5. Analysis of feasibility of HEOS cold knife hysteroscopic micro-shear separation in clinical treatment of patients with intrauterine adhesion

Abstract

Objective To evaluate the clinical efficacy and safety of hysteroscopy endo-operative system （HEOS） cold knife hysteroscopic micro-shear separation in the clinical treatment of patients with intrauterine adhesion. Methods 200 patients with intrauterine adhesion were enrolled and divided into the observation （n = 100） and control groups （n = 100） according to different treatment methods. In the control group， patients were treated with traditional hysteroscopic electrosurgical separation， and those in the observation group received HEOS cold knife hysteroscopic micro-shear separation. The surgical indexes， clinical efficacy and postoperative complications were statistically compared between two groups. Results In the observation group， the operation time was significantly shorter， and the amount of intraoperative bleeding and absorption of uterine distention fluid were significantly less than those in the control group （all P < 0.05）. There was a significant difference in clinical efficacy between two groups （P < 0.05）. In the observation group， the cure rate was 61%， higher than 45% in the control group （P < 0.05）. In the observation group， postoperative bleeding occurred in 1 case， 2 cases of infection and 1 case of pelvic adhesion. The incidence of postoperative complications was 4%. In the control group， postoperative bleeding occurred in 5 cases， 2 cases of infection， 3 cases of ectopic pregnancy and 5 cases of pelvic adhesion. Abstract

Objective To investigate the efficacy and safety of anlotinib combined with PD-1 blockades for patients with advanced epithelial ovarian cancer (EOC). Methods A retrospective study was performed, enrolling 33 patients with advanced EOC who failed standard systematic therapy. All patients were administered with anlotinib combined with PD-1 blockades. The efficacy and safety profile were determined during treatment. Results The objective response rate of the 33 patients was 36.4% (95%CI: 20.4%-54.9%) and the disease control rate of the patients was 81.8% (95%CI: 64.5%-90.0%). The median PFS and OS of the 33-patient cohort were 7.6 months (95%CI: 3.1-12.1) and 19.6 months (95%CI: 15.1-24.1), respectively. The most common treatment-related adverse reactions were fatigue (66.7%), nausea and vomiting (54.5%), hypertension (48.5%), and diarrhea (39.4%). Furthermore, multivariate Cox regression analysis indicated that ECOG performance status and FIGO stage were independent factors for predicting the PFS of the combination regimen for patients with advanced EOC. Conclusion Anlotinib combined with PD-1 blockades preliminarily exhibit satisfactory efficacy and tolerable safety profile for patients with advanced EOC.

19. Distribution and drug resistance of pathogens in urinary tract infections in Zhoupu area, Shanghai during 2016 to 2020

Abstract

Objective: To study the distribution and drug resistance of pathogens in urinary tract infections(UTI), and to provide evidence for rational treatment of the disease. Methods: The results of bacterial culture from morning clean-catch mid-streamurinein the patients with UIT during July 2016 to June 2020 were retrospectively analyzed to determine the distribution and drug resistance of pathogens. Result A total of 3 543 specimens were collected, 810 strains of pathogens were isolated, and the positive rate was 22.9%, including 558 strains of Gram-negative bacteria (68.9%), 113 strains of Gram-positive cocci (14.0%), 128 strains of Mycoplasma(15.8%), 11 strains of fungi(1.4%). The majority of bacteria strains were Escherichia coli (337 strains, 41.6%) and Klebsiella pneumoniae (67 strains, 8.3%). Ureaplasma urealyticum(UU) accounted for 10.6% (86 strains) inisolated mycoplasma. Escherichia coli and Proteusisolates were sensitive to aminoglycoside and carbapenem antibiotics and the resistant rates were lower than 5%; Klebsiella pneumoniae was resistant to ceftriaxone and levofloxacin and its′ resistant rates were 58.3% and 55.2%, respectively.All Enterococci strains were resistant to penicillin, ampicillin, gentamicin, and levofloxacin, which had higher resistance rates than those in Enterococcus faecalis; Enterococcus faecalis had high resistant rates to levofloxacin and moxifloxacin(73.3% and 64.4%, respectively). Staphylococcus aureus and Enterococcus were sensitive to sulfonamides, glycopeptides and common broad-spectrum antibiotics, and the resistant rates were lower than 5%. The drug resistant rate of UU to ciprofloxacin was 69.9%, and UU was sensitive to both minocycline and doxycycline. Conclusions: The majority of pathogens isolated from UTI patients in Zhoupu District of Shanghai is Gram-negative bacilli, which shows different degree of resistance to common antibiotics. Therefore, the regular monitoring of UTI pathogenic bacteria is important to determine how to choose antibiotics in clinical practice.

A base‐promoted net‐[3+2] cycloaddition of nitriles and 1‐arylpropynes for the synthesis of pyrroles is described. The developed method provides convenient access to various 2,5‐disubstituted or 2,4,5‐trisubstituted pyrroles in 40% to 96% yields (32 examples).
