The National Lung Screening Trial (NLST) is a randomized multicenter study comparing low-dose helical computed tomography (CT) with chest radiography in the screening of older current and former heavy smokers for early detection of lung cancer, which is the leading cause of cancer-related death in the United States. Five-year survival rates approach 70% with surgical resection of stage IA disease; however, more than 75% of individuals have incurable locally advanced or metastatic disease, the latter having a 5-year survival of less than 5%. It is plausible that treatment should be more effective and the likelihood of death decreased if asymptomatic lung cancer is detected through screening early enough in its preclinical phase. For these reasons, there is intense interest and intuitive appeal in lung cancer screening with low-dose CT. The use of survival as the determinant of screening effectiveness is, however, confounded by the well-described biases of lead time, length, and overdiagnosis. Despite previous attempts, no test has been shown to reduce lung cancer mortality, an endpoint that circumvents screening biases and provides a definitive measure of benefit when assessed in a randomized controlled trial that enables comparison of mortality rates between screened individuals and a control group that does not undergo the screening intervention of interest. The NLST is such a trial. The rationale for and design of the NLST are presented.

iology educators increasingly recognize that students learn concepts most successfully through acombination of hands-on experience, cooperative problemsolving, and independent study (Batchelder and Root 1994,Thorley and Gregory 1994,Herreid 1998).Students who areexploring careers in the biological sciences can experience thedaily practice of professionals through a burgeoning numberof internships, which are usually separate from their coursework.However, academic programs that integrate servicelearning with formal classroom study are relatively uncom-mon in the biological sciences,except in certain health or pre-medical curricula (Kelly 1995), agricultural extension pro-grams (Rilla and Ponzio 1994), and environmental studiesdepartments (Kaufman and Zeigler 1993, Touval and Dietz1994,Ward and Zotlowski 1999).We describe one such program in which rigorous coursework at the University of Connecticut prepared a group of stu-dents for a subsequent 10-week summer internship at a non-profit institution, the Connecticut chapter of The NatureConservancy (TNC). Called the REAL Team, for rapid eco-logical assessment of the landscape, this program trainedstudents in both broad ecological concepts and specific tax-onomic skills,knowledge that they could then apply to theirwork with a conservation organization.Instead of crowdingan internship and classroom instruction into a single se-mester,the program spanned a spring semester of academicstudy and a subsequent summer-long appointment to a pro-fessional project.We found that this program offers many ed-ucational benefits to students, strengthens cooperation be-tween academic and professional institutions, and providesan adaptable model for the development of similar programselsewhere.In recent reviews, service-learning programs that haveevolved within university curricula have been critically ex-amined for the ways in which these ambitious projects fos-ter both professional and academic learning. Students benefit from service-learning projects in many ways: directlyapplying classroom concepts to real-world problems, ob-serving professional role models,developing a sense of placeby contributing to their local community,honing teamworkskills, and adopting efficient time management in meetingdeadlines (Dillon and Van Riper 1993).However,ambitiousprojects also carry the risk of failure for students and facultycoordinators and can entail substantial supervision on the partof both the academic and extramural partners.A semester of16 weeks or less is a very brief time in which to identify an ap-propriately scaled project; establish and maintain smoothcollaboration among students and their host agency; anddeliver a tangible product that meets the needs of the educator,agency, and students (Hornig 1999). The host agency mustevaluate the net worth of the work the students producewith respect to the time invested by its own employees in as-sisting students with their project.In addition,such a programcan be sustained only if it is an integral part of the depart-mental curriculum and supported by the academic partner’sinfrastructure with sufficient budget allocations for the ap-propriate courses (Korfmacher 1999).

4. Durational characteristics of vocal and subvocal speech: implications concerning phonological organization and articulatory difficulty

Two experiments were conducted to investigate whether phonetic sequences which are considered to be “difficult” to articulate show evidence of difficulties at an organizational level and/or at the level of overt articulation. In the first experiment, subjects produced rapid vocal and subvocal repetitions of a variety of bi-syllabic stimuli that were judged to differ in articulatory difficulty (e.g. “toy boat” vs. “tow boat”). With each repetition of a stimulus, subjects tapped on the silent switch of a response box in order to provide a measure of stimulus duration. A substantial range of durations was observed across both the vocal and the subvocal stimuli, suggesting that some stimuli were indeed more difficult to repeat than others. In addition, stimuli which were found to be longer in duration in the vocal condition also tended to be longer in the subvocal condition. This was interpreted as an indication that certain words are difficult to produce not only due to articulatory (i.e. execution) factors, but also due to planning factors. In the second experiment, subjects performed a task similar to that of the first experiment, except that they repeated the individual monosyllables which made up the bi-syllabic stimuli of Experiment 1 (e.g. “tow,” “toy,” “boat,” etc.). Bi-syllabic stimulus durations were then obtained in Experiment 2 by adding the durations of appropriate monosyllables together (e.g. “toy” + “boat”). Based on comparisons that were made between the “summed” bi-syllables of Experiment 2 and the original bi-syllables of Experiment 1, it was concluded that words such as “toy boat” (vs. “tow boat”) are difficult to produce not so much because of properties of the particular segments they consist of, but because of “interference” effects among various segments. One of the primary sources of this interference seemed to be a result of similar segments occurring in similar syllable positions.

A number of physical and psychological changes occur as a result of the normal aging process. These changes often result in an increase in the time subjects require to perform various motor (and sensory) tasks. Although the effects of aging upon a variety of behaviors have been quite well documented, considerably less information is available concerning how normal aging may affect speech production. The present study examined temporal characteristics of the speech of 10 normal, elderly adults and 10 young adults who produced a variety of words and sentences at both normal and fast speaking rates. Acoustic analyses indicated that the elderly adults' segment, syllable, and sentence durations were 20 to 25% longer than those of the young adults at both the normal and the fast rates of speech. In addition to comparisons that were made between these two groups of subjects, comparisons were also made with durations of the speech of young children studied in previous research. It was observed that the elderly subjects tended to produce durations comparable to those of 6- and 7-year-old children.

Acoustical analyses indicated that the elderly adults' segment, syllable, and sentence durations were 20%–25% longer than those of the young adults at both speaking rates. It was also found that the durations of the older subjects' speech at the fast rate were approximately equal to those of the younger adults' speech at a normal rate. In addition to comparisons made between these two groups of subjects, comparisons were also made with durations of the speech of young children studied in previous research.
