To reduce the frequency of fragility fractures, Fracture Liaison Services are being created worldwide. One of the most difficult parts in their organization is considered to be the creation of an effective system of long-term patient management. Many factors are involved in this process, concerning the patient, the team of specialist, the peculiarities of the organization of primary health care, and many others. On the one hand, it has been shown that the inclusion of patients in FLS increases their commitment to the implementation of doctor’s recommendations. On the other hand, there are a number of barriers depending both on the characteristics of the patient himself (polymorbidity, severity of the condition, decreased cognitive function, fear of side effects, low motivation to treat osteoporosis) and on various aspects in the medical care organization. The article analyzes the importance of training patients and medical personnel, approaches to the choice of therapeutic tactics, optimization of the process of succession management of patients, the availability of a system of information exchange between medical institutions. Using the example of the Yaroslavl’ FLS, the possibilities of improving various aspects of medical care are analyzed, as well as local problems that arose at various stages of the development of this service, and the measures that were applied to resolve them.

The Russian Association on Osteoporosis was established in 1995 and by now it has regional branches in 53 cities of the ­Russian Federation with total 116 members. The Association also includes the Society of Patients OSTEORUS (­Chairman — OB ­Ershova). The next meeting of the RAOP Presidium was held on December 18, 2021, at which the results of work in 2021 were summed up and plans for 2022 were outlined.

[English] Due to continuous aging of population and increase in the number of elderly people, osteoporosis became socially significant disease leading to disability, increasing mortality and thereby putting an additional burden on the public healthcare system. Screening to identify groups with a high probability of fracture is recommended using the FRAX® Tool for all postmenopausal women and men over 50 years old (А1). In the presense of major pathological fractures (hip, spine, multiple fractures) it is recommended to diagnose osteoporosis and prescribe treatment regardless of the results of spine and hip double X-ray absorptiometry (DXA) or FRAX® (B2). It is recommended to evaluate C-terminal telopeptide when prescribing antiresorptive therapy and procollagen type 1 N-terminal propeptide (P1NP) when prescribing anabolic therapy to patients receiving osteoporosis treatment at baseline and 3 months after the start of therapy in order to assess the effectiveness of treatment early and adherence to the therapy (А2). It is recommended to diagnose osteoporosis and prescribe treatment to patients with high individual 10-year probability of major pathological fractures (FRAX®) regardless of the results of spine and hip DXA (В3). It is recommended to diagnose osteoporosis and prescribe treatment with a decrease in BMD, measured by DXA, by 2.5 or more T-score standard deviations in femoral neck, and/or in total hip, and/or in lumbar vertebrae, in postmenopausal women and men over 50 years old (А2). It is recommended to prescribe bisphosphonates, denosumab or teriparatide to prevent pathological fractures and increase BMD in patients with postmenopausal osteoporosis, osteoporosis in men, glucocorticoid-induced osteoporosis (А2). When the clinical effect of therapy in osteoporotic patients without pathological fractures is achieved (BMD T-score > -2.0 SD in femoral neck and absence of new fractures), it is recommended to interrupt bisphosphonates therapy for 1-2 years with subsequent follow-up (B2). In patients with vertebral fractures, hip fractures or multiple fractures, it is recommended to continue ceaseless long-term treatment of osteoporosis (В3). All drugs for the treatment of osteoporosis are recommended to be prescribed in combination with calcium and cholecalciferol (А2). In order to reduce the risk of recurrent fractures by prescribing osteoporosis therapy timely and maintaining long-term follow-up of patients over 50 years old with pathological fractures, it is recommended to create Fracture Liaison Services (В2). [Русский] В связи с непрерывным старением населения и увеличением количества лиц старшего и пожилого возраста остеопороз стал социально значимым заболеванием, приводя к инвалидности, повышая уровень смертности и создавая тем самым дополнительную нагрузку на систему здравоохранения. Скрининг для выявления групп с высокой вероятностью переломов рекомендован с использованием Алгоритма FRAX® среди всех женщин в постменопаузе и мужчин старше 50 лет (А1). При наличии патологических переломов крупных костей скелета (бедренной кости, тел(а) позвонков(-а), множественных переломов) в анамнезе рекомендовано устанавливать диагноз остеопороза и назначать лечение независимо от результатов рентгеноденситометрии поясничного отдела позвоночника и проксимального отдела бедренной кости или FRAX® (B2). Рекомендуется определение С-концевого телопептида при назначении антирезорбтивной терапии и исследование уровня N-терминального пропептида проколлагена 1-го типа (P1NP) при назначении анаболической терапии пациентам, получающим лечение остеопороза, исходно и через 3 месяца от начала терапии с целью ранней оценки эффективности лечения и приверженности к терапии (А2). Рекомендовано устанавливать диагноз остеопороза и назначать лечение пациентам c высокой индивидуальной 10-летней вероятностью основных патологических переломов (FRAX®) независимо от показателя рентгеноденситометрии поясничного отдела позвоночника и проксимального отдела бедренной кости (В3). Рекомендовано устанавливать диагноз «остеопороз» и назначать терапию при снижении МПК, измеренной при DXA, на 2,5 и более стандартных отклонения по Т-критерию в шейке бедренной кости, и/или в целом в проксимальном отделе бедренной кости, и/или в поясничных позвонках у женщин в постменопаузе и у мужчин старше 50 лет (А2). Для предупреждения патологических переломов и повышения МПК у пациентов с постменопаузальным остеопорозом, остеопорозом у мужчин, глюкокортикоидным остеопорозом рекомендуется прием бисфосфонатов, деносумаба или терипаратида (А2). Пациентам с остеопорозом без патологических переломов при достижении клинического эффекта терапии (МПК до -2,0 SD по Т-критерию в шейке бедренной кости и отсутствие новых переломов) рекомендовано на 1-2 года прекратить терапию бисфосфонатами с последующим динамическим наблюдением (B2). Пациентам с переломами тел позвонков, бедренной кости или множественными переломами в анамнезе рекомендуется продолжать непрерывное длительное лечение остеопороза (В3). Все препараты для лечения остеопороза рекомендуется назначать в сочетании с препаратами кальция и колекальциферола (А2). С целью своевременного назначения терапии остеопороза и обеспечения длительного наблюдения за пациентами в возрасте 50 лет и старше с патологическими переломами для снижения риска повторных переломов рекомендуется создавать Службы профилактики повторных переломов (В2).

Background. To reduce the number of osteoporotic fractures in the world in recent years, — Fracture Liaison Service has been created. Since 2019, the development of a register of patients included in the Secondary Fracture Prevention named «Prometheus» has begun in Russia.Materials and methods. An information system for storing, searching and processing information has been created, and a registration card has been developed. Six FLS participate in the register management. As of 10.03.2021, data on 387 patients were introduced in the register.Results. Among the included patients, 46.25% received a fracture of the proximal femur, 20.41% — of the vertebral bodies and 16.80% of the proximal humerus, the rest had fractures of other localizations. The risk of fractures was assessed in all patients. A high risk of major osteoporotic fractures according to FRAX was determined in 160 (43.24%) people. Two-energy X-ray absorptiometry (DXA) was performed in 152 (39.28%) patients, and osteoporosis was detected in 103 (67.76%), osteopenia — in 42 (27.63%) patients. The risk of falls was assessed in 100% of the included patients, and the risk of falls was determined to be high in 343 (88.63%) patients. Evaluation of various laboratory parameters to find the causes of secondary osteoporosis was performed in most patients. After consultation with the FLS doctor, anti-osteoporotic drugs were prescribed to 241 (64.27%) people. Among the rest, 70 (52.24%) patients had contra-indications and 31 (23.13%) people needed further examination.Conclusions. Thus, the use of the register allows you to evaluate all the main sections of the organization of assistance within the FLS and compare them with international standards to identify «gaps» and improve the work of the service.

Fractures of the proximal femur are a severe, life-threatening injury with enormous social, medical and economic consequences. Even with a well-organized system of medical care, up to 12-17% of patients die within the first year after injury, and the long-term risk of death doubles. Specially designed population-based studies in the Russian Federation have shown a high incidence of hip fractures in patients at the age of 50 and older. However, there is currently no unified system of medical care organization for all regions of Russia, which allows hospitalization and operation of patients with hip fractures during the first hours after trauma. Hip fracture in an elderly person is an absolute indication for the hospitalization. However most patients have severe comorbidities, osteoporosis, an increased risk of falls and mortality because of any reason. The main surgical treatments for hip fractures include osteosynthesis of the fracture or hip arthroplasty. It is well known that after the occurrence of any fragility fracture the risk of subsequent fractures increases by 2-3 times. To ensure the continuity of patient management, provide prescription of anti-osteoporotic drugs and long-term follow-up of patients it is necessary to manage them within the special Fracture Liaison Services (FLS). The article is devoted to the organization of medical care in patients of older age groups who have received hip fractures. A review of modern approaches to the treatment and organization of care for elderly patients with hip fractures is based on the methodological recommendations «Complex measures aimed at preventing falls and fractures in the elderly» developed in the framework of the federal project «The Older Generation», the draft national clinical guidelines «Fractures of the proximal femur», as well as the results of our own research.

Due to the increase in life expectancy, the number of elderly and senile people with various chronic diseases is growing. One of the most common diseases of the musculoskeletal system is osteoarthritis. According to existing recommendations among all medications the leading place in the treatment of patients with osteoarthritis is given to symptomatic slowacting drugs (SYSADOA). Data from numerous studies show that these patients are often multimorbid, and this fact significantly restricts the appointment of many medications. In such cases, the use of drugs from the SYSADOA group becomes the basis of therapy due to their safety. Medications of this group include chondroitin sulfate and glucosamine sulfate. They are the main structural components of cartilage and synovial fluid and they are used for the synthesis of glycosaminoglycans, they reduce the degradation of cartilage, and also exhibit analgesic and anti-inflammatory effects. In addition, a number of other pleiotropic effects of these drugs have been shown, including a positive effect on muscles in sarcopenia, on reducing the risk of death from several cancers and complications of diabetes mellitus. In recent years, there has also been evidence of the use of the natural egg membrane preparation (NEM), which contains natural glycosaminoglycans and proteins necessary for maintaining the condition of the cartilage and synovial membrane. Studies have shown its anti-inflammatory effect, reducing the intensity of pain and improving functional indicators in patients with osteoarthritis. There was also a decrease in the production of pro-inflammatory cytokines in osteoarthritis for the plant-derived substance harpagophytum, which has therapeutic potential in the treatment of diseases associated with inflammation and oxidative stress.

Osteoporosis is considered a serious public health and social concern that catches public attention worldwide. A set of recommendations for management of patients with fragility fractures were developed on an initiative of the European League Against Rheumatism (EULAR) and the European Federation of National Associations of Orthopedics and Traumatology (EFORT) in December 2016. Authors have reviewed the recommendations through provision of health care practices in the Russian Federation. A protocol of hip fracture surgery within 48 hours of admission to trauma services for fragility cases represent a needful tool for optimization of medical care system. Specific techniques and implant designs for fracture fixation in osteoporotic bone are described with the focus on subsequent fractures prevention and fracture risk assessment. Effective rehabilitation is shown to rely on patient's understanding of the pathogenesis of the pathological process. Specific drug therapies and non-pharmacological treatment can be administered for osteoporotic patients with low-energy fractures.

Uranium-containing samples of magnesium potassium phosphate (MPP) compound were synthesized using a nitric acid uranium solution. Uranium is incorporated in the MPP compound in the form of potassium uranyl phosphate with the structure of metaankoleite natural mineral, K(UO2)PO4·3H2O. The differential and integral uranium leach rates, determined in accordance with GOST (State Standard) R 52 126–2003 on the 28th day of contact of the compound with water, are 1.7 × 10–6 and 2.7 × 10–6 g cm–2 day–1, respectively, and the degree of leaching is 0.014%. High hydrolytic durability of the compound with respect to uranium leaching reduces the risk of release of uranium isotopes from radioactive waste into the environment.

Introduction. Hip fracture (HF) are the most serious complication of osteoporosis and a serious public health problem, causing significant economic damage. They are characterized by a high level of mortality as an inpatient and outpatient stages. Rate of severe outcome depends on the organization of medical care of these patients. Goal. To evaluate the frequency of lethality in patients undergoing HF. Subjects and methods. A longitudinal observational cohort study among residents of the city of Yaroslavl at the age of 50 years and older who received HF for the period from 01.09.2010 to 31.08.2011 years, with follow-up of 24 months (up visits 3, 6, 12 and 24 month after the date of fracture). The analysis of lethality was performed by using of life tables and the analysis of the «censored» cases. Results. Overall lethality in hospital was 9.66%, in 3 months - 21,75%, after 6 months - 26,46%, 12 months -29,82%, 24 months - 34,53%. The next characteristics of the lethality were analyzed at every stage of observation: a density of probability, function of the intensity and immediate risk of the death, the expected median of lifetime. Conclusion. The analysis allowed to obtain the most accurate and complete data on the lethality in patients with HF and to evaluate several additional features of this indicator.

Introduction. The incidence of fractures of the proximal femur (FPF), fractures of the forearm and humerus was studied in two cities of the Russian Federation. This index was used to create the Russian model FRAX and to assess the present and future burden of fractures. Objective. So far, little is known about the epidemiology of fractures in Russia. The aim of the study was to determine the incidence of major fractures to create a Russian model of FRAX and evaluate the present and future burden of fractures. Materials and methods. In well-defined populations of two Russian cities, we had determined a number FPF and forearm fractures of the humerus for 2-2-year period. Data were combined and the resulting incidence used to calculate FRAX model for the Russian Federation and to calculate the projected number of fractures in the Russian Federation in 2010 and 2025. Results. A total of 6012 fractures were identified. Among all FPF, 27% of Pervouralsk and 1.8% in Yaroslavl were not registered on the official data of medical institutions. The incidence of fractures increased with age and was higher in women than in men. The probability of experiencing a FPF in later life at the age of 50 years was 4% of men and 7% of women. Total number FPF in 2010 was 112 thousand cases and it is expected to rise by 2025 to 159 thousand a year. The estimated number of major osteoporotic fractures will increase from 590 thousand to 730 thousand cases for the same period. Conclusions. Osteoporotic fractures are a serious health problem for Russia. It is necessary to take urgent measures to improve emergency care at FPF and long-term care for this and other osteoporotic fractures.

In order to reduce the incidence of recurrent fractures in patients that had already experienced low-energy (osteoporotic) fracture, the International Osteoporosis Foundation initiated the program «Capture the fracture». Established Best Practice Framework represents 13 standards to evaluate the effectiveness of the centers for the prevention of recurrent fractures, organized in different countries. In 2012 the Russian Association on Osteoporosis started the project PROMETHEUS [Creation the system to prevent recurrence of fractures in patients with osteoporosis]. In order to implement this program in the city of Yaroslavl we conducted a pilot project to establish a center for the prevention of recurrent fractures in patients who have received osteoporotic fracture. In this article we assess the effectiveness of the work of this center with the use of international standards, identify key problems and propose solutions.

Last years a lot of researches in the world were devoted to the problem of relationship between cardiovascular diseases and osteoporosis at elderly patients. According to results of these researches were shown several uniforms pathogenetic mechanisms, and now it allows us to tell about them as about «calcium scarce illnesses». Now there are bases to recognize existence of the general pathologic mediators of the bone and cardiovascular systems which leads to develop of diseases of heart and vessels. Knowledge of the general laws of patho-genesis will allow developing more effective measures on preventive maintenance of their complications and decreasing the frequency of these diseases.

Актуальность. Создание центров по профилактике повторных переломов (ЦППП) является эффективном способом снижения заболеваемости остеопоротическими (ОП) переломами. Во многих странах мира в настоящее время идет организация подобных центров, работа которых строится на максимальном выявлении пациентов, проведении у них оценки риска последующих переломов и назначении необходимой терапии. Подобный центр был создан в г. Ярославле на базе больницы СМП им. Н.В. Соловьева, в 2015 г. Международным фондом остеопороза его работа была признана соответствующей «Серебряному» уровню достижений в рамках оценки «Базы Лучших Практик». Цель. Оценить риск последующих переломов у лиц, включенных в программу профилактики повторных переломов. Материал и методы. В ЦППП в течение 1 года обратился 691 человек, средний возраст составил 72,54±9,86 года Оценка риска последующие переломов включила в себя: оценку уровня травмы; анализ факторов риска (ФР) ОП-переломов; подсчет абсолютного риска переломов (FRAX); измерение МПК по данным DXA (проводилась на остеоденситометре Hologic Explorer, США, 2006 г.). Результаты. Среди включенных больных у 11 (1,59%) был выявлен высокоэнергетический перелом, у 9 (1,30%) патологический на фоне опухоли. Поэтому дальнейшие оценки выполнялись у 671 больных. У 89 (13,26%) пациентов опрос не был проведен (тяжелое состояние, деменции, летальный исход или ранняя выписка из стационара). Среди опрошенных ФР ОП отмечены у 526 (90,38%) пациентов, у большинства выявлены сразу несколько из них (два - у 225 (38,66%), три - у 85 (14,60%), четыре и более - у 19 (3,26%)). Перенесенные ранее низкоэнергетические переломы выявлены у 261 (44,85%) человека, при этом два перелома - у 75 (12,89%), три и более - у 16 (2,75%). Всего подсчет числа клинических переломов на данную популяцию выявил 959 случаев. К другим наиболее часто встретившимся ФР ОП-переломов относились: курение (12,20%), ранняя менопауза (22,32% среди включенных женщин), наличие перелома проксимального отдела бедра у родителей (6,01%). Показатель FRAX (без учета того перелома, по поводу которого пациент был включен в ЦППП) был подсчитан у 573 (98,45%) больных, так как 9 пациентов были старше 90 лет. При этом высокий риск переломов выявлен у 2 (1,35%) мужчин и 114 (26,82%) женщин. Оценка МПК выполнена у 292 (50,17%) больных, остальным она не проводилась ввиду тяжести состояния после операции (из них более 90% имели перелом проксимального отдела бедра). При этом ОП был выявлен у 141 (48,29%), остеопения у 117 (40,07%), норма - у 34 (11,64%). Анализ встречаемости различных значений МПК в зависимости от локализации перелома не выявил достоверных различий. Выводы. Таким образом, оценка риска повторных переломов у пациентов, включенных в наблюдение в ЦППП, выявил большую распространенность ФР ОП, особенно часто отмечались перенесенные в анамнезе переломы (что демонстрирует наличие «каскада» ОП-переломов у них), ранняя менопауза, курение, перелом бедра у родителей. Однако показатель FRAX показал довольно низкую частоту превышения порогового значения для основных ОП-переломов. При выполнении DXA оказалось, что ОП отмечен менее чем у 50% пациентов, а более чем у 10% перелом произошел на фоне нормальных значений МПК.

Актуальность: На сегодняшний день во многих странах мира создаются регистры пациентов, перенесших перелом проксимального отдела бедренной кости (ППОБ). Данные программы регистрации больных используются для полу чения и анализа разнообразных параметров, включая эпидемиологические характеристики, различные показатели оказания медицинской помощи больным, исходы (в стационаре и при длительном наблюдении за пациентами). Цель: оценить возможность получения данных о пациентах с ППОБ при использовании регистра и создать модель прогнозирования риска летального исхода в стационаре. Материал и методы. В больнице скорой медицинской помощи г. Ярославля был создан регистр пациентов, перенесших ППОБ, получивший название «РЕАЛЬНОСТЬ». Данная программа представляет собой интернет-ресурс, являющийся клиент-серверной информационной системой, доступ к которой осуществляется посредством веббраузера. Особенностью созданного регистра является возможность автоматизированного поступления данных из отделения статистики больницы. Благодаря этому происходит создание карт пациентов внутри системы (что позволяет избежать потерь случаев), и поступают все основные параметры, вносимые в данном подразделении больницы. В анализ были включены данные по больным в возрасте 50 лет и старше, получившим ППОБ (шифры по МКБ Х S 72.0 (перелом шейки бедра), S 72.1 (чрезвертельный перелом) и S 72.2 (подвертельный перелом), прошедшим лечение в ГАУЗ ЯО Кб СМП им. Н.В. Соловьева за период с 01.01.2013 по 16.04.2015 гг. На основании этих данных была создана модель прогнозирования развития летального исхода за врем пребывания больного в стационаре. Создана модель биноминальной многофакторной логистической регрессии, выполнена процедура ROC-анализа с построением соответствующей ROC-кривой. Статистический анализ выполнен с помощью прикладных программ STATISTICA 12 и MedCalc Statistical Software version 15.8. Результаты. За оцениваемый период летальность в стационаре составила 2,77% (47/1695). Полученная модель логистической регрессии для оценки вероятности развития летального исхода включила в себя такие параметры, как возраст, число часов до операции, основной диагноз (по МКБ Х), проведенную хирургическую операцию (0 - не оперированы, 1 - тотальное замещение тазобедренного сустава, 2 - остеосинтез, 3 - гемиартропластика). В ходе проведенного ROC-анализа чувствительность у данной модели составила 63,51 (95% ДИ 61,0; 66,0), специфичность - 83,33 (95% ДИ 65,3; 94,4). Соотношение вероятности риска летального исхода 3,81 (95% ДИ 1,7; 8,5) +LR и 0,44 (95% ДИ 0,4; 0,5) -LR. Выводы. Использование автоматизированной системы экспорта определенного ряда данных в созданном регистре пациентов с ППОБ позволяет получать и анализировать многие необходимые параметры, касающиеся оказанной медицинской помощи. По результатам анализа данных, поступивших в регистр, летальность в стационаре в данной группе больных составила 2,77%. Модель, прогнозирующая вероятность развития летального исхода за время госпитализации, включила в себя возраст, длительность дооперационного койко-дня (в часах), основной диагноз, проведенную хирургическую операцию.

At thepresent time there is a great interest in médical researche devoted to investigation the role of cytokines (interleukins) in the pathogenesis of cardiovascular diseases (atherosclerosis, arterial hypertension, ischemic heart disease, etc.), and osteoporosis. The review provides data on the conducted research on the importance of cytokines in the event of these diseases.

Медико-социальную и экономическую значимость остеопороза определяют его неуклонно нарастающая распространенность, а также затраты на лечение собственно остеопороза и его прямых осложнений - переломов, приводящих к развитию хронической боли, деформации скелета и потере трудоспособности. В Российской Федерации данные об экономических и социальных последствиях остеопоротических переломов практически отсутствуют, и имеют ограниченную ценность для применения в экономике здравоохранения. Исследование проведено в рамках международного многоцентрового исследования ICUROS. Целью исследования являлось единообразное (с использованием единой методологии сбора и анализа данных) изучение странах экономических и социальных последствий остеопоротических переломов. В Российской Федерации в исследовании участвовали 9 центров. В тезисах впервые использована окончательная версия базы данных. Материал и методы. Всего включено 1143 пациента от 50 лет и старше, перенесших низкоэнергетический перелом, из них 251 (21,96%) - с переломами шейки бедра, 255 (22,33%) - с переломами запястья, 219 (19,16%) - с переломами позвонков, 182 (15,92%) - с переломами плечевой кости, и 236 (20,65%) - с переломами лодыжки. Опрос пациентов проводился непосредственно после перелома и касался информации о качестве жизни (КЖ) до перелома и непосредственно после перелома (Фаза I). Далее сбор информации осуществлялся через 4 (Фаза II), 12 (Фаза III) и 18 месяцев (Фаза IV) после перелома. Средний возраст включенных пациентов составил 65,2 года (95% доверительный интервал (ДИ) от 64.65 до 65,72). Одним из интегральных параметров КЖ исследовании был опросник EQ-5D, включающий данные по подвижности, самообслуживанию, повседневной активности, боли и дискомфорту, выраженности тревоги и депрессии. Отдельные шкалы данного опросника были преобразованы в общий балл, стандартизованный к шкале от нуля до единицы. Результаты. Ретроспективные данные о КЖ по стандартизованному суммарному баллу EQ-5D составили 0,824 (95% ДИ: 0,812-0,836). На момент перелома у пациентов значимо падало КЖ - 0,290 (95% ДИ: 0,273-0,307) (р0,05). Выводы. Таким образом, если общая субъективная оценка здоровья по ВАШ возвращается к значениям «до перелома» уже к 4 месяцам после перелома, то суммарный балл по EQ-5D) достигает значений, близких к исходным, только к 12 месяцам, и продолжает улучшаться и через 18 месяцев. Эти данные свидетельствуют о важности изучения отдельных компонентов качества жизни пациентов, входящих в EQ-5D, а не только суммарных результатов.

Актуальность. В ходе проведенных эпидемиологических исследований по оценке инцидентности ППОБ было получено, что в возрасте 50-69 лет данный показатель выше у мужчин по сравнению с женщинами, в то время как после 70 лет он значительно преобладает у женщин. В связи с этим было выполнено исследование для оценки факторов, которые могли стать причиной выявленной закономерности. Цель. Создание диагностической модели прогнозирования вероятности получения ППОБ у мужчин в возрасте до 70 лет. Материал и методы. Было проведено сплошное исследование по типу случай-контроль среди мужчин г. Ярославля в возрасте 40-69 лет, получивших ППОБ с 01.09.2010 по 01.02.2012 гг. (n=128). Контрольную группу составили мужчины того же возраста без переломов, прошедших диспансеризацию и отнесенных к 1-й группе здоровья (n=50). Изученные в группах исследования факторы риска остеопороза, сопутствующие заболевания, лабораторные показатели (уровень свободного тестостерона, 17-эстрадиола, пролактина, паратиреоидного гормона, тиреотропного гормона, 25(ОН)-витамина D), результаты оценки МПК по DXA были последовательно включены в модели биноминальной многофакторной логистической регрессии. После получения уравнения логистической регрессии выполнена процедура ROC-анализа с построением соответствующих ROC-кривых. В результате получены несколько вариантов моделей, имевших достаточную статистическую значимость для прогнозирования развития ППОБ у пациентов в возрасте 40-69 лет. С целью выявления наиболее прогностически мощной комбинации учетных переменных проведена процедура сравнения площади под кривой (AUC) в каждой из созданных моделей. Статистический анализ выполнен с помощью прикладных программ STATISTICA 12 и MedCalc Statistical Software version 15.8. Результаты.
