Abstract

HIV pre-exposure prophylaxis (PrEP) has shown high efficacy and effectiveness for HIV prevention; however, many individuals with PrEP indications are not receiving PrEP. PrEP Coordinators work closely with patients and health care providers to increase PrEP access, and they provide unique insights into the inner workings of PrEP care and service delivery. In this study, we discuss key challenges and recommendations for improved PrEP service delivery (including training PrEP Coordinators to manage PrEP panels, making PrEP a part of routine care and optimizing electronic health records, designating a PrEP "champion" who can strengthen communication and leadership, using a proactive approach to increase PrEP retention, and training providers and PrEP Coordinators to meet youth-specific needs) from our discussions with the PrEP Coordinators who led PrEP panel management in San Francisco Department of Public Health primary care clinics.

Abstract

BackgroundMany interventions delivered to improve health may benefit not only direct recipients but also people in close physical or social proximity. Our objective was to review all published literature about the spillover effects of interventions on health outcomes in low-middle income countries and to identify methods used in estimating these effects.MethodsWe searched 19 electronic databases for articles published before 2014 and hand-searched titles from 2010 to 2013 in five relevant journals. We adapted the Cochrane Collaboration's quality grading tool for spillover estimation and rated the quality of evidence.ResultsA total of 54 studies met inclusion criteria. We found a wide range of terminology used to describe spillovers, a lack of standardization among spillover methods and poor reporting of spillovers in many studies. We identified three primary mechanisms of spillovers: reduced disease transmission, social proximity and substitution of resources within households. We found the strongest evidence for spillovers through reduced disease transmission, particularly vaccines and mass drug administration. In general, the proportion of a population receiving an intervention was associated with improved health. Most studies were of moderate or low quality. We found evidence of publication bias for certain spillover estimates but not for total or direct effects. To facilitate improved reporting and standardization in future studies, we developed a reporting checklist adapted from the CONSORT framework specific to reporting spillover effects.ConclusionsWe found the strongest evidence for spillovers from vaccines and mass drug administration to control infectious disease. There was little high quality evidence of spillovers for other interventions.

Abstract

The landscape and institutional changes in the region of Madrid over the last three decades have shaped current decision-making process in the planning system. Madrid is characterised by the intensive use of land and a range of stakeholders related to the stratified administrative structure, which generates social & environmental conflicts. This research links different themes such as territorial planning and governance, using an integrative perspective to search for sustainable strategies. The appreciative systems approach, based on conceptual modelling, is used to understand the complex processes involved. The modelling process provides a valuable tool of simplifying the complexity. The development of the conceptual model is divided into two processes; the first model exercise is an abstraction of the decision-making process in land planning. Attributes of the planning and governance systems in Madrid are taken into account. Later, this conceptualization is complemented with quantitative data from the 1950's. This thesis harmonises the quantitative method with the discursive model to understand the complexity of the emergent conflicts in regional land planning and management. Notions such as collaborative planning, adaptive management and multilevel governance, which have recently become fashionable, provide the theoretical discussion. A stakeholder engagement exercise was carried out to encourage the participation of stakeholders in the policy-making process. This thesis suggests a set of future scenarios, alternative policy options, under a common framework of sustainability. The seven policy interventions are detailed in a booklet written in Spanish and English: "Tozvards sustainable land planning in the Autonomous Community of Madrid: Guidelines and Recommendations". A simulation model - decision support tool - was used to explore these scenarios and enhance stakeholder participation. This research was instrumental in helping stakeholders communities to adapt to emerging circumstances and changes in the regional planning process.

Abstract

APC/C-Cdh1 desempeña una función esencial en el desarrollo del sistema nervioso, donde coordina la salida de los progenitores neurales del ciclo celular y el inicio de la neurogénesis y, con ello, el tamaño del encéfalo y organización de la corteza cerebral. Recientemente, hemos identificado una nueva mutación de Cdh1 (Asp187Gly) que causa pérdida de actividad de APC/C-Cdh1 y provoca microcefalia, retraso psicomotor y epilepsia en humanos. Estos trastornos están asociados tanto con alteraciones en la neurogénesis, como en la oligodendrogénesis y mielinización. Es más, un examen preliminar imagenológico del encéfalo del paciente que portaba la mutación de Cdh1 mostró una disminución de la sustancia blanca y disgénesis del cuerpo calloso. Por todo ello, creemos que APC/C-Cdh1 podría desempeñar un papel clave en la oligodendrogénesis y la mielinización durante el desarrollo postnatal.

Abstract

Hepatic steatosis is caused by metabolic imbalances that could be explained in part by an increase in de novo lipogenesis that results from increased sterol element binding protein 1 (SREBP-1) activity. The nuclear receptor liver receptor homolog 1 (LRH-1) is an important regulator of intermediary metabolism in the liver, but its role in regulating lipogenesis is not well understood. Here, we have assessed the contribution of LRH-1 SUMOylation to the development of nonalcoholic fatty liver disease (NAFLD). Mice expressing a SUMOylation-defective mutant of LRH-1 (LRH-1 K289R mice) developed NAFLD and early signs of nonalcoholic steatohepatitis (NASH) when challenged with a lipogenic, high-fat, high-sucrose diet. Moreover, we observed that the LRH-1 K289R mutation induced the expression of oxysterol binding protein-like 3 (OSBPL3), enhanced SREBP-1 processing, and promoted de novo lipogenesis. Mechanistically, we demonstrated that ectopic expression of OSBPL3 facilitates SREBP-1 processing in WT mice, while silencing hepatic Osbpl3 reverses the lipogenic phenotype of LRH-1 K289R mice.
