Introduction: End-Stage Renal Disease (ESRD) patients necessitate dialysis when kidney transplantation is not feasible. Hemodialysis patients exhibit higher mortality rates compared to the general population due to uremia and an increased burden of comorbidities. In this vulnerable population, defective innate and adaptive immunity contribute to infectious diseases being a leading cause of hospitalization and mortality. Given the COVID-19 pandemic and the potential for a diminished antibody response to the COVID-19 vaccine among dialysis patients, this study aimed to measure IgG antibody titers in this patient group following COVID-19 vaccination and compare them to healthy individuals. Materials and Methods: This cross-sectional study enrolled hemodialysis patients who had received a minimum of two doses of the COVID-19 vaccine within the past two to six months. Informed consent was obtained from the patients for antibody titer testing. Additional information was collected using a checklist. A control group consisting of individuals who had also received at least two doses of the COVID-19 vaccine was selected and matched to the patient group based on vaccine type, number of doses, and timing of administration. Relevant data for both groups were recorded in the checklist. IgG titers were measured using the indirect ELISA technique to quantify specific IgG against the Spike antigen in the serum samples of both patients and controls. A comparison of IgG titers between the two groups was conducted using SPSS version 26 software. Results: The hemodialysis group comprised 44.1% males, while the control group consisted of 67.6% males. The mean age for the hemodialysis and control groups was 59.97±15.92 and 54.79±21.77, respectively. Underlying diseases were present in 76.5% of the hemodialysis group and 58.8% of the control group, with hypertension being the most common comorbidity in both groups. Sinopharm was the most commonly administered vaccine in both groups for both the first and second doses. Vaccine side effects were reported by 50% of hemodialysis patients and 17.6% of the control group. Furthermore, 55.9% of the hemodialysis group and 35.3% of the control group had a history of prior COVID-19 infection before vaccination. The positive IgG titer rates were 94% in the hemodialysis group and 91% in the control group, with no significant difference observed between the two groups (P<0.999). The relationship between positive IgG titers and group membership was not significant across other investigated variables. Conclusion: The present study revealed no significant difference in IgG titers against the S1 antigen between hemodialysis patients and controls who had received a minimum of two doses of the COVID-19 vaccine. Furthermore, IgG titers were not associated with age, sex, underlying diseases, vaccine side effects, or behavioral parameters. In addition, an inverse correlation was observed between the duration since the last vaccine dose and the initiation of dialysis, and IgG titers. [ABSTRACT FROM AUTHOR]

Introduction: Nosocomial infections have always been one of the main concerns of the healthcare system of any society, and nurses as frontline health care providers have a significant role in reducing the prevalence and occurrence of nosocomial infections. The purpose of this study is to determine the knowledge and attitude of nurses towards the principles and criteria of isolation of infectious patients in Valiasr educational and treatment center of Zanjan province. Material and Methods: This cross-sectional study was conducted on 186 nurses working in different departments of Waliasr Medical Training Center in Zanjan, Iran. The study was conducted in the form of a questionnaire among the nurses of the above-mentioned medical training center in all three work shifts using a convenient sampling method. Data analysis was done using SPSS software and statistical test (Chi-Square). Results: 186 nurses with an average age of 31.6 years participated in this plan. The results of the study demonstrated that 14% of the participants had a low knowledge level, followed by an average knowledge level (55.9%), and a good knowledge level (30.1%). In terms of attitude, 33.3 % of the participants had a poor attitude, followed by an average attitude (52.2%), and a good attitude (14.5%). Conclusion: Considering the average level of knowledge and attitude of most of the studied people towards the principles and criteria of isolation of infected patients, the level of knowledge and attitude of the treatment staff can be increased by developing comprehensive educational programs and providing sufficient and standard facilities. We aimed to review the records of cancer and kidney transplant patients of out of 1135 COVID-19 patients, who were referred to our hospital (Valiasr) in Zanjan, from March 16th, 2020, to June 11th, 2020. This was single-center, historical cohort study. Patients were divided into different subgroups and compared of disease outcomes. The only predictor of death was lactate dehydrogenase (LDH). The rate of red cell distribution width (RDW) in patients with active cancer was higher than kidney transplant patients and was statistically significant. There was no statistically significant difference in mortality between active and non-active cancer groups. Female sex and low SpO2 has increased the chances of ICU admission. Patients with active cancer generally have severe and more complicated disease and RDW can be a predictable option. [ABSTRACT FROM AUTHOR]

Background: Primary infection with BK virus (BKV) is occurred during childhood and usually asymptomatic, but after initial infection, BKV may persist lifelong in the kidney and genitourinary tract. Reactivation may occur in individuals with compromised immunity such as renal transplant recipients. Due to the role of BKV in BK virus-associated nephropathy (BKVAN) and potentially renal allograft rejection, the detection of BKV in renal transplant candidates is very important. The aim of this study was to evaluate the frequency of BK viremia in end stage renal disease cases who were candidates for renal transplantation. Methods: In this cross-sectional study, 50 cases with end stage renal disease who were candidates for renal transplantation were recruited from the main dialysis unit in Tehran, Iran. Presence of BK viremia was determined in plasma samples of cases using real time PCR. Results: A total of 50 renal transplant candidates with mean age 37.8±13 yr were enrolled in the study. Fifty two percent of subjects were male. Forty six (92%) of them were under HD and 4 (8%) were on PD. BK virus was not detected in any plasma samples of renal transplant candidates. Conclusion: This study showed absence of BK viremia in our renal transplant candidates. However, due to the important role of BKV in BKVAN and renal graft failure and rejection, further studies involving larger number of cases are required to elucidate the rate of the BKV in renal transplant candidates.
