To investigate the feasibility and safety of the retropubic mid-urethral sling (MUS) procedure with a resorbable mesh implant made of poly-4-hydroxybutyrate (P4HB). A prospective clinical cohort study with 24 months follow-up. A tertiary academic hospital. Seventeen women with moderate to severe stress urinary incontinence (SUI). A retropubic MUS procedure with P4HB sling. Seventeen women underwent an uncomplicated MUS procedure with a new sling made of resorbable P4HB. Following safety criteria, there has been no need to stop the study before 24 months' end point. Three serious adverse events were reported during follow-up: (1) inability to void urine after 1 week postsurgery, (2) a hysterectomy and bilateral oophorectomy performed due to a precancerous endometrial lesion at 16 months after initial MUS procedure and (3) exposed mesh with a part of device sheath left behind. These participants underwent an uncomplicated reintervention with no additional and/or permanent harm. Reinterventions because of failure of cure contained 3 second MUS procedures and one admission of urethral bulking agent. Seventy-six percent and 47 percent of patients were objectively cured of SUI at 12 and 24 months, respectively. Preliminary evidence suggests that a sling made of P4HB can be safely used for MUS procedures and could be an alternative for permanent polypropylene slings in patients that prefer the use of nonpermanent material. A Ni(meso-daco) [where mese-daco= mercaptoethyl)(sulfinatoethyl)diazacyclooctane] (Ni-2) complex of W[(CO).sub.5] was isolated to compare its donor ability to the analogous ([[nu].sup.1]-Ni-1)W[(CO).sub.5] adduct (where Ni-1 is Ni(bme-daco) and (bme-daco)=bis(mercaptoethyl)-diazacycloethyl)sulfinatoethyl)diazacyclooctane]. It was shown that modification of the thiolate S to sulfinate could serve as a block to bidentate coordination that prevails in the nickel dithiolato complexes without significantly altering the monodentate ligation and donor ability of such metalloligands.

The synthesis of Ni[N.sub.2][S.sub.2] metalloligands, including the solid-phase synthesis of resin-bound Ni[(CGC).sup.2-], and sulfur-based derivatization with W[(CO).sub.5] and Rh[(CO).sub.2.sup.+] are carried out and described. Through comparison to analogous well characterized, solution-phase complexes, Attenuated Total Reflectance FTIR spectroscopy establishes the presence of unique heterobimetalic complexes both in solution and immobilized on resin beads.

The physical properties and structures of a series of six complexes of the type (NiN2S2)W(CO)4 are used to establish electronic and steric parameters for square planar NiN2S2 complexes as bidentate, S-donor ligands. It is found that there is little difference in donor abilities of the five neutral ligands, while the dianionic ligand transfers more electron density onto the W(CO)4 moiety, and the spatial orientations of the ligands remain fixed with respect to the W(CO)4 moiety.

The preparation and characterization of the three new linear compounds of the type Co3(dpa)(sub 4)X(sub 2) in which X = NCS-, CN- and N(CN)(sub 2)(super -) are reported. For all the compounds, the magnetic behavior is quite similar and as the temperature increases, the effective moments increase gradually, but since saturation is not reached, even at 400 K, the high-spin state cannot be assigned.

Study Objective: To determine complications and related reintervention rates associated with use of the Uphold Vaginal Support System (Boston Scientific, Boston, MA) for symptomatic vaginal apical prolapse.Design: A multicenter retrospective study.Setting: Two teaching hospitals.Patients: Fifty-nine women with symptomatic vaginal apical prolapse.Intervention: Vaginal apical prolapse surgery using the Uphold Mesh Kit system with or without other concomitant procedures.Measurements and Main Results: A chart review was performed, including the following parameters: perioperative and postoperative complications, repeat surgery, and recurrence rate. A total of 59 patients met the criteria for inclusion in the study. Bladder perforation occurred perioperatively in 1 patient. Postoperative voiding difficulties were observed in 16 patients (27.1%), including 9 women (15.2%) who left the hospital with an indwelling catheter in place. There were 5 cases (8.5%) of transient groin pain, all of which resolved spontaneously. One patient developed a vaginal hematoma. Nine women (15%) required reoperation, including 4 (6.7%) because of recurrent prolapse and 1 (2%) for pelvic pain considered related to the mesh. Three patients (5%) required release of a midurethral sling (MUS) that had been placed concomitantly with the Uphold system. Two patients (3%) required a MUS for de novo stress incontinence.Conclusion: Use of the Uphold Vaginal Support System for symptomatic vaginal apical prolapse was associated with a significant risk of obstructed micturition. In our study population, 15% required repeat surgery, mainly for recurrent pelvic organ prolapse and de novo stress urinary incontinence. Modification of the nickel dithiolate metalloligand, Ni(bme-daco) [where bme-daco = bis(mercaptoethyl)-diazacyclooctane] or Ni-1, by oxygenation of one thiolate into a sulfinate, Ni(mese-daco) [where mese-daco = (mercaptoethyl)(sulfinatoethyl)diazacyclooctane] or Ni-2, restricts the ligating ability to monodentate and is expected to reduce the donor ability of the remaining thiolate S. Nevertheless, the Ni-2 complex forms a stable thiolate S-bound adduct of W0(CO)5, (Ni-2)W(CO)5, a complex whose ν(CO) IR spectrum reports insignificant differences in the donor abilities of Ni-1 and Ni-2 in (η¹-NiN2S2)W(CO)5 complexes. In the presence of the strong sulfophile CuI, a CuNi2 trimetallic, (Ni-2)2CuBr, was isolated. Another trimetallic, (μ-η²-NI-1)[W(CO)5]2, demonstrated the Ni(bme-daco), Ni-1, unit to bridge low-valent metals in a transoid configuration, yielding W-W distances of over 5 Å. According to the v(CO) stretching frequencies and associated computed Cotton-Kraihanzel force constants of the tungsten carbonyl adducts, there is little difference in donor abilities of the five neutral NiN2S2 metallodithiolate ligands in the series. The dianionic Ni(ema)2- (ema = N,N'- ethylenebis(2-mercaptoacetamide)) complex transfers more electron density onto the W(CO)4 moiety. A ranking of donor abilities and a comparison with classical bidentate ligands is as follows: Ni(ema)= > {[NiN2S2]0} > bipy ≈ phen > Ph2PCH2CH2PPh2 > Ph2PCH2PPh2. Electrochemical data from cyclic voltammetry find that the reduction event in the (NiN2S2)W(CO)4 derivatives is shifted to more positive potentials by ca. 0.5 V compared to the ca. -2 V NII/I redox event in the free NiN2S2 ligand, consistent with the electron drain from the nickel-dithiolate ligands by the W(CO)4 acceptor. Differences in NiII/I ΔE1/2 values appear to have a ligand dependence which is related to a structural feature of the hinge angle imposed by the (μ-SR)2 bridges. Thus the angle formed by the intersection of NiN2S2/WS2C2 planes has been established by X-ray diffraction analyses as a unique orientational feature of the nickel-dithiolate ligands in contrast to classical diphosphine or diimine ligands and ranges in value from 136 to 107°. Variable- temperature 13C NMR studies show that the spatial orientations of the ligands remained fixed with respect to the W(CO)4 moiety to temperatures of 100 °C. [ABSTRACT FROM AUTHOR]
