01 juli 2023, Item does not contain fulltext, Cerebral small vessel disease (SVD) is common during ageing and can present as stroke, cognitive decline, neurobehavioural symptoms, or functional impairment. SVD frequently coexists with neurodegenerative disease, and can exacerbate cognitive and other symptoms and affect activities of daily living. Standards for Reporting Vascular Changes on Neuroimaging 1 (STRIVE-1) categorised and standardised the diverse features of SVD that are visible on structural MRI. Since then, new information on these established SVD markers and novel MRI sequences and imaging features have emerged. As the effect of combined SVD imaging features becomes clearer, a key role for quantitative imaging biomarkers to determine sub-visible tissue damage, subtle abnormalities visible at high-field strength MRI, and lesion-symptom patterns, is also apparent. Together with rapidly emerging machine learning methods, these metrics can more comprehensively capture the effect of SVD on the brain than the structural MRI features alone and serve as intermediary outcomes in clinical trials and future routine practice. Using a similar approach to that adopted in STRIVE-1, we updated the guidance on neuroimaging of vascular changes in studies of ageing and neurodegeneration to create STRIVE-2.

Abstract

Objective: Traditionally, asymmetric spatial processing (i.e., hemispatial neglect) has been assessed with paper-and-pencil tasks, but growing evidence indicates that computer-based methods are a more sensitive assessment modality. It is not known, however, whether simply converting well-established paper-and-pencil methods into a digital format is the best option. The aim of the present study was to compare sensitivity in detecting contralesional omissions of two different computer-based methods: a “digitally converted” cancellation task was compared with a computer-based Visual and Auditory dual-tasking approach, which has already proved to be very sensitive. Methods: Participants included 40 patients with chronic unilateral stroke in either the right hemisphere (RH patients, N = 20) or the left hemisphere (LH patients, N = 20) and 20 age-matched healthy controls. The cancellation task was implemented on a very large format (173 cm × 277 cm) or in a smaller (A4) paper-and-pencil version. The computer-based dual-tasks were implemented on a 15′′ monitor and required the detection of unilateral and bilateral briefly presented lateralized targets. Results: Neither version of the cancellation task was able to show spatial bias in RH patients. In contrast, in the Visual dual-task RH patients missed significantly more left-sided targets than controls in both unilateral and bilateral trials. They also missed significantly more left-sided than right-sided targets only in the bilateral trials of the Auditory dual-task. Conclusion: The dual-task setting outperforms the cancellation task approach even when the latter is implemented on a (large) screen. Attentionally demanding methods are useful for revealing mild forms of contralesional visuospatial deficits.

6. Vuosina 2008–2016 OYS:ssa kuolleiden lasten soveltuvuus elinluovuttajiksi

Abstract

Tiivistelmä. Suomessa lapsikuolleisuus on nykyään pienempiä maailman tasolla. Lasten kuolleisuus on Suomessa pienentynyt koko viime vuosisadan ajan. Lain mukaan ihminen on kuollut, kun kaikki hänen aivotoimintonsa ovat pysyvästi loppuneet. Kuolema voidaan todeta sydämen vielä sykkiessä, kun tutkittaessa aivoperäiset vasteet puuttuvat ja todetaan että hengityskeskuksen toiminta on loppunut. Aivokuoleman määritelmä on sama lapsilla ja aikuisilla. Lapsilla aivokuolemat ovat harvinaisia, jopa lasten teho-osastoilla, jonka takia jokainen aivokuollut tulisi tunnistaa. Elinsiirrot ovat kustannusvaikutteisia hoitomuotoja, jotka usein pelastavat potilaan hengen vaikeimmissa maksan, sydämen ja keuhkojen sairauksissa. Lapset eivät säästy elinsiirtojonoissa olemiselta ja lista elinsiirtoa odottavista lapsista on kasvanut. Lapsipotilaiden määrä elinluovuttajia sen sijaan on monien vuosien ajan pysynyt samalla tasolla. Tämän tutkimuksen avulla pyrimme selvittämään, toimiiko Pohjois-Pohjanmaan sairaanhoitopiirin alueella lapsipotilaiden kohdalla elinsiirtokandidaattien tunnistaminen ja hoito asetusten mukaisesti. Kartoitimme mahdollisia ongelmakohtia luovuttajakandidaattien tunnistamisessa. Tämä tutkimus on takautuva rekisteritutkimus. Tutkimukseen otimme mukaan kaikki vuosina 2008–2016 OYS:ssa kuolleet lapset (alle 18-vuotiaat). Tutkimustiedot kerättiin ESKO-potilasjärjestelmästä ja tutkimuksessa käytettiin aineiston analyysiin SPSS-ohjelmaa. Tutkimustulosten perusteella aivokuolleiksi luokiteltiin 10 potilasta (5,7%). Näistä potilaista elinluovuttajana toimi yhteensä viisi potilasta. Kahden potilaan kohdalla aivokuolemaa ei tunnistettu ja huoltajien mielipidettä elinluovutuksesta ei kysytty. Lisäksi huoltajien kielteinen mielipide elinluovutuksesta esti kahden potilaan kohdalla aivokuolemadiagnoosin lopullisen tekemisen. Lasten yleisimmät kuolinsyyt OYS:ssa näinä vuosina olivat synnynnäiset rakennepoikkeavuudet ja onkologiset sairaudet.

Abstract

Tiivistelmä Aivojen pienten suonten tautiin liittyviä valkean aineen muutoksia voidaan ehkäistä ja jo kehittyneiden muutosten etenemistä hidastaa. Verenpaineen hoitaminen ja tupakoimattomuus ovat keskeisiä, ja ennen kaikkea tulee toimia ajoissa eli ennen mahdollisia laajoja patologisia muutoksia pienissä suonissa. Verenpainetta on syytä mitata ja tarvittaessa hoitaa jo nuoresta aikuisiästä alkaen. Myös kolesterolipitoisuutta pienentävällä hoidolla saattaa olla merkitystä., In this review we have gathered information on how to prevent vascular white matter changes and how to slow down their progression. It is important to control blood pressure and avoid smoking, above all in an early phase when the small vessels are without widespread pathological changes. Blood pressure must be measured and treated, if needed, already in young adulthood. Cholesterol-lowering treatment may also help prevent white matter changes.

Abstract

Hanna Jokinen tutkimusryhmän jäsenenä (VICCCS Grp) Introduction: Progress in understanding and management of vascular cognitive impairment (VCI) has been hampered by lack of consensus on diagnosis, reflecting the use of multiple different assessment protocols. A large multinational group of clinicians and researchers participated in a two-phase Vascular Impairment of Cognition Classification Consensus Study (VICCCS) to agree on principles (VICCCS-1) and protocols (VICCCS-2) for diagnosis of VCI. We present VICCCS-2. Methods: We used VICCCS-1 principles and published diagnostic guidelines as points of reference for an online Delphi survey aimed at achieving consensus on clinical diagnosis of VCI. Results: Six survey rounds comprising 65-79 participants agreed guidelines for diagnosis of VICCCS-revised mild and major forms of VCI and endorsed the National Institute of Neurological Disorders-Canadian Stroke Network neuropsychological assessment protocols and recommendations for imaging. Discussion: The VICCCS-2 suggests standardized use of the National Institute of Neurological Disorders-Canadian Stroke Network recommendations on neuropsychological and imaging assessment for diagnosis of VCI so as to promote research collaboration. Abstract

ObjectiveTo address the variability in prevalence estimates and inconsistencies in potential risk factors for poststroke cognitive impairment (PSCI) using a standardized approach and individual participant data (IPD) from international cohorts in the Stroke and Cognition Consortium (STROKOG) consortium.MethodsWe harmonized data from 13 studies based in 8 countries. Neuropsychological test scores 2 to 6 months after stroke or TIA and appropriate normative data were used to calculate standardized cognitive domain scores. Domain-specific impairment was based on percentile cutoffs from normative groups, and associations between domain scores and risk factors were examined with 1-stage IPD meta-analysis.ResultsIn a combined sample of 3,146 participants admitted to hospital for stroke (97%) or TIA (3%), 44% were impaired in global cognition and 30% to 35% were impaired in individual domains 2 to 6 months after the index event. Diabetes mellitus and a history of stroke were strongly associated with poorer cognitive function after covariate adjustments; hypertension, smoking, and atrial fibrillation had weaker domain-specific associations. While there were no significant differences in domain impairment among ethnoracial groups, some interethnic differences were found in the effects of risk factors on cognition.ConclusionsThis study confirms the high prevalence of PSCI in diverse populations, highlights common risk factors, in particular diabetes mellitus, and points to ethnoracial differences that warrant attention in the development of prevention strategies.

Abstract

Introduction: Progress in understanding and management of vascular cognitive impairment (VCI) has been hampered by lack of consensus on diagnosis, reflecting the use of multiple different assessment protocols. A large multinational group of clinicians and researchers participated in a two-phase Vascular Impairment of Cognition Classification Consensus Study (VICCCS) to agree on principles (VICCCS-1) and protocols (VICCCS-2) for diagnosis of VCI. We present VICCCS-2. Methods: We used VICCCS-1 principles and published diagnostic guidelines as points of reference for an online Delphi survey aimed at achieving consensus on clinical diagnosis of VCI. Results: Six survey rounds comprising 65–79 participants agreed guidelines for diagnosis of VICCCS-revised mild and major forms of VCI and endorsed the National Institute of Neurological Disorders–Canadian Stroke Network neuropsychological assessment protocols and recommendations for imaging. Discussion: The VICCCS-2 suggests standardized use of the National Institute of Neurological Disorders–Canadian Stroke Network recommendations on neuropsychological and imaging assessment for diagnosis of VCI so as to promote research collaboration.

Abstract

Introduction Numerous diagnostic criteria have tried to tackle the variability in clinical manifestations and problematic diagnosis of vascular cognitive impairment (VCI) but none have been universally accepted. These criteria have not been readily comparable, impacting on clinical diagnosis rates and in turn prevalence estimates, research, and treatment. Methods The Vascular Impairment of Cognition Classification Consensus Study (VICCCS) involved participants (81% academic researchers) from 27 countries in an online Delphi consensus study. Participants reviewed previously proposed concepts to develop new guidelines. Results VICCCS had a mean of 122 (98–153) respondents across the study and a 67% threshold to represent consensus. VICCCS redefined VCI including classification of mild and major forms of VCI and subtypes. It proposes new standardized VCI-associated terminology and future research priorities to address gaps in current knowledge. Discussion VICCCS proposes a consensus-based updated conceptualization of VCI intended to facilitate standardization in research.

Abstract

INTRODUCTION: The Stroke and Cognition consortium (STROKOG) aims to facilitate a better understanding of the determinants of vascular contributions to cognitive disorders and help improve the diagnosis and treatment of vascular cognitive disorders (VCD). METHODS: Longitudinal studies with ≥75 participants who had suffered or were at risk of stroke or TIA and which evaluated cognitive function were invited to join STROKOG. The consortium will facilitate projects investigating rates and patterns of cognitive decline, risk factors for VCD, and biomarkers of vascular dementia. RESULTS: Currently, STROKOG includes 25 (21 published) studies, with 12,092 participants from five continents. The duration of follow-up ranges from 3 months to 21 years. DISCUSSION: Although data harmonization will be a key challenge, STROKOG is in a unique position to reuse and combine international cohort data and fully explore patient level characteristics and outcomes. STROKOG could potentially transform our understanding of VCD and have a worldwide impact on promoting better vascular cognitive outcomes.

Abstract

Introduction Numerous diagnostic criteria have tried to tackle the variability in clinical manifestations and problematic diagnosis of vascular cognitive impairment (VCI) but none have been universally accepted. These criteria have not been readily comparable, impacting on clinical diagnosis rates and in turn prevalence estimates, research, and treatment. Methods The Vascular Impairment of Cognition Classification Consensus Study (VICCCS) involved participants (81% academic researchers) from 27 countries in an online Delphi consensus study. Participants reviewed previously proposed concepts to develop new guidelines. Results VICCCS had a mean of 122 (98–153) respondents across the study and a 67% threshold to represent consensus. VICCCS redefined VCI including classification of mild and major forms of VCI and subtypes. It proposes new standardized VCI-associated terminology and future research priorities to address gaps in current knowledge. Discussion VICCCS proposes a consensus-based updated conceptualization of VCI intended to facilitate standardization in research.

Abstract

Background and purpose: Impairment of executive functions (EFs) is a common cognitive symptom post‐stroke and affects independence in daily activities. Previous studies have often relied on brief cognitive tests not fully considering the wide spectrum of EF subdomains. A detailed assessment of EFs was used to examine which of the subdomains and tests have the strongest predictive value on post‐stroke functional outcome and institutionalization in long‐term follow‐up. Methods: A subsample of 62 patients from the Helsinki Stroke Aging Memory Study was evaluated with a battery of seven neuropsychological EF tests 3 months post‐stroke and compared to 39 healthy control subjects. Functional impairment was evaluated with the modified Rankin Scale (mRS) and Instrumental Activities of Daily Living (IADL) scale at 3 months, and with the mRS at 15 months post‐stroke. Institutionalization was reviewed from the national registers of permanent hospital admissions in up to 21‐year follow‐up. Results: The stroke group performed more poorly than the control group in multiple EF tests. Tests of inhibition, set shifting, initiation, strategy formation and processing speed were associated with the mRS and IADL scale in stroke patients. EF subdomain scores of inhibition, set shifting and processing speed were associated with functional outcome. In addition, inhibition was associated with the risk for earlier institutionalization. Conclusions: Executive function was strongly associated with post‐stroke functional impairment. In follow‐up, poor inhibition was related to earlier permanent institutionalization. Abstract

The objective of this thesis was to determine the effects of design, operational and furnish quality parameters on pressure screen and hydrocyclone performance. The general contradictory interrelationships between capacity and selectivity in pressure screening and cleaning are commonly recognized, but deep understanding of the effects of design, operational and furnish quality parameters on the state of operation is missing. As separation selectivity is closely dependent on thickening and pulp passage, an operation curve for separation was applied and its application further developed to examine the parameters affecting pressure screen and hydrocyclone separation. New information was found on the geometry of the screen plate, furnish quality parameters in pressure screen fibre fractionation, and the hydrocyclone separation of fibres, sand and gases. The capacity increases achieved by changes in the wire screen plate geometry were found to be achieved at the cost of separation selectivity in probability screening. The capacity of the screen plate was affected by the flow on the screen plate and through it. The hydraulic resistance both in the forward and reverse flow directions was found to be of great importance for the capacity of the screen plate. Previously unreported knowledge was found regarding the effects of wire width, height and shape. Pressure screening capacity was found to decrease with increasing fibre length, fibre network strength and flocculation. Pressure screen fibre fractionation selectivity was increased by broadening of the fibre length distribution of the feed furnish. The furnish properties also affected the separation selectivity of fibres, sand and gases in the hydrocyclone. An increase in fibre network strength reduced the separation selectivity of the hydrocyclone separation of fibres and sand. Any increase in the specific surface area, and especially in the amount of fines, was found to make gas removal more challenging. It was concluded that a broader range of the specific surface distribution can increase the selectivity of fibre fractionation in the hydrocyclone. Knowledge of the general effects of design, operational and furnish quality parameters on the performance of pressure screen and hydrocyclone separation was deepened as a result of this work, which provides a framework for studying these effects further in pursuit of the general objective of maximizing capacity and selectivity while minimizing energy and investment costs.

Abstract

This chapter addresses the issue of becoming a teacher, with the induction phase as a critical transition from initial teacher education to the lifelong professional learning of teachers, and mentoring as a tool of support. It presents newly qualified teachers’ views on initial teacher education and discusses the need for fostering the connection between teacher training and the induction year. New teachers’ views on teacher induction and mentoring are based on research findings in Finland and other Nordic countries. Another important question addresses how initial teacher education can facilitate the metamorphosis of a student teacher into an autonomous teacher. The new teachers’ experiences of mentoring have been highly positive, and they wished to see it made a standard practice. The mentoring meetings have served them as a useful interaction that supported them in their work and enabled them to learn from each other and from an experienced mentor. The mentees considered mentoring a highly important tool for inducting new teachers and fostering their professional growth. Abstract

Corpus callosum (CC) is the main tract connecting the hemispheres, but the clinical significance of CC atrophy is poorly understood. The aim of this work was to investigate clinical and functional correlates of CC atrophy in subjects with age-related white matter changes (ARWMC). In 569 elderly subjects with ARWMC from the Leukoaraiosis And DISability (LADIS) study, the CC was segmented on the normalised mid-sagittal magnetic resonance imaging (MRI) slice and subdivided into five regions. Correlations between the CC areas and subjective memory complaints, mini mental state examination (MMSE) score, history of depression, geriatric depression scale (GDS) score, subjective gait difficulty, history of falls, walking speed, and total score on the short physical performance battery (SPPB) were analyzed. Significant correlations between CC atrophy and MMSE, SPPB, and walking speed were identified, and the CC areas were smaller in subjects with subjective gait difficulty. The correlations remained significant after correction for ARWMC grade. Abstract

In this study we describe a previously unreported error in the vertical-displacement time series made with GPS-based Data well DWR-G4 wave buoys and introduce a simple method to correct the resulting wave spectra. The artefact in the time series is found to resemble a sawtooth wave, which produces an erroneous trend following an f −2 power law in frequency space. The correction method quantifies the amount of erroneous trend below a certain maximum frequency and removes the spurious energy from all frequencies assuming the above-mentioned f −2 power law. The presented correction method is validated against an experimental field test, and its impact on the measured significant wave height is quantified. Background/Aims: The Alzheimer's Disease Assessment Scale cognitive subscale (ADAS-cog) is a widely used rating instrument. The Vascular Dementia Assessment Scale cognitive subscale (VADAS-cog) includes additional tests reflecting mental speed and executive functions. The objective of this study was to compare the results of the two scales among subjects with various degrees of white matter hyperintensities (WMHs). Methods:In the multicentre, multinational Leukoaraiosis and Disability in the Elderly (LADIS) study, 616 non-disabled subjects between the ages of 65 and 84 were examined using MRI, the ADAS-cog and VADAS-cog. The WMH rating from the MRI divided the patients into groups of mild (n = 280), moderate (n = 187) and severe (n = 149) degrees of change. Results: Covariance analysis controlling for the effect of age and education revealed that the ADAS-cog differentiated only the mild and severe WMH groups, while the differences between all three groups were highly significant with the VADAS-cog. Conclusions:The VADAS-cog significantly differentiated between all the white matter groups. In comparison, the ADAS-cog differentiated only severe changes. Accordingly, the VADAS-cog may be a more sensitive endpoint in studies of patients with white matter load and vascular burden of the brain. Copyright (c) 2007 S. Karger AG, Basel. Background: Previous research has indicated that corpus callosum atrophy is associated with global cognitive decline in neurodegenerative diseases, but few studies have investigated specific cognitive functions.Objective: To investigate the role of regional corpus callosum atrophy in mental speed, attention and executive functions in subjects with age-related white matter hyperintensities (WMH).Methods: In the Leukoaraiosis and Disability Study, 567 subjects with age-related WMH were examined with a detailed neuropsychological assessment and quantitative magnetic resonance imaging. The relationships of the total corpus callosum area and its subregions with cognitive performance were analysed using multiple linear regression, controlling for volume of WMH and other confounding factors.Results: Atrophy of the total corpus callosum area was associated with poor performance in tests assessing speed of mental processing--namely, trail making A and Stroop test parts I and II. Anterior, but not posterior, corpus callosum atrophy was associated with deficits of attention and executive functions as reflected by the symbol digit modalities and digit cancellation tests, as well as by the subtraction scores in the trail making and Stroop tests. Furthermore, semantic verbal fluency was related to the total corpus callosum area and the isthmus subregion.Conclusions: Corpus callosum atrophy seems to contribute to cognitive decline independently of age, education, coexisting WMH and stroke. Anterior corpus callosum atrophy is related to the frontal-lobe-mediated executive functions and attention, whereas overall corpus callosum atrophy is associated with the slowing of processing speed.
