Purpose: As the demand for natural cosmetics increases, properly managing production and quality control should be prioritized. Chlorogenic acid (CGA), epicatechin gallate (ECG), and rosmarinic acid (RMA) were selected to validate the cosmetic composite LOL, a mixture of 50% EtOH extract of Ligularia stenocephala (LS), Orostachys japonica (OJ), and Lavandula angustifolia (LA). Methods: Highperformance liquid chromatography with a diode array detector (HPLC-DAD) was used to simultaneously analyze the three LOL compounds from LOL (Ligularia stenocephala , Orostachys japonica , Lavandula angustifolia ). Linearity, accuracy, and precision analyses were performed using HPLC-DAD, and the anti-oxidative effects were evaluated through a 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical-scavenging assay. Results: All three extracts showed a high DPPH radical-scavenging effect, and the detected marker compounds showed significant linearity (R2≥0.9997). The limit of detection (LOD) of CGA, ECG, and RMA was 0.83 μg/mL, 0.33 μg/mL, and 0.20 μg/mL, respectively. Meanwhile, the limit of quantification (LOQ) of CGA, ECG, and RMA was 2.51 μg/mL, 0.99 μg/mL, and 0.61 μg/mL, respectively. Method validation showed acceptable precision (intra-and inter-day precision, CGA, ECG, and RMA, 1.77, 1.42, 0.80% and 1.09, 0.72, 1.42%, respectively) and recovery (CGA, ECG, and RMA, 102.00%–117.78%, 94.40%-108.06%, and 93.79%–107.05%). Conclusion: The contents of the three markers were 13.99 μg/mL CGA, 8.28 μg/mL ECG, and 8.40 μg/mL of RMA, respectively. This study also suggests that the developed method can validate the LOL cosmetic composition quality control process.

Background and Objectives Ankyloglossia often results in articulation disorders, which depends on age, articulation ability, and mobility of the tongue. A primary treatment strategy includes surgery and/or speech therapy. This study aimed to evaluate the changes of the tongue length, tongue mobility, and percentage of consonants correct (PCC) after frenotomy. We investigated the optimal surgery timing for ankyloglossia.Subjects and Method A total of 136 patients underwent frenotomy for the tongue tie between January 2016 and December 2019. There were 90 males and 46 females, with the mean age of 5.23, which ranged from 2 to 10 years. The length of the tongue was measured for all patients during the operation preoperatively and postoperatively. Supramolecular assembly of end-functionalized polymers, forming block copolymer-like supramolecules based on ionic interaction, has been utilized as a simple and facile method for generating functionalized nanoporous thin film. Here, the binary blend film of aminated poly(ethylene oxide) dendrimer (APEO-G) and sulfonated polystyrene (SPS) at a stoichiometric composition after benzene/water solvent vapor annealing exhibits spherical domains in multilayers over a large area. By controlling the number of end-functional arms of dendrimer via divergent ring-opening polymerization of ethylene oxide as well as the molecular weights of SPS, the domain sizes can be controlled ranging from mainly 34 to 54 nm, even to 131 nm. Our supramolecular-assembly system provides an alternative approach to fabricating a functional nanotemplate by easily etching domains with selective solvent treatment and leaving functional groups at the pore surfaces.

Islet cell transplantation has been an effective method for the treatment of type 1 diabetes. The transplanted islets release insulin in response to changes in blood glucose levels. The clinical application of islet transplantation, however, has been hindered because of some critical problems including immune responses to grafted islets and side effects caused by overdosed immunosuppressive drugs. Herein, surface modification technology using poly(ethylene glycol) (PEG)-dendron was proposed to safeguard islets from the host immune system. PEG-dendron was synthesized by a divergent polymerization method and utilized to cover the islet antigen surface. Successful conjugation of PEG-dendron on the islet surface was achieved without affecting islet morphology, viability, and functionality at a concentration of 1.00%. Surface modification using PEG-dendron effectively prevented protein absorption and immune activation. Foremost, it improved the survival rate of islet grafts in vivo when combined with a low dose of immunosuppressive drugs. In conclusion, PEG-dendron is a potential candidate for the surface modification of pancreatic islets to mitigate immune responses after transplantation.

Study Design: Here we perform a retrospective analysis regarding an incidental dural tear (IDT) during biportal endoscopic spinal surgery (BESS).Purpose: This study investigates the causes of IDT specifically related to technical procedures of BESS with the aim of lowering its risk during training.Overview of Literature: The incidence of dural tear is reported 0.5%–18% in open spinal surgery and 1.7%–4.3% during endoscopic spinal surgery. Because conversion to open surgery for direct repair could become necessary during endoscopic spinal surgery, prevention of this complication is essential.Methods: We have retrospectively studied IDTs by four surgeons during 1 or 2 years after starting BESS for lumbar degenerative diseases and analyzed the locations, sizes, and specific endoscopic conditions specific to each.Results: Twenty-five cases (1.6%) of IDTs among 1,551 cases of BESS occurred; 13 cases (52%) of these were within the first 6 months. Purpose: Recently, the demand for natural extracts and naturally derived ingredients from core raw materials has increased in the cosmetic industry, but their manufacturing and quality control standards are not sufficiently established. This study aims to standardize the compositions of marker compounds from natural cosmetics using ethanol extracts of Chrysanthemum indicum (CI) and Cymbopogon cirtratus (CC). The method is then validated. Methods: In liquid chromatography (LC)/mass spectrometry, astragalin and p -coumaric acid were determined as the marker compounds for quantitative analysis. The performances (linearity, accuracy, and precision) of these two marker components were simultaneously analyzed in a mixture of both natural products using high-performance liquid chromatography with a diode array detector (HPLC-DAD). Results: The marker compounds of CI and CC in the extracts were determined as astragalin and p -coumaric acid, respectively. The detected marker compounds from the CI and CC showed significant linearity (R2≥0.9999). The astragalin and p -coumaric acid markers achieved limits of detection of 1.20 and 0.07 ng/mL, respectively, and limits of quantification of 3.59 and 0.22 ng/mL, respectively. The marker concentrations were 0.24 μg/mL in CI and 1.55 μg/mL in CC. Conclusion: This study suggests that the developed method may be an important basic data for the manufacturing and quality control of natural cosmetic.

Interbody fusion is a common surgical technique for diseases of the lumbar spine. Biportal endoscopic-assisted lumbar interbody fusion (BE-LIF) is a novel minimally invasive technique that has a long learning curve, which can be a barrier for surgeons. Therefore, we analyzed the learning curve in terms of operative time and evaluated the outcomes of BE-LIF. A retrospective study of fifty-seven consecutive patients who underwent BE-LIF for degenerative lumbar disease by a single surgeon from January 2017 to December 2018 was performed. Fifty patients underwent a single-level procedure, and 7 underwent surgery at two levels. The mean follow-up period was 24 months (range, 14–38). Total operative time, postoperative drainage volume, time to ambulation, and complications were analyzed. Clinical outcome was measured using the Oswestry Disability Index (ODI), Visual Analog Scale (VAS) score for back and leg pain, and modified Macnab criteria. The learning curve was evaluated by a nonparametric regression locally weighted scatterplot smoothing curve. Cases before the stable point on the curve were designated as group A, and those after the stable point were designated group B. Operative time decreased as the number of cases increased. A stable point was noticed on the 400th day and the 34th case after the first BE-LIF was performed. All cases showed improved ODI and VAS scores at the final follow-up. Overall mean operative time was171.74±35.1 min. Mean operative time was significantly lower in group B (139.7±11.6 min) compared to group A (193.4±28.3 min). Time to ambulation was significantly lower in group B compared to group A. VAS and ODI scores did not differ between the two groups. BE-LIF is an effective minimally invasive technique for lumbar degenerative disease. In our case series, this technique required approximately 34 cases to reach an adequate performance level.

Background. Symptomatic postoperative spinal epidural hematoma (PSEH) is a devastating complication that could develop after lumbar decompression surgery. PSEH can also develop after biportal endoscopic spine surgery (BESS), one of the recently introduced minimally invasive spine surgery techniques. Gelatin-thrombin matrix sealant (GTMS) is commonly used to prevent PSEH. This study aimed at analyzing the clinical and radiological effects of GTMS use during BESS. Methods. A total of 206 patients with spinal stenosis who underwent decompression by BESS through a posterior interlaminar approach from October 2015 to September 2018 were enrolled in this study. Postoperative magnetic resonance imaging (MRI) was performed in all patients for evaluation of PSEH. Patients in whom GTMS was not used during surgery were assigned to Group A, and those in whom GTMS was used were classified as Group B. In the clinical evaluation, the visual analog scale (VAS) of the leg and back, Oswestry Disability Index (ODI), and modified MacNab criteria were used. The incidence rate and degree of dural compression of PSEH on postoperative MRI were measured. Results. The average age of the patients was 68.1±11.2 (42–89) years. The overall incidence rate of PSEH was 20.9% (43/206). The incidence rates in Groups A and B were 26.4% and 13.6%, respectively, showing a significant difference (p=0.023). The VAS-leg and ODI improvement was significantly different depending on the intraoperative use of GTMS. However, there was no statistically significant difference between the two groups in terms of the VAS-back improvement. Groups A and B showed “good” and “excellent” rates according to the modified MacNab criteria in 79.4% and 87.6% of patients, respectively, showing statistically significant difference (p=0.049). In Group A, two patients underwent revision surgery due to PSEH, while none in Group B had such event. Conclusion. Intraoperative use of GTMS during BESS may be related to reduction in the occurrence rate of PSEH. Specifically, patients with GTMS appliance showed marked decrease in the occurrence of PSEH and had better clinical outcomes.

Background context Numerous minimal invasive techniques treating lumbar spinal stenosis have been introduced. Clinical results using biportal endoscopic spinal surgery has recently been introduced as a treatment option for lumbar spinal stenosis. The purpose of this study was to compare the clinical and radiologic outcome between microscopic unilateral laminotomy bilateral decompression and biportal endoscopic unilateral laminotomy bilateral decompression in patients with degenerative lumbar spinal stenosis. Method A total of 89 patients were evaluated for this study. Only single-level patients were enrolled for accurate comparison. Patients that underwent biportal endoscopic surgery were assigned to Group A, and patients that underwent microscopic surgery were designated Group B. Clinical outcomes were evaluated using modified Macnab criteria, Oswestry Disability Index, and Visual Analog Scale. Postoperative complications were checked until final follow up. Plain radiographs before and after surgery were compared to analyze the change of alignment. Result There was a significant difference between Group A and B in VAS of back on postoperative 2 months. Other clinical measurements except for postoperative 2 months VAS of back showed no significant difference. There were no significant differences between Group A and Group B regarding preoperative and postoperative radiological findings. Conclusion Two different decompression techniques preserve the spinal structure and exhibit a favorable clinical outcome and have the advantage of not causing postoperative instability in the short term follow up. Biportal endoscopic surgery may leads to less postoperative back pain than microscopic surgery, which may allow early ambulation and shorter hospitalization period.

Herpetic anterior uveitis-associated ocular inflammation is commonly manifested with ocular hypertension and glaucoma. Relative to other viruses, cytomegalovirus (CMV) positive hypertensive anterior uveitis is associated with high recurrences of uveitis, as well as with uncontrolled intraocular pressure (IOP) and a subsequent higher requirement for future glaucoma surgery. To gain novel insights into the pathogenesis of ocular hypertension in these patients, we investigated the proteome changes of the aqueous humor (AH) derived from the CMV hypertensive anterior uveitis (CMV-HAU; n = 10) patients and non-glaucoma (cataract; n = 10) patients using liquid chromatography with tandem mass spectrometry. Among a total of 562 proteins identified, fifty and fifteen proteins were significantly elevated and decreased, respectively, in the AH of CMV-HAU patients compared to the control subjects by ≥2 fold. Gene ontology (GO) enrichment and network analyses of elevated proteins revealed that the enrichment of protein was involved in the complement activation, the humoral immune response mediated by the circulating immunoglobulins, proteolysis, and platelet degranulation. In the AH of CMV-HAU, GDF (growth/differentiation factor)-15, the inflammatory marker belonging to the TGF-β superfamily proteins, was significantly increased, while vasorin, an anti-TGF-β protein, levels were decreased. The trabecular meshwork cells infected with CMV exhibited a significantly increased expression of inflammatory markers. Collectively, these data indicate increased complement factor associated inflammation and humoral immunity in CMV-HAU associated ocular hypertension.

In the biological microenvironment, cells are surrounded by an extracellular matrix (ECM), with which they dynamically interact during various biological processes. Specifically, the physical and chemical properties of the ECM work cooperatively to influence the behavior and fate of cells directly and indirectly, which invokes various physiological responses in the body. Hence, efficient strategies to modulate cellular responses for a specific purpose have become important for various scientific fields such as biology, pharmacy, and medicine. Among many approaches, the utilization of biomaterials has been studied the most because they can be meticulously engineered to mimic cellular modulatory behavior. For such careful engineering, studies on physical modulation (e.g., ECM topography, stiffness, and wettability) and chemical manipulation (e.g., composition and soluble and surface biosignals) have been actively conducted. At present, the scope of research is being shifted from static (considering only the initial environment and the effects of each element) to biomimetic dynamic (including the concepts of time and gradient) modulation in both physical and chemical manipulations. This review provides an overall perspective on how the static and dynamic biomaterials are actively engineered to modulate targeted cellular responses while highlighting the importance and advance from static modulation to biomimetic dynamic modulation for biomedical applications.

Inotodiol, an oxysterol found only in Chaga mushroom, has received attention from the pharmaceutical industry due to its strong antioxidant and anti-allergic activities. However, the production of inotodiol is still challenging, and its fundamental properties have yet to be investigated. This study aims to develop an efficient method to produce high-purity inotodiol from Chaga mushroom. Then, pure inotodiol was used to assess its physicochemical properties and biological activities. By optimizing the solvent used for extraction and purification, a new method to produce inotodiol was developed with high purity (>97%) and purification yield (33.6%). Inotodiol exhibited a melting point (192.06 °C) much higher than lanosterol and cholesterol. However, the solubility of inotodiol in organic solvents was notably lower than those of the other two sterols. The difference in the hydroxyl group at C-22 of inotodiol has shown the distinctive physicochemical properties of inotodiol compared with cholesterol and lanosterol. Based on those findings, a nonionic surfactant-based delivery system for inotodiol was developed to improve its bioavailability. The inotodiol microemulsion prepared with 1–2% Tween-80 exhibited homogenous droplets with an acceptable diameter (354 to 217 nm) and encapsulation efficiency (85.6–86.9%). The pharmacokinetic analysis of inotodiol microemulsion in oral administration of 4.5 mg/kg exhibited AUC0–24h = 341.81 (ng·h/mL), and Cmax = 88.05 (ng/mL). Notably, when the dose increased from 4.5 to 8.0 mg/kg, the bioavailability of inotodiol decreased from 41.32% to 33.28%. In a mouse model of sepsis, the serum level of interleukin-6 significantly decreased, and the rectal temperature of mice was recovered in the inotodiol emulsion group, indicating that inotodiol microemulsion is an effective oral delivery method. These results could provide valuable information for applying inotodiol in functional food, cosmetic, and pharmaceutical industries.

Severe fever with thrombocytopenia syndrome (SFTS) is a new tick-borne viral disease, and most SFTS virus (SFTSV) infections occur via bites from the tick Haemaphysalis longicornis; however, SFTSV transmission can also occur through close contact with an infected patient. SFTS is characterized by acute high fever, thrombocytopenia, leukopenia, elevated serum hepatic enzyme levels, gastrointestinal symptoms, and multiorgan failure and has a 16.2 to 30% mortality rate. In this study, we found that age, dyspnea rates, aspartate aminotransferase (AST), alanine aminotransferase (ALT), lactate dehydrogenase, multiorgan dysfunction score (MODS), viral load, IL-6 levels, and IL-10 levels were higher in patients with fatal disease than in patients with nonfatal disease during the initial clinical course of SFTS. In addition, we found that IL-6 and IL-10 levels, rather than viral load and neutralizing antibody titers, in patients with an SFTSV infection strongly correlated with outcomes (for severe disease with an ultimate outcome of recovery or death).

Etch characteristics of ovonic threshold switch (OTS) materials composed of Ge–As–Te for a phase-change random access memory (PCRAM) has been investigated using reactive ion etching via hydrogen-based gases such as H2, CH4, NH3, CH4 + H2, and CH4 + NH3. Among the investigated hydrogen-based gases, NH3 showed the highest etching rate of about 0.52 nm s−1, but the formation of nitride compounds and the increased roughness were observed on the OTS surface by nitrogen. The use of other hydrogen-based gases such as CH4 and CH4 + H2 showed the deposition and low OTS etch rate, respectively, due to the presence of carbon in CH4. Even though H2 showed the better etch characteristics due to the no surface residues or compounds on the OTS surface related to carbon or nitrogen in the etch gases, the best OTS etch characteristics such as the second highest etch rate of 0.45 nm s−1, the lowest surface roughness of 0.21 nm, and no surface residues or compounds were observed with CH4 + NH3 due to the removal of carbon and nitrogen on the surface by the formation of volatile CN compounds while maintaining a high hydrogen atomic concentration in the plasma.

Background Symptomatic postoperative spinal epidural hematoma is a serious complication that may occur after lumbar spine surgery. We analyzed epidural hematoma using postoperative magnetic resonance imaging (MRI) after biportal endoscopic spinal surgery and its impact on clinical outcome. Methods The subjects of this study were 158 patients who underwent single-level decompression using the biportal endoscopic spinal surgery technique from 2015 to 2017. MRI was performed in all patients before and after surgery, and postoperative MRI was used to identify epidural hematoma. The preoperative and postoperative visual analog scale (VAS) score for leg and back pain, Oswestry Disability Index (ODI), and modified Macnab criteria were recorded for evaluation of clinical outcome. The group with postoperative spinal epidural hematoma (group A) and without hematoma (group B) were comparatively analyzed, both radiologically and clinically. Results The mean age of the patients was 67.9 ± 11.1 years (range, 49–89). The total number with grade 0 (no hematoma) was 119 levels patients (75.3%) on the T2 axial image of postoperative MRI. The total number of patients with hematoma was 39 (24.7%) according to T2-weighted axial postoperative MRI. Two patients underwent revision surgery because of hematoma-related symptoms. The improvement of clinical outcome measures including VAS leg, VAS back, and ODI was significantly different between group A and B (P Conclusions The incidence of postoperative spinal epidural hematoma after biportal endoscopic spinal surgery according to postoperative MRI was higher than expected, regardless of the patients' postoperative symptoms. Postoperative hematoma has a decisive influence on postoperative results, and revision surgery might be necessary if canal encroachment is >50% with concomitant symptoms.

Alzheimer's disease (AD) is linked to an extensive neuron loss via accumulation of amyloid-beta (Aβ) as senile plaques associated with reactive astrocytes and microglial activation in the brain. The objective of this study was to assess the therapeutic effect of WS-5 ethanol extract in vitro and in vivo against Aβ-induced AD in mice and to identify the extract’s active constituents. In the present study, WS-5 exerted a significant inhibitory effect on acetylcholinesterase (AChE). Analysis by transmission electron microscopy (TEM) revealed that WS-5 prevented Aβ oligomerization via inhibition of Aβ1-42 aggregation. Evaluation of antioxidant activities using 1, 1-diphenyl-2-picrylhydrazyl (DPPH) demonstrated that WS-5 possessed a high antioxidant activity, which was confirmed by measuring the total antioxidant status (TAS). Furthermore, the anti-inflammatory properties of WS-5 were examined using lipopolysaccharide-stimulated BV-2 microglial cells. WS-5 significantly inhibited the lipopolysaccharide–induced production of nitric oxide and two proinflammatory cytokines, TNF-α and IL-6. The memory impairment in mice with Aβ-induced AD was studied using the Morris water maze and passive avoidance test. Immunohistochemistry was performed to monitor pathological changes in the hippocampus and cortex region of the mouse brain. The animal study showed that WS-5 (250 mg/kg) treatment improved learning and suppressed memory impairment as well as reduced Aβ plaque accumulation in Aβ-induced AD. HPLC analysis identified the extract’s active compounds that exert anti-AChE activity. In summary, our findings suggest that WS-5 could be applied as a natural product therapy with a focus on neuroinflammation-related neurodegenerative disorders.

Biportal endoscopic spinal surgery (BESS) is a minimally invasive spinal surgery, which is basically similar to microscopic spinal surgery in terms of the use of floating technique and technically similar to conventional percutaneous endoscopic spinal surgery in terms of the use of endoscopic or arthroscopic instruments. Using two independent portals (viewing and working) and maintaining a certain distance from the bony and neural structures allow closer access to the target lesion through a panoramic view by free handling of the scope and instruments rather than through a fixed view by docking into the Kambin's triangle. Minimally invasive surgery allows for reduced dissection and inevitable muscle injury, preserving stability and reducing risks of restabilization. The purpose of fusion surgery is the same as that of the three surgical techniques stated above. Its wider range of view helps to overcome limitations of conventional endoscopic spinal surgery and to supplement the weak points of microscopic spinal surgery, such as limited working space in a tubular retractor and difficulty in accessing the contralateral area. This technique provides an alternative to unilateral or bilateral decompression of lumbar central spinal stenosis, foraminal stenosis, low-grade spondylolisthesis, and adjacent segment degeneration. Early clinical outcomes are promising despite potential for complications, such as dural tearing and postoperative epidural hematoma, similar to other procedures. Merits of BESS include decreased postoperative infection rate due to continuous irrigation throughout the procedure and decreased need for fusion surgery for one- or two-level lumbar stenosis by wide sublaminar and foraminal decompression with minimal sacrifice of stabilizing structures.

OBJECTIVE/HYPOTHESIS Adenotonsillar problems might affect the voices of patients with pediatric dysphonia, which is very common. This study aimed to evaluate the prevalence of dysphonia in patients with adenotonsillar problems and to demonstrate the impact of tonsillectomy and adenoidectomy (T & A) on their voice postoperatively. STUDY DESIGN Single-institution retrospective study. METHODS Subjects were recruited from those children admitted for the purpose of T & A, and all underwent the auditory-perceptual assessment by speech therapists preoperatively. If children demonstrated scores >2 in the G parameter, we performed subjective (pediatric voice handicap index [pVHI], severity, talkativeness scale) and objective (Multi-Dimensional Voice Program) voice analyses preoperatively and 1 and 3 months postoperatively. RESULTS Among the 1,197 patients, 91 (7.6%) patients showed dysphonia with a score >2 in the G parameter preoperatively. The follow-up voice analysis was completed in 51 and 22 patients after 1 and 3 months, respectively. Although there were no significant differences in the amount of speech preoperatively and postoperatively, the average visual analog scale score for dysphonia severity was significantly decreased at postoperative 1 month and postoperative 3 months. The average total pVHI score, jitter, shimmer, noise-to-harmonic ratio, and soft phonation index were significantly decreased at 1 and 3 months postoperatively. Subjective scores given by parents did not correlate with the acoustic parameters; however, the postoperative subjective parameters were significantly correlated with objective parameters. CONCLUSIONS Voice problems were significantly improved after T & A in the short term and long term. In those with pediatric dysphonia, decreased mouth breathing and compliance with vocal hygiene would be helpful for voice improvement. LEVEL OF EVIDENCE 4 Laryngoscope, 131:2369-2375, 2021.

Study design A prospective study. Objective To evaluate the change in cervical epidural pressure (CEP) during biportal endoscopic lumbar discectomy (BELD). Summary of background data In percutaneous uniportal endoscopic lumbar discectomy, irrigation fluid (IF) introduced into the spinal canal during surgery can compress the thecal sac, and act as a potential risk for neurological complications by disturbing cerebrospinal fluid (CSF) circulation and increasing intracranial pressure. Methods Thirty consecutive patients, who underwent BELD, which was performed under automated pump system, an infusion pressure of 30 mmHg were enrolled. The change in CEP on C7-T1 level was measured. CEP was measured in each of the five phases of the procedure (1st phase-making surgical portals; 2nd phase-creating a workspace; 3rd phase-performing neural decompression and discectomy; 4th phase-factitious increase of pressure by clogging the outflow; 5th phase-dismission from fluid irrigation system). Neurological complications and independent risk factors were evaluated. Results In the final 27 patients, changes in CEP during surgery were similar. The baseline CEP was 14.8 ± 2.8 mmHg, and the mean CEP in the 3rd phase 18.8 ± 5.1 mmHg was not significantly higher. In the 4th phase, however, the CEPs rose with linear correlation as the pressure increased. In the 5th phase, the elevated CEP returned to baseline in 2.5 ± 5.6 minutes. No patient had neurological complications. No statistically significant risk factors were observed. Conclusion In BELD, which is performed to allow continuous lavage with infusion pressure set to 30 mmHg, CEP does not increase beyond the physiological range. Therefore, BELD may be considered as a potentially safe technique. Level of evidence 4.

In this study, the granulation behavior of food powders was simulated by taking into account the changes in the rheological properties and water content of the powders. Several food powders were studied and classified into self-agglomerating or non-agglomerating powders depending on the presence of a glass transition temperature. Cricket powder, which is incapable of self-agglomeration, was mixed with dextrin powder to enable the granulation process. Cricket + dextrin powder was used as a simulation model, and the glass transition temperature of dextrin and a newly derived evaporation model were adopted in our simulation studies. Similar tendencies were observed in both the experimental and simulation results. Our granulation model can describe all of the following: nucleation, granulation based on the powder properties and changes in particle water content due to evaporation; these behaviors have not been modeled in this way previously.

Foraminal decompression using a minimally invasive technique to preserve facet joint stability and function without fusion reportedly improves the radicular symptoms in approximately 80% of patients and is considered one of the good surgical treatment choices for lumbar foraminal or extraforaminal stenosis. However, proper decompression was not possible because of the inability to access the foramen at the L5-S1 level due to prominence of the iliac crest. To overcome this challenge, endoscopy-based minimally invasive spine surgery has recently gained attention. Here, we report the technical skills required in unilateral extraforaminal biportal endoscopic spinal surgery using a 30° arthroscope to enable foraminal decompression at the L5-S1 level. Two 0.8-cm portals were created 2 cm lateral from the lateral border of the pedicles at the L5-S1 level. After sufficient working space was made, half of the superior articular process (SAP) in the hypertrophied facet joint was removed using a high-speed burr and a 5-mm wide osteotome, whereas the remaining inside part of the SAP was removed using a Kerrison punch and pituitary punch. The foraminal ligamentum flavum should be removed to inspect the conditions of the L5 exiting root and disc. Removing of the extruded disc could decompress the L5 root. The extraforaminal approach using a 30° arthroscope is considered a minimally invasive alternative technique for decompressing foraminal stenosis at the L5-S1 level that preserves facet stability and provides symptomatic relief.

The stenosing foramen of L5-S1 by several degenerative diseases is one of the challenging areas on surgical approaching because of the deeper depth and steep slope in the lumbosacral junction. The floating view using unilateral biportal endoscopic spine surgery rather than docking into the Kambin's zone can make the foraminal structures seen panoramically and permit dynamic handling of various instruments without destroying the facet joint and causing iatrogenic instability. Fine discrimination of structural margins in helps of the higher magnification and gentle manipulation of neural structures just as in open spine surgery could be guaranteed using floating technique from the target structures. Selective decompression with preserving innocent structures including facet joints could relieve foraminal lesions at the L5-S1 and decrease the necessity of fusion surgery caused by wider decompression and iatrogenic instability.

Background context Open laminectomy has been regarded as the standard surgical method up to date in degenerative spinal stenosis or herniation of intervertebral disc. The conventional method may lead to instability and provoke chronic lower back pain by scarifying facet joint, posterior ligamentous complex as well as paraspinal muscle. For this reason, the new technique using an endoscope, which could protect soft tissue and facet joint, recently got spotlight. Purpose The aim of this study is to introduce a new spinal surgical technique using a 30-degreed endoscopy through bi- or tri- portals and to report the preliminary result of this technique. Study design retrospective study Methods One hundred five patients who were suffering from neurologic symptoms by degenerative lumbar spine disease were included even after preoperative conservative treatment. Two or three portals were used for each level. One portal was used for viewing, the others, for working of a certain instrument. Unilateral laminotomy was followed by bilateral decompression under 30° endoscopy. Clinical outcomes were analyzed in view of modified-Macnab criteria, Oswestry Disability Index (ODI), Visual analog scale (VAS), and postoperative complications were analyzed. Results The ODI improved from 67.4 ± 11.5 preoperatively to 22.9 ± 12.4 postoperatively. VAS for leg decreased from 7.7 ± 1.5 to 2.4 ± 1.3 at final follow up. Eighty-eight percent of the patients were improved over a level of good based on the Macnab criteria. There were not infection case. Conclusions The 30-degreed endoscopy had the advantages of obtaining a wider view. Full endoscopic decompression using 30-degreed endoscopy allowed satisfactory result clinically and reduction of surgical infection. It could be alternative method of microscopic laminectomy.

Background context Lumbar foraminal stenosis or extraforaminal stenosis is a common cause of spinal cord radiculopathy. Recently, several authors have introduced an endoscopy-based spinal surgery technique. Purpose The study aimed to introduce far lateral approach of biportal arthroscopic technique using 30° arthroscopy for foraminal decompression of L5-S1. Study design Technical note. Patient sample 12 consecutive patients. Outcome measures The leg VAS with modified Macnab criteria was measured. Result The leg VAS improved from VAS 7.5 to 1.8. Conclusion Far lateral approach of BASS with 30° arthroscopy is an alternative method that can decompress foraminal stenosis minimally invasively.

Lumbar spine fusion has been widely accepted as a treatment for various spinal pathologies, including the degenerative spinal diseases. Transforaminal interbody fusion (TLIF) using minimally invasive surgery (MIS-TLIF) is well-known for reducing muscle damage. However, the need to use a tubular retractor during MIS-TLIF may contribute to some limitations of instrument handling, and a great deal of difficulty in confirming contralateral decompression and accurate endplate preparation. Several studies in spinal surgery have reported the use of the unilateral biportal endoscopic spinal surgery (technique for decompression or discectomy). The purpose of this study is to describe the process of and technical tips for TLIF using the biportal endoscopic spinal surgery technique. Biportal endoscopic TLIF is similar to MIS-TLIF except that there is no need for a tubular retractor. It is supposed to be another option for alternating open lumbar fusion and MIS fusion in degenerative lumbar disease that needs fusion surgery.

The relationships between tensile properties and precipitates of a high-strength low-alloy steel depending on the isothermal conditions were investigated. While the isothermally treated steel at 300–500 °C for 1 and 24 h had no significant difference, the steel treated at 500 for 336 h, denoted as 500–336 h, not only showed a decrease in tensile stress but also exhibited a highly increased elongation. Transmission electron microscopy and atom probe tomography were utilized to evaluate the precipitates distribution. The results showed that, in the case of 500–336 h sample, the fraction of precipitates with a radius over 10 nm is the highest and that of a few nano-sized precipitates is the lowest among all samples. It can be explained that the coarsening of originally nano-sized precipitates, occurred by diffusion of dissolved carbon in 500–336 h, mainly affects the tensile behavior.

In order to discover lifespan-extending compounds made from natural resources, activity-guided fractionation of Zingiber officinale Roscoe (Zingiberaceae) ethanol extract was performed using the Caenorhabditis elegans (C. elegans) model system. The compound 6-gingerol was isolated from the most active ethyl acetate soluble fraction, and showed potent longevity-promoting activity. It also elevated the survival rate of worms against stressful environment including thermal, osmotic, and oxidative conditions. Additionally, 6-gingerol elevated the antioxidant enzyme activities of C. elegans, and showed a dose-depend reduction of intracellular reactive oxygen species (ROS) accumulation in worms. Further studies demonstrated that the increased stress tolerance of 6-gingerol-mediated worms could result from the promotion of stress resistance proteins such as heat shock protein (HSP-16.2) and superoxide dismutase (SOD-3). The lipofuscin levels in 6-gingerol treated intestinal worms were decreased in comparison to the control group. No significant 6-gingerol-related changes, including growth, food intake, reproduction, and movement were noted. These results suggest that 6-gingerol exerted longevity-promoting activities independently of these factors and could extend the human lifespan.
