Unsteady, 3D particle tracking velocimetry (PTV) data are applied as an inlet boundary condition in a direct numerical simulation (DNS). The considered flow case is a zero pressure gradient (ZPG) turbulent boundary layer (TBL) flow over a flat plate. The study investigates the agreement between the experimentally measured flow field and its simulated counterpart with a hybrid 3D inlet region. The DNS field inherits a diminishing contribution from the experimental field within the 3D inlet region, after which it is free to spatially evolve. Since the measurement does not necessarily provide a spectrally complete description of the turbulent field, the spectral recovery of the flow field is analyzed as the TBL evolves. The study summarizes the pre-processing methodology used to bring the experimental data into a form usable by the DNS as well as the numerical method used for simulation. Spectral and mean flow analysis of the DNS results show that turbulent structures with a characteristic length on the order of one average tracer particle nearest neighbor radius r¯NN or greater are well reproduced and stay correlated to the experimental field downstream of the hybrid inlet. For turbulent scales smaller than r¯NN, where experimental data are sparse, a relatively quick redevelopment of previously unresolved turbulent energy is seen. The results of the study indicate applicability of the approach to future DNS studies in which specific upstream or far field boundary conditions (BCs) are required and may provide the utility of decreasing high initialization costs associated with conventional inlet BCs., Deutsche Forschungsgemeinschaft, Projekt DEAL

IMPORTANCE: Ventricular tachycardia (VT) is associated with high mortality in patients with cardiac sarcoidosis (CS), and medical management of CS-associated VT is limited by high failure rates. The role of catheter ablation has been investigated in small, single-center studies. OBJECTIVE: To investigate outcomes associated with VT ablation in patients with CS. DESIGN, SETTING, AND PARTICIPANTS: This cohort study from the Cardiac Sarcoidosis Consortium registry (2003-2019) included 16 tertiary referral centers in the US, Europe, and Asia. A total of 158 consecutive patients with CS and VT were included (33% female; mean [SD] age, 52 [11] years; 53% with ejection fraction [EF]

Cardiac sarcoidosis is a component of an often multiorgan granulomatous disease of still uncertain cause. It is being recognized with increasing frequency, mainly as the result of heightened awareness and new diagnostic tests, specifically cardiac magnetic resonance imaging and18F-fluorodeoxyglucose positron emission tomography scans. The purpose of this case-based review is to highlight the potentially life-saving importance of making the early diagnosis of cardiac sarcoidosis using these new tools and to provide a framework for the optimal care of patients with this disease. We will review disease mechanisms as currently understood, associated arrhythmias including conduction abnormalities, and atrial and ventricular tachyarrhythmias, guideline-directed diagnostic criteria, screening of patients with extracardiac sarcoidosis, and the use of pacemakers and defibrillators in this setting. Treatment options, including those related to heart failure, and those which may help clarify disease mechanisms are included.

Background An electrical storm (ES) is a life-threatening condition that affects up to 20% of patients with implantable cardioverter defibrillators. In this small retrospective study, we report our results with left video-assisted thoracoscopic sympathectomy/ganglionectomy (VATSG) to treat refractory ES in low–ejection fraction patients who were not candidates for catheter ablation. Methods We identified 12 patients who presented with ES and underwent a total of 14 video-assisted thoracoscopic sympathectomy/ganglionectomy, including 3 patients on venoarterial extracorporeal membrane oxygenation. We reviewed demographic data, survival to discharge, number of cardioversions (before and after VATSG), need for readmissions, and need for right-sided procedures. Results In the 30 days before a left VATSG, mean number of shocks was 22.67 for all patients. For the patients who survived to discharge, the mean was 3.55 since surgery and the median was zero shocks after a median follow-up of 358 days. Six patients did not experience further cardioversions since the last VATSG and 5 were not readmitted for ventricular tachycardia. Two patients had staged bilateral procedures owing to recurrences; of those, 1 did not require further cardioversions. Conclusions Limited left VATSG is an appropriate and effective initial treatment for ES patients who are not candidates for catheter ablation, including those on venoarterial extracorporeal membrane oxygenation for hemodynamic support.

Computational results for a full-scale simulation of a Gulfstream G-III aircraft are presented. In support of a NASA airframe noise flight test campaign, Exa Corporation’s lattice Boltzmann PowerFLOW® solver was used to perform time-accurate simulations of the flow around a highly detailed, full-scale aircraft model. Free-air boundary conditions were used at a Mach number of 0.23 and a Reynolds number of 10.5 × 10(exp 6) based on mean aerodynamic chord. This paper documents the simulation campaign for the baseline aircraft configuration at several flight conditions, including multiple flap deflections and main landing gear deployed or retracted. The high-fidelity, synthetic data were post-processed using a Ffowcs-Williams and Hawkings integral approach to estimate farfield acoustic behavior, with pressures on the model solid surface or a permeable surface enveloping the acoustic near field used as input. The numerical approach, simulation attributes, and the effects of grid resolution, gear deployment, and multiple flap deflections, are discussed as well.

Computational results are presented for a high-fidelity, full-scale, full-span Gulfstream G-III aircraft model equipped with flap and main landing gear (MLG) noise reduction technologies. The simulations, which were conducted in support of a NASA airframe noise flight test campaign of the same technologies, use the lattice Boltzmann solver PowerFLOW® to capture time-accurate flow data with sound propagation to the far field accomplished using a Ffowcs-Williams and Hawkings (FWH) acoustic analogy approach. The aerodynamic and aeroacoustic behavior of the aircraft were investigated in the approach configuration with combinations of flap and landing gear deployments. The simulated flap concept is an Adaptive Compliant Trailing Edge (ACTE) flap that replaces the Fowler flap system on the G-III aircraft. The simulated MLG noise reduction concept is comprised of porous fairings and a collection of other smaller fairings fitted around the flow-facing components. Using the Fowler flap results as a reference, comparisons are presented on the noise reduction effectiveness of the ACTE flap system. Investigations were made on the effects of using the porous fairings and ACTE flap as noise reduction concepts in tandem. The ACTE flap was found to reduce the total airframe noise level at all flap deflection angles when compared to the Fowler flap equipped model. As anticipated, a reduction in aerodynamic performance was also found when the ACTE flap system was used. The MLG fairings were shown to further reduce the total airframe noise level of the G-III.

This paper presents a design modification process carried out on a real two-wheel main landing gear configuration driven by a numerical prediction of its aeroacoustic performances. Extensive flow simulations performed using the lattice Boltzmann method are used to explore a wide range of design parameters and different combinations of low-noise devices applied to the inner and outer parts of the wheels. The search of optimal configurations is supported by a deep analysis of the source mechanisms performed by combining several flow and acoustic post-processing techniques, including standard FfowcsWilliams and Hawkings, planar and stereo beam-forming computations and an analysis of the volume source terms in different frequency bands. A combined use of brake and wheel fairings leads to a noise reduction of more than 1.5 EPNdB, corresponding to a 16% reduction of the on-ground surface where the EPNL exceeds 65 EPNdB, on a fully detailed geometry.

Numerical simulations have been performed for a vertical tail configuration with deflected rudder. The suction surface of the main element of this configuration is embedded with an array of 32 fluidic actuators that produce oscillating sweeping jets. Such oscillating jets have been found to be very effective for flow control applications in the past. In the current paper, a high-fidelity computational fluid dynamics (CFD) code known as the PowerFLOW(Registered TradeMark) code is used to simulate the entire flow field associated with this configuration, including the flow inside the actuators. The computed results for the surface pressure and integrated forces compare favorably with measured data. In addition, numerical solutions predict the correct trends in forces with active flow control compared to the no control case. Effect of varying yaw and rudder deflection angles are also presented. In addition, computations have been performed at a higher Reynolds number to assess the performance of fluidic actuators at flight conditions.
