Telegram ist in den vergangenen Jahren zu einem relevanten Bestandteil der politischen Öffentlichkeit geworden. Der Instant-Messaging-Dienst unterstützt sowohl den interpersonalen Austausch als auch die Informationsverteilung an nahezu beliebig große Publika. Zugleich entzieht er sich bislang vergleichsweise erfolgreich allen Regulierungsversuchen, die Verbreitung von Desinformationen, Hassrede oder anderen problematischen Inhalten einzuschränken. Dadurch ist Telegram vor allem, aber nicht nur, bei Akteur*innen und Gruppen beliebt, die am Rande oder jenseits des demokratisch akzeptablen Spektrums agieren. Das vorliegende Themenheft versammelt sieben Beiträge, die mit verschiedenen Methoden die Rolle von Telegram in der deutschsprachigen politischen Öffentlichkeit erkunden und weiteren Forschungsbedarf identifizieren.

Abstract Background Carotid stenting requires dual antiplatelet therapy to effectively prevent thromboembolic complications. However, resistance to clopidogrel, a key component of this therapy, may lead to persistent risk of these complications. The aim of this study was to determine, if the implementation of routine platelet function testing and adjusting therapy was associated with lower incidence of thromboembolic complications and death. Methods All consecutive patients treated with carotid artery stenting in a single institution over 8 years were enlisted in a retrospective study. Platelet function testing was performed, and efficient antiplatelet therapy was set before the procedure. Incidence of procedure-related stroke or death within periprocedural period (0–30 days) was assessed. The results were evaluated in relation to the findings of six prominent randomized control trials. Results A total of 241 patients were treated for carotid stenosis, seven patients undergo CAS on both sides over time. There was 138 symptomatic (55,6%) and 110 asymptomatic stenoses (44,4%). Five thromboembolic complications (2,01%) occurred, four of them (1,61%) was procedure-related. Two patients died because of procedure-related stroke (0,82%). Incidence of procedure-related stroke or death was significant lower compared to the results of CREST study (2,01% vs. 4,81%, P = 0,0243) in the entire cohorts, and to the results of ICSS study in the symptomatic cohorts (2,86% vs. 7,37%, P = 0,0243), respectively. Conclusions Tailored antiplatelet therapy in carotid stenting is safe and seems to be related with lower incidence of procedure-related death or stroke rate. Larger prospective studies to assess whether platelet function testing-guided antiplatelet therapy is superior to standard dual antiplatelet should be considered.

Abstract Background Endovascular aneurysm repair (EVAR) has created new possibilities for patients with abdominal aortic aneurysms (AAAs), and in recent years it has become tremendously popular. Use of EVAR in selected groups of patients allows mortality and morbidity to be reduced in comparison to open repair. However, complications such as endoleaks (ELs) can be of great concern and warrant urgent therapy to prevent sac rupture. Case presentation The case report presents urgent endovascular treatment of a high-risk type IA EL in a polymorbid 68-year-old patient 7 years after primary EVAR. The principle of treatment was parallel implantation of the proximal SG extension with the renal SG into the right renal artery (chimney technique). The subsequent type II collateral EL was treated by direct transabdominal AAA sac puncture and thrombin embolization. Conclusion EL can be a cause for urgent intervention, but specific anatomic features often require specialized SG types which are not readily available. The chimney technique allows the use of immediately available stent grafts to address endoleak in the setting of impending abdominal aneurysm rupture.

Background & Aims: Hepatitis B virus (HBV) was identified as an enveloped DNA virus with a diameter of 42 nm. Multivesicular bodies play a central role in HBV egress and exosome biogenesis. In light of this, it was studied whether intact virions wrapped in exosomes are released by HBV-producing cells. Methods: Robust methods for efficient separation of exosomes from virions were established. Exosomes were subjected to limited detergent treatment for release of viral particles. Electron microscopy of immunogold labeled ultrathin sections of purified exosomes was performed for characterization of exosomal HBV. Exosome formation/release was affected by inhibitors or Crispr/Cas-mediated gene silencing. Infectivity/uptake of exosomal HBV was investigated in susceptible and non-susceptible cells. Results: Exosomes could be isolated from supernatants of HBV-producing cells, which are characterized by the presence of exosomal and HBV markers. These exosomal fractions could be separated from the fractions containing free virions. Limited detergent treatment of exosomes causes stepwise release of intact HBV virions and naked capsids. Inhibition of exosome morphogenesis impairs the release of exosome-wrapped HBV. Electron microscopy confirmed the presence of intact virions in exosomes. Moreover, the presence of large hepatitis B virus surface antigen on the surface of exosomes derived from HBV expressing cells was observed, which conferred exosome-encapsulated HBV initiating infection in susceptible cells in a , large hepatitis B virus surface antigen/Na+–taurocholate co-transporting polypeptide-dependent manner. The uptake of exosomal HBV with low efficiency was also observed in non-permissive cells. Conclusion: These data indicate that a fraction of intact HBV virions can be released as exosomes. This reveals a so far not described release pathway for HBV.

Abstract In this study we performed a detailed analysis on the scale-size of field-aligned currents (FACs) at auroral latitudes, using the well-calibrated magnetic data from the non-dedicated magnetic field mission, Gravity Recovery and Climate Experiment Follow-On (GRACE-FO). With two spacecraft following each other, the GRACE-FO provides a good opportunity to identify the variation of FACs with different scale lengths. The results show that the auroral FACs can be classified into two groups: the small-scale ones, shorter than some tens of kilometers, dominated by kinetic Alfvén waves, are quite dynamic; and the large-scale ones, typically larger than 150 km, can be considered as quasi-static and persist longer than 1 min. The GRACE-FO observations also reveal that the small-scale FACs at the same location sometimes can persist over 25 s, e.g., around dusk and dawn hours, which is longer than the typical persistent period (10 s) of kinetic Alfvén waves as earlier reported. The FAC structures show clear magnetic local time dependence, with higher correlations between the spacecraft around dusk and dawn hours; lower correlations are found around midnight and lowest correlations around noon, implying that the small-scale FACs most frequently appear at the noon cusp region. Slightly better correlations of FACs between two spacecraft are found during local summer, and such seasonal dependence is dominated by the correlations of small-scale FACs at noon. However, further analysis shows that the small-scale FACs at noon have largest occurrence and intensity during local summer, which reveals that when interpreting the cross-correlation analysis the intensity of FACs needs to be taken into account. Abstract Electric currents flowing in the terrestrial ionosphere have conventionally been diagnosed by low-earth-orbit (LEO) satellites equipped with science-grade magnetometers and long booms on magnetically clean satellites. In recent years, there are a variety of endeavors to incorporate platform magnetometers, which are initially designed for navigation purposes, to study ionospheric currents. Because of the suboptimal resolution and significant noise of the platform magnetometers, however, most of the studies were confined to high-latitude auroral regions, where magnetic field deflections from ionospheric currents easily exceed 100 nT. This study aims to demonstrate the possibility of diagnosing weak low-/mid-latitude ionospheric currents based on platform magnetometers. We use navigation magnetometer data from two satellites, CryoSat-2 and the Gravity Recovery and Climate Experiment Follow-On (GRACE-FO), both of which have been intensively calibrated based on housekeeping data and a high-precision geomagnetic field model. Analyses based on 8 years of CryoSat-2 data as well as ~ 1.5 years of GRACE-FO data reproduce well-known climatology of inter-hemispheric field-aligned currents (IHFACs), as reported by previous satellite missions dedicated to precise magnetic observations. Also, our results show that C-shaped structures appearing in noontime IHFAC distributions conform to the shape of the South Atlantic Anomaly. The F-region dynamo currents are only partially identified in the platform magnetometer data, possibly because the currents are weaker than IHFACs in general and depend significantly on altitude and solar activity. Still, this study evidences noontime F-region dynamo currents at the highest altitude (717 km) ever reported. We expect that further data accumulation from continuously operating missions may reveal the dynamo currents more clearly during the next solar maximum.

Abstract In this study, we have derived field-aligned currents (FACs) from magnetometers onboard the Defense Meteorological Satellite Project (DMSP) satellites. The magnetic latitude versus local time distribution of FACs from DMSP shows comparable dependences with previous findings on the intensity and orientation of interplanetary magnetic field (IMF) B y and B z components, which confirms the reliability of DMSP FAC data set. With simultaneous measurements of precipitating particles from DMSP, we further investigate the relation between large-scale FACs and precipitating particles. Our result shows that precipitation electron and ion fluxes both increase in magnitude and extend to lower latitude for enhanced southward IMF B z , which is similar to the behavior of FACs. Under weak northward and southward B z conditions, the locations of the R2 current maxima, at both dusk and dawn sides and in both hemispheres, are found to be close to the maxima of the particle energy fluxes; while for the same IMF conditions, R1 currents are displaced further to the respective particle flux peaks. Largest displacement (about 3.5°) is found between the downward R1 current and ion flux peak at the dawn side. Our results suggest that there exists systematic differences in locations of electron/ion precipitation and large-scale upward/downward FACs. As outlined by the statistical mean of these two parameters, the FAC peaks enclose the particle energy flux peaks in an auroral band at both dusk and dawn sides. Our comparisons also found that particle precipitation at dawn and dusk and in both hemispheres maximizes near the mean R2 current peaks. The particle precipitation flux maxima closer to the R1 current peaks are lower in magnitude. This is opposite to the known feature that R1 currents are on average stronger than R2 currents.

In light of an increasing number of vaccinated and convalescent individuals, there is a major need for the development of robust methods for the quantification of neutralizing antibodies; although, a defined correlate of protection is still missing. Sera from hospitalized COVID-19 patients suffering or not suffering from acute respiratory distress syndrome (ARDS) were comparatively analyzed by plaque reduction neutralization test (PRNT) and pseudotype-based neutralization assays to quantify their neutralizing capacity. The two neutralization assays showed comparable data. In case of the non-ARDS sera, there was a distinct correlation between the data from the neutralization assays on the one hand, and enzyme-linked immune sorbent assay (ELISA), as well as biophysical analyses, on the other hand. As such, surface plasmon resonance (SPR)-based assays for quantification of binding antibodies or analysis of the stability of the antigen–antibody interaction and inhibition of syncytium formation, determined by cell fusion assays, were performed. In the case of ARDS sera, which are characterized by a significantly higher fraction of RBD-binding IgA antibodies, there is a clear correlation between the neutralization assays and the ELISA data. In contrast to this, a less clear correlation between the biophysical analyses on the one hand and ELISAs and neutralization assays on the other hand was observed, which might be explained by the heterogeneity of the antibodies. To conclude, for less complex immune sera—as in cases of non-ARDS sera—combinations of titer quantification by ELISA with inhibition of syncytium formation, SPR-based analysis of antibody binding, determination of the stability of the antigen–antibody complex, and competition of the RBD-ACE2 binding represent alternatives to the classic PRNT for analysis of the neutralizing potential of SARS-CoV-2-specific sera, without the requirement for a BSL3 facility.

PURPOSE:We aimed to assess the safety and effectiveness of mechanical recanalization in patients with ischemic stroke in the anterior circulation within 8 h since symptoms onset and with unknown onset time. We compared time intervals

Objective: The aim of our study was to evaluate duplex ultrasonography (DUS) and magnetic resonance angiography (MRA) in detection of haemodynamically significant renal artery stenosis (RAS). Methods: The study included patients with high clinical suspicion of renovascular hypertension (RVH). The imaging of renal arteries was performed by DUS, MRA and digital subtraction angiography (DSA). Significant RAS was defined as maximum systolic velocity ≥ 180 cm/sec (DUS) or as 60% reduction of the endoluminal arterial diameter (MRA, DSA). The results of DUS and MRA were assessed in respect to the results of DSA. Results: Arterial supply of 186 kidneys in 94 patients was evaluated. DSA revealed significant RAS in 61 kidneys evaluated. DUS was not able to examine arterial supply in 18 kidneys of 13 patients. In the detection of significant RAS, DUS was characterized by sensitivity and specificity of 85 % and 84 %. MRA achieved satisfactory imaging quality in all but one kidney evaluated. The sensitivity and specificity of MRA in the detection of significant RAS was 93 % and 93 %, respectively. Conclusion: In patients with high clinical probability of RVH, MRA proved to be more reliable and superior in both sensitivity and specificity to DUS in the detection of significant RAS.

The authors present a case of thrombosis on the St. Jude Medical 19 aortic valve prosthesis. The diagnosis was confirmed by transthoracic and transoesofageal echocardiography, cardiac fluoroscopy revealed restricted movement of the aortic valve prosthesis leaflet. Thrombolytic therapy was complicated with brain embolism that was successfully percutaneously removed from the cerebral artery by the mechanical device. The patient has fully recovered without any neurological residual symptoms. This case report should be instructive to other clinicians who encounter the same complications after thrombolytic treatment.

A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. The patient underwent multiple consecutive operations and interventions having, among others, finally replaced the entire thoracic aorta with the use of the hybrid elephant trunk technique.

Bronchial artery aneurysm is a very rare condition. Its rupture can cause a life threatening hemorrhage. We report a case of a 50-year-old patient with a ruptured bronchial artery aneurysm who presented with epigastric pain. It is the first published case with such signs of a rupture of a bronchial artery aneurysm. CT scan confirmed the diagnosis. The patient was treated with endovascular embolization.
