Abstract

BackgroundFunctional impairment of interferon, a natural antiviral component of the immune system, is associated with the pathogenesis and severity of COVID-19. We aimed to compare the efficacy of interferon beta-1a in combination with remdesivir compared with remdesivir alone in hospitalised patients with COVID-19.MethodsWe did a double-blind, randomised, placebo-controlled trial at 63 hospitals across five countries (Japan, Mexico, Singapore, South Korea, and the USA). Eligible patients were hospitalised adults (aged ≥18 years) with SARS-CoV-2 infection, as confirmed by a positive RT-PCR test, and who met one of the following criteria suggestive of lower respiratory tract infection: the presence of radiographic infiltrates on imaging, a peripheral oxygen saturation on room air of 94% or less, or requiring supplemental oxygen. Patients were excluded if they had either an alanine aminotransferase or an aspartate aminotransferase concentration more than five times the upper limit of normal; had impaired renal function; were allergic to the study product; were pregnant or breast feeding; were already on mechanical ventilation; or were anticipating discharge from the hospital or transfer to another hospital within 72 h of enrolment. Patients were randomly assigned (1:1) to receive intravenous remdesivir as a 200 mg loading dose on day 1 followed by a 100 mg maintenance dose administered daily for up to 9 days and up to four doses of either 44 μg interferon beta-1a (interferon beta-1a group plus remdesivir group) or placebo (placebo plus remdesivir group) administered subcutaneously every other day. Randomisation was stratified by study site and disease severity at enrolment. Patients, investigators, and site staff were masked to interferon beta-1a and placebo treatment; remdesivir treatment was given to all patients without masking. The primary outcome was time to recovery, defined as the first day that a patient attained a category 1, 2, or 3 score on the eight-category ordinal scale within 28 days, assessed in the modified intention-to-treat population, defined as all randomised patients who were classified according to actual clinical severity. Safety was assessed in the as-treated population, defined as all patients who received at least one dose of the assigned treatment. This trial is registered with ClinicalTrials.gov, NCT04492475.FindingsBetween Aug 5, 2020, and Nov 11, 2020, 969 patients were enrolled and randomly assigned to the interferon beta-1a plus remdesivir group (n=487) or to the placebo plus remdesivir group (n=482). The mean duration of symptoms before enrolment was 8·7 days (SD 4·4) in the interferon beta-1a plus remdesivir group and 8·5 days (SD 4·3) days in the placebo plus remdesivir group. Patients in both groups had a time to recovery of 5 days (95% CI not estimable) (rate ratio of interferon beta-1a plus remdesivir group vs placebo plus remdesivir 0·99 [95% CI 0·87-1·13]; p=0·88). The Kaplan-Meier estimate of mortality at 28 days was 5% (95% CI 3-7%) in the interferon beta-1a plus remdesivir group and 3% (2-6%) in the placebo plus remdesivir group (hazard ratio 1·33 [95% CI 0·69-2·55]; p=0·39). Patients who did not require high-flow oxygen at baseline were more likely to have at least one related adverse event in the interferon beta-1a plus remdesivir group (33 [7%] of 442 patients) than in the placebo plus remdesivir group (15 [3%] of 435). In patients who required high-flow oxygen at baseline, 24 (69%) of 35 had an adverse event and 21 (60%) had a serious adverse event in the interferon beta-1a plus remdesivir group compared with 13 (39%) of 33 who had an adverse event and eight (24%) who had a serious adverse event in the placebo plus remdesivir group.InterpretationInterferon beta-1a plus remdesivir was not superior to remdesivir alone in hospitalised patients with COVID-19 pneumonia. Abstract

We sought to validate prognostic scores in coronavirus disease 2019 including National Early Warning Score, Modified Early Warning Score, and age-based modifications, and define their performance characteristics.DesignWe analyzed prospectively collected data from the Adaptive COVID-19 Treatment Trial. National Early Warning Score was collected daily during the trial, Modified Early Warning Score was calculated, and age applied to both scores. We assessed prognostic value for the end points of recovery, mechanical ventilation, and death for score at enrollment, average, and slope of score over the first 48 hours.SettingA multisite international inpatient trial.PatientsA total of 1,062 adult nonpregnant inpatients with severe coronavirus disease 2019 pneumonia.InterventionsAdaptive COVID-19 Treatment Trial 1 randomized participants to receive remdesivir or placebo. The prognostic value of predictive scores was evaluated in both groups separately to assess for differential performance in the setting of remdesivir treatment.Measurements and main resultsFor mortality, baseline National Early Warning Score and Modified Early Warning Score were weakly to moderately prognostic (c-index, 0.60-0.68), and improved with addition of age (c-index, 0.66-0.74). For recovery, baseline National Early Warning Score and Modified Early Warning Score demonstrated somewhat better prognostic ability (c-index, 0.65-0.69); however, National Early Warning Score+age and Modified Early Warning Score+age further improved performance (c-index, 0.68-0.71). For deterioration, baseline National Early Warning Score and Modified Early Warning Score were weakly to moderately prognostic (c-index, 0.59-0.69) and improved with addition of age (c-index, 0.63-0.70). All prognostic performance improvements due to addition of age were significant (p < 0.05).ConclusionsIn the Adaptive COVID-19 Treatment Trial 1 cohort, National Early Warning Score and Modified Early Warning Score demonstrated moderate prognostic performance in patients with severe coronavirus disease 2019, with improvement in predictive ability for National Early Warning Score+age and Modified Early Warning Score+age. Area under receiver operating curve for National Early Warning Score and Modified Early Warning Score improved in patients receiving remdesivir versus placebo early in the pandemic for recovery and mortality. Although these scores are simple and readily obtainable in myriad settings, in our data set, they were insufficiently predictive to completely replace clinical judgment in coronavirus disease 2019 and may serve best as an adjunct to triage, disposition, and resourcing decisions.

Abstract

BackgroundExternal ventricular drain (EVD) is a standard approach for both monitoring intracranial pressure (ICP) and draining cerebrospinal fluid (CSF) for patients with subarachnoid hemorrhage. Documenting an accurate ICP value is important to assess the status of the brain, which would require the EVD system to be leveled properly and closed to CSF drainage for an adequate period. It is suggested that a minimum period of 5-minute EVD closure is needed before documenting a true ICP; however, there is no commonly agreed upon standard for documenting ICP. To obtain an insight into how well the intermittent EVD clamping procedure is performed for ICP documentation, we conducted a retrospective analysis of ICP recordings obtained through EVD from 107 patients with subarachnoid hemorrhage.MethodsThe EVD was kept open for continuous CSF drainage and then intermittently closed for ICP documentation. For each EVD closure, mean ICP, standard deviation of ICP, duration of EVD closure, and time interval between 2 adjacent EVD closures were studied. The total number of EVD closures was calculated for each patient. We developed an algorithm to evaluate whether ICP reached a new equilibrium before the EVD was reopened to drainage. The percentage of EVD closures that reach the equilibrium was calculated.ResultsThe 107 patients had 32 755 EVD closures in total, among which 65.9% instances lasted less than 1 minute and only 16.3% of all the EVD closure episodes lasted longer than 5 minutes. The median duration of each EVD closure was 25 seconds (interquartile range, 10.2 seconds to 2.33 minutes). Only 22.9% of the EVD closures reached ICP equilibrium before EVD reopening.ConclusionA standard guideline and proper training are needed for bedside nurses, and a potential tool that can render ICP trend at a proper scale at bedside would help clinicians correctly document ICP.

BackgroundUnhealthy alcohol use has adverse effects on HIV treatment. Abstract

Quality of life (QoL) is relevant to people living with HIV (PLWH) with increased life expectancy because of antiretroviral therapy. Our cross-sectional study examined associations between sociodemographic, HIV-related and psychological variables, and QoL, overall and by age. PLWH (n = 614) completed questionnaires at enrollment in an alcohol treatment study. QoL was assessed by the 12-item Short Form Survey, which includes physical and mental domains. Linear regression models evaluated the association of age and other factors with mental and physical QoL. PLWH younger than 50 years (n = 310) reported poorer mental QoL but better physical QoL compared to older PLWH (n = 304). Poorer mental QoL was associated with substance use, depression, and anxiety. Poorer physical QoL was associated with depression and history of injection drug use. We identified age-group differences in QoL for this primary care-based sample. Health care providers can use our findings to guide patient-centered care.

Abstract

BACKGROUND:Limited primary care-based research has examined hazardous drinking risk factors and motivation to reduce use in persons with HIV (PWH). METHODS:We computed prevalence ratios (PR) for factors associated with recent (5) included: moderate/severe depression (PR: 1.43; 1.03, 2.00) and other substance use (PR: 1.49; 1.11, 2.01). Lower importance was associated with incomes above $50,000 (PR: 0.65; 0.46, 0.91) and marijuana use (PR: 0.65; 0.49, 0.87). HIV-specific factors (e.g., CD4 and HIV RNA levels) were not associated with alcohol outcomes. CONCLUSIONS:This study identified modifiable participant characteristics associated with alcohol outcomes in PWH, including anxiety and depression severity, tobacco use, and other substance use.

18. Cognitive Function and Health-Related Quality of Life in Older Adults with HIV

Abstract

Combination antiretroviral therapy has dramatically decreased AIDS-related mortality, allowing Persons Living with the Human Immunodeficiency Virus (PLWH) to live into advanced ages and, by virtue of older age, become at heightened risk for the same comorbidities experienced by healthy older adults. Data suggest that both increased age and HIV are independent risk factors for cognitive impairment and cognitive impairment directly affects Health-Related Quality of Life (HRQOL). This dissertation contributes to the science of caring for older PLWH. It explores the presence of cognitive function as measured by two tools. The Patient’s Assessment of Own Function Inventory (PAOFI) is a self-report measure of cognitive function and the Neuropsychological Test Battery (NTB) is a more time intensive clinical measure of cognitive function. This cross-sectional study was a secondary data analysis that determined if PAOFI is predictive of NTB performance and to determine cognitive and depressive symptom correlates to HRQOL in a cohort of PLWH age 60 and older. This sample included a total of 73 older PLWH in the San Francisco Bay Area. Self-report of cognitive function had no correlations with clinical measures of cognitive function in older PLWH. Depression explained more of the variance in PAOFI, controlling for the NTB. Older PLWH who reported better physical HRQOL also reported better cognitive function. Furthermore, those who reported better mental HRQOL scores reported less self-report of cognitive impairment, less depression, and scored better on the NTB. This study determined that self-report alone is not enough to detect cognitive impairment in older PLWH. Instead, the mental health summary score of the HRQOL was a better measure of cognitive function as assessed by the NTB. This work has broad implications for clinical care of older PLWH. A more comprehensive understanding of an older PLWH’s HRQOL has significant implications to improve the quality of care in older PLWH with cognitive impairment.

Introduction: ICUs are stressful environments where patient care is complex and demanding; death is not an unusual outcome. Nurses can become emotionally distressed from experiencing multiple, unexpected, or traumatic patient deaths. Performing a reflective exercise immediately post death is one coping strategy which encourages healthcare staff and family to debrief their feelings in a timelier manner. Objectives: The aim of this project was to educate the nurses in the ICU with emotional coping skills pertaining to the effects of patient deaths. The objective was to implement a nurse-guided reflective exercise following each patient death in three adult ICUs in a quaternary academic medical center. Methods: The current change project used the Joanna Briggs Institute's Getting Research into Practice and Practical Application of Clinical Evidence Systems for auditing and strategy development. After standardizing the reflective exercise procedure, teaching methods were employed to support implementation. One followup audit measured compliance with completing a reflective exercise for all deaths occurring in the three ICUs. Contextual data were collected for completed exercises including type of death and words to describe nurses' emotions. Results: The reflective exercise script was revised and the standard procedure posted to the intranet for easy access. All ICU nurses received education on the use of this reflective exercise. For each of the two ICUs where this practice was new, compliance with nurses initiating and completing a reflective exercise was at a rate of 17 and 2%, respectively. For the ICU where reflective exercise was reintroduced, compliance was 30%. All deaths included in this study were anticipated (e.g., patient on comfort care) and calm, with the exception of one post cardiac arrest death. Conclusion: Despite multipronged teaching for why, when, and how to use reflective exercise, there was minimal success in completing reflective exercises in the two units where the practice was new. This outcome was most likely due to higher than anticipated levels of needed support or the unpredictability of frequency of deaths on each unit. However, the main achievement was having the Minute of Silence procedure with a standardized script readily available at all times for nursing staff. Reimplementation success in the other ICU was attributed to more recently established reflective practices. Further exploration of barriers, such as type of death, and re-education are essential strategies to bolster and sustain practice and to provide support for our nursing staff to prevent emotional distress. Abstract

Background: Unhealthy alcohol use has adverse effects on HIV treatment. Screening, brief intervention, and referral to treatment (SBIRT) has some evidence of efficacy but may not be sufficient for those with low motivation or comorbid substance use.Objective: To examine the effectiveness of motivational interviewing (MI) and emailed feedback (EF) among primary care HIV-positive patients, compared with treatment as usual care (UC) only, which included SBIRT.Design: Randomized clinical trial.Participants: Six hundred fourteen adult HIV-positive patients in Kaiser Permanente Northern California who reported prior-year unhealthy alcohol use.Intervention: Participants were randomized to either three sessions of MI (one in person and two by phone), information regarding alcohol risks via EF through a patient portal, or UC alone. MI and EF participants who reported unhealthy alcohol use at 6 months were offered additional MI and EF treatment, respectively.Main Measures: Participant-reported unhealthy alcohol use (defined as ≥ 4/≥ 5 drinks per day for women/men), alcohol problems at 12 months, based on blinded telephone interviews. Secondary outcomes included drug use and antiretroviral (ART) adherence.Key Results: At 12 months, there were no overall group differences, but in all three arms, there were declines in unhealthy alcohol use and alcohol-related problems (p < 0.001). Participants reporting low motivation to reduce drinking at baseline were less likely to report unhealthy alcohol use if they received MI vs. EF and UC (p = 0.013). At 6 months, reported illegal drug use/misuse of prescription drugs other than marijuana was lower in the MI arm than EF or UC (p = 0.012). There were no differences in ART adherence between groups.Conclusions: In a randomized trial of HIV-positive patients using two behavioral interventions compared with SBIRT alone, participants in all three conditions reduced unhealthy alcohol use. 34. Mental and Physical Quality of Life by Age Groups in People Living With HIV.

Abstract

Quality of life (QoL) is relevant to people living with HIV (PLWH) with increased life expectancy because of antiretroviral therapy. Our cross-sectional study examined associations between sociodemographic, HIV-related and psychological variables, and QoL, overall and by age. PLWH (n = 614) completed questionnaires at enrollment in an alcohol treatment study. QoL was assessed by the 12-item Short Form Survey, which includes physical and mental domains. Linear regression models evaluated the association of age and other factors with mental and physical QoL. PLWH younger than 50 years (n = 310) reported poorer mental QoL but better physical QoL compared to older PLWH (n = 304). Poorer mental QoL was associated with substance use, depression, and anxiety. Poorer physical QoL was associated with depression and history of injection drug use. We identified age-group differences in QoL for this primary care-based sample. 38. BMD‐001, a nanoparticle containing miR‐485‐3p antisense oligonucleotide, blocks Alzheimer's disease progression.

Abstract

Background: Alzheimer's disease (AD) is a form of dementia characterized by progressive memory decline and cognitive dysfunction, which affects more than 44 million people worldwide. Currently, there is no effective therapy for AD despite its increasing global incidence; thus, effective treatment strategies for AD are urgently needed. Method: In a previous report, we confirmed that expression of miR‐485‐3p was increased in the brain tissues, CSF, and plasma exosomes of AD patients than in healthy controls. In addition, the therapeutic potential of downregulation of miR‐485‐3p was also shown in 5XFAD AD model mice. Based on this target validation, we developed a nanoparticle containing antisense oligonucleotide (ASO) targeting miR‐485‐3p, named BMD‐001, which has a high CNS‐targeting ability to cross the blood‐brain barrier (BBB). BMD‐001 was injected into 10‐month‐old 5XFAD mice by intravenous injection, once weekly for four weeks. Behavioral tests were performed at 11 months and their brain pathology was examined after 8‐week‐washout at 13 months. Result: To evaluate in vivo pharmacokinetics of BMD‐001, the plasma levels of Cy5‐labeled ASO encapsulated in the nanoparticle were measured after intravenous injection along with a naked ASO without formulation of nanoparticle. The half‐life of the ASO in the nanoparticle was 93 min while that of the naked ASO was only 43 sec. Furthermore, we also observed that the brain accumulation of the fluorescent ASO in the nanoparticle was 64‐fold higher than the naked ASO and was maintained up to 16 hours, as monitored by in vivo imaging. After BMD‐001 injection, miR‐485‐3p expression was decreased in the brain tissue. We found that BMD‐001 enhanced Aβ clearance via phagocytosis of Aβ in vitro and in vivo. Furthermore, BMD‐001 reduced Aβ plaques, tau pathology, inflammasome, neuroinflammation, and cognitive decline in the 5XFAD mice. Conclusion: Collectively, our findings suggest that BMD‐001 is delivered to the brain through the efficient BBB penetration. Furthermore, BMD‐001 can be a promising candidate for treatment of AD pathology and cognitive decline, establishing a new paradigm in the AD field. [ABSTRACT FROM AUTHOR]

40. PLANET SHADOWS IN PROTOPLANETARY DISKS. II. OBSERVABLE SIGNATURES

Abstract

We calculate simulated images of disks perturbed by embedded small planets. These 10-50 M [?] bodies represent the growing cores of giant planets. We examine scattered light and thermal emission from these disks over a range of wavelengths, taking into account the wavelength-dependent opacity of dust in the disk. We also examine the effect of inclination on the observed perturbations. We find that the perturbations are best observed in the visible to mid-infrared (mid-IR). Scattered light images reflect shadows produced at the surface of perturbed disks, while the infrared images follow thermal emission from the surface of the disk, showing cooled/heated material in the shadowed/brightened regions. At still longer wavelengths in the submillimeter, the perturbation fades as the disk becomes optically thin and surface features become overwhelmed by emission closer toward the midplane of the disk. With the construction of telescopes such as TMT, GMT, and ALMA due in the next decade, there is a real possibility of observing planets forming in disks in the optical and submillimeter. However, having the angular resolution to observe the features in the mid-IR will remain a challenge.

41. Planet Shadows in Protoplanetary Disks. I. Temperature Perturbations

Abstract

Planets embedded in optically thick passive accretion disks are expected to produce perturbations in the density and temperature structure of the disk. We calculate the magnitudes of these perturbations for a range of planet masses and distances. The model predicts the formation of a shadow at the position of the planet paired with a brightening just beyond the shadow. We improve on previous work on the subject by self-consistently calculating the temperature and density structures under the assumption of hydrostatic equilibrium and taking the full three-dimensional shape of the disk into account rather than assuming a plane-parallel disk. While the excursion in temperatures is less than in previous models, the spatial size of the perturbation is larger. We demonstrate that a self-consistent calculation of the density and temperature structure of the disk has a large effect on the disk model. In addition, the temperature structure in the disk is highly sensitive to the angle of incidence of stellar irradiation at the surface, so accurately calculating the shape of the disk surface is crucial for modeling the thermal structure of the disk.

42. Constraints on the Formation of the Planet in HD 188753

Abstract

The claimed discovery of a Jupiter-mass planet in the close triple-star system HD 188753 poses a problem for planet formation theory. A circumstellar disk around the planet's parent star would be truncated close to the star, leaving little material available for planet formation. In this paper we attempt to model a protoplanetary disk around HD 188753A using a fairly simple a-disk model, exploring a range of parameters constrained by observations of T Tauri-type stars. The disk is truncated to within 1.5-2.7 AU, depending on model parameters. We find that the in situ formation of the planet around HD 188753A is implausible.

43. Type I Migration in a Nonisothermal Protoplanetary Disk

Abstract

We calculate rates of type I migration of protoplanets in a nonisothermal three-dimensional protoplanetary disk, building on planet-disk models developed in previous work. We find that including the vertical thickness of the disk results in a decrease in the type I migration rate by a factor of ~2 from that of a two-dimensional disk. The vertical temperature variation has only a modest effect on migration rates, since the torques at the midplane are weighted heavily because both the density and the perturbing potential are maximized at the midplane. However, temperature perturbations resulting from shadowing and illumination at the disk's surface can decrease the migration rate by up to another factor of 2 for planets at the gap-opening threshold at distances for which viscous heating is minimal. This would help to resolve the timescale mismatch between the standard core-accretion scenario for planet formation and the survival of planets and could help explain some of the rich diversity of planetary systems already observed.

44. Disk Temperature Variations and Effects on the Snow Line in the Presence of Small Protoplanets

Abstract

We revisit the computation of a "snow line" in a passive protoplanetary disk during the stage of planetesimal formation. We examine how shadowing and illumination in the vicinity of a planet affects where in the disk ice can form, making use of our method for calculating radiative transfer on disk perturbations with some improvements on the model. We adopt a model for the unperturbed disk structure that is more consistent with observations and use opacities for reprocessed dust instead of interstellar medium dust. We use the improved disk model to calculate the temperature variation for a range of planet masses and distances and find that planets at the gap-opening threshold can induce temperature variations of up to +-30%. Temperature variations this significant may have ramifications for planetary accretion rates and migration rates. We discuss in particular the effect of temperature variations near the sublimation point of water, since the formation of ice can enhance the accretion rate of disk material onto a planet. Shadowing effects can cool the disk enough that ice will form closer to the star than previously expected, effectively moving the snow line inward.

45. Radiative Transfer on Perturbations in Protoplanetary Disks

Abstract

We present a method for calculating the radiative transfer on a protoplanetary disk perturbed by a protoplanet. We apply this method to determine the effect on the temperature structure within the photosphere of a passive circumstellar disk in the vicinity of a small protoplanet of up to 20 Earth masses. The gravitational potential of a protoplanet induces a compression of the disk material near it, resulting in a decrement in the density at the disk's surface. Thus, an isodensity contour at the height of the photosphere takes on the shape of a well. When such a well is illuminated by stellar irradiation at grazing incidence, it results in cooling in a shadowed region and heating in an exposed region. For typical stellar and disk parameters relevant to the epoch of planet formation, we find that the temperature variation due to a protoplanet at 1 AU separation from its parent star is about 4% (5 K) for a planet of 1 Earth mass, about 14% (19 K) for planet of 10 Earth masses, and about 18% (25 K) for planet of 20 Earth masses, We conclude that even such relatively small protoplanets can induce temperature variations in a passive disk. Therefore, many of the processes involved in planet formation should not be modeled with a locally isothermal equation of state.
