1. Kynurenines and aerobic exercise capacity in chronic kidney disease: A cross-sectional and longitudinal study.

Abstract

Background: The causes of reduced aerobic exercise capacity (ExCap) in chronic kidney disease (CKD) are multifactorial, possibly involving the accumulation of tryptophan (TRP) metabolites such as kynurenine (KYN) and kynurenic acid (KYNA), known as kynurenines. Their relationship to ExCap has yet to be studied in CKD. We hypothesised that aerobic ExCap would be negatively associated with plasma levels of TRP, KYN and KYNA in CKD. Methods: We included 102 patients with non-dialysis CKD stages 2–5 (CKD 2–3, n = 54; CKD 4–5, n = 48) and 54 healthy controls, age- and sex-matched with the CKD 2–3 group. ExCap was assessed as peak workload during a maximal cycle ergometer test. Plasma KYN, KYNA and TRP were determined by high-performance liquid chromatography. Kidney function was evaluated by glomerular filtration rate (GFR) and estimated GFR. The CKD 2–3 group and healthy controls repeated tests after five years. The association between TRP, KYN, KYNA and ExCap in CKD was assessed using a generalised linear model. Results: At baseline, there were significant differences between all groups in aerobic ExCap, KYN, KYNA, TRP and KYN/TRP. KYNA increased in CKD 2–3 during the follow-up period. In CKD 2–5, KYNA, KYN/TRP and KYNA/KYN were all significantly negatively associated with ExCap at baseline, whereas KYN and TRP were not. Kynurenines were significantly correlated with GFR (p < 0.001 for all). Including GFR in the statistical model, no kynurenines were independently associated with ExCap at baseline. At follow-up, the increase in KYN and KYN/TRP was related to a decrease in ExCap in CKD 2–3. After adjusting for GFR, increase in KYN/TRP remained an independent significant predictor of a decline in ExCap in CKD 2–3. Conclusion: Aerobic ExCap was inversely associated with plasma levels of kynurenines in CKD at baseline and follow-up. [ABSTRACT FROM AUTHOR]

2. Decreased mitochondrial‐related gene expression in adipose tissue after acute sprint exercise in humans: A pilot study.

Abstract

The aim was to examine the acute effects of sprint exercise (SIT) on global gene expression in subcutaneous adipose tissue (AT) in healthy subjects, to enhance understanding of how SIT influences body weight regulation. The hypothesis was that SIT upregulates genes involved in mitochondrial function and fat metabolism. A total of 15 subjects performed three 30‐s all‐out sprints (SIT). Samples were collected from AT, skeletal muscle (SM) and blood (brachial artery and a subcutaneous AT vein) up to 15 min after the last sprint. Results showed that markers of oxidative stress, such as the purines hypoxanthine, xanthine and uric acid, increased markedly by SIT in both the artery and the AT vein. Purines also increased in AT and SM tissue. Differential gene expression analysis indicated a decrease in signaling for mitochondrial‐related pathways, including oxidative phosphorylation, electron transport, ATP synthesis, and heat production by uncoupling proteins, as well as mitochondrial fatty acid beta oxidation. This downregulation of genes related to oxidative metabolism suggests an early‐stage inhibition of the mitochondria, potentially as a protective mechanism against SIT‐induced oxidative stress. Abstract

Vibrio vulnificus is a zoonotic pathogen that can cause death by septicaemia in farmed fish (mainly eels) and humans. The zoonotic strains that have been isolated from diseased eels and humans after eel handling belong to clade E (or serovar E (SerE)), a clonal complex within the pathovar (pv.) piscis. The aim of this study was to evaluate the accuracy of MALDI-TOF mass spectrometry (MS) in the identification of SerE, using the other two main pv. piscis-serovars (SerA and SerI) from eels as controls. MALDI-TOF data were compared with known serologic and genetic data of five pv. piscis isolates or strains, and with the non pv. piscis reference strain. Based on multiple spectra analysis, we found serovar-specific peaks that were of ~3098 Da and ~ 4045 Da for SerE, of ~3085 Da and ~ 4037 Da for SerA, and of ~3085 Da and ~ 4044 Da for SerI. Therefore, our results demonstrate that MALDI-TOF can be used to identify SerE and could also help in the identification of the other serovars of the species. This means that zoonosis due to V. vulnificus could be prevented by using MALDI-TOF, as action can be taken immediately after the isolation of a possible zoonotic V. vulnificus strain.

8. Enhanced interleukin‐6 in human adipose tissue vein after sprint exercise: Results from a pilot study.

Abstract

Background: Low‐volume sprint exercise is likely to reduce body fat. Interleukin (IL‐6) may mediate this by increasing adipose tissue (AT) lipolysis. Therefore, the exchange of AT IL‐6 and glycerol, a marker of lipolysis, was examined in 10 healthy subjects performing three 30‐s all‐out sprints. Methods: Blood samples were obtained from brachial artery (a) and a superficial subcutaneous vein (v) on the anterior abdominal wall up to 9 min after the last sprint and analysed for IL‐6 and glycerol. Results: Arterial IL‐6 increased 2‐fold from rest to last sprint. AT venous IL‐6 increased 15‐fold from 0.4 ± 0.4 at rest to 7.0 ± 4 pg × mL−1 (p < 0.0001) and AT v–a difference increased 45‐fold from 0.12 ± 0.3 to 6.0 ± 5 pg x mL−1 (p < 0.0001) 9 min after last sprint. Arterial glycerol increased 2.5‐fold from rest to 9 min postsprint 1 (p < 0.0001) and was maintained during the exercise period. AT venous and v–a difference of glycerol increased 2‐fold from rest to 9 min postsprint 1 (p < 0.0001 and p = 0.01, respectively), decreased until 18 min postsprint 2 (p < 0.001 and p < 0.0001), and then increased again until 9 min after last sprint (both p < 0.01). Conclusions: The concurrent increase in venous IL‐6 and glycerol in AT after last sprint is consistent with an IL‐6 induced lipolysis in AT. Glycerol data also indicated an initial increase in lipolysis after sprint 1 that was unrelated to IL‐6. Increased IL‐6 in adipose tissue may, therefore, complement other sprint exercise‐induced lipolytic agents. [ABSTRACT FROM AUTHOR]

15. Facing the Unknown: Distributed Project Team Guidelines.

Abstract

Teams working in distributed projects go through the same stages and encounter the same problems as typical face-to-face teams. The main difference when the environment is moved to a distributed setting is that some problems become more complex, other problems become less noticeable, and some new obstacles occur. One of the main problems with distributed work is getting to know each other and building trust, but there are other, more practical issues that also need to be dealt with when people are working together at a distance. This paper presents a set of guidelines developed from research with various distributed project based courses. Guidelines include: synchronize the teams; choose technology that works; choose the right technology for the right activity; discuss norms and rules for behavior and communication; discuss and carefully plan any face-to-face visits; have team building and social activities; and think proactively. The conclusion is that students are not conscious of the difficulties of distributed communication and collaboration, and they need guidance in resolving and avoiding common problems. (Author/AEF)

Abstract

Syftet med studien är att belysa hur förskollärare beskriver förutsättningar för ett inkluderande arbetssätt i förskolan med barn i behov av särskilt stöd. Studien utgår ifrån följande frågeställning: Vilka faktorer påverkar förskollärares förutsättningar att genomföra ett inkluderande arbete i förskolans verksamhet? Vi har grundat forskningsarbetet i ett fenomenografiskt perspektiv där variation av förskollärares uppfattningar och erfarenheter har identifierats. Studien är utförd som en kvalitativ studie med semistrukturerade intervjuer av förskollärare verksamma i samma kommun och de gjordes på plats och över webb-verktyget Microsoft Teams. Studiens resultat pekar på vikten av förskollärares kompetens, erfarenhet, förhållningssätt samt samverkan med vårdnadshavare som förutsättningar till en inkluderande utbildning för barn i behov av särskilt stöd.

Abstract

Recommendations on physical activity and sedentary behaviour for improved health have been prepared by Professional Associations for Physical Activity (YFA) and approved by the Swedish Society of Medicine. All adults should do aerobic physical activity 150-300 minutes at moderate or 75-150 minutes at high intensity, or combined, at a weekly basis. For additional health benefits, muscle-strengthening activity should be performed on at least 2 days a week, and sedentary time should be limited and replaced by physical activity. Older adults should, as part of their weekly physical activity, do multicomponent physical activity that emphasizes balance and strength on at least 2-3 days a week to enhance functional capacity and prevent falls. The benefits of physical activity outweigh the risks. The Swedish National Board of Health and Welfare recommends that healthcare providers offer counselling with exercise on prescription to individuals with physical activity under the recommended dose.

Bakgrund: Operationssjuksköterskan ansvarar för patientens perioperativa omvårdnad där målet är att utföra individanpassad och professionell vård på en personcentrerad nivå. Den dagkirurgiska och den polikliniska verksamheten ökar i takt med att vården utvecklas vilket innebär att allt fler patienter opereras under regional anestesi idag. Detta ställer högre krav på operationssjuksköterskor vilket medför att de måste förstå hur patienter kan uppleva en vaken operation för att kunna ge god och personcentrerad vård. Forskningsfråga: Hur upplever patienter som genomgår regional anestesi att vara vakna under operation? Metod: Uppsatsen är utformad som en kvalitativ metasyntes där tolv studier inkluderats i resultatet. Metasyntes innebär att tidigare forskning förs samman och skapar en djupare nivå av ett fenomen. Resultat: Resultatet visade att patienter huvudsakligen behöver information för att kunna ha kontroll och känna sig delaktiga under operationen. Det visade sig att personalen har en betydande roll för att patienterna ska känna sig trygga under operationen. Resultatet visade att patienterna använder sig av olika copingstrategier, bland annat musik, för att hantera situationen de befinner sig i under en operation. Slutsats: Personalen har en betydande roll för patientens upplevelse under en operation. För att de ska känna sig delaktiga och ha kontroll över situationen behöver patienterna vara väl informerade. Det krävs att operationssjuksköterskor avsätter tid till patienten för att skapa en relation som kan ligga till grund för att kunna tillämpa en personcentrerad vård. Dock är detta en utmaning för operationssjuksköterskor idag då tiden med patienter före en operation ofta är mycket kort och intensiv. Abstract

The effects of endurance exercise with different degrees of oxygen delivery were studied in mRNA expression of vascular endothelial growth factor (VEGF), fibroblast growth factor-2 (FGF-2) and hypoxia-inducible factor (HIF) subunits alpha and beta in human skeletal muscle. Muscle biopsies were taken before and after 45 minutes of one-legged knee extension exercise performed under conditions of nonrestricted or restricted blood flow at the same absolute workload. Findings indicate that VEGF but not FGF-2 gene expression is upregulated correlating to changes in HIF.

Abstract

The effect of the activation status of the pyruvate dehydrogenase complex on the kinetics of skeletal muscle phosphocreatine is investigated in nine male subjects to determine the determinant of the oxygen deficit incurred during submaximal exercise. Results reveal that the availability of acetyl groups is a significant factor in increasing mitochondrial respiratory activity at the beginning of the physical activity and would contribute to the oxygen deficit.
