Abstract

Respiratory diseases and cardiovascular diseases (CVDs) have high prevalence and share common risk factors. In some respiratory diseases such as sleep apnoea and COPD, the evidence of their negative impact on the prognosis of CVDs seems clear. However, in other diseases it is less evident whether there is any direct relationship. With this in mind, our objective was to provide information that may be helpful to better understand the relationship between respiratory pathology and CVDs.There are different reasons for this relationship, such as shared risk factors, common pathophysiological mechanisms, side effects of treatment and the direct effect in the heart and great vessels of respiratory diseases.Indeed, aging and smoking are risk factors for CVDs and also for respiratory diseases such as obstructive sleep apnea (OSA), COPD and interstitial lung diseases (ILD).Furthermore, there are common pathophysiological mechanisms that affect both respiratory diseases and CVDs, such as accelerated atherosclerosis, microvascular dysfunction, endothelial dysfunction, inflammation, hypoxemia and oxidative stress.Besides that, it is well known that lung cancer, sarcoidosis and amyloidosis may directly affect the heart and great vessels.Finally, side effects of drugs for respiratory diseases and the discontinuation of treatments that are necessary for CVDs, such as β-blockers and aspirin, may have a deleterious impact on the cardiovascular system.In conclusion, the coexistence of respiratory diseases and CVDs is very common. It makes modifying diagnostic and therapeutic management necessary and is also a relevant prognostic factor. Resumen: Las enfermedades respiratorias y cardiovasculares (ECVs) tienen una alta prevalencia y comparten factores de riesgo comunes. En algunas enfermedades respiratorias como la apnea del sueño y la EPOC, la evidencia de su impacto negativo en el pronóstico de las ECVs parece clara. Sin embargo, en otras enfermedades es menos evidente si existe alguna relación directa. Considerando esto, nuestro objetivo fue aportar información que pueda ser útil para comprender mejor la relación entre la patología respiratoria y las ECVs.Existen diferentes razones para esta relación, como factores de riesgo compartidos, mecanismos fisiopatológicos comunes, efectos secundarios del tratamiento y el efecto directo en el corazón y los grandes vasos de las enfermedades respiratorias.De hecho, el envejecimiento y el tabaquismo son factores de riesgo para las ECVs y también para las enfermedades respiratorias como la apnea obstructiva del sueño (AOS), la EPOC y las enfermedades pulmonares intersticiales (EPID).Además, existen mecanismos fisiopatológicos comunes que afectan tanto a las enfermedades respiratorias como a las ECVs, como la aterosclerosis acelerada, la disfunción microvascular, la disfunción endotelial, la inflamación, la hipoxemia y el estrés oxidativo. Además, es bien conocido que el cáncer de pulmón, la sarcoidosis y la amiloidosis pueden afectar directamente al corazón y a los grandes vasos.Finalmente, los efectos secundarios de los fármacos para enfermedades respiratorias y la suspensión de tratamientos necesarios para las ECVs, como los β-bloqueantes y la aspirina, pueden tener un impacto deletéreo sobre el sistema cardiovascular.En conclusión, la coexistencia de enfermedades respiratorias y ECVs es muy frecuente, lo que obliga a modificar el manejo diagnóstico y terapéutico y es, además, un factor pronóstico relevante.

Abstract

ABSTRACT The use of valganciclovir as secondary prophylaxis for cytomegalovirus (CMV) infection in solid organ transplantation (SOT) is the most prevalent therapeutic choice, but it has been dismissed by the presence of resistance and toxicity. Letermovir (LMV) is indicated as primary prophylaxis in hematopoietic stem cell transplantation, but there is scarce clinical data on its use as secondary prophylaxis in SOT. We present two cases of SOT recipients (lung, liver) who underwent LMV secondary prophylaxis. One patient developed the L595S (UL97)-resistant mutation to ganciclovir/valganciclovir and experienced several CMV relapses under LMV therapy, but no LMV resistance mutations were detected. The second patient developed the C325F (UL56)-resistant mutation to LMV under secondary prophylaxis which was rescued by foscarnet. This observation reviews previously published clinical data on LMV secondary prophylaxis and recommends a cautious use of LMV in the clinical practice due to the early development of UL56 resistance mutations. IMPORTANCE This observation provides comprehensive data on the clinical correlates of both cytomegalovirus (CMV) genotypic follow-up and clinical monitoring and outcomes for two different solid organ transplantation recipients that received letermovir as secondary prophylaxis. Our study emphasizes that monitoring of CMV disease in the patient and early genotypic detection of resistance mutations are essential when using new antiviral drugs for off-label indication in patients experiencing CMV relapses or not responding to standard antiviral therapy. These cases and the bibliography reviewed can be helpful for other researchers and clinicians working in the field to optimize the use of new treatments for transplant recipients since drug-resistant CMV infection is an important emerging problem even with new developments in antiviral treatment.

Abstract

Abstract This article presents the evolution of daily sunshine duration (SD) records at the San Fernando Observatory, a coastal place located in southern Iberian Peninsula, during the period 1881–1890. This period is of great interest because in August 1883, the Krakatoa volcano erupted injecting into the stratosphere large amount of gases and solid aerosol particles. The analysis of these daily SD records shows a considerable reduction in the maximum values of each month from the year 1883, obtaining the minimum values in 1884. In addition, the annual SD values in San Fernando fell around 400–500 hr/year during several years after the eruption. This strong reduction in SD records may be mainly associated with the scattering effects of the injected particles to the atmosphere which remains there for a long time. It must be noted that the signal of the Krakatoa eruption over the SD and, therefore, over the incoming solar radiation is extremely complicated to register due to the limited number of sites with records extending back into the 19th century worldwide. To our knowledge, the results shown in this work are the first evidence of the 1883 Krakatoa eruption effects over SD records.

Abstract

Introduction Prolonged use of antivirals to prevent the development of cytomegalovirus (CMV) disease in lung transplant patients has been shown to have significant side effects, for which alternatives are being sought to reduce their use. The monitoring of cell immunity against CMV could be an alternative as it has shown to be useful in identifying transplant patients at low risk of infection, who could benefit from shorter prophylaxis. The aim of the CYTOCOR study is to demonstrate that the combination of a reduced prophylaxis strategy with subsequent CMV-specific immunological monitoring would allow CMV infection to be controlled in lung transplant patients as effectively as the usual strategy (prophylaxis followed by pre-emptive therapy), while reducing the side effects of antivirals due to the shorter duration of prophylaxis.Methods and analysis Phase III randomised, open, multicentre, parallel, non-inferiority clinical trial to study the efficacy and safety of the combination of a prophylaxis strategy up to month +3 post-transplant followed by immuno-guided prophylaxis using the QuantiFERON-CMV technique up to month +12 post-transplant to prevent CMV disease in CMV-seropositive lung transplant recipients. This strategy will be compared with a combination of a usual prophylaxis strategy up to month +6 post-transplant followed by pre-emptive therapy up to month +12. To study the incidence of CMV disease, patients will be followed up to 18 months post-transplantation. A total of 150 patients are expected to be recruited for the study.Ethics and public dissemination The clinical trial has been approved by the Research Ethics Committees and authorised by the Spanish Agency of Medicines and Medical Devices (AEMPS).If the hypothesis of this clinical trial is verified, the dissemination of the results could change clinical practice by increasing knowledge about the safety and efficacy of discontinuing valganciclovir prophylaxis in lung transplant recipients.Trial registration number NCT03699254.

Abstract

Early daily actinometric and cloud cover data from Cáceres (SW of Spain) were recovered for the period 1913–1920. The task of retrieving and digitising early actinometric data series turn out highly required due to the lack of solar radiation data throughout the first half of twentieth century. Firstly, the Arago actinometer and the procedure employed to record the actinometric data were described in detail. Additionally, in spite of being a short series, trends from recovered actinometric data were analysed. A negative trend of (−0.16 ± 0.03) °C/year was obtained for all-sky conditions, reaching a marked value of (−0.26 ± 0.04) °C/year when cloud-free days were selected in the analysis. Both trends were statistically significant at the 95% confidence level. The Katmai eruption in June 1912 likely caused anomalous low actinometric records in 1913 and 1914. These results suggest a decrease of solar radiation in Cáceres during the second decade of the twentieth century which is in accordance with the final stage of the long-term negative trend in solar radiation data observed at several European sites from late nineteenth century to the beginning of the twentieth century (‘early dimming’).

8. Heavy Rainfall and Landslide Event in January 1831 at the Pedregoso Mountains (Cabeza Del Buey, SW Spain)

Abstract

In this work, a landslide event that took place on January 1831 at the Pedregoso Mountains, Cabeza del Buey, SW Spain, is described. This landslide had not been documented to date and was only described in the local press. This event involved an estimated amount of dislodged material in the order of 104 m3. The amount of meteorological data is very scarce as the event occurred before the setting up of the national meteorological service in Spain. However, data from the relatively near location of SW Iberia suggest that the landslide was preceded by a prolonged period of unusually high precipitation totals and that this intense wet period is compatible with the large-scale atmospheric configuration in the winter of 1829–1830. In fact, the North Atlantic Oscillation (NAO) index for that winter achieved one of the most negative values observed in the bicentennial period spanning 1821 to 2019. This multidisciplinary work represents the first attempt to report and describe the main triggering mechanism for an historical landslide in the Extremadura region that is similar to other great historical landslides which have already been documented for other locations in Spain.

9. Pro-Pluvia Rogation Ceremonies in Extremadura (Spain): Are They a Good Proxy of Winter NAO?

Abstract

Rogation ceremonies are religious requests to God. Pro-pluvia rogations were celebrated during dry conditions to ask God for rain. In this work, we have recovered 37 pro-pluvia rogations from 14 documentary sources (e.g.,: ecclesiastical manuscripts, books, and different magazines and newspapers). All of the rogations were celebrated in Extremadura region (interior of southwest of Spain) during the period 1824−1931. Climate of Extremadura is strongly dominated by the North Atlantic Oscillation (NAO). Therefore, pro-pluvia rogations have been associated to the NAO index and the relationship between them has been analyzed. The most relevant results are found in the relationship between pro-pluvia rogations in month n and the positive values of the NAO index for months n-1 and n-2, being statistically significant at 95% confidence level. Thus, the results evidence that the rogation ceremonies of Extremadura are a good proxy for the NAO index.

10. Earliest meteorological readings in San Fernando (Cádiz, Spain)

Abstract

Cádiz and San Fernando are two nearby towns with a wealth of meteorological records due to their connection with the Spanish Royal Navy officers and enlightened merchants. Several previous works have already recovered a significant amount of meteorological records of interest in these localities. However, unnoticed previously more than 40,000 daily meteorological observations recorded at the Royal Observatory of the Spanish Navy (located in San Fernando) during the period 1799–1813 remained neither digitized nor studied. Here, we have carried out this important task describing the different steps undertaken to achieve it as well as the results obtained. The dataset is composed by different meteorological variables such as atmospheric pressure, air temperature, precipitation, or state of the sky. As a first step a quality control was carried out to find possible errors in the original data or in the digitization process. Moreover, the antique units were converted to modern units. Also, the metadata and an analysis of the data have been described. Finally, we study in detail the meteorological conditions in October 1805, during the Battle of Trafalgar and to check the possible local effects of the unknown volcanic eruption of 1809. 12. SunMap: A Solar Image Processing Software for Obtaining Synoptic Maps

Abstract

A new open-source software, called SunMap, has been developed to obtain synoptic maps in an easy and quick way from multiple full-disc solar images. Our objective is to provide a free and straightforward application for heliophysicists and geophysicists interested in generating solar synoptic maps. SunMap allows comparison of structures and patterns of solar activity over various periods. Thus, the short- and long-term evolution of solar regions of interest can be studied. To reach this goal, different solar images taken day by day are stored in a single map that uses a sequence of images and allows the positioning of each observable element on it. A simple comparison between a synoptic map generated by SunMap and another previously constructed map is presented to show the versatility of this new available software.

13. A Sunspot Catalog by Rafael Carrasco at the Madrid Astronomical Observatory for the Period 1931 – 1933

Abstract

A sunspot catalog was published by the Madrid Astronomical Observatory from sunspot observations made by Rafael Carrasco and his assistants for the period November 1931 – December 1933. We have digitized this catalog and carried out a quality control to detect inconsistencies. We present a machine-readable version of this sunspot catalog together with an erratum list. Moreover, we compared the Madrid sunspot number and area with other reference series. We found that sunspots in the northern hemisphere were predominant during this period, confirming previous studies. We have also analyzed the group morphological typologies registered in the catalog and show a butterfly diagram drawn with this new information. A comparison with the sunspot catalog by the Royal Greenwich Observatory shows that almost all the groups included in the Carrasco’s catalog are also present in the aforementioned catalog.

14. Rediscovering the observations of solar prominences from 1906 to 1957 recorded at the Madrid Astronomical Observatory

Abstract

The Madrid Astronomical Observatory implemented a solar observation program from 1876 to 1986. In addition to sunspots, the observers at this observatory recorded other solar features such as prominences. In this work, we have consulted the documentary sources of the Madrid Astronomical Observatory (the information is not digitally available), digitized the records of the observers on the annual number of prominences, and constructed a homogeneous series of the total and hemispheric annual number of prominences with heights of 25″ and more for the period 1906–1957. To evaluate the quality of the data and assess their potential, we have compared the Madrid prominence series with the number of prominences recorded by the Astronomical Observatory of the University of Coimbra and other time series such as the sunspot number index, solar radio flux at 10.7 cm, and sunspot areas. We have also analyzed the hemispheric prominence numbers and the asymmetry index. We obtained the strongest correlation between Madrid and Coimbra prominence series (r = 0.7), whereas the correlations between the Madrid prominence series and the other solar activity time series are similar (r ≈ 0.6). In addition, we found that the correlation coefficient between the Madrid prominence series and the sunspot number is lower than that from the Coimbra prominence series and the sunspot number. We suggest that these differences are a consequence of the way prominences were counted in the Madrid Astronomical Observatory.

16. Design of a Compact Camera Obscura

Abstract

The camera obscura is a well-known optical device in the form of a closed box with a hole in one of its walls through which light rays pass, forming an inverted image of the external objects on the opposite wall, as can be seen in Fig. 1(a). Despite the simplicity of its basic design, they have been widely used by scientists and artists. In particular, dark cameras have been used throughout history to measure Earth-Sun distance. To do this, ancient scientists used cameras of enormous dimensions to accurately measure the variations in the apparent diameter of the Sun that depend on the Earth-Sun distance. This is because the farther the light travels inside the camera, the larger the projected image will be [Fig. 1(b)]. In this work, the construction of a compact dark camera for educational purposes is presented so that, having reduced dimensions, it allows the images to be very large.

17. A Catalog of Faculae, Prominences, and Filaments for the Period 1929–1944 from the Astronomical Observatory of the University of Coimbra

The Astronomical Observatory of the University of Coimbra (Portugal) published a catalog with solar observations such as sunspots, faculae, prominences, and filaments for the period 1929–1944. In previous works, a machine-readable version on sunspot observations made in Coimbra was published. Here we extend that work and present a digital version of the facula, prominence, and filament observations made in that observatory. We have applied a quality control to the catalog, obtaining that the percentage of problematic or suspicious data found is lower than 1% of the total number of observations. In addition, we show an analysis of this catalog, as well as some comparisons between solar indices calculated from Coimbra data and those from other sources. Historical observations of faculae, prominences, and filaments are not as common as sunspot records, and in addition, few historical series of these solar features are available in digital version. For that reason, the catalog of solar observations published by the Coimbra Observatory is of enormous value. The recovery, publication, and availability of this catalog provide the scientific community with a valuable data set of solar characteristics that will help us to study in more detail the past solar magnetic field and long-term solar activity.

18. The record precipitation and flood event in Iberia in December 1876: description and synoptic analysis

Abstract

The first week of December 1876 was marked by extreme weather conditions that affected the south-western sector of the Iberian Peninsula, leading to an all-time record flow in two large international rivers. As a direct consequence, several Portuguese and Spanish towns and villages located in the banks of both rivers suffered serious flood damage on 7 December 1876. These unusual floods were amplified by the preceding particularly autumn wet months, with October 1876 presenting extremely high precipitation anomalies for all western Iberia stations. Two recently digitised stations in Portugal (Lisbon and Evora), present a peak value on 5 December 1876. Furthermore, the values of precipitation registered between 28 November and 7 December were so remarkable that, the episode of 1876 still corresponds to the maximum average daily precipitation values for temporal scales between 2 and 10 days. Using several different data sources, such as historical newspapers of that time, meteorological data recently digitised from several stations in Portugal and Spain and the recently available 20th Century Reanalysis, we provide a detailed analysis on the socio-economic impacts, precipitation values and the atmospheric circulation conditions associated with this event. The atmospheric circulation during these months was assessed at the monthly, daily and sub-daily scales. All months considered present an intense negative NAO index value, with November 1876 corresponding to the lowest NAO value on record since 1865. We have also computed a multivariable analysis of surface and upper air fields in order to provide some enlightening into the evolution of the synoptic conditions in the week prior to the floods. These events resulted from the continuous pouring of precipitation registered between 28 November and 7 December, due to the consecutive passage of Atlantic low-pressure systems fuelled by the presence of an atmospheric-river tropical moisture flow over central Atlantic Ocean.

20. An Early Assessment of the Forecast by the Solar Cycle 25 Prediction Panel

Abstract

The effects of solar activity on technology have increased the interest in solar forecasting both in the scientific community and in society. With two and a half years of data available, we are approximately halfway to the maximum of Solar Cycle 25. Several research groups around the world published their predictions for this cycle. In this work, we assess the forecast made by the Solar Cycle 25 Prediction Panel. Analyzing the sunspot number data available, we conclude that Solar Cycle 25 is one of the weakest cycles since mid-18th century and, however, it is stronger than the initial forecast of the highest range predicted by the Solar Cycle 25 Prediction Panel for the two first years and a half of its ascending phase.

Abstract

Long-term studies on hemispheric asymmetry can help to understand better the solar dynamo. We present the hemispheric sunspot number calculated from daily sunspot observations made at the Madrid Astronomical Observatory for the period 1935–1986 (corresponding approximately to Solar Cycles 17–21). From this data set, we also analyzed the asymmetry index and hemispheric phase shifts. We conclude that the northern hemisphere was predominant in Solar Cycles 17–20, whereas the southern hemisphere was predominant in Solar cycle 21. The strongest asymmetries are found in Solar Cycles 20 (with a relative difference between both hemispheres of 44%) and 19 (39%). A normalization of the Madrid hemispheric sunspot number was also made with respect to the sunspot number (Version 2). Our results agree with previous studies on hemispheric asymmetry around the mid-20th century and their secular trends.

Abstract

Highlights • The vitamin D endocrine system have a variety of actions on cells and tissues involved in COVID-19 progression. • Early calcifediol (25-hydroxyvitamin D) treatment to hospitalized COVID-19 patients significantly reduced intensive care unit admissions-Calcifediol seems to be able to reduce severity of the COVID-19. • Calcifediol seems to be able to reduce severity of the disease., Objective The vitamin D endocrine system may have a variety of actions on cells and tissues involved in COVID-19 progression especially by decreasing the Acute Respiratory Distress Syndrome. Calcifediol can rapidly increase serum 25OHD concentration. We therefore evaluated the effect of calcifediol treatment, on Intensive Care Unit Admission and Mortality rate among Spanish patients hospitalized for COVID-19. Design Parallel pilot randomized open label, double-masked clinical trial. Setting University hospital setting (Reina Sofia University Hospital, Córdoba Spain.) Participants 76 consecutive patients hospitalized with COVID-19 infection, clinical picture of acute respiratory infection, confirmed by a radiographic pattern of viral pneumonia and by a positive SARS-CoV-2 PCR with CURB65 severity scale (recommending hospital admission in case of total score > 1). Procedures All hospitalized patients received as best available therapy the same standard care, (per hospital protocol), of a combination of hydroxychloroquine (400 mg every 12 h on the first day, and 200 mg every 12 h for the following 5 days), azithromycin (500 mg orally for 5 days. Eligible patients were allocated at a 2 calcifediol:1 no calcifediol ratio through electronic randomization on the day of admission to take oral calcifediol (0.532 mg), or not. Patients in the calcifediol treatment group continued with oral calcifediol (0.266 mg) on day 3 and 7, and then weekly until discharge or ICU admission. Outcomes of effectiveness included rate of ICU admission and deaths. Results Of 50 patients treated with calcifediol, one required admission to the ICU (2%), while of 26 untreated patients, 13 required admission (50 %) p value X2 Fischer test p < 0.001. Univariate Risk Estimate Odds Ratio for ICU in patients with Calcifediol treatment versus without Calcifediol treatment: 0.02 (95 %CI 0.002−0.17). Multivariate Risk Estimate Odds Ratio for ICU in patients with Calcifediol treatment vs Without Calcifediol treatment ICU (adjusting by Hypertension and T2DM): 0.03 (95 %CI: 0.003-0.25). Of the patients treated with calcifediol, none died, and all were discharged, without complications. The 13 patients not treated with calcifediol, who were not admitted to the ICU, were discharged. Of the 13 patients admitted to the ICU, two died and the remaining 11 were discharged. Conclusion Our pilot study demonstrated that administration of a high dose of Calcifediol or 25-hydroxyvitamin D, a main metabolite of vitamin D endocrine system, significantly reduced the need for ICU treatment of patients requiring hospitalization due to proven COVID-19. Calcifediol seems to be able to reduce severity of the disease, but larger trials with groups properly matched will be required to show a definitive answer.

Abstract

Newly discovered descriptions about the great aurora observed in March 1582 are presented in this work. These records were made by Portuguese observers from Lisbon. Both records described the aurora like a great fire in the northern part of the sky. It was observed during three consecutive nights, according to one of the sources. Thus, we present a discussion of these auroral records in order to complement other works that studied the aurora sighted in March 1582.

Abstract

Objectives: Comparison of adherence to nebulized sodium colistimethate (Promixin) and agreement between the multidimensional FACED, E-FACED and BSI scales in a cohort of patients with non-cystic fibrosis BQ and chronic bronchial infection (CBI) by PsA. Patients and Methods: A multicenter, observational, longitudinal prospective study of a cohort of adults with non-CF BQ, all with PsA CBI and treated with Promixin. FACED, EFACED and BSI scales in the previous year, their modification after a year of treatment with the nebulized therapy and the concordance between them are collected. Results: 99 patients were started average age 63±14.7 years. In the year prior to treatment FEV1 was 55±19%, exacerbations 3.8±2.6 and 0.5±0.7 hospital admissions/year. At the end FEV1 was 59±22%, exacerbations/year 1.4±1.2 and hospital admissions/year 0.26±0.6. In the pretreatment assessment there is a low concordance between the EFACED and BSI scales (R=0,248; p Conclusions: In our cohort there is good adherence to nebulized treatment with Promixin® and good nebulizer management, regardless of the severity of BQ. These results have their clinical impact in terms of functional improvement, reduction of exacerbations and changes in the level of severity prior to treatment. Given the concordance between the prognostic scales, both could be applicable to assess sensitivity to change in a therapeutic intervention with nebulized antibiotics.

Abstract

Objective: To assess the clinical and economic impact, in terms of reduction of costs associated with exacerbations, in a cohort of patients with non-cystic fibrosis (non-CF) BQ and PsA CBI treated with Promixin. Patients and Methods: A multicenter, observational, longitudinal prospective study of a cohort of adults with non-CF BQ and PsA CBI treated with Promixin. Data were collected in the year before and after nebulized treatment (pulmonary function, exacerbations without hospital admission, mean per pat/y and total number of admissions and per pat/y). Results: 86 pat, 45 males, 62±15 y. Results are shown in the following data table. Conclusions: Adherence to nebulized treatment is excellent in our patients. In clinical terms, this situation has allowed an improvement in lung function and a significant reduction in the number and severity of exacerbations and hospital admissions. Regarding this issue, the average cost per patient and year of treatment has been reduced by more than €1500 after onset of nebulized therapy.

Resumen Esta revision pretende exponer de forma sucinta aquellas circunstancias clinicas previas al trasplante pulmonar que pueden repercutir negativamente en el pronostico del trasplante a corto y largo plazo. Se plantean los metodos de rastreo y diagnostico de comorbilidades comunes de impacto pronostico negativo sobre el trasplante, tanto de patologias pulmonares como extrapulmonares, y se proponen medidas dirigidas a su correccion. La coordinacion y el intercambio de informacion entre los centros que remiten a los candidatos y los centros trasplantadores permitiran detectar y corregir estas comorbilidades con el fin de minimizar los riesgos y de mejorar las expectativas de supervivencia de los pacientes trasplantados.

Abstract

The aim of this review is to give an overview of the clinical circumstances presenting before lung transplant that may have negative repercussions on the long and short-term prognosis of the transplant. Methods for screening and diagnosis of common comorbidities with negative impact on the prognosis of the transplant are proposed, both for pulmonary and extrapulmonary diseases, and measures aimed at correcting these factors are discussed. Coordination and information exchange between referral centers and transplant centers would allow these comorbidities to be detected and corrected, with the aim of minimizing the risks and improving the life expectancy of transplant receivers.

Resumen La linfangioleiomiomatosis (LAM) es una enfermedad rara caracterizada por la proliferacion anormal de celulas musculares lisas inmaduras y una destruccion quistica del pulmon, que condiciona el pronostico de la enfermedad. Los angiomiolipomas renales suelen ser muy frecuentes en esta enfermedad, generalmente de curso asintomatico, salvo complicaciones. Ante la ausencia de un tratamiento curativo, las ultimas publicaciones reflejan resultados esperanzadores en la terapia molecular para evitar el deterioro funcional y el control del tamano de los angiomiolipomas. Entre estas terapias destacan los inhibidores del complejo mTOR, sobre todo sirolimus. Presentamos un caso clinico de una paciente diagnosticada de LAM sometida a trasplante pulmonar con reduccion del tamano del angiomiolipoma renal tras el tratamiento con el inhibidor mTOR everolimus.

Abstract

a b s t r a c t Lymphangioleiomyomatosis (LAM) is a rare disease characterized by abnormal proliferation of immature smooth muscle cells and cystic lung destruction, which determines the prognosis of the disease. The kidney angiomyolipomas are usually very common in this disease and are usually asymptomatic unless complications arise. In the absence of a curative treatment, recent publications show promising results in molecular therapy to prevent functional decline and to control the size of the angiomyolipomas. These therapies include mTOR complex inhibitors, especially sirolimus. We report a case of a patient diagnosed with LAM who underwent lung transplantation with reduction of renal angiomyolipoma size after treatment with the mTOR inhibitor everolimus.

Abstract

The present work describes the development and analysis of a Spanish annoyance scale for use in community noise assessments. The use of different descriptors and intervals for different authors prevents comparing studies of assessment of noise reactions. It is not clear how many semantic distinctions can be made to describe subjective reactions to noise. There is also great variability in language use reflecting educational, social class, regional, and subcultural differences. Finally, scaling annoyance responses, when the considered intervals are not equally distributed, may seriously distort results when analyzed with parametric statistics. Using three different questions, almost 400 subjects were asked to rate 25 different Spanish descriptors of annoyance. Our attempt to produce a Spanish standardized annoyance scale with descriptors marking clear semantic distinctions, roughly equidistant from each other, and having wide acceptability is presented here. Taking into account that Spanish is the official language in 21 countries and it is spoken by more than 300 million people, one of our goals in this meeting is to discuss our method and try to generalize it in order to obtain a real, universal Spanish annoyance scale.

Recientemente, algunos autores han mostrado que algunas auroras pueden ser observadas a latitudes relativamente bajas cuando la actividad geomagnética es baja o moderada. Este tipo tan especial de aurora recibe el nombre de "aurora esporádica". Presentamos y analizamos en este trabajo un posible caso de "aurora esporádica" observada desde México el 19 de abril de 1843.
