BackgroundCardiovascular diseases are still a significant cause of death and hospitalization. In 2019, circulatory diseases were responsible for 29.9% of deaths in Portugal. These diseases have a significant impact on the hospital length of stay. Length of stay predictive models is an efficient way to aid decision-making in health. This study aimed to validate a predictive model on the extended length of stay in patients with acute myocardial infarction at the time of admission.MethodsAn analysis was conducted to test and recalibrate a previously developed model in the prediction of prolonged length of stay, for a new set of population. The study was conducted based on administrative and laboratory data of patients admitted for acute myocardial infarction events from a public hospital in Portugal from 2013 to 2015.ResultsComparable performance measures were observed upon the validation and recalibration of the predictive model of extended length of stay. Comorbidities such as shock, diabetes with complications, dysrhythmia, pulmonary edema, and respiratory infections were the common variables found between the previous model and the validated and recalibrated model for acute myocardial infarction.ConclusionPredictive models for the extended length of stay can be applied in clinical practice since they are recalibrated and modeled to the relevant population characteristics.

ABSTRACT: Introduction: The G-spot, a putative erogenous area in the anterior vaginal wall, is a widely accepted concept in the mainstream media, but controversial in medical literature. Aim: Review of the scientific data concerning the existence, location, and size of the G-spot. Methods: Search on Pubmed, Pubmed Central, Cochrane, clinicaltrials.gov and Google Scholar from inception to November 2020 of studies on G-spot's existence, location and nature. Surveys, clinical, physiological, imaging, histological and anatomic studies were included. Main Outcome Measure: Existence, location, and nature of the G-spot. Results: In total, 31 eligible studies were identified: 6 surveys, 5 clinical, 1 neurophysiological, 9 imaging, 8 histological/anatomical, and 2 combined clinical and histological. Most women (62.9%) reported having a G-spot and it was identified in most clinical studies (55.4% of women); in 2 studies it was not identified in any women. Imaging studies had contradictory results in terms of its existence and nature. Some showed a descending of the anterior vaginal wall, that led to the concept of clitourethrovaginal complex. In anatomic studies, one author could systematically identify the G-spot, while another group did not find it. Studies on innervation of the vaginal walls did not systematically identify an area with richer innervation. Conclusion: The different studies did systematically agree on the existence of the G-spot. Among the studies in which it was considered to exist, there was no agreement on its location, size, or nature. Resumen: El trabajo evalúa la eficacia del programa de educación parental Grupo La-ços de Inclusão (Grupo Lazos de Inclusión), aplicado en una asociación de arte inclusiva, de la Región Autónoma de Madeira. Han participado en el estudio 11 padres (ocho madres y tres padres) de nueve niños/jóvenes con necesidades especiales y de tres niños/jóvenes sin ninguna necesidad especial. Se ha realizado una evaluación pre-test y post-test y los resultados previos indican para una disminución de la percepción: del estrés global de los participantes; de exigencias de como relacionarse con el niño; del nivel de la salud de los padres, que pueden interferir na relación con el niño. Aunque los resultados parecen ser positivos, todavia se destacan algunas limitaciones y se sacan reflexiones para futuras intervenciones de educación parental.

6. Erylusamides: Novel Atypical Glycolipids from Erylus cf. deficiens

Among marine organisms, sponges are the richest sources of pharmacologically-active compounds. Stemming from a previous lead discovery program that gathered a comprehensive library of organic extracts of marine sponges from the off-shore region of Portugal, crude extracts of Erylus cf. deficiens collected in the Gorringe Bank (Atlantic Ocean) were tested in the innovative high throughput screening (HTS) assay for inhibitors of indoleamine 2,3-dioxygenase (IDO) and showed activity. Bioassay guided fractionation of the dichloromethane extract led to the isolation of four new glycolipids, named erylusamide A–D. The structures of the isolated compounds were established by 1D and 2D nuclear magnetic resonance (NMR) spectroscopy, high-resolution electrospray ionization mass spectrometry (HR-ESI-MS) and chemical derivatization. The metabolites shared a pentasaccharide moiety constituted by unusual highly acetylated ᴅ-glucose moieties as well as ᴅ-xylose and ᴅ-galactose. The aglycones were unprecedented long chain dihydroxyketo amides. Erylusamides A, B and D differ in the length of the hydrocarbon chain, while erylusamide C is a structural isomer of erylusamide B.

8. Urban growth in Latin American cities : exploring urban dynamics through agent-based simulation

The high rates of urban growth in Latin America during the 1960s and 1970s produced rapid urbanisation and housing problems. Planning policies as well as the research community have approached urban growth as a static problem rather than as a spatial form that emerges from the urban development process and that is part of a constant dynamic process. This thesis focuses on a specific kind of urban growth that happens in Latin American cities, called 'peripherisation'. This is characterised by the formation of low-income residential areas in the peripheral ring of the city and a perpetuation of a dynamic core-periphery spatial pattern. The dynamics of growth and change in Latin American cities are explored using agent-based simulation. The objective is to increase the understanding of urban spatial phenomena in Latin American cities, which is essential to providing a basis for future planning actions and policies. The thesis consists of two parts. The first part presents an overview of urban growth and dynamics in Latin American cities, drawing on previous work on urbanisation in Latin American cities, spontaneous settlements and inner city dynamic processes. The second part focuses on the development of a simulation model based on the theoretical framework established in the first part. A brief review of the literature of automata models is presented, with particular reference to agent-based simulation for land-use dynamics. The Peripherisation Model is introduced, its computer implementation described, and sensitivity analysis tests reported. Simulation exercises were used to revisit assumptions about urbanisation issues in Latin American cities and investigate important aspects of growth and change in these cities. These exercises allowed the problem of urban growth in Latin American cities to be unfolded through their dynamics, relating these dynamics to urban morphology, and thus presenting a new and important perspective on the phenomenon.

11. Connectivity of sponge grounds in the deep sea: genetic diversity, gene flow and oceanographic pathways in the fan-shaped sponge Phakellia ventilabrum in the northeast Atlantic

- Abstract
Little is known about dispersal in deep-sea sponges, yet understanding patterns of gene flow and connectivity is essential for their effective management. Given rising pressure from harmful anthropogenic activities, schemes that manage resource extraction whilst conserving species diversity are increasingly necessary. Here, we used ddRADseq derived SNPs to investigate the genetic diversity and connectivity for the deep-sea sponge Phakellia ventilabrum across the northeast Atlantic Ocean (from the Cantabrian Sea to Norway). The analysis of 166 individuals collected from 57 sampling stations were grouped into 17 different areas, including two MPAs, one SAC and other areas with different levels of protection. The 4,017 neutral SNPs we uncovered indicated high connectivity and panmixis amongst the majority of areas, spanning a ca. 2,500-kilometre range and depths of 99–900 m. This was likely due to the presence of strong ocean currents aiding larval transport, as supported by our migration analysis and also by 3D particle tracking modelling using information on the reproductive cycle of P. ventilabrum. We also observed significant genetic similarity between samples from the Cantabrian Sea and Roscoff (France) as compared to the remainder of the collection areas, likely arising from physical drivers such as prevailing current circulation patterns and topographic features, acting as barriers for gene flow. Despite this, our results suggest that all protected areas studied are well connected with each other. The relatively low genetic diversity observed in all areas, though, highlights the potential fragility of this species to changing climates, which might compromise resilience to future threats.

A atenção à sobredotação e a alunos com altas capacidades, na Região Autónoma da Madeira (RAM), tem sido referenciada pelo seu pioneirismo no contexto português. Neste artigo descreve-se a evolução desde os anos 90 dos mecanismos de atendimento das crianças e adolescentes com elevadas capacidades, e respetivo enquadramento legal. Os dados foram recolhidos a partir de entrevistas a alguns dos intervenientes neste processo e através da consulta de documentos disponíveis. Assim, foi possível organizar o percurso histórico da sobredotação na Madeira, apresentando um breve enquadramento da emergência do apoio à sobredotação e descrevendo três fases sequenciais do mesmo: (a) Implementação de serviços de atendimento; (b) Consolidação da prestação de serviços; e (c) Serviços prestados na atualidade. De uma forma geral, apesar da evolução e modificação dos serviços, o investimento tem-se centrado no processo de identificação e de intervenção, na formação e na prática baseada em evidência, procurando fazer convergir as atuações com as orientações internacionais na área. Apontam-se algumas reflexões sobre as políticas públicas na área da sobredotação, tendo como premissa a escola inclusiva e uma melhoria de serviços que promovam a igualdade de oportunidades e a diferenciação das práticas educativas, em função das características e necessidades de cada aluno.  , Attention to giftedness and high ability students, in the Autonomous Region of Madeira (RAM), has been acknowledged for being pioneer in Portugal. The main purpose of this paper is to outline the evolution of practices for high abilities students, and their legal framework, since the 90s, in RAM. Data were collected from interviews with some of the actors in this process and through analysis of available documents. Thus, it was possible to organise the historical path of giftedness in Madeira. So, it is presented a brief framework of the emergence of giftedness educational support and three sequential phases are also described: (a) Implementation of support services; (b) Consolidation of the provision of services; and (c) Current services provided. In general, despite the evolution and modification of the organisation of the services, the investment has been on the identification process and intervention, on evidence-based practice, seeking convergence of intervention with international guidelines. Some reflections on public policies about giftedness are pointed out, based on a comprehensive school and a possible improvement of practices that might facilitate equal opportunities and differentiation, according to the characteristics and needs of each student.  , La atención a los estudiantes superdotados y con altas capacidades en la Región Autónoma de Madeira (RAM) se ha mencionado como pionera en el contexto portugués. En este estudio se pretende trazar la evolución de los mecanismos de atención de los niños y adolescentes con altas capacidades en la RAM, con su respectivo marco legal, de lo que viene sucediendo desde los años 90. Los datos fueron recogidos a partir de entrevistas a algunos de los participantes en este proceso y a través de la consulta de documentos disponibles. Así, fue posible organizar el recorrido histórico de la superdotación en Madeira, presentando un breve encuadre del surgimiento del apoyo a la sobredotación y describiendo tres fases secuenciales del mismo: (a) Implementación de servicios de atención; (b) Consolidación de la prestación de servicios; y (c) Servicios prestados en la actualidad. En general, a pesar de la evolución y la modificación de los servicios, la inversión se ha centrado en el proceso de identificación y de intervención, en la formación y en la práctica basada en evidencia, buscando hacer converger las actuaciones con las orientaciones internacionales en el área. Dans cette étude, nous prétendons tracer l’évolution des mécanismes d’accueil des enfants et adolescents á haut potentiel, ainsi que leur respectif cadre juridique, ce qui se passe depuis les années 90 dans la RAM. Les données ont été recueillies á partir d’entretiens de certains des intervenants du processus et á partir de l’examen des documents disponibles. Il a été ainsi possible d’organiser le parcours historique des surdoués á Madère, en présentant un bref encadrement de l’urgence du soutien au surdouement et en décrivant trois phases séquentielles de celui-ci : (a) application de mesures d’accueil ; (b) consolidation de la prestation de services ; et (c) services réalisés actuellement. D’une forme générale, et malgré l’évolution et modification des services, l’investissement s’est centré sur le processus d’identification et intervention, sur la formation et sur la pratique basée sur l’évidence, visant á faire converger les agissements avec les orientations internationales dans le domaine. Sont aussi abordées certaines réflexions sur les politiques publiques dans le domaine du surdouement, ayant comme principe l’école inclusive et la possible amélioration de services qui puissent faciliter l’égalité des opportunités et la différentiation des pratiques éducatives en fonction des caractéristiques et des besoins de chaque élève.

Objetivos: O principal objetivo deste estudo in vitro foi avaliar, com base no método dos elementos finitos, o uso de implantes curtos em reabilitação mandibular total e como o comprimento desses implantes influencia a distribuição de tensões durante a aplicação de cargas mastigatórias em reabilitações mandibulares, de acordo com o conceito All‐on‐4®. Materiais e métodos: Foi realizada a modelização de um implante comercial em SolidWorks®. Foram também modelizadas 2 mandíbulas: uma real, baseada num exame de tomografia computorizada (TC) e usando o Mimics®; e uma segunda construída virtualmente em SolidWorks®. De seguida, os implantes foram colocados de acordo com o protocolo All‐on‐4®. Os 2 implantes anteriores, com um comprimento constante de 8 mm, foram colocados verticalmente na zona incisivos inferiores. Os 2 implantes posteriores foram colocados na área pré‐molar com uma angulação distal constante de 30°, e com comprimentos de 8, 6 e 4 mm. Nos implantes, foi colocada uma barra fixa ferulizada que simula a reabilitação implantossuportada. O conjunto foi colocado tanto sobre a mandíbula virtual, como sobre a mandíbula real. Sobre os modelos construídos foram simuladas cargas mastigatórias, tendo sido registados os valores máximos obtidos. Resultados: Concluiu‐se que os valores médios obtidos para as amostras geradas em cada tipo de mastigação no modelo virtual para o implante e osso foram significativamente diferentes dos valores médios registados no modelo real, excetuando na mastigação bilateral no modelo com implantes de 8 mm e na mastigação unilateral sobre o cantilever no modelo com implantes de 4 mm. Os níveis mais baixos de tensão implantar foram registados nos modelos com implantes de 8 mm. Conclusões: A utilização de implantes curtos deve ser feita cuidadosamente, sobretudo em reabilitações tipo All‐On‐4®. Mais estudos devem ser realizados, a fim de compreender a influência da angulação do implante na distribuição de tensões. Esforços futuros devem procurar a criação de modelos virtuais mais fidedignos.

La finca s’emplaça a Montbó, zona Est del terme municipal de Canet d’Adri, i té una superfície total de 103,82 Ha., amb una part de terreny forestal i una altra destinada a terreny agrícola. S’accedeix a través de la carretera de Montcal a Montbò. L’any 2005 aquestes edificacions van ser objecte d’unes obres de consolidació consistents en l’execució de cèrcols i nous forjats i cobertes La presència de roques granítiques superficials de mida considerable i els desnivells del terreny van condicionar la disposició i organització de les edificacions. Totes les edificacions van ser construïdes sobre la zona rocosa, facilitant l’ús agrícola, cultiu de cereals, a les zones on hi ha més terra. Dins de la finca on procedirem a actuar, hi trobem una edificació no original construïda sobre els murs d’una antiga bassa d’aigua que s’utilitzava per abastir les masies. Si avancem en direcció llevant hi trobem les masies objectes del treball: Can Basiu, Can Virolesc, Annexe. Les tres masies formen part de la classificació coneguda com a segona família, és tracta de masies amb carener perpendicular a la façana i dos vessants laterals. Les masies Can Basiu i Can Virolesc, presenten una coberta a dos vessants, planta baixa i planta primera, mentre que l’annexe, de dimensions molt més reduïdes que les anteriors, presenta una superfície irregular amb dos plantes, planta baixa i planta primera i una coberta dividida en dos parts, una part a dues vessant i l’altra a una sola vessant. L’any 2005 es va dur a terme unes obres de consolidació consistents en l’execució de cèrcols i nous forjats i cobertes de les masies. Continuant el camí vers l’orient arribem a l’església de Sant Joan de Montbò. L’església no està inclosa en la rehabilitació de les masies, però pel seu gran valor patrimonial hem considerat interessant esmentar algunes de les característiques que presenta. Es tracta doncs, d’un conjunt d’edificacions que per les seves característiques i l’activitat productora que feien corresponen a les anomenades masies cerealístiques. La seva principal activitat era el conreu dels cereals, es van anar construint fins al segle XX adoptant la configuració de la masia clàssica. Solien ser edificacions elegants i esveltes de planta generalment rectangular i coberta a dues vessants. Analitzarem cada de les masies per separat per poder descriure els diferents elements constructius que hi trobem i facilitar una millor comprensió del conjunt de les masies.

20. Phylogeography, genetic diversity and structure of the poecilosclerid sponge Phorbas fictitius at oceanic islands

11 páginas, 5 tablas, 2 figuras., In this study we assessed the sequence variation in the I3-M11 partition of the mtDNA cytochrome c oxidase subunit I gene (COI) in ten populations of the Atlanto-Mediterranean demosponge Phorbas fictitius (Porifera: Poecilosclerida) at two spatial scales: a regional scale comparing mainland (Iberian) and insular (Macaronesian) populations, and a local (Archipelagic) scale focusing on different island populations of the Azores archipelago. A multiple approach combining diversity measures, FST estimates, phylogenetic inference and nested clade phylogeographic analysis was used to assess the genetic structure and elucidate the evolutionary history of this species. Genetic differentiation, based of F ST estimates, was found among most populations at both scales revealing highly structured populations. This results of a presumably low dispersal potential and bathymetric range of the species, and the geographical isolation of the studied populations. However we found evidence of long distance dispersal events between some populations. Phylogenetic and network analyses indicate a separation of insular (Macaronesian) and mainland (Iberian) clades with only two haplotypes shared between these areas. The high genetic diversity and prevalence of ancestral haplotypes suggest the Macaronesian islands as the likely place of origin of this species with posterior expansion to mainland locations via current-mediated dispersal of larvae or sponge fragments. This study adds to the growing evidence of structured populations in the marine realm and highlights the importance of the Macaronesian islands on the evolutionary history of the Northeast Atlantic marine biota., This study is part of the PhD-project of J. 24. Dynamics and spatial patterns of residential segregation: analysis of the case of Marília/SP between 2000 and 2010 using segregation indices and agent-based modeling

- Abstract
No início dos anos 2000 o país passou por mudanças políticas e econômicas que provocaram uma redução nas desigualdades sociais. O senso comum supunha que, como consequência da diminuição dessas desigualdades, a segregação também seria minimizada. Evidências empíricas, no entanto, apontam que houve um aumento na segregação nos espaços metropolitanos. Poucos estudos, porém, buscam analisar o grau e a alteração nos padrões espaciais da segregação nas cidades médias, entre 2000 e 2010, a partir de uma perspectiva quantitativa. Além disso, apesar de pesquisas qualitativas concluírem que a segregação aumentou nas cidades médias, neste período de tempo, em decorrência de processos de multiplicação e diversificação dos desenvolvimentos imobiliários no estilo condominial, redefinições nas relações de centro-periferia e valorização de setores específicos destas cidades, existe uma lacuna entre estas formulações teóricas sobre processos e a dinâmica da segregação no período, e pesquisas que testem efetivamente essas hipóteses. Esta tese visa a contribuir com a redução das lacunas mencionadas a partir de duas abordagens metodológicas complementares que requerem ferramentas distintas. A primeira, objetiva comparar e quantificar o grau da segregação, além de analisar as transformações/manutenção nos seus padrões espaciais a partir de índices de segregação. A segunda visa a testar hipóteses teóricas, de maneira espacial e temporalmente explícita, sobre a dinâmica da segregação no período, utilizando um modelo de simulação baseado em agentes. Os resultados dos índices revelam que, em Marília, no período, houve a manutenção dos padrões espaciais de segregação em macroescala e um agravamento em microescala. Foi possível verificar um aumento na segregação dos grupos de alto (renda de dez a 20 salários mínimos e acima de 20 salários mínimos) e de baixíssimo nível socioeconômico (até dois salários mínimos). Os grupos de alto nível socioeconômico encontram-se mais isolados no setor centro-leste da cidade, enquanto o de baixíssimo nível socioeconômico encontra-se mais concentrado e isolado nas zonas periféricas localizadas ao norte e sul da cidade. Os exercícios de simulação do modelo baseado em agentes, por sua vez, apontaram que a emergência de novos empreendimentos imobiliários no estilo condominial, desenvolvidos para atender a demanda habitacional dos grupos de alto nível socioeconômico e localizados no setor centro-leste da cidade, contribuíram para agravar o isolamento destes grupos nestas localidades. A multiplicação e diversificação dos condomínios fechados, somados a outros mecanismos como: a valorização do setor-centro leste da cidade e os efeitos de dependência (path-dependence), ou das trajetórias históricas de ocupação, também contribuíram para o aumento da segregação dos grupos de baixo nível socioeconômico, mantendo as habitações acessíveis à população de baixo nível socioeconômico nas áreas periféricas. As simulações, ainda, apresentaram projeções futuras de aumento da segregação dos grupos de baixo e baixíssimo nível socioeconômico nas áreas periféricas ao sul e norte da cidade em decorrência da alocação empreendimentos pertencentes à da faixa 1 do programa \"Minha Casa, Minha Vida\". In the early 2000s, the country underwent political and economic changes that brought about a reduction in social inequalities. Common sense assumed that, as consequence of the reduction of social inequalities, segregation would also be minimized. Empirical evidence, however, points out that there has been an increase in segregation in metropolitan spaces. Few studies, however, attempt to analyze the degree and change in spatial patterns of segregation in medium-sized cities, between 2000 and 2010, from a quantitative perspective. Qualitative research also have concluded that segregation has increased in medium-sized cities in this period, pointing to processes that may have triggered this increase such as: multiplication and diversification of gated real estate developments, redefinitions in the center-periphery and appreciation of specific sectors of these cities. However, there is a gap between theoretical formulations and conclusions about segregation dynamics and tests of these hypotheses. This thesis aims to contribute to the reduction of the mentioned gaps from two complementary methodological approaches that require different tools. The first, aims to quantify the degree of segregation and analyze the transformations / maintenance in their spatial patterns from segregation indices. The second approach aims to test theoretical hypotheses about the dynamics of segregation between 2000 and 2010 in a spatially and temporally explicit manner using an agent-based simulation model. The indices results show that in Marília in this period, there was a maintenance of segregation patterns on a macro scale perspective and a worsening of segregation on a micro scale. It was possible to see an increase in the segregation of high-income groups (income of ten to twenty minimum wages and above twenty minimum wages) and of very low socioeconomic status (income up to two minimum wages). The groups of high socioeconomic level are more isolated in the central-eastern sector of the city, while the very low socioeconomic level group is more concentrated and isolated in the peripheral areas located on the extreme north and south of the city. The simulation exercises of the agent-based model point out that the emergence of gated communities, developed to meet the housing demand of groups of high socioeconomic level and located in the central-eastern sector of the city, contributed to aggravate the isolation of these groups in these locations. The multiplication and diversification of gated communities, added to other mechanisms such as: the appreciation of the center-east sector of the city and the effects of path-dependence, also contributed to the increase in the segregation of the low socioeconomic groups, keeping housing accessible to the low socioeconomic population in peripheral areas. The simulations also showed future projections of increased in segregation of low and very low socioeconomic groups in the peripheral areas to the south and north of the city due to the allocation of popular housing, belonging to the band 1 of the federal program \"Minha Casa, Minha Vida\".
