21. Detection of Carbapenem-resistance in CRE by Comparative Assessment of RAPIDEC® CARBA NP and Xpert™Carba-R Assay.

Abstract

Introduction: Carbapenem-resistant Enterobacteriaceae (CRE) infections resist nearly most available antimicrobials, resulting in poor clinical outcomes. Saudi Arabia has a relatively high CRE prevalence. This study aims to evaluate the sensitivity of Rapidec Carba NP test and GeneXpert Carba-R assay compared with conventional manners for detection of carbapenemase-producing Enterobacteriaceae. Methods: This is a cross-sectional study including a total of 90 CRE isolates examined at two tertiary hospitals in KSA from October 2020 to December 2021. Gram-negative Enterobacteriaceae were identified by using Vitek 2 system and were furtherly tested for imipenem and meropenem susceptibility by E- test strips, followed by Rapidec Carba NP test and the Xpert™Carba-R assay. Results: Carbapenem-resistant K. pneumoniae (78.9%) and carbapenem-resistant E. coli (14.4%) were the two most common isolates species. Colistin (98.9%) and tigecycline (88.9%) were the most effective antibiotics against CRE isolates, followed by amikacin (52.2%), gentamicin (33.3%), cotrimoxazole (15.6%), and ciprofloxacin (8.9%). blaOXA-48 was the predominant carbapenemase gene (44.4%), followed by blaNDM (32.2%). blaKPC gene was not detected. The Rapidec Carba NP and the Xpert™Carba-R demonstrated an overall sensitivity of 69.3% and 88%, respectively, in comparison to gold standard detection of meropenem and imipenem resistance by Vitek 2 system and E- test strips. Discussion: RAPIDEC® CARBA NP may be a beneficial screening test for detecting CRE, but for confirmation of the results, Xpert Carba-R assay is more sensitive, significantly lowering the turnaround time compared to reference traditional methods. Abstract

Background: Tuberculosis (TB) is one of the deadliest health problems. Accurate rapid diagnosis and prompt treatment are crucial for cure. The aim of this study was to investigate the role of GeneXpert PCR in the rapid detection of Mycobacterium tuberculosis complex (MTBC) directly from clinical samples and to determine the anti- TB drug resistance patterns in pulmonary tuberculosis. Methods: This is a cross-section analysis, involving a total of 92 patients with pulmonary TB (primary TB resistance); 37 Saudis and 55 non-Saudis. Respiratory specimens were processed, and examined by flurochrome stain. Culture and susceptibility testing was performed by VersaTREK, a liquid culture system. Adding, MTBCwas directly detected from the sample by GeneXpert PCR. Results: GeneXpert assay achieved 96 % sensitivity and 100 % specificity for the detection of MTBC in sputum. Resistance to pyrazinamide was (6.5%), followed by rifampicin (2.2%), streptomycin and isoniazid (1.1%). Monoresistance to pyrazinamide and streptomycin (5.4%) and (1.1%) respectively. Multidrug resistane (MDR)-TB was (1.1%) and there was no substantial difference in anti- TB resistance between Saudi and non-Saudi TB patients. Conclusions: The GeneXpert PCR is a very helpful tool for the detection of MTBC in respiratory specimens with a high sensitivity, specificity and accuracy within a shorter time as compared to conventional methods. Recommendations: Costs-benefit analysis of GeneXpert versus other recently deployed TB diagnostic systems may be required on a wider scale. [ABSTRACT FROM AUTHOR]

Abstract

Toxoplasma gondii, a common parasitic infection, has a special affinity to the brain. It has a life-long existence without an apparent clinical disease. While the etiology of Bipolar disorder (BD) remains unclear, epidemiological studies suggest a role for infections. CNS is particularly susceptible to oxidative stress because of its high metabolic rate and its low levels of antioxidant defenses. Oxidative stress is a contributor to the initiation and progression of many neurological illnesses. Oxidative stress injury is a constantly and compelling finding associated with BD and toxoplasmosis. This cross-sectional study has investigated a possible role of toxoplasma-induced oxidative stress in the development of BD. Healthy controls and BD patients were examined for anti ToxoplasmaIgG and two; protein (3-nitrotyrosine) and DNA (8-hydroxy-2′-deoxyguanosine) oxidative stress markers. Toxoplasmapositivity was higher (40%) among BD patients compared to controls (12%). Significantly higher levels of anti-ToxoplasmaIgG were detected in BD patients compared to controls. Nitrotyrosine (796.7±106.28) and especially 8-OHdG (20.31±8.38) were significantly higher, among Toxo-positive BD compared to Toxo-negative BD (675.97±144.19; 7.44±2.86) and healthy controls (464.02±134.6; 4.17±1.43). These findings might indicate a role for Toxoplasmainfection in the development of BD possibly through creating a highly oxidative brain environment.

Abstract

Strain JCET was isolated from the fecal sample of a 24-year-old obese man living in Jeddah, Saudi Arabia. It is an aerobic, Gram-positive, rod-shaped bacterium. This strain exhibits a 16S rRNA nucleotide sequence similarity of 97.5 % with Bacillus niacini, the phylogenetically closest species with standing nomenclature. Moreover, the strain JCET presents many phenotypic differences, when it is compared to other Bacillus species, and shows a low MALDI-TOF Mass Spectrometry score that does not allow any identification. Thus, it is likely that this strain represents a new species. Here we describe the features of this organism, together with the complete genome sequence and annotation. The 4,762,944 bp long genome (1 chromosome but no plasmid) contains 4,654 protein-coding and 98 RNAs genes, including 92 tRNA genes. The strain JCET differs from most of the other closely Bacillus species by more than 1 % in G + C content. In addition, digital DNA-DNA hybridization values for the genome of the strain JCET against the closest Bacillus genomes range between 19.5 to 28.1, that confirming again its new species status. On the basis of these polyphasic data made of phenotypic and genomic analyses, we propose the creation of Bacillus jeddahensis sp. nov. that contains the strain JCET.
