods This study is a secondary analysis of the National Lung Screening Trial randomized clinical study. Tobacco dependence was evaluated by using the Fagerstrӧm Test for Nicotine Dependence, the Heaviness of Smoking Index, and time to first cigarette (TTFC); descriptive statistics were performed. Clinical outcomes (smoking cessation, lung cancer, and mortality) were assessed with descriptive statistics and χ2 tests stratified according to nicotine dependence. Logistic and Cox regression models were used to study the influence of dependence on smoking cessation and mortality, respectively. Results Patients with high dependence scores were less likely to quit smoking compared with low dependence smokers (TTFC OR, 0.50 [95% CI, 0.42-0.60]). Indicators of high dependence, as measured according to all three metrics, were associated with worsening clinical outcomes. TTFC showed that patients who smoked within 5 min of waking (indicating higher dependence) had higher rates of lung cancer (2.07% for > 60 min after waking vs 5.92% ≤ 5 min after waking; hazard ratio [HR], 2.56 [95% CI, 1.49-4.41]), all-cause mortality (5.38% for > 60 min vs 11.21% ≤ 5 min; HR, 2.19 [95% CI, 1.55-3.09]), and lung cancer-specific mortality (0.55% for > 60 min vs 2.92% for ≤ 5 min; HR, 4.46 [95% CI, 1.63-12.21]). Conclusions Using TTFC, a one-question assessment of tobacco dependence, at the time of lung cancer screening has implications for personalizing tobacco treatment and improving risk assessment.

The Multicenter International Lymphangioleiomyomatosis Efficacy and Safety of Sirolimus (MILES) trial demonstrated that sirolimus stabilized lung function and improved measures of functional performance and quality of life in patients with lymphangioleiomyomatosis. The physiologic mechanisms of these beneficial actions of sirolimus are incompletely understood.To prospectively determine the longitudinal computed tomographic lung imaging correlates of lung function change in MILES patients treated with placebo or sirolimus.We determined the baseline to 12-month change in computed tomographic image-derived lung volumes and the volume of the lung occupied by cysts in the 31 MILES participants (17 in sirolimus group, 14 in placebo group) with baseline and 12-month scans.There was a trend toward an increase in median expiratory cyst volume percentage in the placebo group and a reduction in the sirolimus group (+2.68% vs. +0.97%, respectively; P = 0.10). The computed tomographic image-derived residual volume and the ratio of residual volume to total lung capacity increased more in the placebo group than in the sirolimus group (+214.4 ml vs. +2.9 ml [P = 0.054] and +0.05 ml vs. -0.01 ml [P = 0.0498], respectively). A Markov transition chain analysis of respiratory cycle cyst volume changes revealed greater dynamic variation in the sirolimus group than in the placebo group at the 12-month time point.Collectively, these data suggest that sirolimus attenuates progressive gas trapping in lymphangioleiomyomatosis, consistent with a beneficial effect of the drug on airflow obstruction. We speculate that a reduction in lymphangioleiomyomatosis cell burden around small airways and cyst walls alleviates progressive airflow limitation and facilitates cyst emptying.

Pulmonary embolism (PE) remains a common and important clinical condition that cannot be accurately diagnosed on the basis of signs, symptoms, and history alone. The diagnosis of PE has been facilitated by technical advancements and multidetector CT pulmonary angiography, which is the major diagnostic modality currently used. Ventilation and perfusion scans remain largely accurate and useful in certain settings. Lower-extremity ultrasound can substitute by demonstrating deep vein thrombosis; however, if negative, further studies to exclude PE are indicated. In all cases, correlation with the clinical status, particularly with risk factors, improves not only the accuracy of diagnostic imaging but also overall utilization. Other diagnostic tests have limited roles. Imaging is paramount in the setting of blunt trauma and is now the standard of care at any trauma center. Although anteroposterior radiography has inherent limitations, the ability to acquire a radiograph in the trauma bay with little interruption in clinical survey, monitoring, and treatment, as well as radiography's accepted role in screening for traumatic aortic injury, supports the routine use of chest radiography. Chest CT or CT angiography is the gold-standard routine imaging modality for detecting thoracic injuries caused by blunt trauma. There is disagreement on whether routine chest CT is necessary in all patients with histories of blunt trauma. Ultimately, the frequency and timing of CT chest imaging should be site specific and should depend on the local resources of the trauma center as well as patient status. Ultrasound may be beneficial in the detection of pneumothorax, hemothorax, and pericardial hemorrhage; transesophageal echocardiography is a first-line imaging tool in the setting of suspected cardiac injury. In the blunt trauma setting, MRI and nuclear medicine likely play no role in the acute setting, although these modalities may be helpful as problem-solving tools after initial assessment. An estimated 150,000 pulmonary nodules are identified each year, and the number is likely to increase given the results of the National Lung Screening Trial. Decision tools are needed to help with the management of such pulmonary nodules. We examined whether adding any of three novel functions of nodule volume improves the accuracy of an existing malignancy prediction model of CT scan-detected nodules.Swensen's 1997 prediction model was used to estimate the probability of malignancy in CT scan-detected nodules identified from a sample of 221 patients at the Medical University of South Carolina between 2006 and 2010. Three multivariate logistic models that included a novel function of nodule volume were used to investigate the added predictive value. Several measures were used to evaluate model classification performance.With use of a 0.5 cutoff associated with predicted probability, the Swensen model correctly classified 67% of nodules. The three novel models suggested that the addition of nodule volume enhances the ability to correctly predict malignancy; 83%, 88%, and 88% of subjects were correctly classified as having malignant or benign nodules, with significant net improved reclassification for each (P.0001). All three models also performed well based on Nagelkerke R2, discrimination slope, area under the receiver operating characteristic curve, and Hosmer-Lemeshow calibration test.The findings demonstrate that the addition of nodule volume to existing malignancy prediction models increases the proportion of nodules correctly classified. This enhanced tool will help clinicians to risk stratify pulmonary nodules more effectively.

The respiratory and the cardiovascular systems are intimately connected. Because of the high degree of morphological and functional interaction, pathophysiological processes in one compartment are likely to induce adaptive changes in the other. Computed tomography (CT) plays a central role in the diagnostic work up of both thoracic and cardiac disorders. Historically, these two systems have been evaluated separately; however, CT technology has evolved remarkably over recent decades. Up-to-date advanced imaging strategies allow for a combined assessment of the cardiopulmonary unit. Besides improved techniques of electrocardiogram (ECG)-synchronization for obtaining both morphological and functional information, latest advances of dual-source CT (DSCT) have shown great promise for even more comprehensive integrated cardiothoracic imaging.

Objective This study aimed to determine the relationship between main pulmonary artery diameter (MPAD) and pulmonary hypertension (PH) in scleroderma patients with and without interstitial lung disease. Methods We retrospectively reviewed 48 subjects with scleroderma who underwent a chest computed tomography (CT) and right heart catheterization with 6 months of each other. Patients were divided into 2 groups based on the absence or presence of interstitial lung disease on chest CT. Subset analysis was performed based on available pulmonary function tests and divided into groups by forced vital capacity (FVC). Computed tomographic scans were scored for extent of fibrosis and ground glass opacity. Pulmonary artery and ascending aorta measurements were obtained by 2 independent observers. Univariate and multivariate models were used to evaluate the correlation between MPAD and mean pulmonary artery pressure (mPAP) measured by right heart catheterization. Receiver operating characteristic analysis was performed for diagnostic accuracy of the MPAD measurement in predicting PH. Results Strong correlations between mPAP and MPAD were found in this study regardless of the presence or absence of mild to moderate interstitial fibrosis on chest CT. When dividing patients based on FVC, the correlation between mPAP and MPAD was substantially attenuated. An MPAD value of 30.8 mm yielded the highest sensitivity and specificity at 81.3% and 87.5%, respectively. Conclusions In scleroderma patients, an enlarged main pulmonary artery (>30 mm) predicts PH even in the presence of mild to moderate fibrosis although the relationship may be attenuated in the presence of a low % FVC.

Rib fracture is the most common thoracic injury, present in 10% of all traumatic injuries and almost 40% of patients who sustain severe nonpenetrating trauma. Although rib fractures can produce significant morbidity, the diagnosis of associated complications (such as pneumothorax, hemothorax, pulmonary contusion, atelectasis, flail chest, cardiovascular injury, and injuries to solid and hollow abdominal organs) may have a more significant clinical impact. When isolated, rib fractures have a relatively low morbidity and mortality, and failure to detect isolated rib fractures does not necessarily alter patient management or outcome in uncomplicated cases. A standard posteroanterior chest radiograph should be the initial, and often the only, imaging test required in patients with suspected rib fracture after minor trauma. Detailed radiographs of the ribs rarely add additional information that would change treatment, and, although other imaging tests (eg, computed tomography, bone scan) have increased sensitivity for detection of rib fractures, there are little data to support their use. Daily routine chest radiographs in the intensive care unit (ICU) have been a tradition for many years. Anecdotal reports of misplacement of life support items, acute lung processes, and extra pulmonary air collections in a small number of patients served as a justification for routine chest radiographs in the ICU. Having analyzed this practice, the ACR Appropriateness Criteria Expert Panel on Thoracic Imaging has made the following recommendations: • When monitoring a stable patient or a patient on mechanical ventilation in the ICU, a portable chest radiograph is appropriate for clinical indications only. • It is appropriate to obtain a chest radiograph after placement of an endotracheal tube, central venous line, Swan-Ganz catheter, nasogastric tube, feeding tube, or chest tube. The strongest data contributing to these recommendations were derived from a meta-analysis of 8 trials comprising 7,078 ICU patients by Oba and Zaza [1].

Despite decline in smoking rates, lung cancer remains the leading cause of cancer-related deaths with an estimated 224,000 new cases in 2014 (Siegel et al., CA Cancer J Clin 64:9–29, 2014). Tobacco remains the major causative agent, and as smoking behaviors between men and women have evolved, the gap in lung cancer between males and females has narrowed (Thun and Jemal, Tob Control 15:345–347, 2006). Approximately one in four lung cancers occur in never smokers (Pallis and Syrigos, Crit Rev Oncol Hematol 88:494–503, 2013). Other environmental risks for lung cancer include radon, occupational exposure, and air pollution. In addition to the environmental risks described previously, there does appear to be a component of both hormonal and genetic susceptibility to lung cancer development (Pallis and Syrigos, Crit Rev Oncol Hematol 88:494–503, 2013; Matakidou et al., Br J Cancer 93:825–833, 2005).

Occupational lung disease is a category of disease entities characterized by a reaction of the lung parenchyma to inhaled aerosolized particles found in the environment. This document summarizes the imaging appropriateness data for silicosis, coal worker pneumoconiosis, and asbestosis. The main points of the document are that computed tomography is more sensitive than radiography, computed tomography without contrast generally suffices for evaluation, and fluorodeoxyglucose-positron emission tomography may have utility in patients with mesothelioma. PURPOSE: To review the computed tomography (CT) imaging features of pulmonary nocardiosis (PN) at the time of initial presentation. MATERIALS AND METHODS: All patients from 1991 to 2008 with PN were identified (n=105). Patients without CT scan available at initial presentation were excluded (n=52). For the remaining 53 patients, standardized radiographic features were recorded. The patients were grouped by predisposing condition. Analysis includes descriptive summary statistics as well as associations among radiographic findings, associated findings, and host characteristics. Parametric and nonparametric statistical methods were used. RESULTS: Median age of the patients was 52 years (range, 6 to 82 y). Some form of immunosuppression was present in 83% of the cases. Preexisting structural abnormalities of the lung were uncommon (bronchiectasis, 7; chronic obstructive pulmonary disease, 3). Twenty (38%) patients had interstitial opacities. Airspace disease was seen in 34 (64%) cases. Thirty (57%) cases revealed discrete nodules, 25 patients had 1 to 6 nodules (mean, 2), and 5 patients had fewer than 6 nodules, with the mean size of the largest nodule being 1.67 cm. Masses were seen in 11 patients (21%), 9 of whom had concomitant nodules. Cavitary lesions, including nodules, masses, or airspace disease, occurred in 40% of the cohort. Mediastinal lymphadenopathy was present in 8 (15%) patients. Fifteen patients (28%) had pleural effusions; the effusions were unilateral in 10 patients. Analysis of radiographic associations with patient groups found discrete nodules to be more often associated with immunosuppression compared with the nonimmunosuppressed group (66% vs. 11%; P=0.0067). CONCLUSION: The CT presentation of PN is heterogeneous. Airspace disease appeared most frequently (in 64% of the cases), and nodules were present in 57% of the cases. Nocardiosis should be considered in the differential diagnosis of immunosuppressed patients with new nodules or masses.

Background We evaluated the association between hemodynamic parameters of chronic congestive heart failure (CHF) and mediastinal lymphadenopathy (MLA) in heart transplantation (HT) candidates and the effect of HT on MLA. We also described the results of lymph node (LN) biopsies of MLA in the patients. Methods Patients who underwent HT evaluation over an 8-year period and had chest CT scans were evaluated retrospectively. Data collected included LN sizes pre-HT and post-HT, echocardiographic measurements, radionuclide-derived ejection fraction, and right-sided heart catheterization hemodynamics. MLA was defined as LNs > 1 cm in smallest dimension. Results Of 118 patients, 53 patients had MLA. MLA had weak statistically significant correlations with elevated mean pulmonary artery pressure (MPAP), mitral regurgitation (MR), tricuspid regurgitation (TR), right atrial pressure (RAP), and pulmonary capillary wedge pressure (PCWP). Thirty-six patients with MLA underwent HT, and nine of the 36 had post-HT chest CT scans. All nine patients showed a decrease in LN size post-HT (mean LN diameter pre-HT = 1.16 ± 0.137 cm, post-HT = 0.75 ± 0.32 cm). Seven of 53 patients with MLA underwent biopsies. Four had benign LNs, one had sarcoidosis, and two had lung cancer. Conclusions MPAP, MR, TR, RAP, and PCWP had weak statistically significant correlations with MLA. HT led to regression of MLA in patients who underwent CT scans post-HT, implying that MLA is related to CHF. However, we also identified clinically important causes of MLA; therefore, biopsy should be considered if enlarged LNs fail to regress after maximal medical management of CHF.

The National Lung Screening Trial (NLST) is a randomized multicenter study comparing low-dose helical computed tomography (CT) with chest radiography in the screening of older current and former heavy smokers for early detection of lung cancer, which is the leading cause of cancer-related death in the United States. Five-year survival rates approach 70% with surgical resection of stage IA disease; however, more than 75% of individuals have incurable locally advanced or metastatic disease, the latter having a 5-year survival of less than 5%. It is plausible that treatment should be more effective and the likelihood of death decreased if asymptomatic lung cancer is detected through screening early enough in its preclinical phase. For these reasons, there is intense interest and intuitive appeal in lung cancer screening with low-dose CT. The use of survival as the determinant of screening effectiveness is, however, confounded by the well-described biases of lead time, length, and overdiagnosis. Despite previous attempts, no test has been shown to reduce lung cancer mortality, an endpoint that circumvents screening biases and provides a definitive measure of benefit when assessed in a randomized controlled trial that enables comparison of mortality rates between screened individuals and a control group that does not undergo the screening intervention of interest. The NLST is such a trial. The rationale for and design of the NLST are presented.

In order to appropriately manage patients with lung cancer, it is necessary to properly stage the tumor. The ACR Appropriateness Criteria is designed to provide an overview of the value of different imaging techniques in the non-invasive staging of lung cancer and allow for the rational selection of imaging studies to arrive at the appropriate clinical stage.

Background: Despite the unprecedented efficacy of immunotherapy with PD-1/PD-L1 blockade in non-small cell lung cancer, treatment failure is common and an area of unmet clinical need. IL-2/IL-15Rbg agonists, which potently augment the activity of CD8+ T and NK cells, have shown the ability to induce complete and durable responses in select cancers. However, this class of drugs has been neither combined with PD-1 targeted immunotherapies nor examined extensively in NSCLC. We hypothesized that the addition of an IL-15 superagonist (ALT-803) would be safe and improve the efficacy of anti-PD-1 mAb (nivolumab) in metastatic NSCLC patients in second-line therapy. Methods: In this phase Ib trial, patients with metastatic NSCLC who were eligible to initiate or to continue receiving nivolumab (anti-PD-1 mAb) after first-line therapy were enrolled to trial, including patients with disease relapsed on or refractory to PD-1 blockade. Nivolumab was administered in combination with ALT-803 at four dose levels. The primary objective of the study was to define safety and tolerability and a recommended phase II dose of ALT-803 in combination with nivolumab, with secondary objectives including immune correlates and clinical parameters including response rate. Results: In a first-in-human combination, 21 NSCLC patients were treated at 4 dose levels of ALT-803 in combination with the approved dose of nivolumab. No dose-limiting toxicities were observed, and the most common adverse events were fever, flu-like symptoms, and injection site reactions. The recommended phase II dose (RP2D) of therapy is weekly subcutaneous ALT-803 at 20 mcg/kg in combination with intravenous nivolumab 240 mg every 14 days. Multiple objective responses were observed, including when ALT-803 was added to a failing PD-1 mAb regimen. Discussion: The recommended phase II dose of ALT-803 plus nivolumab was determined and the regimen can safely be administered in the outpatient setting. Responses among patients with PD-1 mAb relapsed disease with the addition of ALT-803 to their regimen demonstrate for the first time evidence of antitumor activity for a new class of agents for NSCLC and in patients with disease progression on PD-1/PD-L1 pathway blockade. Detection of small indeterminate pulmonary nodules (4 to 10 mm in diameter) in clinical practice is increasing, largely because of increased utilization and improved imaging technology. Although there currently exists software for CT scan machines that automate nodule volume estimation, the imprecision associated with volume estimates is particularly poor for nodulesor = 6 mm in diameter, with greater imprecision associated with increasing CT scan slice thickness. This study examined the effects of the volume estimation error associated with four CT scan slice thicknesses (0.625, 1.25, 2.50, and 5.00 mm) on estimates of volume doubling time (VDT) for solid nodules of various sizes.Data reflecting the accuracy of 1,624 automated volume estimations were obtained from experiments incorporating volume estimation software, performed on a commercially available lung phantom. These data informed mathematical simulations that were used to estimate imprecision around VDT estimates for hypothetical pairs of volume estimates for a given solid pulmonary nodule observed at different time points.The confidence intervals around the VDT estimates were extremely wide for 2.50- and 5.00-mm slice thicknesses, often encompassing values traditionally associated with both benignity and malignity for simulated 1- and 2-mm growths in diameter.Because of the inaccuracy in automated volume estimation, the confidence a clinician should have in estimating VDT should be highly dependent on the degree of observed growth and on the CT scan slice thickness. The performance of CT scanners with slice thicknesses ofor = 2.5 mm for assessing growth in pulmonary nodules is essentially inadequate for 1-mm changes in nodule diameter.

At some institutions, catheter angiography is used for confirmation of aortic injuries and equivocal MDCT findings. Because of the speed and efficiency of 64-MDCT, we believe that diagnostic catheter angiography may be obsolete. The purpose of this study was to review our experience with 64-MDCT in the evaluation of acute traumatic aortic injury (ATAI).The trauma registry at a level 1 trauma center was reviewed to find cases of ATAI occurring between March 1, 2005, and July 31, 2007. MDCT images were correlated with transcatheter angiograms when obtained. Surgical and clinical reports were reviewed to confirm abnormal and normal findings and the stability of the conditions of patients undergoing conservative treatment.After level 1 or level 2 trauma, 1,344 patients underwent contrast-enhanced 64-MDCT. Twenty-four patients (1.79%) were found to have 25 aortic injuries. All patients had direct MDCT signs of ATAI. Ten catheter angiograms were obtained after MDCT. The presence of direct signs was confirmed in three cases. In five cases, indirect signs were found to be normal findings. In two cases, the findings remained equivocal after MDCT and conventional angiography. Fourteen patients underwent surgical repair of the aorta, six underwent conservative management, and four patients died of other injuries. No patient with equivocal or indirect findings needed surgical repair. The sensitivity of 64-MDCT was 96.0%; specificity, 99.8%; positive predictive value, 92.3%; negative predictive value, 99.9%; and accuracy, 99.8%.Direct signs of ATAI on contrast-enhanced 64-MDCT scans do not have to be confirmed with catheter angiography. In our population, diagnostic transcatheter angiography was of limited value for clarifying equivocal or indirect MDCT findings.

OBJECTIVE. The purpose of this study was to compare the frequency of detection of pulmonary nodules on cardiac CT scans acquired with a limited field of view with the frequency of detection at full field of view.CONCLUSION. Viewing of cardiac CT scans obtained only at a limited field of view can result in missing more than 67% of nodules larger than 1 cm and more than 80% of nodules smaller than 1 cm.

To determine the effects of iso-osmolality contrast medium compared with a low-osmolality agent on renal function (serum creatinine [SCr] and glomerular filtration rate [GFR]) in high-risk patients undergoing intravenous contrast material-enhanced CT.This HIPAA-compliant study was IRB-approved; formal consent was obtained. One hundred seventeen patients (83 men, 34 women; mean age, 64.3 years; range, 18-86 years) with decreased renal function underwent contrast-enhanced CT with either iso-osmolality iodixanol (n = 61) or low-osmolality iopromide (n = 56). Outcome measures were of SCr increase or GFR decrease for 3 days after CT, a SCr increase (ofor=0.5 mg/dL [44.2 micromol/L, 25%] oror=1.0 mg/dL [88.4 micromol/L, 50%]), a GFR reduction (ofor=5 mL/min), and patient outcome at 30- and 90-day follow-up.Iodixanol decreased SCr (mean +/- standard deviation) from 1.77 mg/dL +/- 0.24 (156.47 micromol/L +/- 21.22) at baseline to 1.65 mg/dL +/- 0.35 (145.86 micromol/L +/- 30.94, P = .046) at day 1, 1.73 mg/dL +/- 0.53 (152.93 micromol/L +/- 46.85, not significant) at day 2, and 1.73 mg/dL +/- 0.55 (152.93 micromol/L +/- 48.62, not significant) at day 3 (not significant). Iopromide increased SCr from 1.75 mg/dL +/- 0.32 (154.7 micromol/L +/- 28.29) at baseline to 1.8 mg/dL +/- 0.42 (159.12 micromol/L +/- 15.59) at day 1, 1.77 mg/dL +/- 0.49 (156.47 micromol/L +/- 43.32) at day 2, and 1.77 mg/dL +/- 0.62 (156.47 micromol/L +/- 54.81) at day 3 (not significant). Iodixanol increased and iopromide decreased GFR on all 3 days after CT (not significant). Fewer patients in the iodixanol group (8.5%) than in the iopromide group (27.8%) had SCr increase 0.5 mg/dL or higher (or=25%, P = .012). Two patients in each group had SCr increase of 1.0 mg/dL or more (not significant). More patients in the iopromide group (42.3%) than in the iodoxanol group (24.1%) had a GFR reduction of 5 mL/min or higher (P = .0426). No patient had a contrast material-related adverse event at 30- or 90-day follow-up.Intravenous contrast material application in high-risk patients is unlikely to be associated with permanent adverse outcomes. SCr levels after contrast material administration are lower in iodixanol than iopromide groups.

Objective Many trauma transfer patients arrive after hours with imaging studies performed at the transferring institution. Little attention has been given to the impact on on-call radiology residents and the potential medicolegal consequences. The purpose of this study was to assess the impact of outside trauma studies on on-call radiology residents. Methods A 20-question survey using the Web site www.surveymonkey.com was created to focus on the handling of outside images on trauma transfers. An institutional review board exemption for consent was obtained. The survey was distributed to all radiology residency program directors throughout the United States. Results Sixty-four program directors responded to our survey. Eighty percent of program directors worked at Level 1 trauma centers, 94% of program directors were located in medium to large cities and had an average of 27 residents (range 8-74), and 69% of programs reported receiving between 1 and 5 outside trauma studies per day, most without radiologic reports from the transferring hospital. After hours (defined by periods of time when no attending staff is present), these studies are read only by radiology residents at 45% of institutions. Written documentation by residents is often lacking when they are the sole radiology interpreters, and in many of these cases, the only documentation of the interpretation is a note in the patient's chart written by the referring clinician. The issue of transfer study interpretation was deemed a "definite" issue in 45% of programs, with 78% of program directors having been approached at least once regarding this issue. Conclusion Trauma patients are usually transferred to accepting teaching institutions without written radiologic reports, which generally do not meet either American College of Radiology or American Cancer Society guidelines. Approximately half of trauma transfer patients are managed solely on the basis of radiology resident interpretation, exposing residents to medicolegal liability. There is no consensus among programs regarding the optimal handling of these studies. This issue is in critical need of attention from the radiology community to enhance timeliness of communication and patient safety, and to protect radiology residents.
