TEMA: artigos relacionados a emissões otoacústicas no diagnóstico diferencial da perda auditiva induzida por ruído (PAIR). OBJETIVOS: evidenciar a importância da utilização das emissões otoacústicas no diagnóstico diferencial da perda auditiva induzida por ruído (PAIR). CONCLUSÃO: confirmou-se que tanto as emissões otoacústicas transientes (EOAT) quanto às emissões otoacústicas por produto de distorção (EOAPD) são úteis no diagnóstico precoce da PAIR sendo que as EOAT se revelam mais sensíveis às mudanças temporárias de limiar que as EOAPD e as EOAPD se destacam mais eficazes no diagnóstico precoce da PAIR em indivíduos com audição aparentemente normal por captar respostas nas frequências mais agudas que são primeiramente atingidas na PAIR. Acredita-se, ainda, que este estudo possa beneficiar a população quanto à prevenção e controle da PAIR e incentivar a criação de políticas públicas que contribuirão com a prevenção das alterações auditivas dentro do âmbito ocupacional.BACKGROUND: articles related of otoacoustic emissions in differential diagnosis of noise-induced hearing loss (NIHL). PURPOSES: to demonstrate the importance of using otoacoustic emissions in the differential diagnosis of noise-induced hearing loss (NIHL). CONCLUSION: it was confirmed that both the transient-evoked otoacoustic emissions (TEOAE) and distortion-product otoacoustic emissions (DPOAE) are useful in the early diagnosis of and the TEOAE and are more sensitive to temporary alterations in the DPOAE threshold, and DPOAE stand out more effectively in the early diagnosis of NIHL in individuals with apparently normal hearing responses by capturing responses more acute frequencies in that are first affected in NIHL. It is also believed that this study can benefit the population on the NIHL prevention and control and foment the creation of public policies that shall contribute to the prevention of hearing impairment within the occupational field.

A terceirização trouxe mudanças nas atividades praticadas pela Administração Pública, que passou a se focar intensamente nas suas atividades-fim, repassando as atividades-meio para as empresas privadas, por intermédio de contratos. A gestão de contratos administrativos precisa estar ajustada aos princípios da legalidade, impessoalidade, moralidade, publicidade e eficiência. Este trabalho versa sobre a gestão de contrato de terceirização na Administração Pública. No caso específico, o contrato de prestação dos serviços terceirizados de limpeza firmado entre empresa terceirizada e a Universidade Federal de Santa Catarina (UFSC), Instituição Pública Federal de Ensino Superior, vinculada ao Ministério da Educação. O objetivo principal deste estudo é analisar os desafios e perspectivas na gestão do Contrato de prestação de serviços terceirizados de limpeza na UFSC. Para tal, foi adotada como procedimento de investigação a análise bibliográfica e documental, mostrando matérias que ajudam a elucidar o panorama das faces e interfaces que permeiam e norteiam o tema principal desta pesquisa. O estudo está embasado em informações coletadas por meio de entrevistas estruturadas, aplicadas junto a onze fiscais do Contrato das unidades de ensino, um fiscal administrativo, um gestor do Contrato, um Pró-Reitor de Administração e um preposto da empresa terceirizada. Trata-se de um estudo de caso com abordagem qualitativa e finalidade descritiva. Para as entrevistas, houve a aplicação de roteiros de perguntas divididas em cinco blocos, contemplando as seguintes categorias de análise: Caracterização; Capacitação; Atribuições; Qualidade; Avaliação. Os resultados encontrados no presente estudo revelaram inúmeros desafios na gestão do Contrato de prestação de serviços terceirizados de limpeza da UFSC, que precisam ser devidamente analisados e transpostos, entre os quais se destacam: qualificação profissional; qualidade dos serviços prestados; padronização de procedimentos de gestão/fiscalização; parâmetros de avaliação; rotatividade de pessoal da contratada; comprometimento dos envolvidos. Por outro lado, como perspectivas identificadas neste estudo estão as recentes ofertas, pela Administração da UFSC, de cursos de capacitação para fiscalização de contratos administrativos e a elaboração ? em andamento ? de um manual de fiscalização de contratos, da própria Instituição. Abstract : Outsourcing has brought about changes in activities in public administration, which has recently focused intensely on its core competencies, leaving its secondary activities, peripheral or non-core for private companies, by means of contracts. Management of administrative contracts must comply with the principles of legality, impersonality, morality, publicity and efficiency. This study concerns the management of outsourcing contract in public administration, more specifically the management of the outsourcing contract concerning cleaning services between the third-party company and The Federal University of Santa Catarina (Universidade Federal de Santa Catarina - UFSC), a public federal institution of higher education, linked to the Ministry of Education. The main purpose of the study is to analyze the challenges and perspectives concerning the management of the outsourcing contract related to cleaning services at UFSC. In order to accomplish that, a bibliographical and document analysis was carried out as procedure for the investigation, showing material which help to elucidate the panorama of faces and interfaces which permeate and guide the main theme of this research. The study is based on information collected from structured interviews, applied to eleven supervisors of the contract at each teaching center, the administrative supervisor, the manager of the contract, the university´s Dean of Administration and the representative of the outsourcing company. The investigation is a case study with a qualitative approach and a descriptive design. For the interviews, there was a guide with questions divided into five blocks: characterization, qualification; attribution, quality and evaluation. Results found in the present study revealed innumerable challenges in the management of the outsourcing cleaning services at UFSC which need to be properly analyzed and overcome, among which the most important are: professional qualification; quality in the services offered; standard procedures regarding management and control; parameters of evaluation; turnover of the contracted employees and commitment of those involved. On the other hand, perspectives found in this study include courses offered by the administration at UFSC for supervision/control of administrative contracts and the on-going development of a specific manual for supervision/control of contracts by UFSC.

In the current era of malaria eradication, reducing transmission is critical. Assessment of transmissibility requires tools that can accurately identify the various developmental stages of the malaria parasite, particularly those required for transmission (sexual stages). Here, we present a method for estimating relative amounts of Plasmodium falciparum asexual and sexual stages from gene expression measurements. These are modeled using constrained linear regression to characterize stage-specific expression profiles within mixed-stage populations. The resulting profiles were analyzed functionally by gene set enrichment analysis (GSEA), confirming differentially active pathways such as increased mitochondrial activity and lipid metabolism during sexual development. We validated model predictions both from microarrays and from quantitative RT-PCR (qRT-PCR) measurements, based on the expression of a small set of key transcriptional markers. This sufficient marker set was identified by backward selection from the whole genome as available from expression arrays, targeting one sentinel marker per stage. The model as learned can be applied to any new microarray or qRT-PCR transcriptional measurement. We illustrate its use in vitro in inferring changes in stage distribution following stress and drug treatment and in vivo in identifying immature and mature sexual stage carriers within patient cohorts. We believe this approach will be a valuable resource for staging lab and field samples alike and will have wide applicability in epidemiological studies of malaria transmission.

O processo de terceirização nas organizações públicas, mais especificamente nas Instituições Federais de Ensino Superior, a exemplo da Universidade Federal de Santa Catarina (UFSC), trouxe significativas transformações nas atividades praticadas pela administração pública. Em vista disso, a UFSC passou a focar mais intensamente nas suas atividades-fim, deixando suas atividades-meio nas mãos de empresas terceirizadas. Esse estudo tem como objetivo a análise da gestão/fiscalização do contrato de serviços de limpeza e conservação prestados por empresa terceirizada à Universidade Federal de Santa Catarina, aplicada, especificamente, no Centro de Comunicação e Expressão (CCE). A metodologia é de natureza aplicada e abordagem qualitativa. Quanto aos objetivos é descritiva, e, no que diz respeito à técnica, a pesquisa é bibliográfica e documental, complementada por informações coletadas no local de estudo, por meio de entrevistas com os atores sociais ligados diretamente ao tema. O resultado do estudo revelou que há limitações na gestão do serviço terceirizado do CCE, as quais precisam ser solucionadas, a fim de se garantir a efetividade do sistema de limpeza e conservação.

TEMA: artigos relacionados a emissões otoacústicas no diagnóstico diferencial da perda auditiva induzida por ruído (PAIR). OBJETIVOS: evidenciar a importância da utilização das emissões otoacústicas no diagnóstico diferencial da perda auditiva induzida por ruído (PAIR). CONCLUSÃO: confirmou-se que tanto as emissões otoacústicas transientes (EOAT) quanto às emissões otoacústicas por produto de distorção (EOAPD) são úteis no diagnóstico precoce da PAIR sendo que as EOAT se revelam mais sensíveis às mudanças temporárias de limiar que as EOAPD e as EOAPD se destacam mais eficazes no diagnóstico precoce da PAIR em indivíduos com audição aparentemente normal por captar respostas nas frequências mais agudas que são primeiramente atingidas na PAIR. Acredita-se, ainda, que este estudo possa beneficiar a população quanto à prevenção e controle da PAIR e incentivar a criação de políticas públicas que contribuirão com a prevenção das alterações auditivas dentro do âmbito ocupacional. 25. Evidence for spleen dysfunction in malaria-HIV co-infection in a subset of pediatric patients

The spleen has an important role in the clearance of malaria parasites, and the role of HIV co-infection on this process is yet to be described. Using a combination of histological and molecular methods, we systematically evaluated parasite load across multiple organs from HIV-positive and HIV-negative cases of an autopsy study of pediatric comatose children with malaria infection (n = 103) in Blantyre, Malawi. Quantification of parasite load across organs was done using histology. A subset of cases was further characterized for parasite localization and stage of development using immunohistochemistry-based labeling of parasite and host cells (5 HIV-positive, 10 HIV-negative), and quantitative RT-PCR (qRT-PCR) of asexual and sexual-specific genes (4 HIV-positive, 5 HIV-negative). The results were compared with clinical information including HIV status. The HIV positive rate was 21% for the group studied (20 of 95) and HIV-positive patients had a significantly shorter duration of time between onset of illness and death, and were significantly older than HIV-negative patients. We found that spleens of HIV-positive cases had significantly higher parasite loads compared to those of HIV-negative cases in each the three methods we used: (i) standard histology, (ii) immunohistochemistry-based labeling of Plasmodium lactate dehydrogenase (pLDH), and (iii) molecular detection of asexual parasite transcript apical membrane antigen 1 (AMA1). Immunohistochemistry-based labeling of macrophage marker CD163 in a subset of spleens revealed fewer activated macrophages containing engulfed parasites and a greater number of free unphagocytosed parasites in the HIV-positive cases. The mechanism by which HIV infection is associated with more rapid progression to severe cerebral malaria disease is possibly impairment of parasite destruction by splenic macrophages, supported by published in vitro studies showing inefficient phagocytosis of malaria parasites by HIV-infected macrophages.

26. Characterization of Malaria Sexual Stage Development in the Human Host

Due to an increase in malaria control programs in the last decade, the world has witnessed dramatic reductions in the number of infections and deaths caused by the malaria parasite. With malaria eradication on the global health agenda, a shift toward transmission-focused research has led to a renewed focus on a previously neglected stage of malaria: the sexual stage (gametocyte). Malaria's sexual stages are the only stages in the human host that are transmitted to the mosquito vector, and are therefore of critical importance for blocking transmission of this devastating disease. The process through which developing gametocytes sequester outside of the bloodstream during their 8-10 day maturation is not well understood and stands to be exploited as a potential target for therapeutic intervention. In Chapter 1, we discuss the current state of knowledge on the development of these stages in the human host. In Chapter 2, we investigate anatomical enrichment sites for developing gametocytes in the human host using autopsy tissue from cases of fatal malaria. Immunohistochemistry (IHC) and quantitative reverse transcriptase PCR-based assessments identified the bone marrow as a preferential enrichment site of developing gametocytes. Co-localization with host proteins revealed the enrichment of gametocytes inside the extravascular space of the bone marrow, often observed in contact with erythroblastic island structures. In vitro experiments with erythrocyte precursor cells, as well as in vivo co-localization studies demonstrated that gametocytes can develop within the cells of the hematopoietic system of the bone marrow. In Chapter 3, we present an assay and analysis tool for inferring the presence of young and mature asexual and sexual stages in the peripheral blood of infected patients based on gene expression data. We apply this assay to malaria patient cohorts and in vitro drug perturbation time course experiments, and demonstrate its use in identifying young and mature gametocyte carriers, as well as characterizing the effect of a given perturbation on parasite development. This body of work aims to contribute to the overall knowledge base for malaria’s elusive gametocytes as well as to establish tools for performing future assessments on these transmissible stages.
