Infectious diseases are the leading cause of morbidity and mortality in low and middle income countries (LMICs). Rapid diagnosis of infections in LMICs presents many challenges, especially in rural areas where access to health care, including diagnostics, is poor. Microscopy is one of the most commonly used platforms to diagnose bacterial infections on clinical samples. Fluorescence microscopy has high sensitivity and specificity but to date is mostly performed within a laboratory setting due to the high-cost, low portability and highly specialist nature of equipment. Point-of-care diagnostics could offer a solution to the challenge of infection diagnosis in LMICs. In this paper we present frugal, easy to manufacture, doped polydimethylsiloxane filtering optical lenses that can be integrated into smartphone microscopes for immediate detection of fluorescently labelled bacteria. This provides a breakthrough technology platform for point-of-care diagnostics.

Objective\ud \ud To investigate quality of life (QoL) and association with surgical complexity and disease burden after surgical resection for advanced ovarian cancer in centres with variation in surgical approach.\ud \ud \ud \ud Design\ud \ud Prospective multicentre observational study.\ud \ud \ud \ud Setting\ud \ud Gynaecological cancer surgery centres in the UK, Kolkata, India, and Melbourne, Australia.\ud \ud \ud \ud Sample\ud \ud Patients undergoing surgical resection (with low, intermediate or high surgical complexity score, SCS) for late-stage ovarian cancer.\ud \ud \ud \ud Main Outcome Measures\ud \ud Primary: change in global score on the European Organisation for Research and Treatment of Cancer (EORTC) core quality-of-life questionnaire (QLQ-C30). Secondary: EORTC ovarian cancer module (OV28), progression-free survival.\ud \ud \ud \ud Results\ud \ud Patients’ preoperative disease burden and SCS varied between centres, confirming differences in surgical ethos. QoL response rates were 90% up to 18 months. Mean change from the pre-surgical baseline in the EORTC QLQ-C30 was 3.4 (SD 1.8, n = 88) in the low, 4.0 (SD 2.1, n = 55) in the intermediate and 4.3 (SD 2.1, n = 52) in the high-SCS group after 6 weeks (p = 0.048), and 4.3 (SD 2.1, n = 51), 5.1 (SD 2.2, n = 41) and 5.1 (SD 2.2, n = 35), respectively, after 12 months (p = 0.133). In a repeated-measures model, there were no clinically or statistically meaningful differences in EORTC QLQ-C30 global scores between the three SCS groups (p = 0.840), but there was a small statistically significant improvement in all groups over time (p < 0.001). The high-SCS group experienced small to moderate decreases in physical (p = 0.004), role (p = 0.016) and emotional (p = 0.001) function at 6 weeks post-surgery, which resolved by 6–12 months.\ud \ud \ud \ud Conclusions\ud \ud The global QoL of patients undergoing low-, intermediate- and high-SCS surgery improved at 12 months after surgery and was no worse in patients undergoing extensive surgery.\ud \ud \ud \ud Tweetable Abstract\ud \ud Compared with surgery of lower complexity, extensive surgery does not result in poorer quality of life in patients with advanced ovarian cancer.

We investigated URS and impact on survival in whole patient cohorts with AOC treated within gynaecological cancer centres that participated in the previously presented SOCQER 2 study. National cancer registry datasets were used to identify FIGO Stage 3,4 and unknown stage patients from 11 cancer centres that had previously participated in the SOCQER2 study. Patient outcomes’ association with surgical ethos were evaluated using logistic regression and Cox proportional hazards. Centres were classified into three groups based on their surgical complexity scores (SCS); those practicing mainly low complexity, (5/11 centres with >70% low SCS procedures, 759 patients), mainly intermediate (3/11, 35–50% low SCS, 356 patients), or mainly high complexity surgery (3/11, >35% high SCS, 356 patients). Surgery rates were 43.2% vs. 58.4% vs. 60.9%. across mainly low, intermediate and high SCS centres, respectively, p < 0.001. Combined surgery and chemotherapy rates were 39.2% vs. 51.8% vs. 38.3% p < 0.000 across mainly low, intermediate and high complexity groups, respectively. Median survival was 23.1 (95% CI 19.0 to 27.2) vs. 22.0 (95% CI 17.6 to 26.3) vs. 17.9 months (95% CI 15.7 to 20.1), p = 0.043 in mainly high SCS, intermediate, and low SCS centres, respectively. In an age and deprivation adjusted model, compared to patients in the high SCS centres, patients in the low SCS group had an HR of 1.21 (95% CI 1.03 to 1.40) for death. Mainly high/intermediate SCS centres have significantly higher surgery rates and better survival at a population level. Centres that practice mainly low complexity surgery should change practice. This study provides support for the utilization of URS for patients with advanced OC.

Study Title: Trauma, Social Harm and Health in Criminal Justice Involved Women: The Women's Risk Needs Assessment Validation Research Study Background: Existing research shows that women’s offending is characterised by 5 main risk factors linked to crime: trauma and abuse; mental health; substance use; children, parents and relationships; poverty and accommodation. But we still know relatively little about the interventions meant to prevent and tackle female criminal behaviour. Research studies often do not calculate or report their results about women, are mostly conducted in prison and very little research is conducted with justice involved women in the community. Currently,Women’s Centres which support criminal justice involved women in the UK, all use different assessments of risks (for example of re-offending) and needs of the women who come to them for support. None of the current assessments are independently and publicly validated or demonstrate statistically significant scales with robust scoring and weighting. In 2017, we independently reviewed 136 peer-reviewed tools for assessing the needs and outcomes of justice involved women, and concluded that the Women’s Risk Needs Assessment (WRNA) was the most validated, gender responsive and trauma-informed tool for this population internationally. This study will be the first to validate the WRNA Probation module, in England and it will test how accurate the assessment is with criminal justice involved women in this national context. Study Design: An observational validation study to test the predictive validity of the WRNA in England.Criminal justice involved women seeking support from Women’s Centres following release from custody will be identified by caseworkers as potential participant and consented to the study. In the initial caseworker interview, participants will undertake the WRNA assessment that identifies risks, needs and strengths that predict criminal behaviour. Follow up will be conducted at 6 and 12 months using criminal offending data to link to recidivism and assess the predictive accuracy of the WRNA. Study Participants: English speaking women who are over 18, with involvement in the criminal justice system (police custody/triage/probation/prison or at risk of justice involvement) who have capacity to consent to involvement will be included in the study.Participants will be recruited from the following pathways which feed into the Women’s Centres:Through-the-gate support following prison (TTG); Release on temporary license (ROTL) services; or, through Liaison and Diversion (L&D) from Police custody, street triage or community (probation) referrals received by the Women’s Resource Centres (WRCs) caseworkers from other services. Sample Size: It is estimated that 700 criminal justice involved women in England are required for the validation, to allow for attrition rates. Planned Study Period: The study will begin in May 2021 and data collection will take place across an estimated 6 month period.With 6 month and 12 month follow ups after the initial assessment interview. End date will depend on the point at which a sufficiently sized sample is achieved at follow up.

This paper explores the multiple, entangled geographies of Palestine-Israel. It traces a range ways in which broadly geopolitical ideas have been brought to bear on the Middle East over the past several decades and the implications of this work for our understanding of this part of the world. Specifically, the paper engages with the imaginative geographies through which Palestine-Israel has been constructed, the geopolitical and biopolitical machinations of Israeli occupation, as well as narratives and practices of identity. The argument is that territory, sovereignty and security are bound up with bodies, mobility and identity, and that geo- and biopolitics need to be thought together in order to access the operations of power at multiple scales that compose and exceed the state, and in order to understand the materiality of everyday life within and beyond the context of politics.

13. Protection of hDAF-transgenic porcine endothelial cells against activation by human complement: role of the membrane attack complex

Xenograft rejection in the discordant pig-to-primate model is dependent on binding of natural antibodies to gal-alpha [1-3]-gal epitopes on the porcine endothelial cell (EC). This leads to complement activation and deposition of activation products onto the membrane and results in perturbation of EC function and thrombus formation. Here we investigated the ability of human complement activation products to directly induce activation of porcine EC, with subsequent upregulation of adhesion and pro-coagulant molecules. Porcine aortic EC were isolated from wild-type and hDAF-transgenic pigs and incubated with human serum, either in the presence or absence of the soluble complement inhibitor TP10 (sCR1). Recombinant C5a, C1q-IgG immune complexes, C6-deficient human serum and serum containing anti-C9 Ab were used to identify EC activating complement products. Heat-inactivated human serum was used as a negative control. Cells were stained with antibodies against human C3, the MAC or with antibodies cross-reactive for porcine E-Selectin, VCAM-1 or Tissue Factor, and analyzed by flow cytometry. We found upregulation of E-Selectin and Tissue Factor on wild-type EC after incubation with human serum. This effect coincided with the deposition of C3 and MAC on the membrane of these cells. The addition of TP10 inhibited EC activation by up to 95%. In contrast, greatly reduced C3 and MAC deposition was detected on hDAF transgenic cells, and no complement-mediated EC activation was seen. Experiments with C6-deficient serum and incubation with anti-C9 Ab indicate a major role of the MAC in serum-mediated EC activation, whereas neither C5a nor C1q-IgG caused activation of EC. These data provide further explanation of the protective role of human DAF in the pig-to-primate xenotransplantation model.

Objectives. This paper explores how ethnicity has been represented in research on the health practices of Chinese populations in the UK and suggests ways in which such research might be enriched by adopting an interdisciplinary approach. Design. A systematic literature review of studies was conducted on research with ‘Chinese’ in the UK. Results. The review highlighted that research with Chinese populations is frequently grounded in assumptions about the homogeneity of Chinese ethnic and cultural identities, and health practices, which undermines the generalizability of findings and conclusions. Conclusions. There was a lack of clarity surrounding the term ‘Chinese’ as an ethnic and national label that can lead to racialised constructions of ethnicity. An interdisciplinary approach is a valuable tool for enriching understandings of culturally-specific accounts of health and illness, and to address ways in which Chinese populations negotiate different health care systems and models of health.
