Abstract

The male breast is a non-functional and rudimentary organ, but similarly to the female breast, it can be affected by various diseases. In contrast to female breast cancer, male breast cancer has a low incidence, and there is no established breast cancer screening program for male patients. Therefore, the diagnostic evaluation is usually performed in male patients with symptoms such as palpability or pain in the breasts. Furthermore, most adult male patients who visit breast clinics sometimes present with not only breast symptoms but also axillary symptoms, and both the breast and axilla are usually examined during breast ultrasonography in daily clinical practice. The purpose of this pictorial essay was to present the sonographic features of various palpable breast and axillary lesions in adult male patients.

Abstract

A cardiopulmonary exercise test (CPET) is essential for lung resection. However, performing a CPET can be challenging. This study aimed to develop a machine learning model to estimate maximal oxygen consumption (VO2max) using data collected through a patch-type single-lead electrocardiogram (ECG) monitoring device in candidates for lung resection. This prospective, single-center study included 42 patients who underwent a CPET at a tertiary teaching hospital from October 2021 to July 2022. During the CPET, a single-lead ECG monitoring device was applied to all patients, and the results obtained from the machine-learning algorithm using the information extracted from the ECG patch were compared with the CPET results. According to the Bland–Altman plot of measured and estimated VO2max, the VO2max values obtained from the machine learning model and the FRIEND equation showed lower differences from the reference value (bias: −0.33 mL·kg−1·min−1, bias: 0.30 mL·kg−1·min−1, respectively). In subgroup analysis, the developed model demonstrated greater consistency when applied to different maximal stage levels and sexes. In conclusion, our model provides a closer estimation of VO2max values measured using a CPET than existing equations. This model may be a promising tool for estimating VO2max and assessing cardiopulmonary reserve in lung resection candidates when a CPET is not feasible. [ABSTRACT FROM AUTHOR]

Abstract

Aims: The objective of this study was to examine the effects of interferon-gamma injection on changes in collagen content, after the creation of partial bladder outlet obstruction in the rats. Methods: A total of 50 Sprague-Dawley female rats were subjected to partial bladder outlet obstruction by using metal rods. The rats were divided into five groups (n = 10 in each group): normal control (N), sham operation (S), bladder outlet obstruction for 4 weeks (BOO), interferon-gamma injection after sham operation (S + IFN), and interferon-gamma injection after bladder outlet obstruction (BOO + IFN). Interferon-gamma was subcutaneously injected (100,000 units per injection, LG chemical Co., Seoul, Korea) daily for 4 weeks in the injection groups, after which all rats were sacrificed. Results: The collagen area percentage (collagen/collagen + muscle) in each bladder, calculated through image analysis, was 40.7 ± 1.6% in the N, 38.2 ± 2.0% in the S, 26.6 ± 3.8% in the S + IFN, 19.9 ± 2.5% in the BOO, and 12.4 ± 2.0% in the BOO + IFN group (mean ± standard error). Abstract

A 64-year-old man presented with gross hematuria 1 day in duration in June 1999. He also had obstructive urinary symptoms 1 year in duration. History was remarkable for transurethral prostatic resection and orchiectomy for carcinoma of the prostate (clinical stage T3cN1M1b) 4 years earlier. According to the previous medical records, excretory urography had revealed marked hydronephrosis on the right side. Computerized tomography had shown diffuse seminal vesicle invasion, external iliac lymphadenopathy greater than 2 cm. and hydroureter on the right side. Bone scan had been normal. One month after bilateral orchiectomy prostate specific antigen (PSA) had decreased to 1.1 from 40.9 ng./ml. The patient had been lost to followup for more than 3 years. When the patient presented to us PSA had increased to 14.2 ng./ml. Excretory urography showed right hydronephrosis and an attempt at retrograde ureteral pyelography failed because the right ureteral orifice could not be identified. Transurethral prostatic resection was performed and urinary symptoms resolved. Computerized tomography revealed a hydroureter with multiple intraluminal ureteral masses. However, the seminal vesicle was normal with no lymphadenopathy. Bone scan was normal. We performed right percutaneous nephrostomy, and antegrade pyelography revealed multiple smooth marginated filling defects at the middle and lower ureter with hydroureter (fig. 1). Right nephroureterectomy with bladder cuff excision was performed. In the ureter 3 yellowish irregular elevated tumors were identified. The first mass emerged 16 cm. from the ureteropelvic junction.
