To evaluate the microbial air quality during dental clinical procedures in a large clinical setting with increasing patient capacity.This was a single-center, observational study design evaluating the microbial air quality and aerosol distribution during normal clinical sessions at 5% (sessions 1 and 2) and at50% (session 3) treatment capacity of dental aerosol generating procedures. Sessions 1 and 2 were evaluated on the same day with a 30-minute fallow time between the sessions. Session 3 was evaluated on a separate day. For each session, passive air-sampling technique was performed for three collection periods: baseline, treatment, and post-treatment. Blood agar plates were collected and incubated at 37°C for 48 hours. Colonies were counted using an automatic colony counter. Mean colony forming units (CFU) per plate were converted to CFU/m²/h.Kruskal Wallis test was performed to compare the mean CFU/m²/h between the clinic sessions. Statistically significant differences were observed between sessions 1 and 2 (P0.05), but not between sessions 2 and 3 (P0.05). Combining all clinical sessions, the mean CFU/m²/h were 977 (baseline), 873 (treatment), and 1,631 (post-treatment) for the collection periods. A decrease-to-increase CFU/m²/h trend was observed from baseline to treatment, and from treatment to post-treatment that was observed for all clinic sessions and was irrespective to treatment capacity. Higher amounts of CFU/m²/h were found near the air exhaust outlets for all three clinic sessions. Microbial aerosol distribution is most likely due to the positions and power levels of the air inlets and outlets, and to a lesser extent with patient treatment capacity.Dental clinics should be designed and optimized to minimize the risk of airborne transmissions. The results of this study emphasize the need to evaluate dental clinic ventilation systems.

BACKGROUND: During the COVID-19 pandemic, American Society for Testing and Materials level 3 and level 2 medical face masks (MFMs) have been used for most health care workers and even for the first responders owing to a shortage of N95 respirators. However, the MFMs lack effective peripheral seal, leading to concerns about their adequacy to block aerosol exposure for proper protection. The purpose of this study was to evaluate the peripheral seal of level 3 and level 2 MFMs with a 3-dimensional (3D-) printed custom frame. METHODS: Level 3 and level 2 MFMs were tested on 10 participants with and without a 3D-printed custom frame; the efficiency of mask peripheral seal was determined by means of quantitative fit testing using a PortaCount Fit Tester based on ambient aerosol condensation nuclei counter protocol. RESULTS: The 3D-printed custom frame significantly improved the peripheral seal of both level 3 and level 2 MFMs compared with the masks alone (P < .001). In addition, both level 3 and level 2 MFMs with the 3D-printed custom frame met the quantitative fit testing standard specified for N95 respirators. PRACTICAL IMPLICATIONS: The 3D-printed custom frame over level 3 and level 2 MFMs can offer enhanced peripheral reduction of aerosols when using collapsible masks. With the shortage of N95 respirators, using the 3D-printed custom frame over a level 3 or level 2 MFM is considered a practical alternative to dental professionals.

To improve room temperature stability and oral bioavailability of mavatrep (JNJ-39439335, a transient receptor potential vanilloid subtype-1 antagonist), various formulations were initially developed and evaluated in 2 phase 1 open-label, randomized, 3-way crossover studies in healthy participants. Study 1 evaluated 2 new overencapsulated tablet formulations (formulations B and C) relative to an overencapsulated early tablet formulation (formulation A), using mavatrep HCl salt form. Because these tablets were still not room-temperature stable, in study 2: two free-base solid dispersion amorphous formulations (formulations D and E) were evaluated relative to the best encapsulated formulation from study 1 (formulation C) and also food effect. Both studies had screening (∼4 weeks), treatment (study 1: n = 18, 6-sequenced; formulations B and C [2 × 25 mg] versus A [2 × 25 mg]; study 2, part 1: n = 24, formulations D and E [2 × 12.5 mg] versus C [1 × 25 mg]; study 2, part 2: n = 16, best formulation from part 1 fed versus fasted, 2 × 12.5 mg) with a 21-day washout period and a follow-up. Mavatrep exhibited consistent pharmacokinetics across formulations. Following rapid absorption (median tmax , 1.5-6.5 hours), plasma concentrations declined multiexponentially (mean t1/2 , 67-104 hours). The new encapsulated tablet formulation (formulation C, capsule filler: poloxamer 407) was the best formulation (Cmax and AUC values 2-3-fold > than the other 2) from study 1. Background Uncooperative pediatric mask induction is linked to perioperative anxiety. Although some risk factors for uncooperative inductions have been reported, there are no large cohort studies that identify intrinsic patient characteristics associated with cooperation. Aim The primary aim was to identify patient characteristics associated with cooperative mask inductions. The secondary aim was to determine whether preoperative interventions were associated with increased cooperation. Methods This retrospective cohort study included patients 2-11 years old and ASA class I-IV who underwent mask induction. Our primary outcome of interest was cooperation with mask induction, which was correlated against the Induction Compliance Checklist. The variables analyzed for association with cooperation were age, sex, ASA class, class of surgery, preferred language, and race. Interventions examined for association with induction cooperation included premedication with midazolam, exposure to distraction technology, parental presence, and the presence of a Child Life Specialist. Multivariate mixed-effects logistic regression was used to assess the relationship between patient characteristics and cooperation. A separate multivariate mixed-effects logistic regression was used to examine the association between preoperative interventions and cooperation. Results 9692 patients underwent 23 474 procedures during the study period. 3372 patients undergoing 5980 procedures met inclusion criteria. The only patient characteristic associated with increased cooperation was age (OR 1.20, p-value 0.03). Involvement of Child Life Specialists was associated with increased cooperation (OR 4.44, p-value = 0.048) while parental/guardian presence was associated with decreased cooperation (OR 0.38, p-value = 0.002). Conclusion In this cohort, increasing age was the only patient characteristic found to be associated with increased cooperation with mask induction. Preoperative intervention by a Child Life Specialists was the sole intervention associated with improved cooperation.

This single-center, double-blind, placebo-controlled, sequential-group phase 1 study evaluated the safety, tolerability, and pharmacokinetics (PK) of mavatrep (JNJ-39439335), a transient receptor potential vanilloid 1 antagonist, in healthy Japanese and caucasian subjects. In part 1, a single-ascending-dose study, 50 subjects (25 each healthy Japanese and caucasians) were enrolled and received a single oral dose of 10, 25, or 50 mg mavatrep. Caucasian subjects were matched to Japanese subjects with respect to age (±5 years) and body mass index (±5 kg/m2 ). In part 2, a multiple-ascending-dose study, 36 Japanese subjects were enrolled and received once-daily oral doses of 10, 25, or 50 mg of mavatrep for 21 days. The single-dose PK of mavatrep and its metabolites was similar in the Japanese and caucasian subjects after adjustment of body weight. Following multiple dosing in Japanese subjects, a steady-state condition was reached in approximately 14 days. M2 and M3 are major circulating metabolites with mean exposure > 10% of mavatrep. Nonrenal clearance was the major route of elimination for mavatrep, M2, and M3. Mavatrep exhibited a long half-life, ranging from 68 to 101 and 82-130 hours for Japanese and caucasian subjects, respectively. After single and multiple dosing, mavatrep was well tolerated. The most common adverse events observed were thermohypoesthesia, feeling cold, chills, and feeling hot. Mavatrep and its metabolites exhibited similar PK profiles after single ascending doses in healthy Japanese and caucasian men.

Introduction The office-based endovascular facility has increased in number recently due in part to expedient patient experience. This study analyzed treatment outcomes of procedures performed in our office-based endovascular suite. Methods Treatment outcomes of 5134 consecutive procedures performed in our office-based endovascular suites from 2006 to 2013 were analyzed. Five sequential groups (group I–V) of 1000 consecutive interventions were compared with regard to technical success and treatment outcomes. Results Our patients included 2856 (56%) females and 2267 (44%) males. Procedures performed included diagnostic arteriogram, arterial interventions, venous interventions, dialysis access interventions, and venous catheter management, which were 1024 (19.9%), 1568 (30.6%), and 3073 (60.0%), 621(12.1%), and 354 (6.9%), respectively. The complication rates for group I, II, III, IV, and V were 3%, 1.5%, 1%, 1.1%, and 0.7%, respectively. The complication rate was higher in group I when compared to each of the remaining four groups ( p Conclusions Endovascular procedure can be performed safely in an office-based facility with excellent outcomes. Lessons learned in establishing office-based endovascular suites with efforts to reduce procedural complications and optimize quality patient care are discussed.

Predation is an important selective force that influences animal color patterns. Some larval populations of the streamside salamander, Ambystoma barbouri, inhabit streams with fish predators. Other larval salamanders are found in shallow, ephemeral streams that are predator-free. Quantitative melanophore cell counts and estimates of percent body area pigmented indicated that larval coloration is strongly correlated with stream type. Larvae that coexist with fish tend to be lighter than larvae from streams that are fishless and ephemeral. Two approaches dem- onstrated that lightly pigmented salamander larvae better match the common background in relatively permanent streams and are less conspicuous to fish than dark larvae. First, using a model based on the spectral sensitivity of the fish and reflectance properties of salamanders and natural stream backgrounds, we showed that light larvae are three times more cryptic than dark larvae on rocks. Second, lighter larvae had higher survival than darker salamanders on rocks in a predator- choice experiment. It is not clear why larvae in ephemeral streams are darker. Larvae in ephemeral streams should be active to feed and develop rapidly and reach sufficient size to metamorphose before seasonal drying. Several hypotheses may explain why larvae tend to be darker in ephemeral streams, such as increased thermoregulatory ability, better screening of ultraviolet radiation (in these shallower streams), or better background matching to terrestrial predators. Among populations where salamander larvae coexist with fish, there are differences in relative crypsis. Larvae from populations with fish and relatively high gene flow from ephemeral populations (where larvae are dark) tend to be darker (with more melanophores) and more conspicuous to predators than those from more genetically isolated populations, where larvae are lighter and more cryptic. These differences illustrate the role of gene flow as a constraint to adaptive evolution.

The gopher tortoise (Gopherus polyphemus) is a keystone species, for its burrows house more than 300 species of animals. Habitat destruction and its consequent fragmentation have led to a decline in populations within southeastern ranges of the United States. Haemogregarina (intracellular protozoan parasites) have previously been identified in the blood of gopher tortoises. High levels of blood parasitaemia have shown to be a potential indication of stress resulting from overcrowding. The goals of this study are: (1) to characterize hemoparasite species through the use of Polymerase Chain Reaction (PCR) from tortoise blood samples and the ticks attached to tortoises and (2) to determine if free ranging gopher tortoises that live in poorly maintained habitats exhibit higher prevalence and parasitaemia levels within their blood when compared to tortoises inhabiting better maintained sites with prescribed fires. Research goals will be examined at four sites: Blazing Star Preserve (BSP), Pine Jog Preserve (PJP), Florida Atlantic University Preserve (FAUP), and Johnathan Dickinson State Park (JDSP), to determine both hemoparasite prevalence and parasitaemia levels. PJP and JDSP are sites currently practicing fire management and herbicides, whereas FAUP utilizes mechanical and chemical management while BSP is not managed. We hypothesize that parasitaemia will be higher in poorly managed sites as a result of crowding, when compared to well managed sites. This study will benefit current conservation and management practices for gopher tortoises in South Florida and help provide a baseline study for reptile hemoparasites.

Stroke is the most common cause of morbidity and death in the Western world, following ischemic heart disease and cancer. Stroke can be of two types, ischemic or hemorrhagic, with ischemic stroke accounting for approximately 85% of the total number of strokes. Well-recognized environmental risk factors for stroke include hypertension, smoking, diabetes mellitus, atrial fibrillation, and atherosclerosis. Computed tomography (CT) scanning is used to diagnose hemorrhagic stroke but is relatively ineffective and may remain normal in patients with mild ischemic strokes. Magnetic Resonance Imaging (MRI) is more sensitive in detecting ischemia than CT, especially in the diagnosis of mild stroke but it is still not 100% sensitive or precise. A simple and low-cost, rapid blood test to confirm a clinical and imaging diagnosis of ischemic stroke would be extremely useful. Based on this, the central idea of this paper is to develop a method that would be applicable to a statistically viable sample set to provide candidate biomarkers for distinguishing stroke types. In search of these candidate biomarkers, different analytical separation techniques have been used to screen for major differences in the proteomes of patients plasma samples with proteomics for identification.

Replacing multiple adjacent failing teeth with compromised osseous and gingival architecture with implants in the esthetic zone is often challenging and demanding for the clinician. Multiple procedures are usually required and entail a preservation technique, a re-creation approach, or a combination of both. This article describes an interdisciplinary approach (orthodontics, periodontics, and prosthodontics) for tissue architecture modification in multiple adjacent failing teeth with osseous and gingival tissue discrepancies in the esthetic zone. The rationale and limitations are also discussed.

Bone density was evaluated in designated implant sites using a novel volumetric computerized tomographic device. Those measurements were then compared with traditional quantitative computerized axial tomography and subjective bone density evaluation.Sixty-three potential sites for implant placement in jaws from 9 human cadavers were used. Indicator rods 2 mm in diameter were placed in all sites. Radiographic images representing 1-mm buccolingual slices immediately mesial and distal to the rods were selected. Bone density in Hounsfield units was assessed using quantitative cone-beam computerized tomography (QCBCT) and quantitative computerized tomography (QCT) in a standardized implant area superimposed on the images. Bone density was also subjectively evaluated by 2 independent examiners using the Lekholm and Zarb classification.The QCBCT bone density values were generally found to be higher than the corresponding QCT measurements. The correlations between the QCT and QCBCT values, however, were very high in spite of this systematic difference between the 2 methods. The Lekholm and Zarb ratings for the 2 examiners showed correlation coefficients ranging between 0.46 and 0.60 for the relationships with the QCBCT values. For each of the scores used for the subjective classification, however, a wide range of corresponding QCBCT values was observed.High dosage has been the major impediment to the utilization of CT in implant dentistry. The development of a reliable volumetric CT alternative with reduced radiation should provide an effective method for the assessment of both bone quantity and bone density.Access to objective radiographic bone density values should constitute a valuable supplement to subjective bone density evaluations prior to implant placement. QCBCT could be considered an alternative diagnostic tool for preoperative bone density evaluation, especially since the reported radiation dose is minimal.

This article examines and compares the extent of microleakage after 24 hours in freshly extracted teeth using three newer or "third generation" dentin bonding agents. Gluma/Pekalux and Scotchbond 2/Silux were more effective in preventing microleakage in cementum/dentin margins than was Universal Bond/Prisma Micro-fine.

A procedure is described for preparation of primary cultures of epithelial-like cells from the mucosa of the abomasum of young sheep.

Courtney and Forrester (op. cit.) called it C. spiculigerum. Hartwich (1964, Mitt. Zool. Mus. Berlin 40: 15-53) considered that more than one species went by C. spiculigerum and erected C. rudolphii for those specimens reported by Huizinga and by others. This name has been accepted by most recent workers (see references listed by Barus et al., 1978, Helminths of fish-eating birds of the Palaearctic Region I, Nematoda. W. Junk, Publ., Czech. Acad. Sci., Prague, 318 p.), but there are several possible and proposed, older synonyms. In fact, our specimens from the Double-crested Cormorant, White Pelican, Brown Pelican, and Least Bittern, Ixobrychus exilis, collected in Mississippi, as well as additional specimens from Louisiana and Florida were identified as C. microcephalum (Rudolphi 1809) using the key by Barus et al. (op. cit). The interlabial tips are rounded in adults, rather than bifurcated as reported for C. rudolphii. On the other hand, the 4.7to 6.2-mm-long spicules in 12 males 21.0 to 34.5 mm long are 18 to 25% of the body length, which fits within the length-range of those reported for C. rudolphii, but is a smaller percentage of total body length. The spicule tips are similar to those of C. rudolphii, and apparently, the postanal papillae are similar in the two species. We have seen a third species represented by a few specimens. Contracaecum micropapillatum (Stossich 1890) infected one migrating White Pelican. Oglesby (1960, Auk 77: 354) reported more than 1,100 specimens identified as "probably C. micropapillatus" from a dead White Pelican in Apalachee Bay, Florida. We wish to thank W. Adult Sigmodon hispidus were given 50 third-stage larvae of Angiostrongylus costaricensis orally, intraperitoneally, subcutaneously, and on abraded and unabraded skin. Larvae could not penetrate unbroken skin but established normal infections in the cecal vasculature by all other routes. Significantly more adults were recovered after oral and intraperitoneal inoculation than subcutaneously or through abraded skin. In a single animal given larvae subcutaneously, adult worms were recovered from the pulmonary arteries, an abnormal location for this species of metastrongylid nematode, which usually occurs in the ileocolic mesenteric arteries.
