Abstract

Introduction: Cardiac surgery with cardiopulmonary bypass and cardioplegic arrest is known to be responsible for ischaemia and reperfusion organ injury. In a previous study, ProMPT, in patients undergoing coronary artery bypass or aortic valve surgery we demonstrated improved cardiac protection when supplementing the cardioplegia solution with propofol (6 mcg/ml). The aim of the ProMPT2 study is to determine whether higher levels of propofol added to the cardioplegia could result in increased cardiac protection. Methods and Analysis: The ProMPT2 study is a multi-centre, parallel, three-group, randomised controlled trial in adults undergoing non-emergency isolated coronary artery bypass graft surgery with cardiopulmonary bypass. A total of 240 patients will be randomised in a 1:1:1 ratio to receive either cardioplegia supplementation with high dose of propofol (12 mcg/ml), low dose of propofol (6 mcg/ml) or placebo (saline). The primary outcome is myocardial injury, assessed by serial measurements of myocardial troponin T up to 48 hours after surgery. Secondary outcomes include biomarkers of renal function (creatinine) and metabolism (lactate). Ethics and Dissemination: The trial received research ethics approval from South Central – Berkshire B Research Ethics Committee and Medicines and Healthcare products Regulatory Agency in September 2018. Any findings will be shared though peer-reviewed publications and presented at international and national meetings. 10. Using Digital Globes to Explore the Deep Sea and Advance Public Literacy in Earth System Science

Abstract

Digital globes are new technologies increasingly used in informal and formal education to display global datasets and show connections among Earth systems. But how effective are digital globes in advancing public literacy in Earth system science? We addressed this question by developing new content for digital globes with the intent to educate and excite the public about biological and geophysical processes and exploration in the deep ocean. We developed the content in tight collaboration with scientists, educators, and graphic artists. We used global datasets, including a new dataset for locations of deep-sea vents, and imagery from deep-diving vehicles to create two narratives, Life Without Sunlight and Smoke and Fire Underwater, each targeting a set of Earth Science and ocean literacy principles. Here, we evaluate perceived learning outcomes for the narratives delivered as movies and as live, docent-led presentations with the room-sized Science On a Sphere®. Both narratives led to median responses of ''Some'' to ''Quite a bit'' of perceived learning per literacy principle. Perceived learning outcomes were greatest for adult lifelong learners, followed closely by students (under age 18); adult educators generally reported less learning. We found no significant differences in the effectiveness of our movies compared to docent-led presentations for any of the literacy principles, and all presentations led to ''Quite a bit'' of excitement for all viewers. Our evaluation provides an initial indication that digital globes can be effective in teaching the scientific literacy principles, and we make recommendations for additional assessment of digital globes as educational platforms.

11. Use of Fine-Scale Geospatial Units and Population Data to Evaluate Access to Emergency Care

Abstract

Introduction: Time to facility is a crucial element in emergency medicine (EM). Fine-scale geospatial units such as census block groups (CBG) and publicly available population datasets offer a low-cost and accurate approach to modeling geographic access to and utilization of emergency departments (ED). These methods are relevant to the emergency physician in evaluating patient utilization patterns, emergency medical services protocols, and opportunities for improved patient outcomes and cost utilization. We describe the practical application of geographic information system (GIS) and fine-scale analysis for EM using Ohio ED access as a case study.Methods: Ohio ED locations (n=198), CBGs (n=9,238) and 2015 United States Census five-year American Community Survey (ACS) socioeconomic data were collected July—August 2016. We estimated drive time and distance between population-weighted CBGs and nearest ED using ArcGIS and 2010 CBG shapefiles. We examined drive times vs. ACS characteristics using multinomial regression and mapping.Results: We categorized CBGs by centroid-ED travel time in minutes: 30 (1.5%; n=141). CBGs with increased median age, Hispanic and non-Hispanic Black population, and college graduation rates had significantly decreased travel time. CBGs with increased low-income populations (adjusted odds ratio [AOR] [1.03], 95% confidence interval [CI] [1.01-1.04]) and vacant housing (AOR [1.06], 95% CI [1.05-1.08]) had increased odds of >30 minute travel time.Conclusion: Use of fine-scale geographic analysis and population data can be used to evaluate geographic accessibility and utilization of EDs. Methods described offer guidance to approaching questions of geographic accessibility and have numerous ED and pre-hospital applications.

Abstract

My thesis has at its heart an interrogation into the practice I have co-created with Tangled Feet, a physical ensemble who use processes of improvisation and devising, and who make work which includes apertures for audience influence, often outside of traditional theatre spaces. I construct an argument that this practice, which foregrounds the body and its potential for kinaesthetic affectivity, and which privileges liveness over leaving a legacy, poses challenges for mainstream theatre historiographies and critical mechanisms which often fail to adequately include the embodied presence of the artist and spectator in the analysis. I use other theorists to explore how a 'text-centricity' evident in British culture which arguably shores up existing power dynamics also propagates reductive binaries that ignore the body's capacity to theorise. I highlight how elements of performance such as kinaesthetic affectivity are often neglected or de-prioritised in the written text, and address this through modelling close analysis of my own and others' performance work with due focus on these elements. In socially-engaged practice like Tangled Feet's, the ways that audiences interact with the work, particularly in public spaces, are of fundamental importance to the political context of the performance. With particular reference to Arts Council England's policies concerning the broadening definition of 'theatre' in the last decade, and attendant focus on 'accessibility', I examine the alternative value system that funding structures create around this type of practice. By bringing into dialogue different theorists and disciplines, including a neuroscientific view of embodied cognition and my own detailed practice observations, my thesis signals what some of the potentials of physical performance might be if we can release it from textual expectations, and how it can challenge conventions, social relationships and political structures.

16. Household Hardships, Public Programs, and Their Associations with the Health and Development of Very Young Children: Insights from Children's HealthWatch

Abstract

America's low-income families struggle to protect their children from multiple threats to their health and growth. Many research and advocacy groups explore the health and educational effects of food insecurity, but less is known about these effects on very young children. Children's HealthWatch, a group of pediatric clinicians and public health researchers, has continuously collected data on the effects of food insecurity alone and in conjunction with other household hardships since 1998. The group's peer reviewed research has shown that a number of economic risks at the household level, including food, housing and energy insecurity, tend to be correlated. These insecurities alone or in conjunction increase the risk that a young child will suffer various negative health consequences, including increases in lifetime hospitalizations, parental report of fair or poor health, or risk for developmental delays. Child food insecurity is an incremental risk indicator above and beyond the risk imposed by household-level food insecurity. The Children's Healthwatch research also suggests public benefits programs modify some of these effects for families experiencing hardships. This empirical evidence is presented in a variety of public venues outside the usual scientific settings, such as congressional hearings, to support the needs of America's most vulnerable population through policy change. Children's HealthWatch research supports legislative solutions to food insecurity, including sustained funding for public programs and re-evaluation of the use of the Thrifty Food Plan as the basis of SNAP benefits calculations. Children's HealthWatch is one of many models to support the American Academy of Pediatrics' call to "stand up, speak up, and step up for children." No isolated group or single intervention will solve child poverty or multiple hardships. However, working collaboratively each group has a role to play in supporting the health and well-being of young children and their families.

18. Finding Canada outside: Building National Identity through Place-Based Outdoor Education

Abstract

In a country as diverse as Canada, spread over an incomprehensibly large land mass, the connections between citizens may require more imagination. One way that these connections have been traditionally imagined in Canada is through national myths, including the myth of the wilderness. This myth draws the Canadian identity out of an "untouched" wilderness landscape. As much as there are problems with the wilderness myth of Canada, the land provides a valuable connection between all disparate members of Canadian society. And so, the author proposes a much more inclusive re-imagining of this myth, in which people draw national identity from the land in all the variety of its meanings and uses. In this way, Canadians can work to develop attachments to their specific pieces of land, while acknowledging the interconnections of the national landscape. In this article, the author discusses how to build national identity through place-based outdoor education. She argues that educators need to introduce their students to the places in which they live, and encourage them to find the connections between their selves and their places in addition to fostering an understanding of their connections to others and other people's places.

Abstract

Assessed a random sample of 102 female students from a small private college concerning levels of individuation, intimacy, intimidation, parental triangulation, and personal authority. These concepts formed the construct of Personal Authority and were tested regarding their relationship to self-esteem, physical health, and college adjustment. Abstract

This paper examines Ericksonian techniques of reframing, symptom prescription, extension of the symptom pattern, reversal of the symptom, and use of ritual by reviewing each of these as used in the popular situation-comedy, “The Andy Griffith Show”. Excerpts from the show are used to explore the value of accepting the symptom as presented by the client. Use of these excerpts are seen by the authors as a valuable classroom teaching tool for therapy students, as well as a model for colleague relationships.

Abstract

Seeing a face gaze at an object elicits rapid attention shifts toward the same object. We tested whether gaze cueing is predictive: do people shift their attention toward objects others are merely expected to look at? Participants categorized objects while a face either looked at this object, at another object, or straight ahead. Unbeknownst to participants, one face would only look at drinks and the other at foods. We tested whether attention was drawn toward objects “favored” by a face even when currently looking straight ahead. Indeed, while gaze expectations initially had a disruptive effect, participants did shift attention to the faces’ favored objects once learning had been established, as long as emotional expressions had indicated personal relevance of the object to the individual. These data support predictive models of social perception, which assume that predictions can drive perception and action, as if these stimuli were directly perceived. [ABSTRACT FROM PUBLISHER]
