Background: Changes in the proteoglycan metabolism of the intima of arteries belong to the initial lesions of atherosclerosis (AS). The accumulation of proteoglycans, alterations of pericellular glycoproteins and modulations of collagen turnover also play a fundamental role in the progression of AS. They influence lipid retention, cell behavior and calcinosis. The decisive role played by the matrix metalloproteinases (MMPs) and their inhibiting factors (tissue inhibitors of metalloproteinases [TIMPs]) in these processes is not yet fully understood and therefore the subject of this overview. The causes of the abrupt change of a long-term existing stabile AS to a vulnerable plaque as well as the participation of age-related vascular wall remodeling in the progression of AS also remain open questions. Discussion: Apart from the well-known risk factors for AS, less well-known influences like the disturbances of gene expression in vascular smooth muscle cells affect an MMP/TIMP imbalance. The various consequences of this imbalance range from intima cell proliferation as an early change in AS as well as accelerated progression to the destabilization of fibrous plaques by increased collagenolysis as well as the formation of aneurysms. Infectious or toxic influences may trigger these mechanisms; an involvement of age-related vessel wall changes should also be considered. The prognostic significance of circulating MMP concentrations for the existence of instabile plaques are of great interest, as is the plaque stabilizing effect of statins by suppression of MMPs. Conclusions: MMPs navigate the behavior of vascular wall cells in different AS stages, in adaptive remodeling, in normal aging and in non-atherosclerotic vessel disease. The clinical relevance of a disturbance in the MMP/TIMP balance is demonstrated firstly by the initiation of AS due to migration and proliferation of intima cells and secondly in the collagenolysis, necrotic transformation and apoptosis of existing fibrous lesions resulting in instabile rupture proned plaques. Investigations into the genetic typing of MMPs and the results of experimental gene deficiency models have significantly contributed to the clarification of these facts.

Summary The histologic features and morphometric data of the endocrine pancreas (surgical specimens) of 12 infants suffering from persistent hyperinsulinemic hypoglycemia are reported. In 3 cases, a focal nesidioblastosis, in another 3 cases, a diffuse nesidioblastosis were observed. 2 infants demonstrate B -cell adenomata and 4 adenomata combined with a diffuse nesidioblastosis. In respect to volume density of the endocrine pancreas and diameter of the islets, there are no differences between cases with diffuse nesidioblastosis and age-matched controls. Structural changes of the endocrine pancreas in cases with nesidioblastosis are detectable by their chromogranin-A-immunoreactivity. Further investigations are necessary to clarify if there are disturbances in the relationships between chromogranin A and peptide hormones additionally to structural changes in the pancreas.

Autoradiographic studies on plane preparations using 3H-thymidine and pulse cytophotometric investigations on scrape preparations of aortic endothelium from 34 rabbits have brought the following results: By means of multiple 3H-thymidine-injections the number of labelled endothelial cells is higher in young growing animals than in older rabbits; pulse cytophotometrically we did not find any indication of polyploidization processes; from that we conclude a decrease of the growth fraction with advancing age. In comparison to visual counting the automatic image analysis is a highly effective method for the evaluation of autoradiograms. After multiple 3H-thymidine applications the mean blacking intensity of the labelled cell nuclei is lower in 2 months old animals than in 12 months old rabbits; that may mean an increase in the S-phase duration of the proliferating endothelial cells of the older animals. In the area with branching sites there are more 3H-thymidine-labelled endothelial cells than in the neighbouring vascular segments. The significance of these findings for the preferential incidence of arteriosclerosis in this area is discussed.

Zusammenfassung Uber die Ergebnisse einer kombinierten histoautoradiographischen und immunhistochemischen Untersuchung an Biopsie-Gewebe von 15 Hirntumoren wird berichtet. Meningiome, Oligodendrogliome und hoher differenzierte Astrozytome (G I und G II) zeigten eine um 1 % schwankende Fraktion von Zellen in der S-Phase, wahrend 4 der 7 anaplastischen Astrozytome hohere 3H-Thymidinmarkierungsindices (2,1; 3,0; 3,5; 11,4) aufwiesen. Die proliferative Heterogenitat nimmt mit steigendem Malignitatsgrad der Hirntumoren zu. Die GFAP-Reaktivitat der gliomatosen Tumoren (PA P-Technik) ist stark variabel und daher zur Bewertung ihrer Dignitat nur begrenzt geeignet. Bei 3 hochmalignen Tumoren (2 Glioblastome, 1 anaplastisches Astrozytom) wurde ein inverses Verhalten der untersuchten Parameter (hohe S-Phasenfraktion, geringe bzw. fehlende GFAP-Expression) nachgewiesen. Die nach In-vitro-Markierung von Hirntumorbiopsien mit 3H-Thymidin ermittelten Indices stellen ein relatives Mas fur die Grose der Wachstumsfraktion dar; sie sind wahrscheinlich klinisch-prognostisch wichtige Parameter.

Die in fruhen postnatalen Stadien einsetzende Proliferationshemmung der Herzmuskelzellen scheint daher nicht durch eine Storung der DNS-Synthese bedingt zu seine.

In order to study the physiological regeneration of the endothelium, "Hautchen", preparations of aortic endothelium were produced from 15 rabbits of different ages and studied autoradiographically. By determining the number of 3H-thymidine-labelled cells, the mitotic rate and the total number of cells per 0.12 mm2, we arrived at the following conclusions: (1) The mean generation time of endothelial cells increases with advancing age. The extension of the mean generation time of the endothelial cells, as deduced from the decrease in the 3H-thymidine labelling index and in the mitotic rate, mainly occurs during the first four months of life. (2) Neither between topologically different aortic segments nor between such portions of the aorta as are exposed to different flow-mechanical stresses could any considerable differences with regard to the reproduction rate of their endothelial cells be detected. The raised endothelial turnover in the area of the iliac bifurcation requires further studies.

Nach lokaler Bestrahlung von Kaninchenaorten mit einem 90Sr-90Y-Strahler von 3, 15 und 30 krad, wurde die Media in einem Zeitraum von 3 Tagen bis zu 8 Monaten nach der Bestrahlung untersucht. In der Aortenmedia kommt es zu einer Ablosung der elastischen Elemente von den glatten Muskelzellen, zu mottenfrasartigen Erscheinungen und schlieslich zu einem vollstandigen Untergang der elastischen Lamellen sowie zu einer Vakuolisierung der Muskelzellen mit nachfolgendem vakuolaren Zerfall. Die Veranderungen der elastischen Fasern sind sowohl Folge einer direkten toxischen Schadigung als auch einer indirekten Schadigung durch den fehlenden Substanznachschub nach Untergang der Muskelzellen. Die in den Randpartien nach langer Zeit zu beobachtende Elasticahyperplasie geht mit einer Funktionszunahme der glatten Muskelzellen einher. Sie ist in gleicher Weise wie die Zunahme der kollagenen Fasern im Bestrahlungsgebiet als reaktives Ereignis anzusehen.

Es wurde untersucht, ob sich aus dem Zustand der Lungenelastica von 110 Wasserleichen verwertbare Hinweise auf deren Liegezeit im Wasser ergeben. Eine solche Beziehung war bis zu einer Wasserliegezeit von zwei Monaten nicht zu erkennen. Nach zwei-bis dreimonatiger Liegezeit der Leichen im Wasser fanden sich in der Mehrzahl der Falle deutliche Unterschiede im Erhaltungszustand der verschiedenen elastischen Elemente der Lunge. Diesem Befund kommt lediglich eine unterstutzende Rolle bei der Liegezeitbestimmung von Wasserleichen zu.

Es wurde der Einflu\ von Hydrocortisonacetat auf den Einbau von3H-Thymidin,3H-Prolin und35S-Sulfat in die experimentelle Intimaproliferation untersucht. Die Intimaproliferation wurde durch doppelte Stenosierung der Aorta abdominalis bei Ratten erzeugt. Die Untersuchungen hatten folgende Ergebnisse: 1. Hydrocortisonacetat bewirkt unter den verwendeten Versuchsbedingungen eine erhebliche Hemmung der proliferativen IntimaverAnderungen. Der3H-Thymidin-Index ist dabei erniedrigt. 2. Fur die nach Hydrocortison-Gaben beobachtete Erhohung der Silberkorndichte uber den mit3H-Thymidin markierten Intimazellkernen werden unter Berucksichtigung der Ergebnisse karyometrischer Untersuchungen in Frage kommende Ursachen diskutiert. 3. Zum Zeitpunkt des Proliferationsmaximums ist der Einbau von3H-Prolin und35S Sulfat in die proliferierte Intima nach Hydrocortisonacetat-Applikation herabgesetzt.

A recently developed method for the detection of monoamines in tissues has been studied by means of a modified Caspersson-Zeiss Universal Microspectrophotometer. This histochemical reaction involves a condensation of the monoamines with formaldehyde vapour to an isoquinoline derivative, that shows an intense fluorescence when under ultraviolet or blue violet light. In this study a model system is used, consisting of microdroplets of various catecholamines and indolalkylamines in a protein solution. Fluorescence and excitation spectra, and fading of the emitted light during irradiation, has been used for identification of the different amines. The fluorescence maximum at a neutral pH for catecholamines is around 480 mµ, for 5-hydroxytryptamine (5-HT) around 525 mμ. Together with the rapid fading of 5-HT and the fact that 5-HT — but not catecholamines — has an additional excitation maximum at around 300 mu, this can be used for the certain identification of and differentiation between very small amounts of these two monoamines. These findings are valid also in tissues which contain a known catecholamine (e.g. sympathetic nerve endings and ganglia, adrenal medulla) and 5-HT (enterochromaffin cells, mast cells from rat and mouse) and also in tissues which contain both, but in different cells (brain stem of rat). The quantitative aspects of the reaction are investigated by determining the dry mass of a microdroplet by interference microspectrophotometry, and measuring the total amount of fluorescence at the fluorescence peak of the very same droplet.
