1. Retinitis linked to human herpesvirus type 6: a case study in a splenectomised patient

Abstract: Abstract Human herpesvirus-6 (HHV-6) is a member of the HHV family and is a rare cause of infectious uveitis. We report a case of a splenectomised patient, hospitalised due to invasive pneumococcal disease, who was diagnosed with retinitis in the right eye, with good visual acuity (0.1 LogMAR). Given the presence of HHV-6 Polymerase chain reaction (PCR) in the cerebrospinal fluid and the serum and the coexistence of a severe central nervous system infection, the ophthalmological features were attributed to the HHV-6 infection. He was treated with topical corticosteroid eyedrops, cyclopentolate, and prednisolone acetate ointment and systemically with intravenous ganciclovir 5 mg/kg bid for 14 days and then oral valganciclovir 900 mg bid for 4 weeks. The diagnosis was promptly presumed, enabling the early initiation of appropriate treatment and contributing to the preservation of the good visual acuity initially observed.

2. Primary Congenital Glaucoma presenting with Acute Corneal Hydrops: a challenging approach to amblyopia prevention

Abstract: Introduction: Acute hydrops is a rare initial manifestation of Primary Congenital Glaucoma (PCG). It can result in significant corneal opacity, posing an additional risk for amblyopia development in PCG patients. We report a case of a newborn with PCG presenting with acute corneal hydrops and significant corneal scarring, discussing the management and challenges of amblyopia prevention. Case Presentation: A 3-day-old female newborn was referred to our department due to bilateral corneal clouding. Anterior segment examination revealed bilateral corneal edema, with severe corneal prolapse in the right eye consistent with acute hydrops. The anterior chamber structures of the right eye were indiscernible due to significant corneal edema. The left eye presented a deep, quiet anterior chamber with no dysgenesis of anterior segment structures. Intraocular pressure (IOP) measurements were >40 mmHg in both eyes. Corneal diameters were 10 mm OD and 12 mm OS. Fundoscopic posterior segment assessment was not possible due to unclear media. B-scan ultrasonography revealed no significant changes. The patient underwent urgent bilateral trabeculotomy. Postoperatively, intense mechanical stretching induced by severely increased IOP was visible, with the right cornea exhibiting a central opacity managed with topical corticosteroids and showing improvement. Right corneal opacity spared the superior cornea, and atropine drops combined with left eye occlusion were employed to mitigate right eye deprivation amblyopia. Eighteen months later, the patient can fixate and follow objects with the right eye. Left eye corneal transparency was restored, and bilateral IOP control was achieved. Conclusion: Despite prompt surgical treatment, acute hydrops presents a postoperative management challenge and may result in irreversible corneal scarring. Timely management of corneal opacities is crucial to prevent amblyopia. A multidisciplinary approach is essential for the best visual outcome.

Abstract: O vírus citomegálico (CMV) é um tipo de vírus de ácido desoxirribonucleico de dupla cadeia que pertence à família herpesviridae. Após uma infeção primária, o vírus torna-se latente em vários tipos de glóbulos brancos. A infeção por CMV pode permanecer latente ou tornar-se ativa, especialmente em indivíduos imunodeprimidos, como aqueles submetidos a transplantes de células progenitoras hematopoiéticas (TPH), onde a reativação do CMV pode ocorrer. Neste contexto, a infeção por CMV é comum e associada a elevadas taxas de morbilidade e mortalidade. A pneumonite é uma das complicações mais graves, com taxas de mortalidade superiores a 50%. Além disso, mesmo na ausência de doença específica do órgão, a infeção por CMV está relacionada com um aumento da mortalidade não relacionada com a recidiva da neoplasia hematológica. Dada a frequência e gravidade desta infeção em doentes submetidos a TPH, é crucial implementar estratégias eficazes de monitorização, prevenção e tratamento. Este protocolo foi desenvolvido para identificar os grupos de doentes que se beneficiam de uma abordagem sistematizada para a infeção por CMV e definir a estratégia mais adequada para cada grupo. A monitorização da carga viral de CMV no sangue periférico é fundamental, especialmente em doentes com risco moderado a elevado de infeção ativa. A profilaxia primária com letermovir (fármaco antiviral) é recomendada para reduzir a incidência de infeção ativa, especialmente em doentes com alto risco. A profilaxia secundária com valganciclovir (fármaco antiviral) é recomendada após um episódio de infeção ativa, enquanto o tratamento de antecipação e de doença é baseado na monitorização da carga viral e na resposta clínica. Este protocolo visa melhorar a abordagem da infeção por CMV em doentes submetidos a TPH, garantindo uma abordagem preventiva e terapêutica eficaz e segura.

5. Contacts with health services for COVID-19-related reasons during the first wave of the pandemic in Portugal: a longitudinal study

Abstract: Introduction Access to COVID-19-related care during the pandemic deserve attention and study to inform future strategies to deal with similar health emergencies in the future. We aimed to describe access to care for COVID-19-related reasons during the first 9 weeks of the pandemic in Portugal, to quantify the frequency of remote and in-person contacts with healthcare for COVID-19-related reasons and to assess the association between individual and context characteristics and contacts with health services.Methods We conducted an internet-based open cohort study with a non-probabilistic sample of 12 006 persons aged 16 years or more. One questionnaire was sent daily from 23 March 2020 for 9 weeks.Results General practitioners (GPs) were the main points of contact of patients with health services for COVID-19-related reasons. In our population, the main drivers found for any difference in the probability of contact with health services for COVID-19-related reasons were perceived high-risk contacts, followed by the existence of COVID-19 main symptoms. There were 17% more (0.17, 95% CI 0.15 to 0.18) contacts with health services among participants who reported personal contact with a confirmed case, 7% more (0.07, 95%CI 0.06 to 0.07) contacts if the contact had been with a suspected case and 6% more (0.06, 95%CI 0.05 to 0.07) contacts among participants who reported COVID-19 main symptoms in the previous 24 hours. Sociodemographic and household factors were not associated with major differences in healthcare contacts. The probability of contact with any health service for COVID-19-related reasons was highest at the beginning of the pandemic.Conclusion Most contacts with health services for COVID-19-related reasons during the first wave of the pandemic were with GPs, highlighting the role of first-contact care with these clinicians in Portuguese Primary Care, particularly in the context of the COVID-19 pandemic. Sociodemographic and household factors were not associated with major risk differences, suggesting that universal coverage and equity worked in our study sample at this pandemic stage.

6. Vaccination After Haematopoietic Stem Cell Transplant: A Review of the Literature and Proposed Vaccination Protocol

Abstract: Background/Objectives: Haematopoietic stem cell transplantation (HCT) induces profound immunosuppression, significantly increasing susceptibility to severe infections. This review examines vaccinations’ necessity, timing, and efficacy post-HCT to reduce infection-related morbidity and mortality. It aims to provide a structured protocol aligned with international and national recommendations. Methods: A systematic review of current guidelines and studies was conducted to assess vaccination strategies in HCT recipients. The analysis included the timing of vaccine administration, factors influencing efficacy, and contraindications. Recommendations for pre- and post-transplant vaccination schedules were synthesised, specifically for graft-versus-host disease (GVHD), immunosuppressive therapy, and hypogammaglobulinemia. Results: Vaccination is essential as specific immunity is often lost after HCT. Inactivated vaccines are recommended to commence three months post-transplant, including influenza, COVID-19, and pneumococcal vaccines. Live attenuated vaccines remain contraindicated for at least two years post-transplant and in patients with ongoing GVHD or immunosuppressive therapy. Factors such as GVHD and immunosuppressive treatments significantly impact vaccine timing and efficacy. The review also underscores the importance of pre-transplant vaccinations and ensuring that patients’ close contacts are adequately immunised to reduce transmission risks. Conclusions: Implementing a structured vaccination protocol post-HCT is critical to improving patient outcomes. Timely and effective vaccination strategies can mitigate infection risks while addressing individual patient factors such as GVHD and immunosuppression. This review highlights the need for tailored vaccination approaches to optimize immune reconstitution in HCT recipients.

Abstract: Introduction: Assessing the integrity of the posterior ligament complex (PLC), as a key element in the characterization of an unstable Thoracolumbar fracture (TLF), is challenging, but crucial in the choice of treatment. Research question: How to create a reproducible score using combined parameters of Computed Tomography (CT) to predict nonobvious PLC injury. How CT parameters relate with PLC status. Material and methods: Retrospective analysis of neurologically intact patients with an acute traumatic TLF, who underwent CT and Magnetic Resonance Imaging (MRI) within 72 h, in the Emergency Department of a single institution between January 2016 and 2022. Four investigators rated independently 11 parameters on CT and PLC integrity on MRI. The interrater reliability of the CT parameters was evaluated, and two risk scores were created to predict PLC injury on CT using the coefficients of the multivariate logistic regression. 12. The Effect of Corporate Social Responsibility on Brand Image and Brand Equity and Its Impact on Consumer Satisfaction

Abstract: This paper aims to study the effect of corporate social responsibility (CSR) on brand image and brand equity and its impact on consumer satisfaction. The study follows a quantitative methodology, using the implementation of an online questionnaire distributed to people who bought, during the pandemic, a product that used a CSR action. Subsequently, data were analyzed through Smart PLS, following the Structural Equation Model. It was possible to conclude that the CSR initiatives positively affect consumer satisfaction through the mediating effect of brand image and brand equity. Further, brand image and brand equity improve when companies use CSR initiatives, and, despite what previous research has concluded, consumer satisfaction is not affected directly when CSR initiatives are used. This research has also showed that CSR’s impact on brand image is higher for men, and CSR initiatives’ impact on brand equity is also higher for regular purchase consumers.

Abstract: A column experiment at a laboratory level was carried out to assess the effect of the application of nanotechnology in the decontamination of soils and alluvial deposits with high levels of potentially toxic elements (PTEs). A suspension of zero-valent iron nanoparticles (nZVI) was injected at three different concentrations in selected samples (two sediments, one soil). For most of the elements, the retention by nZVI was proportional to the concentration of the suspension and the trend was similar. Metals were immobilized by adsorption on the surface layer of the nanoparticles and/or by complexation, co-precipitation, and chemical reduction. By day 60 following injection, the nZVI lost reactivity and the retained species were desorbed and back into the soluble phase. The definition of spatial patterns for PTEs’ distribution allowed for the construction of contamination risk maps using a geostatistical simulation approach. The analysis obtained from the extractable contents of five target elements (Zn, Cu, Cd, Pb, As) was cross-checked with the estimated map network to assess their retention efficiency. Data from the analysis of these elements, in the extractable phase and in the porewater of the sediments/soils, indicate the nZVI injection as a suitable technique for reducing the risk level of PTEs in contaminated Fe-rich tropical environments.

15. Towards a public health approach for palliative care: an action-research study focused on engaging a local community and educating teenagers

Abstract: Abstract Background Education sessions about palliative care among teenagers are uncommon in developed countries. However, very little is known either about the impact of this type of intervention or about how this age-group perceives its impact. The purpose of this study was therefore to (i) implement an education program about palliative care among teenagers and (ii) to investigate the impact of the program on the participants. Methods An action-research study was conducted at a local community parish in Portugal in November 2015. An education programme was purposively built about palliative care, using active educational strategies adapted for teenagers. Quantitative and qualitative techniques and instruments were used for data collection: questionnaire; reflective diaries; interviews and written testimony. The program had three stages: preparation; intervention; and evaluation. Qualitative data were analysed using thematic content analysis; quantitative data were analysed descriptively. Results 69 people (47 teenagers) participated in the education program. Findings show that the education program contributed to creating awareness about palliative care. Both the teenagers and other participants assessed the education program positively. At the end of the program, teenagers had a constructive message about palliative care. Conclusions The education-intervention contributed to create awareness about palliative care among the participant teenagers, who ended the program with a positive message about palliative care. Based on our findings, the following policy implications can be drawn: (1) Further research is needed to evaluate the effect of education programs about palliative care among younger age groups (teenagers and children), particularly in relation to the changing of attitudes toward palliative care. (2) Education about palliative care should be promoted to local communities, involving all age groups, to foster involvement, participation and empowerment. (3) Compassionate communities should be promoted to enhance the health and wellbeing of all citizens at the end of their life.

16. Prevalence of general and abdominal obesity in Portugal: comprehensive results from the National Food, nutrition and physical activity survey 2015–2016

Abstract: Abstract Background This study includes, for the first time, estimates of general and abdominal obesity prevalence for all ages of the Portuguese population, using common standardized methodologies. Results are compared by sex, age groups, educational level and geographical regions. Methods Participants were a representative sample of the Portuguese population aged between 3 months and 84 years of age (n = 6553), enrolled in the National Food, Nutrition and Physical Activity Survey, 2015–2016. Objective anthropometric measurements included length/height, weight and body circumferences, performed according to standard procedures. Body mass index (BMI) was classified according to the World Health Organization (WHO) growth charts for children and adolescents, and WHO criteria for adults. Abdominal obesity was defined in adults as waist-hip ratio ≥ 0.85 in women or ≥ 0.90 in men. Prevalence estimates and 95% confidence intervals (95%CI) were weighted according to a complex sampling design, considering stratification by seven geographical regions and cluster effect for the selected Primary Health Care Unit. Results The national prevalence of obesity is 22.3% (95%CI: 20.5–24.0), significantly higher in women. Obesity prevalence is much higher in the elderly (39.2%, 95%CI. 34.2–44.2), while children and adolescents have the lowest prevalence around 8–9%. In a regression model, three knot points denoting an inflection of obesity prevalence across the life span were observed around 5, 15 and 75 years. The prevalence of pre-obesity at national level is 34.8% (95%CI: 32.9–36.7), higher in men, and almost 18% of children and 24% of adolescents have pre-obesity. The sex- and age-standardized prevalence of obesity ranged from 38.3% (95%CI: 34.6–42.1) to 13.1% (95%CI: 10.3–15.9) for the less and the most educated individuals, respectively. Although some geographical region disparities, obesity prevalence did not significantly differed across regions (p = 0.094). The national prevalence of abdominal obesity in adults is 50.5% (95%CI: 47.9–53.1), particularly high in the elderly (80.2%). Conclusion Almost 60% of the general Portuguese population is obese or pre-obese. Women, elderly and less educated individuals present the highest obesity prevalence. Abdominal obesity, in particular, seems to be a relevant public health problem among the elderly men.

O domínio despótico do homem sobre o universo alterou a estrutura profunda do planeta, a deterioração do ambiente está a avançar a um ritmo avassalador colocando as gerações atuais e, princi-palmente, as gerações futuras, em risco. Este artigo explora a questão dos refugiados ambientais para quem a “casa comum” é mais (in)comum. A nossa análise parte da narrativa do começo do livro do Génesis e da Encíclica Laudato Si (e outros documentos da Igreja Católica) e compara o modo de agir do homem sobre a natureza, propondo o sentido humano da ecologia.The relationship of human beings with nature has been changing. According to the Biblical narrative of the beginning, Man should take care of nature, not master or dominate it. This “oppressive” domination over the universe has undergone profound changes in the structure of the planet. Environmental deterioration is advancing at an overwhelming pace, putting current generations and especially future generations at risk. This Article describes environmental refugees as victims to whom the "common house" has become an “in-common house”. Abstract: In areas contaminated by potentially toxic elements (PTEs), knowledge of processes of metal mobilisation is the basis for the choice of appropriate remediation methodologies. The mobilisation of metals is a function of several factors, and the response to these factors must be well known during the planning of remediation strategies. The activity of an ore metallurgical plant in South-East Brazil resulted in major contamination by several heavy metals. Reversing the contamination’s negative impact required geochemical assessment of the area, including the physicochemical characterisation, quantification, and delimitation of PTEs, and the rating of the solubilisation/mobilisation capacity of these elements. The definition of spatial patterns for PTEs’ distribution allowed the construction of contamination risk maps which work as a tool for the mitigation and control of the contamination plume. The chemical analysis of interstitial water and selective and sequential extraction methodologies showed that elements that occur in the environment in critical concentrations (Zn, Cd, Pb, As) are mostly associated with easily mobilised forms (soluble, exchangeable cations, associated with Mn oxides). Given the great mobility of the contamination plume, any process of removal of contaminated material becomes unfeasible, thus the strategy of remediation for the stream and associated alluvial deposits must be based on methods of in situ decontamination.

20. Characteristics associated with media use in early adolescence

Abstract: This study aimed to identify socioeconomic and behavioral characteristics associated with media use in early adolescence. The sample was comprised of 1,680 adolescents (884 girls and 796 boys) aged 13, attending private and public schools in Porto, Portugal, during 2003/2004. Adolescents completed questionnaires pertaining to demographic, social and behavioral characteristics, including the time spent watching television and playing computer games on week and weekend days. Logistic and proportional ordinal regressions showed that attending public schools, sleeping less time, using tobacco and presenting low levels of physical activity were factors associated with high media use. In boys living with one of their parents as well as living with younger and older parents were also associated with high media use. Besides the association with less healthier behaviours, we also found higher media use among adolescents from lower social classes and less structured families, which may increase their exposure to negative influence of the media.

Abstract: El cambio climático es uno de los principales desafíos globales enfrentados por la humanidad y en las últimas décadas se volvió una cuestión relevante en la agenda internacional. En 1992, se creó la Convención Marco de las Naciones Unidas sobre el Cambio Climático (CMNUCC), cuya actividad incluye respuestas de mitigación y adaptación. Actualmente, ambas son consideradas igualmente importantes, pero la adaptación fue históricamente marginalizada de la Convención. En este artículo se presenta la trayectoria de la adaptación como pauta en la CMNUCC, se debate su evolución institucional y los mecanismos de financiamiento de la Convención. Por último, se explora la relación entre adaptación y desarrollo. Las conclusiones apuntan hacia la insuficiencia de la CMNUCC en la implementación de medidas de la adaptación al cambio climático, pero destaca su papel relevante en el debate, como fuente de financiamiento, y también como inductora de acciones nacionales y subnacionales, especialmente en los países en desarrollo.

22. Prevalência do consumo de risco de álcool no idoso: estudo numa unidade dos cuidados primários da região de Braga

Abstract: Introdução: O consumo de álcool está associado a riscos e benefícios dependendo da quantidade ingerida. Perceber os padrões de consumo de uma população torna-se fundamental na prevenção dos riscos associados ao álcool. Os idosos, pelas suas particularidades, representam um grupo de interesse maior, que atualmente está subestudado nesta área. Considerando o papel central dos médicos de família na avaliação global do utente, a sua ação preventiva é fulcral. Objetivos: Estimar a prevalência do consumo de risco do álcool em idosos numa unidade de cuidados primários e caracterizar a população em estudo. Métodos: Estudo transversal de uma população de 1.225 indivíduos com 65 ou mais anos de idade. Definiu-se uma amostra aleatória representativa constituída por 210 idosos. A recolha dos dados foi efetuada com recurso à ficha individual do utente. Por consumo de risco definiu-se aquele superior ou igual a 14 unidades padrão de etanol (168g) por semana. Resultados: Da amostra de 210 idosos, com idade média de 73,7±7,7 anos, 57,6% eram mulheres. A prevalência encontrada para o consumo de álcool foi de 63% (IC95%: 56-69) e de consumo de risco de 32,9% (IC95%: 26-39) – nos homens 56,2% (IC95%: 49-62) e nas mulheres 15,7% (IC95%: 10-20), sendo 36,7% (IC95%: 30-41) da amostra abstinente. Verificou-se associação significativa entre o género masculino e o consumo de risco do álcool. A idade e a escolaridade não apresentaram associação com o consumo de risco. Discussão: No idoso, o consumo excessivo de álcool pode ter consequências particularmente gravosas. Não obstante, verifica-se uma escassez de estudos que avaliem o padrão de consumo de álcool nesta população. Observou-se que 63,3% dos idosos consome álcool, com predomínio do género masculino, o que não difere dos dados nacionais. De realçar, porém, que a prevalência do consumo de risco, que inclui um terço da amostra, é superior à encontrada noutros estudos similares. Conclusão: O consumo de risco do álcool é prevalente no idoso, pelo que o médico de família deve estar consciente deste problema.

Abstract: Many naturally occurring xanthones are chiral and present a wide range of biological and pharmacological activities. Some of them have been exhaustively studied and subsequently, obtained by synthesis. In order to obtain libraries of compounds for structure activity relationship (SAR) studies as well as to improve the biological activity, new bioactive analogues and derivatives inspired in natural prototypes were synthetized. Bioactive natural xanthones compromise a large structural multiplicity of compounds, including a diversity of chiral derivatives. Thus, recently an exponential interest in synthetic chiral derivatives of xanthones (CDXs) has been witnessed. The synthetic methodologies can afford structures that otherwise could not be reached within the natural products for biological activity and SAR studies. Another reason that justifies this trend is that both enantiomers can be obtained by using appropriate synthetic pathways, allowing the possibility to perform enantioselectivity studies. In this work, a literature review of synthetic CDXs is presented. The structures, the approaches used for their synthesis and the biological activities are described, emphasizing the enantioselectivity studies.

Abstract: Introdução: Os cuidados paliativos estão intimamente ligados ao conceito de qualidade de vida. Neste trabalho, focaremos a nossa atenção na necessidade de avaliar a qualidade de vida em cuidados paliativos pediátricos oncológicos. Objetivo: Descrever/comparar os instrumentos de medição da qualidade de vida em cuidados paliativos pediátricos oncológicos. Material e Métodos: Pesquisa da bibliográfica dos vários instrumentos de medição da qualidade de vida das crianças em cuidados paliativos, em inglês e português, entre 2000 e 2013, nas bases de dados reconhecidas para este efeito. Resultados: Encontraram-se quinze instrumentos de medição: 10 genéricos e cinco específicos. Identificou-se, para cada instrumento de medição, país de origem, faixa etária para que se encontra direcionado, modalidade de preenchimento, número de dimensões avaliadas, descrição das dimensões, número de perguntas, propriedades psicométricas e validação para português. Discussão: Verifica-se que tem havido uma crescente preocupação em medir a qualidade de vida em idade pediátrica e que a maioria dos instrumentos de medição foi desenhada, nos Estados Unidos, após 1994, coincidindo temporalmente com a definição de qualidade de vida da Organização Mundial de Saúde. Relativamente à faixa etária, a maioria dos instrumentos foi desenvolvida para crianças com idade igual ou superior a oito anos não existindo nenhum para ser respondido apenas pela criança. Podemos verificar que a maioria dos instrumentos de medição, sobretudo os mais atuais, procuram envolver a criança na avaliação da sua própria qualidade de vida relacionada com a saúde através do autopreenchimento (n = 10). Porém, há ainda uma dependência substancial dos pais para a medição da qualidade de vida relacionada com a saúde dos seus filhos, apesar dos estudos que demonstram diferenças na perceção de qualidade de vida relacionada com a saúde entre a criança e os pais. Mas como muitas crianças não têm capacidade de fornecer dados sobre a sua qualidade de vida relacionada com a saúde, quer devido à idade quer porque estão doentes ou com incapacidade funcional, a única possibilidade de obter informação sobre a qualidade de vida relacionada com a saúde destas crianças é o recurso aos pais, a quem é pedido que reflitam sobre a perceção da sua criança, ou adolescente, podendo, assim, justificar-se o preenchimento integral pelos pais de alguns instrumentos de medição. Os instrumentos de medição de qualidade de vida relacionada com a saúde em pediatria são geralmente multidimensionais, destinados a medir o ponto de vista subjetivo em relação ao impacto que a doença e o tratamento têm sobre o bem-estar físico, psicológico e social. Assim, a grande variedade de dimensões, as diferenças no número de dimensões e no número de perguntas entre os diversos instrumentos tendem a refletir os diferentes estádios de desenvolvimento psicomotor da população-alvo. Conclusão: As dimensões mais comummente medidas foram a capacidade física, impacto emocional/psicológico, impacto social e na escola, seguidas da dor e desconforto e nível de atividade. A maioria dos instrumentos é desenvolvida para crianças com idade igual/superior a oito anos. Grande parte dos questionários é de autopreenchimento. Outros podem ser preenchidos/respondidos pelos pais. A maioria dos instrumentos testaram a sua coerência interna (n = 9) e, em menor número, a reprodutibilidade teste-reteste (n = 7) e concordância entre observadores (n = 2). A maioria dos questionários relatou a sua validade de conteúdo (n = 10) e validade de construção (n = 9), poucos analisaram a validade de critério (n = 2). Dos instrumentos analisados, seis questionários estão validados para a população portuguesa (cinco genéricos e um específico de doença oncológica).

27. Estilos de vida e percepção do estado de saúde em idosos Portugueses de zonas rural e urbana.

Abstract: The perception of health status is essential for better planning in health, not only due to its role as a determinant of health, but also because it is related with the adoption of health-promoting behaviours. The aim of the present study was to explore the relationship between lifestyles and self-rated health, in Portuguese elderly, according to their residence in rural or urban areas. We evaluated 80 individuals from Rebordelo, Trás-os-Montes (rural area) and 383 from Porto (urban area), with 60 or more years and with four or less years of education. Data were collected by trained interviewers, through a structured questionnaire, to evaluate social, demographic, health and behavioural characteristics. Anthropometric measures were also obtained. Leisure-time physical activity, tobacco, alcohol and fruit and vegetable consumption were the lifestyles evaluated. To evaluate self-rated health, each person rated their health status, according to five options (excellent, very good, good, reasonable or poor), later grouped into two: poor versus the others. Odds ratio (OR) and 95% confidence intervals (95%CI) were calculated by unconditional logistic regression, after adjustment for sex and education. About 41% of the elderly from the rural area and 32% from the urban area rated their health status as poor. In multivariate analysis, in the rural area, poor self-rated health was inversely associated with the practice of leisure-time physical activity (OR = 0.22, 95%CI 0.06-0.80). In urban area, alcohol consumption was associated with a lower probability of self-rating health as poor (considering not drinking as reference, >0-15 g alcohol/day: OR = 0.42, 95%CI 0.24-0.72; >15 g alcohol/day: OR = 0.60, 95%CI 0.32-1.11). 31. Synthesis and Anti-Inflammatory Evaluation of a Library of Chiral Derivatives of Xanthones Conjugated with Proteinogenic Amino Acids

Abstract: In recent decades, the relationship between drug chirality and biological activity has been assuming enormous importance in medicinal chemistry. Particularly, chiral derivatives of xanthones (CDXs) have interesting biological activities, including enantioselective anti-inflammatory activity. Herein, the synthesis of a library of CDXs is described, by coupling a carboxyxanthone (1) with both enantiomers of proteinogenic amino esters as chiral building blocks (2–31), following the chiral pool strategy. The coupling reactions were performed at room temperature with good yields (from 44 to 99.9%) and very high enantiomeric purity, with most of them presenting an enantiomeric ratio close to 100%. To afford the respective amino acid derivatives (32–61), the ester group of the CDXs was hydrolyzed in mild alkaline conditions. Consequently, in this work, sixty new derivatives of CDXs were synthetized. The cytocompatibility and anti-inflammatory activity in the presence of M1 macrophages were studied for forty-four of the new synthesized CDXs. A significant decrease in the levels of a proinflammatory cytokine targeted in the treatment of several inflammatory diseases, namely interleukin 6 (IL-6), was achieved in the presence of many CDXs. The amino ester of L-tyrosine (X1AELT) was the most effective in reducing IL-6 production (52.2 ± 13.2%) by LPS-stimulated macrophages. Moreover, it was ≈1.2 times better than the D-enantiomer. Indeed, enantioselectivity was observed for the majority of the tested compounds. Thus, their evaluation as promising anti-inflammatory drugs should be considered.

Abstract: Objective/background To describe and characterize insomnia symptoms and nightmare profiles in Portugal during the first six weeks of a national lockdown due to COVID-19. Patients/methods An open cohort study was conducted to collect information of the general population during the first wave of SARS-CoV-2/COVID-19 pandemic in Portugal. We analyzed data from 5011 participants (≥16 years) who answered a weekly questionnaire about their well-being. Two questions about the frequency of insomnia and nightmares about COVID-19 were consecutively applied during six weeks (March–May 2020). Latent class analysis was conducted and different insomnia and nightmare profiles were identified. Associations between individual characteristics and both profiles were estimated using odds ratios (ORs) and 95% confidence intervals (CI). Results Five insomnia (No insomnia, Stable-mild, Decreasing-moderate, Stable-severe, Increasing-severe) and three nightmares profiles (Stable-mild, Stable-moderate, Stable-severe) were identified. Being female, younger, perceiving their income as insufficient and feelings of fear towards COVID-19 were associated with higher odds of insomnia (Women: OR = 6.98 95%CI: 4.18–11.64; ≥60 years: OR = 0.30 95%CI: 0.18–0.53; Insufficient income: adjusted OR (aOR) = 8.413 95%CI: 3.93–16.84; Often presenting fear of being infected with SARS-CoV-2 infection: aOR = 9.13 95%CI: 6.36–13.11), and nightmares (Women: OR = 2.60 95%CI: 1.74–3.86; ≥60 years: OR = 0.45 95%CI: 0.28–0.74; Insufficient income: aOR = 2.60 95%CI: 1.20–5.20; Often/almost always presenting fear of being infected with SARS-CoV-2 infection: aOR = 6.62 95%CI: 5.01–8.74). Having a diagnosis of SARS-CoV-2 virus infection was associated with worse patterns of nightmares about the pandemic. Conclusions Social and psychological individual factors are important characteristics to consider in the development of therapeutic strategies to support people with sleep problems during the COVID-19 pandemic.

Abstract: Average adult height is an indicator of population health and a marker of socioeconomic inequalities. This study aimed to assess how socioeconomic differences affect intergenerational height increase between adults born in 1990 and their parents. Data from a population-based cohort of subjects born in 1990 (EPITeen) were analysed. Participants’ adult height was objectively measured. Parental height, education, and occupation were reported by the parents. The height difference between daughters and their mothers (n=707), and sons and their fathers (n=647) was calculated. A generalised linear model was used to assess the association between parental education and occupation, separately, and the intergenerational height difference, adjusted for maternal age at birth, smoking during pregnancy, birthweight adjusted for gestational age, and birth order. Females were on average 1.46cm (SD=6.62) taller than their mothers, and males 3.00cm (SD=7.26) taller than their fathers. The highest height gain was shown in those with less advantaged socioeconomic background. In the adjusted model, sons whose mothers had 0-6 years of education grew 3.9cm taller (β=3.894; 95%CI:2.345;5.443) and daughters 1.5cm taller (β=1.529; 95%CI:0.180;2.878) (compared to >12y maternal education); for paternal education, sons and daughters grew 3.5cm (β=3.480; 95%CI:1.913;5.047) and 1.9cm taller (β=1.895; 95%CI:0.526;3.265), respectively. A higher height increase was found in participants with less advantaged maternal and paternal occupational level. Adults born in 1990 are taller than their parents, and height gain was higher in males than females. Adults from a lower socioeconomic status experienced the highest height gain, suggesting a reduction in height inequality.

Abstract: Background and aim Given the controversy regarding metabolically healthy obesity, we studied the association between duration and degree of body mass index (BMI) from adolescence to early adulthood and metabolic status of both overweight/obese and under/normal weight subjects. Methods and results Participants of the EPITeen cohort were evaluated at 13, 17, 21 and 24 years (n = 1040). Duration and degree of BMI in the 11-year period was summarized through the area under the curve of BMI (BMIAUC). Metabolic health at 24 y was defined as optimal levels of lipids, blood pressure and glucose. The association between BMIAUC per year and metabolic health was estimated through binary logistic regression models, adjusted for confounders and stratified by BMI. The proportion of metabolically healthy overweight/obesity at 24 y was 13.4%. After adjustment for sociodemographic and behavioural factors, the increase of one kg/m2 in BMI on average per year during the period between 13 and 24 y was associated with 14% lower odds of being metabolically healthy among under/normal weight participants (OR = 0.86, 95% CI 0.78–0.94); and 8% lower odds of metabolic health among obese/overweight participants (OR = 0.92, 95% CI 0.85–1.00). After additional adjustment for waist circumference, the association was attenuated, especially in the obese/overweight group (OR = 1.03, 95% CI 0.93–1.14). About 20% of the metabolically healthy obese/overweight at 13 y transitioned to metabolically unhealthy obesity/overweight at 24 y. Conclusion The results support the hypothesis that the healthy obesity phenotype could be explained by a lower exposure to adiposity, either by shorter time or lower quantity, and a more favourable body fat distribution. Abstract: To develop the first Ophthalmology joint guidelines with Paediatric Rheumatology with recommendations on the screening, monitoring and medical treatment of juvenile idiopathic arthritis-associated uveitis (JIA-U), endorsed by the Portuguese Society of Ophthalmology (SPO).A systematic literature review was conducted to include publications up to July 14th 2020, with no language restrictions, in order to include all the international position papers/guidelines concerning the medical management of JIA-U and randomised clinical trials assessing the efficacy and safety of medical treatment in this field. We searched through MEDLINE (PubMed), Scopus, Web of Science and Cochrane Library. The Delphi modified technique to generate consensus was used. Preliminary evidence statements were subject to an anonymous agreement assessment and discussion process using an online survey, followed by further discussion and update at a national meeting. A draft of the manuscript with all recommendations was then circulated among all participants and suggestions were incorporated. The final version was again circulated before publication.Twenty-six recommendations were developed focusing on the following topics: general management (3), screening and follow-up of uveitis (4), treatment (17) and health education in JIA-U among patients and families (2).These guidelines were designed to support the shared medical management of patients with JIA-U and emphasize the need for a multidisciplinary approach between Ophthalmology and Paediatric Rheumatology regarding the comprehensive care of JIA-U. 38. A carboxyl-functionalized covalent organic polymer for the efficient adsorption of saxitoxin

Abstract: Saxitoxin (STX), the most widely distributed neurotoxin in marine waters and emerging cyanotoxin of concern in freshwaters, causes paralytic shellfish poisoning in humans upon consumption of contaminated shellfish. To allow for the efficient monitoring of this biotoxin, it is of high importance to find high-affinity materials for its adsorption. Herein, we report the design and synthesis of a covalent organic polymer for the efficient adsorption of STX. Two β-keto-enamine-based materials were prepared by self-assembly of 2,4,6-triformylphloroglucinol (Tp) with 2,5-diaminobenzoic acid (Pa-COOH) to give TpPa-COOH and with 2,5-diaminotoluene (Pa-CH3) to give TpPa-CH3. The carboxylic acid functionalized TpPa-COOH outperformed the methyl-bearing counterpart TpPa-CH3 by an order of magnitude despite the higher long-range order and surface area of the latter. The adsorption of STX by TpPa-COOH was fast with equilibrium reached within 1 h, and the Langmuir adsorption model gave a calculated maximum adsorption capacity, Qm, of 5.69 mg g-1, making this material the best reported adsorbent for this toxin. More importantly, the prepared TpPa-COOH also showed good reusability and high recovery rates for STX in natural freshwater, thereby highlighting the material as a good candidate for the extraction and pre-concentration of STX from aquatic environments. published

39. Long-Term Safety and Efficacy of Ixekizumab in Patients With Axial Spondyloarthritis: 3-year Data From the COAST Program

Abstract: ObjectiveTo report safety and efficacy of ixekizumab (IXE) from the COAST program at 3 years, including 1 year from the originating studies (COAST-V, COAST-W, and COAST-X), and 2 years from COAST-Y.MethodsIn COAST-Y, patients continued with the dose received at the end of the originating study at week 52: 80 mg IXE either every 4 weeks (Q4W) or every 2 weeks (Q2W). Placebo-treated patients from COAST-X received IXE Q4W in COAST-Y. Starting at week 116 (week 64 of COAST-Y), patients receiving IXE Q4W could be escalated to Q2W. Safety for patients receiving ≥ 1 dose of IXE and efficacy for patients receiving ≥ 1 dose of IXE Q4W was assessed. Data are summarized as observed.ResultsFor the 932 patients who received ≥ 1 dose of IXE (Q2W or Q4W) through 3 years, treatmentemergent adverse events (TEAEs) occurred at an incidence rate (IR) of 38.0 per 100 patient-years (PYs). The most frequently reported were infections (IR 25.7 per 100 PYs) and injection site reactions (IR 7.4 per 100 PYs); the majority of TEAEs were mild or moderate in severity. In total, 7.1% of TEAEs led to discontinuation (IR 3.1 per 100 PYs). All patient groups receiving IXE Q4W assessed through 3 years saw sustained improvements in Ankylosing Spondylitis Disease Activity Score, clinically important improvement, and other efficacy end points.ConclusionThe 3-year safety profile of IXE in the COAST program is consistent with the previously established long-term safety profile. IXE Q4W provided sustained improvement of disease activity in patients who received treatment through 3 years. 41. Landslide events in Portugal under future climate change scenarios

Abstract: It is expected that landslide events will occur more frequently, throughout the century, as a direct consequence of climate change. The main triggering factor, over Portugal mainland, is extreme precipitation. Thus, the aim of this study relied on the assessment of the projected future changes in the extreme precipitation over Portugal mainland and quantifying the correlation between extreme rainfall events and landslide events through Rainfall Triggering Thresholds (RTT). This methodology was applied for two specific locations within two Portuguese areas of great geomorphological interest.To evaluate the possible projected changes in the extreme precipitation, we used the Iberia02 dataset and the EURO-CORDEX models’ runs at a 0.11º spatial resolution. First, it was analyzed the models’ performance to simulate extreme values in the precipitation series. The simulated precipitation relied on RCM-GCM models’ runs, from EURO-CORDEX, and a Multimodel ensemble mean. The extreme precipitation assessment relied on the values associated to the highest percentiles, and to the values associated to the RTTs’ percentiles. To evaluate the possible future changes of the precipitation series, both at the most representative percentiles and RTTs’ percentiles, a comparison was made between the simulated values from EURO-CORDEX historical runs (1971-2000) and the simulated values from EURO-CORDEX future runs (2071-2100), considering two emission scenarios: RCP 4.5 and RCP 8.5. In the models’ performance, the Multimodel ensemble mean appeared to be within the best representing models. As for the projected changes in the extreme precipitation for the end of the century, when following the RCP 4.5 scenario, most models projected an increase in the extreme values, whereas, when following the RCP 8.5 scenario, most models projected a decrease in the extreme values. Abstract: The COVID-19 pandemic has placed today’s society in an unprecedented scenario. During Portugal’s first home confinement period (March–July 2020), the online-based “COVID-19 in Trials and Tribulations” project was implemented to support families with school-aged children. The project was grounded on the self-regulation framework and delivered through Facebook® and Instagram® pages. Being responsive to ongoing developments of the pandemic, activities were conveyed in two phases. Phase 1 occurred during lockdown (school was suspended). Phase 2 occurred while students were enrolled in at-distance (online) school. The present study aimed to examine the reach of the project, while examining the content and format of delivery that generated the most engagement among the users (4500 Facebook® effective followers; 1200 Instagram® effective followers) during the confinement period. Results showed that, at the individual page level, Facebook® had higher reach indicators compared to Instagram®, except for video. At the Facebook® post level, followers and users showed more engagement with the page prior to the at-distance schooling phase; however, videos still generated engagement (p = 0.002). Both the post type (p < 0.01) and frequency (p < 0.001) of publication were suggested to be good predictors of engagement. The information gathered will help design and inform future interventions that may be implemented as new lockdowns are set in place.

44. Promoção do autocuidado da pessoa idosa/família com ostomia intestinal no pós-operatório

Abstract: À medida que assistimos ao envelhecimento da população mundial, é esperado um aumento da prevalência de doenças que afetam predominantemente as pessoas mais velhas. O tumor colorretal não é exceção, atualmente em Portugal temos assistido a esta alteração epidemiológica, com o aumento de novos casos anuais, bem como, um aumento da idade média da população afetada. A estratégia de tratamento do tumor colorretal é na maior parte das vezes cirúrgica, sendo habitualmente necessário realizar uma ostomia de eliminação intestinal [OEI]. A pessoa idosa/ família que é submetida a uma OEI, experiencia uma transformação a nível físico, psicológico, social e espiritual, com impacto na autonomia e qualidade de vida. Esta nova condição exige apoio e acompanhamento, facilitando o processo de transição e adaptação a esta nova realidade. Neste processo de transição o enfermeiro é o profissional mais habilitado, possibilitando à pessoa idosa/família com OEI a aquisição de conhecimentos e habilidades, essenciais para o autocuidado. Durante o diagnóstico da situação, identificamos a ausência de uma estrutura de cuidados uniformizada e sistematizada no processo de promoção de autocuidado da pessoa idosa/ família com OEI, prejudicando o desenvolvimento de habilidades e capacidades de autocuidado no pós-operatório. Neste sentido, considerou-se de extrema relevância a realização deste projeto de intervenção, tendo como finalidade a melhoria da qualidade de cuidados prestados a esta população, através de um conjunto de ações baseado na metodologia de projeto. Para dar resposta aos objetivos definidos foi essencial identificar as intervenções de enfermagem que promovem o autocuidado, através de uma revisão scoping com a finalidade de responder à seguinte questão de investigação: Quais as intervenções de enfermagem que contribuem para a promoção do autocuidado na pessoa idosa/família com OEI, no pós-operatório? Assim, de acordo com, a evidência encontrada e o modelo teórico Teoria do Défice de Autocuidado (Orem, 2001), foi possível construir um procedimento setorial estruturado, um guia orientador para as pessoas com OEI e uma Checklist de autoavaliação, contribuindo para uma intervenção de cuidados individualizada, com vista à promoção do autocuidado da pessoa/idosa família com OEI, no pós-operatório.

Abstract: From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. 47. Importance of the Spatial Distribution of Rare Earth Elements in the Bottom Sediments of Reservoirs as a Potential Proxy for Tracing Sediments Sources. A Case Study in the Dominican Republic

Abstract: The geochemical composition of rare earth elements (REE) in the bottom sediments of two Dominican reservoirs and in soils from their catchments was studied to identify possible sources of the deposited materials. Knowledge of the origin of the sediments will serve to control the excessive rates of erosion and sedimentation that occur annually due to periodic extreme climatic events that promote excessive silting of the lakes, followed by loss of storage capacity and degradation of water quality. The REE contents of sediments and soils were normalized to the North American Shale Composite (NASC) and the ratio of light/heavy rare earths (LREE/HREE ratio), Ce and Eu anomalies, and some fractionation parameters were determined. The REE patterns are more homogeneous in the sediments, indicating uniform sedimentation in both deposits. The sediment data reflect depletion of REE from the sources, enrichment of light REE (LREE) and some middle REE (MREE), and positive Eu and Ce anomalies. All data were plotted in correlation diagrams between some fractionation parameters of light–middle–heavy REE and anomalies of Ce and Eu. The similarity of the ratios between these parameters in all samples and the overlap of data from soils and rocks on the sediment projection in the diagrams allowed a good discrimination of the main sources of the materials.

48. Longitudinal multimodal functional macular analysis after half-dose photodynamic therapy for central serous chorioretinopathy

Abstract: Half-dose photodynamic therapy (HD-PDT) has been widely used for central serous chorioretinopathy (CSC) with good anatomical results. However, long-term functional outcomes after this treatment remain uncertain. This study aimed a longitudinal multimodal macular assessment, correlating functional and anatomical outcomes.This is a retrospective study performed in a tertiary referral center including 111 eyes from 95 CSC patients. Data on best corrected visual acuity (BCVA), central macular thickness (CMT), central retinal sensitivity (CRS) using microperimetry (MP) and multifocal electroretinography (mfERG) at baseline and 3, 6, 12, 18, 24, 36, 48 and 60 months after treatment were registered. A correlation analysis was performed.Mean follow-up was 34.5 ± 26.3 months. A significant improvement in BCVA and CMT was registered in all the visits. CRS significantly improved until 24 months (p 0.001 at 12 months, p 0.05 at 24 months), worsening afterwards. The mfERG amplitude of N1 and P1 waves significantly improved in the first 12 months, aggravating afterwards. The implicit time improved until 24 months, deteriorating after 48 months. This long-term decline was also described in some inactive untreated fellow eyes CONCLUSIONS: A multimodal longitudinal analysis of CSC patients after HD-PDT shows that, after the first 12 to 24 months, the significant sustained improvement in BCVA and CMT is not paired by a sustained improvement in macular sensitivity or electrical response. This long-term functional deterioration might result from the disease itself and not directly from the treatment .

49. A problemática da responsabilidade individual na saúde pública em Portugal

Abstract: Introdução: A prevenção em saúde é o principal mote da Organização Mundial de Saúde (OMS) e há muitas décadas é já aplicada nas acções de saúde pública de todos os países da União Europeia, inclusive Portugal. Objetivo: Discutir a questão da responsabilidade social e individual no actual cenário da Saúde Pública em Portugal, face a uma proposta política de possível abandono de um modelo de responsabilidade partilhada na saúde pública. Métodos: Este estudo aborda o Estado social e saúde verificadas ao longo das últimas décadas na Europa e em Portugal, discute a Ética e saúde pública cujas acções se situam na intersecção entre Riscos, Efeitos na Saúde e Prevenção, dicerne a Responsabilidade individual e social que emergem dos valores que orientam ou deveriam orientar as nossas relações com os outros, debate a Justiça e direito à saúde acom base na realidade da saúde em Portugal, que parece ser um dilema ético para quem toma decisões acerca do estabelecimento de prioridades no uso dos recursos financeiros. Resultados: O estudo relatou que a atual situação das finanças públicas, especialmente em Portugal, deveria remeter para a busca de novas perspectivas em que a participação de todos os agentes de saúde torna-se indispensável; em particular médicos, enfermeiros e outros técnicos, no sentido de apresentarem as suas posições quanto aos modos como desejam exercer a sua atividade profissional, numa clara definição de direitos e de deveres. Conclusão: Ressalta-se a necessidade de o Estado, com o empenho dos profissionais de saúde, estabelecer os princípios políticos, financeiros e clínicos para um plano de saúde eficiente e eficaz.
