2. Biomechanical and histomorphometric analysis of osseodensification drilling versus conventional technique: a systematic review and meta-analysis.

Objectives: This systematic review aimed to search the literature for the answer to the following questions. In human studies: Does the osseodensification technique increase the resonance frequency analysis given in implant stability quotient value and the insertion torque value compared to the conventional technique? In animal studies: Does the osseodensification technique increase implant stability quotient, bone-to-implant contact, and bone area fraction occupancy values over the conventional technique? Data sources: A search forstudies was carried out in eight databases until August 2021. Out of the 447 publications found, 11 were included. Results: In human studies, osseodensification technique showed better results for implant stability quotient values with a summarized median difference of 8.57. As for secondary stability, there was no significant difference, with summarized median difference of 4.49 in favor of the osseodensification technique. In animal studies, all results were favorable to the osseo-densification technique. Regarding insertion torque, bone-to-implant contact, and bone area fraction occupancy between counterclockwise osseodensification technique vs conventional, the mean difference was 46.79 for insertion torque, 2.17 for bone-to-implant contact, and 2.11 for bone area fraction occupancy. High heterogeneity was observed between the studies. The risk of bias in humans was moderate in three studies and low in one; and in animal studies, four presented moderate risk, two low risk, and one high risk. The certainty of evidence ranged from low to moderate. Conclusion: The osseodensification technique showed improvement concerning the resonance frequency and the insertion torque value of implants in human studies. In addition, it increased the values of bone-to-implant contact, bone area fraction occupancy, and implantstability quotient in animal studies, when compared to the conventional technique. Objective: This study aimed to compare the alveolar sealing performance between free gingival graft (FGG) and porcine collagen membrane (MS) and qualitatively assess patient-centered outcomes via a visual analog scale. Method and materials: Eighteen patients were randomly divided into control (FGG) and test (MS) groups. After extraction, all the alveoli were filled with bovine bone grafts (small granules) and sealed. Follow-up was during the immediate postoperative period and after 3, 7, 15, 30, 60, 90, and 120 days. After 180 days, before implant placement, tissue samples were obtained for histologic analysis. The epithelial tissues were morphometrically measured for each sample. Qualitative information on the patient's perception of the treatment was collected after 7 days. Results: A faster healing was observed for the MS group. After 60 days, all the sites from the MS were partially healed, in contrast with only five from the FGG. The histologic results after 120 days showed for the FGG group a predominant acute inflammatory process, whereas chronic processes were observed for the MSgroup. The mean epithelial heights found for the FGG and MS were 535.69 pm and 495.33 pm, respectively (P = .54). The intragroup analysis showed significant variance among the data (P<.001) for both groups. The qualitative result showed statistically more significative comfort for the MS group (P < .05). Conclusion: Within the limitations of the study, both techniques effectively promote alveolar sealing. However, the visual analog scale result was superior and more signiffcant for the MS group, with faster wound healing and lower discomfort. [ABSTRACT FROM AUTHOR]

Background: The aim of this systematic review was two‐fold: (i) to evaluate the long‐term (≥5 years) stability of the gingival margin position, keratinized tissue width (KTW) and gingival thickness (GT) in sites that underwent root coverage (RC) or gingival augmentation (GA); and (ii) to assess the influence of different local variables on the long‐term stability of dental and gingival tissues. Materials and Methods: Randomized controlled trials (RCTs) and non‐RCTs reporting short‐term (i.e., 6–12 months after baseline surgical intervention) and long‐term (≥5 years) follow‐up data after surgical treatment of adult patients presenting single or multiple mucogingival deformities, defined as sites presenting gingival recession defects (GRDs) and/or (KTW) deficiency (i.e., <2 mm), were considered eligible for inclusion. MEDLINE–PubMed, EMBASE and Cochrane Central Register of Controlled Trials databases were searched for articles published up to 15 May 2023. Mixed‐effects multiple linear regression was used to assess the association between KTW, type of surgical procedure and time (i.e., independent variables) on the stability of the gingival margin in sites that received RC or GA therapy. Results: Of the 2569 potentially eligible records, 41 (reporting 40 studies) met the eligibility criteria. Graphical estimates including data from all RC procedures found an upward trend in recession depth (RD) increase over time. Conversely, it was observed that in 63.63% of RC studies and in 59.32% of RC treatment arms KTW increased over time, particularly in sites treated with subepithelial connective tissue grafts (SCTGs). Conversely, sites that underwent GA procedures generally exhibited an overall reduction of KTW over time. However, sites treated with free gingival grafts (FGGs) showed a decrease in RD after 10 years of follow‐up. Three main findings derived from the pooled estimates were identified: (i) Gingival margin stability was associated with the amount of KTW present during short‐term assessment (i.e. the greater the KTW at 6–12 months after treatment, the more stable the gingival margin). (ii) The use of autogenous soft‐tissue grafts was associated with lower RD increase over time. (iii) Treatment approaches that contribute to the three‐dimensional enhancement of the gingival phenotype, as clearly demonstrated by FGG, were associated with gingival margin stability. Conclusions: The extent of apical migration of the gingival margin appears to be directly related to the amount of KTW and GT upon tissue maturation. Interventions involving the use of autogenous grafts, either SCTG or FGG, are associated with greater short‐term KTW gain and lower RD increase over time. [ABSTRACT FROM AUTHOR]

Objectives: The goal of this study was to assess the newly formed bone and the remnant biomaterial by comparing four different bone grafts used to treat critical‐size defects, associated or not with the non‐resorbable membrane. Materials and Methods: Two calvaria critical‐size bone defects were created in 50 male Wistar rats. They were divided into blood (G1), autogenous (G2), bioglass (G3), hydroxyapatite (G4), and xenograft (G5) groups, associated or not with e‐PTFE. The experimental periods were 15 and 45 days. Sections were prepared for histomorphometric assessment. All data were analyzed by the mixed‐effects model with multiple comparisons (significance level, p <.05). Results: A similar level of new bone was observed for all groups, associated with a high level of vascularization. G1 and G2 ensured sovereignty over the greater quantity of new bone. A non‐significant result was reported comparing groups with and without membranes. No significant result was found between the experimental synthetic biomaterials (G3 and G4). G5L achieved 22.0% of new bone after 45 days (p >.05). All groups had a stable volume of biomaterial kept in the short term (p >.05). G2 was the best material for new bone formation and final volume of biomaterial, followed by G4 < G5 < G3. Thus, it is possible that G4 had a better degradation profile among the experimental groups. Conclusions: The best results were found in the autogenous group, with higher resorption and integration; non‐significative new bone was found among the experimental groups; and the regeneration of critical bone defects using an e‐PTFE barrier did not present significant results on new bone formation. Foram selecionados 30 defeitos com 2 ou 3 paredes ósseas em caninos e pré molares inferiores em 10 pacientes. No exame inicial e 8 meses após durante a reentrada cirúrgica, foram obtidas imagens digitalizadas e apurados os parâmetros Nível Clínico de Inserção (NCI), Nível da Margem Gengival (NMG),. Profundidade de Sondagem (PS), Nível do Defeito Ósseo (NDO) e Nível da Crista Óssea Alveolar (NCOA). Foram realizados retalhos mucoperiosteais para a instrumentação periodontal e os defeitos aleatoriamente divididos em sítios teste tratados com barreira e controle. As imagens digitalizadas foram interpretadas por subtração radiográfica para determinação da Densidade Óptica (DO) e Área de Preenchimento (AP). Os valores médios para todos os parâmetros foram testados pela análise de. variância e teste t de Student. Os resultados mostraram diferença estatística significativa (P < 0,01) entre os exames inicial e final para os parâmetros biométricos NCI, NMG, PS e NDO nos sítios teste e controle. Houve diferença estatística significativa (P < 0,01) para o parâmetro clínico NDO e radiográficos DO e AP entre os sítios teste e controle. Estes achados mostraram que ocorreu maior preenchimento ósseo nos sítios tratados seguindo os princípios da regeneração tecidual guiada Abstract: The aim of this clinical and radiographic study was to evaluate, in a split mouth design, the results of guided tissue regeneration using a biodegradable membrane comparing to conventional therapy in intra-bony defects. Thirty defects of 2 or 3 bony walls in mandibular canines and premolars were selected in 10 patients. Clinical Attachment Levei (CAL), Gingival Margin Levei (GML), Probing Oepth (PO), Sone Oefect Levei (SOL) and Alveolar Sone C resta I Levei (ASCL) were assessed at baseline and reentry procedures after 8 months. Mucoperiosteal flaps were raised to periodontal instrumentation and sites randomized in test treated with the barrier or control sites. Digital subtraction radiography were also taken to assess differences in Optical Oensity (00) and Sone Fill Area (SFA). ANOVA and Student t test were employed to test the differences between means values for ali parameters. The results showed statistically significant difference (P < 0.01) between baseline and reentry for CAL, GML, PO and SOL in both groups, and statistically significant (P < 0.01) greater reduction in SOL, greater gain 00 and greater SFA between test and control sites. It mentions the use of osseointegrated dental implants and the use of adhesive cementation of ceramic fragments.

O objetivo deste estudo foi comparar as características morfológicas, o potencial proliferativo e a produção protéica de fibroblastos do ligamento periodontal (FLP) e de fibroblastos gengivais (FG). Os fibroblastos foram cultivados pela técnica do explante a partir de fragmentos da gengiva e do ligamento periodontal de um mesmo indivíduo. As células foram isoladas e plaqueadas para análise por microscopia de contraste de fase e microscopia óptica. O índice de proliferação celular foi determinado por contagem automática de células e pelo ensaio de incorporação de bromodioxiuridina (BrdU). A produção de proteína total foi verificada por eletroforese em gel de poliacrilamida e o perfil enzimático por análise zimográfica. Os FLP são maiores e mais alongados que os FG em condições de subconfluência e confluência celular. Os FLP demonstraram um potencial proliferativo significantemente maior que os FG. Os perfis protéico e enzimático foram similares entre os FLP e FG. Os resultados demonstram que os FLP e FG são diferentes na morfologia e na capacidade proliferativa, porém são semelhantes na produção protéica. The objective of this study was to compare fibroblasts from the periodontal ligament (PLF) and gingival fibroblasts (GF) as to morphology, proliferation rate and protein synthesis. PLF and GF were explanted from tissues of the same patient. To characterize and compare the morphology of cells, PLF and GF were plated and analyzed under phase-contrast and optical microscopies. Proliferation rates were determined by means of automated counts carried out in days 1, 4, 7, 15 and 21, and also by means of the bromodeoxyuridine labelling index (BrdU). Total protein content was analyzed by means of electrophoresis in 10% polyacrylamide gel and zimography containing gelatin as substrate. PLF were bigger and more elongated than GF in subconfluence and confluence conditions. The proliferative rate of PLF was higher than that of GF at 1, 4, and 7 days (p < 0.05). At 15 and 21 days, there was no statistically significant difference as to the number of cells. PLF presented a significantly greater proliferative potential, in relation to GF (p < 0.05). The synthesis of protein in a period of 24 hours was similar for both PLF and GF. Our results demonstrated that PLF and GF are different as to morphology and proliferative capacity, however, they do not differ as to protein synthesis.

Background: The aim of this clinical and radiographic study was to evaluate the effect of guided tissue regeneration using a bioabsorbable barrier in the treatment of intrabony defects in humans.Methods: Intrabony osseous defects (2 or 3 walls) around mandibular canines and premolars were treated in 10 systemically healthy patients with ages ranging from 35 to 56 years. Prior to the surgical phase, patients were enrolled in a strict maintenance program including oral hygiene instructions and scaling and root planing (presurgical Pi and GI < 10%). Patients were seen for professional prophylaxis during the duration of the study. Clinical measurements were performed with an electronic probe at baseline and at reentry 8 months following surgical therapy. Measurements included clinical attachment levels (CAL), gingival margin levels (GML), probing depths (PD), bone defect levels (BDL), and alveolar crest level (ACL). A split mouth design was used. Quadrants were randomly assigned for treatment by GTR (experimental) or open flap debridement alone (control). Standardized radiographs were taken at baseline and at reentry. Digital images were analyzed by subtraction to assess changes in area (A) and optical density (OD). Data were evaluated using paired t test.Results: Statistically significant differences were found in both groups when comparing baseline and post-treatment values for CAL, GML, PD, and BDL (P < 0.0 1). Greater reductions in BDL and gain in A and OD were observed in the GTR group when compared to control (P < 0.01). Both therapies were effective in improving the clinical parameters assessed.Conclusion: Clinical and radiographic findings from this study demonstrated more bone fill in sites treated with GTR. Esse trabalho é um estudo prospectivo, paralelo e comparativo realizado em pacientes com doença periodontal crônica generalizada divididos em dois grupos, sendo 10 pacientes com diabetes melito insulino-dependente (DM) e 10 pacientes não diabéticos (NDM) acompanhados por 3 meses. Os dados obtidos nos períodos inicial e final foram divididos em parâmetros clínicos: índice de placa, Índice gengival, nível clínico de inserção, nível da margem gengival e profundidade de sondagem; parâmetros hematológicos: hemácias, hemoglobina, hematócrito, volume corpuscular médio, hemoglobina corpuscular média, concentração de hemoglobina corpuscular média, leucócitos, eosinófilos, metamielócitos, segmentados, linfócitos, monócitos, plaquetas, bastonetes, basófilos, glicemia, hemoglobina glicosilada (HbA1c), prova do laço, tempo de coagulação e sangramento e parâmetros microbiológicos, ¿pool¿ de bactéria por paciente, para os sítios com PS>5mm e regiões de bifurcação. As amostras foram obtidas com curetas tipo Gracey e analisadas através da técnica de reação de polimerase em cadeia ¿ PCR para verificação da freqüência de Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis e Tannarella forsythensis. As avaliações clinicas e hematológicas não demonstraram diferenças estatísticas significantes entre grupos NDM e DM nos períodos iniciais e final. A avaliação microbiológica demonstrou diminuição de Tannarella forsythensis estatisticamente significativa apenas para o grupo NDM (p=0,0313) após 3 meses. Este estudo sugere que os pacientes com DM apresentaram respostas clinica e laboratorial similares ao grupo NDM após a terapia de raspagem e alisamento radicular realizados em única sessão após o período de acompanhamento de 3 meses Abstract: The aim of this study was to compare the clinical, hematological and microbiological responses of periodontal treatment for both groups after full-mouth scaling and root planning. This study is a prospective, parallel, comparative, clinical, hematological and microbiological study performed in patients with generalized chronic periodontal disease separated in two groups, with 10 patients with diabetes mellitus insulin-dependent (DM) and 10 patients non-diabetics (NDM) attended by 3 months. The data from baseline and 3 months were separated in clinical parameters, plaque Index, gingival index, clinical attachment level, gingival margin level and probing depth; hematological parameters, red blood cell count, hemoglobin, hematocrit, mean corpuscular volume, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, leucocytes, eosinophils, metamielocytes, segmenteds, linfocytes, monocytes, platelets, bastonets, basophil, glucose, HbA1c, bleeding time and coagulation time and microbiological parameters, pool of bacterial per patients that were obtained from sites with PD>5mm and furcation sites. The samples were obtained with Gracey curettes and analyzed by technical of polymerase chain reaction ¿ PCR, to verify the frequency of Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis e Tannarella forsythensis. The clinical and hematological evaluations had no statistics significance changes between groups NDM and DM. The microbiological evaluation showed decrease of Tannarella forsythensis statistically significant for only NDM group (p=0,0313). Neste estudo foram avaliados a morfologia, o tamanho e a composição química das seguintes partículas: osso bovino orgânico cortical e esponjoso com partículas microgranular medindo entre 0,25 e 1,0 mm e macrogranular medindo entre 1,0 e 2,0 mm, osso bovino cortical inorgânico com partículas medindo entre 0,25 e 1,0 mm, hidroxiapatita com partículas medindo entre 0,75 e 1,0 mm e osso humano descalcificado, congelado e seco medindo entre 0,25 a 0,5 mm. Para a analise da morfologia e do tamanho das partículas, as amostras foram preparadas em portaespécime, metalizadas em ouro e analisadas a vácuo em microscópio eletrônico de varredura (SEM). Para a análise da composição química, as partículas não foram metalizadas e foram analisadas por microanálise de raio X, por espectroscopia por dispersão de energia (EDS). A análise visual realizada por SEM, demonstrou que as partículas de osso bovino orgânico e inorgânico, osso humano e hidroxiapatita apresentaram formato irregular e tamanho variável, maior do que o mencionado no rótulo pelo fabricante. A análise da composição química realizada por microanálise (EDS) detectou a presença de elementos como: sódio, cálcio e fósforo, que são comuns à composição do tecido ósseo, porém revelaram a presença de elementos químicos nas partículas de osso bovino orgânico. Esses resultados sugerem que o osso bovino orgânico não é composto puramente por colágeno e proteína Abstract: This article evaluates the morphologic and chemical composition of the following bone substitutes: cancellous and cortical organic bovine bone with micro and macro particles size ranging from 0,25 to 1,0 mm and 1,0 to 2,0 mm respectively, inorganic bovine bone with particles size ranging from 0,25 to 1,0 mm, hydroxyapatite with particles size ranging from 0,75 to 1,0 mm and demineralized freeze-dried bone allograft with particles size raging from 0,25 to 0,5 mm. The samples were coated with gold in an ion coater, the morphology was observed and the measurements of particle size were performed on vacuum condition by scanning electron microscopy (SEM). The chemical composition was evaluated by microanalysis EDS using samples without covering. The SEM analyses provided visual evidence that all materials analyzed have irregular shape and the particle size values were larger than the values mentioned by the manufacturer. The microanalysis EDS detected the presence of sodium, calcium and phosphorus that are common elements of the bone tissue. However, mineral elements were detected in all analyzed particles of organic bovine bone excepting for macro cancellous organic bovine bone. Orientador: Marcio Zaffalon Casati Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: O objetivo desse estudo prospectivo foi avaliar os parâmetros clínicos e radiográficos de implantes dentários instalados em pacientes parcialmente edêntulos, com indicação de reabilitação protética unitária implanto suportada, que possuíam histórico de periodontite agressiva generalizada (PAG), periodontite crônica generalizada (PCG) e pacientes sem histórico de periodontite (PPS). Os implantes foram divididos em 3 grupos: Grupo PAG (n= 19); Grupo PCG (n=30) e Grupo PPS (n=21) e foram acompanhados por um período de 60 meses. Foram instalados implantes unitários de estágio único, reabilitados após 3 meses com próteses aparafusadas. Os parâmetros clínicos periodontais e peri-implantares de índice de placa, sangramento a sondagem, profundidade de sondagem (PS), profundidade de sondagem peri-implantar (PSP), nível de inserção clínica relativo (NICR), nível de inserção clínica relativo peri-implantar (NICRP), posição da margem gengival relativa (PMGR) e posição relativa da margem da mucosa peri-implantar (PRMMP) foram avaliados 7 dias após a instalação das próteses e após 6, 12, 24, 36, 48 e 60 meses. Para a análise radiográfica foi realizada a mensuração linear da distância entre a plataforma do implante e o primeiro ponto de contato entre o osso e o implante nas faces mesial e distal na instalação das próteses (baseline) e nos intervalos de 6, 12, 24, 36, 48 e 60 meses após. O resumo poderá ser visualizado no texto completo da tese digital Abstract: The objective of this prospective study was to evaluate the clinical and radiographic parameters of dental implants installed in partially edentulous patients, with indication for implant supported unit prosthetic rehabilitation, who had a history of generalized aggressive periodontitis (GAP), generalized chronic periodontitis (GCP) and patients without history of periodontitis (PWP). The implants were divided into 3 groups: GAP Group (n = 19); GCP Group (n = 30) and PWP Group (n = 21) and were followed for a period of 60 months. Single-stage single implants were installed, rehabilitated after 3 months with screwed prostheses. Orientador: Wilkens Aurelio Buarque e Silva Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: O objetivo neste trabalho foi avaliar, por meio da técnica da extensometria elétrica, as deformações ocorridas nos análogos de implantes e nas infraestruturas metálicas de próteses parciais fixas, que foram posicionados na região dos quatro incisivos superiores com variação na disposição e na configuração protética. Foram confeccionadas duas infraestruturas: corpo-de-prova I - infraestrutura metálica fixada sobre os análogos de implantes na região dos incisivos laterais, e a região dos incisivos centrais correspondentes ao pôntico, formando uma estrutura fixa de 4 elementos; corpo-de-prova II - infraestrutura metálica fixada sobre os análogos de implantes na região dos incisivos centrais, e a região dos incisivos laterais correspondentes aos cantilevers, formando uma estrutura fixa de 4 elementos. Os extensômetros elétricos foram posicionados nas regiões cérvico-vestibular e cérvico-palatina dos análogos. Os corpos-de-prova foram submetidos à carga progressiva até 150N em angulação de 45 graus na superfície palatina na região de contato entre os dois incisivos centrais, simulando oclusão funcional aleatória. Como resultado, foi observado que todos os extensômetros registraram deformação com características de compressão ou tração. Nota-se ainda que houve diferenças entre os resultados nos análogos dos implantes e estrutura, sendo que nos análogos observamos diferenças estatísticas, entre as faces vestibulares e palatinas e em relação à posição da fixação estar na região central ou lateral. Nas estruturas não houve diferenças estatísticas significantes. Podemos concluir que todos os extensômetros registraram deformação, e que os corpos de prova que receberam forças em 45 graus entre os incisivos centrais, os implantes receberam maiores transmissões de força do que a estrutura, e que o posicionamento da fixação na região dos incisivos centrais, aumentou a tensão nos implantes dessa região, com tendência de que o posicionamento da fixação na região dos incisivos laterais leva a uma diminuição da tensão nos implantes, mas aumenta na estrutura Abstract: The objective of this study was to evaluate, through the electrical strain gauge technology, deformations occurring in the implant analogs and the metallic infrastructure of fixed partial dentures, fixed on these implants, which were positioned in the area of ??the four maxillary incisors with variation in layout and prosthetic configuration. Were prepared for this purpose two infrastructures: test body I - metal infrastructure fixed on the implant analogues in the region of the lateral incisor, and central incisor region corresponding to the pontic forming a fixed structure 4 elements; test body II - metal infrastructure fixed on the implant analogues in the region of the central incisor and the region corresponding to the lateral incisors cantleveres, forming a fixed structure of 4 elements. The strain gauges were placed on cervical-vestibular and cervical cleft of similar regions. The specimens were subjected to progressive load of 150N at an angle of 45 ° in the palatal surface in the contact area between the two central incisors, simulating a random functional occlusion. As a result, it was observed that all gauges recorded deformation with compression characteristics and traction. Note also that there were differences between the results in implants and structure, and the implants observe statistical differences between the buccal surfaces and palatine and the place position is in the center or in the lateral region. The structures there were no significant statistical differences. We conclude that FPDs receiving forces in 45 degrees between the central incisors, implants suffered greater force transmissions of the structure, and the positioning of the fixation in the region of the central incisors, leads to an increase of stress in the implants that region. Orientador: Frederico Andrade e Silva Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: O objetivo deste estudo foi comparar o nível de reabsorção óssea em implantes com superfícies física e quimicamente tratadas, carregados com coroas unitárias, inseridos em osso previamente enxertado. Foram avaliadas as imagens digitalizadas das radiografias periapicais de 20 implantes, sendo 10 com superfícies SLA® e 10 com superfícies SLActive® (Instituto Straumann®, Basel, Suíça). Coroas metalocerâmicas foram instaladas doze semanas após a colocação dos implantes no primeiro grupo (SLA) e seis semanas no segundo (SLActive). Radiografias periapicais foram realizadas imediatamente após a colocação das coroas (T0), e aos 3 (T1), 6 (T2) e 12 (T3) meses. Após a digitalização das radiografias, foi medido o Nível Médio de Reabsorção Óssea (NMRO) nos quatro períodos experimentais. Os resultados foram submetidos ao teste ANOVA e Dunnett revelando que o NMRO no grupo SLA variou entre 0,08mm±0,64 (T0) a 0,44mm±0,66 (T3). No grupo SLActive foi de -0,05mm±0,60 (T0) a -0,05mm± 0,41 (T3). 38. Preemptive vs preventive analgesia with lumiracoxib in impacted lower third molar surgery

Orientador: Eduardo Dias de Andrade Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: A analgesia preemptiva consiste na instituição de um regime analgésico previamente ao estímulo nociceptivo, com o objetivo de prevenir a hiperalgesia e a subsequente amplificação da dor. O objetivo deste trabalho foi avaliar a analgesia preemptiva com o lumiracoxibe, inibidor seletivo da cicloxigenase-2 (COX-2), em exodontias bilaterais de terceiros molares mandibulares retidos, comparando-a à analgesia preventiva. Foram selecionados 16 voluntários com bom estado de saúde geral, com indicação para a remoção dos dois terceiros molares mandibulares retidos, em posições simétricas e assintomáticos. O estudo foi delineado de forma duplo-cego e cruzado. Em uma das cirurgias os voluntários foram tratados com lumiracoxibe 400 mg, via oral, em dose única, administrada uma hora antes da cirurgia, seguido da administração de placebo, logo após o término do procedimento. Na cirurgia contralateral, de modo inverso, o tratamento consistiu do uso do placebo antes da intervenção, seguido da administração de lumiracoxibe 400 mg após o término do ato cirúrgico. A eficácia dos regimes analgésicos foi avaliada pelos seguintes parâmetros: tempo decorrido para a tomada do primeiro comprimido de analgésico a partir do final da intervenção; quantidade de comprimidos de analgésico tomada nas primeiras 24 h do pósoperatório; escala visual analógica nos tempos de 4, 8, 12 e 24 h e qualidade do tratamento. Não houve diferenças estatisticamente significantes entre os tratamentos preemptivo e preventivo no que se refere ao tempo requerido para a tomada da primeira dose de analgésico: 241,5 (±322,5) e 137,2 (± 246,48) minutos (p=0,3633); quantidade de comprimidos analgésicos tomados no período pósoperatório: 2,6 (±2,56) e 4,2 (± 5,13) comprimidos (p=0,5471), qualidade do tratamento (p=0,6794) e escala visual analógica (p=0,3916). Concluiu-se que o tratamento com lumiracoxibe, prévio ao estímulo nociceptivo (analgesia preemptiva) ou posterior a ele (analgesia preventiva), promove efeito analgésico efetivo e similar em cirurgias de terceiros molares mandibulares retidos. Abstract: Preemptive analgesia is defined as an analgesic regimen established before the occurrence of noxious stimulus in order to prevent hyperalgesia and subsequent amplification of pain. The aim of this study was to evaluate the preemptive analgesia with lumiracoxib, a selective cicloxigenase-2 (COX-2) inhibitor, in impacted third molars surgery, compared to the protocol of preventive analgesia. Sixteen subjects who required elective surgical removal of symmetric and asymptomatic impacted mandibular third molars, in an outpatient setting were selected for the study, characterized as double-blind and crossover. In one of the surgeries, the volunteers where treated with lumiracoxib 400 mg orally in a single dose given one hour before surgery, followed by administration of placebo, immediately after the end of the procedure. Conversely, in contralateral surgery, treatment consisted of using placebo before intervention, followed by administration of lumiracoxib 400 mg after surgery. The effectiveness of analgesia was evaluated by the following parameters: time elapsed from the end of the intervention, until the first analgesic intake in the postoperative period; number of analgesic tablets consumed in the first 24 hours after surgery; visual analog scale (VAS) at of 4, 8, 12 and 24 h after the surgery and treatment quality. There were o statistically significant differences between preemptive and preventive treatments in relation to the time required for the first analgesic intake: 241.5 (± 322.5) and 137.2 (± 246.48) minutes (p = 0.3633), number of analgesic tablets taken in the postoperative period: 2.6 (± 2.56) and 4.2 (± 5.13) tablets (p = 0.5471), quality of treatment (p = 0.6794) and visual analog scale (p = 0.3916). It was concluded that treatment with lumiracoxib, prior to noxious stimulation (preemptive analgesia) or after it (preventive analgesia), promotes similar analgesic effect in lower impacted third molars surgery. Objective: The aim of this study was to evaluate the effect of ozone therapy on new bone formation and inflammation modulation in defects of rat calvaria filled with autogenous bone., Material and Methods: Critical size defects were created in the calvaria of 24 male Wistar rats. The animals were randomly divided into four groups according to the treatment: G1: clot; G2: clot and covered with xenogenic membrane; G3: particulate autogenous bone graft; G4: autogenous bone graft and application of 3 mL O
2 /O3 gas mixture (10 µg/ml). The defects were filled immediately after surgery with a bilateral retroauricular application, in the region immediately above the incision. After 21 days, the animals were euthanized, and the samples were processed for morphometric evaluations designed to measure both the intensity of the inflammatory infiltrate, and the presence of new bone formation in the defect., Results: The results showed a lower inflammation score and higher mean of newly formed bone in the region of the defect for the group associated with ozone therapy (G4). - Abstract
Aesthetic concerns and functional abnormalities, such as dentin hypersensitivity, are often associated with gingival recession defects. Root coverage procedures aim to restore both gingival aesthetics and function in recession defects. The coronally positioned flap combined with the subepithelial connective tissue graft is one of the most widely used root coverage procedures. The present report illustrates four different indications where this procedure has been successfully employed. An isolated Miller class II recession defect associated with frenum pull, multiple adjacent Miller class I defects in the aesthetic zone, an isolated Miller class I defect associated with dentin hypersensitivity, and an isolated Miller class II defect on a retained deciduous tooth are the four diverse conditions treated by periodontal plastic surgery. Different approaches were used to create the coronally positioned flap. Treatment resulted in complete root coverage, resolution of hypersensitivity, and satisfaction of the patients' aesthetic concerns. An effective and predictable treatment modality, such as the coronally positioned flap combined with the subepithelial connective tissue graft, should be considered when treatment planning for gingival recession defects.

41. [Characteristics of implant surface and implant-abutment gap in two- and one-stage systems].

- Abstract
The aim of this study was to evaluate under scanning electron microscopy the characteristics of the implant surface and the gap between the components of two- and one-stage systems. Three two-stage implants coated with RBM and three one-stage implants coated with TPS were selected. In the two-stage implants, the esthetic-cone abutments were adapted and screw tightened with 20 N/cm . In the one stage implants, solid abutments were adapted and torque tightened mechanical frictional with 30 N/cm. The specimens were mounted on stubs and analyzed under scanning electron microscopy. The gap in each implant was measured in four different points and repeated three times. The paired Student-t test was applied to detect the difference in the gap extension. The results showed that no significant differences (P > 0.05) were found regarding the gap extension between two- and one-stage systems and that the treatments produced different surface roughness.

- Abstract
The objective of this study was to compare fibroblasts from the periodontal ligament (PLF) and gingival fibroblasts (GF) as to morphology, proliferation rate and protein synthesis. PLF and GF were explanted from tissues of the same patient. To characterize and compare the morphology of cells, PLF and GF were plated and analyzed under phase-contrast and optical microscopies. Proliferation rates were determined by means of automated counts carried out in days 1, 4, 7, 15 and 21, and also by means of the bromodeoxyuridine labelling index (BrdU). Total protein content was analyzed by means of electrophoresis in 10% polyacrylamide gel and zimography containing gelatin as substrate. PLF were bigger and more elongated than GF in subconfluence and confluence conditions. The proliferative rate of PLF was higher than that of GF at 1, 4, and 7 days (p < 0.05). At 15 and 21 days, there was no statistically significant difference as to the number of cells. PLF presented a significantly greater proliferative potential, in relation to GF (p < 0.05). The synthesis of protein in a period of 24 hours was similar for both PLF and GF. Our results demonstrated that PLF and GF are different as to morphology and proliferative capacity, however, they do not differ as to protein synthesis.
