Integrating running gait coordination assessment into athlete monitoring systems could provide unique insight into training tolerance and fatigue-related gait alterations. This study investigated the impact of an overload training intervention and recovery on running gait coordination assessed by field-based self-testing. Fifteen trained distance runners were recruited to perform 1-week of light training (baseline), 2 weeks of heavy training (high intensity, duration, and frequency) designed to overload participants, and a 10-day light taper to allow recovery and adaptation. Field-based running assessments using ankle accelerometry and online short recovery and stress scale (SRSS) surveys were completed daily. Running performance was assessed after each training phase using a maximal effort multi-stage running test-to-exhaustion (RTE). Gait coordination was assessed using detrended fluctuation analysis (DFA) of a stride interval time series. Two participants withdrew during baseline training due to changed personal circumstances. Four participants withdrew during heavy training due to injury. The remaining nine participants completed heavy training and were included in the final analysis. Heavy training reduced DFA values (standardised mean difference (SMD) = −1.44 ± 0.90; p = 0.004), recovery (SMD = −1.83 ± 0.82; p less than 0.001), performance (SMD = −0.36 ± 0.32; p = 0.03), and increased stress (SMD = 1.78 ± 0.94; p = 0.001) compared to baseline. DFA values (p = 0.73), recovery (p = 0.77), and stress (p = 0.73) returned to baseline levels after tapering while performance trended towards improvement from baseline (SMD = 0.28 ± 0.37; p = 0.13). Reduced DFA values were associated with reduced performance (r2 = 0.55) and recovery (r2 = 0.55) and increased stress (r2 = 0.62). Field-based testing of running gait coordination is a promising method of monitoring training tolerance in running athletes during overload training.

Abstract Background This study aims to investigate the association between bony morphological features and ACL injury and to identify potential risk factors for the identification and prevention of ACL injuries. The study used knee MRIs to examine the distal femoral and proximal tibia morphology of 500 patients aged 19–50, including 250 cases with an injured ACL and 250 controls with a non-injured ACL. Previous literature has shown that ACL injuries are a common occurrence in sports and can have serious long-term consequences. Several risk factors have been identified, including anatomical variations in the bony structures of the knee. However, the relationship between bony morphology and ACL injury is not well understood, and there is a need to investigate the potential role of these factors in the identification and prevention of ACL injuries. This study was necessary because of the high incidence of ACL injuries and the potential long-term consequences, such as osteoarthritis, associated with this injury. By identifying potential risk factors, such as bony morphological features, it may be possible to develop targeted prevention strategies to reduce the incidence of ACL injuries. The study also adds to the existing literature by providing new insights into the relationship between bony morphology and ACL injury, and by identifying specific parameters that can be used to predict ACL injury. Results There was a significant difference in bony morphology between ACL-injured and non-injured patients. The most convincing parameters of femur and tibia were notch width index (NWI), medial tibial plateau depth (MTPD), and lateral tibial plateau slope (LTPS). Aims: To investigate factors that contribute to patient decisions regarding attendance for arthroplasty during the COVID-19 pandemic. Methods: A postal questionnaire was distributed to patients on the waiting list for hip or knee arthroplasty in a single tertiary centre within the UK. Patient factors that may have influenced the decision to attend for arthroplasty, global quality of life (QoL) (EuroQol five-dimension three-level (EQ-5D-3L)), and joint-specific QoL (Oxford Hip or Knee Score) were assessed. Patients were asked at which ‘COVID-alert’ level they would be willing to attend an NHS and a “COVID-light” hospital for arthroplasty. Independent predictors were assessed using multivariate logistic regression. Results: Of 540 distributed questionnaires, 400 (74.1%; 236 awaiting hip arthroplasty, 164 awaiting knee arthroplasty) complete responses were received and included. Less than half (48.2%) were willing to attend for hip or knee arthroplasty while a UK COVID-19 epidemic was in circulation (COVID-alert levels 3 to 5). Patients with worse joint-specific QoL had a preference to proceed with surgery at COVID-alert levels 3 to 5 compared to levels 1 and 2 (hip arthroplasty odds ratio (OR) 1.54 (95% confidence interval (CI) 1.45 to 1.63); knee arthroplasty OR 1.16 (1.07 to 1.26)). The odds of patients with worse joint-specific QoL being willing to attend for surgery at COVID-alert levels 3 to 5 increased further if surgery in a private, “COVID-light” hospital was available (hip arthroplasty OR 3.50 (95% CI 3.26 to 3.71); knee arthroplasty OR 1.41 (95% CI 1.29 to 1.53). Conclusion: Patient decisions surrounding elective surgery have been influenced by the global COVID-19 pandemic, highlighting the importance of patient involvement in ensuring optimized provision of elective surgery during these challenging times.

Saussurea costus (Falc.) Lipsch., (Asteraceae) known in English as Costus, is a threatened Himalayan medicinal plant listed on CITES (2014) Appendix I, Schedule VI of the Wildlife Protection Act (India) 1972, and Critically Endangered on the IUCN Red List. Wild collection of Costus is banned in India and permission is required for its cultivation and marketing. In the past 100 years of cultivation, various policy and management issues have impacted commercialization of the species. In 2015, we conducted surveys in the village of Kanol, Chamoli District, Uttarakhand, India to determine the status of Costus cultivation, problems associated with its marketing and the potential for expanded propagation to enhance local livelihoods. Forty-nine farmers in the study area were cultivating costus and interest in its cultivation had increased in the recent past due to the availability of governmental support. Annually, 1,250–2,950 kg roots (dried) and 20–57 kg seeds from this plant were produced by farmers within the study area. The area under Costus cultivation per farm was fairly constant (0.5 or 0.6 ha.) from 2012 to 2014, and the production of roots per farm ranged from 128 to 156 kg per year. Market prices for dried roots per kg had decreased over time. We found that regional and national marketing of the plant was not a problem for farmers, but export of cultivated products was a major challenge due to existing laws. In addition, local understanding related to post-harvest value addition, and self-reliance in Costus cultivation, was generally poor. Better national policies that increase the prospects for export, and more outreach to local villagers, are needed to improve the conservation and sustainable uses of Costus.

Purpose: Reddit is a social media platform that allows health care professionals (HPs) to anonymously interact with patients. We analyzed content about radiation therapy (RT) on Reddit. Methods and Materials: Reddit.com/r/cancer was queried with 20 search terms related to RT: IMRT, 3D-CRT, SBRT, EBRT, XRT, radiation, radiotherapy, RT, radio, rad, rads, gamma, gamma knife, gammaknife, cyber knife, cyberknife, cyber, brachytherapy, brachy, and cobalt. The search aimed to identify all posts discussing RT. A random sample of posts and their top 3 comments was selected to generate qualitative thematic codes per author consensus, which were used to characterize all posts and their top 3 voted comments. Posts were analyzed for time to first reply, mention of any specific RT modality, whether a self-identified HP responded, and time to first highly voted comment by an HP. Results: Exactly 190 posts about RT by 178 users were shared from February 2011 to May 2018, and 468 replies by 295 users were also analyzed. Objectives: Long-term quality-of-life data following stereotactic body radiation therapy (SBRT) for recurrent head and neck cancer (rHNC) is underreported. We report patient-reported quality-of-life (PR-QOL) after at least 1 year post-treatment.Methods and Materials: A retrospective review was performed on 64 patients receiving SBRT for previously-irradiated rHNC. PR-QOL was prospectively evaluated using the University of Washington Quality of Life Questionnaire. The mixed effects proportional odds model was used to assess post-treatment overall PR-QOL changes, as well as the effects of late toxicities, tumor volume > 25 cc, local failure, nodal recurrence, distant failure, prior neck dissection, performance status other than ECOG 0 or Karnofsky 100, sex, age >65, squamous vs. non-squamous primary histology, and specific organ recurrence.Results: SBRT had no significant effect on overall PR-QOL at days 1-90 post-treatment (SBRT effect 0.035, p = 0.93) and days 91–365 (SBRT effect −0.30, p = 0.45). Beyond day 365, overall PR-QOL was significantly worse than baseline (SBRT effect −0.77, p =.03). Grade ≥3 late toxicities (p = 0.0072) and tumor volume > 25 cc (p = 0.032) predicted significantly worse overall PR-QOL. Oral cavity recurrence predicted significant decrements in chewing (p = 0.0006), swallowing (p = 0.0301), and taste PR-QOL (p = 0.02). Nasal recurrence predicted significant decrements in taste PR-QOL (p = 0.030). Grade ≥3 late dysphagia predicted significant decline in chewing (p = 0.039) and swallowing (p = 0.0004). Grade ≥3 late osteonecrosis predicted significant differences in pain PR-QOL (p = 0.0026).Conclusion: PR-QOL across several domains declines immediately after SBRT for previously-irradiated rHNC before returning to baseline levels at 1 year. Long-term PR-QOL declines thereafter. Patients with grade ≥3 late toxicities or tumor volume >25 cc report reduced long-term overall PR-QOL, likely representing late disease progression. Specific organ recurrence and grade ≥3 late toxicities predict decrements in specific PR-QOL domains.

A male in his 30s who was recently diagnosed with HIV arrived at the emergency department exhibiting an altered mental state and acute respiratory distress. Initial laboratory tests revealed a high anion gap metabolic acidosis, elevated liver enzyme levels and bicytopenia. A CT scan identified a miliary pattern. Bronchoscopy with bronchoalveolar lavage displayed epithelial and inflammatory cells. However, subsequent tests ruled out the presence of fungi, Pneumocystis organisms, malignancies, granulomas and viral inclusions. Broadspectrum antibiotics with emphasis on Mycobacterium tuberculosis and antifungal treatments were administered. The regimen was adjusted after a positive urine test for the Histoplasma antigen. The patient later manifested signs and symptoms, including increased ferritin level, fever, splenomegaly, diminished natural killer cell function and heightened interleukin-2 receptor levels, confirming haemophagocytic lymphohistiocytosis. Given the patient’s gravely decompensated state, the treatment incorporated dexamethasone, and the patient’s vasopressor-resistant septic shock was addressed with methylene blue.
