Abstract

In this paper, a rare case of primary linitis plastica type of colonic cancer is presented with review of the literature.A 35-year-old man was admitted to our department of surgery seeking for the treatment of complete stenosis of the rectum on March 22, 1976. At once, primary loop sigmoid colostomy and biopsy at the white and thick rectal wall were performed. The postoperative roentgenologic examinations reveraled complete narrowing of the rectum about 10 cm in length and the endoscopic examinations showed edematous swollen mucous membrane covered with copius mucus. Neither tumor nor ulcer was recognized. Based upon the histological examinations and clinical findings, the final diagnosis of primary linitis plastica type of carcinoma of the rectum was made. Hence, the combined therapies of oral administration of 5-fluorouracil and cobalt-60 irradiation were begun resulting in marked resolution of the neoplasm and remarkable relief of the clinical symptoms 2 months after the onset of treatment during the period of 20 months. But, in Novomber 1977, numerous dermal metastases came out at the lumbar region the head and the chest wall. The patient was gradually emaciated and died of cachexia on June 2, 1978.In the literature of the world from 1951 through 1976, only 34 cases of primary linitis plastica type of colonic carcinoma have been reported. The clinical aspects of this case was discussed as comparison with the 34 cases.

Abstract

A report is made on the two cases of massive hemorrhage from acute rectal ulcer. A 65-year-old female with senile dementia was admitted for brain contusion. She was underwent a bilateral carotid angiography three weeks after admission and between the 6th and the 22nd day after the examination, five times of intermittent massive rectal bleeding was observed.Another 67-year-old female suffering from cerebral thrombosis had six times of intermittent massive rectal bleeding between the 8th and the 26th day after admission.In the above-mentioned cases, findings of the rectal mucosa were similar to each other, and its main characteristics were of extensive edema and friability accompanied by numerous ulcers varying in size from pinpoint to 2 cm in diameter. Pulsating bleeding from edematous mucosa around the ulcer was also seen in each cases. Moreover the location of the ulcers and the bleeding points shifted during a short term of their clinical courses.The characteristics of the two cases were similar to those of ischemic proctitis, but in the case of ischemic proctitis none of the stressors nor the massive bleedings are found.

Abstract

A case of omental torsion secondary to right inguinal hernia in a 51-year-old man was operated on in Naga Hospital. Preoperative computed tomography (CT) showed a characteristic whirling fatty mass and entering into a right inguinal hernia. Omental torsion is a relatively rare disease, and the preoperative diagnosis is difficult because of nonspecific clinical signs and symptoms. In this report, we gathered 36 patients with omental torsion from the English language literature since 1986, with the addition of the patient herein reported, and clarified the symptoms, imaging findings, and management of omental torsion.

Abstract

A 67-year-old man was admitted to our hospital with nausea and epigastralgia, and a diagnosis of smoldering type adult T-cell leukemia (ATL) associated with advanced gastric carcinoma was made. The gastric carcinoma had caused pyloric stenosis, and investigations revealed regional lymph node metastasis. The patient underwent total gastrectomy, splenectomy, cholecystectomy, and lymph node dissection with a Roux-en-Y anastomosis. Histological examination of the regional lymph nodes revealed not only metastases of gastric carcinoma, but also of ATL lymphoma, indicating a final diagnosis of advanced gastric carcinoma with locoregional lymph node due to both metastasis of the gastric carcinoma and the ATL lymphoma. Despite the administration of postoperative adjuvant chemotherapy comprised of cisplatin/adriamycin/5-fluorouracil in combination with oral etoposide and immunotherapy using ubenimex, paraplegia suddenly developed caused by the metastasis of ATL to the epidural space. Resection of this metastatic tumor for decompression of the spinal cord resulted in resolution of the paraplegia; however, the patient died about 1 month later from rapid systemic tumor growth.
