Abstract

Background: The importance of physical rehabilitation in optimizing outcomes in critically ill patients is recognized. However, the frequency and benefit of mobilization in pediatric patients undergoing veno-arterial (VA) extracorporeal membrane oxygenation (ECMO) are unclear. This study evaluates a cohort of pediatric VA-ECMO patients to characterize the spectrum of mobility and evaluate associations of mobilization with clinical outcomes. Methods: We analyzed 688 patients (8-18 years) who underwent primary ECMO runs and had mobilization data collected in the Extracorporeal Life Support Organization Registry. Demographics, pre-ECMO support, location and duration of support, and outcomes, including survival to hospital discharge, heart transplant, and ECMO-related complications, were collected. Propensity score modeling was performed with entropy weighting to compare outcomes between mobile and nonmobile patients. Results: Of the 688 patients included, 10% achieved some degree of mobility (69/688); the majority of those exercised in bed. After propensity score matching, mobility was associated with an increased likelihood of being discharged alive (odds ratio (OR) 1.16, 95% cardiac index (CI) 1.04, 1.30) and receiving a heart transplant (OR 1.15, 95% CI 1.02, 1.29), and a lower likelihood of dying on ECMO (OR 0.90, 95% CI 0.81, 1.00). There was no association between mobility and ECMO being discontinued due to complication (OR 1.03, 95% CI 0.97, 1.10). Conclusion: Mobilization in a pediatric VA-ECMO cohort was achieved by 1/10 patients, is associated with heart transplant and survival to hospital discharge, and is not associated with ECMO-related adverse events. Mobility in certain pediatric VA-ECMO patients is feasible and may represent an opportunity to improve outcomes.

Abstract

Coherent double neutral-pion photoproduction on the deuteron, γd→π0π0d, has been experimentally studied at incident photon energies ranging from 0.75 to 1.15 GeV. The total cross section as a function of the γd center-of-mass energy shows resonance-like behavior, which peaks at approximately 2.47 and 2.63 GeV. The measured angular distribution of deuteron emission is rather flat, which cannot be reproduced by the kinematics of quasi-free π0π0 production with deuteron coalescence. In π0d invariant-mass distributions, a clear peak is observed at 2.14±0.01 GeV/c2 with a width of 0.09±0.01 GeV/c2. The spin-parity of this state is restricted to 1+, 2+ or 3− from the angular distributions of the two π0s. The present work shows strong evidence for the existence of an isovector dibaryon resonance with a mass of 2.14 GeV/c2. The 2+ assignment is consistent with the theoretically predicted D12 state, and also with the energy dependence of the πd partial-wave amplitude P23 for the π±d→π±d and π+d→pp reactions. 4. Etch Control and SiGe Surface Composition Modulation by Low Temperature Plasma Process for Si/SiGe Dual Channel Fin Application

Abstract

In this study, we discuss Si-SiGe etch characteristics as well as SiGe surface composition modification. It is required to etch Si and SiGe simultaneously for Si/SiGe dual channel Fin-FETs. Therefore, etch control of these two materials is desired. However, not only halogen chemistries but also physical sputtering etch SiGe selective to Si. We found that Si can be etched faster than SiGe by hydrogen plasma. Our analysis presents that hydrogen bonds selectively with Si rather than Ge, which leads to Si selective removal. As for SiGe surface modification, realizing Si-rich surface in SiGe is known to improve SiGe/high-k interface quality in advanced CMOS. It is also presented that the low-temperature hydrogen plasma induces Si-surface segregation (i.e., Si-rich surface) in SiGe, which is confined near the top-surface region. We proposed this may be caused by ion-energy-driven surface reaction. Our study also shows that Ge/Si ratio increases with plasma exposure time, which has correlation with surface roughness. Using the hydrogen plasma and conventional halogen plasma, we successfully demonstrate to etch Si/SiGe dual channel fins with depth and CD value control.

Abstract

Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). Abstract

The total cross sections were measured for coherent double neutral-pion photoproduction on the deuteron at incident energies below 0.9 GeV for the first time. No clear resonance-like behavior is observed in the excitation function for Wγd=2.38–2.61 GeV, where the d⁎(2380) dibaryon resonance observed at COSY is expected to appear. The measured excitation function is consistent with the existing theoretical calculation for this reaction. The upper limit of the total cross section is found to be 0.034 μb for the dibaryon resonance at Wγd=2.37 GeV (90% confidence level) in the γd→π0π0d reaction.

Abstract

Nitrogen isotope ratios (δ15N) of NH4+ in dairy manure compost piles with and without bulking agent (10 % w∕w) were compared to understand the effects of the use of bulking agent on nitrogen conversion during manure composting. The δ15N–NH4+ values in each of three pile zones (top, side and core) were also compared. At the end of the process, piles with bulking agent showed significantly higher δ15N values (17.7 ± 1.3 ‰) than piles without bulking agent (11.8 ± 0.9 ‰), reflecting the significantly higher nitrogen conversion and NH3 loss in the former. The samples from the top zone, especially in the piles with bulking agent, showed very high NH4+ concentrations with significantly high 15N (δ15N: 12.7–29.8 ‰) values, indicating that extremely high nitrogen conversion, nitrification–denitrification activity of the microbes and NH3 volatilization occurred in this zone.

Abstract

The adherence of bacteria to the host tissues is a first step in the development of infections. Bacterial fimbriae have been shown to play an important role in the interaction between bacteria and host cells or among bacterial cells. Optical density of serum IgG specific Porphyromonas gingivalis strain ATCC 33277 fimbriae antibodies has been estimated in 32 cases of Early-Onset Periodontitis and 14 healthy periodontal subjects as control. The mean level of IgG were significantly elevated (p=0,000) in their cases. The data support the role of P. gingivalis as a key pathogen in Early-Onset Periodontitis.

9. Stockmanship Competence and Its Relation to Productivity and Economic Profitability: The Context of Backyard Goat Production in the Philippines

Abstract

A stockperson has a significant influence on the productivity and welfare of his animals depending on his stockmanship competence. In this study, stockmanship competence (SC) is defined as the capacity of the stockperson to ensure the welfare of his animals by providing his animals’ needs. The study was conducted to evaluate the SC of backyard goat raisers and examine its relationship to goat productivity and economic profitability. There were 101 respondents for this study who have all undertaken farmer livestock school on integrated goat management (FLS-IGM). Interview was conducted in Region I, Philippines on September 3 to 30, 2012 and March 4 to 17, 2013. Data on SC, goat productivity and farmer’s income were gathered. Questions regarding SC were formulated based on the Philippine Recommendations for Goat Production and from other scientific literature. Housing, feeding, breeding and health and husbandry management were the indicators used in computing stockmanship competence index score (SCIS). Pearson correlation using Statistical Package for Social Science (SPSS) was carried out to analyse the relationship between SCIS, productivity and income. Based from the results of the study, a majority of the respondents raised native and upgraded goats. The computed mean SCIS before and after undergoing FLS-IGM were 38.52% and 75.81% respectively, a percentage difference of 65.23%. Both index scores resulted in significant differences in productivity and income. The median mature weight and mortality rate of goats before FLS-IGM was 14 kg and 30% respectively. After FLS-IGM, median mature weight was 19 kg and mortality rate decreased from 30% to 11.11%. Likewise, fewer goat diseases were observed by farmers who were able to undergo FLS-IGM. With regard to income, there was a 127.34% difference on the median net income derived by farmers. Result implies that improved SC could lead not only to increased productivity and income of backyard goat raisers but also to better animal welfare.

Abstract

C57Black/6 and SV129 mice are widely used for the production of transgenic mutants in molecular stroke research but the ischemic susceptibility of these strains is influenced by differences in vascular anatomy and the responsiveness to excitotoxins and vasodilatory stimuli. To differentiate between these opposing effects on infarct size, the vascular territory of the two strains was correlated with the hemodynamic, metabolic, and morphological consequences of permanent middle cerebral artery (MCA) occlusion. The vascular anatomy was studied by latex infusion, brain infarction by vital staining, the size of the ischemic penumbra by imaging of ATP and protein synthesis, and blood flow by laser–Doppler flowmetry. In C57Black/6 mice the MCA-supplied vascular territory and the size of brain infarcts were significantly larger than in SV129 mice but the size of the penumbra and the residual blood flow in the center of the MCA-supplying territory were similar in both strains. These findings suggest that differences in infarct size in C57Black/6 and SV129 mice are determined mainly by the vascular anatomy and not by differences in collateral vascular responsiveness or excitotoxicity.

Abstract

A precursory seismic quiescence lasting 1.5 ± 0.5 years was observed prior to the 1987 M 6.7 Chiba-toho-oki earthquake, Central Japan. This event was the largest mainshock to occur in the region in 60 years. A quantitative analysis of the seismicity rates, using two independent catalogs provided by the NIED and JMA networks, shows that the precursory seismic quiescence is centered in the shallower part of the rupture zone of the subsequent mainshock, at a depth of 20-40 km. At the hypocenter of the 1987 Chiba-toho-oki mainshock, a 50% increase in the seismicity rate was detected in the NIED data, coinciding in time with the onset of quiescence (1986.4 ± 0.5). The simultaneous appearance of both quiescence in the shallow part of the rupture zone observed in two catalogs, and a rate increase in the immediate hypocenter region, suggest that these phenomena are causally linked to the subsequent mainshock. However, a quantitative analysis of both catalogs reveals that the precursory quiescence and rate increase are not unique, since rate changes of this duration and significance often occur in the data. A rate change of this significance rating could probably not be detected as a precursor in a real time approach. For the aid of real time monitoring of seismicity rate changes, we introduce the method to calculate the 95-percentile of confidence level for the significant rate changes.

Abstract

We investigate some characteristics of foreshock activity of moderate and large earthquakes which occurred in the present century in Albania and the surrounding area. Using a prediction algorithm, based on possible foreshocks, we obtained a probabilistic relation between possible foreshocks and mainshocks. From documentary and instrumental data for the period 1901-1994 for the area between 39.0°- 43.0°N and 18.5°-21.5°E we evaluated the probability of the occurrence of mainshocks immediately after their possible foreshocks. The result shows that the probability that mainshocks with magnitude M ³ 6.0 are preceded by a foreshock with magnitude M ³ 4.4, distance £ about 50 km and time £ 10 days is 38% (6/16). The probability that one earthquake with M ³ 4.4 will be followed by a larger earthquake with M ³ 6.0 within about 50 km and 10 days is 1.3% (6/468), but the probability increases to 33% (1/3) if 7 earthquakes with M ³ 4.4 occur within about 50 km and 10 days. From instrumental data for the period 1971-1994, the probability that mainshocks with M ³ 5.0 are preceded by a foreshock with magnitude M ³ 4.0 is 33% (5/15). The probability that one earthquake with M ³ 4.0 will be followed by a larger earthquake with M ³ 5.0 within about 50 km and 10 days is 1.9% (5/262), but the probability increase to 5.6% (1/18) if 3 earthquakes with M ³ 4.0 occur within about 50 km and 10 days. We also found a regional variation of foreshock activity with activity decreasing from the Vlora-Elbasani-Dibra transversal seismic belt to the Ionian-Adriatic seismic zone to the interior part of Albania seismic zone.

Abstract

A laser-driven repetition-rated 1.9 MeV proton beam line composed of permanent quadrupole magnets (PMQs), a radio frequency (rf) phase rotation cavity, and a tunable monochromator is developed to evaluate and to test the simulation of laser-accelerated proton beam transport through an integrated system for the first time. In addition, the proton spectral modulation and focusing behavior of the rf phase rotation cavity device is monitored with input from a PMQ triplet. In the 1.9 MeV region we observe very weak proton defocusing by the phase rotation cavity. The final transmitted bunch duration and transverse profile are well predicted by the PARMILA particle transport code. The transmitted proton beam duration of 6 ns corresponds to an energy spread near 5% for which the transport efficiency is simulated to be 10%. The predictive capability of PARMILA suggests that it can be useful in the design of future higher energy transport beam lines as part of an integrated laser-driven ion accelerator system.

Aim Incisional hernia is a benign disease and the indications for elective surgery are relative. Therefore, it is expected to expand the indications and improve patient quality of life by reducing invasiveness of surgery and anesthesia. We have been performing incisional hernia repair under local anesthesia for the past 12 years and report our clinical results. Material & Methods From 2009 to 2021, a consecutive group of 64 patients with incisional hernia was included. We used 0.5% lidocaine with epinephrine as local anesthesia. Pethidine was used as analgesic and dexmedetomidine as sedative. An incision of 30 mm at the top of the hernia bulge was made and a wound protector with sealing silicon cap was attached. We used three 5-mm trocars and a 5-mm flexible laparoscope. A flat self-fixating mesh with was placed over the primarily closed fascia and fixed using laparoscopic tacking devices. Results Forty-nine patients operated without endoscopy (Op). The others (n=15) required endoscopic subcutaneous dissection (LA). The mean ages of Op and LA were 66.9 and 64.3, respectively. The mean operating time of Op was significantly shorter than that of LA (100 vs 132, P=0.006). Similar hernia recurrence rete was observed (4.1% vs 6.7%, respectively). Clavien-Dindo Grade III and IV complications were not observed in both groups. Conclusions Incisional hernia repair under local anesthesia can be performed relatively safely even on elderly patients with serious co-morbidities, and the possibility of contributing to the improvement of patient quality of life by expanding the indications for surgery was expected.

Abstract

We present photometric and spectroscopic observations of the Type Icn supernova (SN) 2021ckj. Spectral modeling of SN 2021ckj reveals that its composition is dominated by oxygen, carbon and iron group elements, and the photospheric velocity at peak is ~10000 km/s. From the light curve (LC) modeling applied to SNe 2021ckj, 2019hgp, and 2021csp, we find that the ejecta and CSM properties of Type Icn SNe are diverse. SNe 2021ckj and 2021csp likely have two ejecta components (an aspherical high-energy component and a spherical standard-energy component) with a roughly spherical CSM, while SN 2019hgp can be explained by a spherical ejecta-CSM interaction alone. The ejecta of SNe 2021ckj and 2021csp have larger energy per ejecta mass than the ejecta of SN 2019hgp. The density distribution of the CSM is similar in these three SNe, and is comparable to those of Type Ibn SNe. This may imply that the mass-loss mechanism is common between Type Icn (and also Type Ibn) SNe. The CSM masses of SN 2021ckj and SN 2021csp are higher than that of SN 2019hgp, although all these values are within the diversity seen in Type Ibn SNe. The early spectrum of SN 2021ckj shows narrow emission lines from C II and C III, without a clear absorption component, in contrast with that observed in SN 2021csp. The similarity of the emission components of these lines implies that the emitting regions of SNe 2021ckj and 2021csp have similar ionization states, and thus suggests that they have similar properties of the ejecta and CSM, which is inferred also from the LC modeling. Medicare’s Hospital Trust Fund is projected to become insolvent sometime during 2028 and there will be insufficient funds to cover the costs of beneficiaries’ care if reforms are not made before then. Many options have been proposed on ways to extend the trust fund’s solvency. Some proposals focus on controlling costs and other proposals include options for raising revenues. A fresh perspective on this policy dilemma may arise by considering Japan’s statutory health insurance (SHI) and its financing mechanisms. Japan could be a useful model because it has an older population and it is facing similar fiscal challenges before Medicare. Japan could offer some useful perspectives from its cost containment efforts to extend Medicare’s solvency.

Abstract

Magnetic reconnection in laser-produced magnetized plasma is investigated by using optical diagnostics. The magnetic field is generated via Biermann battery effect, and the inversely directed magnetic field lines interact with each other. It is shown by self-emission measurement that two colliding plasmas stagnate on a mid-plane forming two planar dense regions, and that they interact later in time. Laser Thomson scattering spectra are distorted in the direction of the self-generated magnetic field, indicating asymmetric ion velocity distribution and plasma acceleration. In addition, the spectra perpendicular to the magnetic field show different peak intensity, suggesting an electron current formation. These results are interpreted as magnetic field dissipation, reconnection, and outflow acceleration. Abstract

Measurements of the cross sections are conducted for coherent photoproduction of the neutral pion and eta meson on the d, gamma deutron to pi0 eta deutron, at the incident energy in the range of the reaction threshold to 1.15 GeV. A rapidly increasing trend below 1 GeV is observed in the total cross section. The data are effectively reproduced by theoretical calculations incorporating the meson-deutron final-state interactions. However, the measured deutron angular distribution d sigma/d Omegad in the center-of-mass frame (gamma d) is rather flat, and it differs significantly from the strong backward-peaking behavior expected in the kinematics of d formation after pi0 eta photoproduction on an initial bound nucleon. In addition, the d sigma/dMeta d differential cross section exhibits a prominent enhancement near the eta d threshold, while d sigma/dMpi d exhibits an enhancement near the known pi d resonance with I=1 and JP=2+. From these observations, we have concluded that two reaction sequences occur; gamma d to pi0 D01 to pi0 eta d and eta D12 to pi0 eta d, where DIJ denotes a state with a baryon number of 2, an isospin of I, and a spin of J. The enhancement corresponding to D01 is expected to be a theoretically predicted eta NN bound state with I=0 and JP=1- or a virtual state of eta d. It is found that attraction between eta d is certainly very strong.

Abstract

The gas-phase etching of SiO2 was examined using HF and methanol vapor at temperatures below 0 °C and at low pressure. The etching rate of SiO2 increased with decreasing temperature and showed a maximum around –30 °C. The obtained etching rate was a maximum of 40 nm min−1 at plasma-enhanced chemical vapor deposition SiO2. The etching rate of SiN examined for comparison was more than ten times smaller than that of SiO2 under the same condition. As a result, the etching selectivity of SiO2 to SiN was found to be over 20 at –40 °C. When utilizing a low temperature of less than –30 °C, gas-phase etching of SiO2 showing a high etching rate and selectivity was achieved.

Abstract

BixZnCo2−xO4 (0 ≤ x ≤ 0.2) nanoparticles with different x values have been prepared by the sol–gel method; the structural, morphological, thermal and thermoelectric properties of the prepared nanomaterials are investigated. XRD analysis confirms that Bi is completely dissolved in the ZnCo2O4 lattice till the x values of ≤0.1 and the secondary phase of Bi2O3 is formed at higher x value (x > 0.1). The synthesized nanomaterials are densified and the thermoelectric properties are studied as a function of temperature. The electrical resistivity of the BixZnCo2−xO4 decreased with x value and it fell to 4 × 10−2 Ω m for the sample with x value ≤ 0.1. The Seebeck coefficient value increased with the increase of Bi substitution till the x value of 0.1 and decreased for the sample with higher Bi content (x ≤ 0.2) as the resistivity of the sample increased due to secondary phase formation. With the optimum Seebeck coefficient and electrical resistivity, Bi0.1ZnCo1.9O4 shows the high-power factor (α2σ550 K) of 2.3 μW K−2 m−1 and figure of merit of 9.5 × 10−4 at 668 K respectively, compared with other samples. The experimental results reveal that Bi substitution at the Co site is a promising approach to improve the thermoelectric properties of ZnCo2O4.

Coherent photoproduction of the neutral pion and eta meson on the deuteron, $\gamma{d}${$\to$}$\pi^0\eta{d}$, has been experimentally studied at incident photon energies ranging from the reaction threshold to 1.15 GeV. The total cross section demonstrates a rapid rise below 1 GeV. The data are underestimated by the existing theoretical calculations based on quasi-free $\pi^0 \eta$ photoproduction on the nucleon followed by deuteron coalescence. At the same time, the data are rather well reproduced by the calculations taking into account the final-state interaction. We have also measured for the first time the differential cross sections: the $\pi^0 \eta$ invariant-mass distribution $d\sigma/dM_{\pi \eta}$, the $\pi^0 d$ invariant-mass distribution $d\sigma/dM_{\pi d}$, the $\eta d$ invariant-mass distribution $d\sigma/dM_{\eta d}$, and the distribution over the deuteron emission angle $d\sigma/d\Omega_d$ in the overall center-of-mass frame. The measured cross section $d\sigma/d\Omega_d$ does not exhibit strongly backward-peaking behavior predicted by the calculations. At all incident energies, an increase in $d\sigma/dM_{\eta d}$ near the $\eta d$ threshold is observed, which indicates a bound or virtual $\eta d$ state resulting from a strong attraction between $\eta$ and a deuteron. The possibilities of using coherent $\pi^0\eta$ photoproduction on a nucleus to study the $\eta$-nuclear interaction are also discussed., Comment: 15 pages, 9 figures

Abstract

Prediction models for enteric methane (CH4) emissions from beef cattle proposed by various groups may not perform with similar accuracy for the low- and middle-income countries in South-east Asia (SE-Asia) because beef cattle in these countries are raised under different climatic conditions with diverse feeding systems, and have different CH4 emission characteristics. The objectives of this study were to: i) predict CH4 emission (g d−1 animal−1), yield [g kg−1 dry matter intake; DMI)−1], intensity [g kg−1 average daily gain)−1], and CH4 conversion factor (Ym) using an intercountry database of individual animal records from SE-Asia; ii) evaluate the impact of different dietary forage contents (all-, high- and low-forage) representing the diverse feeding systems on CH4 emission, yield, intensity and Ym in SE-Asia; and iii) cross-validate equations from this study with published data. A total of 398 individual animal observations of beef cattle from SE-Asia were used for this analysis. Linear models developed by incrementally adding covariates revealed that CH4 emission model using only DMI fitted to all data had a root mean square prediction error (RMSPE) of 16.9%. Subsets containing data with 100% forage in the diet (all-forage), 50–85% (high-forage) and < 50% (low-forage) had an RMSPE of 16.5%, 14.7%, and 17.4%, respectively. Linear multiple equation based on DMI and dietary NDF concentration (DMI + NDF_C, RMSPE = 15.2%; all-data) improved prediction accuracy over that of DMI alone. The DMI + NDF_C models for all-forage (RMSPE = 14.6%) and high-forage subsets (RMSPE = 13.3%) except for low-forage (RMSPE = 16.4%), improved the precision and accuracy of CH4 emission prediction. Methane yield and CH4 emission intensity could not be reliably modelled with the current database. The present study provides improved CH4 prediction models for beef cattle managed under diverse feeding systems in SE-Asia and affirmed that region-specific models are needed to reliably predict beef cattle CH4 emission at national or regional levels, particularly for low- and middle-income countries.

Abstract

Although renal function often improves after pediatric left ventricular assist device (LVAD) implantation, recovery is inconsistent. We aimed to identify hemodynamic parameters associated with improved renal function after pediatric LVAD placement. A single-center retrospective cohort study was conducted in patients less than 21 years who underwent LVAD placement between June 2004 and December 2015. The relationship between hemodynamic parameters and estimated glomerular filtration rate (eGFR) was assessed using univariate and multivariate modeling. Among 54 patients, higher preoperative central venous pressure (CVP) was associated with eGFR improvement after implantation (p = 0.012). However, 48 hours postimplantation, an increase in CVP from baseline was associated with eGFR decline over time (p = 0.01). In subgroup analysis, these associations were significant only for those with normal pre-ventricular assist device renal function (p = 0.026). In patients with preexisting renal dysfunction, higher absolute CVP values 48 and 72 hours after implantation predicted better renal outcome (p = 0.005). Our results illustrate a complex relationship between ventricular function, volume status, and renal function. Additionally, they highlight the challenge of using CVP to guide management of renal dysfunction in pediatric heart failure. Better methods for evaluating right heart function and volume status are needed to improve our understanding of how hemodynamics impact renal function in this population.

Abstract

Intergenic transcription is a common feature of eukaryotic genomes and performs important and diverse cellular functions. Here, we investigate the iab-8 ncRNA from the Drosophila Bithorax Complex and show that this RNA is able to repress the transcription of genes located at its 3’ end by a sequence-independent, transcriptional interference mechanism. Although this RNA is expressed in the early epidermis and CNS, we find that its repressive activity is limited to the CNS, where in wild-type embryos, it acts on the Hox gene, abd-A located immediately downstream of it. The CNS specificity is achieved through a 3’ extension of the transcript, mediated by the neuronal-specific, RNA-binding protein, ELAV. Loss of ELAV activity eliminates the 3’ extension and results in the ectopic activation of abd-A. Thus, a tissue-specific change in the length of a ncRNA is used to generate a precise pattern of gene expression in a higher eukaryote.

Abstract

BACKGROUND The incidence of neurological complications related to ventricular assist devices (VAD) remains high and includes life-threatening conditions such as intracranial hemorrhage or ischemic stroke. Although no definitive management guidelines exist, operative interventions may be required for major neurological injuries. AIMS This case series describes the perioperative management of children at a single center who underwent neurosurgical procedures for major intracranial bleeds or ischemic strokes while on VAD support. METHODS A database review identified all pediatric VAD patients who underwent a neurosurgical procedure for an intracranial hemorrhage or ischemic stroke from April 2014 to January 2020. Data regarding patient characteristics, preoperative medical management, intraoperative anesthetic management, and postoperative outcomes were collected using retrospective chart review. RESULTS Ninety VADs were implanted in 78 patients. Five neurosurgical interventions were performed: four for intracranial hemorrhages and one for an ischemic stroke. All four patients with hemorrhages were receiving anticoagulation at the time of their event and the three patients on warfarin received emergent reversal with prothrombin concentrate complex and vitamin K. Three patients also received pre-procedural platelet transfusions. Two of the five procedures were emergent bedside external ventricular drain placements, and three were surgical operations. All three patients who underwent operative procedures received invasive hemodynamic monitoring and were supported with a combination of inotropes and afterload reduction. One patient required a massive blood product transfusion. The two patients who underwent external ventricular drain placement had no further surgical interventions and died from the severity of their neurological injuries. All three patients who underwent operative procedures survived to transplantation and discharge home. CONCLUSIONS Perioperative concerns for the anesthesiologist include VAD hemodynamic management, bleeding, VAD thrombosis, and prevention of secondary brain injury. A systematic, multidisciplinary approach to management is paramount to attain favorable outcomes.

Abstract

Male zebra finches, Taeniopygia guttata, acquire their song during a sensitive period for auditory-vocal learning by imitating conspecific birds. Laboratory studies have shown that the sensitive period for song acquisition covers a developmental phase lasting from 25 to 65 days post hatch (dph); formation of auditory memory primarily occurs between 25 and 35 dph. The duration of the sensitive period is, however, dependent upon model availability. If a tutor is not available early in development, birds will learn from an adult male introduced to their cage even after they reach 65 dph. Birds who are exposed to a second tutor as late as 63 dph can successfully adjust their song ‘template’ to learn a new song model. However, if second-tutor song exposure occurs after 65 dph, learning of a new tutor’s song will not occur for most individuals. Here, we review the literature as well as novel studies from our own laboratory concerning sensitive periods for auditory memory formation in zebra finches; these behavioral studies indicate that there are developmental constraints on imitative learning in zebra finches.

Sparing T cells from inhibition Programmed cell death 1 (PD-1) is an inhibitory receptor that normally keeps T cell immune responses in check. Immunotherapy targeting PD-1 has proven successful for certain types of cancer, but it remains unclear how PD-1 is regulated. Sugiura et al. found that a costimulatory molecule, CD80, can restrict PD-1 function during the activation of T lymphocytes. Binding of CD80 to the PD-1 ligand PD-L1 in cis on primary activated dendritic cells interfered with the ability of PD-L1 to access PD-1 on T cells, which would otherwise have inhibited T cell activation. Functional insights into PD-L1–CD80 interactions may explain the outcomes of anti–PD-1 and anti–PD-L1 cancer therapy. Science , this issue p. 558

Abstract

In this study, we discuss Si-SiGe etch characteristics as well as SiGe surface composition modification. It is required to etch Si and SiGe simultaneously for Si/SiGe dual channel Fin-FETs. Therefore, etch control of these two materials is desired. However, not only halogen chemistries but also physical sputtering etch SiGe selective to Si. We found that Si can be etched faster than SiGe by hydrogen plasma. Our analysis presents that hydrogen bonds selectively with Si rather than Ge, which leads to Si selective removal. As for SiGe surface modification, realizing Si-rich surface in SiGe is known to improve SiGe/high-k interface quality in advanced CMOS. It is also presented that the low-temperature hydrogen plasma induces Si-surface segregation (i.e., Si-rich surface) in SiGe, which is confined near the top-surface region. We proposed this may be caused by ion-energy-driven surface reaction. Our study also shows that Ge/Si ratio increases with plasma exposure time, which has correlation with surface roughness. Using the hydrogen plasma and conventional halogen plasma, we successfully demonstrate to etch Si/SiGe dual channel fins with depth and CD value control.
